Abstract: Using an 185-kg NaI[Tl] array, COHERENT has measured the inclusive electron-neutrino charged-current cross section on ${}^{127}$I with neutrinos produced by the Spallation Neutron Source at Oak Ridge National Laboratory. Iodine is one the heaviest targets for which low-energy ($\leq 50$ MeV) inelastic neutrino-nucleus processes have been measured, and this is the first measurement of its inclusive cross section. After a five-year detector exposure, COHERENT reports a flux-averaged cross section for electron neutrinos produced at a pion decay-at-rest source of $9.2^{+2.1}_{-1.8}\times10^{-40}$ cm$^2$. This corresponds to a value that is $\sim$41% lower than predicted using the MARLEY event generator with a measured Gamow-Teller strength distribution. In addition, the observed visible spectrum from charged-current scattering on ${}^{127}$I has been measured between 10 and 55 MeV, and the exclusive zero-neutron and one-or-more-neutron emission cross sections are measured to be $5.12_{-3.1}^{+3.4} \times 10^{-40}\mbox{ cm}^2$ and $2.2_{-0.5}^{+0.4} \times 10^{-40}\mbox{ cm}^2$, respectively., Comment: 7 pages, 6 figures

Abstract: Large-scale low-background detectors are increasingly used in rare-event searches as experimental collaborations push for enhanced sensitivity. However, building such detectors, in practice, creates an abundance of radioassay data especially during the conceptual phase of an experiment when hundreds of materials are screened for radiopurity. A tool is needed to manage and make use of the radioassay screening data to quantitatively assess detector design options. We have developed a Materials Database Application for the nEXO experiment to serve this purpose. This paper describes this database, explains how it functions, and discusses how it streamlines the design of the experiment.

Abstract: The Spallation Neutron Source (SNS) at Oak Ridge National Laboratory is a pulsed source of neutrons and, as a by-product of this operation, an intense source of pulsed neutrinos via stopped-pion decay. The COHERENT collaboration uses this source to investigate coherent elastic neutrino-nucleus scattering and other physics with a suite of detectors. This work includes a description of our geant4 simulation of neutrino production at the SNS and the flux calculation which informs the COHERENT studies. We estimate the uncertainty of this calculation at the ∼10% level based on validation against available low-energy π+ production data.

Abstract: The COHERENT experiment located in Neutrino Alley at the Spallation Neutron Source (SNS), Oak Ridge National Laboratory (ORNL), has made the world's first two measurements of coherent elastic neutrino-nucleus scattering (CEvNS), on CsI and argon, using neutrinos produced at the SNS. The COHERENT collaboration continues to pursue CEvNS measurements on various targets as well as additional studies of inelastic neutrino-nucleus interactions, searches for accelerator-produced dark matter (DM) and physics beyond the Standard Model, using the uniquely high-quality and high-intensity neutrino source available at the SNS. This white paper describes primarily COHERENT's ongoing and near-future program at the SNS First Target Station (FTS). Abstract: Using neutrinos produced at the Spallation Neutron Source (SNS) at Oak Ridge National Laboratory (ORNL), the COHERENT collaboration has studied the Pb($ν_e$,X$n$) process with a lead neutrino-induced-neutron (NIN) detector. Data from this detector are fit jointly with previously collected COHERENT data on this process. A combined analysis of the two datasets yields a cross section that is $0.29^{+0.17}_{-0.16}$ times that predicted by the MARLEY event generator, consistent with no NIN events at $1.8σ$. This is the first inelastic neutrino-nucleus process COHERENT has studied, among several planned exploiting the high flux of low-energy neutrinos produced at the SNS.

Abstract: We study a possible calibration technique for the nEXO experiment using a $^{127}$Xe electron capture source. nEXO is a next-generation search for neutrinoless double beta decay ($0\nu\beta\beta$) that will use a 5-tonne, monolithic liquid xenon time projection chamber (TPC). The xenon, used both as source and detection medium, will be enriched to 90% in $^{136}$Xe. To optimize the event reconstruction and energy resolution, calibrations are needed to map the position- and time-dependent detector response. The 36.3 day half-life of $^{127}$Xe and its small $Q$-value compared to that of $^{136}$Xe $0\nu\beta\beta$ would allow a small activity to be maintained continuously in the detector during normal operations without introducing additional backgrounds, thereby enabling in-situ calibration and monitoring of the detector response. In this work we describe a process for producing the source and preliminary experimental tests. We then use simulations to project the precision with which such a source could calibrate spatial corrections to the light and charge response of the nEXO TPC., Comment: 24 pages, 16 figures

