A problemática da mobilidade humana é um tema coevo e que merece a mais cuidada atenção da comunidade internacional, sobretudo devido à volatilidade da perceção do fenómeno pela sociedade global, profundamente digitalizada e galvanizada. Evidenciando a preeminência de Trump na politização infodémica da migração internacional, refletimos analiticamente sobre a volubilidade humana numa América polarizada e egoísta, cerrada sob si mesma e envolta em medidas anti-imigração, desumanizantes e preconceituosas, em desdém pelas necessidades humanitárias dos migrantes mais desfavorecidos. Paradoxalmente, a híper securitização das fronteiras favoreceu o aumento da procura por rotas transfronteiriças clandestinas, controladas pelo crime organizado transnacional, subjugando uma parte substancial dos fluxos migratórios internacionais ao seu jugo. Em contraciclo, circunscrevemos a premência de uma nova abordagem colaborativa, holística e humanista pelo constructo internacional, unificado em torno da regulação homogénea e segura dos fluxos das pessoas em movimento (Commission on Human Security, 2003) e na plena valorização dos Direitos Humanos, por via de uma compreensão congruente da mobilidade humana, informada por factos, pela ciência e pela ética. The problematic of human mobility is a coeval issue that deserves the most careful attention from the international community, especially given the volatility of the phenomenon's perception by the deeply digitalised and galvanised global society. Evidencing Trump's pre-eminence in the infodemic politicisation of international migration, we reflect analytically on human volubility in a polarised and selfish America, closed in on itself and shrouded in anti-immigration, dehumanising and prejudiced measures, in disdain for the humanitarian needs of the most disadvantaged migrants. Paradoxically, the hyper-securitisation of borders has favoured the increase of the demand for clandestine cross-border routes, controlled by transnational organised crime, subjugating a substantial part of the international migratory flows to its yoke. In countercycle, we circumscribe the urgency of a new collaborative, holistic and humanist approach by the international construct, unified around the homogeneous and secure regulation of the flows of people on the move (Commission on Human Security, 2003) and in the full valorisation of Human Rights, through a congruent understanding of human mobility, informed by facts, science, and ethics. La problemática de la movilidad humana es una cuestión coetánea que merece la máxima atención por parte de la comunidad internacional, sobre todo teniendo en cuenta la volatilidad de la percepción del fenómeno por parte de la sociedad global, profundamente digitalizada y galvanizada. Evidenciando la preeminencia de Trump en la politización infodémica de las migraciones internacionales, reflexionamos analíticamente sobre la volubilidad humana en una América polarizada y egoísta, encerrada en sí misma y envuelta en medidas antiinmigratorias, deshumanizadoras y prejuiciosas, en el desprecio a las necesidades humanitarias de los migrantes más desfavorecidos. Paradójicamente, la hipersecuritización de las fronteras ha favorecido el aumento de la demanda de rutas transfronterizas clandestinas, controladas por el crimen organizado transnacional, sometiendo a su yugo una parte sustancial de los flujos migratorios internacionales. Objective: To determine whether pre-hospital statin use is associated with lower renal replacement therapy requirement and/or death during intensive care unit stay. Methods: Prospective cohort analysis. We analyzed 670 patients consecutively admitted to the intensive care unit of an academic tertiary-care hospital. Patients with ages ranging from 18 to 80 years admitted to the intensive care unit within the last 48 hours were included in the study. Results: Mean age was 66±16.1 years old, mean body mass index 26.6±4/9kg/m2 and mean abdominal circumference was of 97±22cm. The statin group comprised 18.2% of patients and had lower renal replacement therapy requirement and/or mortality (OR: 0.41; 95%CI: 0.18-0.93; p=0.03). The statin group also had lower risk of developing sepsis during intensive care unit stay (OR: 0.42; 95%CI: 0.22-0.77; p=0.006) and had a reduction in hospital length-of-stay (14.7±17.5 days versus 22.3±48 days; p=0.006). Statin therapy was associated with a protective role in critical care setting independently of confounding variables, such as gender, age, C-reactive protein, need of mechanical ventilation, use of pressor agents and presence of diabetes and/or coronary disease. Conclusion: Statin therapy prior to hospital admission was associated with lower mortality, lower renal replacement therapy requirement and sepsis rates.

