To test the efficacy of cognitive-behavioural therapy (CBT) for lifestyle modification in patients with metabolic syndrome (MetS).76 MetS patients completed this clinical trial, with 18 months follow-up. 45 participants from the experimental group (EG - CBT) and 31 to the control group (CG - usual care). The CBT programme was performed by a psychologist in a face-to-face group format, during 12 weekly sessions lasting 90 minutes. The intervention for the CG consisted of workshops with basic information about MetS and it's associated cardiovascular risk.Efficacy of (CBT) in (MetS) patients.Results showed reduction in weight (mean difference - MD -2.633, 95%CI [-4.322, -0.943]; p.003), waist circumference (MD -2.944, 95%CI [-5.090, -0.798]; p.008), body mass index (MD -0.915, 95%CI [-1.494, -0.335]; p.003), systolic (MD -0.046, 95%CI [-0.685, -0.023]; p.0002) diastolic blood pressure (MD -4.777, 95%CI [-7.750, -1.804]; p.002), and cardiovascular risk score after 18 months. An increase in adherence to the Mediterranean diet and assertiveness and a reduction in anger were observed in EG. The CG did not show any significant differences.The CBT focused on changes in lifestyle seems to be effective in the reduction of MetS and cardiovascular risk factors.Registered at clinicaltrials.gov (NCT02949622) - PROMETS (Multimodal Intervention Program for Patients with Metabolic Syndrome).

Resumen Antecedentes y objetivo El sindrome metabolico (SM) es un conjunto de condiciones metabolicas que incluyen obesidad abdominal, reduccion de las concentraciones de colesterol unido a las high density lipoproteins (c-HDL, «lipoproteinas de alta densidad»), trigliceridos elevados, aumento de la presion arterial e hiperglucemia. Dado que se trata de una enfermedad multicausal, el objetivo de este estudio es identificar las variables psicologicas, emocionales y de estilo de vida que pueden ejercer una influencia sobre los diferentes componentes del SM. Pacientes y metodos Estudio transversal con 103 pacientes diagnosticados de SM (47 varones y 56 mujeres). Se recogieron medidas antropometricas, clinicas y analiticas para valorar las variables asociadas al SM. Tambien se evaluaron las principales variables psicologicas y emocionales. Resultados Se realizaron diferentes pruebas de regresion lineal multiple para identificar que variables eran predictoras del SM. Las variables dependientes fueron el indice de masa corporal (IMC), la circunferencia abdominal, el c-HDL y la calidad de vida, y las variables predictoras fueron el estres psicologico, la ira y la adherencia a la dieta mediterranea. Los resultados mostraron que el estres psicologico era un predictor de la calidad de vida (β = −0,55, p ≤ 0). De igual modo, la ira fue un predictor del IMC (β = 0,23, p = 0,047) y de la circunferencia abdominal (β = 0,27, p = 0,021). Segun lo previsto, la adherencia a la dieta mediterranea fue un predictor del c-HDL (β = 0,2, p = 0,045) y de la calidad de vida (β = −0,18, p = 0,031). Conclusiones Los resultados confirman un vinculo entre la adherencia a ciertos habitos alimentarios y el estilo de vida; sin embargo, se situan un paso por delante y resaltan la importancia de los factores psicologicos y emocionales como el estres y la ira en algunos componentes del SM.

Background and objective Non-adherence to treatment is usually a clinical problem in patients with systemic lupus erythematosus (SLE). Increasing the knowledge of predictors of treatment adherence can be meaningful in the clinical setting. The main objective of the present study was to analyze the influence of sociodemographic, clinical and psychological variables on the degree of treatment adherence in a sample of Spanish women with SLE. Patients and method This is an observational–transversal study. All participants were evaluated for the degree of treatment adherence, their clinical status, psychopathological manifestations, the level of perceived stress and self-efficacy. The sample was divided into two groups (adherent vs non-adherent). The factors associated with a lack of adherence in this sample were analyzed by means of logistic regression. Results This study comprises 72 women with SLE (average age = 36.72 ± 12.2 years). Almost 64% of patients with SLE were non-adherent to treatment. The results showed that a low educational level, being unemployed, living with a partner and alcohol abuse were associated with low treatment adherence. There were significant mean differences between groups in psychopathological subscales of somatization, obsession–compulsion and general psychopathological indices. There were also mean differences between groups for the level of perceived stress. The use of non-steroidal anti-inflammatory drugs, suffering arthrosis and scoring higher in dimensions of psychopathology were significant predictors of treatment adherence, explaining between 35% and 47% of its variability. Conclusions Including the clinical and psychopathological manifestations as important aspects in the clinical reasoning of health professionals could improve the adherence to treatment of patients with SLE.

Resumen Antecedentes y objetivo El incumplimiento terapeutico constituye un importante problema asistencial en pacientes con lupus eritematoso sistemico (LES). Conocer los factores asociados de este puede ser de gran utilidad en el ambito clinico. El objetivo de esta investigacion fue analizar la influencia de las variables sociodemograficas, clinicas y psicologicas sobre la adherencia al tratamiento en una muestra espanola de mujeres con LES. Pacientes y metodo En este estudio transversal-observacional se evaluo el grado de adherencia al tratamiento, el estado clinico y psicopatologico, el estres psicologico y la autoeficacia en mujeres con LES. La muestra se dividio en 2 grupos: adherentes y no adherentes. Mediante un analisis de regresion logistica se identificaron los factores asociados a la falta de adherencia en esta poblacion. Resultados Se evaluaron 72 pacientes (edad media: 36,7 ± 12,2 anos). El 63,9% no tuvieron adherencia. Un bajo nivel educativo, estar desempleada, vivir en pareja y el consumo de alcohol se asociaron a una baja adhesion al tratamiento. Se encontraron diferencias entre el grupo adherente y el no adherente en las subescalas psicopatologicas de somatizacion, obsesion-compulsion e indices psicopatologicos generales, asi como en el estres percibido, obteniendo puntuaciones mas altas las pacientes adherentes. El uso de antiinflamatorios no esteroideos (AINE), tener artrosis y mayor alteracion psicopatologica fueron predictores significativos de la adherencia al tratamiento, explicando entre el 35 y el 47% de la variabilidad de esta. Conclusiones La falta de adherencia al tratamiento de las pacientes con LES fue alta y parece estar influida por factores clinicos y psicopatologicos.

