Introduction Acute kidney injury (AKI) has been identified as one of the most common and significant problems in hospitalized patients with COVID-19. However, studies examining the relationship between COVID-19 and AKI in low- and low-middle income countries (LLMIC) are lacking. Given that AKI is known to carry a higher mortality rate in these countries, it is important to understand differences in this population. Methods This prospective, observational study examines the AKI incidence and characteristics of 32,210 patients with COVID-19 from 49 countries across all income levels who were admitted to an intensive care unit during their hospital stay. Results Among patients with COVID-19 admitted to the intensive care unit (ICU), AKI incidence was highest in patients in LLMIC, followed by patients in upper-middle income countries (UMIC) and high-income countries (HIC) (53%, 38%, and 30%, respectively), whereas dialysis rates were lowest among patients with AKI from LLMIC and highest among those from HIC (27% vs. 45%). Patients with AKI in LLMIC had the largest proportion of community-acquired AKI (CA-AKI) and highest rate of in-hospital death (79% vs. 54% in HIC and 66% in UMIC). The association between AKI, being from LLMIC and in-hospital death persisted even after adjusting for disease severity. Conclusions AKI is a particularly devastating complication of COVID-19 among patients from poorer nations where the gaps in accessibility and quality of healthcare delivery have a major impact on patient outcomes.

O transtorno dismórfico corporal (TDC) é um transtorno psiquiátrico caracterizado pela preocupação excessiva e distorcida com a aparência física. Na especialidade da cirurgia plástica, é comum que pacientes busquem procedimentos estéticos para corrigir supostas imperfeições corporais decorrentes do TDC. O objetivo desta revisão sistemática é avaliar a prevalência do TDC em pacientes candidatos e/ou submetidos a procedimentos estéticos na especialidade da cirurgia plástica. Foram selecionados estudos que abordam a prevalência do TDC em pacientes submetidos a procedimentos estéticos, publicados entre 2010 e 2022. Os resultados indicam uma prevalência elevada de TDC em pacientes que buscam procedimentos estéticos na cirurgia plástica. A revisão também destaca a importância da avaliação psicológica pré-operatória e do tratamento adequado para pacientes com TDC ou outras condições mentais que possam interferir no resultado da cirurgia e na satisfação do paciente. Uma abordagem multidisciplinar envolvendo cirurgiões plásticos e profissionais de saúde mental é fundamental para garantir a segurança e a satisfação dos pacientes submetidos a procedimentos estéticos.

This study aims to analyze the level of knowledge of the on forms of transmission and prophylaxis of the main human verminosis among students of a public school. To evaluate the knowledge of the students, a questionnaire with closed questions was applied, adapting popular scientific language, involving aspects of etiology, evolutionary cycle, transmission, symptomatology, diagnosis and prophylaxis. The study included 185 high school students aged between 14 and 21 years, most of them female. Evaluating the questionnaire responses, it was that 54.59% reported knowing what verminosis are and most cited as better known to ascariasis, 70.81% stated that this disease is caused by worm. 56.75% confirmed that they have already participated in lectures on prevention of intestinal parasites and/or verminosis, 45.94% said their teachers work the content on intestinal parasites and/or verminosis. Most research participants cited abdominal pain, nausea and vomiting as majores suspicions of verminosis. 74.05% of students reported that verminosis are linked to a lack of sanitation coverage. In view of the data obtained in this research it was possible to realize that it is necessary to reformulate the teaching-learning process for what is taught at school students can apply in their daily lives thus promoting the improvement of public health.

OBJETIVO: O seguinte estudo objetivou descrever a semiologia do paciente portador de hemorragia digestiva alta, considerando como determinante na avaliação de potencias focos hemorrágicos. METODOLOGIA: Foram realizadas buscas nas plataformas do SciELO, LILACS, PubMed, Scopus e Google Scholar,utilizando os descritores gastrointestinal bleeding, peptic ulcerous disease e varicose hemorrhage, sendo identificados 35 estudos, dos quais foram incluídos 13 artigos completos. Desses estudos, 5 avaliaram as principais etiologias, 2 o surgimento de novos testes diagnósticos, 2 analisaram os aspectos epidemiológicos e 1 a sintomatologia apresentada pelo acometimento da hemorragia digestiva alta. Observou-se inicialmente a abundâncias de informações conceituais sobre o sangramento, como um transtorno clínico comum, acompanhada de inúmeras manifestações, considerando que o foco hemorrágico pode ocorrer em qualquer porção do trato gastrointestinal. Neste estudo, todas as publicações eleitas apresentaram o quadro semiológico composto por algia abdominal, indícios de choque hipovolêmico e taquicardia, alguns exibiram quedas abruptas da pressão arterial, odinofagia, êmese, náuseas e estado ictérico. Os pacientes implicados, cronicamente, já manifestaram ocorrências prévias, devido ao caráter recidivante torna-se essencial investigar a existência de varizes, fístula aorto-entérica, angiodisplasia e doença ulcerosa. CONCLUSÃO: Elucida-se que a hemorragia digestiva alta representa a principal causa de sangramento do trato gastrointestinal, majoritamente manifesta-se como hematêmese ou melena e cursam com o quadro sintomatológico que auxilia na avaliação da gravidade deste e o embasamento de potenciais focos de sangramento e que contribuam para disseminação de informações e intervenções futuras.

