Abstract: PURPOSE: To describe data on epidemiology, microbiology, clinical characteristics and outcome of adult patients admitted in the intensive care unit (ICU) with secondary peritonitis, with special emphasis on antimicrobial therapy and source control. METHODS: Post hoc analysis of a multicenter observational study (Abdominal Sepsis Study, AbSeS) including 2621 adult ICU patients with intra-abdominal infection in 306 ICUs from 42 countries. Time-till-source control intervention was calculated as from time of diagnosis and classified into 'emergency' (< 2 h), 'urgent' (2-6 h), and 'delayed' (> 6 h). Relationships were assessed by logistic regression analysis and reported as odds ratios (OR) and 95% confidence interval (CI). RESULTS: The cohort included 1077 cases of microbiologically confirmed secondary peritonitis. Mortality was 29.7%. The rate of appropriate empiric therapy showed no difference between survivors and non-survivors (66.4% vs. 61.3%, p = 0.1). A stepwise increase in mortality was observed with increasing Sequential Organ Failure Assessment (SOFA) scores (19.6% for a value ≤ 4-55.4% for a value > 12, p < 0.001). The highest odds of death were associated with septic shock (OR 3.08 [1.42-7.00]), late-onset hospital-acquired peritonitis (OR 1.71 [1.16-2.52]) and failed source control evidenced by persistent inflammation at day 7 (OR 5.71 [3.99-8.18]). Compared with 'emergency' source control intervention (< 2 h of diagnosis), 'urgent' source control was the only modifiable covariate associated with lower odds of mortality (OR 0.50 [0.34-0.73]). CONCLUSION: 'Urgent' and successful source control was associated with improved odds of survival. Appropriateness of empirical antimicrobial treatment did not significantly affect survival suggesting that source control is more determinative for outcome. 2. Floral Categorization by Bumblebees: The Perceptual and the Functional

Abstract: This study examines generalization and perceptual similarity judgments in bumblebees (Bombus impatiens) and addresses whether category formation of floral stimuli is based upon perceptual floral features alone or foraging experiences from reward associations. Previous experiments have found that bees transfer learning based on category membership, but little is known as to how membership is formed. Two experiments using twelve bumblebee colonies examined a) if bees were able to generalize between flowers with little or no perceptual similarity, and b) if bees were able to change which features of the flowers they generalize under different circumstances. All bees were given discrimination training in a radial arm maze using objects and corresponding photographs. Bees in Experiment 1 were trained to categorize stimuli based upon their reward associations while ignoring their perceptual features. Bees in Experiment 2 were trained to categorize either by floral type or dimensionality (2D vs 3D) of the stimuli. Results of tests using unrewarding stimuli revealed that bees were unable to group items without perceptual similarity cues. When perceptually similar flowers did not share the same reward, bees still generalized between them, regardless of experience. However, the reward values of the flowers were not spontaneously disregarded by the bees. Generalizations differed based on the bee’s experiences, although preferences for perceptual similarity remained. Bees grouped flowers based on the relevant features learned through experience rather than just relying on perceptual similarity, but also do not immediately discount similar flowers during foraging, giving a preliminary insight into the role of function in perceptual similarity judgments.