Social support can introduce favorable changes in lifestyle to reduce the cardiovascular risk. The aim of this study was to verify these changes in the clinical population participating in this study and present the psychometric properties of the scales of social support for 'Eating Habits' and 'Exercise' in patients diagnosed with metabolic syndrome.135 participants attending a programme for changing lifestyle habits. Sociodemographic, psychological, and lifestyle variables were assessed at the Hospital Universitario Virgen de las Nieves (HUVN) in Granada (Spain) between 2013 and 2014. The following procedures were used: Confirmatory factor analysis (CFA), internal consistency, sensibility to change and temporal stability.The AFC confirmed the original structure of both scales, except for the exclusion of factor 2 (family) from the social support scale for the year. For the social support scale, the local adjustment, all items presented high values of factor loads and individual reliability (λ≥0.64 and R2≥0.41, respectively). For the exercise scale, the local adjustment, the items presented high values of factor loads and individual reliability (λ≥0.62 and R2≥0.38, respectively). Internal consistency values were between adequate and excellent, with Cronbach's alpha figures between 0.714 and 0.864. Regarding sensitivity to change, the experimental group increased the perception of social support for food and exercise. The control group did not show significant differences.Our results show adequate indices for validity and reliability of the measures. Both measures appeared to be useful to assess social support in patients diagnosed with metabolic syndrome.El apoyo social puede introducir cambios favorables en el estilo de vida de cara a reducir el riesgo cardiovascular. El objetivo de este estudio fue verificar dichos cambios en la población clínica que participó en el mismo y presentar las propiedades psicométricas de dos escalas de apoyo social: los hábitos alimentarios y el ejercicio. Ambas fueron aplicadas en pacientes con síndrome metabólico (SM).Participaron 135 sujetos en un programa de modificación de estilo de vida. Las medidas sociodemográficas, psicológicas y de estilo de vida fueron tomadas en el Hospital Universitario Virgen de las Nieves (HUVN) de Granada (España) durante 2013 y 2014. Se realizaron los siguientes análisis: análisis factorial confirmatorio(AFC), de la consistencia interna, de la sensibilidad al cambio y de la estabilidad temporal.El AFC confirmó la estructura original de ambas escalas, excepto por la exclusión del factor 2 (familia) de la escala de apoyo social para el ejercicio. Para la escala de apoyo social, el ajuste local, todos los ítems presentaron altos valores de cargas factoriales y fiabilidades individuales (λ≥0,64 y R2≥0,41, respectivamente). Para la escala de ejercicio, el ajuste local, los ítems presentaron altos valores de cargas factoriales y fiabilidades individuales (λ≥0,62 y R2≥0,38, respectivamente). Los valores de consistencia interna resultaron entre adecuados y excelentes, con cifras de alfa de Cronbach entre 0,714 y 0,864. En cuanto a la sensibilidad al cambio, el grupo experimental aumentó la percepción del apoyo social para la alimentación y para el ejercicio. El grupo de control no presentó diferencias significativas.Los resultados muestran niveles adecuados de validez y fiabilidad, demostrando que las escalas son adecuadas para evaluar el apoyo social en pacientes con SM.

The correct diagnosis, classification and therapeutic management of thrombotic microangiopathies (TMA) continue to be a challenge for the clinician. We report a rare case of eosinophilic granulomatosis with polyangiitis (EGPA) as a trigger for complement-mediated TMA in a 57-year-old man who was successfully treated with corticoids, cyclophosphamide and therapeutic plasma exchange. Additionally, we review few other cases reported in the literature and the pathophysiological pathway of association between TMA and EGPA. We found that the mutual relationships between the inflammation triggered by vasculitis, the exacerbated complement activation, together with hypereosinophilia and endothelial damage seem to be the key in explaining the connection between both entities. We suggest that an understanding of the multi-causal nature of TMAs is crucial for the correct diagnosis and treatment of these patients.

Objective To compare the presence of subclinical atherosclerosis measured by means of pulse wave velocity (PWV) in women with primary Sjogren's syndrome (SS) versus a healthy age- and sex-matched control group, and to identify factors independently associated with PWV in primary SS. Methods Forty-four women with primary SS and 78 age-matched healthy women without overt cardiovascular (CV) diseases were assessed for traditional and nontraditional CV risk factors. PWV was also performed. A linear regression analysis was used to identify factors independently associated with PWV in primary SS. Results Women with primary SS had significantly higher PWV than controls (P = 0.030), and the frequency of increased PWV was significantly higher in this group (25% versus 8%; P = 0.013). The proportion of patients ages ≤50 years (ratio 4.6) with increased PWV was almost 2-fold higher than those ages >50 years (ratio 2.4) with respect to controls. Positivity for anti-SSB was more frequent in patients with normal PWV than in those with increased PWV (61% versus 18%; P = 0.034). Women with primary SS and increased PWV had lower levels of 25-hydroxyvitamin D (25[OH]D; P = 0.047) than primary SS patients with normal PWV. In addition, 25(OH)D levels tended to correlate inversely with PWV in women with primary SS (P = 0.067), but not in controls (P = 0.97). In multivariate analysis, the Framingham Risk Score (FRS) and Sjogren's Syndrome Damage Index emerged as factors independently correlated with PWV. Conclusion Women with primary SS had higher PWV than controls, but a similar FRS. The FRS and chronic damage were found to be independently associated with PWV.

