2. Free colon transfer: a versatile method for reconstruction of pharyngoesophageal defects with a large pharyngostoma

Free transverse colon transfers based on the middle colic vessels were used to reconstruct a pharyngoesophageal defect in 9 patients. In all patients, cancer invaded up to the oropharynx. Resection resulted in a larger pharyngostoma than normally seen following standard total pharyngolaryngoesophagectomy. In such cases, it can be difficult to achieve good swallowing function after reconstruction using the free jejunal transfer. This mainly results from a significant discrepancy between the diameter of the pharyngostoma and that of the jejunum. Compared to the jejunum, the transverse colon has a larger diameter with a longer vascular pedicle and can provide a longer, straight intestinal tube, facilitating easy passage of a food bolus. In our series, all transfers survived well and all patients had good swallowing function postoperatively. We consider that a free colon transfer is more suitable than a free jejunal transfer for pharyngoesophageal reconstruction in patients with a large pharyngostoma.

3. Effects of purified eicosapentaenoic acid ethyl ester on plasma lipoproteins in primary hypercholesterolemia

We investigated the effects of purified eicosapentaenoic acid (EPA) ethyl ester capsules (90% purity), which are free from cholesterol, saturated fatty acids and docosahexaenoic acid (DHA), on plasma lipoproteins and cholesteryl ester transfer protein (CETP) activity. We administered 2.7 g of EPA per day as capsules for 6 months to 14 primary hypercholesterolemic subjects. Total cholesterol, triglyceride and low density lipoprotein (LDL)-cholesterol levels in plasma were significantly reduced. The LDL cholesterol/apoB ratio and LDL particle size did not change. The ratio of high density lipoprotein (HDL)2/HDL3 cholesterol increased from 1.04 to 1.35 (p0.05), while the HDL cholesterol level did not change. CETP activity was significantly reduced. The reduction of CETP activity may explain the increase in the HDL2/HDL3 cholesterol ratio. These results suggest that purified EPA not only reduces LDL cholesterol levels but also acts on HDL metabolism in patients with hypercholesterolemia and therefore will be useful for the treatment of hypercholesterolemia.

5. Response of Muscat of Alexandria table grapes to post-veraison regulated deficit irrigation in Japan

- Abstract
The effects of post-veraison regulated deficit irrigation (RDI) on vine water status, ripening, and quality of table grapes, cv. Muscat of Alexandria grown under a polyhouse and root-zone restriction condition were investigated in the Okayama University Experimental vineyard, Japan. From bud break to veraison all vines were irrigated to a soil moisture tension of 3 kPa at a depth of 15 cm, and were re-irrigated when soil moisture tension approached 15 kPa. Starting 10 d after veraison, 3 irrigation regimes were imposed: (1) Control (C): Re-irrigation immediately when soil moisture tension reached 15 kPa; (2) Moderate Deficit Irrigation (MDI): Re-irrigation 2 d after reaching a soil moisture tension of 15 kPa; and (3) Severe Deficit Irrigation (SDI): Re-irrigation 4 d after reaching a soil moisture tension of 15 kPa. Treatments were continued for 6 weeks until harvest. By the end of the experiment, as the vine water status decreased, only SDI vines were wilted or necrotic in the fruit-zone. In SDI vines, the cumulative effect of increased vine water deficit indicated by lower Ψl resulted in berries that were lower in firmness and acidity, with a small increase in aroma, and a higher TSS than control at harvest. Modulation of interferon-gamma effect by other drug may enhance its tumor specific activity. The apoptosis inducing effect of interferon-gamma and its modulation by cyclosporin-A or tacrolimus (FK-506) were investigated in in vitro and ex vivo experiments. We found that a combination of cyclosporin-A (CsA) and recombinant interferon-gamma (rIFN-gamma) induced significant apoptosis in all four types of human gastric carcinoma cells tested but not in normal cells such as human peripheral blood mononuclear cells (PBMCs), human omentum-derived mesothelial cells, or human umbilical vein endothelial cells (HUVECs) in vitro. Apoptosis was also induced by a combination of rIFN-gamma with FK-506 but not with rapamycin. Next, the apoptosis-inducing effect of endogenous IFN-gamma combined with cyclosporin-A was examined using clinical samples. A streptococcal preparation, OK-432, was administered intraperitoneally for the management of 12 gastric cancer patients with malignant ascites. None of the gascitic fluids obtained before the OK-432 injection showed detectable IFN-gamma level. The OK-432 injection induced a detectable IFN-gamma production ranging from 6 to 89 pg/mL in ascitic fluids from 9 out of the 12 patients. A combination of CsA with ascitic fluids collected after but not before OK-432 injection induced significant apoptosis in MK-1 cells, a gastric carcinoma cell line. A positive correlation was found between the IFN-gamma level and CsA-induced apoptosis. The CsA-induced apoptosis was also blocked by a specific antibody against human IFN-gamma. These results indicated that both recombinant and endogenous IFN-gamma can induce potent tumor-apoptosis when combined with CsA.

We describe a possible layout and parameters of a polarized positron source for CLIC, where the positrons are produced from polarized gamma rays created by Compton scattering of a 1.3-GeV electron beam off a YAG laser. This scheme is very energy effective using high finesse laser cavities in conjunction with an electron storage ring. We point out the differences with respect to a similar system proposed for the ILC.
