1. Validation of the Pictorial Fit-Frail Scale in a Thoracic Surgery Clinic

Abstract

Examine feasibility and construct validity of Pictorial fit frail scale (PFFS) for the first time in older surgical patients.The PFFS uses visual images to measure health state in 14 domains and has been previously validated in outpatient geriatric clinics.Patients ≥65 year-old who were evaluated in a multidisciplinary thoracic surgery clinic from November 2020 to May 2021 were prospectively included. Patients completed an in-person PFFS and Vulnerable Elders Survey (VES-13) during their visit, and a frailty index was calculated from the PFFS (PFFStrans). A geriatrician performed a comprehensive geriatric assessment (CGA) either in-person or virtually, from which a Frailty Index (FI-CGA) and FRAIL scale were obtained. To assess the validity of the PFFS in this population, the Spearman's rank correlations (rspearman) between PFFStrans and VES-13, FI-CGA, FRAIL were calculated.All 49 patients invited to participate agreed, of which 46/49 (94%) completed the PFFS so a score could be calculated. The majority of patients (59%) underwent an in-person CGA and the reminder (41%) a virtual CGA. The cohort was mainly female (59.0%), with a median age of 77 (range: 67-90). The median PFFStrans was 0.27 (IQR 0.12-0.34), PFFS was 11 (IQR 5-14) and 0.24 (IQR 0.13-0.32) for FI-CGA. We observed a strong correlation between the PFFStrans and FI-CGA (rspearman = 0.81, p0.001) and a moderate correlation between PFFStrans and VES-13 and FRAIL score (rspearman 0.68 and 0.64 respectively, p0.001).PFFS had good feasibility and construct validity among older surgical patients when compared to previously validated frailty measurements.

Abstract

Purpose of Review We review the application and limitations of two implementations of the “case-only design” in injury epidemiology with example analyses of Fatality Analysis Reporting System data. Recent Findings The term “case-only design” covers a variety of epidemiologic designs; here, two implementations of the design are reviewed: (1) studies to uncover etiological heterogeneity and (2) studies to measure exposure effect modification. These two designs produce results that require different interpretations and rely upon different assumptions. The key assumption of case-only designs for exposure effect modification, the more commonly used of the two designs, does not commonly hold for injuries and so results from studies using this design cannot be interpreted. Case-only designs to identify etiological heterogeneity in injury risk are interpretable but only when the case-series is conceptualized as arising from an underlying cohort. Summary The results of studies using case-only designs are commonly misinterpreted in the injury literature.

5. An appraisal of Rankine and Perry-Robertson buckling formula under the combined effect of corrosion –shot peening interaction for 304 stainless steel alloy

Abstract

Seawater has been used to immerse metals in several industrial applications. Axial loading is applied to these alloys. These alloys are prone to corrosion failures due to differences in environmental conditions. This research looked at the influence of submerged corrosion on the buckling behaviour of 304 stainless steel long column type and intermediate alloy under increasing compressive dynamic stresses. Compressive buckling experiments were performed on twenty-four 304 stainless steel alloy columns, twelve of which were non-corroded, and the tests were performed under increasing axial dynamical compression loads. Before testing, the remaining columns were submerged for 90 days. For the subterranean columns, a maximum decrease of percent in critical buckling loading was obtained. This work uses the Rankine, and Perry-Robertson formulas to calculate the critical buckling stress of non-corroded and corroded 304 stainless steel alloys. The theoretical conclusions of the critical buckling stress were in good accord with experimental data

Abstract

The kinin receptors are classically involved in inflammation, pain and sepsis. The effects of the kinin B(1) receptor agonist des-Arg(9)-bradykinin (DBK) and lipopolysaccharide (LPS) were investigated by comparing the membrane potential responses of aortic rings from transgenic rats overexpressing the kinin B(1) receptor (B1R) in the endothelium (TGR(Tie2B(1))) and Sprague Dawley (SD) rats. No difference in the resting membrane potential in the aorta’s smooth muscle from the transgenic and SD rats was observed. The aorta rings from SD rats hyperpolarized only to LPS but not to DBK, whereas the aorta rings from TGR(Tie2B(1)) responded by the administration of both drugs. DBK and LPS responses were inhibited by the B(1) receptor antagonist R715 and by iberiotoxin in both cases. Thapsigargin induced a hyperpolarization in the smooth muscle of SD rats that was not reversed by R715, but was reversed by iberiotoxin and this hyperpolarization was further augmented by DBK administration. These results show that the model of overexpression of vascular B(1) receptors in the TGR(Tie2B(1)) rats represent a good model to study the role of functional B(1) receptors in the absence of any pathological stimulus. The data also show that K(Ca) channels are the final mediators of the hyperpolarizing responses to DBK and LPS. In addition, we suggest an interaction between the B1R and TLR4, since the hyperpolarization induced by LPS could be abolished in the presence of R715.

