Multilateral development banks (MDBs) play a pivotal role in financing water and sanitation infrastructure projects and thus have a major impact on the development of basic services. Although information about the MDBs' investments is publicly available, it is dispersed and not easily comparable. A comprehensive compilation of MDBs' water and sanitation investments has long been lacking. To address this gap, we assess water and sanitation financing by the three MDBs most relevant to Africa and Asia between 1960 and 2020: the World Bank, the African Development Bank, and the Asian Development Bank. We compile a new dataset by drawing on 3,639 water and sanitation projects and assess territorial trends, technology choices, distribution of financial burdens, and reforms to institutional arrangements. We find that MDBs' investments align with changing patterns of urbanization and increasingly finance sanitation infrastructures including non-sewered technologies. However, our results also suggest that institutional reforms have addressed utility efficiency through investment in equipment and skills rather than through increased commercialization and private sector participation. The leverage effect of MDB investment on private financing is negligible, whereas co-financing from local governments dominates.

This paper challenges the traditional perspective of a territorial core–periphery pattern as an organizing principle of space by introducing a relational perspective through an empirical approach. We study spatial processes of knowledge creation among advanced producer services in Germany between 2009 and 2019. We use a unique longitudinal dataset to analyse if German regions have become more integrated into the intra-firm location networks nationally and globally, and how this translates into the processes of centralization and peripheralization. The current conceptualization of core–periphery in Germany is too static and does not reflect the economic development of the regions. We find that there is no trend of urban areas becoming more integrated and peripheral areas less integrated into these networks. Instead, our research finds a discrepancy between the federal territorial classification and the functional orientation of German regions. Regions classified as peripheral have not generally decreased in connectivity to other regions. In contrast, some have increased their connectivity and have become more central in relational terms. Overall, we identified peripheralization and centralization as a relational process with dynamic, overlapping and trans-scalar networks.

Citywide inclusive sanitation (CWIS) is becoming the dominant paradigm for achieving safe sanitation for all by 2030. Its technical benefits have been explored, but the bargaining over financial and organizational changes CWIS entails have not yet been adequately addressed. Our case study explains the stalled rollout of CWIS in Dhaka, Bangladesh. We analyse policy pathways over the past 30 years through a combined territorial political economy and power perspective to understand their effects on equality. We highlight how donors link the introduction of CWIS to the organization of sanitation through a market; how the utility uses CWIS as an opportunity to avoid costly responsibilities in non-sewered sanitation; and how service co-production through community-based solutions is neglected. CWIS has successfully overcome the dogmatic technological focus in the sanitation system, but for citywide sanitation to be scaled inclusively, the dogmatic focus in the organization and financing of the sanitation sector must also be overcome.

Zusammenfassung. Ungeplante Rehospitalisationen stellen eine grosse Belastung für Patientinnen und Patienten, deren Angehörige und das Gesundheitssystem dar. Vor allem seit der Einführung der SwissDRG im Jahr 2012 wurden finanzielle Anreize für Spitäler geschaffen, Wiederaufnahmen zu verhindern. Nicht alle Patientinnen und Patienten haben dasselbe Risiko für eine Rehospitalisation. Mittels Prädiktoren aus verschiedenen Bereichen können Risikopatientinnen und -patienten erkannt werden, um bereits frühzeitig adäquate Interventionen zur Vermeidung einer Wiederaufnahme zu implementieren. Prädiktoren lassen sich bei den Patientinnen und Patienten finden, beispielsweise Polypharmazie und viele Komorbiditäten, oder beim Geschlecht, aber auch behandler- und systembezogene Prädiktoren spielen eine Rolle. Als wirksame Interventionen werden in der Literatur frühe Nachkontrollen und Medikationsüberprüfungen vor Austritt genannt.

Corticothalamic pathways, responsible for the top-down control of the thalamus, have a canonical organization such that every cortical region sends output from both layer 6 (L6) and layer 5 (L5) to the thalamus. Here we demonstrate a qualitative, region-specific difference in the organization of mouse corticothalamic pathways. Specifically, L5 pyramidal cells of the frontal cortex, but not other cortical regions, establish monosynaptic connections with the inhibitory thalamic reticular nucleus (TRN). The frontal L5–TRN pathway parallels the L6–TRN projection but has distinct morphological and physiological features. The exact spike output of the L5-contacted TRN cells correlated with the level of cortical synchrony. Optogenetic perturbation of the L5–TRN connection disrupted the tight link between cortical and TRN activity. L5-driven TRN cells innervated thalamic nuclei involved in the control of frontal cortex activity. Our data show that frontal cortex functions require a highly specialized cortical control over intrathalamic inhibitory processes.

