Abstract

The size effect of Ti-6Al-4V submillimeter structures manufactured by selective laser melting, which is critical for metallic mechanical metamaterials of unique mechanical properties, for example, negative Poisson's ratio and ultrahigh modulus, which show promise in biomedical, environmental, energy-related applications, has not been systematically investigated. Presented here are the quantification of the porosities by X-ray microtomography scans, texture analysis, and mechanical characterization of the additively manufactured Ti-6Al-4V microbeams. We found linearly decreasing porosities, increasing mechanical properties, and increasing texture in the microbeam with increasing diameter from 250 to 500 µm. The variation of microstructure in microbeams of different diameters and along the sample height, resulting from the printing parameters and the thermal conditions, leads to the discrepancy between the behavior observed in experiments and finite element simulation. Our results provide the structure-property-processing correlation to improve the manufacturing and prediction of the mechanical behavior of metallic mechanical metamaterials.

Abstract

Depuis 2007, des professionnels de santé mentale en Afrique sub-saharienne ont été formés à la thérapie de désensibilisation et de retraitement par les mouvements oculaires (EMDR). Cette étude qualitative a recouru à un dispositif afrocentré avec analyse thématique pour explorer les adaptations apportées au protocole standard EMDR, visant à le rendre culturellement pertinent pour des patients africains. Les participants étaient 25 thérapeutes EMDR (dont trois hommes, tranche d’âge 32-60 ans,x¯= 44) de cinq pays africains, qui pratiquaient l’EMDR depuis 1 à 11 ans (x¯= 7). Tous ont répondu à un questionnaire d’enquête, huit ont participé à une discussion de groupe, et deux ont fourni une analyse de notes de supervision. Les participants estimaient que l’EMDR était une thérapie utile et bénéfique et la préféraient aux autres thérapies en raison de sa nature non narrative et de ses résultats rapides. Nous avons identifié quatre domaines dans lesquels les thérapeutes africains ont régulièrement adapté le protocole standard : la formulation du texte du protocole, l’expression culturelle des pensées et des émotions, le choix des stimulations et la simplification des échelles quantitatives. Sur la base des résultats de cette étude, nous formulons de nombreuses recommandations pour des adaptations culturelles du protocole EMDR. Celles-ci comprennent des changements de langage pour prendre en compte la communication « orientée vers le nous » des patients ; des interprétations culturelles des pensées et des événements positifs et négatifs ; l’ajout d’activités culturelles telles que la danse, la musique et les pratiques religieuses comme exercices de ressourcement ; l’utilisation de gestes des mains ou de l’échelle picturale avec des visages au lieu d’échelles numériques ; et l’utilisation de tapotements pour les stimulations bilatérales au lieu de mouvements oculaires, parfois considérés comme de la « sorcellerie ». Nous abordons la pertinence de ces résultats pour la pratique et la formation EMDR. Nous recommandons aux chercheurs africains d’étudier plus avant l’acceptabilité, l’utilisation et l’efficacité de l’EMDR dans leur pays.

Abstract

In this work, we report an innovative approach to improve the interface properties of SiC/SiO2 metal oxide semiconductor (MOS) capacitors. High temperature (1350°C) oxidation under different ambient is followed by a combination of post-oxidation annealing (POA) treatments using N2, N2O and NO gases. TOF-SIMS analysis shows silicon and nitrogen peaks near the SiC/SiO2 interface. The silicon peak is attributed to the emission of silicon and carbon atoms during high temperature oxidation. The accumulation of nitrogen is caused by the presence of nitrogen during oxidation or POA. One of the lowest interface-trap densities along with good dielectric strength has been demonstrated with the N2 and NO gas POA treatment.