Abstract: We measured the cross section of coherent elastic neutrino-nucleus scattering (CEvNS) using a CsI[Na] scintillating crystal in a high flux of neutrinos produced at the Spallation Neutron Source at Oak Ridge National Laboratory. New data collected before detector decommissioning have more than doubled the dataset since the first observation of CEvNS, achieved with this detector. Systematic uncertainties have also been reduced with an updated quenching model, allowing for improved precision. With these analysis improvements, the COHERENT Collaboration determined the cross section to be (165−25+30)×10−40 cm2, consistent with the standard model, giving the most precise measurement of CEvNS yet. The timing structure of the neutrino beam has been exploited to compare the CEvNS cross section from scattering of different neutrino flavors. This result places leading constraints on neutrino nonstandard interactions while testing lepton flavor universality and measures the weak mixing angle as sin2θW=0.220−0.026+0.028 at Q2≈(50 MeV)2.

This work includes a description of our Geant4 simulation of neutrino production at the SNS and the flux calculation which informs the COHERENT studies. We estimate the uncertainty of this calculation at about 10% based on validation against available low-energy pion production data.

Abstract: We present the analysis and results of the first dataset collected with the MARS neutron detector deployed at the Oak Ridge National Laboratory Spallation Neutron Source (SNS) for the purpose of monitoring and characterizing the beam-related neutron (BRN) background for the COHERENT collaboration. MARS was positioned next to the COH-CsI coherent elastic neutrino-nucleus scattering detector in the SNS basement corridor. This is the basement location of closest proximity to the SNS target and thus, of highest neutrino flux, but it is also well shielded from the BRN flux by infill concrete and gravel. These data show the detector registered roughly one BRN per day. Using MARS' measured detection efficiency, the incoming BRN flux is estimated to be 1.20 ± 0.56 neutrons/m^2/MWh for neutron energies above ∼3.5 MeV and up to a few tens of MeV. We compare our results with previous BRN measurements in the SNS basement corridor reported by other neutron detectors.

Abstract: We present results of several measurements of CsI[Na] scintillation response to 3-60 keV energy nuclear recoils performed by the COHERENT collaboration using tagged neutron elastic scattering experiments and an endpoint technique. Earlier results, used to estimate the coherent elastic neutrino-nucleus scattering (CEvNS) event rate for the first observation of this process achieved by COHERENT at the Spallation Neutron Source (SNS), have been reassessed. We discuss corrections for the identified systematic effects and update the respective uncertainty values. The impact of updated results on future precision tests of CEvNS is estimated. We scrutinize potential systematic effects that could affect each measurement. In particular we confirm the response of the H11934-200 Hamamatsu photomultiplier tube (PMT) used for the measurements presented in this study to be linear in the relevant signal scale region., Comment: The version accepted by JINST. Abstract: The search for neutrinoless double beta decay probes the fundamental properties of neutrinos, including whether or not the neutrino and antineutrino are distinct. Double beta detectors are large and expensive, so background reduction is essential for extracting the highest sensitivity. The identification, or 'tagging', of the $^{136}$Ba daughter atom from double beta decay of $^{136}$Xe provides a technique for eliminating backgrounds in the nEXO neutrinoless double beta decay experiment. The tagging scheme studied in this work utilizes a cryogenic probe to trap the barium atom in solid xenon, where the barium atom is tagged via fluorescence imaging in the solid xenon matrix. Here we demonstrate imaging and counting of individual atoms of barium in solid xenon by scanning a focused laser across a solid xenon matrix deposited on a sapphire window. When the laser sits on an individual atom, the fluorescence persists for $\sim$30~s before dropping abruptly to the background level, a clear confirmation of one-atom imaging. No barium fluorescence persists following evaporation of a barium deposit to a limit of $\leq$0.16\%. This is the first time that single atoms have been imaged in solid noble element. It establishes the basic principle of a barium tagging technique for nEXO.

