This paper aims to understand how the first phase of exit from the pandemic has changed the temporality of everyday life and produced an affective atmosphere characterized by a positive attitude towards the future. I will start by analyzing the link between utopia and everyday life: on the one hand, as a privileged area of prefigurative practices in the different versions of utopian realism, and on the other, as trapped in the present in the theories of presentification. I will then examine some emblematic cases of a ‘need for the future’ that emerged in the first exit from the pandemic in the spring-summer 2021. Finally, starting from the affective and temporally complex nature of the present as conceptualized by Berlant (Berlant, 2011; Berlant, 2008) and Coleman (Coleman, 2020b; Coleman, 2020a), I will try to account for the push toward the future that the partial exit from the pandemic has perhaps brought out.

IntroductionDysregulated eating (emotional eating, cue-elicited eating, and dietary restraint and restriction) has been linked to being overweight or obese. The present investigation used a random controlled trial (RCT) to test the differential efficacy of remotely delivered Mindfulness-Based Eating Awareness Training (MB-EAT) and Behavioral Weight Loss (BWL) counseling.MethodsThe sample was recruited through advertisements that offered help to people “with problems controlling their eating” or “interested in improving their relationship with food” (n = 135).ResultsRetention was low in both groups (42%), but not dissimilar to retention rates reported in related clinical trials delivered “in person.” Among the participants who completed treatment, we found no between-group differences in any of the treatment outcomes, but participants in both groups experienced significant increases in eating-related mindfulness [Mindful Eating Questionnaire (MEQ) and awareness [Multidimensional Assessment of Interoceptive Awareness (MAIA), and significant decreases in unhealthy eating patterns [Dutch Eating Behavior Questionnaire (DEBQ); Binge Eating Scale (BES), and weight over the course of treatment. Participants in both groups also experienced increases in self-reported depression and anxiety symptoms [Hospital Anxiety and Depression Scale (HADS)], although these increases likely reflected normative changes observed in the population at large during COVID-19.DiscussionOverall, the results suggest that dysregulated eating and weight loss intervention delivered remotely via teleconference can be effective.

After extensive evaluation, one-third of patients affected by polyneuropathy remain undiagnosed and are labelled as having chronic idiopathic axonal polyneuropathy, which refers to a sensory or sensory-motor, axonal, slowly progressive neuropathy of unknown origin. Since a sensory neuropathy/neuronopathy is identified in all patients with genetically confirmed RFC1 cerebellar ataxia, neuropathy, vestibular areflexia syndrome, we speculated that RFC1 expansions could underlie a fraction of idiopathic sensory neuropathies also diagnosed as chronic idiopathic axonal polyneuropathy. We retrospectively identified 225 patients diagnosed with chronic idiopathic axonal polyneuropathy (125 sensory neuropathy, 100 sensory-motor neuropathy) from our general neuropathy clinics in Italy and the UK. All patients underwent full neurological evaluation and a blood sample was collected for RFC1 testing. Biallelic RFC1 expansions were identified in 43 patients (34%) with sensory neuropathy and in none with sensory-motor neuropathy. Forty-two per cent of RFC1-positive patients had isolated sensory neuropathy or sensory neuropathy with chronic cough, while vestibular and/or cerebellar involvement, often subclinical, were identified at examination in 58%. Although the sensory ganglia are the primary pathological target of the disease, the sensory impairment was typically worse distally and symmetric, while gait and limb ataxia were absent in two-thirds of the cases. Sensory amplitudes were either globally absent (26%) or reduced in a length-dependent (30%) or non-length dependent pattern (44%). A quarter of RFC1-positive patients had previously received an alternative diagnosis, including Sjögren’s syndrome, sensory chronic inflammatory demyelinating polyneuropathy and paraneoplastic neuropathy, while three cases had been treated with immune therapies., Currò et al. investigate the prevalence of RFC1 expansions in a cohort of patients with chronic idiopathic axonal polyneuropathy. RFC1 expansions were found in 34% of patients with sensory neuropathy, but in none with sensory-motor neuropathy. Clinical features and diagnostic clues that should lead to suspicion of RFC1 neuropathy are discussed.

Vanishing White Matter Disease (VWMD) is a rare autosomal recessive leukoencephalopathy . The classical presentation is characterized by a severe cerebellar ataxia, spasticity, neurological deterioration with a chronic progressive course and episodes of acute neurological deterioration after stress conditions.We report a 52-year-old man with VWMD and atypical features who manifested two major events of transient aphasia eleven years apart with complete recovery in 48 hours. No cognitive decline was present. Brain MRI revealed typical aspects of VWMD including diffuse leukoencephalopathy with relative sparing of U-fibers. We identified the presence of c.592G>A (p.Glu198Lys) and c.1360 C>T (p.Pro454Ser) mutations in EIF2B5.