7. Machine Learning Approaches for Measuring Neighborhood Environments in Epidemiologic Studies

Abstract

Purpose of review Innovations in information technology, initiatives by local governments to share administrative data, and growing inventories of data available from commercial data aggregators have immensely expanded the information available to describe neighborhood environments, supporting an approach to research we call Urban Health Informatics. This review evaluates the application of machine learning to this new wealth of data for studies of the effects of neighborhood environments on health. Recent findings Prominent machine learning applications in this field include automated image analysis of archived imagery such as Google Street View images, variable selection methods to identify neighborhood environment factors that predict health outcomes from large pools of exposure variables, and spatial interpolation methods to estimate neighborhood conditions across large geographic areas. Summary In each domain, we highlight successes and cautions in the application of machine learning, particularly highlighting legal issues in applying machine learning approaches to Google’s geo-spatial data.

8. Kant on Freedom and Practical Irrationality

Abstract

This chapter argues that we have transcendental freedom, according to Kant, due to being both rational and sensible creatures that face two heterogeneous incentives. Freedom is, accordingly, located at the crossroads between reason and sensibility and is restricted to the choice between rational self-love and self-conceit. It then examines how practical deliberation proceeds and how incentives are incorporated to result in actions in order to identify the different types of practical irrationality that Kant countenances. It argues that there is only one type of practical irrationality in the moral sphere, namely having a bad will, but that there cannot be weakness of will in terms of implementing one’s commitment. In the prudential sphere, by contrast, there is no irrationality with respect to end-setting, yet there might be practical irrationality in pursuing happiness, since there can be actions that lead to more pleasure despite bringing about less happiness.

12. Data from Nanobody–Antigen Conjugates Elicit HPV-Specific Antitumor Immune Responses

Abstract

High-risk human papillomavirus-associated cancers express viral oncoproteins (e.g., E6 and E7) that induce and maintain the malignant phenotype. The viral origin of these proteins makes them attractive targets for development of a therapeutic vaccine. Camelid-derived single-domain antibody fragments (nanobodies or VHHs) that recognize cell surface proteins on antigen-presenting cells (APC) can serve as targeted delivery vehicles for antigens attached to them. Such VHHs were shown to induce CD4+ and CD8+ T-cell responses against model antigens conjugated to them via sortase, but antitumor responses had not yet been investigated. Here, we tested the ability of an anti-CD11b VHH (VHHCD11b) to target APCs and serve as the basis for a therapeutic vaccine to induce CD8+ T-cell responses against HPV+ tumors. Mice immunized with VHHCD11b conjugated to an H-2Db-restricted immunodominant E7 epitope (E749-57) had more E7-specific CD8+ T cells compared with those immunized with E749-57 peptide alone. These CD8+ T cells acted prophylactically and conferred protection against a subsequent challenge with HPV E7-expressing tumor cells. In a therapeutic setting, VHHCD11b-E749-57 vaccination resulted in greater numbers of CD8+ tumor–infiltrating lymphocytes compared with mice receiving E749-57 peptide alone in HPV+ tumor-bearing mice, as measured by in vivo noninvasive VHH-based immune-positron emission tomography (immunoPET), which correlated with tumor regression and survival outcome. 15. Determinations of Compression Buckling for Long and Intermediate Aluminum Alloy 2014-T4 Columns

Abstract

Experimental tests were carried out on long and intermediate columns of 2014-T4 aluminum alloy stressed to compression buckling. 24 samples were used, divided into two groups (12 specimens before shot peening and ultrasonic impact peening (SP +UIP) and the rest after (SP +UIP)). The ultimate tensile strength (UTS) and yield strength (YS) were improved due (SP +UIP) by 2.60% and 3.030%, respectively. ANSYS 18.2 (APDL) with the theories of Euler and Johnson was theories was used to estimate the critical buckling load (Pcr) and compare the results with the experimental work. It was found that there was strong agreement between the theories of Euler and Johnson and theories and the experimental work, with safety factors of (1.6, 2.3) and (1.7, 2.5) for long and medium columns before and after the experiment, respectively (SP +UIP). In contrast, ANSYS 18.2 (APDL) provided good predictions with a safety factor of (2.1 and 2.6) and (1.8 and 2.6) for long and medium columns before and after (SP +UIP), respectively.

