In the human genome, BRCA1 and BRCA2 (for BReast CAncer 1 and 2) genes encode for proteins involved in several functions that are crucial for the maintenance of genome stability and integrity. They participate in DNA damage response and repair pathways and, therefore, act as tumor suppressor genes. Mutations in these genes, which are located in chromosomes 17q21 and 13q13 respectively, are responsible for a great fraction of inherited breast and ovarian cancers, as well as other pathologies, such as Fanconi Anemia. Approximately 30 years ago, a report from a group of the School of Public Health at the University of California about a hypothetical gene that led to predisposition to early-onset breast cancer in certain families changed the history of breast cancer research, diagnosis, and prevention. Nowadays, the accessibility of genetic testing and the availability of different approaches as wide coverage screenings, prophylactic mastectomies, and risk-lowering drugs benefits BRCA1 and BRCA2 mutation carriers enormously. This chapter summarizes the unique trajectory of BRCA research and its scientific and social implications.

Larger thalamic volume has been found in children with obsessive-compulsive disorder (OCD) and children with clinical-level symptoms within the general population. Particular thalamic subregions may drive these differences. The ENIGMA-OCD working group conducted mega- and meta-analyses to study thalamic subregional volume in OCD across the lifespan. Structural T1-weighted brain magnetic resonance imaging (MRI) scans from 2649 OCD patients and 2774 healthy controls across 29 sites (50 datasets) were processed using the FreeSurfer built-in ThalamicNuclei pipeline to extract five thalamic subregions. Volume measures were harmonized for site effects using ComBat before running separate multiple linear regression models for children, adolescents, and adults to estimate volumetric group differences. Antecedentes: Los ahogamientos son un importante problema de salud pública, con más de 320.000 muertes al año en todo el mundo. Años de investigación y concienciación sobre la necesidad de un cambio en los planteamientos de enseñanza acuática, han destacado en la actualidad, la necesidad de que ésta sea dirigida hacia la adquisición de la competencia que conlleve un real dominio del medio acuático, así como un descenso en las lesiones y ahogamientos en el mismo. Objetivo: El objetivo de este documento es mostrar un posicionamiento sobre la educación acuática con una orientación claramente preventiva. Método: Se revisaron las principales aportaciones bibliográficas a nivel internacional sobre la prevención de ahogamientos así como sobre los temas relativos a la educación acuática preventiva. Resultados: Una vez identificadas las subcompetencias que tendrán que ser abordadas, se detallan, así como las recomendaciones pedagógicas para cada una de ellas. Basado en ello, emergen recomendaciones generales para llevar a cabo todo el proceso educativo, destacando la necesidad de concienciación de toda la población para lograr una prevención de ahogamiento integral de forma universal. Conclusiones: Se podría destacar que, de forma general, la educación acuática debe dar un salto cualitativo, sobrepasando la idea de “saber nadar” como único recurso para no ahogarse. Dando paso a nuevas concepciones y metodologías que sean significativas, y sobre todo que tengan transferencia a entornos variables y en circunstancias de riesgo, no olvidando la situaciones individuales y colectivas en el proceso con una perspectiva cultural adecuada.

