Objective: To determine the incidence and clinical effects of antibodies that develop during treatment with natalizumab. Methods: In two randomized, double-blind, placebo-controlled studies (natalizumab safety and efficacy in relapsing remitting multiple sclerosis [MS, AFFIRM] and safety and efficacy of natalizumab in combination with interferon β-1a [INFβ1a] in patients with relapsing remitting MS [SENTINEL]) of patients with relapsing multiple sclerosis, blood samples were obtained at baseline and every 12 weeks to determine the presence of antibodies against natalizumab. Antibodies to natalizumab were measured using an ELISA. Patients were categorized as “transiently positive” if they had detectable antibodies (≥0.5 μg/mL) at a single time point or “persistently positive” if they had antibodies at two or more time points ≥6 weeks apart. Results: In the AFFIRM study, antibodies were detected in 57 of 625 (9%) of natalizumab-treated patients: Twenty (3%) were transiently positive and 37 (6%) were persistently positive. Persistently positive patients showed a loss of clinical efficacy as measured by disability progression ( p ≤ 0.05), relapse rate ( p = 0.009), and MRI ( p ≤ 0.05) compared with antibody-negative patients. In transiently positive patients, full efficacy was achieved after approximately 6 months of treatment, the time when patients were becoming antibody negative. The incidence of infusion-related adverse events was significantly higher in persistently positive patients. Results of SENTINEL were similar to AFFIRM, except with regard to sustained disability progression; differences between persistently positive and antibody-negative patients were not statistically significant. Conclusions: The incidence of persistent antibody positivity associated with natalizumab is 6%. Reduced clinical efficacy is apparent in persistently positive patients. Patients with a suboptimal clinical response or persistent infusion-related adverse events should be considered for antibody testing. The application of Internet of Things (IoT) within industrial environments is fostering the adoption of the digital twin (DT) approach, applied at the edge of the network to handle heterogeneity stemming from siloed application management solutions and from protocols originated by different manufacturing tools and enterprise services. In this challenging context, network heterogeneity also represents a critical element that can significantly limit the design and deployment of DT-oriented applications. This article proposes the Application-driven digital twin networking middleware with the twofold objective of: 1) Simplifying the interaction among heterogeneous devices by allowing DTs to exploit IP-based protocols instead of specialized industrial ones and to enhance packet content expressiveness, by enriching data via well-defined standards. 2) Dynamically managing network resources in edge industrial environments, applying software defined networking to exploit the communication mechanisms most suitable to application requirements, ranging from native IP to more articulated based on packet content.

Ethnic residential segregation in Italy is emerging as a key question, which will be crucial in the definition and implementation of both urban and social policies. This chapter focuses on this phenomenon in the Italian Metropolitan Area of Palermo. We use individual data for all the population residing in the city on 31 December 2011, organised by ethnicity and neighbourhood to describe the spatial distribution and the residential segregation of foreigners in the city, applying many different segregation measures. Results show that the top ten nationalities in Palermo are not uniformly spread across the different parts of the city but tend to cluster in specific areas. The most segregated nationalities are people from China, Cote D’Ivoire, and Bangladesh, while Romanians present the lowest level of segregation.

This article explores the dichotomy between interests and values in the field of the right to housing for homeless people, focusing on the informal practices of reappropriation of spaces (properties owned by the Municipality of Palermo) in the case study of Palermo. The experience we are dealing with here must be addressed in a wider context of structural and latent conflicts concerning the city of Palermo, which are strictly related to the emergency housing issue. Indeed, Palermo’s housing issue is a quite complex and articulated theme, where the ineffectiveness of public policies and the increasing poverty conditions, aggravated by the economic crisis, seem to find in the various forms of the illegal occupation, as for abandoned or unused buildings, an extreme mode to democratically get a denied right.