Objective Verify the efficacy of cognitive behavioral therapy (CBT) in adherence to the Mediterranean diet (MedDiet) in metabolic syndrome (MetS) patients. Design, Setting, Participants, and Intervention In the Multimodal Intervention Program for Patients with Metabolic Syndrome clinical trial, 79 MetS patients completed the intervention. Of those, 48 belonged to the experimental group and 31 to the control group. The intervention received by the experimental group was CBT; the control group followed usual care and attended a workshop on healthy lifestyle. Main Outcome Measure(s) and Analysis Anthropometric, biochemical, psychological, and lifestyle measures were taken before and after the intervention at 3 and 6 months. Analyses included paired t tests, ANOVA, and ANCOVA. Results The ANOVA results showed a statistically significant interaction between the 2 groups in waist circumference (P = .009), triglycerides (P = .015), and adherence to the MedDiet (P = .026). The ANCOVA results indicated between-group difference in waist circumference (P = .026 and .062 at 3 and 6months, respectively), in triglycerides (P = .009 and .860 at 3 and 6 months, respectively), and in MedDiet (P = .024 and .273 at 3 and 6 months, respectively). Conclusions and Implications In interventions in which CBT was applied, significant improvements were observed in MetS patients, especially in adherence to the MedDiet.

Self-efficacy refers to beliefs in individuals' own capacities to do something. With the intent of evaluate the validity and reliability, we investigated the psychometric properties of the Self-Efficacy to Regulate Exercise Scale (SERES) in patients with metabolic syndrome (MetS).135 participants with medical diagnosis of MetS took part in the study (Mage=55.5, SD=7.6). We investigated sociodemographic, psychological, and life-style variables. Participants were recruited at the Hospital Universitario Virgen de las Nieves (HUVN) in Granada (Spain) between 2013 and 2014. In order to characterise the sample, descriptive statistics were used. Factorial analysis, internal consistency and convergent reliability of the SERES were also calculated. Group differences were investigated using Student's t test.A solution with two factors was extracted, which explained 72.7% of the variance of the SERES. The internal consistency values for the total score of the SERES were (0.925 and 0.864) according to the values of Cronbach's alpha and Guttman's two halves, respectively. Significant correlations of the SERES were detected for with physical exercise, assertiveness, stress, anger, and active life-style (e.g., renouncing to sedentary behaviours).This was the first study to report on psychometric properties of the SERES. Results supported the adequacy of the measure for use with patients with MetS. SERES seemed to be useful in evaluating self-efficacy in relation to physical exercise, psychosocial, and lifestyle variables.La autoeficacia percibida se refiere a la creencia en la propia capacidad para hacer algo. El objetivo de este trabajo fue obtener la validez y la fiabilidad de la escala de autoeficacia para el ejercicio físico (AEEF) en pacientes con síndrome metabólico (SM) y para ello fueron verificadas las propiedades psicométricas de la misma.Participaron en el estudio 135 sujetos con una edad media de 55,5 años (DT=7,6) que cumplían los criterios diagnósticos para el SM. Se tomaron medidas sociodemográficas, psicológicas y de estilo de vida de los mismos. Los sujetos fueron reclutados en el Hospital Universitario Virgen de las Nieves (HUVN) de Granada (España) durante los años de 2013 a 2014. La caracterización de la muestra fue realizada a través de análisis descriptivos. Se realizó el análisis factorial, de la consistencia interna y de validez convergente del AEEF; las diferencias intra e intergrupales fueron evaluadas a través de la prueba t de Student.Extrajimos una solución de 2 factores que explicaron el 72,7% de la varianza común. Los valores de consistencia interna para la puntuación total de la AEEF fueron de (0,925 y 0,864) según los valores del alfa de Cronbach y del método de las dos mitades de Guttman, respectivamente. Las correlaciones de la AEEF con la frecuencia de la práctica de ejercicios, la renuncia al sedentarismo, la asertividad, el estrés y la ira fueron estadísticamente significativas.Este es el primer estudio sobre las propiedades psicométricas de la versión española de la AEEF. Los resultados mostraron niveles aceptables de validez y fiabilidad en pacientes con SM. La escala demostró ser útil para evaluar la asociación de la autoeficacia relativa al ejercicio físico con distintas variables psicosociales y de estilo de vida.

Aim To determine if there are ethnic differences in the prevalence of antiphospholipid syndrome (APS), clinical presentation and autoantibody profile between Roma and Caucasian patients with systemic lupus erythematosus (SLE). Method A cross-sectional study was conducted including data from Roma and Caucasian SLE patients consecutively attending six hospitals in Spain. Socio-demographic characteristics, prevalence of APS, clinical and analytical features of SLE and APS were compared between ethnic groups. Results Data from 52 Roma and 98 Caucasian SLE patients were included. Roma SLE patients had a higher risk (odds ratio 2.56, 95% CI 1.02–6.39) and prevalence of APS (28.8% vs. 13.3%, P = 0.027). Furthermore, Roma SLE patients had a statistically significant higher prevalence of abortions (23.5% vs. 10.2%, P = 0.049). In relation to other APS diagnostic criteria, Roma SLE patients had a non-statistically significant higher prevalence of fetal deaths (14.3% vs. 5.1%, P = 0.106) and thrombotic events (21.1% vs. 12.2%, P = 0.160). In relation to SLE clinical features, Roma patients had a significantly higher prevalence of arthritis (75% vs. 57.1%, P = 0.034) and non-significant higher prevalence of serositis (44.2% vs. 29.6%, P = 0.104), discoid lesions (11.5% vs. 5.1%, P = 0.191), oral ulcers (46.1% vs. 34.7%, P = 0.218) and livedo reticularis (21.1% vs. 15.3%, P = 0.374). No statistically significant differences were found in the Systemic Lupus International Collaborating Clinics Damage Index or the autoimmune serological profile. Conclusion Prevalence and risk of APS were significantly higher in Roma SLE patients. Furthermore, Roma patients had a significantly higher prevalence of abortions and a non-significant higher prevalence of fetal deaths and thrombotic events.

