Purpose Equitable inclusion of racial and ethnic participation in clinical trials is crucial to improving disparities in health care, especially for historically marginalized populations. Our study aims to describe the racial and ethnic demographics of patients enrolled in published phase 2 clinical trials involving proton therapy in the United States. Materials and Methods Published manuscripts were identified in PubMed, Embase, World of Science, and Cochrane. Phase 2 trials evaluating proton therapy for US patients were included. For each article in the study, data were collected comprising authors, title, and publication year, and clinical trial numbers were verified. Additional data included tumor site, primary institution, sample size, reported race/ethnicity, and raw number/percentile of race/ethnicity. Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) guidelines were used. Results Overall, 970 titles were identified; 636 remained after duplicate screening, and 75 full-text articles were assessed. We identified 38 eligible manuscripts for inclusion comprising 2648 patients. Only 15 (39%) of the publications reported race/ethnicity. Of these, 8 (21%) and 10 (26%) documented Hispanic or Black trial participants, respectively; however, only 6 (16%) documented trial participation for both Hispanic and Black patients. Of the 1409 patients with a documented race/ethnicity, 89.0% (n = 1254) were non-Hispanic white, 5.3% (n = 75) were Black, and 2.2% (n = 31) were Hispanic. Other and unknown race/ethnicity comprised the remaining patients (3.5%; n = 49). Conclusion We identified underreporting of demographic data in published phase 2 proton therapy trials, which unfortunately mirrored underreporting for cancer drug clinical trials. We also noted dramatic Black and Hispanic patient underrepresentation across the trials in which race and ethnicity are reported. Findings highlight the urgent need to identify and address barriers to proton therapy trials for Black and Hispanic patients ensuring clinical trials in radiation oncology are representative of the patients seen in clinical practice.

Staphylococcus Sp.is the most common type of bacteria found in contamination place, we design this study to compare the contamination accident between two hospitals in Baghdad.One of them isthe Burns Specialist Hospital in the Medical CityinRusafa and another one is Al-Karama Hospital in Karkh. The samples were collected fromOperativeWard No1 (OW1), Operative Ward No2 (OW2), Consulting Pharmacy (CP), Emergency Room (ER), Reception Room (RR), Women's Ward (WW) and Men's Ward (MW).The samples were taken from inside each clinical unit, surfaces, food, and air. The results showed that the number of samples containing Staphylococcus sp. bacteria is 81, including 45 belonging to Al-Karama Burns Ward Hospital and 36 belonging to the Medical City Hospital, Burns Special Hospital. The results of Medical City Hospital showed that Staphylococcus sp.isolates resist many antibiotics, 99.99% of the isolatesfrom patient samples were resistant to(CLR, P, AMP); 92.30% of the isolates from air samples were resistant to(P, MET) and 85.71%of isolates from surfaces and food samples were resist for (AMP, MET). Also the results of Al-KaramaHospital showed that the highest resistance in the staphylococcus sp.isolateswere in patient samplesfor (AMP, SXT), in surface samples for (E), in food samples for (E,P) and in air samples for (P) and the resistance rates were 100% to all these antibiotics.

Our understanding of the genetics and physiology of the spore-forming genus Bacillus is remarkable. On the other hand, though, where these Gram-positive bacteria live and grow is far from clear. The soil, once considered their habitat, may simply serve as a reservoir. A growing number of studies show that Bacillus spores can be found in the intestinal tracts of animals, raising the question of whether this could be where they live and grow. In this study, we have conducted the first evaluation of Bacillus spore formers in soil and in human faeces. Our aim is simply to determine the abundance of aerobic spore-formers. Our results show that soil carries approximately approximately 10(6)spores/g while human faeces an average of up to 10(4)spores/g. The numbers of spores found in faeces, we reason, is too high to be accounted for principally by ingestion of food contaminated with spores from soil. This provides further evidence that Bacillus spore formers may have adapted to survival within the intestinal tract of insects and other animals that ingest them; if so they may well be hitherto undiscovered gut commensals.

