This study aimed to observe the analgesic effect of the cocktail formulation with diprospan during total hip arthroplasty (THA).From September 2018 to April 2019, 120 patients undergoing primary unilateral THA were included in this prospective, randomized, observer-blinded study. Patients were randomized into three groups, according to the different local infiltration analgesia (LIA) strategies: LIA with ropivacaine (the ropivacaine group, n = 40), LIA with a new cocktail containing ropivacaine, diprospan, and morphine (the cocktail group, n = 40), and the control group (n = 40). The primary outcomes included postoperative pain scores. The resting visual analogue scale (VAS) scores were measured at 2, 6, and 12 h after the surgery (a.m. and p.m.) on postoperative day (POD) 1, POD2, and the day of discharge. Movement VAS scores were assessed at 6 h, 12 h after the operation (a.m. and p.m.) on POD1, POD2, and the day of discharge. The secondary outcomes included opioid consumption, postoperative hospital stay, range of motion of the hip at discharge, patient satisfaction, and the results of the follow-up.After the screening, 120 patients were randomized into three groups (40 patients in each group). All of the patients completed the trial. The resting VAS scores in the ropivacaine group and cocktail group at 2 h were lower than those in the control group (P 0.001 and P 0.001, respectively, F = 17.054), and the same trend was also postoperatively found at 6 h (p = 0.005 and P = 0.002, F = 6.212). Twelve hours after the operation, the pain score in the cocktail group was lower than that in the other two groups, but only the difference between the cocktail group and the control group was statistically significant (P = 0.018, F = 3.144). From the morning of the first postoperative day to the a.m. on POD 2, the VAS scores in the cocktail group were significantly lower than those in the ropivacaine group and the control group. Furthermore, the movement VAS scores in the ropivacaine group and the cocktail group were better than those in the control group at 6 and 12 h post-operation (P 0.05). The per capita opioid consumption in the cocktail group was less than that in the ropivacaine group and the control group within 24 h post-operation. There were no significant differences in the comparison of additional indicators among the three groups.The new cocktail with diprospan had a better result and longer duration time for early postoperative pain control in primary THA via the posterolateral approach under general anesthesia, especially for treating resting pain.

Objective The aim of the present study was to evaluate the 10‐year outcomes of cementless total hip arthroplasty (THA) in adult patients with the late sequelae of septic arthritis of the hip. Methods We followed 166 consecutive patients (168 hips) who underwent cementless THA between March 2001 and December 2011. There were 79 men and 87 women, with a mean age of 50.4 years (range 21–76 years) at the time of index THA, all of whom had hip osteoarthritis secondary to hip pyogenic infection. The average duration of follow up was 10.6 years (range 6.9–17.2 years). Preoperative and postoperative clinical ratings were evaluated, including the hip dysfunction and osteoarthritis outcome score (HOOS), the Harris hip score (HHS), range of motion, a 100‐point visual analog scale for hip pain, and the severity of limp and limb length discrepancy (LLD). The anteroposterior and lateral radiographs of the hip and full‐length view of the lower extremities were obtained to assess the position of the components, radiolucent lines, osteolysis, loosening of components, and heterotopic ossification. The intraoperative and postoperative complications were also recorded. Results The mean HSS and hip pain score were 44.2 points (range 29–66 points) and 42.5 points (range 32–64 points), respectively, before the index surgery and significantly improved to 88.1 points (range 78–96 points) and 15.1 points (range 10–26 points), respectively, at final follow‐up examination. The HOOS and range of motion also improved significantly. The mean limb length discrepancy was reduced from 2.6 to 0.8 cm. The limp at last follow‐up examination was moderate in 3 cases because of hip osteoarthritis in the other limb, slight in 26, and absent in 137. A radiolucent line was observed in 12 hips (7.1%) around the acetabular or femoral components. A progressive radiolucent line around the undersized femoral stem in all zones was seen in 1 hip, resulting in aseptic loosening and breakage of the femoral component. There were 8 cases of intraoperative fracture, 7 cases of dislocation, and 7 cases of transient nerve palsy. Recurrence of infection occurred in 2 hips. Revision surgery was conducted in 2 hips because of isolated loosening of the acetabular cup and the femoral stem, respectively. Kaplan–Meier survival was 97% at 10 years, with revision for any reason with any component as an end‐point. Conclusion Improved surgical techniques and development of components with various sizes provided favorable results for cementless THA conducted for late sequelae of sepsis in these young and active patients. Although the incidence of complications was relatively high, the complications were treated successfully., Favorable results can be obtained from cementless THA for late sequelae of pyogenic arthritis. Although the incidence of complications is relatively high, the complications can be treated successfully.

To study the distribution and drug resistance of pathogens causing periprosthetic infections after hip and knee arthroplasty, and to formulate prevention and treatment strategies for drug-resistant bacteria.The data of 146 cases of periprosthetic infection after primary hip and knee arthroplasty from 2010 to 2015 were collected, including 111 cases of periprosthetic infection after hip arthroplasty and 35 cases of periprosthetic infection after knee arthroplasty. The culture positive rate, pathogenic bacteria composition and drug resistance rate were counted over the years, and the change trend of pathogen distribution and drug resistance was analyzed.One hundredand eight strains of pathogenic bacteria were detected in 146 cases, and the positive rate of culture was 73.97%. Gram positive bacteria accounted for 55.48%, Staphylococcus epidermidis and Staphylococcus aureus accounted for 25.34% and 15.07% respectively. Gram negative bacteria accounted for 13.01%, including Enterobacter cloacae, Pseudomonas aeruginosa and Escherichia coli. There were 4 cases of Mycobacterium tuberculosis infection and mixed infection. The results of culture over the years showed that the constituent ratio of Gram positive bacteria had an increasing trend, fluctuating from 39.13% to 76.47%. The results of drug sensitivity showed that the pathogens were highly resistant to β-lactams, quinolones, clindamycin and gentamicin, and the drug resistance rate was increasing, but it was still sensitive to rifampicin, nitrofurantoin, tigecycline, linezolid and vancomycin.Gram positive bacteria are the main pathogens of periprosthetic infection, and the proportion is increasing gradually.The pathogens have high resistance to many kinds of antibiotics, and the resistance rate is still increasing. To strengthen the monitoring of the distribution and drug resistance of pathogenic bacteria is helpful to grasp its change trend and formulate targeted prevention and control strategies.

Background We designed the current study to understand the normal trajectories of interleukin-6 (IL-6) and C-reactive protein (CRP) in the immediate hours and days after primary total knee arthroplasty (TKA) under the management of an enhanced recovery after surgery (ERAS) protocol and examined whether one or the other returned to normal more quickly. Methods In this prospective cross-sectional study, we examined the plasma IL-6 and CRP levels in 100 patients undergoing primary TKA at the following time points: 12 h preoperatively as well as postoperatively 12 h, 48 h, 3 days and 2 weeks. Patients were followed up for 1 year to monitor the postoperative complications, especially the infection. Results IL-6 peaked at 48 h postoperatively. Then IL-6 started to decline at 3 days postoperatively and went back to baseline level at 2 weeks (p = 0.950). CRP peaked at 3 days postoperatively. At 2 weeks, CRP declined to a normal range, without being significantly different from the baseline level (p = 0.816). Conclusion We found that under the ERAS scenario, the postoperative peak of IL-6 and CRP was deferred compared with previous studies. Compared to IL-6, CRP showed a gradual rise after surgery. Both of these two biomarkers returned to normal under the ERAS scenario. Future multiple-center studies with larger sample size can help define the thresholds of IL-6 and CRP for periprosthetic joint infection (PJI) early diagnosis. With these reference data, a clinician can make a quicker decision to perform aspiration to diagnose early PJI and benefits more patients.

