In patients admitted for acute heart failure (HF) indication for drugs which reduce the heart rate (HR) is debated. The multicentre prospective study Reduction of heart Rate in Heart Failure (RedRate-HF) was designed to analyse the hemodynamic effects of an early reduction of HR in acute HF.Hemodynamic parameters were recorded by using the bioimpedance technique, which was shown to be accurate, highly reproducible and sensitive to intra-observer changes. Lowering HR was obtained by ivabradine 5 mg bd, given 48-72 h after admission on the top of optimized treatment. Patients were followed at 24, 48, 72 h after drug assumption and at hospital discharge.Twenty patients of a mean age of 67 ± 15 years, BNP at entry 1348 ± 1198 pg/ml were enrolled. Despite a clinical stabilization, after 48-72 h from admission, HR was persistently70 bpm. Ivabradine was well tolerated in all patients with a significant increase in RR interval from 747 ± 69 ms at baseline to 948 ± 121 ms at discharge, P 0.0001. Change in HR was associated with a significant increase in stroke volume (baseline 73 ± 22 vs. 84 ± 19 ml at discharge, P = 0.03), and reduction in left cardiac work index (baseline 3.6 ± 1.2 vs. 3.1 ± 1.1 kg/m2 at discharge, P = 0.04). Other measures of heart work were also significantly affected while cardiac output remained unchanged.The strategy of an early lowering of HR in patients admitted for acute HF on top of usual care is feasible and safe. The HR reduction causes a positive increase in stroke volume and may contribute to saving energy without affecting cardiac output.

Study Objectives To establish whether the recent EEG and behavioral criteria of arousal disorders apply to sexsomnia. Methods EEG and behavioral markers upon N3 sleep interruptions in videopolysomnography were retrospectively compared in 24 participants with sexsomnia, 41 participants with arousals disorders, and 40 healthy controls. The specificity and sensitivity of previously suggested EEG and behavioral cutoffs for supporting arousal disorders diagnosis were measured in the sexsomnia vs. control groups. Results Participants with sexsomnia and arousals disorders showed a higher N3 fragmentation index, slow/mixed N3 arousal index, and number of eye openings during N3 interruptions than healthy controls. Ten (41.7%) participants with sexsomnia (vs. one sleepwalker and no control) displayed an apparently sexual behavior (masturbation, sexual vocalization, pelvic thrusting, and hand within the pajama) during N3 arousal. An N3 sleep fragmentation index ≥6.8/h of N3 sleep and two or more N3 arousals associated with eye opening was 95% specific but poorly (46% and 42%) sensitive for diagnosing sexsomnia. An index of slow/mixed N3 arousals ≥2.5/h of N3 sleep was 73% specific and 67% sensitive. An N3 arousal with trunk raising, sitting, speaking, showing an expression of fear/surprise, shouting, or exhibiting sexual behavior was 100% specific for a diagnosis of sexsomnia. Conclusions In patients with sexsomnia, videopolysomnography based markers of arousal disorders are intermediate between healthy individuals and patients with other arousal disorders, supporting the concept of sexsomnia as a specialized, but less neurophysiologically severe, NREM parasomnia. Previously validated criteria for arousal disorders partially fit in patients with sexsomnia.

Bilateral subthalamic nucleus deep brain stimulation (STN-DBS) is an effective treatment in advanced Parkinson’s Disease (PD). However, the effects of STN-DBS on freezing of gait (FOG) are still debated, particularly in the long-term follow-up (≥5-years). The main aim of the current study is to evaluate the long-term effects of STN-DBS on FOG. Twenty STN-DBS treated PD patients were included. Each patient was assessed before surgery through a detailed neurological evaluation, including FOG score, and revaluated in the long-term (median follow-up: 5-years) in different stimulation and drug conditions. In the long term follow-up, FOG score significantly worsened in the off-stimulation/off-medication condition compared with the pre-operative off-medication assessment (z = −1.930; p = 0.05) but not in the on-stimulation/off-medication (z = −0.357; p = 0.721). There was also a significant improvement of FOG at long-term assessment by comparing on-stimulation/off-medication and off-stimulation/off-medication conditions (z = −2.944; p = 0.003). These results highlight the possible beneficial long-term effects of STN-DBS on FOG.