Background: Adequate management is crucial to reduce symptoms, hospitalization, and relapses in patients with asthma. Hospitals often struggle to meet treatment guidelines, and no recent data for Switzerland are available. Objectives: The aim of the study was to audit the asthma exacerbation management in the Cantonal Hospital of Baselland in order to evaluate the level of compliance with guidelines in a narrative discussion. Method: The study design is a retrospective observational cohort study. We evaluated all adult patients presenting to the hospital with a physician-diagnosed asthma exacerbation in 2018 and 2019. The asthma management patients received was compared to the Swiss guidelines and the international GINA guidelines. Results: 160 patients were included (mean age: 50 years old, 57.5% female). SpO2 and heart rate were assessed at presentation in nearly all patients. Peak expiratory flow (PEF) was measured in only 14%. Adequate management of asthma exacerbation with inhaled bronchodilator medication in a combination of short-acting beta-agonists and short-acting anticholinergics was administered to 96% of the patients. Patients with severe symptoms received systemic glucocorticosteroids within 6 h in 55%. At discharge, a reliever medication was prescribed for 64% of the patients and 55% received a new or increased controller therapy with inhaled glucocorticosteroid (ICS). 49% of the patients had no follow-up organized. Conclusion: To increase the guideline conformity and quality of asthma exacerbation management, the severity should be better assessed, especially by routinely performing PEF measurements. Treatment needs to be intensified; in particular, the ICS dose should be increased significantly and systemic glucocorticosteroids should be given with a lower threshold.

Zusammenfassung Der Beitrag stellt den digitalen Lesesaal (DLS) der Staatsarchive Basel-Stadt und St.Gallen vor. Ziele, Vorarbeiten und agile Umsetzung sowie Architektur, Design und Funktionen werden auf dem aktuellen Stand präsentiert. Abschließend wird das Vorgehen zur Einführung skizziert. Der DLS ist die zentrale Schnittstelle zum Archivgut. Er schafft direkten Zugang zu digitalem Archivgut und bildet den gesamten Benutzungsprozess des Archivs ab. Der DLS bietet Werkzeuge für Vermittlung von Archivgut und ermöglicht mit Tools für User Generated Content auch Partizipation von Benutzenden.

The noradrenergiclocus coeruleus(LC) supports vital brain functions during wakefulness. In contrast, the LC has been associated with sleep-promotion and -disruption, leaving its functions for sleep uncertain. Here, we show that the LC is essential for the progression of natural, undisturbed sleep because it creates a non-reducible timeframe for the non-rapid-eye-movement (NREM) sleep-REM sleep cycle. Fiber-photometric jGCaMP8s-based LC activity measures, closed-loop optogenetics and mouse sleep-wake monitoring revealed that LC showed ∼50-s-activity fluctuations, during which high activity levels promoted a NREMS with high arousability, while low levels a NREMS with the opportunity for transitions to REMS. The NREMS-REMS cycle was shortened by LC inhibition due to precocious REMS onset. Supporting LC’s pivotal role as a gatekeeper of the NREMS-REMS cycle, REMS entries occurred on intervals no shorter than ∼50 s during REMS-restriction. A stimulus- enriched wake experience strengthened LC fluctuations, which contributed to subsequent NREMS fragmentation and delayed REMS. The LC fluctuations are hence indispensable for the NREMS-REMS cycle but they can become sleep-disruptive as a result of prior wake experience.

The risk of extreme heat mortality is ever increasing with the rapidly changing climate. However, several studies that project future levels of heat mortality have been criticized to be over-pessimistic, as these studies don’t reflect on communities’ ability to adapt to heat. Adaptation to heat is highly complex and not only complicated by the changing climate but also by other mega-trends such as ageing societies or urbanization. In this work, we therefore model future levels of heat-related mortality whilst incorporating location-specific empirical adaptation as a function of mean summer temperature and time.Concretely, we model the relationship between daily temperature and mortality by using quasi-Poisson regression time series analyses with distributed lag nonlinear models, which is a well-established approach in climate change epidemiology. The relationship expresses the change in mortality risk at specific temperature values against an optimum temperature (the so-called temperature of minimum mortality, MMT). We first model this relationship for continuous five-year intervals for several locations. Next, we express the change in MMT as a linear function of mean summer temperature and time. Adaptation is thus incorporated by shifting the MMTs according to future temperature levels and by when they are reached.This modelling approach allows us to assess heat mortality levels for different climate scenarios at the same level of global warming but at different points in time in the future. Potential findings of this approach are relevant, as the public debate and climate policy focuses largely on warming levels (1.5, 2°C) and to a lesser extent on how fast these levels are reached.