The process of urbanisation is considered one of the most significant anthropic alterations of the environmental framework, and the present study attempts to understand the spatial characteristics of urban growth and their impacts on envi- ronmental components in two metropolitan areas of Sicily. Such rapid and unplanned urbanisation in Sicily has increased during the last three decades causing severe pres- sure on various natural resources. In this situation, the level of fragmentation and isolation of natural areas risks becoming worse. The question assumes a particular relevance in those local territorial portions, such as some urban and periurban areas, presenting relevant conditions of naturality close to highly built environments. Such landscapes are usually affected by forms of human pressure that have determined intense deterioration processes, that urban and regional planning tools were not able to control and reduce. Empirical evidence of this study suggests that alternative urban patterns generate differential effects on the environmental system. Starting from this evidence, in this article we explore the reciprocity relationship between built envi- ronment and open territory, with the aim of identifying possible strategies to manage the phenomena of urban sprawl in the metropolitan context.

A spatial analysis of housing and land use complements the socio-economic outlook. Each section describes the most consolidated trends in these areas, but also discusses the most innovative changes and the challenges Sicily has been facing over the years.

In spite of the multifaceted criticalities that characterise the Sicilian territory, South-Eastern Sicily appears to be of particular interest due to the active role that distinguishes it from the rest of the Island. The transformation processes taking place in South-Eastern Sicily, in fact, seem to be guided by new perspectives of sustainability, local development and good governance. The mix obtained from the extraordinary amount of tangible and intangible resources, from some favourable socio-cultural circumstances and from the creation of new transportation infrastructures, has contributed to promoting new forms of reuse of the historical building heritage by Italian and overseas investors who have purchased prestigious properties in the historic centres and in the agricultural areas of the Hyblaean hinterland, triggering unexpected processes of economic revitalisation.

To evaluate the hazard assessment of dredging and disposal of sediments, an in-depth chemical and toxicological characterization of sediments was carried out in Venice Lagoon. The bioaccessible and mobile concentration of trace elements in superficial and deep sediments (in the Holocene-Pleistocene limit) can deeply affect the quality of the aquatic ecosystem. Geospeciation and total concentration of trace elements in sediment cores were assessed and several toxicological tests were carried out. The indices of contamination (e.g., geochemical accumulation index, Igeo, pollution load index, PLI) and of speciation (individual contamination factor, ICF and global contamination factor, GCF) were evaluated. Factor Analysis (FA) was performed to explore possible significant correlations between toxicity data and sediment chemistry. Results underlined that the total concentration of trace elements cannot adequately assess the hazard, while bioavailability, mobility, and toxicity of trace elements allow a thorough evaluation of the environmental threats. The integration of results from chemical characterization ex ante and toxicity assays will provide for a better sustainable handling and management of sediments.

A reduction in mean LDL cholesterol to 1.4-1.8 mmol/L with ezetimibe or statins reduces cardiovascular events in patients with an acute coronary syndrome and diabetes. However, the efficacy and safety of further reduction in LDL cholesterol with an inhibitor of proprotein convertase subtilisin/kexin type 9 (PCSK9) after acute coronary syndrome is unknown. We aimed to explore this issue in a prespecified analysis of the ODYSSEY OUTCOMES trial of the PCSK9 inhibitor alirocumab, assessing its effects on cardiovascular outcomes by baseline glycaemic status, while also assessing its effects on glycaemic measures including risk of new-onset diabetes. Methods ODYSSEY OUTCOMES was a randomised, double-blind, placebo-controlled trial, done at 1315 sites in 57 countries, that compared alirocumab with placebo in patients who had been admitted to hospital with an acute coronary syndrome (myocardial infarction or unstable angina) 1-12 months before randomisation and who had raised concentrations of atherogenic lipoproteins despite use of high-intensity statins. Patients were randomly assigned (1: 1) to receive alirocumab or placebo every 2 weeks; randomisation was stratified by country and was done centrally with an interactive voice-response or web-response system. Alirocumab was titrated to target LDL cholesterol concentrations of 0.65-1.30 mmol/L. in this prespecified analysis, we investigated the effect of alirocumab on cardiovascular events by glycaemic status at baseline (diabetes, prediabetes, or normoglycaemia)-defined on the basis of patient history, review of medical records, or baseline HbA(1c) or fasting serum glucose-and risk of new-onset diabetes among those without diabetes at baseline. the primary endpoint was a composite of death from coronary heart disease, non-fatal myocardial infarction, fatal or non-fatal ischaemic stroke, or unstable angina requiring hospital admission. ODYSSEY OUTCOMES is registered with ClinicalTrials. gov, number NCT01663402. Findings At study baseline, 5444 patients (28.8%) had diabetes, 8246 (43.6%) had prediabetes, and 5234 (27.7%) had normoglycaemia. There were no significant differences across glycaemic categories in median LDL cholesterol at baseline (2.20-2.28 mmol/L), after 4 months' treatment with alirocumab (0.80 mmol/L), or after 4 months' treatment with placebo (2.25-2.28 mmol/L). in the placebo group, the incidence of the primary endpoint over a median of 2.8 years was greater in patients with diabetes (16.4%) than in those with prediabetes (9.2%) or normoglycaemia (8.5%); hazard ratio (HR) for diabetes versus normoglycaemia 2.09 (95% CI 1.78-2.46, p, Sanofi; Regeneron Pharmaceuticals, Sanofi and Regeneron Pharmaceuticals.