The Eating Disorder-15 (ED-15) is a brief measure developed for the routine assessment of eating disorder (EDs), despite its use in research settings having increased over the last few years. To assess the psychometric properties of the ED-15 in a nonclinical sample of Chilean women.A multicultural Spanish-language version was developed through combined translation techniques. University female students (N = 380) (MA bi-factor model replicating the bi-factor structure and providing support for the general factor showed the best fit to the data. Moderate-to-strong correlations with measures of EDs, physical comparison, body appreciation, social physique anxiety, and depressive symptoms are presented as evidence of concurrent and convergent validity. Items analyses revealed strong correlations between manifest variables, their correspondent latent variable and the general factor (rLike previous findings, the current study provides validity to the original bi-factor structure of the ED-15 and additional validity to a general factor. The availability of this multicultural Spanish-language version would provide session-by-session assessment, providing crucial information throughout treatment. Background: Data from published studies about the effect of HFE polymorphisms on ALS risk, phenotype, and survival are still inconclusive. We aimed at evaluating whether the p.H63D polymorphism is a modifier of phenotype and survival in SOD1-mutated patients. Methods: We included 183 SOD1-mutated ALS patients. Mutations were classified as severe or mild according to the median survival of the study population. Patients were screened for the HFE p.H63D polymorphism. Survival was calculated using the Kaplan–Meier modeling, and differences were measured by the log-rank test. Multivariable analysis was performed with the Cox proportional hazards model (stepwise backward). Results: SOD1 severe mutation carriers show more frequent familial history for ALS and shorter survival compared to mild mutation carriers. Carriers and non-carriers of the p.H63D polymorphism did not differ in terms of sex ratio, frequency of positive familial history, age at onset, and bulbar/spinal ratio. In univariate and in Cox multivariable analysis using sex, age at onset, site of onset, family history, country of origin, and mutation severity as covariates, p.H63D carriers had a longer survival (p = 0.034 and p = 0.004). Conclusions: We found that SOD1-mutated ALS patients carrying the p.H63D HFE polymorphism have a longer survival compared to non-carriers, independently of sex, age and site of onset, family history, nation of origin, and severity of mutations, suggesting a possible role as disease progression modifier for the p.H63D HFE polymorphism in SOD1-ALS.

SPTLC1-related disorder is a late onset sensory-autonomic neuropathy associated with perturbed sphingolipid homeostasis which can be improved by supplementation with the serine palmitoyl-CoA transferase (SPT) substrate, l-serine. Recently, a juvenile form of motor neuron disease has been linked to SPTLC1 variants. Variants affecting the p.S331 residue of SPTLC1 cause a distinct phenotype, whose pathogenic basis has not been established. This study aims to define the neuropathological and biochemical consequences of the SPTLC1 p.S331 variant, and test response to l-serine in this specific genotype.We report clinical and neurophysiological characterisation of two unrelated children carrying distinct p.S331 SPTLC1 variants. The neuropathology was investigated by analysis of sural nerve and skin innervation. To clarify the biochemical consequences of the p.S331 variant, we performed sphingolipidomic profiling of serum and skin fibroblasts. We also tested the effect of l-serine supplementation in skin fibroblasts of patients with p.S331 mutations.In both patients, we recognised an early onset phenotype with prevalent progressive motor neuron disease. Neuropathology showed severe damage to the sensory and autonomic systems. Sphingolipidomic analysis showed the coexistence of neurotoxic deoxy-sphingolipids with an excess of canonical products of the SPT enzyme. l-serine supplementation in patient fibroblasts reduced production of toxic 1-deoxysphingolipids but further increased the overproduction of sphingolipids.Our findings suggest that p.S331 SPTLC1 variants lead to an overlap phenotype combining features of sensory and motor neuropathies, thus proposing a continuum in the spectrum of SPTLC1-related disorders. l-serine supplementation in these patients may be detrimental.

Current evidence on obstetric patients requiring advanced ventilatory support and impact of delivery on ventilatory parameters is retrospective, scarce, and controversial.What are the ventilatory parameters for obstetric patients with COVID-19 and how does delivery impact them? What are the risk factors for invasive mechanical ventilation (IMV) and for maternal, fetal, and neonatal mortality?Prospective, multicenter, cohort study including pregnant and postpartum patients with COVID-19 requiring advanced ventilatory support in the ICU.Ninety-one patients were admitted to 21 ICUs at 29.2 ± 4.9 weeks; 63 patients (69%) had delivered. Maximal ventilatory support was as follows: IMV, 69 patients (76%); high-flow nasal cannula, 20 patients (22%); and noninvasive mechanical ventilation, 2 patients (2%). Still working in a framework of weighted Mourre theory, we improve the limiting absorption principle (LAP) that was obtained in [Ma1]. To our knowledge, this is a new result even in the one-dimensional case. The improvement consists in a weakening of the assumptions on the long-range potential and better LAP weights. The improvement relies only on the fact that the generator of dilations (which serves as conjugate operator) is bounded from above by the position operator. To exploit this, Loewner's theorem on operator monotone functions is invoked.

