Background/Aim: There is increasing evidence that patients infected with SARS-CoV-2 develop neurological manifestations such as encephalitis. The purpose of this article was to present a case of viral encephalitis associated with SARS-CoV-2 in a 14-year-old child with Chiari malformation type I. Case Report: The patient manifested frontal headache, nausea, vomiting, skin pallor, right side Babinski sign and was diagnosed with Chiari malformation type I. He was admitted with generalized seizures and suspected encephalitis. Brain inflammation and viral RNA in the cerebrospinal fluid suggested SARS-CoV-2 encephalitis. These findings indicate that the SARS-CoV-2 test in CSF of patients with neurological manifestations, confusion, and fever during the COVID-19 pandemic should be carried out even when there is no evidence of respiratory infection. To our knowledge, this presentation of encephalitis associated with COVID-19 has not yet been reported in a patient with a congenital syndrome such as Chiari malformation type I. Conclusion: Further clinical data are needed to determine the complications of encephalitis due to SARS-CoV-2 in patients with Chiari malformation type I to standardize diagnosis and treatment.

The growing popularity of Deep Neural Networks, which often require computationally expensive training and access to a vast amount of data, calls for accurate authorship verification methods to deter unlawful dissemination of the models and identify the source of the leak. In DNN watermarking the owner may have access to the full network (white-box) or only be able to extract information from its output to queries (black-box), but a watermarked model may include both approaches in order to gather sufficient evidence to then gain access to the network. Although there has been limited research in white-box watermarking that considers traitor tracing, this problem is yet to be explored in the black-box scenario. In this paper, we propose a black-and-white-box watermarking method that opens the door to collusion-resistant traitor tracing in black-box, exploiting the properties of Tardos codes, and making it possible to identify the source of the leak before access to the model is granted. While experimental results show that the method can successfully identify traitors, even when further attacks have been performed, we also discuss its limitations and open problems for traitor tracing in black-box., Comment: This work has been submitted to the IEEE International Workshop on Information Forensics and Security (WIFS) 2023 for possible publication. Introduction: Clubfoot is usually diagnosed immediately after birth simply by looking at the foot. Surgeons have struggled over the years to identify the best method of treatment for congenital clubfoot deformity. This struggle has lessened over the last decade as the Ponseti method of clubfoot manipulation and casting, Achilles tendon tenotomy, and foot abduction bracing has become the primary treatment for idiopathic clubfoot around the world. Today, the Ponseti method is highly acclaimed due to the more satisfactory results and the reduction in the need for extensive surgical releases. Ponseti management is recognized as the gold standard for the treatment of clubfoot adduct congenital. This method has been used today in all pediatric orthopedic centers in the world as the first and best treatment option for significantly reducing clubfoot surgical treatment for correction. Although early treatment through conservative techniques has an excellent result, there are cases in which adequate treatments are not available or the defect may persist, bringing with it a greater challenge when correcting it, even leaving a permanent disability. Objective: In this study, we aim to systematically search the scientific literature around the types and effectiveness of available treatments for CETV in the first weeks of birth and the subsequent stages. Methods: We will conduct a systematic scoping review to identify published empirical evidence from January 1st 2000 to October 30th 2020. We will search through MEDLINE, SCOPUS and Google Scholar databases for relevant studies on the effectiveness of any treatment for clubfoot. We will extract data on the types of treatments and the control treatments employed to assess their effectiveness.

ABSTRACT In this paper we propose to observe under what conditions two subspaces L and S in R^n, of dimensions 1 and n-2, translations of the origin, i.e., affine subspaces, intersect. From a necessary and sufficient condition for such intersection to exist, the cross product or vector product in this vector space is defined. In turn, the demonstration unveils a new method in the resolution of a system of n equations with n unknowns.