This paper presents a study of online communication in one of the most widely used microblogging sites of the 21st century: Twitter. The purpose of this study is to analyse how the corporative account of the MTV UK tries to create a bond with its intended consumer audience through the combination of language and multimodal devices so as to achieve its promotional goals. To do so, all the tweets posted by the account between the 3rd and the 5th of February 2021 were collected and carefully analysed through manual reading from a pragmatic perspective partly rooted upon the theories developed in Computer-mediated Discourse Analysis (CMDA). The results provided evidence that interaction is a crucial element to carry out a successful promotional campaign at an online platform such as Twitter as it is central in the creation of a sense of affiliation between corporative accounts and their intended audience. The analysis showed that the issue of affiliation is intimately related to building an online identity characterized by the blending of the corporative self of the MTV UK as a company and a personal self crafted to achieve interaction. Moreover, the results demonstrated how multimodality and hyperlinks constitute an essential part of online communication because they allow for a broader conveyance of meaning and a greater connectivity to be achieved, and thus should be highly considered for any promotional matter. The findings of the study revealed that online promotion is unavoidably connected to online-self performance and the use Twitter users make of verbal and non-verbal data (e.g., the combination of semiotic modes), therefore making of Twitter an ideal platform for corporative marketing plans. RESUMENEste trabajo presenta un estudio sobre la comunicación en línea en Twitter, una de las plataformas de microblogging más empleadas en el siglo XXI. El objetivo de este trabajo consiste en analizar cómo la cuenta corporativa de la MTV UK pretende establecer un vínculo con el público al que se dirige mediante la combinación del lenguaje y elementos multimodales con el fin de alcanzar unos objetivos promocionales. Para ello, se han recogido todos los tweets publicados por dicha cuenta entre el 3 y el 5 de febrero de 2021, para después analizarlos minuciosamente a través de la lectura manual y desde una perspectiva pragmática basada en parte en las teorías propuestas por el CMDA, marco de análisis del discurso online. Los resultados demostraron que la interacción es un elemento primordial a la hora de realizar una campaña publicitaria exitosa en Twitter debido a su importancia para promover un sentimiento de afiliación entre las cuentas corporativas y su público. El análisis mostró que la afiliación está íntimamente ligada a la creación de una identidad en línea caracterizada por la mezcla de la identidad corporativa de la MTV UK como empresa y una identidad personal creada para fomentar la interacción. Asimismo, los resultados revelaron que la multimodalidad y los hipervínculos constituyen un pilar fundamental de la comunicación en línea ya que permiten que se transmita una mayor cantidad de significado y que se disponga de una mayor conectividad; y por ello se deberían tener en cuenta para realizar cualquier campaña promocional en esta plataforma. Este trabajo reveló que la promoción en línea depende del comportamiento personal en Twitter así como del uso de recursos verbales y no verbales como la multimodalidad, gracias a lo cual Twitter constituye una plataforma perfecta para fines corporativos relativos al marketing. Hintergrund: Die wenigen verfügbaren Daten über die Epidemiologie und Ätiologie von non-CF Bronchiektasen in Deutschland haben bisher gezeigt, dass > 90 % der Patienten überwiegend ambulant behandelt werden (2). Die meisten bisher verfügbaren Daten stammen von stationär oder in spezialisierten Ambulanzen behandelten Patienten. Dabei sind die gewonnen Erkenntnisse nicht immer auf die Besonderheiten der kassenärztlichen Versorgung übertragbar. Die Patientenpopulation dieser Arbeit setzt sich aus ambulanten Patienten mit einer Bronchiektasen-Diagnose zusammen, die sich in lungenfachärztlicher Behandlung an der “Pneumologischen Praxis am Schloss Charlottenburg“ (Dr. Andres de Roux) in Berlin befinden. Grundlage des Variablensatzes ist der Datenerhebungsbogen des PROGNOSIS (Prospective German Non-CF Bronchiectasis Patient Registry) Registers. Methoden: 50 konsekutive Patienten mit der Diagnose einer Non-CF-Bronchiektase wurden in die Kohorte eingeschlossen. Die Datenerhebung erfolgte in SPSS, Version 22 (IBM SPSS Statistics) Statistik Software. Erhoben wurden retrospektive demografische, klinische, epidemiologische und mikrobiologische Daten. Quelle der angegebenen Daten waren die Patientenakten und Anamnesebogen. Ergebnisse: Das mittlere Alter beträgt 66 Jahre (Range 25–96 Jahre), das häufigste Geburtsland war Deutschland (86 %) gefolgt von der Türkei (8 %). Hinsichtlich der Lungenfunktionsparameter betrug der mittlere FEV1 81 % vom Soll und der Median 83 % (Range 23–126 %). Bei Patienten mit Pseudomonasnachweis in der Erkrankungsbiografie war der Median des FEV1 68 % (Range 38–114 %) und ohne Pseudomonasnachweis 87 % (Range 23–126 %). Die häufigste Ätiologie war idiopathisch (44 %) gefolgt von postinfektiös (42 %), COPD (10 %), Primärer Immundefekt (2 %), sonstige (2 %). Pulmonale Exazerbationen innerhalb der letzten zwölf Monaten wiesen 32 % auf, davon hatten 26 % über zwei (6 % zwei, 12 % drei, 8 % vier). Eine mikrobiologische Diagnostik des Sputums in den letzten zwölf Monaten vor Datenerhebung erfolgte bei 36 % der Patienten. Von den 18 Patienten konnte bei 13 ein Keim nachgewiesen werden. Die Keimnachweise waren wie folgt: P. aeruginosa. 33 %, oropharyngeale Flora 17 %, S. aureus 11 %, H. influenzae 6 %, Moraxella 6 %, Klebsiella 6 %, Candida albicans 6 %. Bezüglich der Lokalisation der Bronchiektasen waren der Unterlappen und der Mittellappen/Lingula gleich häufig betroffen (38 %) gefolgt vom Oberlappen (22 %). Am allerhäufigsten waren zwei Lappen betroffen (42 %) gefolgt von einem Lappen (36 %). Das am häufigsten angesetzte Medikament zur Dauertherapie war ein LABA (54 %) gefolgt von einem SABA (52 %). 46 % der Patienten benutzen dauerhaft ein inhalatives Steroid. 10 % der Patienten erhielten eine inhalative Antibiotikatherapie und 46 % eine orale Antibiotikatherapie in den letzten zwölf Monaten. Am allerhäufigsten wurde Levofloxacin verordnet. Schlussfolgerung: Es konnte gezeigt werden, dass die in größeren Kohorten gewonnene Erkenntnisse sich gut auf ein ambulanten, in der vertragsärztliches nicht universitäres Umfeld übertragen lassen. Da die überwiegende Zahl von Bronchiektasenpatienten in Deutschland ausschließlich ambulant behandelt werden ist dies von besonderem Wert. Die aktuellen Daten zeigen, dass in dem Bemühen eine Ätiologie der Erkrankung zu finden im ambulanten Bereich Verbesserungsbedarf besteht. Zudem sollte eine regelmäßige und konsequente Diagnostik des Sputums zum Nachweis von P. aeruginosa erfolgen, da sich auch in kleinen Kollektiven die prognostische Relevanz des Erregers nachweisen lässt., Background: The limited available data on the epidemiology of bronchiectasis in Germany have shown that over 90 % of patients are being treated out of hospital (2). Despite this fact, most of the available epidemiological data have been derived from hospitalization, billing of health services, and insurance data from hospitalized patients. The conclusions provided by this data are not necessarily the same for patients being treated in an outpatient setting. Our study center is a community based doctors’ office with a focus on respiratory and infectious diseases and part of the German bronchiectasis network (PROGNOSIS). For this study an outpatient cohort with the diagnosis of Non-CF bronchiectasis was identified. The set of variables are in accordance with the PROGNOSIS Database. Methods: 50 of these patients were included in this database and SPSS, Version 22 (IBM SPSS Statistics) Statistic Software was used for data analysis. Demographic, clinical, epidemiological and microbiological data were recorded. Results: The average age was 66 years (range 25–96 years), the most common place of birth was Germany (86 %) followed by Turkey (8 %). Regarding the lung function tests data, the average FEV1 was 81 % , the median 83 % (23–126 %). Patients who had Pseudomonas aeruginosa detected in their medical history had a median FEV1 of 68 % (38–14 %), patients without Pseudomonas detection 87 % (23–126 %). The most common etiologies were the following: 44 % idiopathic, 42 % post infectious, 10 % COPD, 2 % immune defect, and 2 % others. 32 % of the patients had a pulmonary exacerbation within the last 12 months, 26 % of these had two or more (6 % two, 12 % three, 8 % four). Sputum microbiology was assessed in 36 % of the patients within the last 12 months, of which 72 % had a positive result. The most frequently isolated pathogen was Pseudomonas (33 %), followed by oropharyngeal flora (17 %), S. aureus (11 %), H. influenzae (6 %), Moraxella (6 %), Klebsiella (6 %), Candida albicans (6 %). Regarding the distribution of the bronchiectasis, the lower and middle lobe were both affected in 38 % of the patients, followed by the upper lobe (22 %). Most commonly affected were two lobes (42 %), followed by one lobe (36 %). Long-acting-beta-2-agonists (LABA) were the most often used longterm- therapy treatment (54 %), followed by short-acting-beta-2-agonists (SABA) (52 %). 72 % of patients received airway clearance therapy. 10 % received inhaled antibiotics and 46 % received oral antibiotics within the last 12 months. The most commonly prescribed antibiotic was Levofloxacin. Conclusion: Our results show several similarities between the main findings of studies with bigger populations in different settings and a small outpatient center. Our data also show the need for improving the efforts to determine the underlining etiology of this disease. Furthermore, it is crucial to have a regular sputum microbiology diagnostic in order to identify and treat Pseudomonas aeruginosa on time, as we were able to prove that this pathogen also has a prognostic relevance in a small subset of patients.