Abstract: The General Antiparticle Spectrometer (GAPS) will carry out a sensitive dark matter search by measuring low-energy ($\mathrm{E} < 0.25 \mathrm{GeV/nucleon}$) cosmic ray antinuclei. The primary targets are low-energy antideuterons produced in the annihilation or decay of dark matter. At these energies antideuterons from secondary/tertiary interactions are expected to have very low fluxes, significantly below those predicted by well-motivated, beyond the standard model theories. GAPS will also conduct low-energy antiproton and antihelium searches. Combined, these observations will provide a powerful search for dark matter and provide the best observations to date on primordial black hole evaporation on Galactic length scales. The GAPS instrument detects antinuclei using the novel exotic atom technique. It consists of a central tracker with a surrounding time-of-flight (TOF) system. The tracker is a one cubic meter volume containing 10 cm-diameter lithium-drifted silicon (Si(Li)) detectors. The TOF is a plastic scintillator system that will both trigger the Si(Li) tracker and enable better reconstruction of particle tracks. After coming to rest in the tracker, antinuclei will form an excited exotic atom. This will then de-excite via characteristic X-ray transitions before producing a pion/proton star when the antiparticle annihilates with the nucleus. This unique event topology will give GAPS the nearly background-free detection capability required for a rare-event search. Abstract: BACKGROUND & AIMS: Primary sclerosing cholangitis (PSC) is an orphan hepatobiliary disorder associated with inflammatory bowel disease (IBD). We aimed to estimate the risk of disease progression based on distinct clinical phenotypes in a large international cohort of patients with PSC. METHODS: We performed a retrospective outcome analysis of patients diagnosed with PSC from 1980 through 2010 at 37 centers in Europe, North America, and Australia. For each patient, we collected data on sex, clinician-reported age at and date of PSC and IBD diagnoses, phenotypes of IBD and PSC, and date and indication of IBD-related surgeries. The primary and secondary endpoints were liver transplantation or death (LTD) and hepatopancreatobiliary malignancy, respectively. Cox proportional hazards models were applied to determine the effects of individual covariates on rates of clinical events, with time-to-event analysis ascertained through Kaplan-Meier estimates. RESULTS: Of the 7121 patients in the cohort, 2616 met the primary endpoint (median time to event of 14.5 years) and 721 developed hepatopancreatobiliary malignancy. The most common malignancy was cholangiocarcinoma (n = 594); patients of advanced age at diagnosis had an increased incidence compared with younger patients (incidence rate: 1.2 per 100 patient-years for patients younger than 20 years old, 6.0 per 100 patient-years for patients 21-30 years old, 9.0 per 100 patient-years for patients 31-40 years old, 14.0 per 100 patient-years for patients 4150 years old, 15.2 per 100 patient-years for patients 51-60 years old, and 21.0 per 100 patient-years for patients older than 60 years). Of all patients with PSC studied, 65.5% were men, 89.8% had classical or large-duct disease, and 70.0% developed IBD at some point. Abstract: The projected performance and detector configuration of nEXO are described in this pre-Conceptual Design Report (pCDR). nEXO is a tonne-scale neutrinoless double beta ($0\nu\beta\beta$) decay search in $^{136}$Xe, based on the ultra-low background liquid xenon technology validated by EXO-200. With $\simeq$ 5000 kg of xenon enriched to 90% in the isotope 136, nEXO has a projected half-life sensitivity of approximately $10^{28}$ years. This represents an improvement in sensitivity of about two orders of magnitude with respect to current results. Based on the experience gained from EXO-200 and the effectiveness of xenon purification techniques, we expect the background to be dominated by external sources of radiation. The sensitivity increase is, therefore, entirely derived from the increase of active mass in a monolithic and homogeneous detector, along with some technical advances perfected in the course of a dedicated R&D program. Hence the risk which is inherent to the construction of a large, ultra-low background detector is reduced, as the intrinsic radioactive contamination requirements are generally not beyond those demonstrated with the present generation $0\nu\beta\beta$ decay experiments. Indeed, most of the required materials have been already assayed or reasonable estimates of their properties are at hand. The details described herein represent the base design of the detector configuration as of early 2018. Where potential design improvements are possible, alternatives are discussed. Abstract: The General Antiparticle Spectrometer (GAPS) is designed to carry out indirect dark matter search by measuring low-energy cosmic-ray antiparticles. Below a few GeVs the flux of antiparticles produced by cosmic-ray collisions with the interstellar medium is expected to be very low and several well-motivated beyond-standard models predict a sizable contribution to the antideuteron flux. GAPS is planned to fly on a long-duration balloon over Antarctica in the austral summer of 2020. The primary detector is a 1m3 central volume containing planes of Si(Li) detectors. This volume is surrounded by a time-of-flight system to both trigger the Si(Li) detector and reconstruct the particle tracks. The detection principle of the experiment relies on the identification of the antiparticle annihilation pattern. Low energy antiparticles slow down in the apparatus and they are captured in the medium to form exotic excited atoms, which de-excite by emitting characteristic X-rays. Afterwards they undergo nuclear annihilation, resulting in a star of pions and protons. The simultaneous measurement of the stopping depth and the dE/dx loss of the primary antiparticle, of the X-ray energies and of the star particle-multiplicity provides very high rejection power, that is critical in rare-event search. Abstract: The search for neutrinoless double beta decay probes the fundamental properties of neutrinos, including whether or not the neutrino and antineutrino are distinct. Double beta detectors are large and expensive, so background reduction is essential for extracting the highest sensitivity. The identification, or 'tagging', of the $^{136}$Ba daughter atom from double beta decay of $^{136}$Xe provides a technique for eliminating backgrounds in the nEXO neutrinoless double beta decay experiment. The tagging scheme studied in this work utilizes a cryogenic probe to trap the barium atom in solid xenon, where the barium atom is tagged via fluorescence imaging in the solid xenon matrix. Here we demonstrate imaging and counting of individual atoms of barium in solid xenon by scanning a focused laser across a solid xenon matrix deposited on a sapphire window. When the laser sits on an individual atom, the fluorescence persists for $\sim$30~s before dropping abruptly to the background level, a clear confirmation of one-atom imaging. No barium fluorescence persists following evaporation of a barium deposit to a limit of $\leq$0.16\%. This is the first time that single atoms have been imaged in solid noble element. It establishes the basic principle of a barium tagging technique for nEXO.