Climate change is causing major shifts in the dynamics of the cryosphere, leading to sea-level rise, glacier break-off events, flooding, and landslides. Geological, thermodynamic and hydraulic processes at the base of an ice mass play a central role in ice flow dynamics, and understanding these is imperative for predicting ice body behavior in a changing climate. To this end, sustained ambient vibrations in glaciated environments can be used to monitor subglacial conditions over significant spatial extent with relatively low-cost acquisition.In earthquake seismology, a well-established methodology to investigate subsurface properties is the horizontal-to-vertical spectral ratio (H/V) of ambient seismic ground unrest. In cryoseismology, the H/V approach is already used to invert for velocity profiles of ice or firn, to obtain bedrock topography and to identify the presence of basal sediments. To date, only a few hours of seismic vibration records are typically used. Yet in such short time records, biases may arise because of the dynamic character of the glacier. Seismic resonances within the soft ice layer and resulting H/V ratios are expected to vary with changes in subglacial hydraulic conditions.We propose to leverage temporal variations in H/V spectra to investigate subglacial processes. As a case study, we first focus on the Glacier de la Plaine Morte (Switzerland), where a seismic array was deployed for four months in summer of 2016. During this time, an ice-marginal lake formed and suddenly drained through and under the glacier, making this seismic record ideal for our purposes. This drainage event is well recorded and strongly influences the H/V in terms of amplitude and resonance frequency. We next present ambient H/V measurements of the Sermeq Kujalleq in Kangia (also known as Jakobshavn Isbræ), one of Greenland’s largest outlet glaciers. Here, the H/V spectra show multiple resonances over time, whose origin we discuss in more detail. For both our study cases, separating variations in source and medium properties is pivotal. Tackling this challenge provides glaciologists with a valuable tool to investigate the poorly accessible subglacial environment, which holds the key to our understanding of ice flow and eustatic sea level rise.

Understanding glaciers evolution is of major concern to evaluate their contribution to sea level rise in the context of global warming. Among the various processes involved in glacier dynamics, fractures like the calving of ice at the front of marine terminating glacier and crevasse formation affect the stress state, potentially modifying the glacier’s velocity. Crevasses also impact the melting rate. The fractures alter the roughness of the ground, increasing the amount of absorbed radiation, and open new networks in which the meltwater is likely to penetrate deeper toward the glacier bed.In this work we propose to model fractures in glacier based on finite strain elastoplasticity, using the Material Point Method (Wolper et al. 2021): we solve the classical governing equations for ice deformation in an Eulerian-Lagrangian framework and we use a strain softening Drucker-Prager constitutive model to simulate plasticity. Thanks to the Lagrangian part of the model, the fractures appear explicitly where high levels of total plastic deformation are reached. The behaviour of a glacier flowing over a step in the bedrock is investigated. The simulations show that crevasse patterns appear with regular spacing between the fractures. We perform a parametric study to determine which parameters affect the length of these patterns and potential dimensionless numbers are outlined.

The use of modern digital technologies in water management is an important driver for obtaining better data for assessing the status of water bodies and their development. These data can be beneficially implemented for the monitoring and management of rivers and especially waterways.In the BMDV-funded project RiverCloud, an autonomous tandem system consisting of an Unmanned Aerial Vehicle (UAV) and an Unmanned Surface Vehicle (USV) is being developed under the coordination of the gia of RWTH Aachen University, which will provide spatially and temporally high-resolution data for the development and maintenance of waterways as well as for river management. The contribution introduces the developed coupled UAV/USV tandem system with its mounted sensors for high resolution data acquisition and continuously accurate georeferencing and presents some significant results using the example of a study area on the Rhine River (Tomateninsel).The data presented are, among others, camera-based flow measurements using an image processing method, discharge data of a precise ADCP (Acoustic Doppler Current Profiler) with 2000 kHz frequency and ten water quality parameters using a multi-parameter probe. All data mentioned were simultaneously collected in two locations of the study area on the Rhine River in September 2022. The 4 seconds videos collected by the UAV-camera were processed using an image processing method based on the surface velocity after implementing a new developed stabilisation tool. The cross-section data collected by ADCP were used for the configuration of the two sites. The agreement between ADCP and camera-based flow and discharge data was very good on both sites with less than 5% deviation for a discharge value of approx. 600 m3/s and 1.63 m/s mean velocity. The water quality parameters collected during the measuring campaign were temperature, conductivity, salinity, pH value, oxygen concentration, oxygen saturation, ammonium, turbidity, Total suspended solids (TSS) and total dissolved solids (TDS). The water quality data were in the expected ranges for river water (e.g. average values: pH 7.8, T 21.8°C, EC 0.35 mS/cm, Sal 0.71%, O2 7.5 mg/l, NH4+ 0.3 mg/l).The results, specific requirements of the developed solution and challenges under the measuring conditions of the study area are presented in this paper. The data collected are used as the input of an overview report for river or waterway water flow and quality monitoring.

Outlet glaciers and ice streams transport ice from the ice sheets to the ocean, where the glaciers lose mass by iceberg calving. Sermeq Kujalleq (SKK, Jakobshavn Isbræ) is one of the largest and most dynamic ice streams of the Greenland Ice Sheet with ice flow velocities up to 40 m/day. With extensive fieldwork and detailed repeated UAV surveys we aim at understanding the complex processes occuring at the ice stream margins and at the calving front of SKK. Such processes are often neclected in numerical models inducing uncertainties in projections of the ice sheet evolution.Within the framework of the COEBELI project we conducted drone photogrammetry surveys in July 2022 at SKK along other field measurements including in-situ GPS, GPRI, seismometers, and time-lapse imagery. Despite challenging weather conditions and constraints due to flying restrictions, we acquired 17 repeated flight surveys over the calving front of SKK during two weeks. As a result, we produced a large imagery data set, which was processed to infer high-resolution ortho-images and digital elevation models (DEM). Comparing the different products enables us to estimate changes in surface topography and ice dynamics. During the observation period several large calving events occurred allowing us to investigate the interaction between frontal processes and ice flow dynamics. With the very detailed data we can study crevasse opening, acceleration at the front, weaknesses in the ice and their origin, and the reaction of the glacier to large calving events.