Background and objective Metabolic syndrome (MetS) is a cluster of metabolic conditions that include abdominal obesity, reduction in cholesterol concentrations linked to high density lipoproteins (HLDc), elevated triglycerides, increased blood pressure and hyperglycaemia. Given that this is a multicausal disease, the aim of this study is to identify the psychological, emotional and lifestyle variables that can have an influence on the different MetS components. Patients and methods A cross-sectional study with 103 patients with diagnostic criteria for MetS (47 male and 56 female). Anthropometric, clinical and analytical measurements were collected to assess the variables associated with MetS. The main psychological and emotional variables were also assessed. Results Different multiple linear regression tests were performed to identify which variables were predictive of MetS. The dependent variables were body mass index (BMI), abdominal circumference, HDLc, and quality of life, and the predictive variables were psychological stress, anger and adherence to a Mediterranean diet. The results showed that psychological stress was a predictor of quality of life (β = −0.55, p ≤ 0). Similarly, anger was a predictor of BMI (β = 0.23, p = .047) and abdominal circumference (β = 0.27, p = .021). As expected, adherence to a Mediterranean diet was a predictor of HDLc (β = 0.2, p = .045) and of quality of life (β = −0.18, p = .031). Conclusions The results confirm a link between adherence to certain dietary habits and lifestyle, however they go one step further and show the importance of psychological and emotional factors like psychological stress and anger in some MetS components.

Objective Homocysteine has been linked to atherosclerosis and hypertension (HT) in the general population. However, there is limited evidence regarding the effect of homocysteine on blood pressure and arterial stiffness in systemic lupus erythematosus (SLE). We examined whether homocysteine is associated with HT and arterial stiffness in women with SLE. Methods In total, 99 women with SLE without a history of cardiovascular disease or diabetes mellitus and 101 matched controls were included in this cross-sectional study. Participants were analyzed for homocysteine levels, cardiovascular risk factors, and arterial stiffness assessed by means of carotid–femoral pulse wave velocity (PWV). Associations between homocysteine, systolic blood pressure (SBP), PWV, and HT were tested using univariate and multivariate analyses. Results Homocysteine levels (mean ± SD 12.3 ± 4.8 versus 9.3 ± 3.8 μmoles/liter), PWV (mean ± SD 7.54 ± 1.1 versus 7.10 ± 1.1 meters/second), SBP (mean ± SD 119 ± 13 versus 115 ± 12 mm Hg), and the prevalence of hyperhomocysteinemia (23% versus 7%) and HT (43% versus 12%) were significantly higher in women with SLE (P < 0.050 for all). In the univariate analysis, homocysteine correlated positively with SBP (P = 0.001) and PWV (P = 0.023) in women with SLE but not in controls. In the multiple linear regression analysis, SBP was independently associated with homocysteine and body mass index (BMI) in women with SLE. Similarly, in the multivariate logistic regression analysis, homocysteine levels (or hyperhomocysteinemia), BMI, and daily prednisone dose were independently associated with HT in women with SLE. Conclusion Homocysteine was independently associated with SBP and HT in women with SLE, but not in controls. Elevated homocysteine levels could increase the risk of HT in SLE.

Objective.To compare the prevalence and disability of headache in patients with systemic lupus erythematosus (SLE) with the general population and to assess the role of chronic psychological stress (CPS) in headache development.Methods.One hundred seventy patients with SLE and 102 control subjects matched for age, sex, and level of education were included in this multicenter, cross-sectional study. CPS, headache-related disability, and chronic analgesic intake (CAI) were evaluated in all participants.Results.No statistical differences in the prevalence of headache between both groups were observed but headache disability was significantly higher in patients with SLE. In addition, a higher average score in the Cohen Perceived Stress Scale (CPSS) and a higher prevalence of patients with CAI were observed in patients with SLE. In multivariate analysis, CPSS score was positively (OR 1.09; 95% CI: 1.03–1.14; p = 0.001) and CAI negatively (OR 0.43; 95% CI: 0.19–0.99; p = 0.049) associated with headache in patients with SLE.Conclusion.Despite the prevalence of headache in patients with SLE and the general population being similar, headache-related disability may be higher in patients with SLE. Moreover, CPS might play a role in the pathogenesis of SLE headache, whereas CAI might have a protective effect against it.