La idea de la enseñanza dialógica inclusiva se explica y operacionaliza en un currículo dialógico inclusivo centrado en las disposiciones de alfabetización cultural. En este estudio, que forma parte de un proyecto europeo de varios países, ocho docentes de secundaria españoles y portugueses y sus estudiantes participaron en ocho sesiones que implementan planes de lecciones dialógicas. The present study investigates the acquisition of positional verbs on bilingual children of Zapotec and Spanish in the indigenous community of Hidalgo Yalalag in Oaxaca, Mexico. The main research question is how bilingual children of Zapotec-Spanish acquire positional verbs in a language shift and language displacement context. Ninety-four young scholars between the ages of five to ten years old took part in this study. They participated in three tasks, a vocabulary task, an acceptability task, and a production task. A sociolinguistic questionnaire was also employed. The vocabulary task goal was to recruit and identify participants for the study. The acceptability judgment task measured the comprehension and interpretation of basic locative sentences, whereas the production task evaluated the production of positional verbs in basic locative sentences. The sociolinguistic questionnaire researched families self-identification, language used mainly at home and neighborhood. Findings show that bilingual children’s performance did not differ across and between ages in the acceptability judgment task and the productions task, differences were perceived when data from the experiments were confronted to data gathered through the sociolinguistic questionnaire, that is bilinguals who belong to Zapotec families where Zapotec is the predominant language at home behave better contrasted to does who do not. The findings of this study revealed that families play an important role in the transmission of the language, and the promotion of the language at home stimulates the acquisition of positional verbs. This study also shows that age is not a feasible variable to evaluate the language acquisition path of positional verbs in contexts of language shift and language displacement. In such scenarios, it is essential to consider the sociolinguistic context that surrounds the language.