Objective To evaluate contextual effects in the form of placebo responses (PRs) for patient-reported pain and function and objectively measured function in osteoarthritis (OA) clinical trials. Methods Two authors independently searched major electronic databases from inception to 20 May 2019. Included studies were randomized, placebo-controlled OA trials of pharmacological agents reporting both patient-reported and objectively measured outcomes. PRs for each type of outcome measure were compared by standardized mean differences (SMDs). The placebo response ratio (PRR) assessed the placebo to treatment effect size. The effect sizes of PRs and PRRs were pooled using a random effects model. Results Twenty-one trials met the inclusion criteria; 20 were double-blinded with one not reporting on blinding status. Compared with patients’ self-reported outcome (PRO) pain, PRs were significantly lower for PRO function (SMD − 0.16 [95% CI = − 0.28, − 0.05], p = 0.006), objectively measured muscle strength (SMD − 0.34 [95% CI − 0.58, − 0.10], p = 0.006), and range of motion (SMD = − 0.31 [95% CI = − 0.54, − 0.08], p = 0.008) function. Generally, PRs for function outcomes (patient-reported and objectively measured) were similar. The overall PRR for different measures ranged from the smallest (most favorable) for walking time/distance (0.30, 95% CI 0.16 to 0.43) to the largest for PRO pain (0.44, 95% CI 0.23 to 0.65). Conclusion Function measures both subjective and objective had less contextual effects than pain measures in OA trials. Our results support the OMERACT-OARSI recommendations to include measures of physical function in all clinical trials of hip and knee OA and suggest that a greater use of function measures might enhance the success rates of pharmacological OA trials. Increasing the availability of mobile health apps should facilitate the acquisition of measured function data. Objective To investigate the effect of acetabular component orientation on the basic stress path above the acetabular dome in the recommended safe zone. Methods A subject-specific normal hip finite element model was generated and a convergence study carried out to determine the number of material properties for trabecular bone using a normal hip model. Four abduction angles (35°, 40°, 45° and 50°) and four anteversion angles (10°, 15°, 20° and 25°) from the recommended safe zone of acetabular cup orientation were chosen to simulate acetabular reconstruction. The distribution and level of periacetabular stress was assessed using a normal hip model as a control and 16 reconstructed acetabula in simulated single-legged stances. Results The error of the average stress between plans four and five (50 and 100 materials for trabecular bone respectively) was 4.8%, which is less than the previously defined 5% error. The effect of acetabular component orientation on stress distribution in trabecular bone was not pronounced. When the acetabular component was at 15° anteversion and the abduction angle was 40° or 45°, the stress level on posterolateral cortical bone above the acetabular dome was as stable as that in the normal hip model. Conclusions Acetabular component orientation affects the basic stress path above the acetabular dome. Thus, orientation should be considered when attempting to restore normal biomechanics in the main load-bearing area.

OBJECTIVE: It has been reported that acute‐phase reactions (APR) after infusion of 5 mg zoledronic acid for the first time is common. This study surveyed the incidence and characteristics of APR in Chinese postmenopausal women receiving 5 mg zoledronic acid intravenously for osteoporosis and to evaluate the efficacy of non‐steroidal anti‐inflammatory drugs (NSAID) in preventing or alleviating APR following the first 5 mg zoledronic acid infusion. METHODS: A total of 2601 patients with an average age of 68.14 ± 9.89 years and a mean body mass index of 22.90 ± 3.24 kg/m(2) from 62 centers in China were treated with 5 mg zoledronic acid intravenously for the first time. The incidence of fever and pain were observed in these patients, and the time of fever or pain onset and duration, and the intensity of fever and grade of pain were also recorded. The dosage, duration, and efficacy of NSAID and safety outcomes were also documented. RESULTS: At the end of the study, 18 patients are eliminated due to incomplete records of temperature. The incidence of fever was 28.65% (740/2583) within 7 days following zoledronic acid infusion; 98.34% (727/740) occurred at 1.03 ± 0.66 days after infusion and lasted 1.72 ± 0.93 days. A total of 456 (17.53%) patients had newly onset pain (312 of 1187, 26.28%) or experienced pain aggravation (144 of 1414, 10.18%), which mostly occurred within 3 days after zoledronic acid infusion. A total of 1246 (47.6%) patients had received NSAID for a median time of 2.63 ± 2.45 days. Using NSAID for at least 2 days could decrease body temperature by 0.54 ± 0.86°C, increase the percentage of pain‐free patients by 6.17%, and reduce the percentage of patients with moderate to severe pain by 8.7%. CONCLUSIONS: Compared with Western populations, Chinese patients had a higher rate of fever and pain after their first zoledronic acid infusion. These symptoms were often mild to moderate in intensity and transient in duration. NSAID could effectively reduce the incidence and severity of such APR.

It is still controversial on the optimal timing of tourniquet used in total knee arthroplasty (TKA). Most previous studies focused on the comparison of different tourniquet application in controversial TKA, while the aim of our work was to compare three strategies of tourniquet application in minimally invasive TKA. 90 patients were enrolled in this study. Based on the different tourniquet application strategies, they were divided into three groups. Group A: using tourniquet during the whole surgery; Group B: tourniquet inflated before incision and deflated after the hardening of the cement; Group C: using tourniquet during the cementation. Blood loss and serum levels of C-reactive protein, IL-6, creatine kinase and myoglobin were checked preoperatively. The HSS knee score, VAS pain score, range of motion (ROM), limb swelling and hospital stays were also recorded. The mean levels of Hb and Hct were lower in Group C (104.2 ± 10.4 g/L, 31.8 ± 3.2 %) than those in Groups A (111.4 ± 14.4 g/L, p = 0.035; 34.1 ± 4.1 %, p = 0.032) and B (112.8 ± 14.3 g/L, p = 0.013; 34.5 ± 3.7 %, p = 0.011) immediately after the surgery. Compared with Groups A and B, both serum inflammation and muscle damage markers were lower in Group C. There were no significant differences between the groups in terms of HSS knee score, ROM, estimated blood loss, swelling ratio, VAS pain score and hospital stays. Using a tourniquet full time in minimally invasive TKA causes less intraoperative blood loss and more excessive inflammation and muscle damage. However, the advantage of part-time using tourniquet did not show in early functional outcomes.

To analyze the incidence and anatomic distribution of venous thrombosis after total hip and knee arthroplasty by using the data of the patients with primary total hip and knee replacement.From December 2013 to December 2014, total hip and knee arthroplasty were performed in 1 686 patients, of which 928 were THA and 758 were TKA. Before and after discharge, all patients were routinely performed double lower limb vein color Doppler ultrasound, the conventional use of anti fibrinolytic drugs, postoperative anticoagulation for 14 d. The types and distribution of thrombosis after operation were statistical analysis.Among 928 cases of primary total hip arthroplasty, there were 30 cases of thrombosis, 27 cases of isolated muscle vein thrombosis, followed by the involvement of the anterior or posterior tibial vein thrombosis, no central thrombosis. Among 758 cases of primary total knee arthroplasty, there were 87 cases of thrombosis, 81 cases peripheral thrombosis, 4 cases of thrombus of center type, the remaining 2 cases for mixed thrombus;74 patients with thrombosis involving a single vein, 65 cases involved muscle vein, 4 cases of femoral vein, 3 cases of posterior tibial vein, 2 cases of superficial vein; 13 cases of thrombosis involving multiple veins, involving muscle vein, posterior tibial veins, the peroneal veins and popliteal vein in 2 or 3 branches. The comparison results showed that the incidence of thrombosis after total knee arthroplasty was higher, the difference was statistically significant (The incidence of thrombosis in patients with primary hip and knee replacement is low, and the incidence and anatomic distribution of the patients with primary hip and knee replacement are different.

