The Great Salt Lake is of environmental and economic value yet is threatened by various factors including drought and water diversion for irrigation purposes. Management efforts are required to preserve this saline lake; such efforts include accurately estimating the exchange flow through an opening in a railroad causeway that divides the lake. This study investigated two modeling approaches for predicting these discharges, a physics-based computational fluid dynamics model and a data-driven artificial neural network model. Good agreement was found between both models, and the advantages each provides to water management efforts are noted. Results indicate that, regardless of the modeling tool, accurate field data is invaluable when studying a hydraulic structure.

The BRCA Challenge is a long-term data-sharing project initiated within the Global Alliance for Genomics and Health (GA4GH) to aggregate BRCA1 and BRCA2 data to support highly collaborative research activities. Its goal is to generate an informed and current understanding of the impact of genetic variation on cancer risk across the iconic cancer predisposition genes, BRCA1 and BRCA2. Initially, reported variants in BRCA1 and BRCA2 available from public databases were integrated into a single, newly created site, www.brcaexchange.org. The purpose of the BRCA Exchange is to provide the community with a reliable and easily accessible record of variants interpreted for a high-penetrance phenotype. More than 20,000 variants have been aggregated, three times the number found in the next-largest public database at the project’s outset, of which approximately 7,250 have expert classifications. The data set is based on shared information from existing clinical databases—Breast Cancer Information Core (BIC), ClinVar, and the Leiden Open Variation Database (LOVD)—as well as population databases, all linked to a single point of access. The BRCA Challenge has brought together the existing international Evidence-based Network for the Interpretation of Germline Mutant Alleles (ENIGMA) consortium expert panel, along with expert clinicians, diagnosticians, researchers, and database providers, all with a common goal of advancing our understanding of BRCA1 and BRCA2 variation. Ongoing work includes direct contact with national centers with access to BRCA1 and BRCA2 diagnostic data to encourage data sharing, development of methods suitable for extraction of genetic variation at the level of individual laboratory reports, and engagement with participant communities to enable a more comprehensive understanding of the clinical significance of genetic variation in BRCA1 and BRCA2., Author summary The goal of this study and paper has been to develop an international resource to generate an informed and current understanding of the impact of genetic variation on cancer risk across the cancer predisposition genes, BRCA1 and BRCA2. Strongly correlated systems of fermions have an interesting phase diagram arising from the Hubbard gap. Excitation across the gap leads to the formation of doubly occupied lattice sites (doublons). This state offers interesting electronic and optical properties. Moreover, when the system is driven out of equilibrium interesting collective dynamics may arise that are related to the spatial propagation of doublons. Here, a novel mechanism that was recently proposed by us [Balzer \textit{et al.}, submitted for publication] is verified by exact diagonalization and nonequilibrium Green functions (NEGF) simulations---fermionic doublon creation by the impact of energetic ions. We report the formation of a nonequilibrium steady state with homogeneous doublon distribution. A physically intuitive picture is given in terms of an analytical model for a two-site system where the doublon formation is explained in terms of a two-fold passage of an avoided crossing (Landau-Zener picture). The effect should be particularly important for strongly correlated finite systems, such as graphene nanoribbons, and directly observable with fermionic atoms in optical lattices. We demonstrate that doublon formation and propagation in correlated lattice systems can be accurately simulated with NEGF. In addition to two-time results we present single-time results within the generalized Kadanoff-Baym ansatz (GKBA) with Hartree-Fock propagators (HF-GKBA), and we present systematic improvements that use correlated propagators (correlated GKBA).

Background It is unclear whether biomarkers of alveolar damage (surfactant protein D, SPD) or conductive airway damage (club cell secretory protein 16, CC16) measured early after intensive care admittance are associated with one-month clinical respiratory prognosis. If patients who do not recover respiratory function within one month can be identified early, future experimental lung interventions can be aimed toward this high-risk group. We aimed to determine, in a heterogenous critically ill population, whether baseline profound alveolar damage or conductive airway damage has clinical respiratory impact one month after intensive care admittance. Methods Biobank study of biomarkers of alveolar and conductive airway damage in intensive care patients was conducted. This was a sub-study of 758 intubated patients from a 1200-patient randomized trial. We split the cohort into a “learning cohort” and “validating cohort” based on geographical criteria: northern sites (learning) and southern sites (validating). Results Baseline SPD above the 85th percentile in the “learning cohort” predicted low chance of successful weaning from ventilator within 28 days (adjusted hazard ratio 0.6 [95% CI 0.4–0.9], p = 0.005); this was confirmed in the validating cohort. 9. Development of Concepts for Handling Firewood for Wood-Burning Stoves Reducing Cases of Burning Moist Firewood