Abstract: Background No effective pharmacological or non-pharmacological interventions exist for patients with long COVID. We aimed to describe recovery 1 year after hospital discharge for COVID-19, identify factors associated with patient-perceived recovery, and identify potential therapeutic targets by describing the underlying inflammatory profiles of the previously described recovery clusters at 5 months after hospital discharge. Methods The Post-hospitalisation COVID-19 study (PHOSP-COVID) is a prospective, longitudinal cohort study recruiting adults (aged ≥18 years) discharged from hospital with COVID-19 across the UK. Recovery was assessed using patient-reported outcome measures, physical performance, and organ function at 5 months and 1 year after hospital discharge, and stratified by both patient-perceived recovery and recovery cluster. Hierarchical logistic regression modelling was performed for patient-perceived recovery at 1 year. Cluster analysis was done using the clustering large applications k-medoids approach using clinical outcomes at 5 months. Inflammatory protein profiling was analysed from plasma at the 5-month visit. This study is registered on the ISRCTN Registry, ISRCTN10980107, and recruitment is ongoing. Findings 2320 participants discharged from hospital between March 7, 2020, and April 18, 2021, were assessed at 5 months after discharge and 807 (32·7%) participants completed both the 5-month and 1-year visits. 279 (35·6%) of these 807 patients were women and 505 (64·4%) were men, with a mean age of 58·7 (SD 12·5) years, and 224 (27·8%) had received invasive mechanical ventilation (WHO class 7–9). The proportion of patients reporting full recovery was unchanged between 5 months (501 [25·5%] of 1965) and 1 year (232 [28·9%] of 804). Factors associated with being less likely to report full recovery at 1 year were female sex (odds ratio 0·68 [95% CI 0·46–0·99]), obesity (0·50 [0·34–0·74]) and invasive mechanical ventilation (0·42 [0·23–0·76]). Cluster analysis (n=1636) corroborated the previously reported four clusters: very severe, severe, moderate with cognitive impairment, and mild, relating to the severity of physical health, mental health, and cognitive impairment at 5 months. We found increased inflammatory mediators of tissue damage and repair in both the very severe and the moderate with cognitive impairment clusters compared with the mild cluster, including IL-6 concentration, which was increased in both comparisons (n=626 participants). We found a substantial deficit in median EQ-5D-5L utility index from before COVID-19 (retrospective assessment; 0·88 [IQR 0·74–1·00]), at 5 months (0·74 [0·64–0·88]) to 1 year (0·75 [0·62–0·88]), with minimal improvements across all outcome measures at 1 year after discharge in the whole cohort and within each of the four clusters. Interpretation The sequelae of a hospital admission with COVID-19 were substantial 1 year after discharge across a range of health domains, with the minority in our cohort feeling fully recovered. Patient-perceived health-related quality of life was reduced at 1 year compared with before hospital admission. Systematic inflammation and obesity are potential treatable traits that warrant further investigation in clinical trials. Abstract: Estimating the prevalence of a disease, such as COVID-19, is necessary for evaluating and mitigating risks of its transmission. Estimates that consider how prevalence changes with time provide more information about these risks but are difficult to obtain due to the necessary survey intensity and commensurate testing costs. Motivated by a dataset on COVID-19, from the University of Notre Dame, we propose pooling and jointly testing multiple samples to reduce testing costs. A nonparametric, hierarchical Bayesian model is used to infer population prevalence from the pooled test results without needing to retest individuals from pools that test positive. This approach is shown to reduce uncertainty compared to individual testing at the same budget and to produce similar estimates compared to individual testing at a much higher budget through simulation studies and an analysis of COVID-19 infections at Notre Dame.

5. Establishing the health and wellbeing needs of mining host community in Brazil, Chile and Peru: a mixed-method approach to identify priority areas for action to help communities meet their SDG3 targets

Abstract: Background: The global mining industry is an important partner in advancing the 2030 Agenda for Sustainable Development. In 2018, Anglo American plc published their Sustainable Mining Plan, containing a goal for improving health and wellbeing aligned with the Sustainable Development Goal 3 (SDG3) targets. Having formed an independent multidisciplinary research consortium, we designed and implemented a mixed-methods approach to attain a deeper understanding of SDG3 priorities within the local context of communities hosting Anglo American mining operations located in Latin America. Methods: In 2019, within the host communities of three mining operations in Chile, three in Brazil, and one in Peru, we conducted a qualitative study which included stakeholder workshops and key informant interviews. We also quantitatively appraised existing health data.Findings emerging from the qualitative and quantitative assessments were compared to identify health and wellbeing priority areas for action relevant to each community. Results: Across the three countries, 120 people took part in workshops and 35 in interviews. In these workshops and interviews, non-communicable diseases (SDG3.4), harmful alcohol consumption (SDG3.5), and pollution, particularly air pollution (SDG3.9), were consistently identified as areas for priority action. There were similarities in the reporting of individual, interpersonal, community, societal, and structural factors underlying these priority areas across the different communities. The availability of quantitative data was generally good at the state level, becoming increasing sparse as we focused on smaller geographies. The priorities identified in the quantitative assessments generally aligned with those highlighted in the qualitative data. Conclusions: We highlight the importance of engaging with local populations to understand and address health needs. To address the priorities identified, intervention packages tailored to the specific needs of host communities, that tackle associated upstream societal level factors, are required. To facilitate this, appropriate monitoring systems and epidemiological investigations should be implemented to better understand the local context and quantify health issues. In the host communities, it is essential for the mining sector to be a key health partner in promoting integrated programmes that contribute to achieving the priority objectives and targets aligned with the SDG3 agenda.