A series of measures in the management of patients with systemic lupus erythematosus (SLE) which usually are not found in the lupus guidelines are discussed. In the lupus patient who has been well-controlled in the long term, the dose of hydroxychloroquine should be progressively reduced, without decreasing more than approximately 600 mg per week. We recommend taking this drug in the morning in patients with insomnia, at night in those with dyspepsia and to separate the intake of the drug from the shower (and the water should be as cool as possible) in those patients with aquagenic pruritus. We do not use prednisone on alternate days and exceptionally divide the dose into ¾ before breakfast and ¼ before dinner. Twenty to 30 min should be used per patient in every scheduled visit to assure a good clinical and human practice. We analyzed the follow-up of 112 consecutive patients from our systemic disease unit and found that 71.4% of them had symptoms that were unexplained by lupus and we only referred 8.9% of them to other specialists, probably because of our general training as internal medicine doctors. We suggest that knowing the views of SLE specialists might be of interest since, well-designed studies that would allow to progress in the understanding of this disease could be performed based on their experience.

Resumen Se comentan una serie de medidas para los pacientes con lupus eritematoso sistemico, que no suelen estar en las guias. Al enfermo muy bien controlado durante anos tendemos a ir reduciendo progresivamente la dosis de hidroxicloroquina, sin bajar de aproximadamente 600 mg/semana. Este farmaco aconsejamos tomarlo por la manana en los pacientes con insomnio, por la noche en los casos de dispepsia y en los que presentan prurito de tipo acuagenico que separen la toma de la ducha, y que esta sea con agua lo menos caliente posible. No usamos el tratamiento con prednisona a dias alternos, y excepcionalmente dividimos la dosis en ¾ antes del desayuno y ¼ antes de la cena. En consultas deberiamos dedicar entre 20 y 30 min por paciente, para hacer una buena practica clinica y humana. En nuestra unidad hemos analizado el seguimiento de 112 enfermos consecutivos, y el 71,4% tenian una sintomatologia no explicable por el lupus, y solamente al 8,9% los derivamos a otros especialistas, probablemente, por nuestra capacitacion general como internistas. Sugerimos que conocer la opinion de los especialistas dedicados a tratar a los enfermos con lupus puede ser de interes, pues a partir de sus experiencias, se pueden programar trabajos bien disenados, que permitirian el avance en el conocimiento de esta enfermedad.

To evaluate psychopathological status and stress level from a sample with SLE; compare mental functioning and stress levels between women with SLE and healthy women; determine whether disease duration, disease activity, cumulative organ damage and stress have an influence on psychopathological symptoms in SLE patients; and evaluate whether perception of stress is related to SLE severity.We conducted a cross-sectional study of 425 participants; 202 women with SLE, with an average age (SD) of 36.61 (10.15), and 223 healthy women, with age-matched controls. The assessment included the clinical characteristics (disease duration, SLE activity, cumulative organ damage, pharmacotherapy), the Symptom Checklist-90-Revised (SCL-90-R) and the Perceived Stress Scale. Descriptive, comparative, univariate and multivariate analysis were performed.SLE patients showed psychopathological alterations in the somatisation, obsessive-compulsive and positive discomfort subscales of SCL-90-R. Women with SLE reported significantly higher scores on the psychopathological dimensions and perceived stress compared to healthy women, except for paranoid ideation. Disease duration, SLE activity, cumulative organ damage, and perceived stress were shown to be significant predictors of psychopathological manifestations, explaining a range, between 20 and 43%, of variance across SCL-90-R dimensions. Moreover, perceived stress was related to SLE activity, after controlling for psychopathological dimensions.The psychopathological manifestations in SLE appeared to be influenced by perceived stress, disease duration, disease activity and cumulative organ damage. In turn, perceived stress was associated with disease severity. This knowledge may contribute to a more comprehensive perspective of these manifestations in the SLE population in the clinical setting.

Although many studies have shown that patients with autoimmune disease present a hypoactive hypothalamic–pituitary–adrenal axis (HPA), controversial results have been described. Our objective was to study HPA axis activity in women with autoimmune disease compared to healthy women. Therefore, we analyzed salivary cortisol over the course of a day, and hair cortisol concentrations from the three preceding months, from 65 women divided into two groups: healthy women (n = 30), with a mean age of 44.70 ± 11.65 years; and women with autoimmune disease (n = 35), with a mean age of 48.26 ± 9.04 years. The latter group comprises women with systemic lupus erythematosus (SLE), Sjögren’s syndrome (SS), and systemic sclerosis (SSc). Perceived stress and psychopathological symptomatology were also evaluated. Autoimmune disease group scored higher on the somatization subscale SCL-90-R and lower on the anxiety subscale than the control group. Regarding HPA axis activation, the area under curve for cortisol levels during the day was higher for the autoimmune disease group. In addition, higher cortisol levels in hair were found in the group with autoimmune disease. Our findings show greater short and long-term HPA axis activity in women with autoimmune disease than in healthy women. We assume that executive function constitutes an integrated set of cognitive processes that mediate working memory, planning, inhibition, flexibility, and decision making. Despite the acknowledged theoretical connection between executive function processes and emotional intelligence, such relationships have rarely been investigated. The purpose of this study was to examine the potential relationship between constructive thinking, conceived as a component of emotional intelligence, and executive function, as indexed by various existing neuropsychological and experimental instruments. We used the Constructive Thinking Inventory as a measure of emotional intelligence. We found that some constructive thinking subscales were able to predict distinct executive function variables. Emotional Coping, Categorical Thinking, and Esoteric Thinking subscales explained performance on various measures of executive function. Thus, we conclude that intervention programs designed to train a specific component of emotional intelligence, namely constructive thinking, could also facilitate performance in executive function processes, and vice versa. Objective.Hypertension (HT) is more prevalent in patients with systemic lupus erythematosus (SLE) than among the general population and it has been associated with atherosclerotic cardiovascular diseases in these patients. We examined the proportion of HT and factors associated with it in young and old women with SLE.Methods.Participants (112 women with SLE and 223 healthy age-matched women) were categorized as young (age ≤ 40 years) or old (age > 40 years). We compared cardiovascular and specific SLE-related variables and inflammatory markers in hypertensive and normotensive women with SLE for each age range. We also assessed the factors independently associated with HT in the entire cohort and in each age range by means of a multivariate regression analysis.Results.The prevalence of HT was higher in women with SLE than in controls (56% vs 29%; p < 0.001), and was proportionally higher in younger women with SLE (40% vs 11%; p < 0.001) than in older women with SLE (74% vs 47%; p = 0.001). After adjustment for potential confounders, HT was associated with renal involvement and higher nonobesity-related insulin levels in younger women with SLE. In older patients, HT was associated with age, renal involvement, and obesity. Finally, in the entire cohort, HT was associated with age, insulin, renal involvement, and the Systemic Lupus Erythematosus Disease Activity Index score.Conclusion.An association between HT and insulin has been identified in women with SLE, particularly younger ones. Factors associated with HT in women with SLE differed depending on their age. HT was more prevalent in women with SLE than in control subjects, being proportionally higher in young women with SLE.

