Background: Acute lymphoblastic leukemia with MLL/AF4 rearrangement remains a major hurdle to improving outcomes. Gene network and circRNAs have been found to participate in tumorigenesis, while their roles in leukemia still need to be explored. Recent patents have shown that circRNAs exhibit the markers for the children ALL, although the target and related mechanism remain to be elucidated. Objective: This study aims to explore the possible targets and mechanisms of ALL with MLLAF4 rearrangement. Methods: We first generated a gene network focusing on MLL-AF4 rearrangement. Cell viability was determined with Cell Counting Kit-8 assay. The cell apoptosis was tested by the Annexin V/PI assay. The RNA-protein complexes were analyzed by qRT-PCR, and the pathway proteins were analyzed by western blot. Results: This gene network was associated with biological processes, such as nucleic acid metabolism and immunity, indicating its key role in inflammation. We found that circ_0008012 was upregulated in MLL/AF4 ALL cells and regulated cell proliferation and apoptosis. Further computed simulation and RIP showed that IKKβ was the strongest protein in the NF-κB pathway binding with circ_0008012. As a result, possible regulation of circ_0008012 is suggested by binding IKKβ in the IKKα:IKKβ:IKKγ compound, which then phosphorylates IκB and activates NF- κB:p65:p300 compound in cell nucleus, thereby leading to leukemia. Conclusion: We identified a gene network for MLL/AF4 ALL. Moreover, circ_0008012 may be a therapeutic target for this subtype of ALL.

Both servitization and digitization are mainstream types of business model innovation. In recent years, there have been an increasing number of studies integrating these two research topics to develop a series of research on digital servitization. Facing the impact of the COVID-19 pandemic, the development of digital servitization capabilities in the manufacturing industry can help companies provide smart product-service systems (PSSs) to address uncertainties and shocks in the external environment. This study aims to explore "the dilemmas and response strategies of digital servitization” using a qualitative multiple case study comparative analysis. Through in-depth interviews, we identified the difficulties and bottlenecks that small and medium-sized manufacturing enterprises (SMEs) may encounter in the process of digital servitization and proposed corresponding response strategies. The results of this study contribute to the literature on digital servitization by accumulating and extending upon existing research. JEL classification numbers: C83, M15, O31. This study aims to discuss the critical success factors of introducing ISO international quality management system, to provide reference for Taiwan's traditional industries when they introduce the ISO international quality management system, and reduce various costs and time process of the introduction. Firstly, this study collects constructs from several relevant literatures, and categorizes 5 primary constructs and 20 secondary constructs based on the actual business situation of Taiwan’s traditional industry. In the second stage, Likert’s five-point scale is used to extract 3 secondary constructs with the highest scores under the 5 primary constructs respectively. The Fuzzy Analytic Hierarchy Process (FAHP) is used to carry out questionnaire survey and obtain the relative weights of various primary and secondary constructs. It obtains the relative weights of 5 primary constructs and several secondary constructs, the primary constructs are “policy-related factors”, “technology-related actors”, “people-related factors”, “documents and data”, and “management review”. Furthermore, it extracts 15 secondary constructs and puts forth 6 propositions as research conclusion. It is hoped that the findings can provide important reference basis for the introduction of ISO international quality management system in Taiwan’s manufacturing industry, and thus speed up the time process and reduce cost of the introduction. However, the mechanisms through which decent work affects workplace deviance have yet to be fully understood. Drawing upon relative deprivation theory, this study examines the relationship between decent work and workplace deviance by focusing on the mediating role of relative deprivation and the moderating role of career calling.Methods: Two waves of data from 307 doctors and 61 supervisors of public hospitals in central China were examined using a the moderated mediation model.Results: Decent work is negatively associated with workplace deviance via the mediation of relative deprivation, and the negative relationship between decent work and relative deprivation is negatively moderated by career calling. The mediating role of relative deprivation is moderated by career calling, and the indirect effect is stronger when career calling is high than when it is low.Conclusion: By focusing on the mediating role of relative deprivation and the moderating role of career calling, this study contributes to the literature by addressing the previously unexamined relationship between decent work and workplace deviance. This study aimed to explore the humanistic nursing needs of patients with stroke and their families and provide a basis for the construction of humanistic nursing practice guidelines for advanced stroke.Methods: From September to October 2019, using the descriptive phenomenological research method, a semi-structured interview outline was formed based on a literature review and subject group discussions. A targeted sampling method was used to investigate 21 patients with stroke and 21 family members, respectively, and their care needs were investigated in depth. Colaizzi’s thematic cluster analysis was used to analyse the data, and a total of 6 themes and 14 sub-themes were summarised.Results: The 6 themes and 14 sub-themes were as follows: ①psychological care, ② security concerns (professional personal integrity, timely response), ③ emotional care (service consciousness, positive incentives, empathy, effective communication, provision of a communication platform, personalised care), ④ respect for rights, ⑤ rehabilitation care (rehabilitation programme, professional guidance, rehabilitation configuration, continuation of services) and ⑥ family care (physical and psychological support, care guidance).Conclusion: Among the humanistic care needs of patients with stroke and their families, the most basic physiological care accounts for the largest proportion of emotional and rehabilitation care, followed by safety care, respect for rights and family care. Based on the actual humanistic care needs of patients with stroke and their families, the practical effect of humanistic care in stroke wards can be improved. This study provides a reference for the construction of humanistic nursing practice guidelines for late stroke.Keywords: stroke, humanistic care, qualitative research