Outcomes of neonatal encephalopathy (NE) have improved since the widespread implementation of therapeutic hypothermia (TH) in high-resource settings. While TH for NE in term and near-term infants has proven beneficial, 30-50% of infants with moderate-to-severe NE treated with TH still suffer death or significant impairments. There is therefore a critical need to find additional pharmacological and non-pharmacological interventions that improve the outcomes for these children. There are many potential candidates; however, it is unclear whether these interventions have additional benefits when used with TH. Although primary and delayed (secondary) brain injury starting in the latent phase after HI are major contributors to neurodisability, the very late evolving effects of tertiary brain injury likely require different interventions targeting neurorestoration. Clinical trials of seizure management and neuroprotection bundles are needed, in addition to current trials combining erythropoietin, stem cells, and melatonin with TH. IMPACT: The widespread use of therapeutic hypothermia (TH) in the treatment of neonatal encephalopathy (NE) has reduced the associated morbidity and mortality. However, 30-50% of infants with moderate-to-severe NE treated with TH still suffer death or significant impairments. This review details the pathophysiology of NE along with the evidence for the use of TH and other beneficial neuroprotective strategies used in term infants. We also discuss treatment strategies undergoing evaluation at present as potential adjuvant treatments to TH in NE.

The COVID-19 pandemic will leave an indelible mark on the careers of current medical trainees. Given the disruptions to medical education, economic impact on institutions, and the uncertainties around future job prospects, trainees are facing unprecedented challenges. This situation is especially concerning for futures of pediatric physician-scientist trainees, where concerns regarding maintaining the pipeline were well documented prior to the emergence of COVID-19. In this Perspectives article, we leverage the unique expertise of our workgroup to address concerns of physician-scientist trainees and to provide suggestions on how to navigate career trajectories in the post-COVID-19 era. We identified and addressed four major areas of concern: lack of in-person conferences and the associated decrease access to mentors and networking activities, decreased academic productivity, diminished job prospects, and mental health challenges. We also suggest actions for trainees, mentors and educational leaders, and institutions to help support trainees during the pandemic, with a goal of maintaining the pediatric physician-scientist pipeline.

EUCLIDS consortium., Fever is the most common reason that children present to Emergency Departments. Clinical signs and symptoms suggestive of bacterial infection are often non-specific, and there is no definitive test for the accurate diagnosis of infection. The ‘omics’ approaches to identifying biomarkers from the host-response to bacterial infection are promising. In this study, lipidomic analysis was carried out with plasma samples obtained from febrile children with confirmed bacterial infection (n = 20) and confirmed viral infection (n = 20). We show for the first time that bacterial and viral infection produces distinct profile in the host lipidome. Some species of glycerophosphoinositol, sphingomyelin, lysophosphatidylcholine and cholesterol sulfate were higher in the confirmed virus infected group, while some species of fatty acids, glycerophosphocholine, glycerophosphoserine, lactosylceramide and bilirubin were lower in the confirmed virus infected group when compared with confirmed bacterial infected group. A combination of three lipids achieved an area under the receiver operating characteristic (ROC) curve of 0.911 (95% CI 0.81 to 0.98). The RABVG ectodomain is a homotrimer, and trimers are often called spikes. They are responsible for the attachment of the virus through the interaction with nicotinic acetylcholine receptors, neural cell adhesion molecule (NCAM), and the p75 neurotrophin receptor (p75NTR). This makes them relevant in viral pathogenesis. The antigenic structure differs significantly between the trimers and monomers. Surfaces rich in hydrophobic amino acids are important for trimer stabilization in which the C-terminal of the ectodomain plays an important role; to understand these interactions between the G proteins, a mechanistic study of their functions was performed with a molecular model of G protein in its trimeric form. This verified its 3D conformation. The molecular modeling of G protein was performed by a I-TASSER server and was evaluated via a Rachamandran plot and ERRAT program obtained 84.64% and 89.9% of the residues in the favorable regions and overall quality factor, respectively. The molecular dynamics simulations were carried out on RABVG trimer at 310 K. From these theoretical studies, we retrieved the RMSD values from Cαatoms to assess stability. Preliminary model of G protein of rabies virus stable at 12 ns with molecular dynamics was obtained.