This study is to compare the therapeutic effect of posterior lumbar interbody fusion (PLIF) and transforaminal lumbar interbody fusion (TLIF) with pedicle screw fixation on treatment in adult degenerative spondylolisthesis. A retrospective analysis of 187 patients to compare the complications and associated predictive factors of the two techniques of one level lumbar fusion. Ninety-one had PLIF with two cages and pedicle fixation (group 1), and ninety-six had TLIF with one cage and pedicle fixation (group 2). The two groups had similar age and sex distribution, and level of pain. Inclusion criteria and outcome measurements were identical in both groups. The two groups were operated on with autograft and cage with pedicle fixation. Before surgery and at the 2-year follow-up, pain (VAS) and functional disability (JOA) were quantified. The results showed there were no intraoperative deaths in our study. In the end 176 cases had 2-year follow-up while 11 cases were lost to follow-up. The follow-up rate was 93.4% (85/91) in the PLIF group and 94.8% (91/96) in the TLIF group. All patients had bone fusion, and there were no cases of cage extrusion. The pain index improved from 7.08 +/- 1.13 to 2.84 +/- 0.89 in PLIF patients and improved from 7.18 +/- 1.09 to 2.84 +/- 0.91 in TLIF patients (P0.001). There were 42 cases of excellent, 29 cases of good, 11 cases of general, and 3 cases of poor results in PLIF group. There were 46 cases of excellent, 31 case of good, 12 case of general, and 2 cases of poor results in TLIF group. The JOA score in all patients was 84.1% of good or excellent (83.5% in PLIF and 84.6% in TLIF, P0.05). The average preoperative slip was 30.1 +/- 7.2% in PLIF group while in the TLIF it was 31.4 +/- 8.3%. Immediately post operatively it was reduced to 7.3 +/- 2.1% and 7.4 +/- 2.7% and at last F/U it was 8.1 +/- 2.8% and 8.2 +/- 2.6%, respectively. The average of reduction rate was 75.2 +/- 6.4% in PLIF and 75.4 +/- 6.2 in TLIF on the initial post operatively X-ray, and 72.6 +/- 5.2% and 72.4 +/- 5.4% on the follow-up. The percentage rate, reduction rate and lost of reduction rate between the two groups was similar (P0.05). The average pre operative disk and foramen height in the PLIF group improved from 6.8 +/- 2.3 and 14.2 +/- 1.7 preoperatively to 11.6 +/- 1.5 and 18.7 +/- 1.8 post operatively, respectively. At last follow up there was minimal lost of correction down to 11.24 +/- 1.2 and 18.1 +/- 1.8, respectively. Similarly in the TLIF group, pre operative disk and foramen height were improved from 6.7 +/- 1.7 and 14.1 +/- 1.8 to 11.4 +/- 1.6 and 18.5 +/- 1.6 immediately post operative. At last follow up minimal lost of correction was noted with average disc height of 11.3 +/- 1.4 and 18.2 +/- 1.7. Both techniques achieve statistical significance in restoration of disc and foraminal (P0.01); however, there was no statistical difference between the two techniques. In conclusion, interbody fusion with either a PLIF technique or a TLIF technique provides good outcomes in the treatment of adult degenerative spondylolisthesis. The TLIF procedure is simpler and is as safe and effective as the PLIF technique.

To validate the function score for adult Tibetans with Kashin-Beck disease (FSAT- KBD) measuring daily life and work functional status of KBD patients.From September to October 2010, 352 adult KBD patients in Rangtang County of Aba Tibetan autonomous region were invited to complete FSAT-KBD. The internal consistency of FSAT-KBD was assessed using Cronbach's alpha coefficients. Principal component analysis with varimax rotation was performed to explore factor structure of the instrument, with item-domain correlations being examined using Spearman's rank correlation tests. Discriminant validity of the FSAT-KBD was assessed by comparing scores of the respondents with different ages and different functional status. Convergent validity of the FSAT-KBD was assessed through a comparison with the medical outcomes study short form health survey (SF-12) and visual analogue scale (VAS).338 KBD patients (a response rate of 96.0%) completed the questionnaire in an average of (3.2 +/- 1.6) min. The instrument achieved a Cronbach's alpha of 0.945, with item-to-domain correlations exceeding 0.0. Two latent factors were extracted, which explained 72.8% of the total variance. The factor structure fitted well with our conceptual hypothesis. Respondents with different age, duration of suffering and number of affected joints had different FSAT-KBD scores. The FSAT-KBD results were correlated with those of SF-12 and VAS.FSAT-KBD is a reliable and valid instrument for measuring daily functional status of adult KBD patients in Aba Tibetan autonomous area in China.

To simulate acetabular morphology and perform acetabular quantitative analysis in high dislocated developmental dysplasia of the hip (DDH) patients using three-dimensional (3D) surface reconstruction technique, in order to understand the acetabular anatomic features and develop operative strategies for acetabular reconstruction.3D pelvic images were reconstructed by Mimics software from CT data of 13 patients (13 hips) with high developmental DDH and 13 normal persons (26 hips). True acetabular superior-inferior diameter, anterior-posterior diameter, acetabular depth, medial wall thickness, abduction angle and anteversion angle were measured and compared between the two groups of participants.Irregular acetabular shape was found in high dislocated group, showing a triangle with wide upper and narrow lower. The acetabular quantitative analysis revealed (38.29 +/- 2.71) mm superior-inferior diameter, (21.74 +/- 5.33) mm anterior-posterior diameter, (15.50 +/- 2.93) mm acetabular depth, (6.80 +/- 2.97) mm medial wall thickness, (49.29 +/- 7.40) degrees abduction angle and (23.82 +/- 11.21) degrees anteversion angle in high dislocated patients. The superior-inferior diameter, anterior-posterior diameter and acetabular depth of high dislocated patients were significantly smaller than those of the normal contirols (P0.05). However, the medial wall thickness, abduction angle and anteversion angle of high dislocated patients were significantly bigger than those of the normal controls (P0.05).3D reconstruction technique can restore true acetabular morphology and perform quantitative analysis. Compared with normal controls, high dislocated DDH patients have acetabular features: irregular shape, lower opening, higher medial wall and bigger abduction and anteversion angles. Joint arthroplasty surgery in high dislocated DDH patients needs to look at these acetabular features.

Nitric oxide has important effects on bone cell function. To verify that nitric oxide can protect against bone loss associated with estrogen deficiency, which is dependent on different concentrations of nitric oxide, we applied different doses of nitric oxide to ovariectomized rats. Fifty 12-week-old Sprague-Dawley female rats had ovariectomies, and 10 rats had sham operations. The ovariectomized rats were randomized into five groups: ovariectomized only; 17-beta-estradiol; low-dose nitroglycerin; middle-dose nitroglycerin; and high-dose nitroglycerin. After 12 weeks, the bone mineral density, dry weight, ash weight, calcium content, and nitric oxide concentration were determined. Compared with these same measurements in the sham-operated group, the bone mineral density, dry weight, ash weight, calcium content, and nitric oxide concentration decreased in the control group. Treatment with low-dose nitroglycerin, middle-dose nitroglycerin, and 17-beta-estradiol maintained bone mineral density and reversed the effects of ovariectomy on dry weight, ash weight and calcium content when compared with those in the control group. There were no differences in the bone mineral density, dry weight, ash weight, or calcium concentration between the ovariectomized-only rats and the rats treated with high-dose nitroglycerin. Results of this study suggest that nitric oxide treatment can counteract bone loss in ovariectomized rats. Furthermore, supplementation with a similar or slightly greater than physiologic concentration of nitric oxide has a potentially positive impact on osteoporosis.

OBJECTIVE: To determine the rate of curing the infection and mid‐ to long‐term outcomes of using extensively coated non‐modular stems in two‐stage revision for infected total hip arthroplasty (THA). METHODS: The clinical data of 33 patients (33 hips) in whom extensively coated non‐modular stems had been used in two‐stage revision THA for deep infection were retrospectively analyzed. All operations received two‐stage reimplantation, which included resection arthroplasty, thorough debridement, insertion of a hand‐molded antibiotic‐impregnated cement spacer with stainless steel reinforcement, a course of intravenous antibiotics, and delayed reimplantation. Microorganism‐specific antibiotics had been chosen according to the results of microbiological studies performed postoperatively. All patients received i.v. antimicrobial therapy for 4 weeks and oral antibiotics to which their organisms were sensitive for a further 6 weeks. Harris hip score (HHS) and plain X‐ray films were used to perform clinical and radiological evaluations. RESULTS: During follow‐up for a minimum of 5 years, no reinfection or loosening were found. Cultures of samples taken during the second stage were all negative for infection. The mean HHS improved from 42 preoperative to 89 at the final follow‐up. All granular bones had fused well with the host bones by 12 months after the surgery. CONCLUSION: Using extensively coated non‐modular stems combined with intramedullary allografts in two‐stage revision for treating infected THAs can achieve satisfactory outcomes.