15. Current Diagnostic Pathways for Alzheimer’s Disease: A Cross-Sectional Real-World Study Across Six Countries

Abstract

Background: Diagnostic pathways for patients presenting with cognitive complaints may vary across geographies. Objective: To describe diagnostic pathways of patients presenting with cognitive complaints across 6 countries. Methods: This real-world, cross-sectional study analyzed chart-extracted data from healthcare providers (HCPs) for 6,744 patients across China, France, Germany, Spain, UK, and the US. Results: Most common symptoms at presentation were cognitive (memory/amnestic; 89.86%), followed by physical/behavioral (87.13%). Clinical/cognitive tests were used in > 95%, with Mini-Mental State Examination being the most common cognitive test (79.0%). Blood tests for APOE ɛ4/other mutations, or to rule out treatable causes, were used in half of the patients. Clinical and cognitive tests were used at higher frequency at earlier visits, and amyloid PET/CSF biomarker testing at higher frequency at later visits. The latter were ordered at low rates even by specialists (across countries, 5.7% to 28.7% for amyloid PET and 5.0% to 27.3% for CSF testing). Approximately half the patients received a diagnosis (52.1% of which were Alzheimer’s disease [AD]). Factors that influenced risk of not receiving a diagnosis were HCP type (higher for primary care physicians versus specialists) and region (highest in China and Germany). Conclusion: These data highlight variability in AD diagnostic pathways across countries and provider types. About 45% of patients are referred/told to ‘watch and wait’. Improvements can be made in the use of amyloid PET and CSF testing. Efforts should focus on further defining biomarkers for those at risk for AD, and on dismantling barriers such low testing capacity and reimbursement challenges.

Abstract

We implemented the HIV preexposure prophylaxis (PrEP) care continuum among individuals receiving medication for opioid use disorder (MOUD). We screened HIV-negative MOUD participants for PrEP eligibility by assessing injection drug use risk factors and sexual behaviors. Implementation of the PrEP care continuum was challenging; less than a third of MOUD participants were aware of PrEP, and very few initiated PrEP. Findings should promote the development of effective interventions to increase engagement in PrEP during MOUD treatment. Abstract

We use the surface detector of the Pierre Auger Observatory to search for air showers initiated by photons with an energy above 1019 eV. Photons in the zenith angle range from 30∘ to 60∘ can be identified in the overwhelming background of showers initiated by charged cosmic rays through the broader time structure of the signals induced in the water-Cherenkov detectors of the array and the steeper lateral distribution of shower particles reaching ground. Applying the search method to data collected between January 2004 and June 2020, upper limits at 95% CL are set to an E-2 diffuse flux of ultra-high energy photons above 1019 eV, 2 × 1019 eV and 4 × 1019 eV amounting to 2.11 × 10-3, 3.12 × 10-4 and 1.72 × 10-4 km-2 sr-1 yr-1, respectively. While the sensitivity of the present search around 2 × 1019 eV approaches expectations of cosmogenic photon fluxes in the case of a pure-proton composition, it is one order of magnitude above those from more realistic mixed-composition models. The inferred limits have also implications for the search of super-heavy dark matter that are discussed and illustrated.

28. Veteran community engagement and social connection needs following inpatient psychiatric hospitalization

To understand barriers and facilitators to engaging in community activities for increasing social connectedness among recently psychiatrically hospitalized veterans, a population at elevated risk for suicide.We completed 30 semistructured qualitative interviews with veterans within 1 week of discharge from inpatient psychiatric hospitalization. Our interviews focused on understanding past and current barriers, facilitators, and needs for engaging in community activities after psychiatric hospitalization. Data were analyzed using a modified grounded theory approach.Veterans shared feeling a lack of belonging and discussed several barriers to community engagement including lack of self-confidence, limited knowledge of opportunities, and negative expectations. Veterans identified several ways to facilitate engagement in community activities such as centralizing information on community activities and providing active support posthospitalization.Veterans by and large valued community and the role of community activities for increasing social connectedness. However, more active intervention for supporting engagement in community activities appears necessary to facilitate connection posthospitalization. Abstract