The present study provides evidence that senescence induces hyperreactivity of α1-adrenoceptor-mediated contraction of the common carotid. Impairment of α1-adrenoceptor responses is linked to increased Ca2+ influx and release of COX-derived vasoconstrictor prostanoids, contributing to carotid dysfunction in the murine model of female senescence (SAMP8). Increased reactivity of the common carotid artery during senescence may lead to morphological and functional changes in arteries of the cerebral microcirculation and contribute to cognitive decline in females. Because the elderly population is growing, elucidating the mechanisms of aging- and sex-associated vascular dysfunction is critical to better direct pharmacological and lifestyle interventions to prevent cardiovascular risk in both sexes.

Abstract: Drug delivery systems based on nanotechnology exhibit a number of advantages over traditional pharmacological formulations. Polymeric nanoparticles are commonly used as delivery systems and consist of synthetic or natural polymers that protect drugs from degradation in physiological environments. In this context, indolamine melatonin has been associated with several biological functions, including antioxidant, antitumor, immunoregulatory, neuroprotective, and cardioprotective effects. However, its availability, half-life, and absorption depend upon the route of administration, and this can limit its therapeutic potential. An alternative is the use of polymeric nanoparticle formulations associated with melatonin to increase its bioavailability and therapeutic dose at sites of interest. Thus, the objective of this review is to provide a general and concise approach to the therapeutic association between melatonin and polymeric nanoparticles applied to different biological disorders and to also highlight its advantages and potential applications compared to those of the typical drug formulations that are available.

This study investigates whether there are differences between the perceptions of owners and tenants in the urban context. Understanding this matter can influence decisions on housing policies, as it helps to comprehend the general satisfaction of subjects. Based on the questionnaire that was administered and on statistical analyses, we observed the importance of the maintenance and infrastructure of the urban context for both groups. The neighborhood's location, services, and resources discriminated between the groups, and highlighted the relevant characteristics for each target audience.

The selection of lima bean accessions in the fresh and dry stages is an excellent tool to increase crop yield in Brazil and ensure a source of vegetable protein for the population. Principal component analysis and nonparametric indices can be used to identify promising accessions based on the desired agronomic variables. The aim of the present study was to select accessions from traditional lima bean varieties in the fresh and dry stages using principal component analysis and nonparametric selection indices. The experiment consisted of a randomized block design, evaluating 13 treatments in four replicates. The experimental plot contained 20 plants in its study area. The first two principal components for the two stages explained more than 80% of the variation found among the accessions. The Mulamba & Mock and Genotype-Ideotype selection indices were efficient in classifying promising varieties for breeding programs. The UFPI 1111 accession can be used in fresh lima bean production, and its UFPI 1248 and 1294 counterparts in lima bean breeding programs. RESUMO A seleção de acessos de feijão-fava nos estádios verde e seco é uma excelente ferramenta para aumentar a produtividade da cultura no país e garantir uma fonte de proteína vegetal para a população. O uso de componentes principais e índices de seleção não-paramétricos podem ser utilizados para identificar acessos promissores com base em características agronômicas desejáveis. O objetivo do presente estudo foi selecionar acessos de variedades crioulas de feijão-fava nos estádios frescos e secos utilizando componentes principais e índices de seleção não paramétricos. O experimento foi disposto em delineamento em blocos casualizados, sendo avaliados 13 tratamentos em quatro repetições. A parcela experimental conteve 20 plantas em sua área útil. Os dois primeiros componentes principais, para os dois estádios, explicaram mais de 80% da variação encontrada entre os acessos. Os índices de seleção de Mulamba & Mock e Genótipo-Ideótipo foram eficientes na classificação de variedades promissoras para programas de melhoramento. O acesso UFPI 1111 pode ser direcionado para a produção de fava verde, e os acessos UFPI 1248 e UFPI 1294 podem ser utilizados para programas de melhoramento de fava seca.

12. Response of semi-arid vegetation to agricultural drought determined by indices derived from MODIS satellite

Indices based on data from remote sensors play an important role in the characterization, mapping and monitoring of a drought event. Thus, the objective of this study was to monitor the dynamics of agricultural drought, through the response of vegetation, in the semi-arid region that comprises the state of Paraíba, Brazil, through MODIS satellite products applied to the VCI (vegetation condition index), TCI (temperature condition index) and VHI (vegetation health index) indices, and to evaluate the correspondence of VHI index with standardized precipitation index (SPI) and agricultural standardized precipitation index (aSPI). In this study, values of NDVI (normalized difference vegetation index) and LST (land surface temperature) - covering the period between 2010 and 2020 - were used to estimate VCI, TCI and VHI. In addition, the correspondence of VHI with SPI and aSPI was evaluated at the 12-month time scale, conducted using Pearson’s correlation analysis. Characteristics of a stressed vegetation predominated in the study region, due to irregularity of precipitation and high temperatures, confirming the possibility of detection of droughts through VHI, VCI and TCI, and that the indices detected remotely and through local data are strongly correlated in drought detection. RESUMO Os índices baseados em dados de sensores remotos desempenham um papel importante na caracterização, mapeamento e monitoramento de uma seca. Com isso, o objetivo deste estudo foi monitorar a dinâmica da seca agrícola, por meio da resposta da vegetação, na região semiárida que compreende o estado da Paraíba, Brasil, por meio de produtos de satélite MODIS aplicados ao VCI (índice de condição da vegetação), TCI (índice de condição térmica) e VHI (índice de saúde da vegetação). E também, avaliar a correspondência do índice VHI com o índice de precipitação padronizada (SPI) e o índice de precipitação padronizada agrícola (aSPI). Neste trabalho, as climatologias de NDVI (índice de vegetação por diferença normalizada) e LST (temperatura da superfície terrestre) - abrangendo o período entre 2010 e 2020 - foram usadas para estimar VCI, TCI e VHI. Além disso, foi avaliado a correspondência do VHI e o índice padronizado de precipitação (SPI) e índice agrícola de precipitação (aSPI), na escala de tempo de 12 meses, conduzidas usando a análise de correlação de Pearson. Na região de estudo predominaram características de uma vegetação estressada, devido à irregularidade de precipitação e altas temperaturas. Confirmando a possibilidade da detecção das secas através do VHI, VCI e TCI. E que os índices detectados remotamente e através de dados locais estão fortemente correlacionados na detecção de seca.