Outlet glaciers and ice streams are the main channels through which ice sheets transport their mass towards the ocean. One of Greenland’s largest outlet glaciers Sermeq Kujalleq in Kangia (Jakobshavn Isbrae) has been broadly researched after experiencing a rapid retreat of the terminus and accompanying speedup to up to 40 m/day in the early 2000’s. However, such short-term ice dynamic variations remain poorly understood making numerical models difficult to constrain and predictions on future sea-level rise uncertain.The short-term ice dynamics of Sermeq Kujalleq consists in transient states and can only be captured by in-situ measurements of high spatial and temporal resolution. Glacier seismology has proven to be a valuable tool to study these dynamics, it provides data with a high temporal resolution and can provide information on processes happening below the ice surface. Within the COEBELI project we combine passive glacier seismology with global navigation satellite system (GNSS) receivers, long-range drones, time-lapse cameras and terrestrial radar interferometry to capture processes such as calving and basal sliding at their respective timescales.Here, we present results from a multi-array seismic deployment at Sermeq Kujalleq in Summer 2022. From May until September two arrays were deployed in the upstream part of the fast-flowing ice stream (>22 km from calving front) and one array on slower moving ice North of the main trunk. For a 3-week period in July, four more arrays were deployed on the fast-flowing ice stream closer to the calving front (50 Hz) icequakes. By studying these different signals, we are able to better constrain the processes and forces that control fluctuating ice-flow velocity and calving events.

Although studies based on physiological models have repeatedly shown that high humidity levels lead to stronger heat stress in humans, findings from epidemiological studies have remained inconclusive on the matter till date. We aim to employ a ‘bottom-up’ strategy of identifying key drivers of compound events to explore the role played by humidity in high heat-related mortality events, spanning across multiple cities in multiple countries. We used daily data on all-cause mortality, mean temperature and mean relative humidity from 11 cities across the world and applied state-of-the-art epidemiological models to compute the daily observed total mortality counts attributable to heat (i.e., limited to days with average temperature exceeding the ‘temperature of minimum mortality’ (MMT) in each city). Each of these days with mean temperature exceeding MMT is considered as an ’event’ and events with highest mortality counts attributable to heat from multiple cities are analysed in a 2D scatter plot of the corresponding percentile rank of temperature and humidity observed during those events. The frequency of such high impact events in the temperature-humidity percentile space across multiple cities, categorised into sub-groups based on the temperature and humidity climatology of the cities, was then studied. It was observed that close to 90% of the high impact events occurred during high temperature (> 90th percentile) and non-high humid ( 50th percentile), were mostly representative of cities with prevailing high humidity conditions on average during the warmest months, when compared across all the cities. Based on our preliminary findings, there is no conclusive evidence that high humidity conditions were prevalent during high heat-mortality impact events, but further analysis incorporating more cities and other climatological variables of interests such as absolute humidity, wet-bulb globe temperature etc. are planned. This novel framework provides valuable insights into the role of humidity in heat stress mortality and can be generalised to address other similar complex research questions in environmental epidemiology.

Maintenance of waterways is often a challenging task with conflicting interests between different parties. They are subject to economical stresses through the industry and transportation sector on the one hand, on the other the European Water Framework Directive dictates to ensure (or lead them back to) a chemically and ecologically “good status” (which benefits the balance between regulating, supporting, providing and cultural ecosystem services).To have a good decision base for ecological measures, local-scale high resolution temporal and spatial data is needed to make sound decisions for a sustainable river management, which extends to assess and monitor the succession of restoration areas and to document change.The objective of the project RiverCloud is the development of a data acquisition platform which enables synchronous data collection with unmanned aerial vehicles (UAV) and unmanned surface vehicles (USV). On the base of this platform different sensors can be used for a holistic data base in semiterrestrial areas of streams: e.g. acoustic doppler current profilers, multi-parameter sensors, multibeam-echosounders and a 360° camera for the USV; Sensors on the UAV consist of a bathymetric range finder and an industrial grade camera for structure from motion application to derive high-resolution point clouds and orthomosaics.Building upon this data base, a further objective is the detection of vegetation structures such as a canopy height, single trees and the balancing of aboveground biomass, as a regulating ecosystem service for carbon sequestration, from high-resolution point clouds by using open source software. The results from remote sensing data are tested against comparative data collected in the field. Workflow, results and benefits for river management will be presented. All data was collected in September 2022 along the river Rhine near Karlsruhe, Germany.