Specialistic Surgeries and Nephrology Policlinic Federico II, Naples, ITALY Introduction: Inguinal hernia repair is one of the most common operations performed in general surgery, especially among elderly patients, due to age-related loss of muscle mass and increase of co-morbidities associated with high intra-abdominal pressure. The purpose of our trial was to assess the safeness and the impact on quality of life of tension free, sutureless hernia repair technique with the use of fibrin glue under local anesthesia in elderly patients. Methods: From January 2010 to December 2012,53 male patients aged 70 and above (mean age 73.9 years) were enrolled; complicated, recurrent, scrotal hernia and ASA IV patients were excluded. Furthermore diabetic patients with glycated hemoglobin level 7% or more were ruled out for presumable neuropathy. Informed consent and data from SF36 questionnaire were collected preoperatively. The Visual Analogue Scale (VAS) for postoperative pain and a new SF36 questionnaire for overall satisfaction at one year, were administered postoperatively. Chronic pain was classified according to Cunningham's criteria. Operative time, length of hospitalization, postoperative use of nonsteroidal anti-inflammatory drugs, complications and recurrences were also assessed. Results: All patients were operated under local anaesthesia (2% Mepivacaine Cloridrate and 7,5mg!ml Ropivacaine) with light sedation; in all cases partially absorbable mesh and plug (polypropylene! polyglecaprone 25) have been implanted and fixed with I ml of fibrin glue. 50 out of 53 patients completed the 2 years follow-up, one died for not related comorbidity. Mean operative time was 54.8 minutes; 46 patients were discharged at home the same day, 5 the following day, 2 patients had to stay one more day for postoperative complications (I haematoma, I urinary retention), no major complications were observed; at two years follow-up, 2 recurrences (4%) have been observed; mean VAS score for post-operative pain, assessed at 6, 12, 24 hours and 7 days after surgery, was 4 or less for 50 (94,3%) patients, only 3 (5,6%) patients referred a score> 4. At one year follow up only 2 (4%) patients suffered of chronic postoperative pain (I mild and I moderate), no severe chronic postoperative pain has been reported. Data from pre and postoperative SF36 questionnaires, analysed by using the Student's t test, showed significant increase of the score both in the Physical Component Summery (PCM) and in the Mental Component Summary (MCS) with a p-value < 0.0001. Conclusion: Inguinal hernia repair with use of fibrin glue and partially absorbable prosthesis under local anaesthesia is a safe technique in elderly patients