Resumo Este trabalho traz reflexões a fim de compreender se existem diferenças entre as percepções de proprietários e inquilinos no contexto urbano. Tal entendimento pode influenciar na definição de políticas habitacionais e busca uma melhor compreensão sobre a satisfação dos indivíduos. A partir do questionário aplicado e de análises estatísticas, observou-se a importância da manutenção e infraestrutura do contexto urbano para ambos os grupos. Localização, serviços e recursos do bairro também se apresentaram como discriminantes entre os grupos, ressaltando as características relevantes para cada público-alvo.

center dot PURPOSE: To optimize artificial intelligence (AI) algo-rithms to integrate Scheimpflug-based corneal tomogra-phy and biomechanics to enhance ectasia detection.center dot DESIGN: Multicenter cross-sectional case-control ret-rospective study.center dot METHODS: A total of 3886 unoperated eyes from 3412 patients had Pentacam and Corvis ST (Oculus Op-tikgerate GmbH) examinations. The database included 1 eye randomly selected from 1680 normal patients (N) and from 1181 "bilateral" keratoconus (KC) patients, along with 551 normal topography eyes from patients with very asymmetric ectasia (VAE-NT), and their 474 unoperated ectatic (VAE-E) eyes. The current TBIv1 (tomographic-biomechanical index) was tested, and an optimized AI algorithm was developed for augmenting accuracy.center dot RESULTS: The area under the receiver operating char-acteristic curve (AUC) of the TBIv1 for discriminating clinical ectasia (KC and VAE-E) was 0.999 (98.5% sen-sitivity; 98.6% specificity [cutoff: 0.5]), and for VAE-NT, 0.899 (76% sensitivity; 89.1% specificity [cutoff: 0.29]). A novel random forest algorithm (TBIv2), devel-oped with 18 features in 156 trees using 10-fold cross -validation, had a significantly higher AUC (0.945; De -Long, P < .0001) for detecting VAE-NT (84.4% sen-sitivity and 90.1% specificity; cutoff: 0.43; DeLong, P < .0001) and a similar AUC for clinical ectasia (0.999; DeLong, P = .818; 98.7% sensitivity; 99.2% specificity [cutoff: 0.8]). Considering all cases, the TBIv2 had a higher AUC (0.985) than TBIv1 (0.974; DeLong, P < .0001).center dot CONCLUSIONS: AI optimization to integrate Scheimpflug-based corneal tomography and biome-chanical assessments augments accuracy for ectasia detection, characterizing ectasia susceptibility in the diverse VAE-NT group. Some patients with VAE may have true unilateral ectasia. Machine learning consider-ing additional data, including epithelial thickness or other parameters from multimodal refractive imaging, will con-tinuously enhance accuracy. Background: Examining the interrelationships between symptoms of depression and sociodemographic and behavioral correlates is challengeful using traditional regression analysis. Objective: to identify the sociodemographic, movement behaviors, and sleep correlates that contribute the most to symptoms of depression in Brazilian older females, using a network analysis approach. Methods: This cross-sectional study analyzed 1019 older females from Brazil. Data (sociodemographic, height (meters), weight (kilograms), symptoms of depression, physical activity, sleep) were self-reported via phone calls. The relationships between symptoms of depression and their correlates were assessed using the Network Analysis (qgraph package of the Rstudio) for entire sample and age groups (60-69; 70-79 and 80+ years old). Results: 60-69 and 70-79 groups have more weekly home exits, with aging “single, widowed or divorced” was progressively higher, and “married or stable union” and Overweight/obesity were progressively fewer (p < 0.05). 60-69 have more education years and fewest medicaments use. Sleep compliance (for the entire sample), body mass index (for the 60-69), compliance with moderate to vigorous physical activity (for the 70-79), and educational level (for 80+) were the variables with the highest expected influence values (p < 0.05) on symptoms of depression (1.370; 1.388; 1.129; and 1.354, respectively). Conclusion: Symptoms of depression vary throughout the aging process and thus determine that intervention strategies encompass these specific factors according to each age group. Poor sleep behavior has a strong positive association with symptoms of depression. This result highlights that health professionals must be aware of the importance of sleep to mitigate the worsening of depression among older Brazilian females.