Wildfires are devastating events destroying large parts of physical assets exposed to them in many regions of the world. Therefore, a high-resolution hazard model is needed to accurately assess socio-economic impacts caused by wildfires. Moreover, a probabilistic representation of the hazard covering the range and likelihood of possible wildfire events under certain conditions allows for a more comprehensive risk assessment. This is crucial for many applications, among others the prioritization of adaptation measures and the pricing of insurance.We determine burning probabilities based on MODIS hotspots and a set of predictors (weather variables, geography, land use) by using a country-specific machine learning model based on the efficient tree boosting system XGBoost. Subsequently, stochastic wildfire events are generated on the basis of these burning probabilities.Lastly, the open-source climate risk assessment platform CLIMADA is used to compute socio-economic impacts as the combination of the newly developed hazard, an exposure and a vulnerability. The used exposure LitPop spatially distributes macroeconomic indicators (e.g. produced capital) as a function of night light intensity and population density. The vulnerability is represented by an impact function that was calibrated on historic fire damage data. Combining the stochastic impacts with their respective probabilities results in a globally consistent country-specific model of wildfire risk to physical assets.

Sermeq Kujalleq in Kangia (Jakobshavn Isbræ), Greenland is one of the most studied glaciers in the world mainly due to its recent retreat associated with extremely fast ice stream flow and high solid ice discharge. However, large limitations remain in the understanding of its short-term ice dynamics as the study of sub-daily variations, generally undetectable in spaceborne observations, requires high-rate field measurements that are challenging to acquire. Here, we present glacier surface velocities determined in Post-Processed Kinematic (PPK) mode from eight autonomous Global Navigation Satellite System (GNSS) stations deployed in July 2022 along the ice stream at a distance of 4 to 30 kilometers from the calving front. During this field campaign, we identified an 8-hour-long glacier speedup which was recorded at all GNSS stations and reached up to 11% of the pre-event velocity, followed by a 12-hour-long slowdown of similar magnitude. We further found the peak velocity was first measured at a GNSS station 16 kilometers away from the calving front, then recorded consecutively at each of the three other downstream GNSS stations with a positive time lag corresponding to a ~3 km/h wave propagation speed. At the station closest to the calving front, the timing of peak velocity corresponded to the occurrence of large-scale calving events. We further present line-of-sight glacier surface velocities measured along three shear margin transects with a terrestrial radar interferometer deployed simultaneously with the GNSS array. Across all profiles, we observed a widespread and simultaneous response of fast- and slow-moving ice suggesting a strong coupling between the main trunk and the shear margins of the ice stream.

Since Paul Grice first propounded his ideas surrounding conversation and implicature in 1967, they have had a continuous and tremendous impact on theorizing, and indeed on the design of entire research programmes, in philosophy, but also in many other disciplines, in particular linguistic pragmatics. Much of what builds on Grice’s original suggestions now belongs to the most powerful hypotheses in the respective fields. But while scholars outside philosophy usually acknowledge Grice’s merits for their own areas of interest, they hardly ever pay attention to his original philosophical intentions. These intentions are the central topic of the present paper. Its primary concern is to show how the theory of conversational implicature enabled Grice to adopt a unique theoretical position within 20th century analytic philosophy. In doing so, it also hopes to eliminate a number of widespread misconceptions regarding the explanatory ambitions of Grice’s original theory.

BackgroundPulmonary embolism (PE) is not only a life-threatening disease but also a public health issue with significant economic burden. The aim of the study was to identify factors—including the role of primary care—that predict length of hospital stay (LOHS), mortality and re-hospitalization within 6 months of patients admitted for PE.MethodA retrospective cohort study was conducted with patients presenting to a Swiss public hospital with PE diagnosed at the hospital between November 2018 and October 2020. Multivariable logistic and zero-truncated negative binomial regression analyses were performed to assess risk factors for mortality, re-hospitalization and LOHS. Primary care variables encompassed whether patients were sent by their general practitioner (GP) to the emergency department and whether a GP follow-up assessment after discharge was recommended. Further analyzed variables were pulmonary embolism severity index (PESI) score, laboratory values, comorbidities, and medical history.ResultsA total of 248 patients were analyzed (median 73 years and 51.6% females). On average patients were hospitalized for 5 days (IQR 3–8). Altogether, 5.6% of these patients died in hospital, and 1.6% died within 30 days (all-cause mortality), 21.8% were re-hospitalized within 6 months. In addition to high PESI scores, we detected that, patients with an elevated serum troponin, as well as with diabetes had a significantly longer hospital stay. Significant risk factors for mortality were elevated NT-proBNP and PESI scores. Further, high PESI score and LOHS were associated with re-hospitalization within 6 months. PE patients who were sent to the emergency department by their GPs did not show improved outcomes. Follow-up with GPs did not have a significant effect on re-hospitalization.ConclusionDefining the factors that are associated with LOHS in patients with PE has clinical implications and may help clinicians to allocate adequate resources in the management of these patients. Serum troponin and diabetes in addition to PESI score might be of prognostic use for LOHS. In this single-center cohort study, PESI score was not only a valid predictive tool for mortality but also for long-term outcomes such as re-hospitalization within 6 months.