Abstract

Two years after the emergence of SARS-CoV-2, our understanding of COVID-19 disease pathogenesis is still incomplete. Despite unprecedented global collaborative scientific efforts and rapid vaccine development, an uneven vaccine roll-out and the emergence of novel variants of concern such as omicron underscore the critical importance of identifying the mechanisms that contribute to this disease. Overt inflammation and cell death have been proposed to be central drivers of severe pathology in COVID-19 patients and their pathways and molecular components therefore present promising targets for host-directed therapeutics. In our review, we summarize the current knowledge on the role and impact of diverse programmed cell death (PCD) pathways on COVID-19 disease. We dissect the complex connection of cell death and inflammatory signaling at the cellular and molecular level and identify a number of critical questions that remain to be addressed. We provide rationale for targeting of cell death as potential COVID-19 treatment and provide an overview of current therapeutics that could potentially enter clinical trials in the near future.

Abstract

A machine part subjected to an axial compressive load is called strut. But, a vertical strut is known as a column. The machine members that must be investigated for the column action are connecting rods, piston rods, screw jack, etc. When a column is subjected to a compressive load and this load is gradually increased, a stage will be reached when the column will be subjected to ultimate load. Beyond this, the column will fail by crushing, and the load will be known as crushing load. When the column is short or intermediate, sometimes this column fails by bending, i.e. buckling. When the column is long, the value of buckling load is low for long columns and is relatively high for intermediate columns. The present investigation focuses on the testing and evaluation of the mechanical end buckling columns (samples) using 304 stainless steel under dry, corrosion, and combined dry with shot peening (SP) process. The buckling behavior of the axial compressive load has been studied experimentally and theoretically using Euler and Rankin theories. The results obtained from the above study manifested that the column, whose slenderness ratio (SR) is more than 120, is denoted as the long column, and the Euler theory can be successfully used. But, when the SR is less than 120, the column is known as an intermediate one. The mechanical and buckling properties exhibited a reduction due to the corrosion media and a reasonable improvement when using SP.

19. The Dignity of Humanity

Abstract

This chapter argues that the normative significance of humanity is not to be understood in axiological terms (and that it is hence somewhat misleading to speak of the “value of humanity”) but is instead to be construed in distinctly deontological terms (and that it is accordingly preferable to speak of the “status of humanity”). It argues, in particular, that humanity has dignity insofar as humanity is the ground of being a member of the domain over which maxims have to be universalizable. Since the domain of universalization would be empty if there should not be anything having dignity, humanity classifies as the ground of the categorical imperative.

Abstract

Necroptosis is a lytic and inflammatory form of cell death that is highly constrained to mitigate detrimental collateral tissue damage and impaired immunity. These constraints make it difficult to define the relevance of necroptosis in diseases such as chronic and persistent viral infections and within individual organ systems. The role of necroptotic signalling is further complicated because proteins essential to this pathway, such as receptor interacting protein kinase 3 (RIPK3) and mixed lineage kinase domain-like (MLKL), have been implicated in roles outside of necroptotic signalling. We sought to address this issue by individually defining the role of RIPK3 and MLKL in chronic lymphocytic choriomeningitis virus (LCMV) infection. We investigated if necroptosis contributes to the death of LCMV-specific CD8+ T cells or virally infected target cells during infection. We provide evidence showing that necroptosis was redundant in the pathogenesis of acute forms of LCMV (Armstrong strain) and the early stages of chronic (Docile strain) LCMV infection in vivo. The number of immune cells, their specificity and reactivity towards viral antigens and viral loads are not altered in the absence of either MLKL or RIPK3 during acute and during the early stages of chronic LCMV infection. However, we identified that RIPK3 promotes immune dysfunction and prevents control of infection at later stages of chronic LCMV disease. This was not phenocopied by the loss of MLKL indicating that the phenotype was driven by a necroptosis-independent function of RIPK3. We provide evidence that RIPK3 signaling evoked a dysregulated type 1 interferone response which we linked to an impaired antiviral immune response and abrogated clearance of chronic LCMV infection.