Resumo Este artigo avalia o efeito do cultivo da soja sobre o nível de desenvolvimento socioeconômico e do crescimento econômico das AMC’s (Áreas Mínimas Comparáveis) brasileiras, entre os anos de 1991 e 2010. Para isso, foram estimados os modelos Diferenças em Diferenças com Escore de Propensão (DDM), avaliando o impacto de ser produtor de soja no Produto Interno Bruto (PIB) per capita e nas dimensões do Índice de Desenvolvimento Humano Municipal (IDH-M). Os resultados demonstraram que a produção de soja não impactou o IDH-M global, apresentando efeito positivo apenas no IDHM-conhecimento. No caso da dimensão longevidade, identificou-se um impacto negativo, possivelmente em decorrência do uso intensivo de agrotóxicos, que afeta a saúde dos trabalhadores rurais e moradores de localidades próximas. No caso do crescimento econômico, o efeito foi positivo, inferindo que “ser uma AMC produtora de soja” tende, na média, a intensificar o PIB per capita.

In the current context of emerging drug-resistant fungal pathogens such as Candida albicans and Candida parapsilosis, discovery of new antifungal agents is an urgent matter. This research aimed to evaluate the antifungal potential of 2-chloro-N-phenylacetamide against fluconazole-resistant clinical strains of C. albicans and C. parapsilosis. The antifungal activity of 2-chloro-N-phenylacetamide was evaluated in vitro by the determination of the minimum inhibitory concentration (MIC), minimum fungicidal concentration (MFC), inhibition of biofilm formation and its rupture, sorbitol and ergosterol assays, and association between this molecule and common antifungal drugs, amphotericin B and fluconazole. The test product inhibited all strains of C. albicans and C. parapsilosis, with a MIC ranging from 128 to 256 µg.mL-1, and a MFC of 512-1,024 µg.mL-1. It also inhibited up to 92% of biofilm formation and rupture of up to 87% of preformed biofilm. 2-chloro-N-phenylacetamide did not promote antifungal activity through binding to cellular membrane ergosterol nor it damages the fungal cell wall. Antagonism was observed when combining this substance with amphotericin B and fluconazole. The substance exhibited significant antifungal activity by inhibiting both planktonic cells and biofilm of fluconazole-resistant strains. Its combination with other antifungals should be avoided and its mechanism of action remains to be established. Resumo No atual contexto de patógenos fúngicos resistentes emergentes tais como Candida albicans e Candida parapsilosis, a descoberta de novos agentes antifúngicos é uma questão urgente. Esta pesquisa teve como objetivo avaliar o potencial antifúngico da 2-cloro-N-fenilacetamida contra cepas clínicas de C. albicans e C. parapsilosis resistentes a fluconazol. A atividade antifúngica da substância foi avaliada in vitro através da determinação da concentração inibitória mínima (CIM), concentração fungicida mínima (CFM), ruptura e inibição da formação de biofilme, ensaios de sorbitol e ergosterol, e associação entre esta molécula e antifúngicos comuns, anfotericina B e fluconazol. O produto teste inibiu todas as cepas de C. albicans e C. parapsilosis, com uma CIM variando de 128 a 256 µg.mL-1, e uma CFM de 512-1,024 µg.mL-1. Também inibiu até 92% da formação de biofilme e causou a ruptura de até 87% de biofilme pré-formado. A 2-cloro-N-fenilacetamida não promoveu atividade antifúngica pela ligação ao ergosterol da membrana celular fúngica, tampouco danificou a parede celular. Antagonismo foi observado ao combinar esta substância com anfotericina B e fluconazol. A substância exibiu atividade antifúngica significativa ao inibir tanto as células planctônicas quanto o biofilme das cepas resistentes ao fluconazol. Sua combinação com outros antifúngicos deve ser evitada e seu mecanismo de ação deve ser estabelecido.

22. EFICÁCIA DA IVERMECTINA E ATAZANAVIR NO TEMPO DE RECUPERAÇÃO DOS SINTOMAS DE COVID-19: UMA COORTE PROSPECTIVA

Objetivo: avaliar a eficácia da Ivermectina e do Atazanavir em comparação com placebo no tempo de resolução dos sintomas e no tempo de duração da doença por COVID-19. Método: estudo observacional, de coorte prospectivo, longitudinal, descritivo e analítico com pacientes sintomáticos ambulatoriais, acompanhados por 06 meses em duas Unidades Básicas de Saúde para atendimento de COVID-19 em Teresina-Piauí, Brasil, no período de novembro a abril de 2021 identificados por amostragem aleatória 1:1:1. Foram realizados exames Reverse transcription polymerase chain reaction (RT-PCR) para confirmação laboratorial da suspeita de infecção pelo novo coronavírus e avaliação sociodemográfica e clínica. Resultados: dos 87 pacientes randomizados, 62,1% (n=54) eram do sexo masculino, com média de idade de 35,1 anos, possuíam companheira (53,9%), baixa renda (50,6%), eutróficos (40,7%) e sem comorbidades de saúde (78,2%). Não houve diferença entre o tempo médio para resolução dos sintomas, que foi de 21 dias (IQR, 8-30) no grupo atazanavir, 30 dias (IQR, 5-90) no grupo ivermectina em comparação com 14 dias (IQR, 9-21) no grupo controle. No dia 180, houve resolução dos sintomas em 100% no grupo placebo, 93,9% no grupo atazanavir e 95% no grupo ivermectina. A duração mediana da doença foi de 08 dias em todos os braços do estudo. Conclusão: o tratamento com atazanavir (6 dias) e ivermectina (3 dias) não reduziu o tempo de resolução dos sintomas e nem o tempo de duração da doença entre os pacientes ambulatoriais com COVID-19 leve em comparação com o grupo placebo. Os resultados não suportam o uso de ivermectina e atazanavir para tratamento de COVID-19 leve a moderado.