L’articolo affronta la questione della segregazione nel quartiere ZEN di Palermo (Sicilia). L’impianto teorico dialoga con la letteratura anglosassone, ma ne prende le distanze nel tentativo di individuare alcune specificità locali del caso. L’obiettivo è duplice: intanto indentificare alcuni “movimenti geostorici” che hanno caratterizzato il quartiere, collegando a ciascuno di questi una fase del processo segregativo; inoltre, rappresentare la segregazione del quartiere in forma cartografica, mediante l’uso di alcuni ideogrammi e mappe mentali. This article deals with the issue of segregation in the ZEN district of Palermo, Sicily. The theoretical framework is built around Anglo-Saxon references on segregation, but takes its distance in an attempt to identify some local peculiarities of the case study. The article has two main goals: the first is to describe a few ‘geohistorical movements’ that characterize the district, each connected to a different stage of the segregation processes; the second is to chart these processes through ideograms and mental maps.

Il presente paper intende soffermarsi sul tema delle fortificazioni costiere in Sicilia e in particolare su quelle ancora esistenti nella costa di Palermo. Realizzate nella seconda metà del XVI secolo per ordine del viceré Giovanni de Vega lungo tutta la costa dell’isola, furono utilizzate come sistema di postazioni di avvistamento di incursioni nemiche e per poter avvisare la popolazione. In uso sino alla prima metà del XIX secolo, quelle oggi ancora erette versano in stato di quasi totale abbandono. Diverse politiche sono state avviate negli anni al fine di recuperare questa importante testimonianza della storia dell’isola, anche se la loro valorizzazione come elemento del paesaggio culturale non sembra avere preso forza. A partire dal caso di Palermo, e in particolare con le due torri ancora esistenti nella frazione di Mondello, si propongono alcune riflessioni su nuovi sistemi di valorizzazione che partano da sistemi di gestione partecipata. Nel caso studio proposto, infatti, si sta inaugurando un interessante processo partecipativo in cui il ruolo del pubblico e del privato si mescolano, con il duplice obiettivo di incrementare l’afflusso turistico e balneare da un lato e di riqualificare il nucleo della borgata storica, sorto intorno alla torre normanna, dall’altro. The paper aims at enquiring the topic of coastal fortifications in Sicily, and particularly the ones that still exist along the coast of Palermo. Built in the second half of the 16th century by order of the viceroy Giovanni de Vega on the whole coast of Sicily, these fortifications were used as watchtowers against enemy invasions and as sighting posts for ensuring the safety of the population. Most were used up to the first half of the 19th century but now lie in a completely neglected state. Several regeneration policies have tried to restore the traditional role of the watchtowers, but none has succeeded so far. Starting from the case of Palermo and the two still existing watchtowers in the seaside borough of Mondello, the paper reflects on the implementation of new valuation systems that stem from participatory management processes. An intriguing participatory process in actually taking place in Mondello right now, involving both public and private actors, with the goals of both increasing tourist flows and regenerating the core historic area that grew around the Norman watchtowers. The process is still in progress, but thoughtprovoking outcomes can already be expected.