Fundação de Amparo à Pesquisa do Estado de São Paulo (FAPESP) Genetic variations in TGF-beta and IFN-gamma may interfere with proinflammatory cytokine production and, consequently, may be involved with inflammatory diseases, as acute kidney injury (AKI). We considered that genetic polymorphisms of these cytokines may have a crucial role in the outcome of critically ill patients. To investigate whether the genetic polymorphisms of rs1800470 (codon 10 T/C), rs1800471 (codon 25 C/G) from the TGF-beta, and rs2430561 (+874 T/A) from IFN-gamma may be a risk factor for ICU patients to the development of AKI and/or death. in a prospective nested case-control study, were included 139 ICU patients who developed AKI, 164 ICU patients without AKI, and 244 healthy individuals. We observed a higher frequency to T/A genotype for IFN-gamma (intermediate producer phenotype) and higher frequency of TT GG and TC GG genotype (high producer) for TGF-beta polymorphism in overall population. However, these polymorphisms have not been shown as a predictor of risk for AKI and death. We found an increased prevalence of high and intermediate producer phenotypes from TGF-beta and IFN-gamma, respectively, in patients in ICU setting. However, the studied genetic polymorphism of the TGF-beta and IFN-gamma was not associated as a risk factor for AKI or death in our population. Background: Patients undergoing orthotropic liver transplant (LTx) often present with chronic kidney disease (CKD). Identification of patients who will progress to end-stage renal disease (ESRD) might allow not only the implementation of kidney protective measures but also simultaneous kidney transplant.Study Design: Retrospective cohort study in adults who underwent LTx at a single center. ESRD, death, and composite of ESRD or death were studied outcomes.Results: 331 patients, who underwent LTx, were followed up for 2.6 +/- 1.4 years; 31(10%) developed ESRD, 6 (2%) underwent kidney transplant after LTx and 25 (8%) remained on chronic hemodialysis. Patients with preoperative eGFR lesser than 60 ml/min per 1.73 m(2) had a 4-fold increased risk of developing ESRD after adjustment for sex, diabetes mellitus, APACHE II score, use of nephrotoxic drugs, and severe liver graft failure (HR = 3.95, 95% CI 1,73, 9.01; p = 0.001). Other independent risk factors for ESRD were preoperative diabetes mellitus and post-operative severe liver graft dysfunction.Conclusion: These findings emphasize low eGFR prior to LTx as a predictor for ESRD or death. the consideration for kidney after liver transplant as a treatment modality should be taken into account for those who develop chronic kidney failure after LTx. O processo crítico-reflexivo na investigação das ciências sociais, mormente nas ciências policiais – intrinsecamente plurais, complexas e multidisciplinares – exige ao investigador um bom domínio da diversidade de expressões e da terminologia correspondente, a par de um conhecimento aprofundado dos principais “blocos de construção” da investigação social genérica (Grix, 2002; Hay, 2007), permeáveis a diferentes tradições eruditas e, muitas vezes, a visões concorrentes. A aparente relação direcional, mas não necessariamente determinista, entre aqueles alicerces da investigação e a emergência de uma propensão plástica, dialética e complementar entre as teorias e metodologias, parece apontar para soluções eventualmente mais ricas, rigorosas e completas dos objetos de estudo, conquanto permitam a corroboração e a análise das imperfeições delas resultantes e/ou escolhas efetuadas. Este exercício analítico debruça-se precisamente sobre as controvérsias e complexidades implícitas às doutrinas fundamentais que precedem a formação de novo conhecimento, realçando o potencial da Plasticidade – enquanto novo conceito lógico, (ainda) indeterminado, de criação, combinação e maleabilidade – no refinamento da forma final como aqueles componentes-chave da investigação são concebidos, harmonizados e incorporados numa estratégia multifacetada que se pretende que seja a melhor e mais completa aproximação ao objeto de estudo, sujeita a uma perene vigilância epistemológica. The critical-reflective process in social science research, especially in the police science – intrinsically plural, complex, and multidisciplinary – requires the researcher to have a good command of the variety of terms and the corresponding terminology, together with an in-depth knowledge of the main "building blocks" of generic social research (Grix, 2002; Hay, 2007), permeable to different erudite traditions and often competing visions. The apparent directional relationship, but not necessarily deterministic, between those foundations of research and the emergence of a plastic, dialectic and complementary propensity between theories and methodologies seems to point to possibly richer, more rigorous, and complete solutions of the objects of study, while allowing the corroboration and analysis of the resulting imperfections and/or choices made. This analytical exercise focuses precisely on the controversies and complexities implicit in the fundamental theories that precede the formation of new knowledge, highlighting the potential of Plasticity – as a new logical, (still) undetermined concept of creation, combination and malleability – in the refinement of the final format in which those key components of research are conceived, harmonized and incorporated into a multi-faceted strategy that is intended to be the best and most complete approximation to the object of study, subject to a perennial epistemological vigilance. info:eu-repo/semantics/publishedVersion

Considérée comme l’une des notions les plus ambivalentes et les plus controversées des relations internationales et des études de sécurité, la sécurité recèle de nombreuses complexités et une multitude d’éléments fondamentaux, sujets à une volatilité constante et, par conséquent, à des interprétations disparates, en fonction du prisme d’analyse. Dans un monde globalisé et interdépendant, la compréhension du modèle international de sécurité corrélative, caractéristique de tout État contemporain, est fondamentale pour parvenir à une perspective holistique de la notion de sécurité nationale. Aujourd’hui, plus que jamais, nous confirmons que la sécurité d’un État donné se construit s’invente par rapport à la condition d’autres États, immergés dans un réseau global, composite, hétérogène et anarchique d’interdépendance sécuritaire (Brodeur, 2010). L’insécurité, à son tour, est associée à la proximité géographique entre les États. En conclusion, le contexte global est essentiel pour l’analyse des grandes puissances et des références systémiques (Brown, 1989), justifiés, par exemple, dans les mécanismes permutatifs de coopération policière. Le courant classique de la sécurité est basé sur le raisonnement selon lequel la sécurité (politico-militaire) d’un nombre significatif d’acteurs au niveau unitaire est assimilée à un groupe de sécurité, quelle que soit sa classification en tant que sous-système ou région (Haas, 1970 ; Haftendorn, 1990).