Abstract: The coherent elastic scattering of neutrinos off nuclei has eluded detection for four decades, even though its predicted cross section is by far the largest of all low-energy neutrino couplings. This mode of interaction offers new opportunities to study neutrino properties and leads to a miniaturization of detector size, with potential technological applications. We observed this process at a 6.7σ confidence level, using a low-background, 14.6-kilogram CsI[Na] scintillator exposed to the neutrino emissions from the Spallation Neutron Source at Oak Ridge National Laboratory. Characteristic signatures in energy and time, predicted by the standard model for this process, were observed in high signal-to-background conditions. Improved constraints on nonstandard neutrino interactions with quarks are derived from this initial data set.

Abstract: BACKGROUND: Monoethylglycinexylidide (MEGX) formation following lignocaine injection has recently been proposed as a simple dynamic liver function test based on a single measurement of its serum concentration. AIM: To determine the optimal sampling time for MEGX determination. METHODS: A modelling analysis of lignocaine and MEGX kinetics was performed in seven normals and in four patients with compensated liver cirrhosis; a similar study was performed in 74 cirrhotic patients, divided into two groups according to disease severity (Pugh score). RESULTS: Only the MEGX fractional formation rate (kf) and formation delay (tau) were significantly altered in cirrhotic patients compared to normals: kf = 0.15 +/- 0.03 vs. 0.32 +/- 0.10 min-1 (mean +/- s.d.); tau = 7.7 +/- 2.0 vs. 3.9 +/- 2.9 min-1. A good correlation was found between kf and late (r = 0.82) but not early (r = 0.63) serum MEGX formation, suggesting that late measurements for the clinical MEGX test are preferred. In the second part of our investigation, by discriminant analysis of MEGX test data for 74 cirrhotic patients, the late MEGX concentrations gave the best discrimination between the two classes. In particular, the 60 min MEGX concentration showed the best diagnostic accuracy (81%), sensitivity (75%) and specificity (84%). The association of this with other MEGX parameters, either singly or derived from the whole curve measurements, did not improve the performance of the method. CONCLUSION: The MEGX test, based on a single determination 60 min after lignocaine injection, may be regarded as a simple and sensitive quantitative liver function test

Abstract: A readout chip for diffraction imaging applications at new generation X-ray FELs (Free Electron Lasers) has been designed in a 65~nm CMOS technology. It consists of a $ 32 \times 32 $ matrix, with square pixels and a pixel pitch of $ 110\ \mu m $. Each cell includes a low-noise charge sensitive amplifier (CSA) with dynamic signal compression, covering an input dynamic range from 1 to $ 10^4 $ photons and featuring single photon resolution at small signals at energies from 1 to 10 keV. The CSA output is processed by a time-variant shaper performing gated integration and correlated double sampling. Each pixel includes also a small area, low power 10-bit time-interleaved Successive Approximation Register (SAR) ADC for in-pixel digitization of the amplitude measurement. The channel can be operated at rates up to 4.5~MHz, to be compliant with the rates foreseen for future X-ray FEL machines. The ASIC has been designed in order to be bump bonded to a slim/active edge pixel sensor, in order to build the first demonstrator for the PixFEL (advanced X-ray PIXel cameras at FELs) imager.

