Rapid antigen tests in the form of lateral flow devices (LFDs) allow testing of a large population for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). To reduce the variability in device interpretation, we show the design and testing of an artifical intelligence (AI) algorithm based on machine learning. The machine learning (ML) algorithm is trained on a combination of artificially hybridized LFDs and LFD data linked to quantitative real-time PCR results. Participants are recruited from assisted test sites (ATSs) and health care workers undertaking self-testing, and images are analyzed using the ML algorithm. A panel of trained clinicians is used to resolve discrepancies. In total, 115,316 images are returned. In the ATS substudy, sensitivity increased from 92.08% to 97.6% and specificity from 99.85% to 99.99%. In the self-read substudy, sensitivity increased from 16.00% to 100% and specificity from 99.15% to 99.40%. An ML-based classifier of LFD results outperforms human reads in assisted testing sites and self-reading.

Arrhythmia is a life-threatening disease that leads to complex physical condition in patients if left untreated. Arrhythmia disorders should be diagnosed early enough to save people's life. Noninvasive and remote monitoring of cardiac arrhythmia problems is now possible to resolve due to the Internet of Things(IoT). IoT has the potential to simplify the healthcare domain. An IoT network for cardiovascular disease prediction retrieves the ECG signals, processes them, and warns physicians in an emergency using an IoT-enabled ECG telemetry structure. It is beneficial for the physician to detect heart failure at the earliest. This research work proposes an IoT-enabled ECG monitoring structure to analyze the ECG signals. The Pan Tompkins Q-R-S detection algorithm analyzes the signal to obtain the ECG signal's complex features. To get heart rate variability features, the expedient can be used to locate R-R intervals from an ECG signal. Dynamic Features and statistical are considered to categorize cardiac arrhythmia syndrome. People can monitor their cardiac disease regularly at home through ECG signals. The system is lightweight, so it needs less maintenance and is less expensive to run. It's beneficial for the surgeon to scrutinize the heart condition efficiently and consistently.