To evaluate the outcomes of open high tibial osteotomy through small incision with annular external fixator for treating genu varum deformity.We reviewed 8 cases of open high proximal tibia osteotomy through small incision with annular external fixator for patients (10 knees) with genu varum due to high tibia varum. The patients were followed up 1, 2, 3, 6, 12, 24 and 36 months after the operations. Data about infection, healing, varus angle, knee distance and Insall-Salvati index were retrieved and analysed.The patients had a mean varus angle of 15. 3 degrees +/- 2. 3 degrees (range, 10 degrees - 28 degrees) before the operations. The operations were all recorded as successful following a (29. 3 +/- 3. 1) month (range, 24-36 months) post-operation follow-up, with external fixator remaining stable and no instrument failures. The bones were healed at an average of (11. 2 +/- 1. 5) weeks (range, 8-12 weeks) and there was no nonunion or delayed union. The operations reduced varus angle of the knees to - 1. 1 degrees +/- 0. 6 degrees (range, - 7 degrees - 3 degrees ), indicating a significant improvement compared to that of the preoperations. There were 2 cases of superficial pin infections.Open high tibial osteotomy through small incision with annular external fixator has good fixation stability and satisfactory clinical outcomes with little complications for treating patients with genu varum deformity.

OBJECTIVE: To assess the difference between drainage and non‐drainage after total hip arthroplasty (THA) in Chinese subjects by evaluating post‐operative complications and joint function. METHODS: One hundred and sixty‐eight patients undergoing THA were randomly allocated into drainage (83 patients) and non‐drainage groups (85 patients). All surgeries were performed by one surgical team using the same pre‐, intra‐, and postoperative techniques. Measured items included: hemoglobin (Hb), superficial and deep wound infection, volume of blood transfusion, wound hematoma thickness, range of motion (ROM) of the hip, wound healing time, ecchymosis and tension vesicles around the wound. RESULTS: Early after THA, the Hb decreased significantly in the drainage group. There was no significant difference between non‐drainage and drainage groups in need for or volume of blood transfusions (9.6% vs 8.2%, P = 0.100; 3.8 units vs 2.9 units, P = 0.089, respectively). In the non‐drainage group, the incidence of superficial infection, ecchymosis and tension vesicles was significantly higher than in the drainage group (10.6% vs 2.4%, P = 0.031; 12.9% vs 3.6%, P = 0.026; 16.5% vs 4.8%, P = 0.013, respectively). In addition, the non‐drainage group had a greater volume of hematomas (P = 0.000). Patients in the non‐drainage group had smaller ROMs early after surgery but the final ROMs did not differ significantly between groups. No deep infection occurred in either group. CONCLUSION: Non‐drainage may reduce postoperative blood loss but has no benefits regarding blood transfusion or deep infection. It may cause more post‐operative complications because of restriction of early postoperative exercise by pain and swelling. Therefore we suggest routine use of drainage after THA.

To systematically analyze and compare the clinical characteristics of orthopedic inpatients in Lushan and Wenchuan earthquake, so as to provide useful references for future earthquakes injury rescue.Based on the orthopedic inpatients in Lushan and Wenchuan earthquakes, the data of the age, gender, injury causes, body injured parts and speed of transport were classified and compared.The duration of patients admitted to hospital lasted long and the peak appeared late in Wenchuan earthquake, which is totally opposed to Lushan earthquake. There was no significant difference in the patient's age and gender between the two earthquakes. However, the occurrence rate of crush syndrome, amputation, gas gangrene, vascular injury and multiple organ dysfunction syndrome (MODS) in Wenchuan earthquake was much higher than that in Lushan earthquake. Blunt traumas or crush-related injuries (79.6%) are the major injury cause in Wenchuan earthquake, however, high falling injuries and falls (56.8%) are much higher than blunt trauma or crush-related injuries (39.2%) in Lushan earthquake. The incidence rate of foot fractures, spine fractures and multiple fractures in Lushan earthquake was higher than that in Wenchuan earthquake, but that of open fractures and lower limb fractures was lower than that in Wenchuan earthquake.The rapid rescue scene is the cornerstone of successful treatment, early rescue and transport obviously reduce the incidence of the wound infection, crush syndrome, MODS and amputation. Popularization of correct knowledge of emergency shelters will help to reduce the damage caused by blindly jumping or escaping while earthquake happens.

OBJECTIVE: To assess whether computed tomography (CT)‐based 3‐dimensional (3D) computerized pre‐operative planning is accurate and reliable in patients with high‐riding dislocation developmental dysplasia of the hip (DDH) undergoing total hip arthroplasty (THA). METHODS: Between September 2009 and February 2011, a prospective study with an inbuilt means of comparing predictive techniques in 20 patients (20 hips) with high‐riding dislocation DDH was undertaken. All patients had pre‐ and post‐operative CT scans, data from which were transferred digitally to Mimics software. 3D pre‐operative planning to predict the acetabular component size, hip rotation center position and acetabular component coverage was performed using Mimics software. The results and post‐operative course were compared with those of the traditional acetate templating technique. RESULTS: Using 3D computerized planning, 14/20 components (70%) were predicted exactly and 6/20 (30%) within one size, whereas with the conventional acetate templating technique, 5/20 components (25%) were predicted exactly, 9/20 (45%) within one size and 6/20 (30%) within two or more sizes. There was a strong correlation between the 3D computerized planned acetabular component size, hip rotation center distance, acetabular component host coverage and that found postoperatively. Five patients were considered to need structural bone graft on the basis of 3D computerized planning; this was highly coincident with the intraoperative findings in all five cases. CONCLUSION: CT‐based 3D computerized pre‐operative planning using Mimics software is an accurate and reliable technique for patients with high‐riding dislocation DDH undergoing THA.

To compare the clinical efficacy and serum concentrations of cobalt, chromium metal ion in three different hard-on-hard bearings after total hip arthroplasty at 2-years postoperatively.Ninety (90) THA patients were divided into ceramic-on-ceramic (COC), ceramic-on-metal (COM), metal-on-metal (MOM) group (n = 30 in each group). At preoperative and 3, 6, 12, 24 months postoperative 5 time points, serum concentrations of cobalt and chromium metal ion were measured, Harris hip score was evaluated, X-rays and color doppler ultrasound examination of the ipsilateral hip also were observed.The excellent rates of Harris hip score were 100% in three groups. Continuous X-rays showed no radiolucent line around the acetabular component, no osteolysis, and no inflammatory pseudotumor. After the THA operation, the metal ion levels in COM and MOM groups increased rapidly, and stabilized at 12 months, then showed a downward trend, but the chromium ion level of MOM continued to rise at 24 months, with a significant difference when compared with that at 12 months (an increase of 0.48 microg/L, P = 0.021). The serum concentrations of metal ion in COC group were relatively constant at all time points, and the cobalt, chromium ion levels of MOM group were significantly higher than those of COC and COM group.The postoperative functional recovery of the three hard-on-hard bearings all were good, and no inflammatory pseudotumor and osteolysis were found. The serum levels of cobalt, chromium ion of COM were lower than those of MOM, but higher than those of COC.

To analyze the results of clinical and bacteriological examinations in patients with crush syndrome who suffered infectious complications after an earthquake in Sichuan, China.A total of 313 bacteriological samples among 147 patients with crush syndrome were collected. Infectious complications, results of microbiological examinations, potential risk factors of infection and mortality were analyzed statistically.In the obtained database, 112 out of the 147 (75.7%) patients had infectious complications, in which, wound infection, pulmonary infection, and sepsis were most common. The time under the rubble and the time from injury to treatment were related to the occurrence of wound infection (P equal to 0.013, odds ratio 2.25; P equal to 0.017, odds ratio 2.31). Sepsis and wound infection were more common in patients who underwent fasciotomy or amputation than in those who did not (P equal to 0.001).Quick rescue and injury treatment can decrease the infection risk in crush syndrome patients. It is better to obtain microorganic proofs before applying antibiotics, and bacteriological and drug sensitivity data should be taken into account, especially considering that most of these infections are hospital-acquired and drug resistance. Emphasizing the accuracy and efficiency of wound management in emergency situations, cautiously assessing the indications for fasciotomy to avoid open wounds from unnecessary osteofascial compartment decompression incisions may decrease the incidence of infection and ameliorate the prognosis.