The article triangulates Schatzki’s practice theory (2002) and Thévenot’s (2001) ‘regimes of engagement’ with the nascent field of ‘social futures’ or future-oriented cultural sociology (e.g. Coleman and Tutton 2017; Mische 2009). We present a reading of Thévenot’s regimes of engagement that is ontologically compatible with Schatzki’s account of social practices and argue that practices are more or less enmeshed in distinct regimes of engagement. We aver that while people engage reality through distinct regimes of engagement they do so, to follow Schatzki, primarily in the context of social practices. We then articulate this account with Giuliana Mandich’s (2019) work on the different modes of engagement with the future, or forms of reflexive projectivity, that are expressed through Thévenot’s regimes: that of practical anticipation for the regime of the familiar; of probability for the regime of the plan; of possibility for the regime of justification; and of discovery for the regime of exploration. This synthesis enables a practice theoretical account of the different modes through which actors engage with the future that is, we suggest, analytically superior to both Thévenot’s and Schatzki’s tendency towards a mono-dimensional view of agents’ projectivity towards the future. We illustrate the synthesis of practices, regimes of engagement and modes of projectivity chiefly through a study of domestic laundry practice conducted by Southerton and Mylan (2018). We suggest this theoretical development may be productively applied to practice theoretical work informing understanding of issues such as sustainable consumption, mobility and energy use, as well as social change more broadly.

During the SARS-CoV-2 pandemic, there was a marked requirement for critical care beds, supplies and trained professionals to assist patients with severe respiratory failure. The Argentine Society of Intensive Care (SATI) designed a study to characterize these aspects in intensive care units (ICUs). Multicenter, prospective cohort study; the participating ICUs completed a form at the end of the study (31/10/2020) on hospital characteristics, number of beds in pre- and intra-pandemic critical areas, incorporation of professionals, technological resources, and workload. Fifty-eight ICUs participated; 28(48%) were located in Buenos Aires Province, 22(38%) in Buenos Aires Autonomous City and 10 (17%) in other provinces; 31 (53%) of UCIs belonged to the public sector; 23 (47%) to the private-social security. In 35/58 (60%) of the hospitals critical care beds increased from 902 to 1575 (75%), 37% in ICU and 63% mainly in Coronary Care Unit and Emergency-shock room. In 41/55 (75%) UCIs, staff were incorporated: 27(49%) physicians (70% intensivists), 36 (65%) nurses, 28 (51%) respiratory therapists, 20(36%) cleaning staff, and 1(2%) others. A 96% of the ICUS reported having sufficient ventilators and 95% enough supplies and PPE. Of all patients on invasive mechanical ventilation, 55% [43-64] had COVID-19. Oxygen therapy was required as noninvasive support in 14% [8-24] of COVID-19 admissions. There was a significant expansion of critical operational areas, secondary to the increase in beds, staff, and adequate availability of ventilators and essential supplies. The burden of critical illness from COVID-19 was intense, with more than half of patients on mechanical ventilation.Durante la pandemia por SARS-CoV-2 hubo un marcado requerimiento de camas de cuidados críticos, insumos y profesionales entrenados para asistir a pacientes con insuficiencia respiratoria grave. La Sociedad Argentina de Terapia Intensiva (SATI) diseñó un estudio para caracterizar estos aspectos en las Unidades de Cuidados Intensivos (UCIs). Estudio multicéntrico, de cohorte prospectiva; las UCIs participantes completaron un formulario al final del estudio (31/10/2020) sobre características hospitalarias, número de camas de áreas críticas pre- e intra-pandemia, incorporación de profesionales, insumos y recursos tecnológicos, y carga de trabajo. Participaron 58 UCIs; 28(48%) de Provincia de Buenos Aires, 22(38%) de Ciudad Autónoma de Buenos Aires, 10(17%) de otras; 31(53%) UCIs pertenecían al sector público; 23(47%) al privado-seguridad social. En 35/58(60%) hospitales las camas de cuidados críticos aumentaron de 902 a 1575(75%); 37% en UCI y 63% principalmente en Unidad Coronaria y Emergencias-shock room. De todos los pacientes en ventilación mecánica invasiva, 55% [43-64] presentaron COVID-19. Se requirió oxigenoterapia como soporte no invasivo en 14% [8-24] de los ingresos por COVID-19. Se registró una importante expansión de las áreas críticas operativas, secundariamente al aumento de camas, personal, y adecuada disponibilidad de respiradores e insumos esenciales. La carga de la enfermedad crítica por COVID-19 fue intensa, constituyendo más de la mitad de los pacientes en ventilación mecánica.

The All of Us Research Program, a health and genetics epidemiologic data collection program, has been substantially affected by the coronavirus disease 2019 (COVID-19) pandemic. Although the program is highly digital in nature, certain aspects of the data collection require in-person interaction between staff and participants. Before the pandemic, the program was enrolling approximately 12,500 participants per month at more than 400 clinical sites. In March 2020, because of the pandemic, all in-person activity at program sites and by engagement partners was paused to develop processes and procedures for in-person activities that incorporated strict safety protocols. In addition, the program adopted new data collection methodologies to reduce the need for in-person activities. Through February 2022, a total of 224 clinical sites had reactivated in-person activity, and all enrollment and engagement partners have adopted new data collection methods that can be used remotely. As the COVID-19 pandemic persists, the program continues to require safety procedures for in-person activity and continues to generate and pilot methodologies that reduce risk and make it easier for participants to provide information.