6. Canopy and surface fuel estimations using RPAS and ground-based point clouds

Abstract: Forest management activities intended to reduce wildfire risk rely on accurate characterizations of the amount and arrangement of canopy and surface fuels. Metrics that describe these fuels are typically estimated with various systems that transform plot-level field data into metrics that can be used within fire behaviour models. Remote sensing data have long been used to estimate these metrics across large spatial scales, but more advanced, high-density point clouds have the potential to estimate these metrics with higher accuracy. This study collected LiDAR and digital aerial photogrammetric (DAP) point clouds from a remotely piloted aerial system (RPAS), as well as mobile laser scanning (MLS) point clouds from a mobile ground-based system, and compared their ability to estimate fuel metrics. This involved the extraction of predictor variables from each point cloud, of which small subsets were used to estimate various fuel metrics. These included six overstory canopy metrics (stand height, canopy cover, tree density, canopy fuel load, canopy bulk density and canopy base height), three diameter at breast height (DBH)–related metrics (stand density index, basal area and quadratic mean diameter) and three surface fuel metrics (total woody debris (TWD), coarse woody debris (CWD) and fine woody debris (FWD)). Overall, canopy metrics were estimated most accurately by the RPAS LiDAR models, although none of the point clouds were able to accurately estimate DBH-related metrics. For the other six canopy metrics, RPAS LiDAR models had an average R2 value of 0.70; DAP – 0.63 and MLS – 0.63. CWD (>7 cm) and TWD loads were estimated most accurately by the MLS models (average R2 values – 0.70), followed by the RPAS LiDAR – 0.38 and DAP – 0.13. None of these models were able to accurately estimate FWD loads (≤7 cm in diameter), with the three types of point clouds having a maximum R2 value of 0.08. Overall, this research shows the relative ability of three types of high-density point clouds to estimate metrics relevant for fire behaviour modeling.

9. The significance of love in relation to looked-after children and child sexual exploitation

Abstract: Drawing on the knowledge that looked-after children are disproportionately involved in child sexual exploitation, this critical systematic literature review considers the significance of ‘love’, using bell hooks’ ‘love ethic’. This literature review applies a systematic approach to retrieving literature and includes ten peer-reviewed journal articles containing primary research from individuals with lived experience of being a looked-after child and child sexual exploitation, or professionals within the field. Thematic analysis is used to identify the themes of risk, agency and relational practice. Together, these explore how dominant discourses of neoliberalism, patriarchy and capitalism shape modern social work, increase the risk of child sexual exploitation for looked-after children and maintain conditions needed for child sexual exploitation to function. bell hooks’ conceptualisation of the ‘love ethic’ is discussed as a possible framework in meeting the unmet emotional and economic needs of looked-after children through using ‘love as action’. ‘Love’, critical thinking and critical reflection are then recommended for social work.

Abstract: Recent evidence suggests that the gut is an additional target for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. However, whether SARS-CoV-2 spreads via gastrointestinal secretions remains unclear. To determine the prevalence of gastrointestinal SARS-CoV-2 infection in asymptomatic subjects, we analyzed gastrointestinal biopsy and liquid samples from endoscopy patients for the presence of SARS-CoV-2.We enrolled 100 endoscopic patients without known SARS-CoV-2 infection (cohort A) and 12 patients with a previous COVID-19 diagnosis (cohort B) in a cohort study performed at a regional hospital. Gastrointestinal biopsies and fluids were screened for SARS-CoV-2 by polymerase chain reaction (PCR), immunohistochemistry, and virus isolation assay, and the stability of SARS-CoV-2 in gastrointestinal liquids in vitro was analyzed.SARS-CoV-2 ribonucleic acid was detected by PCR in the colonic tissue of 1/100 patients in cohort A. In cohort B, 3 colonic liquid samples tested positive for SARS-CoV-2 by PCR and viral nucleocapsid protein was detected in the epithelium of the respective biopsy samples. However, no infectious virions were recovered from any samples. In vitro exposure of SARS-CoV-2 to colonic liquid led to a 4-log-fold reduction of infectious SARS-CoV-2 within 1 hour (Overall, the persistent detection of SARS-CoV-2 in endoscopy samples after resolution of COVID-19 points to the gut as a long-term reservoir for SARS-CoV-2. Since no infectious virions were recovered and SARS-CoV-2 was rapidly inactivated in the presence of colon liquids, it is unlikely that performing endoscopic procedures is associated with a significant infection risk due to undiagnosed asymptomatic or persistent gastrointestinal SARS-CoV-2 infections.