A superenhancer at the retinoic acid receptor alpha (RARA) gene is associated with RARA mRNA overexpression in ∼30% of non-acute promyelocytic leukemia acute myeloid leukemia (AML) and in ∼50% of myelodysplastic syndromes (MDS). RARA overexpression is an actionable target for treatment with tamibarotene, an oral potent and selective RARα agonist. Sensitivity to the RARα agonist tamibarotene was demonstrated in RARA-high but not RARA-low preclinical AML models. The combination of oral tamibarotene plus azacitidine was evaluated in a phase 2 clinical study in 51 newly diagnosed unfit patients with AML identified as RARA-positive (n = 22) or RARA-negative (n = 29) for RARA mRNA overexpression in peripheral blasts using a blood-based biomarker test. In 18 response-evaluable RARA-positive patients, complete remission (CR)/CR with incomplete hematologic recovery rate was 61%, CR rate was 50%, and time to initial composite CR was rapid at 1.2 months. Transfusion independence was attained by 72% of RARA-positive patients. In contrast, 28 response-evaluable RARA-negative patients had response rates that were consistent with azacitidine monotherapy. Tamibarotene in combination with azacitidine was well tolerated. The majority of nonhematologic adverse events were low grade and hematologic adverse events were comparable to single-agent azacitidine, demonstrating that there was no additional myelosuppression when tamibarotene was combined with azacitidine. These results support further evaluation of tamibarotene-based treatment strategies in patients with AML or MDS with RARA overexpression to provide a targeted approach with the goal of improving patient outcomes. Abstract

Objective Although the prevalence of obesity is constantly high, the care situation for patients with obesity is still insufficient. The advent of digital, evidence-based therapies to treat obesity could help alleviate this situation. This study therefore aimed at evaluating the effectiveness of the app-based, multimodal weight loss programme zanadio. Methods To examine the effectiveness for sustained weight loss of zanadio, a randomised-controlled trial (DRKS00024415) was conducted between January 2021 and March 2022. 150 adults with obesity were randomised into an intervention group or a control group. While the participants of the intervention group used zanadio for one year, participants in the control group were not allowed to use the app during the study period but were free to start other weight-loss attempts. The primary endpoint weight change (in %) and the secondary endpoints quality of life, well-being, and waist-to-height ratio, were assessed every three months (T0, T1, T2, T3, T4) via telephone interviews and online questionnaires. For the statistical analyses intention-to-treat and per-protocol analyses were performed. Results After 12 months, based on an intention-to-treat analysis, participants of the intervention group lost on average -7.75% (95% CI=[-9.66, -5.84]) of their initial body weight, while the control group showed no weight loss at the end of the study (M=0.00%, [-1.98, 1.99]). Importantly, the intervention group achieved a clinically relevant (< 5%) and significant weight reduction. These results were confirmed by a per-protocol analysis. Moreover, all secondary endpoints improved significantly in the intervention group, with significantly greater improvements in well-being and waist-to-height ratio than in the control group. Conclusion This study showed that adults with obesity who have used zanadio achieved a significant and clinically relevant weight loss within 12 months and further improved other obesity-related health variables compared to a wait-list control group. Limitations of the study are the mainly female sample and the exclusively self-reported outcome measures. Due to its effectiveness and flexible applicability, zanadio represents a low-threshold and widely accessible treatment offer for patients in Germany, that might alleviate the present care gap for patients with obesity.

Abstract

Organized universal health coverage has not yet been introduced in most developing countries, including Bangladesh. Private health care is affordable only to the high-income group. The aim of this retrospective observational study was to evaluate health service coverage with disease prevention, access to health information, and cost control through health financing to help take appropriate decisions for the betterment of garment workers in Bangladesh. The study was conducted in seven readymade garments (RMG) factories in the Gazipur district from 24 April 2014 to 23 April 2015.A total of 9717 workers aged 18 to 60 years and belonging to the lowest salary groups were included in this Employer-Sponsored Health Insurance (ESHI) scheme. This new model of Health Micro Insurance (HMI) had treatment, laboratory facilities, health education, and medicine supply. The annual coverage for treatment cost was up to 15000 Bangladeshi taka (BDT) or US$192.8 and the premium for enrolment in the scheme was 487 BDT (US$6.3). An agreement foresaw that the surplus cost accrued was equally shared between the insurance company and the factory owner. A common software was used to generate and view all medical information. A total of 4524 (46.6%) workers (60.5% male and 39.5% female) received treatment. The participant's mean age was 28.3 years. The mean consultancy rate was 4.7 times. Participants mostly suffered from gastrointestinal problems(24.4%), and most prescribed medications were anti-ulcer drugs. The median value of drug cost, investigation cost, consultancy fees, and total medical cost were 126 (1.49 USD), 315 (3.71 USD), 200 (2.36 USD), and 734 BDT (8.66), respectively. The annual net premium paid by the factory owner was 4094504 BDT (48744 USD), and the total healthcare cost accrued was 5230156 BDT (62263 USD). This ESHI scheme is a better option for HMI for making healthcare accessible to the largest RMG sector in a developing country like Bangladesh.