To comparatively analyze the medical records of patients with limb fractures as well as rescue strategy in Wenchuan and Yushu earthquakes so as to provide references for post-earthquake rescue.We retrospectively investigated 944 patients sustaining limb fractures, including 891 in Wenchuan earthquake and 53 in Yushu earthquake, who were admitted to West China Hospital (WCH) of Sichuan University.In Wenchuan earthquake, WCH met its three peaks of limb fracture patients influx, on post-earthquake day (PED) 2, 8 and 14 respectively. Between PED 3-14, 585 patients were transferred from WCH to other hospitals outside the Sichuan Province. In Yushu earthquake, the maximum influx of limb fracture patients happened on PED 3, and no one was shifted to other hospitals. Both in Wenchuan and Yushu earthquakes, most limb fractures were caused by blunt strike and crush/burying. In Wenchuan earthquake, there were 396 (396/942, 42.0%) open limb fractures, including 28 Gustilo I, 201 Gustilo II and 167 Gustilo III injuries. But in Yushu earthquake, the incidence of open limb fracture was much lower (6/61, 9.8%). The percent of patients with acute complications in Wenchuan earthquake (167/891, 18.7%) was much higher than that in Yushu earthquake (5/53, 3.8%). In Wenchuan earthquake rescue, 1 018 surgeries were done, composed of debridement in 376, internal fixation in 283, external fixation in 119, and vacuum sealing drainage in 117, etc. While among the 64 surgeries in Yushu earthquake rescue, the internal fixation for limb fracture was mostly adopted. All patients received proper treatment and survived except one who died due to multiple organs failure in Wenchuan earthquake.Provision of suitable and sufficient medical care in a catastrophe can only be achieved by construction of sophisticated national disaster medical system, prediction of the injury types and number of injuries, and confirmation of participating hospitals?exact role. Based on the valuable rescue experiences after Wenchuan earthquake, the rescue was faster, more orderly and effective in Yushu earthquake. Nevertheless, there is still a long way to go in the development of a stronger emergent response to the disasters.

To evaluate the clinical effectiveness and safety of using granular type nano-hydroxyapatite and polyamide 66 (n-HA/PA66) composite in repairing bone defects caused by giant cell tumors.48 patients with giant cell tumors, who underwent lesion curettage, inactivation and cavities fill-in with granular type n-HA/PA66 from December 2007 to May 2011, were followed up. Routine blood tests, liver and kidney functions, serum calcium and phosphorus, and immunologic parameters were examined before and after the surgeries. Radiological examinations were carried out 1 week and 1, 3, 6 and 12 months post operations to monitor the bone repairing process. The n-HA/ PA66 in bone issues was detected with hematoxylin-eosin staining.45 patients completed the follow-up. No significant abnormalities in routine blood tests, serum calcium and phosphorus, and immunologic parameters were found pre- and post-operations. Nor abnormal liver and kidney functional lesions were identified. The radiological examination showed gradual increase in the density of the focal zone after bone implanting operations. The bone density of the implanted areas got close to normal 1 year after operations. The histological examination found that osteoblasts grew into the hole of n-HA/PA66; calcium was deposited on the materials; and large amount of osteocytes inlaid into the composite. The composite was integrated into new bone and surrounding tissues.n-HA/PA66 has good biocompatibility and biological safety. It also has good osteoconduction and osteogenesis activity. The n-HA/PA66 composite is one perfect bone repair material.

To initially evaluate the application of artificial vertebra of n-HA/PA66 in anterior reconstruction of lower cervical spine fracture and dislocation.In this study, 84 patients with lower cervical spine fracture and dislocation received anterior cervical discectomy, spinal canal decompression or subtotal corpectomy, spinal canal decompression and reconstruction by n-HA/PA66 composite artificial vertebral body combined with plate instrumentation. Neurological function was followed up by improvement rate of Frankel and situations of the supporting body was observed by X ray and 3D-CT in 3, 12, 24 months postoperatively. The intervertebral height, physical arc (reflected by Cobb angle) and the locations and fusion rate of the supporting body were assessed in order to evaluate the stability of the cervical spine and alignment improvements.All the patients underwent operation successfully and were followed up for 6 to 24 months with an average of 12 months. The preoperative symptoms were improved to varying degrees. Imaging studies showed that in all cases graft fusion were achieved, and cervical alignments, intervertebral height, cervical spine stability and the locations of the artificial vertebral body were well maintained. No displacement and subsidence of the artificial vertebral body occurred. Postoperative immediate intervertebral height (2.4 ± 0.2) cm, preoperative intervertebral height (1.9 ± 0.1) cm, comparisons of the two groups was statistically significant (q = 2.48, P0.001). The immediate, 3 month, 1 year, 2 year period follow-up group intervertebral height was not statistically significant (P0.05). Preoperative Cobb angle was 9.8° ± 1.2°, postoperative immediate Cobb angle was 16.6° ± 1.2°, comparisons of the two groups was statistically significant (q = 14.25, P0.001). The immediate, 3 month, 1 year, 2 year period follow-up group Cobb angle was not statistically significant (P0.05).n-HA/PA66 artificial vertebral body can provide early cervical spine support and stability and effectively maintain the biological alignment and cervical intervertebral height. It has high rate of graft fusion and is convenient to observe by X-ray. Therefore, n-HA/PA66 can be taken as an ideal graft for anterior lower cervical spine fracture and dislocation operation, but further follow-up study is still required to evaluate the long-term effects.

To evaluate clinical and radiographic results and influencing factors of hybrid total hip arthroplasty (THA).Totally 126 patients (135 hips) from January 1999 to December 2001 accepted hybrid THA were followed up. Components migration, periprosthetic bone changes, the polyethylene wear rate were measured radiologically. Kaplan-Meier analysis was performed to evaluate the survival of the acetabular and femoral components. End point was obvious radiological loosening or revision either or both of the acetabular and femoral components for aseptic loosening, infection or osteolysis debridement surgery.A total of 79 cases (85 hips) had been given follow-up. Using revision as the end point, the survival rate of acetabular was 95.2%, cemented femoral components was 98.8%. Using loosening as the end point for failure, the survival rate of acetabular was 97.6%, cemented femoral components was 100%.The hybrid total hip prosthesis long-term survival rate was satisfactory, especially in the femoral side. As a result of third-cement technology, the long-term survival rate of the femoral components was close to the modern cementless prosthesis.

Although primary open anatomic reduction and stable internal fixation has become a standard treatment principle of Lisfranc joint injuries, there are still some debates existed among surgeons as to the most appropriate treatment of this injury. The aim of this cohort study was to analyze the early (mean, 3.2 years) clinical and radiographic outcomes of one surgeon's experience associated with a standardized protocol of open reduction and internal fixation using headless compression screws (HCS) in a consecutive series of Chinese patients with Lisfranc joint injuries.This study is to retrospectively evaluate the clinical and radiographic outcomes of a series of Chinese patients with Lisfranc joint injuries that underwent open reduction and internal fixation with HCS.There were 29 patients (30 feet) included in our study. The average score of American Orthopaedic Foot and Ankle Society Midfoot score and the Visual Analog Pain Scale were 80.6 and 2.4 points at the final follow-up point, respectively. Anatomic initial reduction was obtained in 26 patients (27 feet, 90%). At the last follow-up point, only one case had the loss of reduction. Posttraumatic osteoarthritis was observed in 10 patients (10 feet, 33%), and 2 of them (2 feet, 7%) were scheduled for arthrodesis because of persistent pain.Our results have shown that fixation of an unstable tarsometatarsal joint with AO 3.0 mm HCS can provide the firm stability, the precise reduction of the joint, and satisfactory short-term clinical and radiographic outcomes without any screw breakage, although posttraumatic osteoarthritis in the midfoot is still a significant challenge for orthopedic surgeons.

To study the effect of internal fixation performed at different times on therapeutic outcomes of Schatzker IV-VI tibial plateau fractures.The clinical data of 42 cases of Schatzker IV-VI tibial plateau fractures treated in our department were analyzed retrospectively. Among these 42 patients, 21 received surgical treatment within 12 h after injury (Group I), the other 21 were first treated by traction or plaster fixation followed by a delayed internal fixation after soft tissue swelling subsided (Group II). The surgical time, complications, length of hospital stay, cost of hospitalization, and time for fracture union, as well as functional recovery were analyzed and compared between the two groups.After 10-28 months follow-up (mean 16.5 months), except 5 cases who lost to follow-up, no differences were found between the two groups regarding surgical time, preoperative and postoperative complications, healing time or the Hospital for Special Surgery (HSS) score at the end of follow-up, but significant differences were found in the length of hospital stay, cost of hospitalization and HSS score at 3 months after operation (P less than 0.05).Under certain conditions, early internal fixation for Schatzker IV-VI tibial plateau fracture is feasible, which can shorten the length of hospital stay, decrease the cost of hospitalization and promote early functional rehabilitation.