- Abstract
INTRODUCTION Burning moist firewood is a common problem, which leads to particle emissions and loss of energy in the firewood, both due to low combustion temperatures. The main problem is that it is difficult for many users to estimate the correct moisture-content of the firewood. Often the users do not store the firewood properly, keeping the firewood from drying fast. Two concepts were developed to solve these problems in order to reduce the number of cases with users burning moist firewood. The dry firewood provides up to 25 % more energy, making the use of wood-burning stoves more sustainable and less polluting. ABSTRACT Burning moist firewood most often happen in the group of users of wood-burning stoves, who does not have the stove as their primary source of heat. This group of users buys their firewood from retailers in amounts of 2 m3 as either "air dried", containing about 25 % water, or "oven dried", containing about 20 % of water. The users find it difficult to estimate the correct moisture content of their firewood, which leads to burning moist firewood. It adds to the problem that the firewood is often not stored properly: covered from rain with a roof, lifted off the ground and with plenty of air-circulation. This was concluded through userinterviews, visits and analysis of the data collected on field work. To obtain a combustion temperature high enough to reduce the emission of particles, the firewood should contain less that 18 % water, but optimally about 15 % water. The first concept is short pieces of firewood. Regular pieces are 0.35 m long, but these will be 0.15 m. Results of drying-tests, performed for Copenhagen University by A. Bergstedt, show that short pieces dry faster than long pieces, but they also absorb moisture faster, which emphasizes the importance of proper storage. Storage and doubt-issues are solved by the other concept; The Cassette. The Cassette will isolate the firewood from moisture, drying it actively by circulating dry air around it. The moisture content will be measured by either moisture-content of the air in the cassette or the total weigh of the firewood. LED's will show when the firewood is ready to be burnt: Red for too moist, green for dry enough. Calculations show that drying 2 m3 firewood, from containing 25 % water to 15 % water, will provide the user with up to 25 % more energy when burning the firewood. By only burning dry firewood, the users will only have to buy 2 m3 instead of 4 m3 to cover their required amount of energy, 2400 kWh/season, since 2m3 of beech containing 25 % water will provide about 2120 kWh and 2 m3 of beech containing 15 % water will provide 2650 kWh (more than the energy needed for one season). This way the user can halve the amount of firewood that must be bought every season. The amount of particles emitted by burning firewood will be lowered in general and the users will be able to use their wood-burning stoves in a more sustainable way by using less firewood.

10. Building 1163/4: design of a net-zero energy building

- Abstract
INTRODUCTION The new bachelor education in building design has made new installations for teaching and workshops necessary to complement building 117. Our challenge is to elaborate strategies to design a building containing those facilities by focusing into making a net-zero energy building. By combining the use of innovative and low-energy consuming systems and an optimized architecture, we expect to reach minimal running energy consumption per student. Our goal is to go further than what is set by the future 2020 Danish energy frame. This building is thought to show the potential of new low-energy designs on DTU Campus. PROJECT DEVELOPMENT After gathering information and defining the functional requirements linked to the new education, we have, in a first phase, performed iDBuild simulations to do a primary optimization of the different rooms' geometry and indoor climate. This phase has raised some indoor environment problems to solve, especially high cooling loads driven mainly by occupants and equipment. This led us to consider innovative and low-energy HVAC (Heat, Ventilation and Air Conditioning) solutions in a second conceptual design phase. We have designed the building to use as much as possible natural and passive means to regulate the indoor environment: Natural ventilation driven by the stack effect in the central atrium Optimization of the solar gains during the heating and cooling seasons Harvesting of natural resources (solar electricity and heating, ground-source energy) The use of Thermally Active Building Systems (TABS) for heating and cooling, com bined with a concrete structure, is an energy efficient way to condition the building environment. It is ideal for night cooling, reduces the electricity consumption of ventilation and uses the property of embedded energy in concrete. Using displacement ventilation in high rooms (ateliers) is a highly efficient ventilation process, which specific fan power is lower than that required by the 2020 energy frame. After a study to support sustainability with low-carbon footprint and minimal strain on resources, and at the same time comply with the requirements linked to the energy systems, a concrete structure has been decided. The building envelope itself uses "Fiberline"-type materials. In accordance to the current DTU buildings layout, the building 116 3/4 is a modern and optimized design. Software are used for instance to optimize the "niched" windows layout according to the visual environment requirements. CONCLUSION The design of building 1163/4 intends to demonstrate how high sustainability standards can be integrated in the pursuit of a good learning environment at DTU. The project is also meant to be a base for further expansion of DTU green campus.