Objective: The aim of the study was to determine the clinical and psychological factors linked to health-related quality of life (HRQOL) in patients with systemic lupus erythematosus (SLE) and test the effectiveness of cognitive behavioural therapy in changing these factors. Methods: We evaluated 34 patients with SLE over a period of 15 months. In order to study the variables related to items of QOL and the physical (PCS) and mental (MCS) component summaries of the SF-36, several multiple linear regression models were constructed. Patients were randomized and distributed into two similar groups; one of them received cognitive behavioural therapy and the other received the usual controlled care. The psychological aspects as well as the related-disease factors were evaluated four times during the study. Results: Self-perceived stress (R2 corrected: 0.314, t: -2.476, p < 0.021), vulnerability to stress (R2 corrected: 0.448, T: -2.166, p < 0.04) and anxiety (R2 corrected: 0.689, T: -7.294, p < 0.00) were predictor variables of MCS. The group of patients who received the therapy improved their level of physical role functioning, vitality, general health perceptions and mental health, compared with the group of patients who only received conventional care. Conclusion: QOL usually depends on multiple factors, some of which are stress and anxiety, which can be modified by a cognitive behavioural therapy, in order to obtain a significant improvement in the HRQOL, irrespective of the activity level of the disease. The objective of this article was to evaluate whether serum uric acid (SUA) correlates with arterial stiffness and inflammation markers in a cohort of women with systemic lupus erythematosus (SLE) without overt atherosclerotic cardiovascular diseases, who attended a community hospital. One hundred and two women with SLE were assessed as part of this cross-sectional study. Carotid-femoral pulse wave velocity (PWV) was measured using an automatic device (Complior®). C-reactive protein (CRP), fibrinogen and homocysteine levels as well as other metabolic results were recorded. Duration and activity of SLE, damage accrual and treatments were recorded. SLE women were categorized as having or not having hyperuricaemia (HU) according to SUA levels (greater than or up to 6.2 mg/dl, respectively). A multiple linear regression analysis was used to determine the independent link between SUA levels and other variables. Women with SLE and HU (n = 15, 15%) had a worse cardiovascular risk profile that included ageing, hypertension, obesity, higher total cholesterol levels, renal failure and presence of metabolic syndrome. Also, the duration of SLE was increased and damage accrual was greater. In the unadjusted analysis, SUA levels correlated with PWV, CRP, fibrinogen and homocysteine. However, in a multivariate linear regression analysis, SUA levels independently correlated with the duration of SLE, creatinine, total cholesterol and homocysteine levels but did not correlate with PWV. In conclusion, SUA was associated with arterial stiffness, but not independently of age and homocysteine levels. Nevertheless, SUA might be an ancillary indicator of subclinical atherosclerosis in SLE women without clinically evident atherosclerotic cardiovascular disease. Lupus (2010) 19, 591—598.

Resumen Fundamento y objetivo El lupus eritematoso sistemico (LES) es una enfermedad autoinmunitaria que afecta la calidad de vida. Hay varios instrumentos especificos que miden la calidad de vida relacionada con la salud (CVRS), pero ninguno se ha adaptado al espanol. El objetivo de este trabajo fue adaptar y validar en la poblacion espanola una medida especifica de la CVRS en los pacientes con LES. Pacientes y metodos La adaptacion se baso en el metodo de traduccion y retrotraduccion de la version inglesa del LupusQoL (Lupus Quality of Life) y su aplicacion en 115 adultos con LES. Resultados El analisis factorial identifico 5 dominios del LupusQoL; la fiabilidad evidencio un alto coeficiente en el alfa de Cronbach de 0,977 y en el metodo de las 2 mitades de Guttman de 0,936; las correlaciones significativas del LupusQoL con el Medical Outcome Survey muestran alta validez convergente del cuestionario, mientras que las correlaciones con el Systemic Lupus Erythematosus Disease Activity Index y el System Lupus International Collaborating Clinics fueron bajas, lo que evidencia su validez discriminante. Conclusion La version espanola del LupusQoL cuenta con estables propiedades psicometricas para medir la CVRS en las personas con LES en ambitos clinicos y de investigacion en la poblacion hispanoparlante.