In order to identify new risk loci shared between these two immune-mediated disorders, we performed a cross-disease meta-analysis including GWAS data from 5,734 SSc patients, 4,588 CD patients and 14,568 controls of European origin. We identified 4 new loci shared between SSc and CD, IL12RB2, IRF1/SLC22A5, STAT3 and an intergenic locus at 6p21.31. Pleiotropic variants within these loci showed opposite allelic effects in the two analysed diseases and all of them showed a significant effect on gene expression. In addition, an enrichment in the IL-12 family and type I interferon signaling pathways was observed among the set of SSc-CD common genetic risk loci. In conclusion, through the first cross-disease meta-analysis of SSc and CD, we identified genetic variants with pleiotropic effects on two clinically distinct immune-mediated disorders. RESUMEN: Con el nombre de “Camino Olvidado” se conoce actualmente a la variante del Camino de Santiago que atraviesa la montaña Palentina. Comienza en Bilbao y termina en Villafranca del Bierzo, atravesando las provincias de Vizcaya, Burgos, Cantabria, Palencia y León. El objetivo de este estudio es destacar el potencial de esta ruta para su inclusión en la promoción del Camino de Santiago. El “Camino Olvidado” puede aportar más mejoras en la economía y el turismo de estas zonas si los peregrinos se alojan en sus albergues, y utilizan las infraestructuras. Para ello he elaborado un plan de dinamización con el fin de crear interés sobre esta sección del Camino. Además, presentaré los resultados obtenidos de la realización de encuestas a peregrinos que estaban haciendo la ruta y de entrevistas a responsables de ayuntamientos y alojamientos. ABSTRACT: The name "Forgotten Way" is how the variant of the Way of Saint James which crosses the Palentina mountain is currently known as. It begins in Bilbao and ends in Villafranca del Bierzo, crossing the provinces of Vizcaya, Burgos, Cantabria, Palencia and León. The aim of this study is to understand the potential route for its inclusion within the promotion of the Way of Saint James. The "Forgotten Way" can bring further improvements to the economy and tourism of these areas if the pilgrims stay in their hostels and make use of the infrastructure. In order to do this, I have created a tourism product dynamization plan to attract interest in this section of the Way. Furthermore, I am presenting results from a survey carried out among pilgrims who were travelling along this route, as well as data obtained from interviews with council officials and hostel owners and managers. El presente Trabajo Fin de Máster pretende ampliar los conocimientos actuales sobre la gestión de temperatura y humos en un salón de actos. Se ha escogido como entorno de estudio en particular el salón de actos de la propia Escuela Técnica Superior de Ingenieros Industriales, perteneciente a la Universidad Politécnica de Madrid y sita en la calle José Gutiérrez Abascal de dicha ciudad. Se realiza un modelado 3D del salón de actos usando el software Revit, conocido instrumento por ingenieros y arquitectos que materializa la metodología BIM. A partir de la importación en esta herramienta de un plano en planta del auditorio se pueden extruir los elementos que conforman la sala. Además de definir las geometrías de los diferentes componentes, se introducen los materiales y sus propiedades, lo que nos permitirá analizar su comportamiento en un entorno virtual La estructura creada se importa a Pyrosim para proceder al estudio del incendio. Este software de simulación computacional de dinámica de fluidos, utiliza el modelo llamado FDS (Fire Dynamics Simulator), que se ajusta al estudio del fuego. Este modelo está desarrollado por el NIST (National Institute of Standards and Technology) y es adecuado para incendios en recintos cerrados. El cálculo computacional nos permite proponer varios escenarios de incendio. Se justifica el diseño de todos ellos, así como los parámetros a introducir en cada caso. Y en uno de los casos, se comparan los resultados obtenidos con los datos teóricos que las normativas existentes, tanto europeas como estadounidenses, proporcionan, lo que permite ajustar el modelo. La tasa de liberación de calor y las características de la malla a emplear serán de crucial importancia para el resultado. El análisis de los resultados del movimiento de los humos en cada caso, la visibilidad, la fracción de CO, la evolución de la temperatura y del nivel de la capa de humos, nos permite estimar las consecuencias sin tener que realizar costosos experimentos para cada caso, que en un proyecto real implicarían elevado coste y tiempo. Destacamos en este punto la ventaja de estos métodos de simulación computacionales (prestacionales) cuyo uso está cada vez más extendido frente a los modelos prescriptivos. Situando múltiples sensores, se han conseguido identificar las salidas de evacuación del salón de actos que se verían afectadas por el humo en cada caso de las situaciones de incendio propuestas, impidiendo la salida de los ocupantes. Al mismo tiempo, se identifican aquellas vías de evacuación que quedan libres de humos con la finalidad de que las personas presentes en el teatro puedan abandonar la sala sin sufrir graves consecuencias y una aproximación del tiempo que tendrían para hacerlo. Por último, se ha propuesto una mejora en la gestión de humos respecto al caso más desfavorable identificado.

The objective of this work is to determine the physical-chemical variations in the water quality of Carrizal River (Manabí), in 21 sampling stations, covering an area of ​​51 km. Seven points were sampled in (i) the microbasin, 5 points in (ii) La Esperanza reservoir and 9 points in (iii) the sub-basin of Carrizal river. Samples were taken according to an experimental design of random blocks with factorial arrangement, and according to the results we show a correlation between the parameters and the different geographical positions monitored. Thus, we calculate a new water quality index called hardness index, which relates the alkalinity, oxide-reduction potential, hardness, conductivity, suspended solids, and total solids. According to this index, it is evident that, the concentration of minerals of calcium, magnesium, sulphates and carbonates, increased along with the concentration of solids in the water, as well as the REDOX potential and electrical conductivity, increased from the upper basin to the lower basin of Carrizal river. This correlation between physical-chemical parameters of the water and the different geographical positions monitored is probably influenced by anthropogenic activities., El presente trabajo tiene como objetivo determinar las variaciones físico-químicas de la calidad del agua del río Carrizal (Manabí) en 21 estaciones de muestreo, cubriendo una extensión de 51 km. Se muestrearon 7 puntos en (i) la microcuenca, 5 puntos en (ii) el embalse La Esperanza y 9 puntos en (iii) la subcuenca del río Carrizal. Se tomaron muestras siguiendo un diseño experimental de bloques al azar con arreglo factorial; según los resultados, se evidenció una correlación entre los parámetros y las diferentes posiciones geográficas monitoreadas. Ante esto, se calculó un nuevo índice de calidad de agua denominado índice de dureza, el cual relaciona la alcalinidad, potencial de óxido-reducción, dureza, conductividad, sólidos suspendidos, sólidos totales. Según este índice, se evidenció que la concentración de minerales de calcio, magnesio, sulfatos y carbonatos, incrementó junto con la concentración de sólidos en el agua, así como el potencial REDOX y la conductividad eléctrica, que incrementaron desde la cuenca alta hasta la cuenca baja del río Carrizal. Esta correlación entre parámetros físico-químicos del agua y las diferentes posiciones geográficas monitoreadas es probablemente influenciada por actividades antropogénicas.

The fractal dimension (FD) is calculated for seven species of the genus Quercus L. in Calabria region (southern Italy), five of which have a marcescent-deciduous and two a sclerophyllous character. The fractal analysis applied to the leaves reveals different FD values for the two groups. The difference between the means and medians is very small in the case of the marcescent-deciduous group and very large when these differences are established between both groups: all this highlights the distance between the two groups in terms of similarity. Specifically, Q.crenata, which is hybridogenic in origin and whose parental species are Q.cerris and Q.suber, is more closely related to Q.cerris than to Q.suber, as also expressed in the molecular analysis. We consider that, in combination with other morphological, physiological and genetic parameters, the fractal dimension is a useful tool for studying similarities amongst species.