With the objective of exploiting hardware capabilities and preparing the ground for the next-generation X-band synthetic aperture radar (SAR) missions, TerraSAR-X and TanDEM-X are now able to operate in staring spotlight mode, which is characterized by an increased azimuth resolution of approximately 0.24 m compared to 1.1 m of the conventional sliding spotlight mode. In this paper, we demonstrate for the first time its potential for SAR tomography. To this end, we tailored our interferometric and tomographic processors for the distinctive features of the staring spotlight mode, which will be analyzed accordingly. By means of its higher spatial resolution, the staring spotlight mode will not only lead to a denser point cloud, but also to more accurate height estimates due to the higher signal-to-clutter ratio. As a result of a first comparison between sliding and staring spotlight TomoSAR, the following were observed: 1) the density of the staring spotlight point cloud is approximately 5.1--5.5 times as high; 2) the relative height accuracy of the staring spotlight point cloud is approximately 1.7 times as high., Comment: Accepted publication in IEEE Journal of Selected Topics in Applied Earth Observations and Remote Sensing

A continuous improvement project (CIP) is a structured improvement project using a team of people "typically representing different departments or units in the organization" working to improve a process or work area over a relatively short period of time, such as a few days or up to several months. A CIP may use different improvement methodologies and tools, and may thus be defined according to the improvement approach. For instance, an organization adopting Lean as an improvement approach is likely to have CIPs implementing Lean tools, such as 5S or value stream mapping. These projects may be referred to as Lean projects in general, although they may also represent accelerated improvement projects such as Kaizen events, Kaizen blitz, or rapid improvement projects. Alternatively, an organization utilizing Six Sigma as an improvement approach may have Six Sigma projects that use the Define-Measure-Analyze-Improve-Control (DMAIC) process and statistical tools. Some organizations adopt an integrated improvement approach, such as Lean Six Sigma, and therefore may have CIPs with an even broader set of tools from which to choose. Lastly, many organizations may have an improvement approach not characterized by any single set of improvement processes and tools, and thus, may be thought of generally as process improvement, or quality improvement, projects using a traditional methodology as plan-do-study/check-act (PDSA or PDCA). In this dissertation, all of these types of improvement projects are referred as CIPs. Since the 1980s, hospitals have been using CIPs to address some of the problems in hospitals, such as quality in healthcare delivery, internal process efficiency, communication and coordination, and the cost of services. Some hospitals have achieved significant improvements, such as reducing the turnaround time for clinical laboratory results by 60 percent and reducing instrumentation decontaminations and sterilization cycle time by 70 percent. However, as with many other companies, hospitals often experience difficulty achieving their desired level of improvements with CIPs. Therefore, the purpose of this dissertation is to identify the critical success factors (CSFs) related to CIP success. In order to achieve this goal, five objectives were achieved: creating a methodology to assess the maturity or evolution of a research field (manuscript #1), identifying a comprehensive list of CSFs for CIPs (manuscript #2), assessing the maturity of the published literature on CIPs in hospitals (manuscript #3), identifying the most important factors related to CIPs in hospitals (manuscript #4) , and conducting an empirical investigation to define the CSFs for CIPs in hospital settings (manuscript #5 and #6). This investigation was conducted in three phases: research framing, variable reduction, and model development and testing. During these phases, the researcher used the following methodologies and data collection tools: systematic literature review, maturity framework (developed as part of this dissertation), expert study, retrospective survey questionnaire, exploratory factor analysis, partial-least squares structural equation modeling, and regression modeling. A maturity framework with nine dimensions was created (manuscript #1) and applied in order to identify a list of 53 factors related to CIP in general, involving any organization (manuscript #2). Additionally, the maturity framework was used to assess the literature available on CIPs in hospitals, considering only the authorship characteristic dimension (manuscript #3). Considering the frequency of new authors per year, the relative new integration of research groups, and the limited set of predominant authors, the research field, or area, of CIPs in hospitals is one with opportunities for improving maturity. Using the systematic literature review from manuscript #3, the list of 53 factors, and the list of predominant authors, a review of the literature was conducted, along with an expert study to more fully characterize the importance of various factors (manuscript #4). A conclusion from this particular work was that it is not possible to reduce the list of 53 factors based on these results, thus, a field study using the complete comprehensive list of factors was determined to have stronger practical implications. A field study was conducted to identify factors most related to CIP perceived success (manuscript #5) and CIP goal achievement (manuscript #6). The final results and practical implications of this dissertation consist in the identification of the following CSFs for CIP success in hospitals: Goal Characteristics, Organizational Processes, Improvement Processes, and Team Operation. These CSFs include several specific factors that, to the researcher's knowledge, have not been previously studied in empirical investigations: goal development process, organizational policies and procedures, CIP progress reporting, and CIP technical documentation. Practitioners involved with CIPs, such as CIP leaders, facilitators, stakeholders/customers, and continuous improvement managers/leaders, can utilize these results to increase the likelihood of success by considering these factors in planning and conducting CIPs. En el Trabajo de Fin de Máster queremos defender una determinada visión de lo que para nosotros representa un docente: cuáles son sus objetivos, sus motivaciones e intereses. Para elaborar este perfil del profesor nos guiaremos a través de cinco competencias que deben servir como hilo invisible y conector de las distintas dimensiones de la figura del docente que queremos defender. Posteriormente, justificaremos la razón por la que hemos elegido los dos temas fundamentales que debemos poner sobre la mesa (Programación didáctica de la educación ético cívica de 4º de la E.S.O y la unidad didáctica Poder y medios de comunicación). Nos hallaremos en disposición para introducir las comparaciones que nos susciten y provoquen dichos temas. La propuesta docente que queremos defender está basada en un modelo flexible, abierto, experiencial y con una cierta carga audiovisual, que ayude a sobrellevar la parte más memorística y puramente teórica. Finalmente concluiremos el trabajo con una reflexión personal con respecto a lo visto en el Máster y establecer, o más bien abocetar, algunas propuesta de mejora en el futuro.