In the paper, YSZ coatings were prepared on nickel-based alloy substrates by anodic electrophoretic deposition. The YSZ suspension solution was obtained under stirring and ultrasonic treatment, in which the anhydrous ethanol and acetylacetone were used as the dispersion medium and ammonium polyacrylate was used as the dispersant of the suspension. The effects of different deposition voltage and deposition time on YSZ coating were investigated. Meantime, the microstructure of the coating surface was observed by metallographic microscope. It was found that the high-quality YSZ coating could be obtained by deposition at 60 V for 2–3 min. Finally, the effect of sintering temperature on coating quality was investigated by X-ray diffractometer and scanning electron microscopy. The results showed that the YSZ coating bonded closely with the substrate after sintering at 1200 oC, and the porosity of the YSZ coating increased after sintering

In brief Impaired spermatogenesis resulting from disturbed cholesterol metabolism due to intake of high-fat diet (HFD) has been widely recognized, however, the role of preprotein invertase subtilin 9 (PCSK9), which is a negative regulator of cholesterol metabolism, has never been reported. This study aims to reveal the role of PCSK9 on spermatogenesis induced by HFD in mice. Abstract Long-term consumption of a high-fat diet (HFD) is an important factor that leads to impaired spermatogenesis exhibiting poor sperm quantity and quality. However, the mechanism of this is yet to be elucidated. Disrupted cholesterol homeostasis is one of many crucial pathological factors which could contribute to impaired spermatogenesis. As a negative regulator of cholesterol metabolism, preprotein invertase subtilin 9 (PCSK9) mediates low density lipoprotein receptor (LDLR) degradation to the lysosome, thereby reducing the expression of LDLR on the cell membrane and increasing serum low-density lipoprotein cholesterol level, resulting in lipid metabolism disorders. Here, we aim to study whether PCSK9 is a pathological factor for impaired spermatogenesis induced by HFD and the underlying mechanism. To meet the purpose of our study, we utilized wild-type C57BL/6 male mice and PCSK9 knockout mice with same background as experimental subjects and alirocumab, a PCSK9 inhibitor, was used for treatment. Results indicated that HFD induced higher PCSK9 expression in serum, liver, and testes, and serum PCSK9 is negatively correlated with spermatogenesis, while both PCSK9 inhibitor treatment and PCSK9 knockout methodologies ameliorated impaired lipid metabolism and spermatogenesis in mice fed a HFD. This could be due to the overexpression of PCSK9 induced by HFD leading to dyslipidemia, resulting in testicular lipotoxicity, thus activating the Bcl-2–Bax–Caspase3 apoptosis signaling pathway in testes, particularly in Leydig cells. Our study demonstrates that PCSK9 is an important pathological factor in the dysfunction of spermatogenesis in mice induced by HFD. This finding could provide innovative ideas for the diagnosis and treatment of male infertility.

Clinical data on carbapenem-resistant Enterobacterales (CRE) bacteremia in the pediatric population are limited. This study investigated the clinical characteristics and outcomes of pediatric CRE bacteremia.Clinical data on bacteremia caused by carbapenem-susceptible and carbapenem-resistant Enterobacterales, including Escherichia coli, Klebsiella spp., Enterobacter spp., Serratia marcescens, Proteus mirabilis, Citrobacter spp., and Morganella spp., in pediatric patients from a children's hospital in Taiwan were retrospectively retrieved and analyzed.From January 2013 to December 2021, 471 clinical isolates of Enterobacterales bacteremia were identified in 451 episodes from 379 pediatric patients. Among all the isolates, the predominant species were E. coli (199/471, 42.2%), Klebsiella spp. (168/471, 35.6%), and Enterobacter spp. (59/471, 12.5%), with carbapenem-resistance rates of 1.5%, 11.9%, and 25.0%, respectively. Overall, 40 (8.4%) showed a carbapenem resistance phenotype. Patients' all-cause mortality rate at 14 days was significantly higher in CRE bacteremia episodes than non-CRE ones (12.5% vs. 3.6%, p 0.05). The predicting factor of a CRE bacteremia episode was the causative agent of Enterobacter spp. (adjusted OR of 2.551, CI 1.073-6.066, p 0.05) and ESBL-producing phenotype (adjusted OR 14.268, CI 5.120-39.762, p 0.001).Bloodstream infections caused by CRE are associated with a higher mortality rate in the pediatric population. Attention must be paid to preventing and managing pediatric patients with CRE infections.