Current evidence on COVID-19 prognostic models is inconsistent and clinical applicability remains controversial. We performed a systematic review to summarize and critically appraise the available studies that have developed, assessed and/or validated prognostic models of COVID-19 predicting health outcomes. We searched six bibliographic databases to identify published articles that investigated univariable and multivariable prognostic models predicting adverse outcomes in adult COVID-19 patients, including intensive care unit (ICU) admission, intubation, high-flow nasal therapy (HFNT), extracorporeal membrane oxygenation (ECMO) and mortality. We identified and assessed 314 eligible articles from more than 40 countries, with 152 of these studies presenting mortality, 66 progression to severe or critical illness, 35 mortality and ICU admission combined, 17 ICU admission only, while the remaining 44 studies reported prediction models for mechanical ventilation (MV) or a combination of multiple outcomes. The sample size of included studies varied from 11 to 7,704,171 participants, with a mean age ranging from 18 to 93 years. There were 353 prognostic models investigated, with area under the curve (AUC) ranging from 0.44 to 0.99. A great proportion of studies (61.5%, 193 out of 314) performed internal or external validation or replication. In 312 (99.4%) studies, prognostic models were reported to be at high risk of bias due to uncertainties and challenges surrounding methodological rigor, sampling, handling of missing data, failure to deal with overfitting and heterogeneous definitions of COVID-19 and severity outcomes. While several clinical prognostic models for COVID-19 have been described in the literature, they are limited in generalizability and/or applicability due to deficiencies in addressing fundamental statistical and methodological concerns. In the progression phase of idiopathic pulmonary fibrosis (IPF) the normal alveolar structure of the lung is lost and replaced by remodeled fibrotic tissue and by bronchiolized cystic airspaces. Although these are characteristic features of IPF, knowledge of specific interactions between these pathological processes is limited. Here, the interaction of lung epithelial and lung mesenchymal cells was investigated in a co-culture model of human primary airway epithelial cells (EC) and lung fibroblasts (FB). Single-cell RNA sequencing (sc-RNA-seq) revealed that the starting EC population was heterogenous and enriched for cells with a basal cell signature. Furthermore, fractions of the initial EC and FB cell populations adopted distinct pro-fibrotic cell differentiation states upon co-cultivation, resembling specific cell populations that were previously identified in lungs of IPF patients. Transcriptomic analysis revealed active nuclear factor NF-kappa-B (NF-κB) signaling early in the co-cultured EC and FB cells and the identified NF-κB expression signatures were also found in "HAS1 High FB" and "PLIN2+ FB" populations from IPF patient lungs. Pharmacological blockade of NF-κB signaling attenuated specific phenotypic changes of EC and prevented FB-mediated interleukin-6 (IL6), interleukin-8 (IL-8) and C-X-C motif chemokine ligand 6 (CXCL6) cytokine secretion, as well as collagen alpha-1(I) chain (COL1A1) and alpha-smooth muscle actin (α-SMA) accumulation. Thus, we identified NF-κB as a potential mediator, linking epithelial pathobiology with fibrogenesis.

ZusammenfassungDie Identifizierung von Gefahren in einem frühen Stadium ist ein zentraler Baustein der öffentlichen Gesundheit im Bereich der Lebensmittelsicherheit. Das Bundesamt für Lebensmittelsicherheit und Veterinärwesen der Schweiz verwendet seit 2018 ein System zur Früherkennung. Dieses stützt sich auf eine Gruppe von Fachpersonen, welche Informationen zu Gefahren identifizieren und periodisch bewerten. Der Beirat, ein wissenschaftliches Gremium von Experten und Expertinnen aus der Wirtschaft, von Hochschulen, Vollzugsbehörden und Referenzzentren erörtert anschließend die Themen und gibt seinerseits Empfehlungen an das Bundesamt für Lebensmittelsicherheit und Veterinärwesen ab. Insgesamt wurden bislang 37 Themen bearbeitet. Monatlich wird ein Newsletter zu Themen der Früherkennung publiziert. Zudem werden unregelmäßig bei Bedarf Briefing Letters mit einer vertieften Recherche zu einem Thema erstellt.

Memory encoding and retrieval rely on specific interactions across multiple brain areas. Although connections between individual brain areas have been extensively studied, the anatomical and functional specificity of neuronal circuit organization underlying information transfer across multiple brain areas remains unclear. Here, we combine transsynaptic viral tracing, optogenetic manipulations, and calcium dynamics recordings to dissect the multisynaptic functional connectivity of the amygdala. We identify a distinct basolateral amygdala (BLA) subpopulation that connects disynaptically to the periaqueductal gray (PAG) via the central amygdala (CeA). This disynaptic pathway serves as a core circuit element necessary for the learning and expression of conditioned fear and exhibits learning-related plasticity. Together, our findings demonstrate the utility of multisynaptic approaches for functional circuit analysis and indicate that disynaptic specificity may be a general feature of neuronal circuit organization.