OBJECTIVES: It is unknown if human papillomavirus (HPV) serum antibody responses vary by anatomic site of infection. We aimed to assess the seroprevalence for HPV 6, 11, 16 and 18 in association with HPV DNA detection in different anatomic sites among women. METHODS: This cross-sectional population-based study analyzed data from 524 women aged 16–64 years living in the San Juan metropolitan area of Puerto Rico (PR). Questionnaires were used to assess demographic and lifestyle variables, while anogenital and blood samples were collected for HPV analysis. Logistic regression models were used to estimate the adjusted prevalence odds ratio (POR) in order to determine the association between HPV DNA infection status in the cervix and anus and serum antibody status, controlling for different potential confounders. RESULTS: Overall, 46.9% of women had detectable antibodies to one or more types whereas 8.7% had HPV DNA for one or more of these types detected in cervix (4.0%) or anus (6.5%). Women with cervical HPV detection tended to be more HPV seropositive than women without cervical detection (adjusted POR (95%CI): 2.41 (0.90, 6.47), p=0.078); however the type-specific association between cervical DNA and serum antibodies was only significant for HPV 18 (adjusted POR (95% CI): 5.9 (1.03, 33.98)). No significant association was detected between anal HPV and seropositivity (p>0.10). CONCLUSION: Differences in the anatomic site of infection could influence seroconversion, however, longitudinal studies will be required for further evaluation. This information will be instrumental in advancing knowledge of immune mechanisms involved in anatomic site response.

Las helmintiasis afectan a más de 1.500 millones de personas en todo el mundo, causando una gran morbilidad y asociándose a importantes tasas de discapacidad. En ganadería, se estiman pérdidas de productividad de unos 1.200 millones de euros al año, a los que hay que sumar los gastos en tratamientos antiparasitarios que rondan los 1.000 millones de euros anuales. En los animales cursan con problemas gastrointestinales, causando pérdidas de peso, retraso en el crecimiento y baja reproductibilidad. En la presente Tesis Doctoral se pretendió establecer un modelo experimental para el estudio de las nematodosis que resultara técnicamente fácil y reproducible. Para ello, se han puesto a punto las condiciones óptimas de infección con el modelo experimental Aspiculuris tetraptera-ratón (CD1), que han resultado en una dosis infectante de 2 hembras grávidas y un tiempo de infección inferior a 30 días para evitar infecciones secundarias consiguiendo infecciones controladas. En este modelo se ha caracterizado la respuesta inmunitaria, tanto a nivel local como sistémico, generada en el hospedador. Se han analizado los niveles de inmunoglobulinas, el recuento leucocitario, los cambios histológicos en la mucosa de colon y los perfiles de expresión de citoquinas en órganos y tejidos inmunoactivos (bazo, nódulo linfático mesentérico y placas de Peyer). La infección controlada del ratón se acompaña de elevadas IgM e IgG específicas en suero e IgA en mucosa. Todo ello acompañado por sobreproducción de quimioquinas atrayentes de neutrófilos y macrófagos en mucosa, y una respuesta inflamatoria Th17 inicial que facilita el establecimiento del parasitismo, y que deriva en una respuesta Th9 local promovida por la IL-25 producida en respuesta a la infección. Como un primer abordaje en el estudio de la prevención de las nematodosis se ha empleado este modelo en ensayos de vacunación con la proteína recombinante PP2A, formulada con distintos adyuvantes (mucoadhesivos como carboximetilcelulosa o Plurónic, paredes bacterianas, polietilenimina y ácido oleico) y utilizando distintas vías de administración. Se seleccionaron aquellas fórmulas que mostraron los mejores valores de eficacia (tasa de protección) y seguridad (efectos adversos), así como un mayor efecto inmunomodulador protector. Con el presente trabajo hemos establecido el modelo de A. tetraptera-ratón que puede ser utilizado para futuros estudios de laboratorio. Los primeros estudios de vacunación, realizados utilizando este modelo, sugieren que la vacuna constituida por el péptido PP2Ar con carboximetilcelulosa y paredes bacterianas (FV-CMC) como coadyuvantes, es una buena candidata para ensayos de inmunización intranasal frente a nematodosis ya que proporcionó buenos niveles de protección al aumentar la rapidez y eficacia de la respuesta inmunitaria del hospedador.