Contains fulltext : 98250.pdf (Publisher’s version ) (Closed access) BACKGROUND AND OBJECTIVES: Radiographic calcification and arterial stiffness each individually are predictive of outcome in dialysis patients. However, it is unknown whether combined assessment of these intermediate endpoints also provides additional predictive value. DESIGN, SETTING, PARTICIPANTS, & MEASUREMENTS: Scoring of abdominal aortic calcification (AAC) using plain lateral abdominal x-ray and measurement of carotid-femoral pulse wave velocity (PWV) were performed in a cohort of 1084 prevalent dialysis patients recruited from 47 European dialysis centers. RESULTS: During a follow-up of 2 years, 234 deaths and 91 nonfatal cardiovascular (CV) events occurred. Compared with the lowest tertile of AAC, the risk of an event was increased by a factor 3.7 in patients with a score of 5 to 15 (middle tertile), and by a factor 8.6 in patients with scores of 16 to 24. Additionally, each 1-m/s increase in PWV was associated with a 15% higher risk. At higher AAC (scores >/= 5), the effect of PWV was attenuated because of a negative PWV x AAC interaction (hazard ratio [HR]: 0.895 and 0.865 for middle and upper AAC tertiles). After accounting for age, diabetes, and serum albumin, AAC and PWV remained independent predictors of outcome. CONCLUSIONS: AAC and central arterial stiffness are independent predictors of mortality and nonfatal CV events in dialysis patients. The risk associated with an increased PWV is less pronounced at higher levels of calcification. Assessment of AAC and PWV is feasible in a clinical setting and both may be used for an accurate CV risk estimation in this heterogeneous population.

Background Irritable bowel syndrome is the most common diagnosis in gastroenterology. Trials suggest certain probiotics to be beneficial. Aim To investigate the effects of multispecies probiotic supplementation (Lactobacillus rhamnosus GG, L. rhamnosus Lc705, Propionibacterium freudenreichii ssp. shermanii JS and Bifidobacterium animalis ssp. lactis Bb 12) on abdominal symptoms, quality of life, intestinal microbiota and inflammatory markers in irritable bowel syndrome. Methods Eighty-six irritable bowel syndrome patients (Rome 11 criteria) participated in this randomized, placebo-controlled 5-month intervention. Patients were randomized to receive daily either multispecies probiotic supplementation or placebo. Irritable bowel syndrome symptoms, quality of life, microarray-based intestinal microbiota stability (n = 20), serum cytokines and sensitive C-reactive protein were monitored. Results The composite irritable bowel syndrome score had at 5 months decreased 14 points (95% CI: -19 to -9) from baseline with the multispecies probiotic vs. three points (95% CI: -8 to 1) with placebo (P = 0.0083). Especially, distension and abdominal pain were affected. A stabilization of the microbiota was observed, as the microbiota similarity index increased with the probiotic supplementation (1.9 +/- 3.1), while it decreased with placebo (-2.9 +/- 1.7). No differences were seen in C-reactive protein. Conclusions This multispecies probiotic seems to be an effective and safe option to alleviate symptoms of irritable bowel syndrome, and to stabilize the intestinal microbiota.