Aims The angiotensin receptor neprilysin inhibitor (ARNI) sacubitril/valsartan reduces mortality and hospitalizations in patients with heart failure and reduced ejection fraction (HFrEF). Favourable effects on haemodynamic and functional parameters have been observed in patients with HFrEF undergoing ARNI therapy, using standard transthoracic echocardiography. Global longitudinal strain (GLS) assessment uses a semi‐automatic procedure to provide a reliable and repeatable method that improves the detection of early changes of contractile function. We aimed to assess the effects of ARNI on GLS and myocardial mechanics in patients with HFrEF. Methods and results Thirty patients with New York Heart Association class II–III HFrEF were treated with ARNI and monitored using standard echocardiographic examination and GLS measurements at baseline, 3 months, and 6 months. ARNI therapy resulted in a significant reduction of ventricular volumes and a significant increase in left ventricular ejection fraction at 6 months but not 3 months by standard transthoracic echocardiography (left ventricular ejection fraction from 28 ± 8% at baseline to 34 ± 12% at 6 months, P

(1) Background: Epilepsy is a frequent comorbidity in patients with brain tumors, in whom seizures are often drug-resistant. Current evidence suggests that excess of glutamatergic activity in the tumor microenvironment may favor epileptogenesis, but also tumor growth and invasiveness. The selective non-competitive α-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid (AMPA) receptor antagonist perampanel (PER) was demonstrated to be efficacious and well-tolerated in patients with focal seizures. Moreover, preclinical in vitro studies suggested a potential anti-tumor activity of this drug. In this systematic review, the clinical evidence on the efficacy and tolerability of PER in brain tumor-related epilepsy (BTRE) is summarized. (2) Methods: Five databases and two clinical trial registries were searched from inception to December 2022. (3) Results: Seven studies and six clinical trials were included. Sample size ranged from 8 to 36 patients, who received add-on PER (mean dosage from 4 to 7 mg/day) for BTRE. After a 6–12 month follow-up, the responder rate (% of patients achieving seizure freedom or reduction ≥ 50% of seizure frequency) ranged from 75% to 95%, with a seizure freedom rate of up to 94%. Regarding tolerability, 11–52% of patients experienced non-severe adverse effects (most frequent: dizziness, vertigo, anxiety, irritability). The retention rate ranged from 56% to 83%. However, only up to 12.5% of patients discontinued the drug because of the adverse events. (4) Conclusions: PER seems to be efficacious, safe, and well-tolerated in patients with BTRE. Further randomized studies should be conducted in more homogeneous and larger populations, also evaluating the effect of PER on tumor progression, overall survival, and progression-free survival.

The most prevalent and deadly primary malignant glioma in adults is glioblastoma (GBM), which has a median survival time of about 15 months. Despite the standard of care for glioblastoma, which includes gross total resection, high-dose radiation, and temozolomide chemotherapy, this tumor is still one of the most aggressive and difficult to treat. So, it is critical to find more potent therapies that can help glioblastoma patients have better clinical outcomes. Additionally, the prognosis for recurring malignant gliomas is poor, necessitating the need for innovative therapeutics. Immunotherapy is a rather new treatment for glioblastoma and its effects are not well studied when it is combined with standard chemoradiation therapy. We conducted this study to evaluate different glioblastoma immunotherapy approaches in terms of feasibility, efficacy, and safety. We conducted a computer-assisted literature search of electronic databases for essays that are unique, involve either prospective or retrospective research, and are entirely written and published in English. We examined both observational data and randomized clinical trials. Eighteen studies met the criteria for inclusion. In conclusion, combining immunotherapy with radiochemotherapy and tumor removal is generally possible and safe, and rather effective in the prolongation of survival measures.