[Introduction] Interstitial lung diseases (ILD) and, in particular, idiopathic pulmonary fibrosis, may have a significant impact on patient survival. Recent studies highlight the need for palliative care (PC) in the management of ILD patients. The aim of this study was to determine the current situation of PC in patients in Spain. [Methods] A 36-question survey addressing the main aspects of PC in ILD patients was designed. The survey was sent via email to all members of the Spanish Society of Pulmonology and Thoracic Surgery. Participation was voluntary. [Results] One hundred and sixty-four participants responded to the survey. Ninety-eight percent said they were interested in PC, 46% had received specific training, and 44% reported being responsible for PC in their ILD patients. Symptom control and end-of-life stage were the most frequent reasons for referral to PC teams. Regarding end-of-life, 78% reported consensual agreement with patients on the limitation of therapeutic efforts, 35% helped prepare an end-of-life advance directive, and 22% agreed on the place of death. [Conclusion] Despite the well-known need for PC in patients with ILD and the notable interest of the survey participants in this subject, there are clear formative and organizational gaps that should be addressed to improve care in this area in ILD patients in Spain. [Introducción] Las enfermedades pulmonares intersticiales difusas (EPID) y, concretamente, la fibrosis pulmonar idiopática, pueden tener un elevado impacto en la supervivencia de los pacientes. Recientes estudios destacan la necesidad de implementar los cuidados paliativos (CP) en el manejo del enfermo con EPID. El objetivo del estudio fue conocer la situación actual de los CP en nuestro país. [Métodos] Se diseñó una encuesta de 36 preguntas, que abordaba los principales aspectos de los CP en el paciente con EPID. Esta encuesta fue remitida a través de correo electrónico a todos los miembros de la Sociedad Española de Neumología y Cirugía Torácica, cuya participación fue voluntaria. [Resultados] Cientosesenta y cuatro participantes respondieron a la encuesta. El 98% manifestó tener interés en los CP, 46% habían recibido formación específica. El 44% refirió ser el responsable de los CP en sus pacientes EPID. El control de síntomas y la fase final de vida fueron los motivos más frecuentes de derivación a los equipos de CP. Referente a la fase final de vida el 78% refirió consensuar con los pacientes la limitación del esfuerzo terapéutico, el 35% realizar un documento de voluntades anticipadas y el 22% consensuar el lugar de fallecimiento. [Conclusión] A pesar de la conocida necesidad del CP en los pacientes con EPID y el notable interés de los participantes de la encuesta en este tema, existen claras lagunas formativas y organizativas, que deberían ser contempladas para mejorar la atención sobre esta área de salud en los pacientes con EPID de nuestro país.

No obstante, el mero añadido no garantiza per se su calidad. Se exponen los resultados de un proyecto de innovación para fomentar el aprendizaje autónomo en los grados de educación infantil y primaria. Durante un semestre todos los alumnos de primer curso pudieron usar un sistema de aprendizaje autónomo en la plataforma online institucional (Moodle), apoyado con feedback formativo automático. El sistema se insertaba en un programa complejo de evaluación formativa. Se presentan resultados atendiendo a dos aspectos: uso real de los estudiantes y valoración final del sistema por parte de estos. ESP

El propósito del estudio fue determinar la prevalencia y las variables relacionadas con la Psicosis Refractaria al Tratamiento (TRP) en una muestra de pacientes con trastornos psicóticos diagnosticados a través de una primera hospitalización psiquiátrica en el Hospital Clínico Universitario de Valladolid (HCUV) entre los años 2004 y 2011. Métodos: Es un estudio observacional retrospectivo, en el que la información se obtuvo a través de los informes de alta hospitalaria del primer episodio de psicosis (FEP) de una muestra de 44 pacientes. Después de al menos 5 años de seguimiento, estos pacientes fueron reevaluados y divididos en dos grupos: TRP y no TRP. Usamos un cuestionario que contestaron los profesionales de Salud Mental a cargo de los pacientes. Realizamos un análisis estadístico bivariante para establecer la asociación entre distintas variables sociodemográficas y clínicas y la evolución de la psicosis. Resultados: Un total de 14 personas (31,8% de la muestra) cumplieron criterios para TRP. El sexo, la edad de comienzo, el uso de sustancias, las características del primer ingreso, la historia familiar de enfermedades mentales o el nivel de funcionamiento premórbido no mostraron diferencias estadísticas entre los dos grupos. La puntuación total en la Escala de Síntomas Positivos y Negativos (PANSS) en el primer episodio de psicosis, la puntuación en la escala de Funcionamiento Personal y Social (PSP) tras ≥5 años de seguimiento, y una mala adherencia al tratamiento antipsicótico mostraron una relación significativa con la presencia de TRP. Conclusión: Es un reto para clínicos e investigadores comprender los mecanismos que subyacen tras la mala evolución de los trastornos psicóticos, y desarrollar intervenciones precoces que permitan prevenir la TRP, o al menos mejorar la sintomatología y fomentar un buen funcionamiento personal y social en estos pacientes. La utilización de clozapina y antipsicóticos inyectables de acción prolongada podrían ser útiles en el tratamiento de la TRP., Grado en Medicina

Objectives: Systemic sclerosis (SSc) and rheumatoid arthritis (RA) are autoimmune diseases that share clinical and immunological characteristics. To date, several shared SSc- RA loci have been identified independently. In this study, we aimed to systematically search for new common SSc-RA loci through an inter-disease meta-GWAS strategy. Methods: We performed a meta-analysis combining GWAS datasets of SSc and RA using a strategy that allowed identification of loci with both same-direction and opposingdirection allelic effects. The top single-nucleotide polymorphisms (SNPs) were followed-up in independent SSc and RA case-control cohorts. This allowed us to increase the sample size to a total of 8,830 SSc patients, 16,870 RA patients and 43,393 controls. Results: The cross-disease meta-analysis of the GWAS datasets identified several loci with nominal association signals (P-value < 5 x 10-6), which also showed evidence of association in the disease-specific GWAS scan. These loci included several genomic regions not previously reported as shared loci, besides risk factors associated with both diseases in previous studies. The follow-up of the putatively new SSc-RA loci identified IRF4 as a shared risk factor for these two diseases (Pcombined = 3.29 x 10-12). In addition, the analysis of the biological relevance of the known SSc-RA shared loci pointed to the type I interferon and the interleukin 12 signaling pathways as the main common etiopathogenic factors. Conclusions: Our study has identified a novel shared locus, IRF4, for SSc and RA and highlighted the usefulness of cross-disease GWAS meta-analysis in the identification of common risk loci.

