Objectives Several plaque and lesion characteristics have been associated with an increased risk for procedural stroke during or shortly after carotid artery stenting (CAS). While technical advancements in stent design and cerebral protection devices (CPD) may help reduce the procedural stroke risk, and anatomy remains important, tailoring stenting procedures according to plaque and lesion characteristics might be a useful strategy in reducing stroke associated with CAS. In this descriptive report of the ongoing Asymptomatic Carotid Surgery Trial-2 (ACST-2), it was assessed whether choice for stent and use or type of CPD was influenced by plaque and lesion characteristics. Materials and methods Trial patients who underwent CAS between 2008 and 2015 were included in this study. Chi-square statistics were used to study the effects of plaque echolucency, ipsilateral preocclusive disease (90–99%), and contralateral high-grade stenosis (>50%) or occlusion of the carotid artery on interventionalists' choice for stent and CPD. Differences in treatment preference between specialties were also analysed. Results In this study, 831 patients from 88 ACST-2 centres were included. Almost all procedures were performed by either interventional radiologists (50%) or vascular surgeons (45%). Plaque echolucency, ipsilateral preocclusive disease (90–99%), and significant contralateral stenosis (>50%) or occlusion did not affect the choice of stent or either the use of cerebral protection and type of CPD employed (i.e., filter/flow reversal). Vascular surgeons used a CPD significantly more often than interventional radiologists (98.6% vs. 76.3%; p < .001), but this choice did not appear to be dependent on patient characteristics. Conclusions In ACST-2, plaque characteristics and severity of stenosis did not primarily determine interventionalists' choice of stent or use or type of CPD, suggesting that other factors, such as vascular anatomy or personal and centre preference, may be more important. Stent and CPD use was highly heterogeneous among participating European centres.

This report describes an endovascular repair of a residual type A dissection using a medical device that is not marked by european conformity (CE) or Food and Drug Administration (FDA). The patient underwent ascending aortic surgery for acute type A dissection. The 2-year angio–computed tomography demonstrated patency of the residual false lumen with evolution into a 6 cm aneurysm, the extension of the dissection from the aortic arch to the aortic bifurcation with thrombosis of the right common iliac artery. There was no CE- or FDA-marked medical device indicated for this case or any other acceptable therapeutic alternative. We used the Najuta thoracic stent graft and successfully handled the pathology in a multiple-phase treatment. Technology is evolving with specific grafts for the ascending and fenestrated grafts for the aortic arch. In this single case the Najuta endograft, in spite of the periprocedural problems, was a valid therapeutic option.

We report an endovascular approach that used to treat a symptomatic extracranial vertebral artery aneurysm associated to an asymptomatic aberrant right subclavian artery aneurysm. A 54-year-old man presented with neck pain, vertigo and loss of balance. The computed tomography (CT) scan demonstrated a left extracranial vertebral artery aneurysm that compressed and eroded the C5 vertebra associated to an aberrant right subclavian artery aneurysm. Endovascular exclusion of the vertebral aneurysm using a covered stent and a hybrid treatment of the aberrant subclavian artery aneurysm were performed. The 13th month follow-up CT scan confirmed the stent-grafts and supra-aortic vessels patency. The endovascular treatment represents a good option for these complex pathologies with excellent immediate results, reduces the complication rate and the hospital stay if compared to open repair. Long-term follow-up is necessary. To our knowledge this is a unique case in the literature.

The aim of this study was to evaluate the usefulness of the temporal approach during coronary artery stenosis (CAS) to overcome tortuosity in case of difficult arch anatomy or tortuous common carotid artery, and to provide cerebral protection of proximal lesions of the supra-aortic trunks during stenting.The superficial temporal artery is exposed through a small incision right in front of the ear and is cannulated. A guidewire is used to descend in the external and common carotid artery and in the arch if needed. Between the end of March 2007 and May 2008 9 patients were treated with the temporal approach: in 5 cases of stenosis of the internal carotid artery with difficult arch anatomy to obtain a through and through guide from the temporal to the femoral artery, in 3 of the supra-aortic trunks to obtain cerebral protection during the procedure and in one case of tandem lesion of the left common and internal carotid artery.The procedure was successful in all 9 cases and there were no complications.The temporal approach proved to be feasible and at low risk; it represents a new possibility to increase the feasibility of carotid artery stenting in patients with difficult anatomy but, above all, it is a good way to obtain cerebral protection during endovascular treatment of proximal lesions of the supra-aortic vessels.