Otosclerosis classically presents with conductive deafness. However, mainly in older patients or those with a progressive (active) form of this disease, it presents with elevated bone conduction (BC) thresholds. In many cases, an elevation in the BC threshold during otosclerosis is not a true indicator of ‘cochlear reserve’ and this apparent loss can be corrected by surgery. It is generally accepted that in those patients, elevated BC thresholds are not due to pure sensorineural hearing loss but to the Carhart effect. Speech discrimination tests are routinely done, and the results of these tests determine if a patient is a good candidate for surgery. In most institutions, those with poor speech discrimination scores are considered to be poor candidates due to inadequate cochlear reserve. Using hearing-aid-assisted pure-tone audiometry, we developed an accurate method to identify suitable candidates for surgery among the patients with elevated BC thresholds. This method is meant to complement speech discrimination tests in predicting the success of surgery in those patients. Different variables render speech discrimination tests unreliable in many patients. The rationale behind the current study was the lack of an easy and accurate clinical method to determine if a patient has a good cochlear reserve, and to predict the postoperative hearing threshold outcome.

Aims: To isolate and characterize spore-former bacteria able to colonize the human gastrointestinal tract (GIT). Methods and Results: A total of 25 spore-formers was isolated from faeces and ileal biopsies of healthy human volunteers and identified at the species level. Physiological analysis was performed to evaluate the ability of the various isolates to form biofilms, to swarm, to produce surfactants and molecules that have antimicrobial activity against selected pathogens. To assess the potential probiotic activity of the isolates, we tested the resistance of cells and spores to simulated gastric conditions, the ability to grow and sporulate in anaerobic conditions and the presence of toxin-encoding genes in their genome. Conclusions: Spore-formers belonging to various bacterial species have been isolated from the gut of healthy human volunteers. These strains appear to be well adapted to the intestinal environment and we propose them as potential probiotic strains for human use and as oral vaccine vehicles. Significance and Impact of the Study: To our knowledge this is the first detailed characterization of spore-forming Bacilli from the human GIT. Our data suggest that the isolated species do not transit, but rather colonize this specific habitat and propose them as probiotic strains for human use.

Detailed, country‐specific epidemiological data are needed to characterize the burden of chronic hepatitis C virus (HCV) infection around the world. With new treatment options available, policy makers and public health officials must reconsider national strategies for infection control. In this study of 15 countries, published and unpublished data on HCV prevalence, viraemia, genotype, age and gender distribution, liver transplants and diagnosis and treatment rates were gathered from the literature and validated by expert consensus in each country. Viraemic prevalence in this study ranged from 0.2% in Iran and Lebanon to 4.2% in Pakistan. The largest viraemic populations were in Pakistan (7001000 cases) and Indonesia (3187000 cases). Injection drug use (IDU) and a historically unsafe blood supply were major risk factors in most countries. Diagnosis, treatment and liver transplant rates varied widely between countries. However, comparison across countries was difficult as the number of cases changes over time. Access to reliable data on measures such as these is critical for the development of future strategies to manage the disease burden.

The hepatitis C virus (HCV) epidemic was forecasted through 2030 for 15 countries in Europe, the Middle East and Asia, and the relative impact of two scenarios was considered: increased treatment efficacy while holding the annual number of treated patients constant and increased treatment efficacy and an increased annual number of treated patients. Increasing levels of diagnosis and treatment, in combination with improved treatment efficacy, were critical for achieving substantial reductions in disease burden. A 90% reduction in total HCV infections within 15 years is feasible in most countries studied, but it required a coordinated effort to introduce harm reduction programmes to reduce new infections, screening to identify those already infected and treatment with high cure rate therapies. This suggests that increased capacity for screening and treatment will be critical in many countries. Birth cohort screening is a helpful tool for maximizing resources. Among European countries, the majority of patients were born between 1940 and 1985. A wider range of birth cohorts was seen in the Middle East and Asia (between 1925 and 1995).

The total number, morbidity and mortality attributed to viraemic hepatitis C virus (HCV) infections change over time making it difficult to compare reported estimates from different years. Models were developed for 15 countries to quantify and characterize the viraemic population and forecast the changes in the infected population and the corresponding disease burden from 2014 to 2030. With the exception of Iceland, Iran, Latvia and Pakistan, the total number of viraemic HCV infections is expected to decline from 2014 to 2030, but the associated morbidity and mortality are expected to increase in all countries except for Japan and South Korea. In the latter two countries, mortality due to an ageing population will drive down prevalence, morbidity and mortality. On the other hand, both countries have already experienced a rapid increase in HCV‐related mortality and morbidity. HCV‐related morbidity and mortality are projected to increase between 2014 and 2030 in all other countries as result of an ageing HCV‐infected population. Thus, although the total number of HCV countries is expected to decline in most countries studied, the associated disease burden is expected to increase. The current treatment paradigm is inadequate if large reductions in HCV‐related morbidity and mortality are to be achieved.