The 2013 Spanish National Security Strategy includes cyberattacks as one of the threats which Spain is exposed to, and which according to the defined strategy routes, Spain will have to face to. Nevertheless, cyberthreats analysis cannot be done disconnectedly from those threats which are, also included in the document. On the opposite, most part of the threats mentioned in the ESN, are able to turn up through a cyberattack, so a global perspective, analyzing the subject, is due. A new question arises from this analysis, in the sense of having in mind when a ciberattack becomes a threat for Defense instead of a threat for security. A legal perspective will be the only proper way to settle when a cyberattack could be considered as a use of force, in order to answer the former question. La Estrategia de Seguridad Nacional de 2013 incluye los ciberataques como una de las amenazas a las que España está expuesta y a las que, siguiendo las líneas de estrategia que se definen, habrá que hacer frente. Sin embargo, el estudio de las ciberamenazas no puede realizarse de forma aislada respecto de otras que, asimismo, son identificadas por el citado documento. Antes al contrario, la práctica totalidad de las amenazas citadas por la ESN, pueden manifestarse a través de un ataque cibernético, de modo que resulta necesario abordar este tema desde una perspectiva global. Este análisis surge la necesidad de plantear en qué momento un ciberataque deja de ser una amenaza para la seguridad y comienza a serlo para la Defensa Nacional. Únicamente un enfoque jurídico que permita discernir en qué ocasiones un ciberataque puede ser considerado uso de la fuerza, podrá dar respuesta a esta cuestión.

ObjectivePersistent infection with high-risk (HR) HPV is a necessary risk factor for the development of cervical cancer. Information on HPV infection is limited in Puerto Rico. This study determined the distribution of HPV types and the association of HR-HPV types with cervical pathology in a clinic-based sample of women in PR.MethodsData from 92 female participants aged 18 to 34 years and recruited from the University of Puerto Rico-Gynecology Clinic, were analyzed. Cervical cytology was performed. HPV testing was performed using L1 consensus primer PCR with MY09/ MY11 primers and typed by dot-blot hybridization. Logistic regression modeling was used to determine the crude and covariate adjusted association between HR-HPV and cervical pathology.ResultsTwenty percent (n = 18) of the patients had abnormal cytology, 45.7% (n = 42) were HPV positive, and 30.4% (n = 28) were HR HPV-positive. Women infected with HR-risk HPV types were 7.9 (95% CI = 2.5-25.5) times more likely to have abnormal cytology as compared to women without HR infection when adjusted by age and age at first sexual intercourse.ConclusionThe burden of HPV infection was high, and, as expected, HR HPVs were strongly associated with dysplasia. A population-based study is needed to estimate HPV prevalence and its association with related malignancies in our population. This will be of great value in determining disease burden and will increase awareness of the HPV vaccination in our population.

8 páginas, 3 figuras, 3 tablas, 3 apéndices., Our soil study of the chromic soils located in the strip of land between Sierra Morena and the Guadalquivir Valley reveals a pH-value close to neutral in 25 samples. The pH-values are close to 5.5 only in the samples taken from the arenosols of the granitic batholith of Los Pedroches. However, it is the percentage of clay, sand and silt that explains the occurrence of grassland dominated by Raphanus raphanistrum, in the company of species with a psammophilous character such as Linaria spartea, Linaria amethystea and Brassica barrelieri. Our soil analysis reveals the presence of acid or base-poor soils. Consequently, the community of Raphanus raphanistrum found there displays an acidophilous, neutro-basophilous and subarenicolous character. Different types of sabulicolous plant communities appear as a result of the gradation of the soil texture. Since the Raphanus communities cannot be considered as genuinely sabulicolous, we have coined the term 'subarenicolous' to describe any plant community whose optimum sand content lies between 50% and 70%. The community of Linaria spartea and Raphanus raphanistrum is neutro-basophilous and is found in at least the Marianic-Monchiquensean and North Hispalensean sectors. An HPV vaccine has been approved for men aged 9 to 26 in the US for the prevention of genital warts and anal cancer. The purpose of this study is to describe 1) HPV vaccine awareness, 2) willingness to get the HPV vaccine and 3) perceived susceptibility to HPV-related cancers and genital warts among men 18-26 years old who attend an STI clinic in San Juan, Puerto Rico (PR).A cross-sectional pilot study consisting of 206 HIV+/HIV- men. For purpose of this analysis, only those participants agedor=26 years old were included in this analysis (n=46).None of the study participants had been vaccinated against HPV. Fewer than a third knew about the HPV vaccine (28.3%). However, more than half (76.9%) were willing to be vaccinated against HPV. Information sources about the HPV vaccine included their female sexual partners (13.0%), a female sexual partner who received the vaccine (8.7%) and a male sexual partner (2.2%). Most participants'reported that the main reason that would increase their willingness to get vaccinated was if a physician recommend the vaccine (95.7%). Perceived susceptibility was low, particularly for anal and oral cancer.This pilot study shows poor awareness of the HPV vaccine, although willingness to getting the HPV vaccine was high among those who knew about the vaccine. Future studies should try to evaluate this paradox and study in depth willingness and barriers to vaccination among male sub-groups, such as men who have sex with men (MSM). These studies should also evaluate predictors of uptake of the HPV vaccine among men in this and other STI clinics in PR, in order to develop interventions to increase male vaccination.