Abstract

Background Medical education in Syria still adopts a traditional, teacher-centered curriculum to this day. These elements imply the existence of issues in the learning environment (LE). This study aims to provide the first evaluation of the LE at the largest medical schools in Syria using the DREEM inventory. Methods The three largest medical schools in Syria are the ones at Damascus University (DU), University of Aleppo (AU), Tishreen University (TU). The Arabic version of the DREEM questionnaire was used. Students across all years of study except year 1 were approached. Both paper-based and electronic surveys were conducted. Results A total of 1774 questionnaire forms were completed (DU:941, AU:533, TU: 300). The overall DREEM score at DU, AU, and TU were 100.8 ± 28.7, 101.3 ± 31.7, and 97.8 ± 35.7 respectively with no significant difference (P = 0.254) between the three universities. DREEM subscales concerning Learning, Atmosphere, Academic Self-perception and Social Self-perception had a low score across all universities. Clinical-stage students reported significantly lower perception (P ≤ 0.001) of the LE in comparison to their pre-clinical counterparts across all subscales. Conclusions The findings of this study highlight the significant shortcomings of the medical LE in Syria. If not addressed properly, the academic, clinical, and professional competence of the healthcare workforce will continue to deteriorate. Moreover, the negative LE might be a predisposing factor for medical students’ exodus. The Syrian medical education system requires leaders who are willing to defy the status quo to achieve a true educational transformation.

Abstract

Background Older adults may have new care needs and functional limitations after surgery. Many rely on informal caregivers (unpaid family or friends) after discharge but the extent of informal support is unknown. We sought to examine the role of informal postoperative caregiving on transitions of care for older adults undergoing routine surgical procedures. Materials and methods We performed a retrospective cohort study using ACS NSQIP Geriatric Pilot Project data, 2014-2018. Patients were ≥65 years and underwent an inpatient surgical procedure. Patients who lived at home alone were compared with those who lived with support from informal caregivers (family and/or friends). Primary outcomes were discharge destination (home vs. post-acute care) and readmission within 30 days. Multivariable logistic regression was used to determine the association between support at home, discharge destination, and readmission. Results Of 18,494 patients, 25% lived alone before surgery. There was no difference in loss of independence (decline in functional status or new use of mobility aid) after surgery between patients who lived alone or with others (18.7% vs. 19.5%, p = 0.24). Nevertheless, twice as many patients who lived alone were discharged to a non-home location (10.2% vs. 5.1%; OR: 2.24, CI: 1.93-2.56). Patients who lived alone and were discharged home with new informal caregivers had increased odds of readmission (OR: 1.43, CI: 1.09-1.86). Conclusion Living alone independently predicts discharge to post-acute care, and patients who received new informal caregiver support at home have higher odds of readmission. These findings highlight opportunities to improve discharge planning and care.

Abstract

American Society of Anesthesiologists guidelines recommend that anesthesiologists revisit do-not-resuscitate orders preoperatively and revise them if necessary based on patient preferences. In patients without do-not-resuscitate orders or other directives limiting treatment however, “full code” is the default option irrespective of clinical circumstances and patient preferences. It is time to revisit this approach based on (1) increasing understanding of the power of default options in healthcare settings, (2) changing demographics and growing evidence suggesting that an expanding subset of patients is vulnerable to poor outcomes after perioperative cardiopulmonary resuscitation (CPR), and (3) recommendations from multiple societies promoting risk assessment and goal-concordant care in older surgical patients. The authors reconsider current guidelines in the context of these developments and advocate for an expanded approach to decision-making regarding CPR, which involves identifying high-risk elderly patients and eliciting their preferences regarding CPR irrespective of existing or presumed code status.

Abstract

Pluralists argue for the distinctness of coinciding objects on the grounds that they have different properties. The grounding problem is the problem of explaining how the supposed difference in properties can arise in the first place. This paper considers this problem as an instance of a more general phenomenon, namely, the problem of dealing with underdetermination in asymmetrical systems admitting of non-trivial automorphisms. It argues in favour of primitivism by developing an account of stochastic grounding that makes room for non-fundamental bruteness and substantially mitigates the costs of primitivism.