Glacier seismology is a valuable tool for investigating ice flow dynamics, but sufficient data acquisition in remote and exposed glaciated terrain remains challenging. For data acquisition on a highly crevassed and remote outlet glacier in Greenland we developed self-sufficient and easily deployable seismic stations, "SG-boxes". The SG-boxes contain their own power supply via solar panel, a three-component omni-directional geophone and a GNSS receiver. The SG-boxes can be deployed and retrieved from a hovering helicopter, allowing for deployment in difficult terrain. To assess their performance we conducted a field test comparing the SG-boxes to established on-ice geophone installations at Gornergletscher in Switzerland. Moreover, data from a first SG-box deployment in Greenland were analyzed. The SG-boxes exhibit consistently higher noise levels relative to colocated conventional geophones and a correlation between noise levels, wind and air temperature is found. Despite their noise susceptibility, the SG-boxes detected a total of 13,114 Gornergletscher icequakes over 10 days, which is 30% of the total number of icequakes detected by conventional geophone stations. Hence, even in sub-optimal weather conditions and without additional noise reduction measures, the SG-boxes can provide unique and valuable data from challenging glaciated terrain where no conventional seismic installations are possible.

Der vorliegende Bericht wurde im Rahmen des Studiums Soziale Arbeit der Hochschule Luzern verfasst. Darin wird das Projekt «Von und Für – ein Projekt von Best Agers für Best Agers» vollumfänglich erläutert und evaluiert. Das Projekt wurde von der Trägerorganisation, dem Gemeinschaftszentrum Bachwiesen, in Auftrag gegeben und von der Studentin Elena Lüthi im Zeitraum Oktober 2021 bis August 2022 konzipiert und umgesetzt. Das Projekt richtet sich an die ältere noch aktive Generation, die sogenannten Best Agers, mit dem Ziel eine Arbeitsgruppe zu schaffen, welche sich für die Bedürfnisse ihrer eigenen Generation einsetzt. Durch Partizipation und Empowerment stellte eine Gruppe aus fünf Best Agers regelmässig stattfindende Brettspielnachmittage auf die Beine. Auf dem Weg zu diesem Endergebnis standen Zusammenhalt und Teamkultur im Fokus, um die Arbeitsgruppe nachhaltig für das Thema und das Zusammenarbeiten in der Gruppe zu gewinnen. Die Universitätsbibliotheken Bern und Basel, die Zentral- und Hochschulbibliothek Luzern sowie die Zentralbibliothek Zürich digitalisieren seit 2019 in Zusammenarbeit mit Google einen großen Teil ihres historischen Buchbestandes. Die Universitätsbibliothek Bern schloss ihr Projekt mit 84 000 digitalisierten Titeln im September 2022 ab, die Zentral- und Hochschulbibliothek Luzern folgte mit über 70 000 Titeln im November 2022. Trotz Unterbrüchen während des Pandemie-Lockdowns konnten die beiden Bibliotheken ihre Projekte erfolgreich durchführen und bieten ihren Kundinnen und Kunden nun einen einfacheren Zugang zu ihrem Bestand aus dem 18. und 19. Jahrhundert. 2023 starten Basel und Zürich mit ihren Anschlussprojekten.

ZusammenfassungAusdifferenzierte und komplexe Gesellschaftssysteme haben keine kollektive Handlungsfähigkeit. Die Erhöhung der kollektiven Handlungsfähigkeit stellt ein Kernelement der Initiative Lernen vor Ort dar mit der Annahme, dass »Bildungsstationen und Bildungsangebote wie ein stimmiges, integriertes System aufeinander bezogen und abgestimmt« sein sollen. Die zweite Annahme besteht darin, dass durch diese Abstimmung »das Lernen im gesamten Lebenslauf erfolgreich sein kann«.

ZusammenfassungDas BMK tritt als Subjekt der Erziehung auf dem Feld des politischen Diskurses (LG) auf. Als Vertreter des sozial-kulturellen Systems investiert es Verständigungskraft ins politische System, um eine positive Kultur für die Grundidee eines Strukturprogramms zu schaffen. In gemischten Gremien entwickelt es ein kreisweites Übergangsmanagement und transferiert Bildungsberatung im Bereich der frühen Bildung.

The dense mixture of iceberg of various sizes and sea ice observed in many of Greenland's fjords, called ice mélange (sikussak in Greenlandic), has been shown to have a significant impact on the dynamics of several Greenland tidewater glaciers, mainly through the seasonal support it provides to the glacier terminus in winter. However, a clear understanding of shorter-term ice mélange dynamics is still lacking, mainly due to the high complexity and variability of the processes at play at the ice–ocean boundary. In this study, we use a combination of Sentinel-1 radar and Sentinel-2 optical satellite imagery to investigate in detail intra-seasonal ice mélange dynamics and its link to calving activity at three major outlet glaciers: Kangerdlugssuaq Glacier, Helheim Glacier and Sermeq Kujalleq in Kangia (Jakobshavn Isbræ). In those fjords, we identified recurrent ice mélange weakening (IMW) episodes consisting of the up-fjord propagation of a discontinuity between jam-packed and weaker ice mélange towards the glacier terminus. At a late stage, i.e., when the IMW front approaches the glacier terminus, these episodes were often correlated with the occurrence of large-scale calving events. The IMW process is particularly visible at the front of Kangerdlugssuaq Glacier and presents a cyclic behavior, such that we further analyzed IMW dynamics during the June–November period from 2018 to 2021 at this location. Throughout this period, we detected 30 IMW episodes with a recurrence time of 24 d, propagating over a median distance of 5.9 km and for 17 d, resulting in a median propagation speed of 400 m d−1. We found that 87 % of the IMW episodes occurred prior to a calving event visible in spaceborne observations and that ∼75 % of all detected calving events were preceded by an IMW episode. These results therefore present the IMW process as a clear control on the calving activity of Kangerdlugssuaq Glacier. Finally, using a simple numerical model for ice mélange motion, we showed that a slightly biased random motion of ice floes without fluctuating external forcing can reproduce IMW events and their cyclic influence and explain observed propagation speeds. These results further support our observations in characterizing the IMW process as self-sustained through the existence of an IMW–calving feedback. This study therefore highlights the importance of short-term ice mélange dynamics in the longer-term evolution of Greenland outlet glaciers.