In obstructive hypertrophic cardiomyopathy (HC), extreme heterogeneity of septal morphology makes septal myectomy particularly challenging. Although cardiovascular magnetic resonance (CMR) reconstructs ventricular anatomy with high spatial resolution, CMR is not used systematically to plan preoperatively septal myectomy. In this study, we report our results with using CMR to plan the extent of septal excision in 112 consecutive HC patients who subsequently underwent myectomy. Depth and length of the myectomy planned at CMR were compared with those of the septal muscle excised in a single piece in all patients. Anterior septum maximal thickness at CMR was 22 ± 5 mm and excised muscle thickness 9 ± 3 mm. Planned myectomy length was 35 ± 11 mm (range 17 to 65) and excised muscle length 38 ± 10 mm (range 10 to 70), indicating extension of septal resection to mid-cavity. The thriving circular economy is expected to contribute to all three dimensions of sustainable development: environmental, economic, and social. This paper aims to propose a framework to assess social indicators to support circular business models. To validate the framework, we conducted a case study in a medium-size Italian footwear luxury industry, using the Value Focused Thinking–VFT. This approach was used to define proper social indicators to measure the perception of the company’s managers related to the level of incorporation of social dilemmas in the company. We collect data through interviews, documental analysis and direct observation from October/2019 until August/2020 and apply a questionnaire in 2020/2021. The novelty of this paper lies in the proposition of a framework to assess the social indicators in broad categories, capable of covering all supply chains: Corporation, Community; Consumers; Suppliers; Human Rights and Human Resources. Another novelty is related to the analysis of indicators in terms of strategic, tactical, and operational levels, similarly to the idea of a Balanced Scorecard, which was allowed by applying the VFT approach.

Fine particulate matter (PM) emission from biomass boilers for non-industrial heating represents one of the most important causes (together with the transport sector) of air pollution, in particular during winter. Separation technologies for fine PM are already well-known and adopted on an industrial scale, as a consequence of strict limits set by national and international regulations. On domestic boilers, the same technologies utilized on an industrial scale are not feasible due to high investment costs. Moreover, the emission limits for small size biomass boilers are higher than for industrial boilers, so high efficiency separation technologies are not needed, and are sometimes not present at all. The main goal of the paper is the development and testing of a mathematical model that is able to foresee the PM removal efficiency of a wet scrubber device. After an experimental validation based on several tests, it was possible to approach the preliminary design of an innovative wet scrubber, which is described in the paper. The main characteristics are (i) removal efficiency over 99.9%, (ii) specific energy consumption under 36 kJ m−3, which is an industrial reference, and (iii) relatively low investment, operation and maintenance costs.

The paper describes two innovative technological solutions developed at the University of Bologna and shows a preliminary design on how they can be integrated in a commercial biomass boiler for residential application. The first innovation is a high efficiency and low cost filter for particulate emissions: the first prototype of bubble-column scrubber was tested in University of Bologna laboratory on a 25 kW th and reaches PM2.5 removal efficiency up to 95%. The second innovation is the integration of a thermoelectric generator able to produce electricity directly from heat exchange. A prototype has been realized and tested in University of Bologna laboratory and represents a new approach in the design and application of thermoelectric generator in the field of biomass boiler. The paper will evaluate how these new technologies can be integrated in a residential size biomass boiler from a technological and economic point of view, considering also operation and maintenance costs.

At present, surgical septal myectomy is regarded as the "gold standard" treatment for most patients with obstructive hypertrophic cardiomyopathy (HCM) and drug-refractory symptoms. However, the best results are obtained by those surgeons who have extensive experience with this operation at a small number of referral centers. In the mid-1990s, percutaneous alcohol septal ablation was introduced as an alternative to myectomy to reduce LV outflow gradient and heart failure symptoms in patients with obstructive HCM. However, certain features of alcohol ablation limit its applicability to carefully selected patients. Because this procedure involves the injection of 1-4 mL of 96% ethanol into a septal perforator of the left anterior coronary artery to produce a myocardial infarction (with septal thinning, outflow tract widening and gradient reduction), alcohol ablation is limited by the size and distribution of the septal perforator branches. In addition, rich blood supply from other septal branches not occluded by the balloon and from the posterior descending coronary artery may restrict myocardial ischemia to portions of the septum that do not contribute to outflow obstruction. Septal hypertrophy may be either mild or particularly marked, and abnormalities of mitral valve apparatus may also play a major role in outflow obstruction. When such features are present, alcohol-induced septal thinning is unlikely to significantly reduce the gradient. In addition, persisting uncertainties regarding the risk for ventricular tachyarrhythmias after alcohol ablation suggest this procedure should be limited to patients of advanced age, patients at unacceptable operative risk due to comorbidities, or those with strong aversion to surgery. Further progress in the treatment for patients with obstructive HCM and severe refractory symptoms will come from assuring proper patient selection for alcohol septal ablation, as well as increasing the number of surgeons and centers experienced in performing septal myectomy.