14. A novel post-fire method to estimate individual tree crown scorch height and volume using simple RPAS-derived data

Abstract: Background An accurate understanding of wildfire impacts is critical to the success of any post-fire management framework. Fire severity maps are typically created from satellite-derived imagery that are capable of mapping fires across large spatial extents, but cannot detect damage to individual trees. In recent years, higher resolution fire severity maps have been created from orthomosaics collected from remotely piloted aerial systems (RPAS). Digital aerial photogrammetric (DAP) point clouds can be derived from these same systems, allowing for spectral and structural features to be collected concurrently. In this note, a methodology was developed to analyze fire impacts within individual trees using these two synergistic data types. The novel methodology presented here uses RPAS-acquired orthomosaics to classify trees based on a binary presence of fire damage. Crown scorch heights and volumes are then extracted from fire-damaged trees using RPAS-acquired DAP point clouds. Such an analysis allows for crown scorch heights and volumes to be estimated across much broader spatial scales than is possible from field data. Results There was a distinct difference in the spectral values for burned and unburned trees, which allowed the developed methodology to correctly classify 92.1% of trees as either burned or unburned. Following a correct classification, the crown scorch heights of burned trees were extracted at high accuracies that when regressed against field-measured heights yielded a slope of 0.85, an R-squared value of 0.78, and an RMSE value of 2.2 m. When converted to crown volume scorched, 83.3% of the DAP-derived values were within ± 10% of field-measured values. Conclusion This research presents a novel post-fire methodology that utilizes cost-effective RPAS-acquired data to accurately characterize individual tree-level fire severity through an estimation of crown scorch heights and volumes. Though the results were favorable, improvements can be made. Specifically, through the addition of processing steps that would remove shadows and better calibrate the spectral data used in this study. Additionally, the utility of this approach would be made more apparent through a detailed cost analysis comparing these methods with more conventional field-based approaches.