Background Appendicitis is the most common general surgical emergency worldwide, but its diagnosis remains challenging. The aim of this study was to determine whether existing risk prediction models can reliably identify patients presenting to hospital in the UK with acute right iliac fossa (RIF) pain who are at low risk of appendicitis. Methods A systematic search was completed to identify all existing appendicitis risk prediction models. Models were validated using UK data from an international prospective cohort study that captured consecutive patients aged 16–45 years presenting to hospital with acute RIF in March to June 2017. The main outcome was best achievable model specificity (proportion of patients who did not have appendicitis correctly classified as low risk) whilst maintaining a failure rate below 5 per cent (proportion of patients identified as low risk who actually had appendicitis). Results Some 5345 patients across 154 UK hospitals were identified, of which two‐thirds (3613 of 5345, 67·6 per cent) were women. Women were more than twice as likely to undergo surgery with removal of a histologically normal appendix (272 of 964, 28·2 per cent) than men (120 of 993, 12·1 per cent) (relative risk 2·33, 95 per cent c.i. 1·92 to 2·84; P, Women in the UK had a disproportionate risk of admission without surgical intervention and had high rates of normal appendicectomy. Aims: The COVID-19 pandemic led to hospitals in the United Kingdom substituting face-to-face (FtF) clinics with virtual clinic (VC) appointments. We evaluated the impact of virtual two-week wait (2-ww) lower gastrointestinal (LGI) consultations on stakeholders at a district general hospital in England. Methods: Patients undergoing index outpatient 2-ww LGI clinic assessment between 01/06/2019-31/10/2019 (FtF group) and 01/06/2020-31/10/2020 (VC group) were identified. Relevant data were obtained using electronic patient records. Compliance with national cancer waiting time targets (WTT) was assessed. Environmental and financial impact analyses were performed. Results: In total, 1531 patients were analysed (median age=70, male=852, 55.6%). Of these, 757 (49.4%) were assessed virtually via telephone; the remainder were seen FtF (n=774, 50.6%). Ninety two (6%, VC=44, FtF=48) patients had malignant pathology and 64 (4.2%) had colorectal cancer (CRC); of these, 46 (71.9%, VC=26, FtF=20) underwent treatment with curative intent. In this paper we report a study of the isotopic composition of Li, Be, B and N, Ne nuclei from a 5 year time period beyond the heliopause using the CRS instruments on Voyager. By comparing the isotopic ratios, 15N/14N and 22Ne/20Ne outside the heliosphere as measured at Voyager, and which are found to be significantly lower than those measured at the same energy inside the heliosphere, we have provided strong evidence that cosmic rays of this energy have lost as much as 200 MeV/nuc or more in the solar modulation process. This is in accordance with the so called force field description of this overall modulation by Gleeson and Axford. The measurements at Voyager confirm that the unusual 14N and 22Ne cosmic ray source abundances relative to solar abundances made earlier inside the heliosphere extend to the lower energies not accessible from near Earth measurements. The low energy Li, Be and B nuclei, which are believed to be purely secondary nuclei, are found to have a (previously unobservable) peak in the differential intensity spectrum at ~100 MeV/nuc. This is in agreement with propagation predictions. The intensities of these nuclei are ~10-20% higher than those predicted in a propagation model with a matter path length lambda = 9 g/cm2 at these low energies. We have measured the intensity and spectra of the cosmic ray secondary isotopes 2H and 3He and the primary isotopes H and 4He between 20-85 MeV/nuc during a 5 year time period after Voyager 1 (V1) crossed the heliopause. The data reported here is from the B-end high energy telescope. The ratios of the intensities of the secondary to primary spectra of these nuclei at low energies are sensitive indicators for determining the amount of matter traversed at these energies by their galactic cosmic ray progenitor nuclei, after acceleration, in this case mostly 4He nuclei. The measurements of secondary 3He abundances indicate that cosmic ray 4He of energies between 30-100 MeV/nuc have traversed between 7-9 g/cm2 of interstellar matter (90% H, 10% He) in a Leaky Box propagation model. This path length is also consistent with the production of secondary 2H nuclei between 20-50 MeV/nuc, which is also produced mainly by 4He in a LBM. The Boron abundance, studied in separate papers, is also consistent with this path length at energies >30 MeV/nuc. These secondary intensities imply that the interstellar cosmic ray path length may be described in a LBM in a manner consistent with a mean path length,gamma= 20.6 beta P-0.45 at rigidities above ~0.5 GV (30 MeV/nuc for A/Z=2 nuclei). We have obtained the energy spectra of cosmic ray He, B, C, O, Mg, S and Fe nuclei in the range 0.5-1.5 GeV/nuc and above using the penetrating particle mode of the High Energy Telescope, part of the Cosmic Ray Science (CRS) experiment on Voyagers 1 and 2. The data analysis procedures are the same as those used to obtain similar spectra from the identical V2 HET telescope while it was in the heliosphere between about 23 and 54 AU. The time period of analysis includes 4 years of data beyond the heliopause (HP). These new interstellar spectra are compared with various earlier experiments at the same energies at the Earth to determine the solar modulation parameter, phi. These new spectra are also compared with recent measurements of the spectra of the same nuclei measured by the same telescope at low energies. It is found that the ratio of intensities at 100 MeV/nuc to those at 1.0 GeV/nuc are significantly Z dependent. Some of this Z dependence can be explained by the Z2 dependence of energy loss by ionization in the 7-10 g/cm2 of interstellar H and He traversed by cosmic rays of these energies in the galaxy; some by the Z dependent loss due to nuclear interactions in this same material; some by possible differences in the source spectra of these nuclei and some by the non-uniformity of the source distribution and propagation conditions. The observed features of the spectra, also including a Z dependence of the peak intensities of the various nuclei, pose interesting problems related to the propagation and source distribution of these cosmic rays., 35 pages, 5 tables, 14 figures

Rosai dorfman disease was defined by Rosai and Dorfman as an idiopathic histiocytic proliferative disorder afflicting nodes in the year 1969. The disease presents in the 10-20 year age group mainly as fever, painless lymphadenopathy, and hypergammaglobulinaemia, however older ages can also be affected. Although it is mainly a node involving disease, 25% extranodal cases have been reported worldwide. The eyes and ocular adnexa, head and neck region upper respiratory tract, skin and subcutaneous tissue, and central nervous system are the commonly involved extranodal sites. We present a case of extranodal testicular Rosai Dorfman in a 40 year old male with right testicular lump which radiologically mimicked a malignancy but on histopathology was diagnosed to be a benign Testicular Rosai Dorfman.

One-dimensional nanostructures are of particular interest in nanoelectronics because of the ease with which they can be utilized in fabricating nanodevices where their long axis facilitates contact to the structure, while the short axis preserves the quantized nature of electronic levels. It is therefore desirable to synthesize precisely controlled semiconductor and metal nanowires to study their properties. With much advancement achieved in producing high quality and appreciably controlled semiconductor nanowires, approaches to precisely control the dimensions of metal nanowires still needs to be explored. Here, we have investigated room-temperature sensing properties of these gold nanowires for hydrogen, ethanol and NH3. The sensitivity and selectivity of the wires for sensing different gases are explored. The sensing devices were fabricated by drop-casting the nanowire dispersion over pre-patterned electrodes ultimately paving the path for cost effective applications of these nanowires. Simple chemical routes and solution processing techniques has been utilized for their subsequent device applications. The driving force behind the solution processed devices is their low cost, large device area, physical flexibility and compatibility with the existing technologies.

