Background: An accurate medication history is essential to ensure continuity of medication therapy when patients are admitted to hospital. To implement a ressource efficient process guaranteeing at the same time a high-quality medication history, one has to consider local circumstances such as patient population, staffing and IT infrastructure. Inclusion criteria: Studies reporting on quantitative outcomes of medication history taking at the time of admission to hospital and studies assessing its quality or efficiency were included. Objective: We aim to provide an overview of existing concepts of medication history taking at hospital admission and their performance in different hospital settings as well as to identify gaps in research in this area. Methods: A literature search of studies assessing concepts of medication history taking was conducted. We extracted data on the concepts’ components, characteristics of the setting and patient populations as well as reported quality and efficiecy outcomes.