Helicobacter pylori is the primary cause of gastric cancer. However, monoclonal Epstein-Barr virus (EBV) nucleic acid is also present in up to 10% of these tumors worldwide. EBV prevalence is increased with male sex, non-antral localization and surgically disrupted anatomy. To further examine associations between EBV and gastric cancer, we organized an international consortium of 11 studies with tumor EBV status assessed by in situ hybridization. We pooled individual-level data on 2,648 gastric cancer patients, including 184 (7%) with EBV-positive cancers; all studies had information on cigarette use (64% smokers) and 9 had data on alcohol (57% drinkers). We compared patients with EBV-positive and EBV-negative tumors to evaluate smoking and alcohol interactions with EBV status. To account for within-population clustering, multi-level logistic regression models were used to estimate interaction odds ratios (OR) adjusted for distributions of sex (72% male), age (mean 59 years), tumor histology (56% Lauren intestinal-type), anatomic subsite (61% noncardia) and year of diagnosis (1983–2012). In unadjusted analyses, the OR of EBV positivity with smoking was 2.2 (95% confidence interval [CI], 1.6–3.2). The OR was attenuated to 1.5 (95% CI, 1.01–2.3) by adjustment for the possible confounders. There was no significant interaction of EBV status with alcohol drinking (crude OR, 1.4; adjusted OR, 1.0). Our data indicate the smoking association with gastric cancer is stronger for EBV-positive than EBV-negative tumors. Conversely, the null association with alcohol does not vary by EBV status. Distinct epidemiologic characteristics of EBV-positive cancer further implicate the virus as a co-factor in gastric carcinogenesis.