Abstract: The primary goal of the COHERENT collaboration is to measure and study coherent elastic neutrino-nucleus scattering (CEvNS) using the high-power, few-tens-of-MeV, pulsed source of neutrinos provided by the Spallation Neutron Source (SNS) at Oak Ridge National Laboratory (ORNL). The COHERENT collaboration reported the first detection of CEvNS [Akimov:2017ade] using a CsI[Na] detector. At present the collaboration is deploying four detector technologies: a CsI[Na] scintillating crystal, p-type point-contact germanium detectors, single-phase liquid argon, and NaI[Tl] crystals. All detectors are located in the neutron-quiet basement of the SNS target building at distances 20-30 m from the SNS neutrino source. The simultaneous measurement in all four COHERENT detector subsystems will test the $N^2$ dependence of the cross section and search for new physics. In addition, COHERENT is measuring neutrino-induced neutrons from charged- and neutral-current neutrino interactions on nuclei in shielding materials, which represent a non-negligible background for CEvNS as well as being of intrinsic interest. The Collaboration is planning as well to look for charged-current interactions of relevance to supernova and weak-interaction physics. Abstract: The General AntiParticle Spectrometer (GAPS) is a balloon-borne instrument designed to detect cosmic-ray antimatter using the novel exotic atom technique, obviating the strong magnetic fields required by experiments like AMS, PAMELA, or BESS. It will be sensitive to primary antideuterons with kinetic energies of $\approx0.05-0.2$ GeV/nucleon, providing some overlap with the previously mentioned experiments at the highest energies. For $3\times35$ day balloon flights, and standard classes of primary antideuteron propagation models, GAPS will be sensitive to $m_{\mathrm{DM}}\approx10-100$ GeV c$^{-2}$ WIMPs with a dark-matter flux to astrophysical flux ratio approaching 100. This clean primary channel is a key feature of GAPS and is crucial for a rare event search. Additionally, the antiproton spectrum will be extended with high statistics measurements to cover the $0.07 \leq E \leq 0.25 $ GeV domain. Abstract: In recent years, vast progress in basic science and technology has ramatically changed scientific perspectives by opening the new orizon of translational medicine. The term “translational” refers to he bench-to-bedside paradigm and addresses the process of applyng ideas, insights and discoveries generated through basic science chemistry, biology, genetics) to improve diagnosis, prevention and reatment of human diseases [1]. The bench-to-bedside process can be divided into several steps, rom basic inquiry into pathophysiology to the identification of otentially targetable pathways, up to the development of new rugs. The final track resides in clinical testing of therapeutic agents nd evaluation of their efficacy. Traditionally, these steps are perormed by independent experts, starting from the basic scientist, hrough the chemistry expert/drug developer, ending with the clincal scientist. These professionals most of the time are physically eparated, and very often use a different terminology with diffiulties in cooperation and communication. The identification of translational approach recognizes the strong need for a close nteraction between individuals in science. Furthermore, sharing nformation among players also favours feedback from the cliniian to the basic scientist, thus making research at least a two-way rocess, from bench-to-bedside and from bedside-to-bench [2]. In the last two decades, gastroenterology has become an area f dynamic translational research, with some targets already sucessfully translated into practice and policy, and some others still rogressing into clinically effective drugs in a number of different onditions (Supplementary Figure S1). Abstract: The General Antiparticle Spectrometer (GAPS) experiment is a balloon payload designed to measure low-energy cosmic antinuclei during at least three $\sim$35-day Antarctic flights, with the first flight planned for December, 2022. With its large geometric acceptance and novel exotic atom-based particle identification method, GAPS will detect $\sim$1000 antiprotons per flight, producing a precision cosmic antiproton spectrum in the kinetic energy range of $0.03 - 0.23$ GeV/$n$ at float altitude (corresponding to $0.085- 0.30$ GeV/$n$ at the top of the atmosphere). With these high statistics in a measurement extending to lower energy than any previous experiment, and with orthogonal sources of systematic uncertainty compared to measurements made using traditional magnetic spectrometer techniques, the GAPS antiproton measurement will be sensitive to physics including dark matter annihilation, primordial black hole evaporation, and cosmic ray propagation. The antiproton measurement will also validate the GAPS exotic atom technique for the antideuteron and antihelium rare-event searches and provide insight into models of cosmic particle attenuation and production in the atmosphere. This contribution demonstrates the GAPS sensitivity to antiprotons using a full instrument simulation, event reconstruction, and solar and atmospheric effects.