The final stage of Fe deficiency is Fe deficiency anaemia, with repercussions for human health, especially in children under 5 years of age. Studies conducted in Brazilian public daycare centres show high prevalence of anaemia. The present study aims to evaluate the availability of Fe in the meals of the Municipal Centers of Early Childhood Education in Maceió. The experimental design comprises selection of algorithms, menu evaluation, calculation of the estimates, comparison between the estimates obtained and the recommendations, and analysis of correlation between meal constituents, and of the concordance between the absorbable Fe estimates. Four algorithms were selected and a monthly menu consisting of 22 d. The correlation analysis showed a moderate positive correlation to animal tissue (AT) v. non-heme iron (r = 0·42; P = 0·04), and negative to AT v. Ca (r = −0·54; P = 0·09) and Ca v. phytates (r = −0·46, P = 0·03). Estimates of absorbable Fe ranged from 0·23 to 0·44 mg/d. The amount of Fe available, unlike the total amount of Fe offered, does not meet the nutritional recommendations on most school days. The Bland–Altman analysis indicated that the Monsen and Balinfty and Rickard et al. showed greater agreement. The results confirm the need to adopt strategies to increase the availability of Fe in school meals.

O aumento da incidência de bactérias resistentes às múltiplas classes de antimicrobianos, principalmente Gram-negativas de prevalência nosocomial, têm se mostrado como grande ameaça à saúde pública internacional. O desenvolvimento de terapias alternativas que não induzem resistência bacteriana se mostra necessária e promissora. Este estudo objetivou avaliar a ação de extratos naturais derivados de plantas e terapia fotodinâmica no controle Pseudomonas aeruginosa e Acinetobacter baumannii cultivadas in vitro, através da determinação de concentrações inibitórias e bactericidas mínimas, e quantificação de unidades formadoras de colônias após exposição dos respectivos agentes, variando em tempo e concentração. Evidenciou-se ação dos extratos de limão taiti, tangerina e laranja amarga em diferentes concentrações. Neste estudo, a tangerina (Citrus tangerina) causou efeito inibitório e bactericida com menor quantidade em ambas as bactérias quando comparada aos outros extratos, porém, com maior concentração necessária na A. baumannii quando comparada à P. aeruginosa. Em relação à terapia fotodinâmica, foi evidenciada redução expressiva na quantidade de bactérias existentes no ambiente exposto, principalmente por maior quantidade de tempo, porem, sem garantir efeito inativação total de tais bactérias. A atividade antimicrobinada evidenciada no estudo, de ambas as terapias, demonstra a possibilidade do desenvolvimento de agentes adjuvantes no controle de replicação bacteriana com baixo custo e boa eficácia, sem risco de resistência bacteriana adicional.

Introdução: A obesidade é uma doença de etiologia multifatorial, se apresentando como uma das mais prevalentes síndromes do mundo. Tal condição pode ser definida como excesso de gordura corporal, mensurada pelo IMC. A moléstia pode apresentar diversas repercussões negativas no bem-estar do indivíduo, como doenças metabólicas e cardiovasculares, além de prejuízos sociais e psicológicos. Objetivo: O presente estudo teve como objetivo realizar uma revisão narrativa acerca das principais consequências da obesidade a longo prazo. Método: Trata-se de uma revisão narrativa com buscas na Scielo, Pubmed e Lilacs, no período de 2012 a 2022, utilizando os termos chave: “obesidade”, “consequências”, “evolução médica”, “obesity” e “consequences”. Resultados: Foram selecionados 10 artigos no total, sendo 4 na Scielo, 4 no Pubmed e 2 na Lilacs, por corresponderem ao objetivo do trabalho. Discussão: A principal consequência da obesidade se apresenta nos distúrbios metabólicos, com destaque para a Diabetes tipo 2, devido ao aumento da inflamação e, portanto, da resistência à insulina. Outros impactos repercutem na osteoartrite, diversos tipos de câncer, doença renal crônica e condições que podem levar a pessoa acometida a óbito. Indivíduos obesos estão vulneráveis a questões sociais, como isolamento e discriminação e podem, portanto, desenvolver doenças psicológicas. Conclusão: Conclui-se que a obesidade deve ser tratada de forma multidisciplinar, sugerindo novos estudos relacionados aos prejuízos laborais sofridos pelos trabalhadores obesos.