Objective: The purpose of the current study was to identify the possible short- and long-term effects of diabetes on the outcome of carotid endarterectomy. Methods: Medical records were reviewed for 781 carotid endarterectomies (in 734 patients) performed by the same vascular surgeon in a university medical center between January 1994 and December 1998. Patients were divided two groups: those with diabetes (n = 181 patients; 193 operations) and those without diabetes (n = 553; 588 operations). The two groups were similar with respect to mean age, male-female ratio, and contralateral lesions. The only significant differences were a higher prevalence of peripheral vascular disease and dyslipidemia in the diabetic group and a higher prevalence of hemispheric transient ischemic attacks among the nondiabetic patients. Carotid color duplex ultrasound scan had been performed in all patients, and in 56 patients from the diabetic group and 56 patients from the nondiabetic group (matched for age, sex, and contralateral lesions), the distal extension of the lesion from the carotid bifurcation had also been defined. Both of these subgroups were fully representative of their respective groups of origin. Carotid endarterectomy was performed after the induction of general anesthesia; electroencephalographic monitoring was continuous. Clamping ischaemia is responsible for the 20-30% of the neurological complications during carotid surgery. It is impossible at present to determine preoperatively for certainty the patients who are at risk and the ones who are not at risk for clamping ischemia. Intraoperative monitoring is mandatory in all the cases to point out cerebral ischemia and to prevent neurological deficit with an adequate brain protection. Methods used to predict brain ischaemia include local anesthesia, EEG, SEP, stump pressure measurement, transcranial doppler, regional cerebral blood flow measurement and evaluation of venous blood gases from the internal jugular vein. Local anesthesia is a safe and simple method of assessing cerebral ischemia during carotid clamping but has some limits particularly in case of brain ischaemia for the technical difficulties to install an indwelling shunt in an awake and often troubled patient. Moreover the only possibility of cerebral protection under local anesthesia is an increase in systemic blood pressure with a resulting growth in cardiac morbidity and mortality. For these reasons operation should be performed under general anesthesia to assure a better farmacogical brain protection when preoperative evaluation demonstrates a reduction of cerebral vasoreactivity, with a consequent high risk of clamping ischemia. At present EEG is one of the most used technique of cerebral monitoring under general anesthesia with a sensibility and specificity in the diagnosis of brain ischaemia that is about 90%. The best cerebral protection is obtained with a selective shunting. Burst suppression, with high dose thiopentone, has recently demonstrated its reliability alone or in association with an indwelling shunt in patients at high risk for clamping ischaemia. Of course a systematic cerebral protection together with an accurate control of the blood pressure is necessary in all the patients. This behaviour allowed us to obtain good results in the last 500 patients with a morbidity and mortality respectively of 1% and 1.6% with no significant differences between patients who tolerated carotid clamping and those with clamping ischaemia.

Carotid artery cross-clamping ischaemia during carotid endarterectomy (CEA) causes 5-30% of perioperative neurological deficits. This study was performed to identify possible clinical situations at higher risk for carotid cross-clamping ischaemia. 606 consecutive patients underwent CEA and were retrospectively studied; they were grouped according to risk factors, presence of associated vascular diseases, clinical pattern, angiographic and CT scan findings. Stump pressure measurement was provided in all patients, perioperative monitoring during CEA was performed by electroencephalogram (EEG) in 469 (77%) and somatosensorial evoked potentials (SEP) in 137 (23%). Local anaesthesia was used in 88 (14.5%) patients. Ischaemic changes during carotid cross-clamping were registered in 118 patients (19.5%). The incidence of cross-clamping ischaemia was then related to different factors; it affected 5.6% of asymptomatics, 25.4% of patients with fixed stroke and 38.5% of those with stenosis and contralateral occlusion. Angiographic and clinical correlation showed that patients with more severe lesions are mostly affected by clamping ischaemia (up to 55% in those with stroke and stenosis with contralateral occlusion). Age, hypertension and diabetes do not significantly affect incidence of ischaemic changes. Positive CT scan increased this risk; statistical relevance was found in regard to patients with unilateral or bilateral stenosis and in those with transient ischaemic attacks. A higher risk can be expected for subjects with more severe clinical and instrumental findings, even if no patients can be considered completely at risk or risk free. Perioperative monitoring is always mandatory and is of great importance in detecting ischaemic changes and preventing cerebral damage using a temporary intraluminal shunt.

