2. Factors Associated with Non-Binary Gender Identity in Psychiatric Inpatients with Suicidal Ideation Assigned Female at Birth: A Case-Control Study

The study aimed to investigate factors associated with non-binary gender identity in Russian female psychiatric inpatients with suicidal ideation. This case-control study included 38 female inpatients with non-binary gender identity and a control group-76 cisgender women matched for age (age range 19-35 years, M age, 21.5 years); both groups were psychiatric inpatients with suicidal thoughts. All patients underwent the Self-Injurious Thoughts and Behaviors Interview and completed the brief Reasons for Living Inventory. We also used the WHO Quality of Life Questionnaire (WHOQOL-100) and the Life Style Index (LSI). Non-binary gender identity in inpatients with suicidal ideation was associated with lower educational level, higher unemployment rate, being more socially reticent in preschool, and lifetime sexual experience with both male and female partners. In addition, they were younger at the time of the first suicidal ideation, suicide plan development, and attempt. Non-binary inpatients had lower scores in freedom, physical safety, and security facets of WHOQOL-100 and a higher level of intellectualization on LSI. People with non-binary gender identity face educational, employment, and communication issues. They also have distinct suicidal thoughts and behavioral profiles. These issues and differences mean unique approaches to suicide prevention for a population of inpatients with non-binary gender identity are needed.

aims: To investigate the level of anxiety as well as attitude toward disease and mitigation strategies in people with non-psychotic mental disorders and healthy controls. background: The new coronavirus disease (COVID-19) pandemic resulted in an unprecedented morbidity and mortality and economic consequences worldwide. Response to pandemic required an urgent and radical change in the medical care delivery with a focus on infectious disease, which is uncommon for most developed countries. Implemented changes led to reduced psychiatric (especially inpatient) care utilization. objective: To investigate the level of anxiety as well as attitude toward disease and mitigation strategies in people with non-psychotic mental disorders and healthy controls. method: A case-control study (1:4) was conducted during a self-isolation regimen in Moscow. Cases were adult patients with NPMD consequently admitted to the Moscow Research and Clinical Center for Neuropsychiatry. Sex and age-matched controls (individuals who have never sought psychiatric help) were selected from a cohort of 7498 Moscow residents who participated in a largecross-sectional survey delivered via social networks and popular Russian web platforms. result: The level of the trait but not state anxiety was higher in people with NPMD. People with mental disorders were significantly more often concerned about the negative impact of the COVID-19 pandemic on their lives, were more likely to be confident that they would contract the coronavirus and their disease would be severe, showed suboptimal adherence to mitigation strategies, were dissatisfied with the amount of information they received about the COVID-19 and were more concerned about becoming a victim of domestic violence. conclusion: People with NPMD have higher level of trait anxiety and are prompted to develop the constellation of maladaptive beliefs toward a pandemic. Objective Interictal dysphoric disorder (IDD) has been regarded as an affective disorder occurring only in people with epilepsy (PWE). Data showing similar characteristics and similar prevalence of IDD in patients with migraine and with psychogenic nonepileptic seizures question the epilepsy-specific nature of IDD. The aim of the study was to investigate the nature of IDD in people with prevalent epilepsy with mood disorders and people with mood disorders who are free of neurological disease. Methods This is a case-control study, with 142 patients with a confirmed diagnosis of epilepsy and major depressive disorder (MDD; cases) and 222 patients with MDD only (controls). MDD diagnosis was confirmed by a structured clinical interview for Diagnostic and Statistical Manual of Mental Disorders, 4th edition (SCID-I-RV). We used the Beck Depression Inventory and the Beck Anxiety Inventory to estimate anxiety and depression levels and the Interictal Dysphoric Disorder Inventory (IDDI) to confirm the presence of IDD. Mann-Whitney U test, Pearson chi-squared, Spearman correlation, and logistic regression were used. Results No differences were found in the prevalence of IDD between PWE with MDD and people with MDD alone (88.73% vs. 85.13%, χ2 = .96, p = .32). There were no differences between the groups overall or for any IDDI subscales (all p > .05). In both groups, IDD symptoms were grouped with the same incidence and had the same duration and periodicity. IDD was not associated with epilepsy (odds ratio = .84, 95% confidence interval = .40-1.98, p = .72). No significant correlation was found between epilepsy, demographic characteristics, and all IDDI subscales (all p > .05). 5. Neurological Events Reported after COVID-19 Vaccines: An Analysis of VAERS