However, the cerebello-subcortical circuitry in FTD has been understudied despite its essential role in cognition and behaviors related to FTD symptomatology. The present study aims to investigate the association between cerebellar and subcortical atrophy, and neuropsychiatric symptoms across genetic mutations. Our study included 983 participants from the Genetic Frontotemporal dementia Initiative including mutation carriers and noncarrier first-degree relatives of known symptomatic carriers. Voxel-wise analysis of the thalamus, striatum, globus pallidus, amygdala, and the cerebellum was performed, and partial least squares analyses (PLS) were used to link morphometry and behavior. In presymptomatic C9orf72 expansion carriers, thalamic atrophy was found compared to noncarriers, suggesting the importance of this structure in FTD prodromes. PLS analyses demonstrated that the cerebello-subcortical circuitry is related to neuropsychiatric symptoms, with significant overlap in brain/behavior patterns, but also specificity for each genetic mutation group. The largest differences were in the cerebellar atrophy (larger extent in C9orf72 expansion group) and more prominent amygdalar volume reduction in the MAPT group. Brain scores in the C9orf72 expansion carriers and MAPT carriers demonstrated covariation patterns concordant with atrophy patterns detectable up to 20 years before expected symptom onset. Overall, these results demonstrated the important role of the subcortical structures in genetic FTD symptom expression, particularly the cerebellum in C9orf72 and the amygdala in MAPT carriers.

A vasectomia é um método de planejamento familiar permanente, popular, bastante eficaz e de custo relativamente baixo. A vasectomia é um método de planejamento familiar de custo relativamente baixo, sem grandes riscos para a vida e saúde do paciente, portanto torna-se com o tempo, muito procurada para evitar concepção. Este trabalho tem como objetivo geral realizar uma revisão sistemática em pesquisas de 2011 a 2022, sobre casos de falha na vasectomia provocada pela recanalização natural do paciente gerando assim filhos biológicos comprovados pelo exame comparativo do material genético dos pais. Tendo como objetivos específicos: a) Identificar quais artigos abordam diretamente sobre a falha da vasectomia entre 2011 a 2202; b) Analisar se existem casos de incidências e similaridade entre os artigos abordados; c) Descrever os principais achados das pesquisas selecionadas comparando-as. O método da pesquisa escolhido é uma revisão de literatura sistemática de caráter misto, foi utilizada como para analisar a qualidade dos dados encontrados a ferramenta Mixed “Methods Appraisal Too ” (MMAT), desenvolvida por Hong et al. (2018) descritivo e do tipo estudo de caso. Os artigos foram identificados a partir dos descritores: Vasectomia. Planejamento familiar. Recanalização.

- Abstract
A Justiça Restaurativa foi introduzida no Poder Judiciário Nacional pelo Conselho Nacional de Justiça por meio da Resolução nº 225 de 31 de maio de 2016 como política pública de acesso à justiça. Desde então, vem sendo implementada em diversos órgãos judiciários como uma abordagem de resolução e transformação de conflitos e violências de forma a criar um espaço onde seus participantes encontrem formas de atender suas necessidades e que os traumas causados por um crime sejam restaurados na mediada do possível. Propõe-se nesse estudo abordar a Justiça Restaurativa, que visa trabalhar com os envolvidos afetados por um crime, a reparação de danos, a responsabilização e a prevenção de novos conflitos através de metodologias de intervenção permeadas pela escuta empática e a garantia de um espaço seguro que possibilite a fala aos participantes, e os auxiliem a expressarem seus sentimentos e atenderem suas necessidades, cumpre verificar suas principais metodologias e objetivos de suas intervenções. A pesquisa é bibliográfica e descritiva, e utiliza o método dedutivo na compreensão da temática proposta. Busca-se responder a seguinte questão: a Justiça Restaurativa tem cumprido seu objetivo de promoção do acesso à Justiça? Como marco teórico apresenta-se a abordagem de Cappelletti e Garth na evolução do conceito de acesso à Justiça e Howard Zehr na compreensão da Justiça Restaurativa e seus princípios norteadores. Constatou-se que a abordagem vem sendo implementada no contexto do judiciário brasileiro, enquanto política pública de acesso à Justiça e propõe tratar os conflitos e a violência numa ótica democrática e consensual, através do diálogo e da participação, almejando-se uma cultura pautada na pacificação social com o uso das metodologias dos círculos de construção de paz e os encontros restaurativos. Verificou-se ainda, que a implementação da referida Política por meio de projetos, programas e ações apresenta-se em expansão, consolidando-se como meio de acessar uma ordem jurídica justa e efetiva na área criminal, com programas já implantados em vários estados, possibilitando a aproximação da comunidade de um sistema de justiça simples, consensual e democrático quanto a participação e acesso efetivo do cidadão, na garantia de seus direitos e exercício da cidadania, enfatiza-se ainda a necessidade de aperfeiçoamento dos programas com o desenvolvimento de avaliação, monitoramento e adequações que garantam o aperfeiçoamento e efetividade da Política de Justiça Restaurativa.