Il presente volume ripropone al pubblico italiano, adeguandola ai nuovi e risolutivi apporti scientifici forniti dall’edizione critica curata da Robert Volk per la serie degli Schriften des Johannes von Damaskos (De Gruyter, Berlin - New York 2009), la versione integrale della Storia di Barlaam e Ioasaf pubblicata dai curatori nel 1980 (Vita bizantina di Barlaam e Ioasaf, Rusconi, Milano) in base all’edizione Woodward-Mattingly. Capofila di tutte le storie cristianizzate del Buddha, questo testo bizantino degli anni intorno al Mille ha una genesi affascinante tra il Caucaso e il Monte Athos, in un intreccio di lingue, culture e religioni diverse. La Storia di Barlaam e Ioasaf racconta di un principe indiano che, grazie agli insegnamenti di un anacoreta, fugge dal palazzo dove il padre l’ha rinchiuso per proteggerlo dai mali del mondo, abbandona il destino regale e avvia il suo percorso mistico-eremitico. Che la storia ricalcasse quella del Buddha se ne erano accorti già gli studiosi di fine Ottocento, ma la matassa dei passaggi e delle mediazioni è stata dipanata solo in anni recenti, anche grazie all’edizione critica pubblicata da Robert Volk nel 2009. Basandosi sul suo testo e sui suoi apparati, Paolo Cesaretti consegna ai lettori una puntuale revisione della traduzione (di entrambi i curatori) e una ristrutturazione delle note e degli indici, che completano l’informazione aggiornata sull’insieme dell’opera fornita nel saggio introduttivo. Tutto questo rinnova profondamente l’edizione firmata dai due studiosi nel 1980. Si possono così apprezzare appieno per la prima volta sia le qualità narrative del testo sia la ricchezza allusivo-sapienziale delle parabole incastonate nel racconto, che hanno affascinato e influenzato molti scrittori nel corso dei secoli, da Iacopo da Varazze a Boccaccio, da Shakespeare a Tolstoj. Il saggio introduttivo della co-traduttrice e co-curatrice (Il Buddha bizantino, pp. vii-cvii) traccia in cento pagine il primo ancorché sintetico bilancio storico-filologico sulla questione del Barlaam e Ioasaf stilato tenendo presente la nuova edizione critica di Robert Volk (Historia animae utilis de Barlaam et Ioasaph, I-II, Berlin – New York 2006-2009, che solo oggi di fatto ha rimpiazzato l’editio princeps ottocentesca di Jean François Boissonade) e in particolare i suoi monumentali prolegomeni (Einführung, ivi, vol. II). Dà anzitutto conto delle nuove e cruciali acquisizioni scientifiche presenti in questi ultimi, e in generale emerse dalla comunità degli studiosi nei più di trent’anni che separano questa nuova edizione italiana dalla prima pubblicata dai due curatori nel 1980 (Vita bizantina di Barlaam e Ioasaf, introd., trad. it., note e repertorio biblico di S. RONCHEY e P. CESARETTI, Milano, Rusconi, 1980, 317 pp.). Delucida quindi l’evoluzione del nucleo narrativo, dalle versioni orientali della vita del Buddha alla cristianizzazione georgiana alla formalizzazione e all’attribuzione autoriale della narrazione greco-bizantina (per cui sono dirimenti le acquisizioni di Volk), sino alla fortuna del Barlaam e Ioasaf e dei suoi apologhi in età moderna. Tutti gli aspetti filologici toccati da Volk sono analizzati in termini sintetici e quanto meglio possibile chiariti, ma sono integrati nel quadro anche gli elementi da Volk volutamente tralasciati, come quelli legati agli studi orientalistici, in particolare sul buddhismo e sul manicheismo, o la fortuna del testo nella storia della letteratura medievale e moderna, o in generale il senso culturale e letterario dell’opera nell’ambito della civiltà bizantina e più circostanziatamente nel contesto della speculazione post-iconoclasta e del cosiddetto enciclopedismo del X-XI sec. Il saggio si presenta dunque come un'introduzione globale al Barlaam e Ioasaf e ai suoi vari aspetti, alla storia degli studi intorno ad esso e all’immensa vicenda della sua fortuna. Ne emerge un “romanzo di filologia” che mostra come lo studio della tradizione dei testi possa toccare il cuore degli snodi culturali e, in questo caso, degli intricati rapporti fra Occidente e Oriente. The present volume re-introduces to an Italian audience the complete version of the Life of Barlaam and Ioasaph, published by the editors in 1980 (Vita bizantina di Barlaam e Ioasaf, Milan: Rusconi), now updated with the results of new research and of the critical edition by Robert Volk (De Gruyter, Berlin - New York 2009). Original source of all of the Christianised stories of Buddha, this Byzantine text dating from the end of the 10th or beginning of the 11th century presents a genesis between the Caucasus and Mount Athos, in a web of different languages, cultures, and religions. The Barlaam and Ioasaph recounts the story of an Indian prince who, influenced by the teachings of an anchorite, flees the palace where his father has imprisoned him to protect him from the evils of the world, abandons his royal destiny, and sets off on his own mystical-hermetic journey. That the story mirrored that of the bodhisattva was recognised by scholars already at the end of the 