Objective.To compare 24-h ambulatory blood pressure (BP) monitoring (ABPM) values and patterns in women with systemic lupus erythematosus (SLE) with those of a matched control group and their relationship with the presence of subclinical atherosclerosis.Methods.ABPM was assessed in 70 women with SLE and in 65 sex- and age-matched controls without a history of clinic cardiovascular disease (CVD). Carotid-femoral pulse wave velocity (PWV), which is a marker of subclinical atherosclerosis and a predictor of future CVD, was measured. Multivariate logistic analysis was used to determine which explanatory variables were independently associated with the non-dipper pattern and the presence of nocturnal hypertension (HTN) in women with SLE.Results.No differences in PWV were found between patients and controls [median 7.3, interquartile range (IQR) 6.5–8.1 m/s vs median 7.1, IQR 6.5–7.8 m/s, p = 0.474]. The frequency of nondipper pattern (p = 0.025) and nocturnal HTN (p = 0.004) was significantly higher in women with SLE than in controls. White-coat and masked HTN were present in 10% and 11% of patients and in 20% and 8% of controls, respectively (p > 0.05 in all cases). The concordance between office and ambulatory HTN in the SLE and control groups was modest (κ = 0.325 and κ = 0.451, respectively). PWV and chronic kidney disease, and PWV and the Systemic Lupus Erythematosus Disease Activity Index were found to be independently associated with nocturnal HTN and nondipper pattern, respectively.Conclusion.Women with SLE were more likely to have an altered nighttime BP pattern than controls. In women with SLE, nondipper pattern and nocturnal HTN were independently associated with increased subclinical atherosclerosis measured by PWV.

This is the first Spanish multicentric inception lupus cohort, formed by SLE patients attending Spanish Internal Medicine Services since January 2009. We aimed to analyse drug therapy during the first year of follow-up according to disease severity.223 patients who had at least one year of follow-up were enrolled upon diagnosis of SLE. Therapy with prednisone, pulse methyl-prednisolone, hydroxychloroquine, immunosuppressives and calcium/vitamin D was analysed.Prednisone was given to 65% patients, at a mean (SD) daily dose of 11 (10) mg/d. 38% patients received average doses7.5 mg/d during the first year. Patients with nephritis and with a SLEDAI ≥6 were treated with higher doses of prednisone. 81% of patients were treated with hydroxychloroquine, with higher frequency among those with a SLEDAI ≥6 (88% vs. 68%, p0.001). The use of immunosuppressive drugs and methyl-prednisolone pulses was higher in patients with a baseline SLEDAI ≥6, however, differences were no longer significant when patients with lupus nephritis were excluded. The use of calcium/vitamin D increased with the dose of prednisone, however, 43% of patients on medium-high doses of prednisone did not take any calcium or vitamin D.This study gives a real-world view of the current therapeutic approach to early lupus in Spain. The generalised use of hydroxychloroquine is well consolidated. There is still a tendency to use prednisone at medium to high doses. Pulse methyl-prednisolone and immunosuppressive drugs were used in more severe cases, but not as steroid sparing agents. Vitamin D use was suboptimal.

Objective The objective of this paper is to examine if there is an association between low levels of 25-hydroxyvitamin D (25(OH)D) and insulin resistance (IR) in nondiabetic women with systemic lupus erythematosus (SLE) and to evaluate its impact on arterial stiffness. Patients and methods In this cross-sectional study 25(OH)D, insulin, insulin resistance measured by the homeostatic model assessment (HOMA-IR), homocysteine, fibrinogen, characteristics of SLE, medications and pulse-wave velocity (PWV) were measured in 106 nondiabetic women with SLE and 101 matched controls. Results Women with SLE tended to have lower 25(OH)D levels ( p = 0.078) and a higher frequency of 25(OH)D deficiency (defined as Conclusion Low 25(OH)D levels were found to be associated with increased IR in nondiabetic women with SLE independently of BMI. Low 25(OH)D levels, but not IR, could be associated with increased arterial stiffness in these patients.

To compare the presence of subclinical atherosclerosis measured by means of pulse wave velocity (PWV) in women with primary Sjögren's syndrome (SS) versus a healthy age- and sex-matched control group, and to identify factors independently associated with PWV in primary SS.Forty-four women with primary SS and 78 age-matched healthy women without overt cardiovascular (CV) diseases were assessed for traditional and nontraditional CV risk factors. PWV was also performed. A linear regression analysis was used to identify factors independently associated with PWV in primary SS.Women with primary SS had significantly higher PWV than controls (P = 0.030), and the frequency of increased PWV was significantly higher in this group (25% versus 8%; P = 0.013). The proportion of patients ages ≤50 years (ratio 4.6) with increased PWV was almost 2-fold higher than those ages50 years (ratio 2.4) with respect to controls. Positivity for anti-SSB was more frequent in patients with normal PWV than in those with increased PWV (61% versus 18%; P = 0.034). Women with primary SS and increased PWV had lower levels of 25-hydroxyvitamin D (25[OH]D; P = 0.047) than primary SS patients with normal PWV. In addition, 25(OH)D levels tended to correlate inversely with PWV in women with primary SS (P = 0.067), but not in controls (P = 0.97). In multivariate analysis, the Framingham Risk Score (FRS) and Sjögren's Syndrome Damage Index emerged as factors independently correlated with PWV.Women with primary SS had higher PWV than controls, but a similar FRS. The FRS and chronic damage were found to be independently associated with PWV.