Background: The scale of COVID-19 and its well documented long-term sequelae support a need to understand long-term outcomes including frailty. Methods: This prospective cohort study recruited adults who had survived hospitalisation with clinically diagnosed COVID-19 across 35 sites in the UK (PHOSP-COVID). The burden of frailty was objectively measured using Fried's Frailty Phenotype (FFP). The primary outcome was the prevalence of each FFP group—robust (no FFP criteria), pre-frail (one or two FFP criteria) and frail (three or more FFP criteria)—at 5 months and 1 year after discharge from hospital. For inclusion in the primary analysis, participants required complete outcome data for three of the five FFP criteria. Longitudinal changes across frailty domains are reported at 5 months and 1 year post-hospitalisation, along with risk factors for frailty status. Patient-perceived recovery and health-related quality of life (HRQoL) were retrospectively rated for pre-COVID-19 and prospectively rated at the 5 month and 1 year visits. This study is registered with ISRCTN, number ISRCTN10980107. Findings: Between March 5, 2020, and March 31, 2021, 2419 participants were enrolled with FFP data. Mean age was 57.9 (SD 12.6) years, 933 (38.6%) were female, and 429 (17.7%) had received invasive mechanical ventilation. 1785 had measures at both timepoints, of which 240 (13.4%), 1138 (63.8%) and 407 (22.8%) were frail, pre-frail and robust, respectively, at 5 months compared with 123 (6.9%), 1046 (58.6%) and 616 (34.5%) at 1 year. Factors associated with pre-frailty or frailty were invasive mechanical ventilation, older age, female sex, and greater social deprivation. Frail participants had a larger reduction in HRQoL compared with before their COVID-19 illness and were less likely to describe themselves as recovered. Interpretation: Physical frailty and pre-frailty are common following hospitalisation with COVID-19. Improvement in frailty was seen between 5 and 12 months although two-thirds of the population remained pre-frail or frail. This suggests comprehensive assessment and interventions targeting pre-frailty and frailty beyond the initial illness are required. Abstract: BACKGROUND: Both infection and vaccination, alone or in combination, generate antibody and T cell responses against severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). However, the maintenance of such responses-and hence protection from disease-requires careful characterization. In a large prospective study of UK healthcare workers (HCWs) (Protective Immunity from T Cells in Healthcare Workers [PITCH], within the larger SARS-CoV-2 Immunity and Reinfection Evaluation [SIREN] study), we previously observed that prior infection strongly affected subsequent cellular and humoral immunity induced after long and short dosing intervals of BNT162b2 (Pfizer/BioNTech) vaccination. METHODS: Here, we report longer follow-up of 684 HCWs in this cohort over 6-9 months following two doses of BNT162b2 or AZD1222 (Oxford/AstraZeneca) vaccination and up to 6 months following a subsequent mRNA booster vaccination. FINDINGS: We make three observations: first, the dynamics of humoral and cellular responses differ; binding and neutralizing antibodies declined, whereas T and memory B cell responses were maintained after the second vaccine dose. Second, vaccine boosting restored immunoglobulin (Ig) G levels; broadened neutralizing activity against variants of concern, including Omicron BA.1, BA.2, and BA.5; and boosted T cell responses above the 6-month level after dose 2. Third, prior infection maintained its impact driving larger and broader T cell responses compared with never-infected people, a feature maintained until 6 months after the third dose. CONCLUSIONS: Broadly cross-reactive T cell responses are well maintained over time-especially in those with combined vaccine and infection-induced immunity ("hybrid" immunity)-and may contribute to continued protection against severe disease. Abstract: This chapter grows out of an engagement between scholars and practitioners interested and invested in questions about humanitarian futures, that is, questions around the future of humanitarianism as both a normative, ethical commitment to life and a practice concerned with saving lives, relieving suffering, and upholding human dignity. These urgent questions are increasingly being considered by scholars and practitioners. While there is an ever-growing body of work concerned with the politics of humanitarianism, both as a normative ideal and as a practice of intervention, recent interventions related to decolonizing humanitarianism have been keen to stress the importance of understanding the differences between them. Recent exposes made by those who have worked in the humanitarian industry have shone a light on the racist and white supremacist practices rooted in humanitarianism’s liberal universalizing claims. Shining a light on racism and white supremacy in the humanitarian industry requires an interrogation of the human subject at the heart of humanitarianism.

Abstract: The COVID-19 pandemic and its aftermath are the most significant socio-economic crises in modern history. The pandemic’s devastating impacts have prompted urgent policy and regulatory action to reduce the risks of future spillover events and pandemics. Stronger regulatory measures for the trade of wildlife are central to discussions of a policy response. A variety of measures, including broad bans on the trade and sale of wildlife to banning specific species for human consumption are among a suite of discussed options. However, the wildlife trade is diverse, complex, and important for the livelihoods of millions of people globally. We argue that reducing the risk of future pandemics stemming from the wildlife trade must follow established principles of governance which include being equitable, responsive, robust, and effective. We demonstrate how incorporating these principles will support the development of context-specific, culturally sensitive, and inclusive responses that recognize the on-the-ground complexity of disease emergence and the social-ecological systems in which the wildlife trade occurs.

Abstract: Models of host-pathogen interactions help to explain infection dynamics in wildlife populations and to predict and mitigate the risk of zoonotic spillover. Insights from models inherently depend on the way contacts between hosts are modelled, and crucially, how transmission scales with animal density. Bats are important reservoirs of zoonotic disease and are among the most gregarious of all mammals. Their population structures can be highly heterogenous, underpinned by ecological processes across different scales, complicating assumptions regarding the nature of contacts and transmission. Although models commonly parameterise transmission using metrics of total abundance, whether this is an ecologically representative approximation of host-pathogen interactions is not routinely evaluated. We collected a 13-month dataset of tree-roosting Pteropus spp. from 2,522 spatially referenced trees across eight roosts to empirically evaluate the relationship between total roost abundance and tree-level measures of abundance and density - the scale most likely to be relevant for virus transmission. We also evaluate whether roost features at different scales (roost-level, subplot-level, tree-level) are predictive of these local density dynamics. Roost-level features were not representative of tree-level abundance (bats per tree) or tree-level density (bats per m2 or m3 ), with roost-level models explaining minimal variation in tree-level measures. Total roost abundance itself was either not a significant predictor (tree-level 3-D density) or only weakly predictive (tree-level abundance). This indicates that basic measures, such as total abundance of bats in a roost, may not provide adequate approximations for population dynamics at scales relevant for transmission, and that alternative measures are needed to compare transmission potential between roosts. From the best candidate models, the strongest predictor of local population structure was tree density within roosts, where roosts with low tree density had a higher abundance but lower density of bats (more spacing between bats) per tree. Together, these data highlight unpredictable and counterintuitive relationships between total abundance and local density. More nuanced modelling of transmission, spread and spillover from bats likely requires alternative approaches to integrating contact structure in host-pathogen models, rather than simply modifying the transmission function.