19th century, but the various stages and mediations were unravelled only in recent years, thanks in large part to the critical edition published by Robert Volk between 2006 (vol. II) and 2009 (vol. I). Basing his work on Volk’s text and apparatuses, Paolo Cesaretti provides readers with a revision of the translation (by both editors) and of the notes and indices. In the introduction Silvia Ronchey provides exhaustive updated information on the entire work (Il Buddha bizantino, pp. vii-cvii). This effort marks a profound renewal of the two scholars’ 1980 edition. It is now possible to fully appreciate both the narrative qualities of the text and the allusive, philosophical richness of the various trajectories of the story, which have fascinated and influenced scores of writers over the centuries, from Jacopo da Varazze to Boccaccio, Shakespeare, and Tolstoy. The co-translator’s and co-editor’s introductory essay (Il Buddha bizantino, pp. vii-cvii) traces in one hundred pages the first, albeit concise, historical-philological evaluation of the issue of the Barlaam and Ioasaph compiled in mind with the new critical edition by Robert Volk (Historia animae utilis de Barlaam et Ioasaph, I-II, Berlin – New York 2006-2009, which only now replaces the nineteenth century editio princeps by Jean François Boissonade) and, in particular, his monumental prolegomena (Einführung, ivi, vol. II). The introduction takes into account the crucial new research results present in the latter and, in general, which have emerged from the more than thirty years of research separating this new Italian edition from the first published by the two editors in 1980 (S. Ronchey and P. Cesaretti, eds., Vita bizantina di Barlaam e Ioasaf, Introduction, Notes, Biblical Repertory, Italian translation. Milan: Rusconi, 1980, 317 pp.). The essay sheds light on the evolution of the narrative nucleus from Eastern versions of the life of Buddha to the Georgian Christianisation, as well as to the formalisation, and authorial attribution of the Greek Byzantine narrative (for which Volk’s results are decisive), up until the success of the History of Barlaam and Ioasaph and its apologists in the modern age. All of the philological aspects touched on by Volk are analysed concisely and clarified as much as possible, with the addition of elements intentionally bypassed by Volk, such as those relating to Buddhist or Manichaean studies, or the reception of the text in the history of Medieval and modern literature or, eventually, the more general cultural and literary meaning of the work in the world of Byzantine civilisation and, more tangentially, in the context of post-iconoclast speculation and the so-called encyclopaedism of the 10th and 11th centuries. Therefore, the essay represents an overall introduction to the Barlaam and Ioasaph and its various aspects and the history of the scholarship surrounding it, as well as the enormous circumstances of its influence. What emerges is a “philological novel” that reveals how the study of textual tradition can touch the heart of cultural exchange and, in this case, the intricate relations between East and West.

I languori della Decadenza o, come il bel titolo di uno dei saggi del volume: ‘la voluttà di scendere’, sono il tema che unisce questa raccolta di saggi di storici, filosofi e filologi: Simone Beta, Ginevra Bompiani, Gioachino Chiarini, Daniela Fausti, Alessandro Fo, Mario Musumeci, Alberto Olivetti, Romano Romani, Silvia Ronchey. Di quel luogo dello spirito, di quell'umore o di quel periodo della storia culturale che chiamiamo decadenza (la definizione dipende dal punto di vista) gli autori cercano di riconoscere e delineare le manifestazioni caratteristiche, colte in momenti ed episodi sparsi nel tempo e osservati da prospettive disciplinari diverse: dall'antica Roma che cade in mano ai barbari alla moderna Europa decadente che vi si specchia, dall'Atlandide di Platone alla Comfortable Thébaïde di Des Esseintes, da Agostino a Benjamin, da Bisanzio a un film degli anni venti del Novecento. ’In realtà’, scrive la curatrice Silvia Ronchey nell'introduzione al volume, ‘questo libro, che nasce da un seminario all'Università di Siena ideato per celebrare laicamente il trapasso al Nuovo Millennio, tratta, più che della decadenza, dell'antidecadenza. Barlaam e Ioasaf è molto più che un re-writing cristiano della leggenda del Buddha: è la storia del principe-filosofo, sacra a oltre venti popoli di circa trenta lingue e dieci diverse confessioni, da Gibilterra al Pacifico, che ha ricevuto a Bisanzio la sua canonizzazione letteraria. Opera da sempre di controversa datazione e ricca di influssi asiatici (dall'originale indiano alle ipòstasi persiane, islamiche, manichee) sia nello svolgersi della narrazione che nelle dieci fabulae che la tramano, la storia del principe e dell'anacoreta riceve il suo specifico di fiaba colta dal sorprendente innesto di una sapienza filosofica antica di millenni su una ambientazione esotica splendida quanto densa di allusività letteraria e, forse, politica.