We continue the investigation of the existence of absolutely continuous (a.c.) spectrum for the discrete Schr��dinger operator $��+V$ on $\ell^2(\Z^d)$, in dimensions $d\geq 2$, for potentials $V$ satisfying the long range condition $n_i(V-��_i ^��V)(n) = O(\ln^{-q}(|n|))$ for some $q>2$, $��\in \N$, and all $1 \leq i \leq d$, as $|n| \to \infty$. $��_i ^�� V$ is the potential shifted by $��$ units on the $i^{\text{th}}$ coordinate. The difference between this article and \cite{GM2} is that here finite linear combinations of conjugate operators are constructed leading to more bands of a.c.\ spectrum being observed. The methodology is backed primarily by graphical evidence because the linear combinations are built by numerically implementing a polynomial interpolation. On the other hand an infinitely countable set of thresholds, whose exact definition is given later, is rigorously identified. Our overall conjecture, at least in dimension 2, is that the spectrum of $��+V$ is void of singular continuous spectrum, and consecutive thresholds are endpoints of a band of a.c. spectrum., arXiv admin note: substantial text overlap with arXiv:2201.00410

RESUMO Objetivo: Descrever e comparar a estrutura das unidades de terapia intensiva argentinas que responderam ao inquérito de autoavaliação de unidades de terapia intensiva desenvolvido pela Sociedad Argentina de Terapia Intensiva. Métodos: Foi realizado um estudo transversal observacional com uso de um inquérito voluntário online por meio do banco de dados de membros da Sociedad Argentina de Terapia Intensiva e outras publicações em mídias sociais. Foram analisadas as respostas recebidas entre dezembro de 2018 e julho de 2020. Foram utilizados testes não paramétricos e estatística descritiva. Resultados: Foram recebidos 392 inquéritos, sendo 244 considerados para a análise. Eram de unidades de terapia intensiva adulto 77% (187/244), e 23% (57/244) eram de unidades de terapia intensiva pediátrica. A taxa de participação foi de 76%. A amostra incluiu 2.567 leitos de unidades de terapia intensiva (1.981 adulto e 586 pediátrica). Observamos nítida concentração de unidades de terapia intensiva nas regiões Centro e Buenos Aires, Argentina. A mediana de leitos foi de dez (intervalo interquartil 7 - 15). A mediana de monitores multiparamétricos, ventiladores mecânicos e oxímetros de pulso foi de um por leito, sem diferenças regionais ou de tipo de unidade de terapia intensiva (adulto ou pediátrica). Embora nossa amostra tenha evidenciado que as unidades de terapia intensiva pediátrica apresentaram proporção de ventilação mecânica/leito maior do que as unidades de terapia intensiva adulto, esse achado não foi linearmente correlacionado. Conclusão: A Argentina tem concentração notável de leitos de cuidados intensivos e complexidade estrutural superior nas regiões de Buenos Aires e Centro, tanto de unidades de terapia intensiva adulto quanto pediátrica. Além disso, observou-se ausência de dados precisos informados sobre a estrutura e os recursos de unidades de terapia intensiva. São necessárias mais opções de melhoria para alocar os recursos de unidades de terapia intensiva nos níveis institucional e regional.

Achieving clean energy goals will require significant advances in regard to addressing the computational needs for next-generation renewable-dominated power grids. One critical obstacle that lies in the way of transitioning today’s power grid to a renewable-dominated power grid is the lack of a faster-than-real-time stability assessment technology for operating a fast-changing power grid. This paper proposes an artificial intelligence (AI) -based method that predicts the system’s stability margin information (e.g., the frequency nadir in the frequency stability assessment and the critical clearing time (CCT) value in the transient stability assessment) directly from the system operating conditions without performing the conventional time-consuming time-domain simulations over detailed dynamic models. Since the AI method shifts the majority of the computational burden to offline training, the online evaluation is extremely fast. This paper has tested the AI-based stability assessment method using multiple dispatch cases that are converted and tuned from actual dispatch cases of the Western Electricity Coordinating Council (WECC) system model with more than 20,000 buses. The results show that the AI-based method could accurately predict the stability margin of such a large power system in less than 0.2 milliseconds using the offline-trained AI agent. Therefore, the proposed method has great potential to achieve faster-than-real-time stability assessment for practical large power systems while preserving sufficient accuracy.

El Trastorno de Evitación/Restricción de la Ingesta de Alimentos (ARFID) se caracteriza por la evitación persistente y/o restricción de la ingesta de alimentos, observándose tres presentaciones clínicas: falta de interés en la comida, selectividad basada en la sensibilidad sensorial, y miedo a las consecuencias aversivas. Las estrategias utilizadas por los padres durante las comidas pueden predisponer o mantener el rechazo alimentario del niño. Objetivo: determinar asociación entre las estrategias parentales utilizadas en la alimentación, con la conducta de rechazo a los alimentos en niños/as ARFID. Pacientes y Método: Investigación de corte transversal. Para la selección de los participantes se utilizó un muestreo no probabilístico. Participaron 24 padres/madres cuyos hijos habían sido diagnosticados con un ARFID. Se consideraron los subtipos de ARFID segun criterios del DSM-5: Apetito limitado, Ingesta selectiva y Miedo a la alimentación. Criterios de exclusión fueron ARFID de causa orgánica y/o trastornos generalizados del desarrollo. Para la recolección de datos se utilizaron el Child Eating Behaviors Questionnaire y el Child Feeding Questionnaire. Resultados: Se evidenció una asociación entre la estrategia alimentaria parental de presión para comer con la conducta de rechazo a los alimentos, y con conductas alimentarias infantiles de subalimen- tación emocional (p = 0,046), lentitud para comer (p = 0,016), rechazo frente a los alimentos (p = 0,019) y respuesta de saciedad (p = 0,003). Conclusión: Las conductas alimentarias frecuentemente percibidas por los padres con hijos diagnosticados con ARFID se relacionan con la dimensión de enfoque negativo hacia la comida, como respuesta de saciedad, rechazo alimentario, lentitud para comer y subalimentación emocional.