In order to perform selective isolation of bacteria tightly bound to the human gut, ileal biopsies of healthy volunteers were treated to wash out the mucus layer and loosely bound bacterial cells. Rod-shaped anaerobic bacteria that had remained attached to the epithelial cells were isolated and identified at the species level. One isolate was identified as belonging to the Bifidobacterium breve species, while all the others were lactobacilli of only two species, Lactobacillus mucosae and Lactobacillus gasseri. Members of these species were found previously in intestinal samples, but their predominance among bacteria strictly associated with the epithelium was not suspected before and suggests that these species may represent a specific subpopulation of tissue-bound bacteria. Physiological analysis indicated that all isolates were able to produce antimicrobials, grow and form biofilm in simulated intestinal fluid after exposure to gastric conditions. Some isolates were able to degrade mucin while none showed cytotoxicity in vitro on HT29 cells. The tight association of the strains isolated with ileal epithelial cells is presumably indicative of a direct interaction with the host cells. For this reason and for the absence of cytotoxicity in vitro, those isolates can be proposed as potential probiotic strains for human use.

Azo dyes belong to the most important group of synthetic colorants and are used extensively in the textile industries and food coloring. the present study the ability of Lactobacillus paracasei. isolate to modify the dyes; Tartrazin, Allura red and Amaranth was investigated. lactic acid bacteria (LAB) isolates were isolated from dried milk and identified by API 50 CHL Kit. All isolates screened for their capability to modify the food coloring Tartrazin, Amaranth, Allura red. The isolate modified dyes under anaerobic conditions, in MRS Broth. The product had a different color or color - less according differences in incubation time (0, 4, 24,72) hr. and different PH (5,6,8). It absorbed light at 427 and 520 nm,504 nm for Tar., Ama., All. Respectively that degrade dyes were characterized as . Were able to decolorize azo dyes (0.OO6 g/l00ml) within 24hr at optimized conditions pH 5- 6- 8, temperature 37oC.

Objective To determine if an elevated score on the Acute Physiology and Chronic Health Evaluation II (APACHE II) scoring system is associated with mortality of acutely ill gynecologic oncology patients. Methods Gynecologic oncology patients admitted to the surgical intensive care unit (ICU) were identified from the ICU data base. Their admission APACHE II score and type of gynecologic cancer were also extracted from the data base. Charts were reviewed to determine the disease status and reason for admission to the surgical ICU. Patient mortality was correlated with APACHE II scores. Results Forty-five gynecologic oncology patients were admitted to the surgical ICU from June 1988 to January 1992. They had a mean age of 62 years and various cancers: ovarian (24), cervical (16), and endometrial (five). The mean APACHE II score was 12 (range 2–26). Eight of 45 (18%) patients died. There was a significant correlation between APACHE II scores and mortality; patients with an APACHE II score of 20 or greater had a 78% risk of death compared to a 3% risk if the score was less than 20 ( P χ 2 test). Conclusion Elevated APACHE II scores are associated with mortality in acutely ill gynecologic oncology patients.

Aims: To identify the diversity of pigmented aerobic spore formers found in the environment and to characterize the chemical nature of this pigmentation. Materials and Results: Sampling of heat-resistant bacterial counts from soil, sea water and the human gastrointestinal tract. Phylogenetic profiling using analysis of 16S rRNA sequences to define species. Pigment profiling using high-performance liquid chromatography-photo diode array analysis. Conclusions: The most commonly found pigments were yellow, orange and pink. Isolates were nearly always members of the Bacillus genus and in most cases were related with known species such as Bacillus marisflavi, Bacillus indicus, Bacillus firmus, Bacillus altitudinis and Bacillus safensis. Three types of carotenoids were found with absorption maxima at 455, 467 and 492 nm, corresponding to the visible colours yellow, orange and pink, respectively. Although the presence of other carotenoids cannot be ruled out, these three predominant carotenoids appear to account for the pigments obtained in most pigmented bacilli, and our analysis reveals the existence of a C30 biosynthetic pathway. Interestingly, we report the presence of a water-soluble pigment that may also be a carotenoid. The function of carotenoids is photoprotection, and carotenoid-containing spores exhibited significantly higher levels of resistance to UV radiation than non–carotenoid-containing Bacillus species. Significance and Impact of the Study: This study demonstrates that pigmented bacilli are ubiquitous and contain new carotenoid biosynthetic pathways that may have industrial importance.