To review the recent developments in the mechanisms of glucocorticoids induced osteonecrosis of femoral head (ONFH) and introduce a new theory of ONFH.Both Chinese- and English-language literatures were searched using MEDLINE (1997 - 2011), Pubmed (1997 - 2011) and the Index of Chinese-language Literature (1997 - 2011).Data from published articles about mechanisms of glucocorticoids induced ONFH in recent domestic and foreign literature were selected. Data extraction Data were mainly extracted from 61 articles which are listed in the reference section of this review.Glucocorticoids are steroid hormones secreted by the adrenal cortex that play a pivotal role in the regulation of a variety of developmental, metabolic and immune functions. However, high dose of exogenous glucocorticoids usage is the most common non-traumatic cause of ONFH. Glucocorticoids can affect the metabolisms of osteoblasts, osteoclasts, bone marrow stromal cells and adipocytes which decrease osteoblasts formation but increase adipocytes formation and cause ONFH finally.Glucocorticoids affect the differentiation of mesenchymal stem cells, through activating or inhibiting the related transcript regulators of osteogenesis and adipogenesis. At last, the size and volume of mesenchymal stem cells derived adipocytes will increase amazingly, but the osteoblasts will be decreased obviously. In the meantime, the activity of the osteoclasts will be activated. So, these mechanisms work together and lead to ONFH.

Objective: To compare the results of long proximal femoral nail antirotation (PFNA‐long) and plate combinations in the treatment of ipsilateral intertrochanteric and femoral shaft fractures. Methods: Between March 2004 and April 2009, 23 patients with ipsilateral intertrochanteric and femoral shaft fractures were treated with PFNA‐long or plate combinations. The patients were divided into two groups. Group I contained 13 patients who were treated with dynamic hip screws (DHS) combined with compression plate fixation. The 10 patients in Group II were treated with PFNA‐long. Results: The average follow‐up was 17.8 and 16.8 months for Groups I and II, respectively. The average union time for intertrochanteric fractures was 17.4 and 16.6 weeks in Groups I and II, respectively, and for femoral shaft fracture 22.2 and 21.5 weeks, respectively. There were nine good, two fair, and two poor functional results in Group I, and eight good, one fair, and one poor in Group II. There was nonunion of two femoral shaft fractures in Group I and one in Group II. There were no significant differences between the two groups in functional outcomes or major complications. Conclusion: Both treatment methods achieve satisfactory functional outcomes in patients with ipsilateral intertrochanteric and femoral shaft fractures. PFNA‐long is the better choice for the treatment of complex fractures, having the advantages of minimal exposure, reduced perioperative blood loss, and achievement of biological fixation of both fractures with a single implant.

To validated the 12-item Health Status Survey (SF-12) of Chinese version for evaluating health-related quality of life (HRQL) of Kashin-Beck disease (KBD) patients in Aba Tibetan autonomous area, China. 338 adult KBD patients in Rangtang County of Aba area were investigated with SF-12. Principal component analysis with varimax rotation was used to test the original factor pattern. Internal consistency reliability was assessed by using Cronbach’s alpha. “Known groups” construct validity was assessed by comparing SF-12 component scores between respondents hypothesized to differ in health-related variables. Two latent factors were extracted explaining 69.4 % of the variance of the questionnaire. The factor loadings were mostly according with the principle concept, except for “vitality” (VT) and “social functioning” (SF) scales that loaded heavier on physical component. The Cronbach’s alpha coefficient was 0.909 for the physical health domain, 0.900 for the mental health domain, and 0.914 for the whole scale. The SF-12 had satisfactory “known group” validity and could well discriminate the differences between patients and healthy controls and between subgroups divided by age, duration of suffering or number of affected joints. The SF-12 could be used to evaluate the HRQL of adult KBD patients in Aba Tibetan autonomous area in China and with good feasibility, reliability and validity.

Traumatic asymmetrical bilateral dislocation of the hip joint is a rare occurrence. Herein we report a case of traumatic asymmetrical bilateral dislocation of the hip caused by a falling object. A 31-year-old healthy male was injured by a falling wall of weighing approximately 100 kg from behind in the lumbosacral area. The patient sustained a pelvic fracture, posterior dislocation of the right hip joint associated with a comminuted fracture of the right acetabulum, and anterior dislocation of the left hip joint. Manual reduction of both hip dislocations was performed in the Emergency Department. The posterior wall fracture of the right acetabulum was treated with open reduction and internal fixation by plate and screw. Reduction of both hips was successful and no femur necrosis or arthrosis was found during the four-year follow-up. Heterotopic ossification was found but it did not affect joint function. Prompt reduction of hip dislocations in the Emergency Department can reduce the incidence of avascular necrosis of the femoral head.

To investigate the clinical results of modified Sutherland pelvic osteotomy for developmental dysplasia of hip (DDH).Sutherland pelvic osteotomy were performed in 10 patients (11 hips) with DDH. Among them, there were 3 male (3 hips) and 7 female (8 hips) patients, aged (32 ± 8) years. During operation, arthroscopes were performed additionally to remove the existing hyperplasia tissue in the fossae ovalis and trimming acetabulum and glenoid labrum, thus to insure the better match between the femoral head and the realigned acetabulum. The change of imaging indexes were acquired by comparing the preoperative X-ray with the postoperative X-ray. The change of hip function and life quality were acknowledged according to contrast and analysis Harris hip score and Short Form 12-items Health Survey (SF-12) before and after osteotomy.All patients were followed up for a mean of (5.2 ± 2.3) years, the osteotomy were all union in 3 months. The acetabular head index was 71 ± 8 before operation, and 86 ± 4 after operation. The pre- and post-operative centre edge angle were (7 ± 9)° and (33 ± 9)°, sharp angle were (48 ± 4)° and (37 ± 5)°, acetabular index angle were (24 ± 8)° and (11 ± 5)° respectively. The average Harris score improved from 42 ± 13 preoperatively to 90 ± 5 postoperatively, with 100% excellent and good results. Every domains of SF-12 was improved in the different extents postoperatively, the improvement of physical component summary was more conspicuous than mental component summary. The imaging indexes, Harris and SF-12 were all improved with significant difference (all P0.05).The modified Sutherland pelvic osteotomy is effective. It could increase the load bearing capacity of hip, and improve the quality of life.

To investigate the therapeutic effects of porous titanium (Ti) on the recovery of rabbit radial bone defect.Bone defects were artificially made in 30 New Zealand rabbits by resecting the 1 cm substantial osseo with periosteum of both radii. The left anterior limbs were implanted with porous titanium, while the right anterior limbs with porous hydroxyapatite (HA). The rabbits were sacrificed at three time points. Both the radii healing statuses were observed by histology and histomorphometry analyses by means of computer graphic processing at the end of 8, 12 and 24 weeks, and biomechanical analyses at the end of 12 and 24 weeks.The histology examination showed that mass newly formed bone had grown into most pores of both the specimens. Along with prolongation of times after operation, the ingrowths of bone cells and effects of bone remodeling in the research side were nearly the same as those in the control side at the end of 12 and 24 weeks. The interface between the new bone and implants showed tight contact in both the groups without an obvious fibrous tissue. The results of histomorphometrical analyses showed that a statistically significant difference was not obtained for % bone area (bone area/ gross implant) between both the groups (P0.05).However, the results of maximum failure load for the Ti group and the HA group were (107.34±27.44) N and (93.42±21.18) N at the end of 12 weeks, (118.56±24.65) N and (102.15±23.37) N at the end of 24 weeks, respectively. Biomechanical properties of the Ti group was stronger than that of the HA group, however, a statistically significant difference was not obtained between both the groups (P=0.102).Porous titanium scaffold can promote the formation of new bone, which contributes to the healing of long tubular bone defect. The porous titanium can enhance the bone repairing effect on segmental bone defect nearly the same as porous hydroxyapatite .