To identify the rates of neurological events following administration of mRNA (Pfizer, Moderna) or adenovirus vector (Janssen) vaccines in the U.S..We utilized publicly available data from the U.S. Vaccine Adverse Event Reporting System (VAERS) collected between January 1, 2021-June 14, 2021. All free text symptoms that were reported within 42 days of vaccine administration were manually reviewed and grouped into 36 individual neurological diagnostic categories. Post-vaccination neurological event rates were compared between vaccine types and to age-matched baseline incidence rates in the U.S. and rates of neurological events following COVID.Of 306,907,697 COVID vaccine doses administered during the study timeframe, 314,610 (0.1%) people reported any adverse event and 105,214 (0.03%) reported neurological adverse events in a median of 1 day (IQR0-3) from inoculation. Guillain-Barre Syndrome (GBS), and cerebral venous thrombosis (CVT) occurred in fewer than 1 per 1,000,000 doses. Significantly more neurological adverse events were reported following Janssen (Ad26.COV2.S) vaccination compared to either Pfizer-BioNtech (BNT162b2) or Moderna (mRNA-1273; 0.15% versus 0.03% versus 0.03% of doses, respectively,P0.0001). The observed-to-expected ratios for GBS, CVT and seizure following Janssen vaccination were ≥1.5-fold higher than background rates. However, the rate of neurological events after acute SARS-CoV-2 infection was up to 617-fold higher than after COVID vaccination.Reports of serious neurological events following COVID vaccination are rare. GBS, CVT and seizure may occur at higher than background rates following Janssen vaccination. Objective To assess the capacity of Generalized Anxiety Disorder-7 (GAD-7) to detect anxiety disorders in a Russian sample of patients with epilepsy and to validate this instrument for rapid screening of anxiety in these patients. Methods Study included 233 patients with epilepsy, both inpatients and outpatients. For all patients Mini-International Neuropsychiatric Interview was conducted as a gold standard for diagnosis of mental disorders. All patients also completed the questionnaires – the Russian version of GAD-7 and Hospital Anxiety and Depression Scale (HADS) to assess convergent validity. Chi-square and Fisher's exact tests were used to compare categorical variables, and the Mann–Whitney test was used for the quantitative ones. Internal consistency was assessed using Cronbach's alpha, Cronbach's alpha at point deletion, and corrected point-to-point correlation. ROC analysis was used to evaluate the properties of the GAD-7 to determine anxiety disorders. Results Among 97 (41.6%) patients with epilepsy diagnosed with any anxiety disorders, 42 (18%) had panic disorder, 37 (15.9%) had agoraphobia, 17 (7.3%) had social anxiety disorder, and 64 (27.5%) had generalized anxiety disorder; 42 patients (18%) showed a combination of several anxiety disorders. The overall GAD-7 score was similar to other epilepsy studies, but higher cutoff scores characterize our sample. The scale performed well in detecting any anxiety disorder with the AUC of 0.866 and the optimal cutoff point > 8 points, and in detecting GAD with AUC = 0.922 and the optimal cutoff point > 9 points, showing overall acceptable sensitivity. Conclusion Russian version of the GAD-7 could be used as a screening tool for any anxiety disorders in PWE with the optimal cutoff score > 8 points.

Introduction Lower quality of life (QoL) scores are associated with suicidal behavior, both in the general population and in psychiatric patients. Nonsuicidal self-injury (NSSI) behavior is a public health concern because of its increasing prevalence and high risk of lifetime suicide attempt. Despite its significance QoL in patients with NSSI is underinvestigated. There is a gap in knowledge on the influence of suicidal ideation (SI) on certain domains and facets of QoL in patients with NSSI . Objectives The study aimed to assess the impact of SI on QoL of patients with NSSI. Methods We conducted a case-control study (1:3): 13 consecutive patients (11 female) with non-psychotic mental disorders and NSSI without lifetime SI were compared to 39 age and gender matched patients with NSSI and SI. All patients were evaluated by a psychiatrist, underwent Self-Injurious Thoughts and Behaviors Interview (Nock MK et al.,2007) and filled out the World Health Organization Quality of Life Assessment100(WHOQOL-100). Mann-Whitney and Fishers exact test were used as statistical methods. Results The overall QoL (p=0.001) and the perception of life (p=0.005) were significantly higher in patients without SI. 8. Tobacco smoking in non-psychotic patients with suicidal ideation