To describe and compare the structure of Argentinean intensive care units that completed the "self-assessment survey of intensive care units" developed by the Sociedad Argentina de Terapia Intensiva.An observational crosssectional study was conducted using an online voluntary survey through the Sociedad Argentina de Terapia Intensiva member database and other social media postings. Answers received between December 2018 and July 2020 were analyzed. Descriptive statistics and nonparametric tests were used.A total of 392 surveys were received, and 244 were considered for the analysis. Seventy-seven percent (187/244) belonged to adult intensive care units, and 23% (57/244) belonged to pediatric intensive care units. The overall completion rate was 76%. The sample included 2,567 ICU beds (adult: 1,981; pediatric: 586). We observed a clear concentration of intensive care units in the Central and Buenos Aires regions of Argentina. The median number of beds was 10 (interquartile range 7 - 15).The median numbers of multiparameter monitors, mechanical ventilators, and pulse oximeters were 1 per bed with no regional or intensive care unit type differences (adult versus pediatric). Although our sample showed that the pediatric intensive care units had a higher mechanical ventilation/bed ratio than the adult intensive care units, this finding was not linearly correlated.Argentina has a notable concentration of critical care beds and better structural complexity in the Buenos Aires and Centro regions for both adult and pediatric intensive care units. In addition, a lack of accurate data reported from the intensive care unit structure and resources was observed. Further improvement opportunities are required to allocate intensive care unit resources at the institutional and regional levels.Descrever e comparar a estrutura das unidades de terapia intensiva argentinas que responderam ao inquérito de autoavaliação de unidades de terapia intensiva desenvolvido pela Sociedad Argentina de Terapia Intensiva.Foi realizado um estudo transversal observacional com uso de um inquérito voluntário online por meio do banco de dados de membros da Sociedad Argentina de Terapia Intensiva e outras publicações em mídias sociais. Foram analisadas as respostas recebidas entre dezembro de 2018 e julho de 2020. Foram utilizados testes não paramétricos e estatística descritiva.Foram recebidos 392 inquéritos, sendo 244 considerados para a análise. Eram de unidades de terapia intensiva adulto 77% (187/244), e 23% (57/244) eram de unidades de terapia intensiva pediátrica. A taxa de participação foi de 76%. A amostra incluiu 2.567 leitos de unidades de terapia intensiva (1.981 adulto e 586 pediátrica). Observamos nítida concentração de unidades de terapia intensiva nas regiões Centro e Buenos Aires, Argentina.A mediana de leitos foi de dez (intervalo interquartil 7 - 15). A mediana de monitores multiparamétricos, ventiladores mecânicos e oxímetros de pulso foi de um por leito, sem diferenças regionais ou de tipo de unidade de terapia intensiva (adulto ou pediátrica). Embora nossa amostra tenha evidenciado que as unidades de terapia intensiva pediátrica apresentaram proporção de ventilação mecânica/leito maior do que as unidades de terapia intensiva adulto, esse achado não foi linearmente correlacionado.A Argentina tem concentração notável de leitos de cuidados intensivos e complexidade estrutural superior nas regiões de Buenos Aires e Centro, tanto de unidades de terapia intensiva adulto quanto pediátrica. Além disso, observou-se ausência de dados precisos informados sobre a estrutura e os recursos de unidades de terapia intensiva. São necessárias mais opções de melhoria para alocar os recursos de unidades de terapia intensiva nos níveis institucional e regional.

This paper is aimed at understanding how we engage with the future in different ways in everyday life. Many empirical studies have emphasised that what we usually call ‘imagination’ of the future takes diverse forms and meanings. Varied narratives of the future that are possible coexist in daily life in a bumpy, semi-conscious and occasionally tense dialogue with one another. To understand this variation of narratives, a thorough exploration of the different modes of engaging with the future that various forms of agency bring into play is required, together with a sensitive empirical analysis. I use Thévenot’s theory of regimes of engagement as a starting point to at least partially explain this variety. Thévenot’s idea that different types of individual involvement in relation to different definitions of the relevant reality (e.g. familiarity, plans and the public domain of justification and exploration) contain interesting implications for the analysis of what I define as modes of engagement with the future. As involved as we are with social reality through specific formats, so are we with the future. As the ‘relevant reality’ is different according to the regime of engagement that we are involved in, the nature of anticipation also varies. The future is ‘made and measured’ within the logic of probability in the regime of plans; of possibility in the regime of justification; of practical anticipation in the regime of familiarity; and of discovery in the regime of exploration. This perspective helps to avoid a reification of the future as something that is ‘there’ and that we simply discover and avoids easy dichotomisation of forms of anticipation of the future as realistic or unrealistic.

