Abstract

In der Arbeit wird uber das Ergebnis von 2 Untersuchungsreihen berichtet: 1. An einem Probandengut von 215 Normalpersonen wurde auf der Grundlage eines ausfuhrlichen Fragebogens und durch Vergleich verschiedener Tests versucht, eine aussagekraftige in kurzer Zeit durchfuhrbare Methode zur praktischen Bestimmung von Handigkeit und Hemispharen-dominanz in der Neurochirurgie zu finden. Sie besteht im Handeklatschtest, einem Zeichentest und einem neuartigen Geschicklichkeitstest. Nachprufung an grosem Krankengut und Beachtung der Dominanzfrage durch alle Neurochirurgen wird empfohlen, da sie nicht nur fur die Neurochirurgie von Wichtigkeit ist, sondern auch zur Klarung offener Fragen auf diesem Gebiet beitragen kann. 2. Ein neurochirurgisches Krankengut von 193 Patienten mit einseitigen Groshirngeschwulsten wurde in bezug auf Handigkeit und Hemispharen-dominanz untersucht. Die Ergebnisse entsprachen der Erwartung, das im allgemeinen bei Rechtshandern die linke Hemisphare, bei Linkshandern die rechte Hemisphare die dominante ist, jedoch sind Ausnahmen und die nicht ganz seltene Ambivalenz der Hemispharen zu beachten. Die Sicherheit der Untersuchungsergebnisse in dieser Reihe war durch die mangelhaften Handigkeitsprufungen der Patienten beeintrachtigt, ist aber auch teilweise durch ihren Zustand vermindert, der eingehende Tests nicht erlaubt. Daher wird vorgeschlagen, gelegentlich von vorsorgenden Untersuchungen, auch mit kurzen Tests, die Handigkeit bei Normalpersonen zu prufen und zu registrieren.

Abstract

An totem Fett-, Muskel-, Leber- und Hirngewebe wurden nach Abkuhlung auf tiefe Temperaturen mittels der Cooperschen Kaltesonde und bei festgelegter Ausgangstemperatur der Gewebe von 37,4°C Temperaturregistrierungen vorgenommen. Es zeigte sich, das die am Cooperschen Kaltegerat eingestellten Temperatursollwerte mit den Temperaturen ubereinstimmen, die man in Wasser wie auch in Geweben in der Nahe der Sondenspitze findet. Die hier gemessenen Temperaturen liegen weithoher. Der Effekt last sich durch die hohe Warmeleitfahigkeit des versilberten Mantels der Kaltesonde erklaren. Das Herdmuster bzw. die Form der Isothermen der entstandenen Kalteherde waren fur alle Kaltegrade und fur alle gepruften Gewebe gleich und lediglich abhangig von der geometrischen Form der Sondenspitze. Bei der Unterkuhlung im Innern verschiedenartiger Gewebe entstanden stets kugelformige Vereisungsgebiete, die ebenfalls fur alle gepruften Gewebe von etwa gleicher Grose waren. Der Wassergehalt der gepruften Gewebe war dabei annahernd gleich, die geringen Unterschiede der Herdgrosen bedurfen noch der Aufklarung. Die Vereisung ist von der Warmeleitfahigkeit und die Warmeleitfahigkeit vom Wassergehalt der Gewebe abhangig. Am lebenden Gewebe sind wahrscheinlich noch andere Gewebseigenschaften zu berucksichtigen. Die unterkuhlte Randzone zwischen der Oberflache der Vereisungskugeln und dem nicht mehr gekuhlten, auf 37,4°C gehaltenen Gewebe ist fur alle Temperaturen von geringer und gleicher Ausdehnung. Eine Kaltekonvektion last sich in Flussigkeiten, nicht dagegen in Geweben nachweisen.

Abstract

The electroanatomy of the caudal nuclei of the thalamus was studied in 23 patients, who underwent stereotactical procedures for the treatment of intractable pain or Torticollis. As in the observations on the ventro oral nucleus of the thalamus, the dominant thalamus when stimulated would produce effects on speech. Nearly all stimulations produced synchronic muscular contractions together with sensory effects in the corresponding region. Usually a large region of the body was affected at the same time. The topographical arrangement differed from the arrangement in the ventro oral nucleus of the thalamus; the face and the inside of the mouth and throat were located highest orally and more medially and anteriorly, corresponding to the nucleus Flechsig and its borderline to the centre median. The leg was posterior, most lateral and deepest ventrally, the other body regions were arranged in between. Stimulation of the VJM produced torsion movements only. Coagulation in the dominant hemisphere often had the consequence of a long lasting Korsakov syndrom. Vegetative effects were much less outspoken than in the anterior parts of the thalamus.

Abstract

Our observations confirm the rule that certain effects of stimulation have to be considered as abnormal integrations of innervation in diseased brains, similar to the peculiar effects which can be produced in Parkinsons disease and in athetosis and which are not seen in patients with normal motility. This may hold true for single observations like the peculiar light phenomena in unexpected places or the erotic behaviour which we have seen in 2 patients, once in the pallidum, once in the amygdala and the Basedow-like syndrome which could be elicited in one of our patients. Another group of phenomena may be observed with high regularity in all individuals. They are more diffuse and less colourful, for instance slowing of thought and tiredness in stimulation of the fornix and vegetative effects like those on “warm and cold” around the anterior commissure, where the production of erythema, sweatting and feeling of warmth is produced on a more dorsal level and goose skin and feeling cold in a more ventral level. This observation show the wide extension of this system which probably has to do with vasomotor control in regulating temperature but also under emotional effects. The focus of most exact functional control of this mechanism is evidently in the oral parts of the brain. The effects in the regions further behind, namely in the basal ganglia, the thalamus, the internal capsule and the hypothalamus may be produced by stimulation of fibre bundles of this system, radiating horizontally and backwards from their oral control until approximately to a point 10 mm behind the intercommissural midpoint, from where on these fibres seem to disappear caudally, so that they cannot be stimulated anymore in the caudal part of the thalamus. With great regularity we see a combination of sham movements and nausea and vomiting in the basal parts of the entire basal ganglia system.

Abstract

1. After the monosynaptic reflex we found the well know silend period. Without background-innervation this inhibition phase has a duration of 300–500 msc. During voluntary innervation the silent period is shortened to 100–140 msc. 2. In a previous publication we could demonstrate that autogenetic inhibition is the main source of the silent period. 3. In spastic patients the silent period is 100–140 msc. wether there is a innervation or not. 4. We assume that the limit of the silent period, the rebound, is due to intraspinal activation of excitatory interneurones following activation of the inhibitory δ-interneurones (spinal contrast of Sherrington). 5. Spasticity is not only a overemphasized γ-innervation but also a hypersensibility of the excitatory interneurones.

Abstract

In the last few years attention has been directed to the possible role of the ventrolateral area of the thalamus in the pathophysiology of tremor and rigor. Neurosurgical experience (Hassler and Riechert 1 ; Cooper 2 ) indicates that lesions of this region are able to influence favorably or to eliminate these pathological phenomena in patients with Parkinson's disease. An apparently plausible explanation for the success of these operations has been offered by Hassler. 3 He subdivides into several zones, the nuclear area called by Walker 4 and other American writers the nucleus ventralis lateralis. Of particular interest is the nucleus ventralis oralis anterior and posterior; the anterior nucleus receives impulses from the pallidum and it transmits them to the premotor cortex (area 6aα). Cerebellofugal impulses enter the nucleus ventralis oralis posterior; from here they are projected to the motor region (area 4γ).