The human species has an emotional connection with nature called “biophilia” (or, love for life). This connection goes from generation to generation, being one of the reasons why green spaces are present in large cities. Quinta da Boa Vista is a municipal park in Rio de Janeiro located in the São Cristóvão neighborhood in the northern part of the city. Its main attraction is the artificial lake system, a leisure attraction for the local community. This article reports the presence of Biomphalaria glabrata and Physa acuta snails around the main lake in the Quinta da Boa Vista Municipal Park in Rio de Janeiro, RJ. Seventy-one B. glabrata and twenty-seven P. acuta snails were collected. The snails were measured and examined for the presence of trematode larvae according to standardized procedures. The prevalence rate of B. glabrata was estimated at 15.5%. Rediae and cercariae resembling those of the species Typhlocoelum cucumerinum (Typhlocoelidae, Digenea) which parasitizes aquatic birds of the order Anseriformes were found. This study reinforces the importance of monitoring park lakes and ponds to better understand the risk of helminth transmission by molluscan vectors.KEY WORDS: Snails; Schistosomiasis; Digenea; Quinta da Boa Vista; Rio de Janeiro.

Purpose In this study, the authors investigated (a) the effects of task, vocal distinctiveness of the competing talkers, and meaningfulness of the competitor on older listeners' identification of a target in the presence of competition and (b) the factors that are most predictive of the variability in target identification observed among older listeners. Method Seventeen older and 5 younger adults identified a target in the presence of a competing message. Identification was measured for 2 target types (message; talker), 3 vocal-distinctiveness levels (same talker; different talkers–same sex; different talkers–different sex), and 2 competitors (meaningful speech; nonmeaningful time-reversed speech). Multiple regression analyses were used to identify variables that were predictive of intersubject variability. Results Significant age-related differences were found in the benefit obtained from increasing vocal distinctiveness. Older listeners showed reduced target identification when (a) the competitor was normal speech compared with time-reversed speech and (b) the target was message identification compared with talker identification. Variability among listeners in the older group was partially explained by auditory and cognitive factors. Conclusion Age-related declines in multitalker environments are not solely due to lower-level deficits in perceptual organization but are also a consequence of an interaction between lower-level and higher-level processes.

Background In patients with chronic heart failure, abnormal ventilation at cardiopulmonary testing (expressed by minute ventilation‐to‐carbon dioxide production, or VE / VCO 2 slope, and resting end‐tidal CO 2 pressure) may derive either from abnormal autonomic or chemoreflex regulation or from lung dysfunction induced by pulmonary congestion. The latter hypothesis is supported by measurement of pulmonary capillary wedge pressure, which cannot be obtained routinely but may be estimated noninvasively by measuring transthoracic conductance (thoracic fluid content 1/kΩ) with impedance cardiography. Methods and Results Preliminarily, in 9 patients undergoing invasive hemodynamics during cardiopulmonary testing, we demonstrated a significant relationship between VE / VCO 2 slope and resting end‐tidal CO 2 pressure with baseline and peak pulmonary capillary wedge pressure. Later, noninvasive hemodynamic evaluation by impedance cardiography was performed before cardiopulmonary testing in 190 patients with chronic systolic heart failure and normal lung function (aged 67±3 years, 71% with ischemia, ejection fraction 32±7%, 69% with implantable cardioverter‐defibrillator or cardiac resynchronization therapy). In this group, we determined the relationship between abnormal ventilation ( VE / VCO 2 slope and resting end‐tidal CO 2 pressure) and transthoracic conductance. This study presents 2 experiments investigating whether listeners with cochlear hearing loss (hearing impaired; HI) and listeners with normal hearing (NH) show differential susceptibility to masking in double-vowel identification. Experiment 1 addressed how double-vowel perception changes as a function of differences in fundamental frequency (ΔF 0 ) of 0 and 2 semitones and the relative amplitudes of the constituent vowels (target-to-masker ratios: –10, –5, 0, 5, 10 dB). When ΔF 0 is 0 semitones, listeners in the HI group often perceive the presence of only 1 vowel, whereas listeners in the NH group generally perceive the presence of 2 vowels. In both groups, ΔF 0 benefits target-vowel identification, with the greatest benefit occurring when the target-to-masker ratio is –10 dB. When identification rates of specific vowels (in d ') were rank ordered, different patterns of vowel dominance were found between the NH and HI groups. In Experiment 2, the effects of ΔF 0 (0 to 4 semitones) were compared for monaural and dichotic presentation of double vowels. Both groups show significant dichotic benefit. In addition, individual listeners in the HI group showed trends toward greater dichotic benefit. In both experiments, identification of competing vowels was significantly worse in HI listeners. The results of this study support the idea that increased susceptibility to masking is a primary factor underlying the degraded double-vowel perception in listeners with hearing loss.