To study the influence of mutation of -1997G--T of COL I A1 gene on the biochemical function of osteoblast, and the pathomechanism of BMD.Spongy bones were obtained to culture osteoblast primarily during total hip or knee replacements. The genotype of osteoblast was identified with PCR-RFLP. The amount of mRNA of COL I A1 and collagen type I were determined by RT-PCR and ELISA. The growth of osteoblast, the activity of bone ALP, and the amount of calcium in cell matrix and calcium nodus were measured.Three genotypes GG, GT and TT in osteoblast were successfully identified. Compared with GG and GT genotypes, lower expression of mRNA of COL I A1 gene, lesser collagen type I, calcium in cell matrix, and calcium nodus were found in the cells with TT genotype (P0.01). No significant differences were found between GG and GT genotype (P0.05). There were no significant differences in age, growth of osteoblast, and activity of bone ALP among the three genotypes (P0.05).Cells with TT genotype have low expression of mRNA of COL I A1 gene and less collagen type I , calcium in cell matrix and calcium nodus. The lower amount of collagen type I synthesized by osteoblast can decrease the matrix outside the bone cells and result in insufficient site for calcium deposition. This may be the cause of lower BMD in patients with TT genotype.

To study the changes of biomechanical properties and microarchitecture in callus during the healing process of osteoporotic fracture.Ovariectomy (OVX) was performed on SD rats to establish osteoporotic rat model. Osteotomy was performed on both groups of rats with OVX and sham operations to establish bone fracture. Bone mineral density (BMD), area of type I collagen, biomechanical properties, and microarchitecture of the fractured bones during healing process were measured.The BMD of both groups of rats were significantly lower at the 12th week than at the 6th week after fracture (P0.05). Both groups of rats had significantly higher biomechanical strength and area of type I collagen at the 12th week than at the 6th week after fracture (P0.05). The OVX group of rats had significantly lower structure model index (SMI) values than the sham group at both 6th and 12th week after fracture (P0.05). Higher values of SMI appeared at the 12th week than at the 6th week in both groups of rats (P0.05).During the healing of fracture, the BMD of the newly-formed bone reaches its peak value at the 6th week after fracture, which is earlier than the occurrence of peak biomechanical strength, area of type I collagen, and SMI. Although the collagen and aggravation of mineral determine the biomechanical properties, BMD by itself may not be able to determine the biomechanical strength because of the change of the 3-D structure of the callus. The combination of collagen and microarchitecture may act as a factor that determines the biomechanical strength of callus independent of BMD.

To retrospectively analyze the clinical and radiological results of the impaction morselized allografting combined with cementless long stem for femoral component revision in patients with significant bone loss.From July 2003 to June 2009, 27 patients (27 hips) underwent revision hip arthroplasty in femurs with bone defect using impaction allografting and cementless components. There were 15 male and 12 female with mean age of 67 years. According to the Paprosky classification system, the bone defects in femur were classified into type II in 3 cases, type III in 21 cases and type IV 3 cases. The patients were followed up postoperatively to observe the clinical and radiological results, and if there had the stem subsidence, periprosthetic fracture, infection and other complications.Twenty-three patients were followed up. The mean follow-up time was 26 months. Harris score improved from 43 points pre-revision to 83 points at the final follow-up. No femoral stem loosening and failure occurred. There were 4 stem with a mean 3.3 mm subsidence. One case had an acute infection after revision and treated successfully by debridement and drainage, antibiotics. No stem need further revision. Radiographic analysis showed all the revised stem was stable, and graft incorporation was seen in at least 1 zone in 100% of the femurs.The results of this study demonstrates that femoral support and vigorous impaction grafting combined with cementless long stem results in an good intermediated-term clinical effects.

To analyze the results of cortical windowing of the femoral diaphysis for well-fixed cement/plug removal during hip revision surgery.From May 2005 to June 2009, 14 patients (14 hips) were undergone revision total hip arthroplasty (THA), window was cut into the cortex of the femur, and the well-fixed cement/plug distal to the window was removed under the direct vision. After reimplanted the cementless revision stem, the cortical lids were replanted and fixed with 2 to 3 cerclage wires. Six patients who had suffered from osteoporosis were undergone morselized bone graft to the osteotomy site. Postoperatively, the patients were maintained at partial weight-bearing (touchdown) for 6 weeks and then advanced as they were able.The length of the cortical windows varied from 2.5 to 6.0 cm (mean, 3.4 cm), the width ranged from 0.8 to 1.4 cm (mean, 1.2 cm). In one patient the window was enlarged during the procedure to facilitate the cement/plug removal. The mean radiologically healing time for the windows was 19 weeks. There was no intraoperative femoral perforation during cement/plug removal. One femoral fracture during the revision stem was implanted. No postoperative periprosthetic fracture and other complications such as infection, implant subsidence occurred during the fellow-up. There was no femoral thigh pain or implant loosening with femoral window.The cortical windowing technique is very helpful to facilitate the well-fixed cement/plug distal to the prosthesis tip removal and the windows heal rapidly and decrease the femoral complications associated with revision THA.

To investigate the effect of Kashin-Beck disease (KBD)-affected feed and T-2 toxin on the bone development of Wistar rats.Seventy-eight ablactation Wistal rats (50% male and 50% female) weighing approximately 65 g were obtained from Sichuan Medical Center (Chengdu, China), and were randomly assigned to three groups (Groups A, B and C). Group A had 24 rats and were fed with commercial rat feed (control); Group B had 30 rats and were fed with commercial rat feed and T2 toxin by intragastric administration; and Group C had 24 rats and were fed with the KBD-affected feed. The histological sections were stained with hematoxylin and eosin (HE) and Masson dye.Weight gain was fastest Group A rats and Group C rats had the lowest weight gain (P0.05). There were no epiphyseal plate chondrocyte necroses in the control group at the first, second, and fourth weeks. In the T-2 toxin group, two rats had chondrocyte-focus necroses at the labrocyte cell zone at the second week. At the fourth week, six rats had chondrocyte-focus or lamellar necroses at the labrocyte cell zone. Three rats had focus necrosis at the proliferation cell zone, and there were three rats with penetration necrosis. In the KBD-affected group, one rat had chondrocyte-focus necrosis at the labrocyte cell zone at the second week and seven rats had chondrocyte-focus necrosis at the labrocyte cell zone at the fourth week. And at the same time, two rats had focus necrosis at the proliferation cell zone, three rats had lamellar necrosis at the labrocyte cell zone, four had focus necrosis at the labrocyte cell zone, and two rats had penetration necrosis. The epiphyseal plate Masson dye of the control group showed deep blue collogen coloration and in the KBD-affected group and T-2 toxin group, collogen showed a pale blue color, the drum dyeing was uneven, and the collogen was showed an absence of color in the region of the necrosis.With KBD-affected feed or T-2 toxin intervention, rats had focus necrosis and lamellar necrosis at the epiphyseal plate. KBD-affected feed rats had less weight gain than T-2 toxin intervention rats, which means there were other etiological factors in KBD-affected feed.

To retrospectively analyse the clinical outcome of emergency treatment of senile intertrochanteric fractures with proximal femoral nail antirotation (PFNA).From September 2008 to March 2009, 35 senile patients with intertrochanteric fracture, aged from 65 to 92 years with an average age of 76.5 years, were treated with PFNA within 24 hours after injury. There were 10 type I fractures, 19 type II and 6 type III according to upgraded Evans-Jensen classification system. All patients were complicated with osteoporosis, and 19 patients had preexisting internal medical diseases. According to the rating scale of disease severity by the American Society of Anesthesiologists (ASA), there were 9 grade I, 14 grade II, 8 grade III, and 4 grade IV.The duration for operation ranged from 45 to 73 minutes with an average of 57.6 minutes. The volume of blood loss during operation ranged from 50 to 120 ml with an average of 77.5 ml. Patients could ambulate 2-4 days after operation (mean 3.5 days). Hospital stay was 4-7 days (mean 5.3 days). Full weight bearing time was 10-14 weeks (mean 12.8 weeks). During hospitalization period, there was no regional or deep infection, hypostatic pneumonia, urinary tract infection and bedsore except for 2 cases of urine retention. All cases were followed up with an average period of 12.3 months, and bone healing achieved within 15-18 weeks (mean 16.6 weeks). No complications such as delayed union, coxa vara or coxa valga, screw breakage or backout occurred and only 2 cases had trochanter bursitis because of thin body and overlong end of the antirotated nail. According to the Harris grading scale, the results were defined as excellent in 21 cases, good in 9 cases and fair in 5 cases, with the excellent and good rates of 85.7%.The emergency treatment of senile intertrochanteric fracture with proximal femoral nail antirotation has the advantages of minimal invasion, easy manipulation, less blood loss, shorter length of stay, less complications, and the preliminary clinical effect is satisfactory.