Objective: To determine the incidence and clinical effects of antibodies that develop during treatment with natalizumab. Methods: In two randomized, double-blind, placebo-controlled studies (natalizumab safety and efficacy in relapsing remitting multiple sclerosis [MS, AFFIRM] and safety and efficacy of natalizumab in combination with interferon β-1a [INFβ1a] in patients with relapsing remitting MS [SENTINEL]) of patients with relapsing multiple sclerosis, blood samples were obtained at baseline and every 12 weeks to determine the presence of antibodies against natalizumab. Antibodies to natalizumab were measured using an ELISA. Patients were categorized as “transiently positive” if they had detectable antibodies (≥0.5 μg/mL) at a single time point or “persistently positive” if they had antibodies at two or more time points ≥6 weeks apart. Results: In the AFFIRM study, antibodies were detected in 57 of 625 (9%) of natalizumab-treated patients: Twenty (3%) were transiently positive and 37 (6%) were persistently positive. Persistently positive patients showed a loss of clinical efficacy as measured by disability progression ( p ≤ 0.05), relapse rate ( p = 0.009), and MRI ( p ≤ 0.05) compared with antibody-negative patients. In transiently positive patients, full efficacy was achieved after approximately 6 months of treatment, the time when patients were becoming antibody negative. The incidence of infusion-related adverse events was significantly higher in persistently positive patients. Results of SENTINEL were similar to AFFIRM, except with regard to sustained disability progression; differences between persistently positive and antibody-negative patients were not statistically significant. Conclusions: The incidence of persistent antibody positivity associated with natalizumab is 6%. Reduced clinical efficacy is apparent in persistently positive patients. Patients with a suboptimal clinical response or persistent infusion-related adverse events should be considered for antibody testing. A mentally retarded girl with congenital blepharophimosis, ptosis, hypoplastic teeth and other traits consistent with Ohdo syndrome is reported. Her mother had had surgery for blepharophimosis as a small child, but did not fulfil the other criteria for Ohdo syndrome. The mother had also been abusing alcohol prior to the pregnancy. Differential diagnoses and possible modes of inheritance are discussed.

THE purpose of this report is to call attention to one of the complications of the recently recognized syndrome known as fetomaternal transfusion. A number of reports1 2 3 4 5 6 7 have described a clinical condition in the newborn infant characterized by severe anemia and shock that develops as a result of antepartum passage of fetal erythrocytes into the maternal circulation. Although it has been recognized that immediate transfusion is life saving in these patients, attention has not been called to the possibility of the early appearance of a severe iron-deficiency anemia in the infant as a sequela to this condition. This article presents selected examples of model-based prediction and control of integrated sewer-wastewater treatment systems, developed within the framework of the Storm- and WastewaterInformatics project (SWI). By exploiting all the available on-line information (e.g. radar based rainfall measurements, flow and water levels, operational parameters at treatment plants) it is possible to dynamically optimize the integrated storm- and wastewater systems according to different criteria (e.g. utilizing the system fully at all times and reducing volumes of combined sewer overflows, loads of pollutants discharged from treatment plants, etc.). The tools developed in the SWI project include (but are not limited to (i) rainfall nowcasting based on radar measurements, (ii) probabilistic flow forecasting based on data assimilation and stochastic models, (iii) prediction and optimization of wet-weather performance of wastewater treatment plants, and (iv) integrated control of the different elements of the integrated wastewater systems. Full-scale testing of these tools in different catchment located in Denmark ensure that the developed tools can represent an important step forwards for on-line operation of combined sewer networks and wastewater treatment plants.

The paper compares quantitative precipitation forecasts using weather radars and numerical weather prediction models. In order to test forecasts under different conditions, point-comparisons with quantitative radar precipitation estimates and raingauges are presented. Furthermore, spatial comparisons of forecasts and observations have shown good results during stratiform conditions, but more scattered results during convective conditions. Finally, the potential for applying forecasts as input to urban drainage models is investigated. Results prove promising.

Development of antithrombotic compounds has traditionally been performed in patients undergoing total hip and knee replacement surgery. A high number of asymptomatic deep-vein thromboses are radiologically detectable, and bleeding and other adverse events (AE) are easy to observe. However, standardization of study procedures and endpoints in early proof-of-concept studies and late pure clinical endpoint studies has been lacking. This has made comparison between studies difficult, economic analyses speculative and potential benefits of applying the drug regimen in non-selected patients uncertain. In this paper, the International Surgical Thrombosis Forum proposes a strategy for the clinical investigation of new pharmacological agents for the prophylaxis of postoperative thrombotic events.First, dose titration safety studies of short duration, in highly selected patients using objective venographic endpoints are recommended. Bleeding should be divided into the quantified volume of surgical bleeding and other adjudicated clinical bleeding events. The number of AE should be described for each dose step and classified according to International Coding of Diagnoses (ICD). Second, a dose confirmatory study of moderate exposure period and sufficient follow-up time is recommended. The exclusion criteria should be restricted to contraindications of the compared drugs and technical procedure.The efficacy, bleeding and AE should be similar to those used in dose-titration studies. In addition, the failure rate of the drug to exert its effect and the net clinical benefit should be calculated.Finally, trials with simple clinical endpoints and long follow-up should be conducted to evaluate the potential benefits of the drug-regimen in non-selected populations.