Purpose This study examined whether audiologists modify practice patterns in their provision of cochlear implant (CI) services to older adults and, if so, whether the nature of such modifications is consistent across clinical sites. Method An online survey was sent to audiologists at CI centers across the United States. Questions addressed demographics, candidacy, programming, outcomes assessment, rehabilitation, and professional development. Results Responses were obtained from 47 audiologists who provided CI services to older adults. The majority of these audiologists did not report modifying CI practice patterns on the basis of age. Counseling appeared to be an exception: Audiologists reported that both the content and delivery of information were modified for older adults. Specific to CI candidacy with older adults, under half of the respondents indicated that assessing an older adult’s cognitive status was part of their clinical protocol. Conclusion The audiologists who responded to the survey acknowledged issues related to aging when providing CI services to older adults. Despite this acknowledgment, the results of the survey suggest a gap in how age-related issues are incorporated into CI service provision. This paper represents a system which explores the reality of colour in relation to sound and dedicated firstly to blind people, the challenges they live with, their psychology and their emotions. It’s an exploration of the infinite range of colours that they have either never been able to see or that they have lost the possibility to see. The system developed is based on the concept of synesthesia: a ‘liaison’ between colour and sound, colour that becomes sound. This paper synthesizes a wider range of study regarding people with diverse psychological and movement conditions. It represents a new method of expression and of self development through music therapy and game, dedicated mainly to children aged between 5 and 13. Finally we conclude with a portable prototype device that has been validated and patented. It is capable of detecting colour and transforming it into sound: a colour sensor identifies colours and transforms this data into musical notes. The prototype is totally adaptable to the requirements of the end user meaning that the user can choose the best combination between colour and sound in order to improve their communication with the environment. We are now developing a mathematical theoretical background in order to formalize the correspondence between colour and musical space.

The purpose of this article is to describe the process used to create and pilot-test a customized reflective electronic portfolio (e-portfolio) that integrates learning across three distinct components of an entry-level 6.5 year Doctor of Physical Therapy (DPT) program. During the 2004-2005 academic year, a pilot test was conducted with 10 students, representative of all 6.5 years in the DPT program. Development of our reflective e-portfolio model consisted of four stages: design a paper-based portfolio model to organize and create linkages among the liberal studies, professional, and experiential components of a DPT program; transform the model into an electronic format; create a demonstration e-portfolio tutorial; and evaluate the e-portfolio model via pilot testing. An interdepartmental team of faculty from physical therapy (PT), cooperative education, educational technology, and two PT students developed the e-portfolio model to map, visualize, and create linkages for student learning. Input to assist with the model design process was gathered from PT faculty and students through three focus groups. Data were collected on student experiences with the e-portfolio through written surveys at three points during the project and through face-to-face discussion. Written reflection, integral to the model, required students to reflect on artifacts selected from existing academic and experiential materials. Our project resulted in the generation of a reflective e-portfolio and tutorial customized for DPT students. Evaluation by users indicated that the reflective e-portfolio helped them organize and integrate academic and experiential learning and illuminate developmental transitions. Further research with a larger sample is indicated to examine the impact of e-portfolios on student learning and professional development.