ZusammenfassungDie Verfahrensrationalität beider Gebietskörperschaften stelle ich nun vergleichend und im Verlauf des Projektarbeitsbogens dar. Die Typen gesellschaftlicher Integration und deren Intensität beschreibe ich zunächst in Bezug auf den Aufgabenkomplex Kernaktivitäten herstellen und entlang der Arbeitsphasen im Bereich der Strategien. Darauf folgen die offenen Codes zum Aufgabenkomplex Spannungslagen ausbalancieren.

OBJECTIVES Existing computerized diagnostic decision support tools (CDDS) accurately return possible differential diagnoses (DDx) based on the clinical information provided. The German versions of the CDDS tools for clinicians (Isabel Pro) and patients (Isabel Symptom Checker) from ISABEL Healthcare have not been validated yet. METHODS We entered clinical features of 50 patient vignettes taken from an emergency medical text book and 50 real cases with a confirmed diagnosis derived from the electronic health record (EHR) of a large academic Swiss emergency room into the German versions of Isabel Pro and Isabel Symptom Checker. We analysed the proportion of DDx lists that included the correct diagnosis. RESULTS Isabel Pro and Symptom Checker provided the correct diagnosis in 82 and 71 % of the cases, respectively. Overall, the correct diagnosis was ranked in 71 , 61 and 37 % of the cases within the top 20, 10 and 3 of the provided DDx when using Isabel Pro. In general, accuracy was higher with vignettes than ED cases, i.e. listed the correct diagnosis more often (non-significant) and ranked the diagnosis significantly more often within the top 20, 10 and 3. On average, 38 ± 4.5 DDx were provided by Isabel Pro and Symptom Checker. CONCLUSIONS The German versions of Isabel achieved a somewhat lower accuracy compared to previous studies of the English version. The accuracy decreases substantially when the position in the suggested DDx list is taken into account. Whether Isabel Pro is accurate enough to improve diagnostic quality in clinical ED routine needs further investigation.

In diesem Artikel werden Ergebnisse von Interviews mit insgesamt zwanzig Fremdsprachenlehrpersonen aus der Deutschschweiz präsentiert, die an einer Studie zur Erforschung der Mehrsprachigkeitsdidaktik im schulischen Fremdsprachenunterricht teilgenommen haben. Diese Lehrpersonen und ihre Klassen (Primar- und Sekundarstufe), die Französisch und Englisch als schulische Fremdsprachen lernen, haben während vier Lektionen à 45 Minuten an mehrsprachigkeitsdidaktischen Aktivitäten gearbeitet und wurden dabei gefilmt. In anschliessenden Interviews wurden die Lehrpersonen um ihre Einschätzungen 1) der soeben durchgeführten Aktivitäten und 2) des Stellenwerts von mehrsprachigkeitsdidaktischen Ansätzen im Fremdsprachenunterricht gebeten. Die Ergebnisse zeigen, dass die Lehrpersonen die Prinzipien der Mehrsprachigkeitsdidaktik grundsätzlich gut finden. Gleichzeitig weisen sie auf erhebliche Herausforderungen bei ihrem Einsatz im Fremdsprachenunterricht hin.This article presents results from interviews with twenty foreign language teachers from German-speaking Switzerland who participated in a study on multilingual pedagogy in the foreign language classroom. The teachers and their classes (primary and secondary levels), who learn French and English as foreign languages, were filmed while working on multilingual teaching activities during four lessons. In subsequent interviews, teachers were asked for their evaluation of 1) the activities just carried out and 2) the place of multilingual teaching approaches in the foreign language classroom. The results show that the teachers generally appreciate the principles of multilingual teaching approaches. At the same time, they point out considerable challenges when implementing such activities in the foreign language classroom.

ZusammenfassungAbschließend reflektiere ich den Ertrag der theoretischen Perspektiven. Konzepte der Gesellschaftstheorie von Schimank haben die (Re-)Konstruktion der Typologie gesellschaftlicher Integration inspiriert, konnten aber den Grenzverkehr zwischen den Teilsystemen nicht angemessen erfassen. Dies gelang mit Konzepten der Handlungstheorie von Münch besser.