Nella seconda metà del XV secolo, dopo la definitiva espugnazione turca di Costantinopoli, nel disegno di riaffermazione dell’autorità papale che culminerà nel pontificato di Enea Silvio Piccolomini, rientrò in maniera essenziale una nuova percezione del titulus di Costantino. Il lavorio diplomatico e il progetto strategico del salvataggio occidentale di Bisanzio, nei due decenni successivi alla caduta di Costantinopoli, avevano e avrebbero avuto il preciso scopo di reintegrare nell’orbita d’influenza papale la titolarità ereditaria dei cesari bizantini, trasferita da Costantino in Oriente e mai estinta. La cattedra della sede di Pietro e lo scettro della cristianità orientale si sarebbero dovuti riunire simbolicamente in una ‘Nuova Bisanzio’, che avrebbe avuto la sua base a Roma e la sua testa di ponte a Mistrà. La portata, la finalità, le implicazioni, l’appassionata coralità di questo estremo tentativo di salvare dai turchi e riannettere all’Occidente il titolo di Costantino fino a qualche anno fa non erano state còlte sino in fondo né dagli studiosi di storia occidentale né da quelli di storia bizantina, per due motivi: perché tutto si svolse nell’angolo cieco tra la visione dell’una e dell’altra, oltre che alla cerniera tra Medioevo e Età Moderna; e inoltre perché quel tentativo risultò perdente, mentre la storia, si sa, è scritta dai vincitori. Il progetto fallì anche perché perirono l’uno dopo l’altro, in un brevissimo arco di tempo, i suoi principali sostenitori. Ma nei decenni in cui fu perseguito assistiamo a un vero e proprio revival della figura di Costantino e a un’accentuazione del primato simbolico e del portato giuridico del suo titolo, nella riflessione e nell’azione politica degli intellettuali umanisti, che si rispecchiò nelle committenze artistiche del tempo. Proprio Costantino è al centro della Battaglia di Costantino e Massenzio, il primo degli affreschi della Leggenda della Vera Croce di Piero della Francesca ad Arezzo. Lo stesso profilo, la stessa barba appuntita, l’identico copricapo si ritrovano attribuiti da Piero alla figura maschile assisa in trono che all’estrema sinistra della Flagellazione di Urbino apre l’apparentemente enigmatica sequenza dei personaggi del dipinto: la figura simbolica del Pilato neotestamentario, inteso nella sua qualità di rappresentante giuridico del potere romano, che l’opinione prevalente degli studiosi novecenteschi ha identificato con quella storica di Giovanni VIII Paleologo. Una lunga scia iconografica, che ha la sua espressione più emblematica e nota negli affreschi di Piero della Francesca ad Arezzo e nella sua Flagellazione, ma che è ben più diffusa e diversificata e si estende sino alla fine del Quattrocento e all’inizio del Cinquecento, moltiplicherà in questi decenni l’icona costantiniana nell’arte pittorica, ma anche in quella eminentemente libresca e umanistica della miniatura, e perfino nelle arti minori come la ceramica, conferendo all’immagine di Costantino i concreti tratti dei basileis bizantini del tempo e sottolineando sia l’indiscussa legittimità sia l’inestimabile valore politico del lascito dei cesari della Seconda Roma. Solo negli anni Settanta del Quattrocento, dopo che la mancata riunificazione ideologico-giuridica della Prima Roma con la Seconda divenne un dato di fatto irrefutabile e insormontabile, il passaggio del titulus di Costantino da un lato ai turchi osmani, dall’altro alla Terza Roma produrrà non solo l’eclissi di Bisanzio nell’autocoscienza politica dell’era moderna, ma anche una metamorfosi nella ricezione della figura storica del fondatore dell’Impero che si era affermata durante il suo revival umanistico.