25. At Voyager 1 Starting on about August 25, 2012 at a Distance of 121.7 AU From the Sun, a Sudden Disappearance of Anomalous Cosmic Rays and an Unusually Large Sudden Increase of Galactic Cosmic Ray H and He Nuclei and Electron Occurred

At the Voyager 1 spacecraft in the outer heliosphere, after a series of complex intensity changes starting at about May 8th, the intensities of both anomalous cosmic rays (ACR) and galactic cosmic rays (GCR) changed suddenly and decisively on August 25th (121.7 AU from the Sun). The ACR started the intensity decrease with an initial e-folding rate of intensity decrease of ~1 day. Within a matter of a few days, the intensity of 1.9-2.7 MeV protons and helium nuclei had decreased to less than 0.1 of their previous value and after a few weeks, corresponding to the outward movement of V1 by ~0.1 AU, these intensities had decreased by factors of at least 300-500 and are now lower than most estimates of the GCR spectrum for these lower energies and also at higher energies. The decrease was accompanied by large rigidity dependent anisotropies in addition to the extraordinary rapidity of the intensity changes. Also on August 25th the GCR protons, helium and heavier nuclei as well as electrons increased suddenly with the intensities of electrons reaching levels ~30-50% higher than observed just one day earlier. This increase for GCR occurred over ~1 day for the lowest rigidity electrons, and several days for the higher rigidity nuclei of rigidity ~0.5-1.0 GV. After reaching these higher levels the intensities of the GCR of all energies from 2 to 400 MeV have remained essentially constant with intensity levels and spectra that may represent the local GCR. 26. Large periodic time variations of termination shock particles between ~0.5-20 mev and 6-14 mev electrons measured by the crs experiment on Voyager 2 as it crossed into the heliosheath in 2007: An example of freshly accelerated cosmic rays?

We have examined features in the structure of the heliosheath using the fine scale time variations of termination shock particles (TSP) between ~0.5 - 20 MeV and electrons between 2.5-14 MeV measured by the CRS instrument as the V2 spacecraft crossed the heliospheric termination shock in 2007. The very disturbed heliosheath at V2 is particularly noteworthy for strong periodic intensity variations of the TSP just after V2 crossed the termination shock (2007.66) reaching a maximum between 2007.75 and 2008.0. A series of 42/21 day periodicities was observed at V2 along with spectral changes of low energy TSP and the acceleration of 6-14 MeV electrons. Evidence is presented for the acceleration of TSP and electrons at the times of the 42/21 day periodicities just after V2 crossed the HTS. Spectra for TSP between 2-20 MeV and electrons between 2.5-14 MeV are derived for three time periods including the time of the HTS crossing. The energy spectra of TSP and electrons at these times of intensity peaks are very similar above ~3 MeV, with exponents of a power law spectrum between -3.0 and -3.6. The ratio of TSP intensities to electron intensities at the same energy is ~500. The electron intensity peaks and minima are generally out of phase with those of nuclei by ~1/2 of a 42 day cycle. These charge dependent intensity differences and the large periodic intensity changes could provide new clues as to a possible acceleration mechanism., Comment: 18 pages, 4 Figures

We developed a robust large-signal model for InP/InGaAs HBTs. DC, small-signal, noise and power characteristics of InP/InGaAs HBTs are measured over a wide range of frequencies and bias conditions. A minimum noise figure (FMIN) of 3.5dB, and a gain of 16.8dB are achieved at 10-GHz. These measurement results are the basis for robust nonlinear models of InP/InGaAs HBT devices.

30. Trends in the Use of Tractor Power for Sugarcane Production in Puerto Rico

The types and number of tractors used on a given farm is a fairly reliable indicator of the degree of its mechanization. A field study was conducted to ascertain the trend in the use of tractor power for sugarcane production in Puerto Rico. A total of 957 tractors were included in the study. It was found that there has been a sharp rise in the use of tractor power in sugarcane production in Puerto Rico since 1969 and yearly acquisition of wheel type has been much higher than for tracklayer tractors. Recently acquired tractors average about 100 horsepower per unit.

Se llevó a cabo una serie de pruebas sobre la compactación de los suelos del Valle de Lajas en la finca del agricultor Sr. Pedro Vivoni. Se realizaron experimentos en cinco piezas distintas para determinar el grado de compactación del suelo en el centro del surco como resultado del tránsito de la cortadora J & L y en los costados del banco causada por el paso de los carretones cargados de caña. También se realizaron pruebas en otras tres piezas para determinar la compactación del suelo en el centro del surco causada por el paso de la cortadora J & L cuando el contenido de humedad del suelo se aproxima a su máxima capacidad. Se tomaron al azar lecturas penetrométricas en el centro del surco y en el costado de los bancos antes y después de pasar la cortadora o los carretones según fuera el caso. La humedad del suelo en el centro del surco y en el costado de los bancos se determinó mediante el secado al horno. Bajo las condiciones de las pruebas la compactación causada por la cortadora J & L no fue severa, pero se notó considerable compactación del suelo en el costado de los bancos causada por los carretones.