Abstract: Human-mediated changes to natural ecosystems have consequences for both ecosystem and human health. Historically, efforts to preserve or restore ‘biodiversity’ can seem to be in opposition to human interests. However, the integration of biodiversity conservation and public health has gained significant traction in recent years, and new efforts to identify solutions that benefit both environmental and human health are ongoing. At the forefront of these efforts is an attempt to clarify ways in which biodiversity conservation can help reduce the risk of zoonotic spillover of pathogens from wild animals, sparking epidemics and pandemics in humans and livestock. However, our understanding of the mechanisms by which biodiversity change influences the spillover process is incomplete, limiting the application of integrated strategies aimed at achieving positive outcomes for both conservation and disease management. Here, we review the literature, considering a broad scope of biodiversity dimensions, to identify cases where zoonotic pathogen spillover is mechanistically linked to changes in biodiversity. By reframing the discussion around biodiversity and disease using mechanistic evidence—while encompassing multiple aspects of biodiversity including functional diversity, landscape diversity, phenological diversity, and interaction diversity—we work toward general principles that can guide future research and more effectively integrate the related goals of biodiversity conservation and spillover prevention. We conclude by summarizing how these principles could be used to integrate the goal of spillover prevention into ongoing biodiversity conservation initiatives.

Abstract: SARS-CoV-2 is transmitted principally via air; contact and fomite transmission may also occur. Variants-of-concern (VOCs) are more transmissible than ancestral SARS-CoV-2. We find that early VOCs show greater aerosol and surface stability than the early WA1 strain, but Delta and Omicron do not. Stability changes do not explain increased transmissibility.

Abstract: Funder: Queensland Government Accelerate Postdoctoral Research Fellowship, Outbreaks of infectious viruses resulting from spillover events from bats have brought much attention to bat‐borne zoonoses, which has motivated increased ecological and epidemiological studies on bat populations. Field sampling methods often collect pooled samples of bat excreta from plastic sheets placed under‐roosts. However, positive bias is introduced because multiple individuals may contribute to pooled samples, making studies of viral dynamics difficult. Here, we explore the general issue of bias in spatial sample pooling using Hendra virus in Australian bats as a case study. We assessed the accuracy of different under‐roost sampling designs using generalized additive models and field data from individually captured bats and pooled urine samples. We then used theoretical simulation models of bat density and under‐roost sampling to understand the mechanistic drivers of bias. The most commonly used sampling design estimated viral prevalence 3.2 times higher than individual‐level data, with positive bias 5–7 times higher than other designs due to spatial autocorrelation among sampling sheets and clustering of bats in roosts. Simulation results indicate using a stratified random design to collect 30–40 pooled urine samples from 80 to 100 sheets, each with an area of 0.75–1 m2, and would allow estimation of true prevalence with minimum sampling bias and false negatives. These results show that widely used under‐roost sampling techniques are highly sensitive to viral presence, but lack specificity, providing limited information regarding viral dynamics. Improved estimation of true prevalence can be attained with minor changes to existing designs such as reducing sheet size, increasing sheet number, and spreading sheets out within the roost area. Our findings provide insight into how spatial sample pooling is vulnerable to bias for a wide range of systems in disease ecology, where optimal sampling design is influenced by pathogen prevalence, host population density, and patterns of aggregation.