This study describes a new noise suppression scheme for hearing aid applications based on the auditory masking threshold (AMT) in conjunction with a modified generalized minimum mean square error estimator (GMMSE) for individual subjects with hearing loss. The representation of cochlear frequency resolution is achieved in terms of auditory filter equivalent rectangular bandwidths (ERBs). Estimation of AMT and spreading functions for masking are implemented in two ways: with normal auditory thresholds and normal auditory filter bandwidths (GMMSE-AMT[ERB]-NH) and with elevated thresholds and broader auditory filters characteristic of cochlear hearing loss (GMMSE-AMT[ERB]-HI). Evaluation is performed using speech corpora with objective quality measures (segmental SNR, Itakura-Saito), along with formal listener evaluations of speech quality rating and intelligibility. While no measurable changes in intelligibility occurred, evaluations showed quality improvement with both algorithm implementations. However, the customized formulation based on individual hearing losses was similar in performance to the formulation based on the normal auditory system.

This study compared how normal-hearing listeners (NH) and listeners with moderate to moderately severe cochlear hearing loss (HI) use and combine information within and across frequency regions in the perceptual separation of competing vowels with fundamental frequency differences (deltaF0) ranging from 0 to 9 semitones. Following the procedure of Culling and Darwin [J. Acoust. Soc. Am. 93, 3454-3467 (1993)], eight NH listeners and eight HI listeners identified competing vowels with either a consistent or inconsistent harmonic structure. Vowels were amplified to assure audibility for HI listeners. The contribution of frequency region depended on the value of deltaF0 between the competing vowels. When deltaF0 was small, both groups of listeners effectively utilized deltaF0 cues in the low-frequency region. In contrast, HI listeners derived significantly less benefit than NH listeners from deltaF0 cues conveyed by the high-frequency region at small deltaF0's. At larger deltaF0's, both groups combined deltaF0 cues from the low and high formant-frequency regions. Cochlear impairment appears to negatively impact the ability to use F0 cues for within-formant grouping in the high-frequency region. However, cochlear loss does not appear to disrupt the ability to use within-formant F0 cues in the low-frequency region or to group F0 cues across formant regions.

Older adults with and without clinically significant hearing loss often report difficulty communicating in multitalker environments. Past work in our laboratory has shown that older listeners with hearing loss benefit less from differences in talkers’ vocal characteristics (e.g., fundamental‐frequency differences) relative to younger listeners with comparable hearing losses. The present study examined the extent to which age‐related changes in the perceptual organization of speech cues are due to task demand. Two experiments used the Coordinate Response Measure to examine how the benefit listeners derive from talker characteristics interacts with the complexity of the response task as a function of both age and hearing status. Stimuli were amplified to assure audibility for listeners with hearing loss. This study describes the formulation and evaluation of a new noise suppression scheme, with the goal of improving speech‐in‐noise perception by hearing‐impaired listeners. Arehart et al. [Speech Commun. (2003)] implemented and evaluated a noise suppression algorithm based on an approach that used the auditory masked threshold in conjunction with a version of spectral subtraction to adjust the parameters used in the subtraction process based on the masked threshold of the noise across the frequency spectrum. That original formulation was based on masking properties of the normal auditory system, with its theoretical underpinnings based on MPEG‐4 audio coding [Johnston (1988)]. The use of renewable energy sources (RES) as alternatives to fossil fuel for the production of thermal and/or electric energy has recently received much attention. Biomasses seem to be realistic alternative fuels leading to environmental, technical and economic benefits. However, to further increase the share of energy produced from biomass plants, it is necessary to improve the critical issues which to date have limited their spreading. In particular, particulate emissions and reliability are identified as the most determining limits. The paper describes two innovative technological solutions developed at the University of Bologna to further increase biomass plant marketability and shows a preliminary design on how they can be integrated in a biomass boiler for residential application. The first innovation is a high efficiency and low cost filter for particulate emissions: to reach a PM collection efficiency of 99.9%, wet scrubber was chosen as technological solution for the application on a 25 kW th biomass boiler installed in University of Bologna laboratory. The second innovation is the integration of a thermoelectric generator able to produce electricity directly from heat exchange. A prototype has been realized and tested in University of Bologna laboratory and represents a new approach in the design and application of thermoelectric generator. The paper shows how these new technologies can be integrated in a small size biomass boiler, including also an economic assessment.
