Echocardiography is the cornerstone imaging technique in the diagnosis of infective endocarditis (IE) but is often misused in clinical practice. Recently, strict negative criteria have been proposed to avoid unnecessary follow-up echocardiograms. We aimed to evaluate the use of echocardiography in real-world clinical daily practice and the usefulness of these criteria in the diagnosis of IE. We retrospectively retrieved every echocardiogram performed in our center for suspected IE between 2014 and 2018, including 905 transthoracic echocardiograms (TTEs). Of these, 451 (49.8%) fulfilled the strict negative criteria (group 1). In this group, IE was seldom diagnosed (n = 4, 0.9%). In 338 patients (37.4%) no signs of IE were evident, but they did not fulfill the strict negative criteria (group 2). A follow-up echocardiogram and definitive diagnosis of IE were more frequent (n = 48, 14.2% and n = 20, 5.9%). Finally, in 116 patients (12.8%) the initial TTE showed typical or suggestive signs of IE, in whom the diagnosis was confirmed in 48 patients (41.4%). A definitive diagnosis of IE was established in a minority of the study population (n = 72, 8%). Only 1 readmission for underdiagnosis of IE was noted on group 2. We conclude that in a real-life setting only a minority of patients in whom IE was suspected had a definitive diagnosis. An initial TTE for suspected IE fulfilling the strict negative criteria predicts both a low probability of requesting a follow-up study and of a definitive diagnosis of IE. (c) 2021 Elsevier Inc. Background The diagnosis of skin wound vitality is currently based on standard histology, but histological findings lack sensitivity in case of a short survival time. New reliable biomarkers of vitality are therefore strongly needed. We assessed the ability of 10 candidate cytokines (IFN-gamma, IL-1 beta, IL-2, IL-4, IL-6, IL-8, IL-10, IL-12p70, IL-13, TNF-alpha) to discriminate between vital and early post-mortem wounds. Methods Twenty-four cadavers with a recent open skin wound (< 3 h) were included (20 men, 4 women, mean age = 51.0 +/- 24.3 years). An early post-mortem wound was performed in an uninjured skin area, and both wounds were sampled at the autopsy (post-mortem interval (PMI) = 66.3 +/- 28.3 h). Needle-puncture sites related to resuscitation cares were included as very early post-mortem wounds (n = 6). In addition to standard histology, cytokines levels were simultaneously measured in each sample using a multiplex sandwich immunoassay, then normalized on healthy skin levels. A quantitative evaluation of IL-8-positive cells in ante- and post-mortem wound samples was also performed. Results In the training set of samples (n = 72), cytokine levels were significantly higher in vital wounds (mean age = 47 +/- 53 min) than in post-mortem wounds (mean PMI = 6.9 +/- 9.0 h) (p < 0.2), except for two cytokines (IFN-gamma and IL-2). IL-8 was the best discriminatory cytokine (Se = 54%, Sp = 100%, AUC = 0.79), while a multivariate model combining IL-4 and IL12p70 was a bit more discriminant (Se = 55%, Sp = 100%, AUC = 0.84). In the validation set (n = 72), the discriminatory power of the cytokines and the predictive model was slightly lower, with IL-8 remaining the best cytokine (Se = 46%, Sp = 96%, AUC = 0.75). The predictive model remained highly specific (Sp = 100%). Both the cytokines and the predictive model allowed the iatrogenic injuries to be correctly classified as post-mortem wounds. Standard histology and immunohistochemistry showed 21% sensitivity and a specificity of 79% and 100%, respectively. Only two iatrogenic wounds could be properly categorized histologically. Conclusion This study suggests that cytokines could be useful biomarkers of skin wound vitality and that the immunoassay method could be more sensitive than immunohistochemistry to identify wounds with a short survival time. Further research is underway to confirm these preliminary data.

Objectives Guidelines recommending 12-month dual antiplatelet therapy (DAPT) in patients with ST-elevation acute coronary syndrome (STEACS) undergoing percutaneous coronary intervention (PCI) were published in year 2012. We aimed to describe the influence of guideline implementation on the trend in 12-month persistence with DAPT between 2010 and 2015 and to evaluate its relationship with DAPT duration regimens recommended at discharge from PCI hospitals. Design Observational study based on region-wide registry data linked to pharmacy billing data for DAPT follow-up. Setting All PCI hospitals (10) belonging to the acute myocardial infarction (AMI) code network in Catalonia (Spain). Participants 10 711 STEACS patients undergoing PCI between 2010 and 2015 were followed up. Primary and secondary outcome measures Primary outcome was 12-month persistence with DAPT. Calendar year quarter, publication of guidelines, DAPT duration regimen recommended in the hospital discharge report, baseline patient characteristics and significant interactions were included in mixed-effects logistic regression based interrupted time-series models. Results The proportion of patients on-DAPT at 12 months increased from 58% (56-60) in 2010 to 73% (71-75) in 2015. The rate of 12-month persistence with DAPT significantly increased after the publication of clinical guidelines with a time lag of 1 year (OR=1.20; 95% CI 1.11 to 1.30). A higher risk profile, more extensive and complex coronary disease, use of drug-eluting stents (OR=1.90; 95% CI 1.50 to 2.40) and a 12-month DAPT regimen recommendation at discharge from the PCI hospital (OR=5.76; 95% CI 3.26 to 10.2) were associated with 12-month persistence. Conclusion Persistence with 12-month DAPT has increased since publication of clinical guidelines. Even though most patients were discharged on DAPT, only 73% with potential indication were on-DAPT 12 months after PCI. A guideline-based recommendation at PCI hospital discharge was highly associated with full persistence with DAPT. Establishing evidence-based, common prescribing criteria across hospitals in the AMI-network would favour adherence and reduce variability.