Recent studies have found a higher incidence of suicidal ideation/plan/attempt and non-suicidal self-injurious behavior among smokers (Poorolajal et al., 2016; Marin et al., 2020). People with mental health problems have been shown to be one of the most smoking populations (Szatkowski et al, 2015). At the same time, the presence of a mental disorder is an independent predictor of the occurrence of suicidal ideation (Klonsky et al, 2017). An important question about the impact of nicotine consumption on the course of self-Injurious thoughts and behaviors in persons with non-psychotic mental disorders remains unclear. The aim of the study is to evaluate the influence of tobacco smoking (TS) on the risk of lifetime suicidal plan (SP), attempt (SA) and nonsuicidal self-injury (NSSI) in patients with nonpsychotic mental disorders (NPMD) and suicidal ideation (SI). Four hundred and 78 consecutive patients with NPMD and SI were included. All patients were evaluated by a psychiatrist, underwent Self-Injurious Thoughts and Behaviors Interview (SITBI) as well as semi-structured interview designed to gather information on demographic and biographical features. Mann-Whitney, chi-square test and stepwise logistic regression were used as statistical methods. The mean age of the sample was 28 (11.46) and most of the patients were female 417 (97.2%). Lifetime TS was reported by 324 (67.8%) patients. We found no differences between smokers and non-smokers in age, gender, educational and occupational statuses, as well as the diagnoses of mental disorders and their number (Tab.1). SP, SA and NSSI were significantly more common in patients with lifetime tobacco smoking history. No significant differences were found in age of onset of self-injurious thoughts and behavior, and total number of SP, SA and NSSI between lifetime smokers and non-smokers (Tab. 2) Stepwise logistic regression analysis revealed an association of lifetime TS with lifetime SA and NSSI, but not with SP (Tab: 3). In consistence with previous studies (Sankaranarayanan et al., 2015; Marin et al., 2020), our results suggested that lifetime tobacco smoking is strongly associated with an increased risk of suicidal and non-suicidal self-injurious behavior. Moreover, we showed suggested that tobacco smoking involved in transition from ideation to action. It is proposed that smoking may increase the risk of suicide through a biological pathway - decreasing the activity of the serotonergic system of hippocampus, reduce brain serotonin function and activate of the hypothalamo-pituitary-adrenal axis (HPA). Thus, nicotine may be able to activate the attenuated responsiveness of the HPA axis to psychological stress (Poorolajal et al, 2016). History of lifetime TS in patients with NPMD and SI is associated with an increased risk of suicidal and nonsuicidal self-injurious behavior.

Nonsuicidal self-injury (NSSI) is recognized as a public health concern for its association with unfavorable outcomes, including suicidal behavior. The aim of this study is to identify factors associated with NSSI among patients with nonpsychotic mental disorders (NPMD) and suicidal ideation in Russia.A retrospective cohort study was conducted in the Moscow Research and Clinical Center for Neuropsychiatry between November 2017 and May 2019. The sample was composed of consecutive patients with lifetime suicidal ideation (from the Self-Injurious Thoughts and Behavior Interview) seen in the center's psychiatric ward for NPMD. The patients were divided into two groups: those with and without lifetime NSSI. Sociodemographic variables, psychiatric diagnosis, family history of mental disorders, history of physical or sexual abuse, sexual behavior, ad-hoc psychiatric treatments, suicidal ideation, plans, and gestures or attempts were investigated.Six thousand, two hundred and four consecutive patients were screened for suicidal ideation. Out of a total of 361 patients (87.3% females) with suicidal ideation, 217 (60.1%) reported NSSI. To translate and validate the English version of the Neurologic Depression Disorders Inventory in Epilepsy (NDDI-E) into the Russian language as an instrument for rapid detection of major depressive episodes (MDE) for patients with epilepsy (PWE) from Russian Federation.One hundred and 75 consecutive PWE were included in the study. All patients were assessed with Mini-International Neuropsychiatric Interview (MINI 6.0.0), Hospital Anxiety and Depression Scale (HADS) and the Russian version of NDDI-E. Chi-square, Fisher's exact and Mann-Whitney tests were used to compare PWE with and without MDE. We analyzed internal structural validity, external validity, and receiver operator characteristics.None of the participants had any difficulties in understanding the questions of NDDI-E. The internal consistency of the inventory was satisfactory (Cronbach's ά = 0.856). Correlation between the NDDI-E and the HADS scores was moderate (r = 0.64, P 0.001), indicating acceptable external validity. NDDI showed good capacity to detect MDE, with area under the curve of 0.919 (95% CI = 0.868-0.955; standard error: 0.019; P 0.001). An optimal cut-off point with the highest Yuden's index (J = 0.699) was 12. At this point NDDI-E showed sensitivity of 88.16% (95% CI = 78.7%-94.4%), specificity of 81.82% (95% CI = 72.8%-88.9%), positive predictive value of 59.3% (95% CI = 48.8%-69.0%), negative predictive value of 95.8% (95% CI = 92.5%-97.7%).Russian version of NDDI-E is an affordable and fast screening tool with a good combination of sensitivity and specificity.

The current study examined the validity of conversational analysis (CA) in Russian patients with seizures, using a scoring table for the Simplified Linguistic Evaluation (SLE). The study sample was composed of 12 adult participants suffering either from epilepsy (ES) or psychogenic nonepileptic seizures (PNES) recruited in the Moscow Research and Clinical Center for Neuropsychiatry. Definitive diagnosis was established only after a habitual event was captured onvEEG. All participants with PNES or ES and at least one mental disorder underwent a 20-minute-long interview recorded on video.