This article is related to the use of the rubric as a resource in the assessment of competences, in order to experience and appreciate the educational scope of this tool in a particular university teaching context. The study, which is part of a broader research project (Cebrian, 2011, dir.), was carried out last year in the Education Degree subject "Prevention and treatment of children’s learning difficulties". The aim was to explore the extent of the rubric’s effectiveness as a tool for self-assessment of learning content and of "teamwork" competence, at the same time as experimenting with new forms of assessment in that area and involving the students in the assessment process. It is an experimental study involving 70 students divided into six experimental groups and six control groups, collecting data through different sources of information and analyzing the evolution of learning according to percentage change formula (Tellado, 2001), to ascertain at what point of the teaching-learning process the rubric is more effective. The results show that this is a tool that contributes to the learning of skills by students, that they are satisfied with its use and that it is in the third application of the resource that the best results are obtained.

This article is related to the use of blogs and rubrics as resources in networked learning assessment in order to value if these tools could serve as complement or supplement in the process. The sample was composed of 118 students who were reading and “Didactic and curriculum” (Pedagogy Degree) from the University of Barcelona; and “New Technology applied to Childhood Education” (Degree in Childhood Education) and “New Technology applied to Primary Education” (Degree in Primary Education), both from de University of Vigo. The methodology is part of I+D Project EDU2010-15432, than have allowed to study two groups of students: group 1 made up for that students who made their personal diary in blogs, as an element to increase the structure, synthesis, self-analysis and reflection about their learning; and group 2 made up for students who, also using blogs, used self-assessment rubrics as a complement or supplement about them. It expects to denote the rubrics influence has been both mastery of content and skills development, and involve students in the assessment process when blogs are used in it. The results show that the use of the rubrics as a complementary element to blogs, intensify the benefits fs those when working competences development.

The Sheffield University Nova Balloon Lifted Solar Telescope (SunbYte) is a high-altitude balloon experiment devised and run largely by students at the University of Sheffield, and is scheduled for launch in October 2017. It was the only UK project in 2016 to be selected for the balloon side of the Swedish-German student programme REXUS/BEXUS (Rocket and Balloon Experiments for University Students; see box on p2.25). The success of the SunbYte team in the REXUS/BEXUS selection process is an unprecedented opportunity for the students to gain valuable experience working in the space engineering industry, using their theoretical knowledge and networking with students and technology companies from all over Europe.