Abstract: Cholangiocarcinoma (CCA) includes a cluster of highly heterogeneous biliary malignant tumours that can arise at any point of the biliary tree. Their incidence is increasing globally, currently accounting for ~15% of all primary liver cancers and ~3% of gastrointestinal malignancies. The silent presentation of these tumours combined with their highly aggressive nature and refractoriness to chemotherapy contribute to their alarming mortality, representing ~2% of all cancer-related deaths worldwide yearly. The current diagnosis of CCA by non-invasive approaches is not accurate enough, and histological confirmation is necessary. Furthermore, the high heterogeneity of CCAs at the genomic, epigenetic and molecular levels severely compromises the efficacy of the available therapies. In the past decade, increasing efforts have been made to understand the complexity of these tumours and to develop new diagnostic tools and therapies that might help to improve patient outcomes. In this expert Consensus Statement, which is endorsed by the European Network for the Study of Cholangiocarcinoma, we aim to summarize and critically discuss the latest advances in CCA, mostly focusing on classification, cells of origin, genetic and epigenetic abnormalities, molecular alterations, biomarker discovery and treatments. Furthermore, the horizon of CCA for the next decade from 2020 onwards is highlighted., Cholangiocarcinoma (CCA) comprises heterogeneous biliary malignant tumours, and their incidence is increasing worldwide. This expert Consensus Statement, endorsed by the ENS-CCA, summarizes the latest advances in CCA, including classification, genetics and treatment, and provides recommendations for CCA management and priorities across basic, translational and clinical research.

Abstract: The most studied physiological function of biliary epithelial cells (cholangiocytes) is to regulate bile flow and composition, in particular the hydration and alkalinity of the primary bile secreted by hepatocytes. After almost three decades of studies it is now become clear that cholangiocytes are also involved in epithelial innate immunity, in inflammation, and in the reparative processes in response to liver damage. An increasing number of evidence highlights the ability of cholangiocyte to undergo changes in phenotype and function in response to liver damage. By participating actively to the immune and inflammatory responses, cholangiocytes represent a first defense line against liver injury from different causes. Indeed, cholangiocytes express a number of receptors able to recognize pathogen- or damage-associated molecular patterns (PAMPs/DAMPs), such as Toll-like receptors (TLR), which modulate their pro-inflammatory behavior. Cholangiocytes can be both the targets and the initiators of the inflammatory process. Derangements of the signals controlling these mechanisms are at the basis of the pathogenesis of different cholangiopathies, both hereditary and acquired, such as cystic fibrosis-related liver disease and sclerosing cholangitis. Abstract: Background Autoimmune hepatitis, primary biliary cholangitis, and primary sclerosing cholangitis represent the three major autoimmune liver diseases (AILDs). Their management is highly specialized, requires a multidisciplinary approach and often relies on expensive, orphan drugs. Unfortunately, their treatment is often unsatisfactory, and the care pathway heterogeneous across different centers. Disease-specific clinical outcome indicators (COIs) able to evaluate the whole cycle of care are needed to assist both clinicians and administrators in improving quality and value of care. Aim of our study was to generate a set of COIs for the three AILDs. We then prospectively validated these indicators based on a series of consecutive patients recruited at three tertiary clinical centers in Lombardy, Italy. Methods In phase I using a Delphi method and a RAND 9-point appropriateness scale a set of COIs was generated. In phase II the indicators were applied in a real-life dataset. Results Two-hundred fourteen patients were enrolled and followed-up for a median time of 54 months and the above COIs were recorded using a web-based electronic medical record program. The COIs were easy to collect in the clinical practice environment and their values compared well with the available natural history studies. Conclusions We have generated a comprehensive set of COIs which sequentially capture different clinical outcome of the three AILDs explored. These indicators represent a critical tool to implement a value-based approach to patients with these conditions, to monitor, compare and improve quality through benchmarking of clinical performance and to assess the significance of novel drugs and technologies. Abstract: Prognosis of cholangiocarcinoma, a devastating liver epithelial malignancy characterized by early invasiveness, remains very dismal, though its incidence has been steadily increasing. Evidence is mounting that in cholangiocarcinoma, tumor epithelial cells establish an intricate web of mutual interactions with multiple stromal components, largely determining the pervasive behavior of the tumor. The main cellular components of the tumor microenvironment (i.e. myofibroblasts, macrophages, lymphatic endothelial cells), which has been recently termed as ‘tumor reactive stroma’, are recruited and activated by neoplastic cells, and in turn, deleteriously mold tumor behavior by releasing a huge variety of paracrine signals, including cyto/chemokines, growth factors, morphogens and proteinases. An abnormally remodeled and stiff extracellular matrix favors and supports these cell interactions. Although the mechanisms responsible for the generation of tumor reactive stroma are largely uncertain, hypoxia presumably plays a central role. In this review, we will dissect the intimate relationship among the different cell elements cooperating within this complex ‘ecosystem’, with the ultimate goal to pave the way for a deeper understanding of the mechanisms underlying cholangiocarcinoma aggressiveness, and possibly, to foster the development of innovative, combinatorial therapies aimed at halting tumor progression. Abstract: Background & Aims Genetic defects in polycystin-1 or -2 ( PC1 or PC2 ) cause polycystic liver disease associated with autosomal dominant polycystic kidney disease (PLD-ADPKD). Progressive cyst growth is sustained by a cAMP-dependent Ras/ERK/HIFα pathway, leading to increased vascular endothelial growth factor A (VEGF-A) signaling. In PC2-defective cholangiocytes, cAMP production in response to [Ca 2+ ] ER depletion is increased, while store-operated Ca 2+ entry (SOCE), intracellular and endoplasmic reticulum [Ca 2+ ] ER levels are reduced. We investigated whether the adenylyl cyclases, AC5 and AC6, which can be inhibited by Ca 2+ , are activated by the ER chaperone STIM1. This would result in cAMP/PKA-dependent Ras/ERK/HIFα pathway activation in PC2-defective cells, in response to [Ca 2+ ] ER depletion. Methods PC2 / AC6 double conditional knockout (KO) mice were generated ( Pkd2 / AC6 KO) and compared to Pkd2 KO mice. The AC5 inhibitor SQ22,536 or AC5 siRNA were used in isolated cholangiocytes while the inhibitor was used in biliary organoid and animals; liver tissues were harvested for histochemical analysis. Results When comparing Pkd2 / AC6 KO to Pkd2 KO mice, no decrease in liver cyst size was found, and cellular cAMP after [Ca 2+ ] ER depletion only decreased by 12%. Conversely, in PC2-defective cells, inhibition of AC5 significantly reduced cAMP production, pERK1/2 expression and VEGF-A secretion. AC5 inhibitors significantly reduced growth of biliary organoids derived from Pkd2 KO and Pkd2 / AC6 KO mice. In vivo treatment with SQ22,536 significantly reduced liver cystic area and cell proliferation in PC2-defective mice. After [Ca 2+ ] ER depletion in PC2-defective cells, STIM1 interacts with AC5 but not with Orai1, the Ca 2+ channel that mediates SOCE. Conclusion [Ca 2+ ] ER depletion in PC2-defective cells activates AC5 and results in stimulation of cAMP/ERK1-2 signaling, VEGF production and cyst growth. This mechanism may represent a novel therapeutic target. Lay summary Polycystic liver diseases are characterized by progressive cyst growth until their complications mandate surgery or liver transplantation. In this manuscript, we demonstrate that inhibiting cell proliferation, which is induced by increased levels of cAMP, may represent a novel therapeutic target to slow the progression of the disease.