To review the challenges and countermeasures in the hospital care for Wenchuan earthquake casualties and draw lessons for the protective response in the future. Medical records and laboratory findings of the victims admitted in West China Hospital (WCH) were retrospectively analyzed. Related data were compared between beforemath and aftermath of the earthquake and between WCH and frontier county hospitals. One thousand and thirty-one earthquake survivors were hospitalized, 1 358 victims underwent surgery and 142 victims were transferred to intensive care unit. The incidence of infection, crush syndrome and multiple organ dysfunction syndrome (MODS) was 39.6%, 20.7% and 2.3% respectively. Wound classification showed that the incidence of extremity damage was 72%, while the incidence of chest trauma, abdominal trauma and brain trauma was less than 10% respectively. Isolating rates of environmental pathogens were increased in the aftermath of earthquake, and the spectrum of the pathogens and related antibiotic sensitivities were quite different from those in the beforemath of earthquake. The social economic and population conditions in the earthquake-stricken areas affected the composition of the victims and the geographic features restricted the efficiency of rescue. Trauma-induced MODS, crush syndrome and severe infections all constituted the dilemma for the hospital care, to resolve whether the multidiscipline team work was proved to be an optimizing choice. For a more effective disaster protective response in the future, the study on rescue plan and the ladder therapies for massive casualties should be potentiated.

To investigate the change in distribution of pathogens and nosocomial antibiotic resistant Gram-negative Bacilli infection in intensive care units one month after an earthquake.A retrospective survey on the distribution of nosocomial Gram-negative bacilli infection in intensive care units before and one month after the Wenchuan Earthquake was conducted in the West China Hospital. MicroScan Walkaway 96SI or PHOENIX 100 Automatic System in combined with manual identification, was employed to identify the Gram-negative bacilli infection and antibiotic resistance.The proportion of wound infection increased from 7.9% to 20.2% one month after the earthquake, but infection in respiratory tract stayed the most common infection. The common pathogens included Acinetobacter spp. (36.2%), Pseudomonas aeruginosa (22.7%), and Klebsiella spp. (12.3%) before the earthquake. One month after the earthquake, Imipenem remained highly sensitive against Escherichia coli and Klebsiella. spp., while their resistance to ceftazidime increased. Amikacin became the most sensitive antibiotics against Pseudomonas aeruginosa. Acinetobacter spp. had increased resistance to imipenem, but was highly sensitive to gatifloxacin and cefoxitin. The prevalence of extended spectrum beta-lactamases (ESBLs) in Klebsiella spp. and Escherichia coli increased from 52.6% and 48.8% before the earthquake to 55.0% and 87.5% one month after the earthquake, respectively.There is a significant change in distribution of pathogens and nosocomial antibiotic resistant Gram-negative Bacilli infection in intensive care units one month after the Earthquake, which might be associated with a sudden increase in injured patients. It is essential to regularly monitor the resistant rate of bacilli to antibiotics.

To investigate the relationships between --1997G/T polymorphisms in collagen I alpha 1 gene (COL I A1) upstream regulatory region and bone mineral density(BMD) in postmenopausal women.There were 318 voluntary subjects in Chengdu city enrolled (212 osteoporotic postmenopausal women and 106 normal control subjects) and their BMD were measured by dual-energy X-ray absorptiometry (DEXA ) at lumbar spine (L2-L4). 2 mL peripheral blood was drawn and genomic DNA was extracted from white blood cells. Specific fragment which includes the objective gene was amplified by polymerase chain reaction (PCR); the genotypes of --1997G/T site in COL I A1 were detected by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) method. The PCR products were digested with restriction endonuclease Eco31 I and the result was observed by agarose gel electrophoresis.In the osteoporotic postmenopausal women group, 82 GG, 99 GT and 31 TT were found. In the normal control group, the number of GG, GT and TT were 51, 45 and 10. The subjects in osteoporotic group had higher T allele frequencies than those in normal control group (P0.05). In the osteoporotic postmenopausal women, the carriers with TT genotype of -1997G/T polymorphism had lower spine BMD than those with GG genotype or GT genotype (P0.05).There is a correlation between -1997G/T polymorphisms in COL I A1 upstream regulatory region and BMD. The TT genotype individuals have lower BMD than other genotype individuals.

To retrospectively analyze the medical treatment of 332 patients with lower leg fracture in Wenchuan earthquake admitted in West China Hospital.From May 12, 2008 to June 15, 2008, 332 patients with lower leg fracture injured in Wenchuan earthquake were treated in our hospital. The data on trauma condition and clinical treatment were collected and analyzed.Among the 332 cases of lower leg fracture, there were 179 cases of open fracture, accounting for 53.9%, in which 91% belonged to Gustilo II or III injury with serious pollution. Many patients had posttraumatic complications, vascular and nerve injury, wound infection or osteofascial compartment syndrome. After medical treatment, blood vessels were reconnected, wound surface was repaired and wound infection was under control.For the patients with lower leg fracture in earthquake, we followed the principle of "complete debridement - restoring the continuity of bone bracket-timely recovering blood supply of limbs and repairing nerve damage - repair the wound surface at stage I or II " so as to reduce the incidence of amputation and infection.

To analyze the clinical and radiographic results of the extended trochanteric osteotomy in revision total hip arthroplasty procedure.The data of 27 patients (27 hips) of the clinical and radiographic results were reviewed between January 1998 and June 2005. There were 16 men and 11 women, and the mean age at the time of revision surgery was 63 years (range 42 to 78 years). Indications for use of the trochanteric osteotomy were removal of well-fixed femoral implants. Clinical evaluation included Harris and WOMAC scores. The radiological evaluation included stem subsidence, trochanteric fragment fractures and stem position change.Nineteen patients (19 hips) were followed up for an average of 5.3 years. All osteotomy sites healed by 16 months,with an average time to union of 16 weeks. No intraoperative fracture at the osteotomy site occurred. There was only one dislocation postoperatively. Three femoral components were subsidence at a mean of 3.4 mm (range 3-7 mm) in the first 6 weeks postoperatively; It united within 6 months without further migration.The extended trochanteric osteotomy facilitates the removal of well-fixed femoral implants, allows reliable reattachment of the trochanteric fragment and results an excellent rate of healing and implant stability in revision total hip arthroplasty during mid-term follow-up. Long-term follow-up is needed.

To assess the effect of perioperative administration of a selective cyclooxygenase 2 inhibitor (celecoxib) on pain management and recovery of function after total knee arthroplasty (TKA).Randomized, controlled trial conducted from January 2005 through February 2006, 60 patients underwent TKA for osteoarthritis or rheumatoid arthritis were randomly divided into group of perioperative, administration of celecoxib (Study group, n = 30) and postoperative administration of celecoxib (Control group, n = 30). Patients in Study group were given oral celecoxib 3 d before TKA, 200 mg twice daily, and extended to 5 d postoperatively; patients in Control group were given oral celecoxib 2 h after TKA, 200 mg twice daily, and extended to 5 d postoperatively. All operations were finished by the same surgeon group.The postoperative patient-controlled analgesia (PCA) consumption was significantly less in Study group than in Control group [(43 +/- 12) ml vs. (53 +/- 12) ml, P0.05]. The pain scores of postoperative 4, 8, 12 h, 1, 2 d in Study group were 6.1 +/- 1.2, 5.0 +/- 1.3, 4.3 +/- 1.1, 3.4 +/- 1.2, significantly less than in Control group (P0.05); There were no intergroup significant differences in the pain scores of postoperative 3, 4, 5 d (P0.05). There were no intergroup significant differences in respect to the side-effect occurrence, operation time and postoperative drainage, postoperative analgesic consumption (P0.05). The time to achieve 90 degrees knee flexion was significantly shorter in Study group than in Control group [(6.2 +/- 1.7) d vs. (8.6 +/- 1.8) d, P0.05].Perioperative administration of the selective Celecoxib holds the effect of preemptive analgesia. Compared with postoperative administration, perioperative administration of celecoxib can alleviate the early postoperative pain score, reduce the consumption of postoperative analgesic, accelerate the recovery of joint motion and thus increase the patient satisfaction.