RESUMEN La obesidad es un problema de salud pública, siendo especialmente importante entre mujeres jóvenes. El objetivo de este estudio fue comparar la vivencia corporal, los hábitos saludables y regulación emocional entre mujeres jóvenes chilenas con obesidad y normopeso. El estudio tuvo un enfoque cualitativo, de alcance exploratorio y diseño descriptivo-analítico. Para describir la muestra se usó el Multidimensional Body Self Relations Questionnaire (MBSRQ) que mide imagen corporal, el Dutch Eating Behavior Questionnaire (DEBQ) para evaluar ingesta emocional, y el Derogatis Symptom Checklist (SCL-90), que mide sintomatología psicológica. Se realizaron entrevistas en profundidad, que fueron analizadas utilizando codificación abierta de la Grounded Theory. Los resultados indican que ambos grupos presentan insatisfacción con la forma y peso de sus cuerpos, predominando en las participantes con obesidad al percibir limitaciones corporales. Las mujeres con obesidad describieron que regularon emociones negativas a través de la alimentación. Las participantes normopeso se caracterizaron por hábitos saludables, como actividad física y alimentación balanceada y en ambos grupos valorizan el cuidado del cuerpo y la estética personal, aunque sólo participantes normopeso presentaron conductas de cuidado efectivas.

Unusual clinical presentations in patients with E200K familial Creutzfeldt-Jakob Disease (fCJD) have been rarely reported. Herein, we described a case of E200K fCJD presenting with subacute cranial multiple neuropathy, initially suspected to be paraneoplastic or due to a leptomeningeal carcinomatosis, considering the neoplastic comorbidity of the patient. Surprisingly, brain MRI was highly suggestive of CJD. Brain histological examination confirmed the diagnosis. Genetic tests led to the definite diagnosis of E200K fCJD. To the best of our knowledge, the current case provides the first report of a histologically-confirmed E200K fCJD starting with cranial multiple neuropathy and may widen the spectrum of the clinical variability of CJD, also in its genetic variant. Unusual presentations may lead, as in this case, to incorrect diagnostic hypothesis and unuseful therapeutic attempts in the first phase of the diagnostic process. Also in the genetic variant of CJD, brain MRI demonstrated a very high sensitivity to detect the typical abnormalities since the earliest phases of the disease.

Objective: To define the prevalence of variants in collagen VI genes through a next-generation sequencing (NGS) approach in undiagnosed patients with suspected neuromuscular disease and to propose a diagnostic flowchart to assess the real pathogenicity of those variants. Methods: In the past five years, we have collected clinical and molecular information on 512 patients with neuromuscular symptoms referred to our center. To pinpoint variants in COLVI genes and corroborate their real pathogenicity, we sketched a multistep flowchart, taking into consideration the bioinformatic weight of the gene variants, their correlation with clinical manifestations and possible effects on protein stability and expression. Results: In Step I, we identified variants in COLVI-related genes in 48 patients, of which three were homozygous variants (Group 1). Then, we sorted variants according to their CADD score, clinical data and complementary studies (such as muscle and skin biopsy, study of expression of COLVI on fibroblast or muscle and muscle magnetic resonance). We finally assessed how potentially pathogenic variants (two biallelic and 12 monoallelic) destabilize COL6A1-A2-A3 subunits. Overall, 15 out of 512 patients were prioritized according to this pipeline. In seven of them, we confirmed reduced or absent immunocytochemical expression of collagen VI in cultured skin fibroblasts or in muscle tissue. Conclusions: In a real-world diagnostic scenario applied to heterogeneous neuromuscular conditions, a multistep integration of clinical and molecular data allowed the identification of about 3% of those patients harboring pathogenetic collagen VI variants.

To identify determinants of oxygenation over time in patients with COVID-19 acute respiratory distress syndrome (ARDS); and to analyze their characteristics according to Berlin definition categories.Prospective cohort study including consecutive mechanically ventilated patients admitted between 3/20/2020-10/31/2020 with ARDS. Epidemiological and clinical data on admission; outcomes; ventilation, respiratory mechanics and oxygenation variables were registered on days 1, 3 and 7 for the entire population and for ARDS categories.1525 patients aged 61 ± 13, 69% male, met ARDS criteria; most frequent comorbidities were obesity, hypertension, diabetes and respiratory disease. On admission, 331(21%), 849(56%) and 345(23%) patients had mild, moderate and severe ARDS; all received lung-protective ventilation (mean tidal volumes between 6.3 and 6.7 mL/kg PBW) and intermediate PEEP levels (10-11 cmHHypoxemia in patients with COVID-19-related ARDS is associated with comorbidities, deadspace and activated coagulation markers, and disease severity-reflected by the PEEP level required.