A 70-year-old man, non-smoker, presented with bilateral digital necrosis, which had developed over the prior month, related to an aggressive Raynaud’s phenomenon of 2 months of evolution (Fig. 1a). The patient had no familial or personal history of previous thrombosis events, heart valve disease, livedo reticularis, nephropathy, thrombocytopenia or neurological manifestations. Upon examination, pulses were present, capillaroscopy was normal, and there was no sclerodactyly or skin sclerosis. Treatment with nifedipine, endovenous alprostidil, bosentan and analgesics was initiated with poor results. A complete blood count and biochemical tests, urinalysis, immunoglobulin, serum protein electrophoresis, cryoglobulin, serology for hepatitis B and C, antinuclear and antineutrophil cytoplasmic antibodies were either normal (the platelet count was 160,000 per mm) or negative. 18FFDG PET-CT did not show tumours or large vessel vasculitis. A biopsy of the ischemic areas of the hands only indicated epidermic necrosis. A thrombophilia workup showed the patient to be heterozygous for the C677T polymorphism of the MTHFR gene and for factor XII and test positively for IgG anti-b2-glycoprotein-1 antibodies in two separate determinations made 12 weeks apart (93.80 and 205 U/mL, respectively). Even though the genetic thrombophilia may have contributed to a procoagulant state, the severity of the lesions led us to consider the diagnosis of antiphospholipid syndrome (APS). Treatment with enoxaparin (60 mg/12 h) was started, and progressive improvements in the pain and reperfusion of the base of the fingers were observed (Fig. 1b). Cat scratch disease is an infectious disorder transmitted by cats that typically affects children and young adults. Immunosuppression is a well-known risk factor for the development of severe and atypical forms of the disease; hence it is under-diagnosed in patients with compromised immunity. We are reporting the first case of cat scratch disease, which presented as fever and fatigue, in a patient with systemic lupus erythematosus while receiving immunosuppressant therapy after a kidney transplant.

Homocysteine has been linked to atherosclerosis and hypertension (HT) in the general population. However, there is limited evidence regarding the effect of homocysteine on blood pressure and arterial stiffness in systemic lupus erythematosus (SLE). We examined whether homocysteine is associated with HT and arterial stiffness in women with SLE.In total, 99 women with SLE without a history of cardiovascular disease or diabetes mellitus and 101 matched controls were included in this cross-sectional study. Participants were analyzed for homocysteine levels, cardiovascular risk factors, and arterial stiffness assessed by means of carotid-femoral pulse wave velocity (PWV). Associations between homocysteine, systolic blood pressure (SBP), PWV, and HT were tested using univariate and multivariate analyses.Homocysteine levels (mean ± SD 12.3 ± 4.8 versus 9.3 ± 3.8 μmoles/liter), PWV (mean ± SD 7.54 ± 1.1 versus 7.10 ± 1.1 meters/second), SBP (mean ± SD 119 ± 13 versus 115 ± 12 mm Hg), and the prevalence of hyperhomocysteinemia (23% versus 7%) and HT (43% versus 12%) were significantly higher in women with SLE (P0.050 for all). In the univariate analysis, homocysteine correlated positively with SBP (P = 0.001) and PWV (P = 0.023) in women with SLE but not in controls. In the multiple linear regression analysis, SBP was independently associated with homocysteine and body mass index (BMI) in women with SLE. Similarly, in the multivariate logistic regression analysis, homocysteine levels (or hyperhomocysteinemia), BMI, and daily prednisone dose were independently associated with HT in women with SLE.Homocysteine was independently associated with SBP and HT in women with SLE, but not in controls. Elevated homocysteine levels could increase the risk of HT in SLE.

Systemic Lupus Erythematosus (SLE) is an auto-immune disease that seriously affects quality of life. There are various specific instruments that measure health-related quality of life (HRQOL), but none of them has been adapted to Spanish. We intended to adapt and validate in a Spanish population a specific HRQOL measure in patients with SLE.The adaptation was carried out using the translation and back-translation method of the English version of the LupusQoL, with its subsequent application to 115 adults with SLE.The factorial analysis identified 5 domains of the LupusQoL; the reliability tests showed a high Cronbach alpha coefficient of 0.977 and a high Guttman two-halves coefficient of 0.936; the significant correlations of the LupusQoL with the SF-36 showed a high convergent validity of the questionnaire; in addition, the correlations with the SLEDAI and SLICC were low, which showed its discriminated validity.The Spanish version of the LupusQoL has stable psychometric properties to measure HRQOL in people with SLE in clinical and research settings in a Spanish-speaking population.

Background: Chronic stress worsens the quality of life (QOL) of lupus patients by affecting their physical and psychological status. The effectiveness of a cognitive-behavioural intervention in a group of patients with lupus and high levels of daily stress was investigated. Methods: Forty-five patients with lupus and high levels of daily stress were randomly assigned to a control group (CG) or a therapy group (TG); they received cognitive behavioural therapy (CBT) which consisted of ten consecutive weekly sessions. The following variables were evaluated at baseline and at 3, 9 and 15 months: (1) stress, anxiety, depression, (2) Systemic Lupus Erythematosus Disease Activity Index, somatic symptoms, number of flares, (3) anti-nDNA antibodies, complement fractions C3 and C4 and (4) QOL. A multivariate analysis of repeated measures and various analyses of variance were carried out. Results: We found a significant reduction in the level of depression, anxiety and daily stress in the TG compared to the CG and a significant improvement in QOL and somatic symptoms in the TG throughout the entire follow-up period. We did not find any significant changes in the immunological parameters. Conclusions: CBT is effective in dealing with patients suffering from lupus and high levels of daily stress as it significantly reduces the incidence of psychological disorders associated with lupus and improves and maintains patients’ QOL, despite there being no significant reduction in the disease activity index.

JOURNAL ARTICLE; A 65-year-old woman with bullous pemphigoid presented with fever and several red-purple nodular subcutaneous lesions on both lower legs 1 week after starting treatment with azathioprine (AZA). Biopsy of a skin nodule was compatible with erythema nodosum (EN) and hypersensitivity reaction to AZA was suspected. AZA was subsequently discontinued, observing complete remission of fever and EN within 2 weeks. This case highlights the importance of recognizing EN as a possible manifestation of hypersensitivity reaction to AZA. Yes