38. A proposal for virtual, telephone-based preoperative cognitive assessment in older adults undergoing elective surgery

Abstract

Objective To assess the feasibility of administering the MoCA 5-minute test/Telephone (T-MoCA), an abbreviated version of the Montreal Cognitive Assessment to older adults perioperatively Design A feasibility study including patients aged ≥ 70 years scheduled for surgery from December 2020 to March 2021 Setting Preoperative virtual clinic Patients Patients ≥70 years undergoing major elective surgery Intervention A study investigator called eligible patients prior to surgery, obtained consent, and completed the preoperative cognitive assessment. Follow-up assessment was completed 1-month postoperatively, and participating clinicians were surveyed at the completion of the study. Measurements An attention test, T-MoCA, Activities of Daily Living (ADL), Instrumental Activities of Daily Living (IADL), and Generalized Anxiety Disorder 2-item (GAD-2) Main results Overall, 37/40 (92.5%) patients completed the pre- and post-operative assessments. The cohort was 50% female, white (97.5%), with a median age of 76 years (interquartile range (IQR) 73-79), and education level was higher than high school in 82.5% of patients. Preoperatively, the median number of medications was 8 (IQR 7-11), 27/40 (67.5%) had medications with anticholinergic effects, and 6/40 (15%) had benzodiazepines. Median completion time of the phone assessment was 10 min (IQR 8.25-12) and 4 min (IQR 3-5) for the T-MoCA with a median T-MoCA score of 13 (IQR 12-14). Most patients (37/40) completed the post-operative assessment, and 6/37 (16.2%) reported they had experienced a change in memory or attention post-operatively. Clinician’s survey reported ease and feasibility in performing T-MoCA as a preoperative cognitive evaluation. Conclusion Preoperative cognitive assessment of older adults using T-MoCA over the phone is easy to perform by clinicians and had a high completion rate by patients. This test is feasible for virtual assessments. Further research is needed to better define validity and correlation with postoperative outcomes.

40. Noumena as Grounds of Phenomena

Abstract

The debate between one-world and two-worlds interpretations of transcendental idealism generally proceeds on both systematic and exegetical grounds. A common objection to two-worlds interpretations is that they are committed to an implausible, if not incoherent, ontology and that considerations of charity consequently speak against attributing such a view to Kant. This chapter focuses primarily on systematic rather than exegetical considerations. It aims to develop an account of the relation between noumena and phenomena that makes sense of a two-world ontology. In particular, the goal is to elucidate this relation in such a way that it neither undermines noumenal ignorance nor ontologizes space and time. Far from being incoherent or implausible, the resulting theory constitutes a powerful metaphysical system.

Abstract

Context Optimal surgical care for older adults with life-threatening conditions, with high risk of poor perioperative outcomes and morality in the months after surgery, should incorporate an understanding of the patient's treatment goals and preferences. However, little research has explored the patient perspective of decision making and advanced care planning during an emergency surgery episode. Objectives We sought to better understand older patients' lived experience making decisions to undergo emergency general surgery (EGS) and perceptions of perioperative advance care planning (ACP). Methods Adults aged 65 and older who underwent one of seven common EGS procedures with lengths of stay more than five days at three Boston-area hospitals were included. Semistructured phone interviews were conducted three months postdischarge. Transcripts were reviewed and coded independently by surgeons and palliative care physicians to identify themes. Results About 31 patients were interviewed. Patients viewed the decision for surgery as a choice of life over death and valued prolonging life. They felt there was no choice but to proceed with surgery but reported that participation in decision making was limited because of severe symptoms, time constraints, and confused thinking. Despite recently surviving a life-threatening illness, patients had not reconsidered their wishes for the future and preferred to avoid future ACP. Conclusion Older patients who survived a life-threatening illness and EGS report receiving goal-concordant care in the moment that relieved symptoms and prolonged life but had not considered future care. Interventions to facilitate postoperative ACP should be targeted to this vulnerable group of older adults.