Possíveis interações medicamentosas devem ser levadas em consideração ao selecionar um regime antirretroviral. Uma revisão detalhada dos medicamentos concomitantes pode ajudar na criação de um regime que minimize as interações indesejáveis. O potencial para interações medicamentosas deve ser avaliado quando qualquer novo medicamento (incluindo agentes de venda livre) é adicionado a um regime antirretroviral existente. A maioria das interações medicamentosas com medicamentos antirretroviral é mediada por inibição ou indução do metabolismo hepático de medicamentos. Este estudo trata-se de uma revisão integrativa, cujo objetivo foi compreender as possíveis interações de drogas em pacientes com infecção pelo HIV em processo de terapia antirretroviral. Após análise dos dados, concluiu-se que há riscos reais de interações medicamentosas a partir do uso de 5 ou mais medicamentos, por um tempo superior a seis anos. Os principais riscos apontados nesse sentido foram interferência na resposta terapêutica, aumento de reações adversas toxidade nos sistemas cardiovascular e nervoso central e dificuldades para detecção de resistência do HIV aos medicamentos antirretrovirais.

Primary gastrointestinal melanomas (PGM) are usually asymptomatic in the early stages, leading to misdiagnosis. We reported the case of a 75-year-old male patient admitted to our service due to hematochezia, and fatigue. Primary investigation showed small bowel intussusception and surgical treatment was performed. Histopathology and immunohistochemistry analysis identified primary small bowel amelanotic melanoma at an advanced tumor stage. Whereas the patient had a sudden impairment of performance status during hospitalization, no curative oncological approach could be performed. This report highlights the clinical and imaging findings that led to the primary gastrointestinal amelanotic melanoma diagnosis in an elderly patient with intestinal intussusception. The early recognition of prior intussusception signs in adults and elder patients may lead to an earlier oncological diagnosis and a better prognosis.

Objetivo: Analisar as evidências científicas acerca da visita familiar ampliada nas unidades de terapia intensiva adulto. Métodos: Revisão integrativa realizada em julho de 2021. Utilizaram-se os descritores “terapia intensiva”, “humanização” e “visitas a pacientes”. Distribuídos na Base de Dados de Enfermagem, Scientific Electronic Library Online, Medical Literature Analysis and Retrieval System Online, Literatura Latino-Americana e do Caribe em Ciências da Saúde; disponíveis nos idiomas inglês, português ou espanhol, entre 2016 e julho de 2021, obtendo um total de nove artigos viáveis que se adequaram ao objetivo da pesquisa. Resultados: Diante da análise dos estudos encontrados, identificou-se que a visita ampliada nas unidades de terapia intensiva reduziu fatores de ansiedade e estresse por parte dos familiares, sem que houvesse aumento dos índices de infecção; porém não obteve impacto significativo em eventos como o delirium. Conclusão: A presença de familiares na unidade de terapia intensiva permite uma evolução na condição psicossocial, na redução de estresse e ansiedade frente a doença. No entanto, os benefícios para a condição clínica são poucos, sendo necessários mais estudos que observem o fenômeno.

We use the surface detector of the Pierre Auger Observatory to search for air showers initiated by photons with an energy above 1019 eV. Photons in the zenith angle range from 30∘ to 60∘ can be identified in the overwhelming background of showers initiated by charged cosmic rays through the broader time structure of the signals induced in the water-Cherenkov detectors of the array and the steeper lateral distribution of shower particles reaching ground. Applying the search method to data collected between January 2004 and June 2020, upper limits at 95% CL are set to an E-2 diffuse flux of ultra-high energy photons above 1019 eV, 2 × 1019 eV and 4 × 1019 eV amounting to 2.11 × 10-3, 3.12 × 10-4 and 1.72 × 10-4 km-2 sr-1 yr-1, respectively. While the sensitivity of the present search around 2 × 1019 eV approaches expectations of cosmogenic photon fluxes in the case of a pure-proton composition, it is one order of magnitude above those from more realistic mixed-composition models. The inferred limits have also implications for the search of super-heavy dark matter that are discussed and illustrated.

Resumo O artigo analisa a comunicação da imagem da capa da Política Nacional de Saúde Integral de Lésbicas, Gays, Bissexuais, Travestis e Transexuais. Trata-se de estudo qualitativo-descritivo com análise de imagem conduzida à luz dos conceitos de Judith Butler. O corpus textual tinha 13 textos, seis figuras e o plano de fundo. As principais cores utilizadas foram o preto e o azul. As figuras humanas que performaram o gênero masculino se sobrepõe àquelas do gênero feminino. Os textos apenas situam o teor do documento enquanto política pública. O princípio do conhecimento difundido foi identidade de gênero, sendo este termo identificado com maior frequência. A capa do documento reforça estereótipos ao representar performances de gênero tradicionais de forma fixa e desconsiderar o uso político das cores do movimento a que se propõe representar.

Resumo O objetivo deste artigo é avaliar a prevalência e fatores associados aos Transtornos Mentais Comuns (TMC) em quilombolas residentes em comunidades rurais localizadas no norte de Minas Gerais, Brasil. Trata-se de estudo transversal realizado em 2019 a partir de entrevistas estruturadas e aplicação do Self Reporting Questionnaire (SRQ-20). Foi conduzida regressão logística binária. A prevalência de TMC foi de 38,7%. Houve maior chance de TMC entre quilombolas do sexo feminino (OR: 2,69; IC95% 2,00-3,62), com 1 a 8 anos de estudo (OR: 1,70; IC95% 1,15-2,51), renda familiar entre 1 a 1,5 salário mínimo (OR: 2,51; IC95% 1,60-3,94); que sofreram discriminação em serviços de saúde (OR: 2,44; IC95% 1,44-4,13); com autorrelato de doença pulmonar (OR: 2,10; IC95% 1,25-3,54), doença cardíaca (OR: 1,58; IC95% 1,01-2,50) e insuficiência renal crônica (OR: 1,97; IC95% 1,08-3,94), e com autopercepção de saúde negativa (OR: 3,07; IC95% 2,31-4,07). A alta prevalência de TMC observada neste estudo demonstra a necessidade de políticas de atenção à saúde mental voltadas para as populações vulnerabilizadas, como as comunidades quilombolas, a fim de mitigar o sofrimento mental e favorecer uma atenção profissional contextualizada com as singularidades das condições de vida e saúde desse grupo populacional.