Background Vascular Ehlers–Danlos syndrome (vEDS) is a heritable connective tissue disorder caused by defects in the type III collagen protein. It is generally considered the most severe form of Ehlers–Danlos syndrome (EDS) due to an increased risk of spontaneous artery or organ rupture. vEDS has an extremely heterogeneous presentation and muscle rupture is considered a minor diagnostic criterium. Methods A patient with a long history of inconclusive examinations and investigations was referred to our unit. The clinical picture was mainly characterized by muscle ruptures, whereas the cardiovascular involvement was limited to mitral regurgitation. We performed a panel analysis of genes associated with inheritable heart diseases using the TruSight Cardio kit (Illumina). A skin biopsy was then performed for functional studies to analyze the different forms of collagen molecules produced in vitro by cutaneous fibroblasts. Results The patient presented the novel variant c.3478A>G (p.Ile1160Val) in COL3A1 (NM_000090.3), whose pathogenicity was supported by biochemical analysis of type III collagen. Conclusion In this report, we describe a case of vEDS with predominant and severe musculoskeletal involvement. Objective: A tendon-transfer is a reconstructive orthopedic surgery where tendons are re-routed from a non-functioning muscle and attached a functioning muscle. Prior work has shown that using a passive implanted device in the ECRL-to-FDP tendon-transfer surgery significantly improves hand grasping function. However, it is still unclear how hand-function improvement, measured by finger joint range of motion and torque, is dependent on the implant’s geometry and location within the tendon network. This paper presents a genetic algorithm that determines the device’s optimal geometry and location. Methods: Hand biomechanical simulation platform was developed to model hand function and also model the tendon-transfer surgery with and without the implant. Finger kinematics and joint torque were used to develop three unique objective functions to optimize the implant’s parameters. Results: The optimized device resulted in an 11X increase in finger kinematics with only a 0.9% decrease in joint torque when compared with the biomechanical function enabled by the current suture-based surgery. Conclusion: Designing implantable devices that modify musculoskeletal function is challenging. Factors like tendon routing and joint kinematics create a complex nonlinear system when considering biomechanical function. A genetic algorithm is an effective tool to tackle these nonlinear landscapes to produce optimized designs. Significance: The state-of-the-art surgical procedure to repair high median-ulnar nerve palsy leads to poor hand function and severely limits the patient’s ability to perform activities of daily life. This work provides a method for defining relevant objective functions for hand biomechanical function and then uses those objective functions with genetic algorithms to optimize the geometry of an orthopedic implant across multiple variables. The achieved biomechanical function is significantly better than hand function enabled by current surgical procedure.

Background: The following article illustrates the development of the identity configuration process of Chilean female patients with an eating disorder (ED) following successful psychotherapeutic treatment. Method: This was a qualitative, descriptive study, which explored patients’ subjective perspectives. In-depth interviews were conducted with 20 female patients, and data were analysed according to procedures established by the Grounded Theory method. Results: Four dimensions of identity were identified: (1) self-image as idealised versus imperfect; (2) knowledge of one’s inner world; (3) impulse control; and (4) life goals. Transcending these themes was the central notion of identity and a shift in this identity from someone who defines themselves by their eating disorder toward a more authentic and complex sense of self. Conclusion: Results suggest that successful treatment facilitates the construction of an authentic identity, which includes developing a more realistic and complex understanding of themselves and their emotional and physical needs. The aim of this study was to ascertain the psychometric properties of the Multidimensional Body–Self Relations Questionnaire (MBSRQ) for a sample of youth in Chile. Cross-sectional survey was conducted with 451 participants. A non-clinical sample of adolescents and young adults between 15 and 25 years responded the MBSRQ and a sociodemographic questionnaire. Factorial analysis identified seven factors and provided empirical evidence that supports the use of these factors to evaluate body perception in Chilean youth. Scores exhibited good reliability in three factors (over 0.80) and acceptable reliability in the other four (over 0.70). Findings suggest that the Spanish MBSRQ was psychometrically sound, with 7 factors which are largely consistent with those identified in the original version and validation study of this scale. These factors possess sufficient internal consistency to make it plausible for use in research and screening with Chilean youth, and potentially useful as an adjunctive measure in the context of clinical decision making. Considered are electric potentials $V$ satisfying a long range condition of the type: $V-��_j ^��V$ decays appropriately for some $��\in \mathbb{N}$ and all $1 \leq j \leq d$, where $��_j ^�� V$ is the potential shifted by $��$ units on the $j^{\text{th}}$ coordinate. More comprehensive results are obtained for specific small values of $��$, such as $��=1,2,3,4$. In this article, we work in a simplified framework in which the main takeaway appears to be the existence of bands where a limiting absorption principle holds, and hence absolutely continuous (a.c.) spectrum, for $��>1$ and $��$ (resp.\ $��>2$ and $D$). Other decay conditions for $V$ arise from an isomorphism between $��$ and $D$ in dimension 2. Oscillating potentials are natural examples in application.

Artificial intelligence provides a convenient route for power grid stability assessment. Compared with simulation-based approaches, artificial intelligence can potentially save time on model development and numerical computation in stability assessment. This paper first reviewed existing literature on using artificial intelligence for power grid stability assessment. Then a machine-leaning-based tool is presented and developed to assess power grid transient stability, frequency stability, and small signals stability. Test results verified the accuracy and effectiveness of the AI tool for power grid stability assessment.

