The Dutch Health Insurance Reform has resulted in more red tape and international studies show that health care reforms might contribute to more job stress and even burnout among physicians. This small study shows that job stress among Dutch GP’s is related to their perception of the consequences of the Dutch Health Insurance Reform. It also provides indications that as soon as GP’s perceive more job stress, they tend to be more concerned about the provision of the required quality of care. ---------------------------------------------------------------------------------------------------------- Gereguleerde marktwerking heeft niet alleen gevolgen voor patiënten, maar ook voor professionals zoals huisartsen. De stelselwijziging met marktwerking resulteert in hogere administratieve lasten en buitenlandse studies wijzen uit dat ingrijpende stelselwijzigingen doorgaans tot een hogere werkdruk en soms zelfs burnout onder artsen leiden. Het blijkt dat ook onder Nederlandse huisartsen meer werkdruk wordt ervaren en dat die werkdruk samenhangt met ervaren negatieve gevolgen van marktwerking. Het blijkt eveneens dat naarmate de werkdruk die de huisartsen ervaren hoger is, zij ook vaker bezorgd zijn of zij wel de vereiste kwaliteit van zorg kunnen leveren. 3 p.

© 2015 Macmillan Publishers Limited. Ruminant livestock are important sources of human food and global greenhouse gas emissions. Feed degradation and methane formation by ruminants rely on metabolic interactions between rumen microbes and affect ruminant productivity. Rumen and camelid foregut microbial community composition was determined in 742 samples from 32 animal species and 35 countries, to estimate if this was influenced by diet, host species, or geography. Similar bacteria and archaea dominated in nearly all samples, while protozoal communities were more variable. The dominant bacteria are poorly characterised, but the methanogenic archaea are better known and highly conserved across the world. This universality and limited diversity could make it possible to mitigate methane emissions by developing strategies that target the few dominant methanogens. Differences in microbial community compositions were predominantly attributable to diet, with the host being less influential. Samples from planted Italian paddy soil exhibited most probable numbers (MPN) of about 107 anaerobic propionate utilizers. In anoxic soil slurries that were either unamended or amended with rice straw production of CH4 was measured together with concentrations of H2, acetate and propionate. After a lag phase, during which ferric iron was depleted, CH4 was produced at a constant rate which was slightly higher in the straw-amended than in the unamended soil. Propionate concentrations were relatively low at about 5-15 μM. However, in the straw-amended soil propionate transiently accumulated to about 35 μM just after onset of methanogenesis. During the period of propionate accumulation H2 partial pressures were elevated and the Gibbs free energy (ΔG) of propionate consumption to acetate, bicarbonate and H2 was endergonic or higher than -3 kJ mol-1 propionate. Propionate concentrations decreased again when the ΔG decreased to more negative values. In unamended paddy soil, propionate did not accumulate transiently and ΔG was always < -6 kJ mol-1 propionate. Propionate radiolabelled in the C-1 or C-2 position was utilized with turnover times of 30-60 min. Propionate turnover rates approximately accounted for the rates of H2/CO2- dependent methanogenesis that were measured in experiments with [14C]bicarbonate. The only radioactive product of [1-14C]propionate was 14CO2. However, [2-14C]propionate was converted to radioactive acetate. CO2 and CH4. This observation indicates that propionate was consumed via a randomizing pathway to CO2 and acetate, the latter being then further degraded by acetotrophic methanogens to CO2 and CH4. Turnover of [1- 14C]propionate was almost completely inhibited by high H2 concentrations, chloroform or molybdate. The MPN of bacteria that utilized propionate either in syntrophy with methanogens or by reduction of sulfate was identical. All these observations suggest thai propionate was consumed by a syntrophic randomizing pathway, probably by bacteria that have also the capacity to reduce sulfate.

Various backscatter theories have been investigated in a tank experiment. A simple t.wo-scale model performed better than other more complicated models. In combination with an air-sea interaction module the two-scale model provides the physical basis for the present Viers scatterometer algorithm. A first test of the algorithm with ECMWF and ERS-1 data shows that the model σ0 values agree with those of the satellite. At low winds the algorithm performs better than the operational CMOD4.

Linked data provide an alternative route for the dissemination of spatial information compared to the traditional SOA-based SDI approach. The traditional approach has provided a wealth of standardized and structured location data based on Geography Markup Language (GML), while linked data provides an open mechanism for sharing and combining this data with anything, once the data is available as linked data. The first part of the paper focuses on deriving linked data from GML data. In the second part, we study how more meaningful data, expressed in Resource Description Framework (RDF) can be created from GML, given the underlying information model, by transforming it from Unified Modeling Language (UML) to Web Ontology Language (OWL).

OBJECTIVE: To learn which factors influencing perineal integrity were modifiable by physicians and pregnant women. DATA SOURCES: Medical, nursing, and midwifery literature was searched mainly for randomized controlled trials. STUDY SELECTION: We chose articles on perineal trauma pattern, sexual dysfunction or satisfaction, urinary incontinence, and pelvic floor function. We identified 80 papers and studied 16 in detail. SYNTHESIS: Five factors affected perineal integrity: episiotomy, third-trimester perineal massage, mother's position in second-stage labour, method of pushing, and administration of epidural analgesia. Episiotomy does not improve perineal outcomes when used routinely. Third-trimester perineal massage was discussed only in inadequate studies. Studies comparing position in birth chairs and recumbent versus upright positions were inadequate for making firm recommendations. Studies of methods of pushing and use of epidural analgesia were limited and uncontrolled; no recommendations were possible. CONCLUSION: Only limiting episiotomy can be strongly recommended. In the absence of strong data to the contrary, women should be encouraged to engage in perineal massage if they wish and to adopt the birth positions of their choice. Caretakers should be aware of the possibility of interfering with placental function when women hold their breath for a long time when pushing.