Abstract

Background Preoperative goals of care (GOC) and code status (CS) discussions are important in achieving an in-depth understanding of the patient's care goals in the setting of a serious illness, enabling the clinician to ensure patient autonomy and shared decision making. Past studies have shown that anesthesiologists are not formally trained in leading these discussions and may lack the necessary skill set. We created an innovative online video curriculum designed to teach these skills. This curriculum was compared to a traditional method of learning from reading the medical literature. Methods In this bi-institutional randomized controlled trial at 2 major academic medical centers, 60 anesthesiology trainees were randomized to receive the educational content in 1 of 2 formats: (1) the novel video curriculum (video group) or (2) journal articles (reading group). Thirty residents were assigned to the experimental video curriculum group, and 30 were assigned to the reading group. The content incorporated into the 2 formats focused on general preoperative evaluation of patients and communication strategies pertaining to GOC and CS discussions. Residents in both groups underwent a pre- and postintervention objective structured clinical examination (OSCE) with standardized patients. Both OSCEs were scored using the same 24-point rubric. Score changes between the 2 OSCEs were examined using linear regression, and interrater reliability was assessed using weighted Cohen's kappa. Results Residents receiving the video curriculum performed significantly better overall on the OSCE encounter, with a mean score of 4.19 compared to 3.79 in the reading group. The video curriculum group also demonstrated statistically significant increased scores on 8 of 24 rubric categories when compared to the reading group. Conclusions Our novel video curriculum led to significant increases in resident performance during simulated GOC discussions and modest increases during CS discussions. Further development and refinement of this curriculum are warranted.

Abstract

Previous work has shown that it is possible to guarantee a reachable workspace for a kinematically redundant robot after an arbitrary locked-joint failure if one artificially restricts the range of its joints prior to the failure. Identifying the optimal articial joint limits has been the subject of previous work to maximize this so-called “failure-tolerant workspace.” Unfortunately, these techniques are not feasible for a highly redundant robot operating in a spatial workspace. This work presents a novel hybrid technique for estimating the failure-tolerant workspace size for robots of arbitrary kinematic structure and any number of degrees of freedom performing tasks in a 6D workspace. The method presented combines an algorithm for computing self-motion manifold ranges to estimate workspace envelopes and Monte-Carlo integration to estimate orientation volumes to create a computationally efficient algorithm. This algorithm is then combined with the coordinate ascent optimization technique to determine optimal artificial joint limits that maximize the size of the failure-tolerant workspace of a given robot. This approach is illustrated on multiple examples of robots that perform tasks in 3D planar and 6D spatial workspaces.

Abstract

This paper presents the gravitational-wave measurement of the Hubble constant (H 0) using the detections from the first and second observing runs of the Advanced LIGO and Virgo detector network. The presence of the transient electromagnetic counterpart of the binary neutron star GW170817 led to the first standard-siren measurement of H 0. Here we additionally use binary black hole detections in conjunction with galaxy catalogs and report a joint measurement. Our updated measurement is H 0 = km s-1 Mpc-1 (68.3% of the highest density posterior interval with a flat-in-log prior) which is an improvement by a factor of 1.04 (about 4%) over the GW170817-only value of km s-1 Mpc-1. A significant additional contribution currently comes from GW170814, a loud and well-localized detection from a part of the sky thoroughly covered by the Dark Energy Survey. With numerous detections anticipated over the upcoming years, an exhaustive understanding of other systematic effects are also going to become increasingly important. 49. Person-Affecting Utilitarianism

Abstract

This chapter is concerned with person-affecting views in population ethics which require the betterness ordering of populations to consist in betterness facts for the members of the populations and thereby require the evaluation of distributions to be reducible to facts about personal good. The first part argues that impersonal versions of utilitarianism involve an objectionable axiology precisely because they are incompatible with a person-affecting view. In particular, the problem is that they do not take personal good seriously, since they do not attribute ethical significance to it, but instead (at best) only consider it to be ethically relevant. As a result, they end up subordinating and sacrificing personal good for the sake of impersonal good and thereby treat persons as mere containers of impersonal good, which gives rise to particularly troubling implications in the case of variable-population comparisons. The second part evaluates the prospects for person-affecting versions of utilitarianism. It will be argued that person-affecting total utilitarianism presupposes comparativism, that is, that existence and non-existence are comparable in terms of personal good, and as such involves various problematic metaphysical commitments. Accordingly, same-number person-affecting utilitarianism turns out to be the only version of utilitarianism that neither has objectionable axiological implications nor requires problematic metaphysical commitments.