Interventions based on mindfulness are increasingly used for feeding issues, looking for an improvement in the emotional regulation and awareness of internal states (i.e., satiety, hunger). The objective of this paper was to describe the perception of mindfulness when controlling binge eating behavior. A total of seven adults with binge eating behavior (20-52 years old), six women and one men participated in the study. All participants receive a training (workshop) about mindfulness and feeding. Based on a qualitative methodological approach with exploratory and descriptive scope, semi-structured interviews were performed pre-post intervention and were analyzed by open coding. Before intervention participants had no awareness in their relationship with food showing an automatic response. After intervention participants reported favorable changes such as increased of awareness as well as cognitive and emotional recognition, which led to changes in eating behavior. The participants made a positive evaluation of the workshop and the group format, highlighting that mindfulness was internalized as a new tool for daily life.RESUMEN Intervenciones basadas en mindfulness (atención plena [AP]) están siendo cada vez más utilizadas para tratar temas alimentarios, buscando mejorar la regulación emocional y la conciencia acerca de los estados internos (e.g., hambre, saciedad). El objetivo de este trabajo fue describir la percepción de la práctica de la AP en el control de la conducta de atracón. Participaron siete adultos que presentaban conducta de atracón (20-52 años de edad), seis mujeres y un hombre. Como parte de este estudio, los participantes recibieron una intervención (taller) sobre AP y alimentación. Con base a un enfoque metodológico cualitativo, de alcance exploratorio y descriptivo, se realizaron entrevistas semiestructuradas pre-post intervención, las cuales fueron analizadas mediante codificación abierta. Antes de la intervención, los participantes sostenían una relación no consciente con la comida, presentando un modo de funcionar automático. Tras la intervención se reportaron cambios favorables, como el aumento de la conciencia y el reconocimiento cognitivo y emocional, lo cual supuso modificaciones en la conducta alimentaria. Los participantes realizaron una evaluación positiva del taller y del formato grupal, destacando que la AP fue interiorizada como una nueva herramienta para la vida diaria.

Considering the high prevalence of obesity and the lack of effective response to non-surgical treatments, bariatric surgery (BS) has been established as an increasingly frequent alternative, with implications on the physical, psychological and social level, highlighting the changes related to body image (BI). The objective of this study was to describe changes in BI expressed by young women undergoing BC. Based on a qualitative methodology, in-depth interviews were conducted with 10 women between 18 and 24 years. The results were grouped in Pre-BC: body dissatisfaction as motivation, low awareness of the obesity condition, shame of the body, frustration and difficulties with dressing; and Post-BS: body satisfaction associated with self-confidence, change in peer relations, new aspects of appearance, clothing associated with identity, difficulties in adapting to a new BI, and health team as support for adaptation. It is concluded that body satisfaction is a central aspect that affects individual and social wellbeing. Post-CB begins a process of adaptation to a new body scheme, with greater body satisfaction, but with difficulties related to body perception.ResumenDada la alta prevalencia de obesidad y la pobre efectividad de los tratamientos no quirúrgicos, la cirugía bariátrica (CB) constituye una alternativa cada vez más frecuente, con implicaciones a nivel físico, psicológico y social, entre las que destacan aquellas relativas a la imagen corporal (IC). El objetivo de este estudio fue describir los cambios en la IC manifestados por mujeres jóvenes sometidas a CB. Con base a una metodología cualitativa, se llevaron a cabo entrevistas a profundidad a 10 mujeres de entre 18 y 24 años de edad. Los resultados fueron categorizados en Pre-CB: insatisfacción corporal como motivación, baja conciencia de la condición de obesidad, vergüenza por el cuerpo, frustración y dificultades con la vestimenta; y Post-CB: satisfacción corporal (SC) asociada a autoconfianza, cambio en las relaciones con pares, nuevos aspectos de la apariencia, vestimenta asociada a identidad, dificultades en la adaptación a la nueva IC y equipo de salud como apoyo para la adaptación. Se concluye que la SC es un aspecto central que incide en el bienestar individual y social. Post-CB se inicia un proceso de adaptación a un nuevo esquema corporal, con mayor SC, pero con dificultades relacionadas a la percepción corporal.

Peripheral myelin protein 2 (PMP2) is a small protein located on the cytoplasmic side of compact myelin, involved in the lipids transport and in the myelination process. In the last years few families affected with demyelinating Charcot-Marie-Tooth neuropathy (CMT1), caused by PMP2 mutations, have been identified. In this study we describe the first case of a PMP2 in-frame deletion. PMP2 was analyzed by direct sequencing after exclusion of the most frequent CMT-associated genes by using a next generation sequencing (NGS) genes panel. Sanger sequencing was used for family's segregation analysis. Molecular modeling analysis was used to evaluate the mutation impact on the protein structure. A novel PMP2: p.I50del has been identified in a child with early onset CMT1 and in three affected family members. All family members show an early onset demyelinating neuropathy without other distinguish features. Molecular modeling analysis and in silico evaluations do not suggest a strong impact on the overall protein structure, but a most likely altered protein function. This study suggests the importance to add PMP2 in CMT NGS genes panels or, at most, to test it after major CMT1 genes exclusion, due to the lack of diagnostic-addressing additional features.