Lysosome-associated membrane protein-2 (LAMP-2) is a highly glycosylated protein and an important constituent of the lysosomal membrane1–7. Here we show that LAMP-2 deficiency in mice increases mortality between 20 and 40 days of age. The surviving mice are fertile and have an almost normal life span. Ultrastructurally, there is extensive accumulation of autophagic vacuoles in many tissues including liver, pancreas, spleen, kidney and skeletal and heart muscle. In hepatocytes, the autophagic degradation of long-lived proteins is severely impaired. Cardiac myocytes are ultrastructurally abnormal and heart contractility is severely reduced. These findings indicate that LAMP-2 is critical for autophagy. This theory is further substantiated by the finding that human LAMP-2 deficiency8 causing Danon’s disease is associated with the accumulation of autophagic material in striated myocytes. peerReviewed

National audience; Le travail a tout d’abord donné lieu à une synthèse des indices permettant de décrire la structure forestière puis à leur comparaison entre placettes de forêts exploitées et non exploitées issues d’un jeu de données existant. Les résultats montrent que la structure horizontale (indices de description de la distribution des diamètres) est sensible à l’exploitation, avec des valeurs en général plus fortes en forêt non exploitée qu’en forêt exploitée. Par contre, les indices décrivant la stratification ou le couvert ne répondent pas à l’abandon d’exploitation. Par ailleurs, si le lien avec la biodiversité semble peu documenté dans la bibliographie, nous avons pu analyser les corrélations sur 5 groupes taxonomiques : flore vasculaire, bryophytes oiseaux, chauve-souris, coléoptères carabiques. Il ressort de ces analyses que : (i)tous les groupes taxonomiques ne répondent ni aux mêmes indices de structure, ni de la même façon : les groupes peu mobiles, vivant au sol (carabes, plantes vasculaires, bryophytes) ont plutôt tendance à répondre à la structure forestière alors que les groupes plus mobiles (chauve-souris, oiseaux) y sont nettement moins sensibles ; (ii)tous les groupes écologiques de la flore vasculaire sont sensibles au couvert forestier (moyenne et écart-type), résultat qui est confirmé par une analyse approfondie de ce groupe (résultats non publiés issus du projet GNB). Les groupes écologiques des coléoptères carabiques et des bryophytes sont plus influencés par la distribution des diamètres (indices de variance). On peut noter que, pour les bryophytes, la magnitude de ces effets est assez forte ; (iii)d’une manière générale, quel que soit l’indice de structure sélectionné, il semble que les indices traduisant la variance sont plus pertinents à décrire la biodiversité que les indices dérivés de la simple moyenne.

Arrhythmogenic cardiomyopathy (ACM) is an inherited arrhythmia syndrome characterized by severe structural and electrical cardiac phenotypes, including myocardial fibrofatty replacement and sudden cardiac death. Clinical management of ACM is largely palliative, owing to an absence of therapies that target its underlying pathophysiology, which stems partially from our limited insight into the condition. Following identification of deceased ACM probands possessing ANK2 rare variants and evidence of ankyrin-B loss of function on cardiac tissue analysis, an ANK2 mouse model was found to develop dramatic structural abnormalities reflective of human ACM, including biventricular dilation, reduced ejection fraction, cardiac fibrosis, and premature death. Desmosomal structure and function appeared preserved in diseased human and murine specimens in the presence of markedly abnormal beta-catenin expression and patterning, leading to identification of a previously unknown interaction between ankyrin-B and beta-catenin. A pharmacological activator of the WNT/beta-catenin pathway, SB-216763, successfully prevented and partially reversed the murine ACM phenotypes. Our findings introduce what we believe to be a new pathway for ACM, a role of ankyrin-B in cardiac structure and signaling, a molecular link between ankyrin-B and beta-catenin, and evidence for targeted activation of the WNT/beta-catenin pathway as a potential treatment for this disease. This paper presents the outline and the achieved results of an experimental study developed to understand the differences on how close architecture spaces with distinct geometric characteristics at contour level, including rounded, curvilinear and sharp, rectilinear elements, are perceived and evaluated. In order to do so, eighteen virtual reality architecture spaces were evaluated by thirty-two test-subjects according to like/dislike aesthetic judgments. As expected, the tested subjects showed a higher level of preference for spaces with rounded, curvilinear contour elements. On another way, when the level of space curvature was high, considering the whole space surface and not only the contour of plan transitions, the level of preference decreased significantly. These results support the idea that rounded, curvilinear elements are interpreted as being more pleasant and preferred than sharp, rectilinear ones and create new knowledge on the how the levels of such preference are more accurate for moderate rather than radical curvature rates. Background/Aim:Liraglutide, a glucagon-like peptide-1 analog, induces weight loss. We investigated whether liraglutide affects gastric accommodation and satiation by measuring the intragastric pressure (IGP) during nutrient-drink consumption and using the barostat technique.Methods:Ten healthy volunteers (HVs) were tested after placebo, 0.3, 0.6 or 1.2 mg liraglutide administration. IGP was studied during intragastric nutrient-drink (1.5 kcal ml(-1)) infusion (60 ml min(-1)), while the HVs scored their satiation on a graded scale until maximal satiation. In a separate session, isobaric distentions were performed using the barostat with stepwise increments of 2 mm Hg starting from minimal distending pressure, although HVs scored their perception; gastric volume was monitored 30 min before and until 60 min after ingestion of 200 ml of nutrient drink.