Ozone (O₃) is harmful to breathe at the surface and acts as a greenhouse gas in the upper troposphere. It has become important to understand how O₃ is distributed throughout the troposphere and how its distribution changes with time. This study focused on variations of O₃ in the tropical upper troposphere over the Atlantic Ocean. It is hypothesized that tropical deep convection reduces O₃ in the upper troposphere because the air processed through the storm updraft originates in a relatively pristine region at low altitudes. NSF/NCAR GV Aircraft observations from the PRE-Depression Investigation of Cloud-systems in the Tropics (PREDICT) project along with cloud top temperatures calculated from infrared satellite images are used. To better examine the relationship of O₃ fluctuations with other variables, deviations from the mean vertical profile of O₃ and potential temperature are calculated. Condensed water content greater than 0.0722 g m-3 and cloud top temperatures colder than the plane’s temperature plus 4˚C identify periods when the GV was in cloud produced by deep convection. We find a decrease in upper tropospheric O₃ concentrations 76% of the time that the GV was in cloud. O₃ concentrations outside of clouds vary as much or more than the variations measured while in convection. O₃ concentrations decrease 61% of the time potential temperature deviations are negative. Instances where in-cloud O₃ was not reduced may indicate O₃ sources from lightning or the transport of air from non-pristine boundary layers. This research contributes to a better understanding of O₃ distribution so that we can better monitor its behavior.

[spa] El objetivo de este estudio es presentar una experiencia de aprendizaje virtual a distancia en el ámbito de la enseñanza de las matemáticas en educación superior. El curso se ofrece como programa de apoyo para alumnos de un máster de Ciencias de la Computación y está específicamente diseñado para satisfacer las necesidades de los estudiantes que iniciaban dicho programa, particularmente la falta de comprensión del lenguaje lógico detectada en varias promociones anteriores de los alumnos del CENIDET. El curso tiene como objetivo el desarrollo de la habilidad de uso del lenguaje lógico, la cual es básica para cursar con éxito el máster de Ciencias de la Computación, así como para su posterior aplicación en contextos profesionales relacionados con la Ingeniería computacional. Dieciocho estudiantes distribuidos por todo México participaron voluntariamente en el estudio bajo la dirección de un tutor. El diseño tecnopedagógico del curso se basa en dos premisas teóricas. Las decisiones didácticas relacionadas con el contenido se fundamentan en varios conceptos derivados de la segunda generación de la Teoría de la Actividad (TA). El concepto de «base de orientación para la acción» ha sido particularmente útil para definir las habilidades que se esperaba que desarrollaran los estudiantes. Las decisiones didácticas relacionadas con la interacción de los participantes se basan en el modelo de enseñanza acelerada en equipo de Slavin. A continuación se expone detalladamente la estructura del curso y se presentan algunos extractos de la interacción de los estudiantes para ilustrar su proceso de aprendizaje., [eng] The aim of this article is to present a distance e-learning experience of mathematics in higher education. The course is offered as a remedial program for master's degree students of Computer Science. It was designed to meet the particular needs of the students entering the master's degree program, as a response to the lack of understanding of logical language which was identified in several previous cohorts of students at CENIDET. The course addresses mathematical abilities of comprehensive functional use of logical language as a basic ability to be developed for later successful participation in the Master of Computer Science and also for later use in professional contexts of Computer Engineering. Eighteen students distributed throughout Mexico volunteered to participate under the guidance of one instructor. The techno-pedagogical design of the course is grounded on two theoretical approaches. Content related instructional decisions are supported by different concepts of the second generation of Activity Theory. The concept of Orienting Basis of an Action was particularly useful to define the skills the students were expected to develop. Instructional decisions related to the participants' interaction are underpinned by Slavin's Team Accelerated Instruction model. We present the course structure in detail and provide some student interaction excerpts in order to illustrate their learning progress.

Monográfico con el título: 'Los mecanismos del cambio cognitivo'. Resumen basado en el de la publicación Se presenta un estudio sobre la colaboración entre estudiantes universitarios para elaborar conjuntamente un texto comunicándose por escrito y asíncronamente. Las condiciones operacionales de este tipo de colaboración sugieren la existencia de formas de construcción conjunta de significados y de características discursivas específicas. Se estudian cuatro casos de escritura colaborativa en pequeño grupo durante seis semanas. El análisis se focaliza en la naturaleza del discurso, en la construcción de significados compartidos, y en el avance de la intersubjetividad entre los participantes. Los resultados permiten identificar fases de intersubjetividad distintas en las que la construcción de significados funciona de manera diferenciada, y en las que el discurso toma formas específicas. Con respecto a la acogida de los solicitantes de asilo en la Unión Europea, podemos encontrar disposiciones para la protección de las personas con discapacidad en una amplia gama de fuentes reguladoras.

Se han revisados todos los enfermos trasplantados de hígado en el H. U. Reina Sofía de Córdoba entre los años 1989 y 2005, seleccionado aquellos hígados con cirrosis y sobre los que asienta una presión tumoral, en su mayoría carcinoma hepatocelular. Los datos generales que se recogieron de cada paciente fueron: edad, sexo, tiempo de evolución, recidiva tumoral, recidiva de la enfermedad, rechazo del injerto, éxitus del paciente y causas. Realizándose un estudio macroscópico de la pieza quirúrgica, así como una revisión histológica, que incluía entre otros, el tipo tumoral, la invasción capsular o la invasción vascular. Obtuvimos 114 casos con hepatocarcinoma de entre los 650 pacientes trasplantados de hígado. En relación con un origen viral en su mayoría (virus de la hepatitis). La importancia de este estudio radica, entre otras cosas, en valorar los posibles factores pronósticos que influyen en la supervivencia del paciente transplantado y que poseen valor estadísticamente significativo. Nostros hemos encontrado dos: la recidiva de la enfermedad y la recidiva tumoral. También tiene valor estadísticamente significativo la relación que ejerce la invasión vascular sobre la recidiva tumoral.