Sister chromatid intertwines (SCIs), or catenanes, are topological links between replicated chromatids that interfere with chromosome segregation. The forma- tion of SCIs is thought to be a consequence of fork swiveling during DNA replication, and their removal is thought to occur because of the intrinsic feature of type II topoisomerases (Top2) to simplify DNA topology. Here, we report that SCIs are also formed independently of DNA replication during G 2 /M by Top2-dependent concatenation of cohesed chroma- tids due to their physical proximity. We demonstrate that, in contrast to G 2 /M, Top2 removes SCIs from co- hesedchromatidsattheanaphaseonset.Importantly, SCI removal in anaphase requires condensin and co- incides with the hyperactivation of condensin DNA supercoiling activity. This is consistent with the long- standing proposal that condensin provides a bias in Top2functiontowarddecatenation.Acomprehensive model for the formation and resolution of toxic SCI entanglements on eukaryotic genomes is proposed.

This deliverable presents the short thematic videos of visualizations from the data produced by Work Packages 1 to 5, and explains in detail the procedure followed to create the videos. The aim of creating these movies was: a) To become familiar with the research topics and data types produced in the various WPs of the project, in order to develop the overall aesthetics and data conversion pipeline that will be used later in D6.16, “Producing one long movie on EoCoE overall results”, and b) To provide dissemination material from the simulations produced in the project, that is appealing and comprehensive to the project target groups. These videos have been disseminated in conferences, workshops, and the website and social media of EoCoE., H2020

Se ascunde faptul căîn anul 2005 mai avusese loc un experiment supraluminal.

Rationale: Because the characteristics of all body fluids depends on patient’s health status, is it possible that disadvantaged and socially vulnerable mothers may have lower amounts of iron in their breast milk, and that their babies receive lower content of the mineral for their normal growth and development. Assuring a preventive treatment of the mother might solve this problem. Objective: to demonstrate breast milk iron content from disadvantaged mothers and impact of personalized iron supplementation program. Materials and Methods: cross-sectional study. Breast milk samples were obtained for ferritin analysis. Health’s services usually provides free folic acid and iron treatment however, treatment compliance is low. Patients were random in two groups: “A: Controls” that had free iron tablets available from Health Centre; and “B: Intervention” group where patients accepted to be periodically contacted at home by health’s team for personalized iron dispensation. Results: 360 patients were included. Profilaxis and treatment compliance were 100% and 97,6% for B group while for “Control” one was 63% and 34%(p0.0001). Higher breast milk iron levels were detected in Intervention’s mothers compared with control’s patients (p0.007). Conclusion: Personalized iron prophylaxis and treatment increased breast milk iron levels. Public health policy must ensure iron dispensation for each underserved mother in order to reduce children problems associate to iron deficiency during the first year of their life.

The extension of bacteriological methods and concepts to the study of animal cells (1, 2, 3) made it possible to test the generality of those mechanisms which have been described for bacterial systems. In practice, this could be done by verifying whether bacterial mechanisms can explain phenomena observed in animal cells-an approach which, although possibly biased, seems at least to provide a solid experimental guideline. A typical example of this trend is the proposition that the neoplastic transformation of mammalian cells mediated by some DNA viruses is the equivalent of the lysogenic state in bacteria (4). This example also indicates the limits of a "microbial" approach to animal cell systems. In fact, it must be recognized that the understanding of lysogeny, as well as other major problems in bacterial physiology, was achieved because both phages and bacteria were susceptible to genetic analysis (5). Two major handicaps hindered the development of genetics in animal cells in vitro. First was the scarcity of good genetic markers, partly due to the frequent disappearance of characters expressed in the organism upon extended cultivation of its cells in vitro (6), and partly due to the apparent unresponsiveness of mammalian cells to mutagens effective in bacteria (7). The authors discuss the CT findings of the late brain damage by the radiochemotherapic treatment for the prophylaxis of meningeal leukemia in 50 patients, 1 divided by 15 years old. The more frequent CT findings are: enlargement of the subaracnoid spaces; ventricular dilatation; hypodensity of cerebral parenchima; calcifications. These findings correspond to two clinical and anatomo-pathological conditions: subacute leucoencephalopathy and mineralizing microangiopathy. A greater damage on central nervous system was demonstrated in younger children (5 years old) at the time of prophylaxis of meningeal leukemia, that were irradiated with NSD = 950 ret.

We evaluate the employment effect of the green part of the largest fiscal stimulus in recent history, the American Recovery and Reinvestment Act (ARRA). Each $1 million of green ARRA created 15 new jobs that emerged especially in the post-ARRA period (2013-2017). We find little evidence of significant short-run employment gains. Green ARRA creates more jobs in commuting zones with a greater prevalence of pre-existing green skills. Nearly half of the jobs created by green ARRA investments were in construction or waste management. Nearly all new jobs created are manual labor positions. Nonetheless, manual labor wages did not increase.