Abstract

Fluent API is an object-oriented pattern for elegant APIs and embedded DSLs. A smart fluent API can enforce the API protocol or DSL syntax at compile time. Since fluent API implementations typically rely on overloading function names, they are hard to realize in functional programming languages. This work shows how functional fluent APIs can be implemented in the absence of name overloading, by relying on parametric polymorphism and Hindley-Milner type inference. The implementation supports fluent API protocols in the regular- and deterministic context-free language classes, and even beyond.

Die Bilanzierung von Umweltwirkungen spielt eine entscheidende Rolle bei der Entwicklung nachhaltiger und umweltfreundlicher Technologien und Konzepte. Dies gilt auch in der Entwicklung von Gebäuden mit adaptiven Hüllen und Strukturen und stellt darüber hinaus weitreichende Anforderungen an alle beteiligten Disziplinen. Die vollständige Integration der Ökobilanzierung in den Planungs- und Auslegungsprozess ermöglicht, Umweltwirkungen als Optimierungsgrößen in den komplexen, dynamischen Berechnungswerkzeugen einzusetzen. Die bisherigen Ergebnisse des SFB 1244 bescheinigen adaptiven Tragwerken und Fassaden großes Potenzial zur Einsparung von Ressourcen und Umweltwirkungen. Die ganzheitliche Betrachtungsweise, sowohl in Bezug auf den Lebenszyklus als auch auf die interdisziplinären Wechselwirkungen, stellt sicher, dass die relevanten Effekte und Einflüsse in der Bilanzierung berücksichtigt werden. Das stellt die Methode der Ökobilanzierung selbst jedoch vor neue Herausforderungen im Umgang mit einer Vielzahl an Varianten und den umfangreichen Wechselwirkungen zwischen Auslegung und Einflüssen auf Parameter in der Nutzungsphase, wie z. B. den Energieverbrauch oder die Lebensdauer.

Abstract

Reducing treatment burden is a priority for people with cystic fibrosis, whose health has benefited from using new modulators that substantially increase CFTR protein function. The SIMPLIFY study aimed to assess the effects of discontinuing nebulised hypertonic saline or dornase alfa in individuals using the CFTR modulator elexacaftor plus tezacaftor plus ivacaftor (ETI).The SIMPLIFY study included two parallel, multicentre, open-label, randomised, controlled, non-inferiority trials at 80 participating clinics across the USA in the Cystic Fibrosis Therapeutics Development Network. We included individuals with cystic fibrosis aged 12-17 years with percent predicted FEVFrom Aug 25, 2020, to May 25, 2022, a total of 672 unique participants were screened for eligibility for one or both trials, resulting in 847 total random assignments across both trials with 594 unique participants. 370 participants were randomly assigned in the hypertonic saline trial and 477 in the dornase alfa trial. Participants across both trials had an average ppFEVIn individuals with cystic fibrosis on ETI with relatively well preserved pulmonary function, discontinuing daily hypertonic saline or dornase alfa for 6 weeks did not result in clinically meaningful differences in pulmonary function when compared with continuing treatment.

Abstract

The sensorimotor system has the constant challenge of dealing with the naturally occurring variability in our movements. Here, we investigated the potential roles of muscular co-contraction and visuomotor feedback responses to regulate movement variability. When we visually amplified movements, we found that the sensorimotor system primarily uses muscular co-contraction to regulate movement variability. Interestingly, we found that muscular co-contraction was modulated relative to inherent visuomotor feedback responses, suggesting an interplay between impedance and feedback control.