The serious and increasing form of the environmental pollution due to the electromagnetic interference is a big problem in all countries. Various forms of EMI may cause electrical and electronic malfunctions, can prevent the proper use of the of the radio frequency spectrum, can ignite flammable or other hazardous atmospheres, and may even have a direct effect on human tissue. The threat of EMI is controlled by adopting the practices of electromagnetic compatibility (EMC), currently NIS-Egypt has the facilities to describe the ability of electrical and electronic devices, unit of equipment or systems to operate without interfering with other systems, also describes the ability of such systems to operate as intended within a specified electromagnetic environment according to the EMC directives which determine tests, measurements, limits and the essential requirements that products must satisfied before freely marketed . The paper gives an over view about the EMC lab at NIS showing both its functions and activities.

Partial discharge (PD) measurement is a proven flaw detection technique for finding defects in the insulating materials and thus accessing the integrity of the tested insulation. The different PD measurement techniques as well as the interpretation of the measurement results have been important research topics in the recent years. This paper contributes to the detection of the presence of free conducting particles in oil insulated apparatus. Acoustic and electrical based PD measurements were undertaken by deliberately introducing metallic particles of various compositions and dimensions in oil. The acoustic signal, formed due to the statistical analysis of the particle impact momentum during motion in oil was used as the basis for designing and training an intelligent classifier for disclosing the characteristics of the contaminating particle. It is proven that measurements and simulation results of the produced acoustics are in homogeneity

Dental stains can be broadly classified as intrinsic or extrinsic. Intrinsic stains are a result of defects in tooth development, fluorosis, or acquired through the use of tetracycline. Extrinsic stains are localized mainly in the pellicle and are generated by the reaction between sugars and amino acids or acquired from the retention of exogenous chromophores in the pellicle. Three clinical methods are currently used for measuring stain removal and tooth whitening in the development of new whitening technologies: Lobene Stain Index, Shade Guide Color Change, and Minolta ChromaMeter. Professional tooth whitening products rely on proven technologies--35% hydrogen peroxide for in-office power bleaching or 10% to 15% carbamide peroxide for at-home bleaching--to reduce intrinsic stain and change the inherent tooth color. Over-the-counter tooth whitening products use a combination of surfactants, abrasives, anticalculus agents, and low levels of hydrogen peroxide to reduce extrinsic stain and help maintain tooth whiteness after professional treatment. Future technologies for whitening teeth could involve the use of activating agents to enhance the performance of hydrogen peroxide and natural enzymes.

The concentration and elimination rate of triclosan in saliva was assessed as part of a six week clinical study which evaluated the antiplaque efficacy of a 0.3% triclosan/PVM/MA copolymer/NaF dentifrice. On day one of the study the concentration of triclosan in whole saliva was determined at various times after dentifrice use for both the triclosan/PVM/MA copolymer/NaF and placebo dentifrice groups. The results indicated that the salivary triclosan levels ranged from 19.7 micrograms/ml at 5 minutes to 1 microgram/ml at 2 hours after use of the 0.3% triclosan dentifrice. The triclosan elimination curve for saliva exhibited a mono-exponential decline with a half-life of 26 minutes. The plaque content of triclosan, one hour after dentifrice use, which was determined at the end of the study, was found to be relatively high (25 micrograms/g) compared to that seen in saliva at the same time (6.2 micrograms/ml). It was concluded that the 0.3% triclosan/PVM/MA copolymer/NaF test dentifrice provided effective delivery and bioavailability of the antiplaque agent, triclosan. This was based on the relatively high salivary and plaque triclosan levels observed and the concomitant reduction in plaque formation seen at the end of the study.