The most recent forms of e-participation seem to provide new issues that need to be discussed. One such discussion involves the role of GIS. Can an ethically aware GIS be conceived? What does it mean for a GIS to be ethical at all? Throughout this paper, first the authors create a theoretical framework to encompass four key elements that lead us to the definition of ethical digital mapping: GIS ethics, social justice, power, and participation. Then they introduce the concept of PPGIS (Public Participation GIS), and argue that only a ‘qualitative turn' can enhance their importance in decision-making processes. Finally, the authors discuss an experiment that is currently taking place in Palermo, Italy, to debate the opportunities qualitative PPGIS may grant.

The analysis of bivariate space–time series with linear and circular components is complicated by (1) multiple correlations, across time, space and between variables, (2) different supports on which the variables are observed, the real line and the circle, and (3) the periodic nature of circular data. We describe a multivariate hidden Markov model that includes these features of the data within a single framework. The model integrates a circular von Mises Markov field and a Gaussian Markov field, with parameters that evolve in time according to a latent (hidden) Markov chain. It allows to describe the data by means of a finite number of time-varying latent regimes, associated with easily interpretable components of large-scale and small-scale spatial variation. It can be estimated by a computationally feasible expectation–maximization algorithm. In a case study of sea currents in the Northern Adriatic Sea, it provides a parsimonious representation of the sea surface in terms of alternating environmental states.

The identification of sea regimes from environmental multivariate times series is complicated by the mixed linear-circular support of the data, by the occurrence of missing values, by the skewness of some variables, and by the temporal autocorrelation of the measurements. We address these issues simultaneously by a hidden Markov approach, and segment the data into pairs of toroidal and skew elliptical clusters by means of the inferred sequence of latent states. Toroidal clusters are defined by a class of bivariate von Mises densities, while skew elliptical clusters are defined by mixed linear models with positive random effects. The core of the classification procedure is an EM algorithm accounting for missing measurements, unknown cluster membership, and random effects as different sources of incomplete information. Moreover, standard simulation routines allow for the efficient computation of bootstrap standard errors. The proposed procedure is illustrated for a multivariate marine time series, and identifies a number of wintertime regimes in the Adriatic Sea.

Motivated by classification issues that arise in marine studies, we propose a latent-class mixture model for the unsupervised classification of incomplete quadrivariate data with two linear and two circular components. The model integrates bivariate circular densities and bivariate skew normal densities to capture the association between toroidal clusters of bivariate circular observations and planar clusters of bivariate linear observations. Maximum-likelihood estimation of the model is facilitated by an expectation maximization (EM) algorithm that treats unknown class membership and missing values as different sources of incomplete information. The model is exploited on hourly observations of wind speed and direction and wave height and direction to identify a number of sea regimes, which represent specific distributional shapes that the data take under environmental latent conditions.

Life expectancy of Italian people is among the highest in the world, at 80 years for males and 85 years for females. Estimates from the National Institute of Statistics show that people aged 65 years and older represent more than 21% of the Italian population. Among them, centenarians represent the age group with the highest growing rate. Hip fracutre incidence increases with advancin age, and currentlu represents one of the major causes of hospitalization in the eldely. Many studies regarding hip fracture in the elderly have been published, but research focusing on centenarians is still very limited despite their growing trend. This manuscript reviews the activity of the Orthogeriatric Unit at San Gerardo University Hospital (Northern Italy) from its establishment in 2007 until the present, and enlightens the limitations of the research in this area reviewing the literature currently available. Over a 7-year period, we admitted a small population of 10 centenarian female patients, all of underwent surgery. Their in-hospital (1 out of 10) and 1-year mortality rates (8 out of 10) are consisten with the existing literature.Centenariasn pose an increasing challenge to physicians and surgeons. Rates of recovery of the pre-fracture condition are undoubtedly lower than those in younger geriatric patients, but surgery and early rehabilitation should not be denied solely due to the extreme age of these subjects. However, the research in this area presents several limitation up to now. The available studies are sparse, describe small populations, and are generally a single-center retrospective review of the local hospital admissions registry. Moreover, the objectives and outcome measurements vary considerably between studies, making a statistical comparison unfeasible. In this scenario, the implementation and analysis of large-scale databases would allow appropriate research among centenarians, inluding cost-effectiveness of specific surgical treatment.