Abstract: We have designed a new class of highly potent bivalent melanocortin receptor ligands based on the nature-derived bicyclic peptide sunflower trypsin inhibitor 1 (SFTI-1). Incorporation of melanotropin pharmacophores in each of the two turn regions of SFTI-1 resulted in substantial gains in agonist activity particularly at human melanocortin receptors 1 and 3 (hMC1R/hMC3R) compared to monovalent analogues. In in vitro binding and functional assays, the most potent molecule, compound 6, displayed low picomolar agonist activity at hMC1R (pEC50 > 10.3; EC50 < 50 pM; pKi: 10.16 ± 0.04; Ki: 69 ± 5 pM) and is at least 30-fold more selective for this receptor than for hMC3R, hMC4R, or hMC5R. The results are discussed in the context of structural homology models of hMCRs in complex with the developed bivalent ligands.

31. Learning from COVID-19: Cross-sectional e-survey of critical care nurses' satisfaction and experiences of their role in the pandemic response across the United Kingdom

Abstract: Since the start of the global COVID-19 pandemic in 2019, critical care nurses across the world have been working under extreme levels of pressure.To understand critical care nurses' experiences of and satisfaction with their role in the pandemic response across the United Kingdom (UK).A cross-sectional electronic survey of critical care nurses (n = 339) registered as members of the British Association of Critical Care Nurses. Anonymous quantitative and open-ended question data were collected in March and April 2021 during the height of the second surge of COVID-19 in the UK via an online questionnaire. Quantitative data were analysed using descriptive statistics and free text responses were collated and analysed thematically.There was a response rate of 17.5%. Critical care nurses derived great satisfaction from making a difference during this global crisis and greatly valued teamwork and support from senior nurses. However, nurses consistently expressed concern over the quality of safe patient care, which they perceived to be suboptimal due to staff shortages and a dilution of the specialist skill mix. Together with the high volume of patient deaths, critical care nurses reported that these stressors influenced their personalwell-being.This study provides insights into the key lessons health care leaders must consider when managing the response to the demands and challenges of the ongoing COVID-19 pandemic. COVID-19 is unpredictable in its course, and what future variants might mean in terms of transmissibility, severity and resultant pressures to critical care remains unknown.Future responses to the challenges that critical care faces must consider nurses' experiences and create an environment that engenders supportive teamwork, facilitates excellent nursing practice and effective safe patient care where critical care nursing may thrive.

Abstract: Background Microvascular abnormalities and impaired gas transfer have been observed in patients with COVID-19. The progression of pulmonary changes in these patients remains unclear. Research Question Do patients hospitalized with COVID-19 without evidence of architectural distortion on structural imaging exhibit longitudinal improvements in lung function measured by using 1H and 129Xe MRI between 6 and 52 weeks following hospitalization? Study Design and Methods Patients who were hospitalized with COVID-19 pneumonia underwent a pulmonary 1H and 129Xe MRI protocol at 6, 12, 25, and 51 weeks following hospital admission in a prospective cohort study between November 2020 and February 2022. The imaging protocol was as follows: 1H ultra-short echo time, contrast-enhanced lung perfusion, 129Xe ventilation, 129Xe diffusion-weighted, and 129Xe spectroscopic imaging of gas exchange. Results Nine patients were recruited (age 57 ± 14 [median ± interquartile range] years; six of nine patients were male). Patients underwent MRI at 6 (n = 9), 12 (n = 9), 25 (n = 6), and 51 (n = 8) weeks following hospital admission. Patients with signs of interstitial lung damage were excluded. At 6 weeks, patients exhibited impaired 129Xe gas transfer (RBC to membrane fraction), but lung microstructure was not increased (apparent diffusion coefficient and mean acinar airway dimensions). Minor ventilation abnormalities present in four patients were largely resolved in the 6- to 25-week period. At 12 weeks, all patients with lung perfusion data (n = 6) showed an increase in both pulmonary blood volume and flow compared with 6 weeks, although this was not statistically significant. At 12 weeks, significant improvements in 129Xe gas transfer were observed compared with 6-week examinations; however, 129Xe gas transfer remained abnormally low at weeks 12, 25, and 51. Interpretation 129Xe gas transfer was impaired up to 1 year following hospitalization in patients who were hospitalized with COVID-19 pneumonia, without evidence of architectural distortion on structural imaging, whereas lung ventilation was normal at 52 weeks.