45. Angiotensin II–Mediated Neuroinflammation in the Hippocampus Contributes to Neuronal Deficits and Cognitive Impairment in Heart Failure Rats

Background: Heart failure (HF) is a debilitating disease affecting >64 million people worldwide. In addition to impaired cardiovascular performance and associated systemic complications, most patients with HF suffer from depression and substantial cognitive decline. Although neuroinflammation and brain hypoperfusion occur in humans and rodents with HF, the underlying neuronal substrates, mechanisms, and their relative contribution to cognitive deficits in HF remains unknown. Methods: To address this critical gap in our knowledge, we used a well-established HF rat model that mimics clinical outcomes observed in the human population, along with a multidisciplinary approach combining behavioral, electrophysiological, neuroanatomical, molecular and systemic physiological approaches. Results: Our studies support neuroinflammation, hypoperfusion/hypoxia, and neuronal deficits in the hippocampus of HF rats, which correlated with the progression and severity of the disease. An increased expression of AT1aRs (Ang II [angiotensin II] receptor type 1a) in hippocampal microglia preceded the onset of neuroinflammation. Importantly, blockade of AT1Rs with a clinically used therapeutic drug (Losartan), and delivered in a clinically relevant manner, efficiently reversed neuroinflammatory end points (but not hypoxia ones), resulting in turn in improved cognitive performance in HF rats. Finally, we show than circulating Ang II can leak and access the hippocampal parenchyma in HF rats, constituting a possible source of Ang II initiating the neuroinflammatory signaling cascade in HF. Conclusions: In this study, we identified a neuronal substrate (hippocampus), a mechanism (Ang II–driven neuroinflammation) and a potential neuroprotective therapeutic target (AT1aRs) for the treatment of cognitive deficits in HF.

The application of silicon (Si) represents one of the alternatives that can be used to mitigate the deleterious effects caused by salinity on plants in fertigated cultivation. The objective was to evaluate the effect of different levels of water salinity, associated with the use of sodium silicate on the production parameters and nutrient levels in the leaf and stem of mini watermelon in a protected environment. To conduct the experiment, a randomized block design was adopted, in a 6 x 2 factorial scheme, with 4 repetitions, with six levels of irrigation water salinity (1.05; 2.12; 3.26; 4.41; 5.91 and 7.32 dS m-1) and two types of Si application: without Si and in foliar solution. The following parameters were evaluated: plant height, stem diameter, number of leaves, leaf and stem fresh and dry mass; biometric variables and total soluble solids content in the fruits; and nutrient contents in leaves and stem. The use of Si associated with fertigation with different salinity levels promoted a positive response for peel thickness, ºBrix, fresh and dry mass of leaves, and Si accumulation in leaves and stem. Salinity alone promoted an increase in pulp weight, ºBrix of the fruits and potassium content in the stem, besides increasing the manganese content and inhibiting the zinc content in the leaves. There was a significant interaction for the manganese content in the stem, with a positive response in the absence of foliar-applied Si and a negative response in the presence of foliar-applied Si under different levels of salinity. RESUMO A aplicação de silício (Si) representa uma das alternativas que podem ser utilizadas para mitigar os efeitos deletérios às plantas pela salinidade em cultivo fertirrigado. Objetivou-se avaliar o efeito de diferentes níveis de salinidade da água, associado ao uso de silicato de sódio sobre os parâmetros produtivos e os teores de nutrientes na folha e no caule da mini melancia em ambiente protegido. Para condução do experimento adotou-se delineamento em blocos casualizados, em esquema fatorial 6 x 2, com 4 repetições, sendo seis níveis de salinidade da água de irrigação (1,05; 2,12; 3,26; 4,41; 5,91 e 7,32 dS m-1) e dois tipos de aplicações de silício: sem silício e aplicação em solução via foliar. Foram avaliados: a altura das plantas, diâmetro do caule, número de folhas, peso da matéria fresca e seca de folhas e caule; variáveis biométricas e teor de sólidos solúveis totais nos frutos; e teor de nutrientes nas folhas e caule. O uso do silício associado a fertirrigação com diferentes níveis de salinidade proporcionaram resposta positiva para espessura de casca, ºBrix, massa fresca e seca de folhas e, acúmulo de silício nas folhas e caule. A salinidade isolada proporcionou aumento para peso de polpa, ºBrix dos frutos e teor de potássio no caule e, incrementou o teor de manganês e inibiu teor de zinco nas folhas. Houve interação significativa para o teor de manganês no caule, com resposta positiva na ausência de silício foliar e negativa com aplicação foliar de silício sob diferentes níveis de salinidade.