En las últimas décadas, desde que la secuenciación del genoma se convirtió en un fenotipo de bajo coste de obtención, numerosos campos de la salud se desarrollaron como consecuencia de esta amplia disponibilidad de datos genómicos. La secuenciación de nueva generación (Next generation sequencing, NGS) trajo posibilidades sin precedentes para la investigación, como los estudios de asociación de todo el genoma (Genome-Wide Asociation Studies, GWAS). La creciente asequibilidad de la secuenciación del genoma y, como consecuencia, la disponibilidad generalizada de datos genómicos abre nuevas oportunidades en el campo de la medicina, allanando el camino hacia la medicina personalizada (también conocida como medicina genómica), los diagnósticos médicos potencialmente mejorados y la predicción de riesgos de enfermedades mediante la realización de pruebas de susceptibilidad en la atención médica de un futuro cercano. Los servicios directos al consumidor (Direct-to-Customer, DTC) como OpenSNP1 o 23andMe2 permiten a las personas compartir sus datos genómicos y determinar sus antepasados. Por otro lado, este rápido aumento también plantea graves riesgos de privacidad debido a la naturaleza intrínsecamente sensible de la información genómica. Dado que la información sobre el ADN es única y está correlacionada entre los miembros de la familia, no puede considerarse solo como una cuestión de privacidad individual. Estos datos genómicos tienen serias implicaciones en la privacidad, ya que incluyen información sobre el fenotipo, la etnia, los miembros de la familia, las enfermedades, etc., de un individuo. Por estos motivos, los casos legales se tuvieron que adaptar a la realización creciente de pruebas de paternidad, y los forenses avanzaron al investigar las escenas del crimen con pruebas de identidad genómica. No obstante, para cubrir la gama más amplia de principios fundamentales de protección de datos, mediante la minimización del uso de datos personales, la maximización de la seguridad de los datos y el empoderamiento de las personas, se acuñó la expresión “Tecnologías de mejora de la privacidad” (PETs, de Privacy Enhancing Technologies en inglés). De acuerdo con la definición de la Agencia de la Unión Europea para la Seguridad de las Redes y la Información (ENISA), las PETs son: "Soluciones de software y hardware, es decir, sistemas que abarcan procesos, métodos o conocimientos técnicos para lograr una funcionalidad específica de protección de datos o privacidad o para proteger contra los riesgos de privacidad de un individuo o un grupo de personas físicas". Las PETs se aplican a medidas técnicas generalizadas para garantizar la privacidad en varios campos, desde la salud electrónica hasta el análisis de datos de tráfico. Estas implicaciones de privacidad pueden a veces ser incluso barreras tanto para la investigación médica como para la disponibilidad de datos genómicos. Por lo tanto, recientemente, algunos grupos de investigación han propuesto técnicas de preservación de la privacidad para procesar datos genómicos. Debido a la necesidad de métodos que mejoren la privacidad, se han así implementado varias soluciones que permiten a los científicos trabajar con datos genómicos mientras se protege la información confidencial. Hasta ahora, los métodos más prometedores se basan en la criptografía. Estas soluciones, además, son escalables y eficientes, lo que garantiza la privacidad de las personas y el poder de gestión de acceso. Investigamos, diseñamos y desarrollamos métodos para proteger esta información sensible de accesos no autorizados durante el procesamiento, minimizando la fuga de datos durante las transacciones de datos y garantizando la seguridad de las soluciones propuestas contra diferentes tipos de comportamientos maliciosos. Además, como aplicación alternativa de nuestros métodos de mejora de la privacidad, se propone una adaptación destinada a la optimización de los métodos de publicidad dirigida sobre los datos de comportamiento en línea del individuo. En resumen, en esta tesis, nos centramos inicialmente en el diseño de protocolos genéricos para aplicaciones de salud digital conscientes de la privacidad, específicamente en el campo de la "privacidad genómica". Extendemos nuestros métodos a la protección de datos en las redes sociales, es decir, al área de la "publicidad dirigida". The field of genomic research has considerably grown in recent years due to the unprece-dented advances conducted by Next Generation Sequencing (NGS). Developments in this field have resulted in the current high availability of genomic data. The increasing affordabil-ity of genome sequencing and, as a consequence, widespread availability of genomic data open up new opportunities in the field of medicine. On the other hand, this rapid increase also poses severe privacy risks due to the inherently sensitive nature of genomic information. As DNA information is unique and correlated among family members, it cannot be regarded as just a matter of individual privacy concern. The availability of genomic data raises profound privacy implications as it includes information about an individual’s phenotype, ethnicity, family members, disease conditions, and more. These privacy implications may sometimes even be barriers to both medical research and the availability of genomic data. Therefore, recently, researchers proposed privacy-preserving techniques to process genomic data. Scien-tists deploy various solutions enabling them to work on genomic data due to the need for privacy-enhancing methods to protect these sensitive pieces of information. So far, the most promising methods are based on cryptography. Frente al tema de la estabilidad laboral de empleados públicos de libre nombramiento y remoción, específicamente sobre prepensionados y sujetos en situación de debilidad manifiesta, resulta pertinente determinar si la jurisprudencia es uniforme. Además, el hecho de medir la decisión judicial con los estándares interamericanos y la teoría de los derechos sociales fundamentales, consolida una estrategia jurídica novedosa, que no solo contribuirá en la construcción de un criterio válido, y eficaz para la Función Pública, sino que también permitirá elevar los márgenes de garantía de los derechos sociales, de cara a un Estado Social de Derecho, fundado en la especial protección constitucional de ciertos grupos sociales en condición de vulnerabilidad, más aún si desarrollan algún tipo de actividad -como la laboral- en el escenario de la administración pública.