Numerous incidents and failures of earth slopes, dykes, levees and embankment dams are caused by internal erosion. At a time of global climate change, rapid pore-water pressure changes in soil masses are frequently induced by extreme rainfall, storm surges, waves, flash floods and so on. Under such complex cyclic hydraulic conditions, the soil erodibility, hydraulic conductivity and subsequent stress–strain behaviour may differ from those under the monotonic seepage condition, and are still poorly understood. In this study, preliminary laboratory tests have been conducted to investigate the development of internal erosion and changes in hydraulic conductivity under one-way cyclic seepage, and the post-erosion stress–strain behaviour. Representative internally stable and unstable soils were tested. The effects of mean hydraulic gradient and cyclic gradient amplitude were investigated in detail. Results show that the erosion development is significantly influenced by both the initial grain size distribution and the pattern of imposed cyclic hydraulic gradient. The cyclic seepage promotes the loss of fine particles and leads to larger hydraulic conductivity. For the internally unstable soil, the eroded soil mass and hydraulic conductivity increased significantly with increasing cyclic gradient amplitude. Nevertheless, the internally stable soil experienced a small amount of particle loss, even under large cyclic gradients. For both types of soil, a larger cyclic gradient amplitude corresponds to a stronger post-erosion contractive shearing behaviour and a smaller critical friction angle.

Post-stroke chronic stress (PSCS) is generally associated with the poorer recovery and more pronounced cognitive dysfunction. Recent evidence has implied that S-ketamine can reduce suicidal ideation in treatment-resistant depression. In this current study, we aimed to investigate whether the administration of S-ketamine ameliorated cognitive deficits under PSCS conditions, which was established by a model combining middle cerebral artery occlusion (MCAO) and chronic restraint stress. Our data suggested that mice exposed to PSCS exhibited depression-like behavior and cognitive impairment, which coincided with astrocytosis as indicated by increased GFAP-positive cells and impairment of long-time potentiation (LTP) in the hippocampal CA1. Subanesthetic doses (10 mg/kg) of S-ketamine have significantly mitigated depression-like behaviors, cognitive deficits and LTP impairment, reduced astrocytosis, excessive GABA, and inflammatory factors, including NLRP3 and IL-18 in astrocytes in the CA1. Besides, neuroprotective effects induced by S-ketamine administration were found in vitro but could be partially reversed by an agonist of the NLRP3 nigericin. Our current data also suggests that the subanesthetic doses of S-ketamine improved cognitive dysfunction via the inhibition of hippocampal astrocytosis in a mouse model of PSCS.

Postoperative ileus (POI) is a relatively common complication after spinal fusion surgery, which can lead to delayed recovery, prolonged length of stay and increased medical costs. However, little is known about the incidence and risk factors of POI after corrective surgery for patients with adolescent idiopathic scoliosis (AIS). This study was performed to report the incidence of POI and identify the independent risk factors for POI after postoperative corrective surgery.In this retrospective cohort study, A total of 318 patients with AIS who underwent corrective surgery from April 2015 to February 2021 were enrolled and divided into two groups: those with POI and those without POI. The Student's t test, Mann-Whitney U test, and Pearson's chi-square test were used to compare the two groups regarding patient demographics and preoperative characteristics (age, sex and the major curve type), intraoperative and postoperative parameters (lowest instrumented vertebra [LIV], number of screws, and length of stay), radiographic parameters (T5-12 thoracic kyphosis [TK], T10-L2 thoracolumbar kyphosis and height [TLK and T10-L2 height], L1-S1 lumbar lordosis [LL], and L1-5 height). Then, a multivariate logistic regression analysis was used to identify independent risk factors for POI, and a receiver operating characteristic (ROC) curve was performed to assess the predictive values of these risk factors.Forty-two (13.2%) of 318 patients who developed POI following corrective surgery were identified. The group with POI had a significantly longer length of stay, more lumbar screws, higher proportions of a major lumbar curve and lumbar anterior screw breech, and a lower LIV. Among radiographic parameters, the mean lumbar Cobb angle at baseline, the changes in the lumbar Cobb angle, and T10-L2 and L1-5 height from before to after surgery were significantly larger in the group with POI than in the group without POI. Multivariate logistic regression analysis showed that large changes in T10-L2 (odds ratio [OR] =2.846, P = 0.007) and L1-5 height (OR = 31.294, p = 0.000) and lumbar anterior screw breech (OR = 5.561, P = 0.006) were independent risk factors for POI. The cutoff values for the changes in T10-L2 and L1-5 height were 1.885 cm and 1.195 cm, respectively.In this study, we identified that large changes in T10-L2 and L1-5 height and lumbar anterior screw breech were independent risk factors for POI after corrective surgery. Improving the accuracy of pedicle screw placement might reduce the incidence of POI, and greater attention should be given to patients who are likely to have large changes in T10-L2 and L1-5 height after corrective surgery.