Abstract: The General Antiparticle Spectrometer (GAPS) is an Antarctic balloon-borne mission to indirectly search for dark matter through sensitive observation of cosmic antiparticles. The first flight is planned for late 2021. GAPS is the first experiment optimized specifically for detection of low-energy (< 0.25 GeV/n) antideuterons, which are recognized as distinctive signals from dark matter annihilation or decay in the Galactic halo. To achieve high sensitivity to cosmic antinuclei in this low-energy range, GAPS uses a novel particle identification method based on exotic atom capture and decay. The GAPS instrument consists of ten planes of 1440 10 cm-diameter, 2.5 mm-thick, 8-strip lithium drifted silicon (Si(Li)) detectors, which constitutes the tracker, surrounded by a plastic scintillator time-of-flight system. A new fabrication technique has been developed to satisfy the stringent requirements of the mission. In this contribution, we describe the front-end electronics of the tracker of GAPS. The system is composed of front-end ASICs and power supplies. The ASICs provide readout and digitization of the signal (with an 11-bit ADC) in a wide dynamic range (10 keV - 100 MeV). Every ASIC has 32 channels and performs the readout for 4 detectors, for a total amount of 11520 channels. The ASIC analog front-end is based on a dynamic compression technique to handle a large range of signal amplitudes and features a low noise performance, achieving the required 4 keV resolution at low energies. Abstract: Background & Aims: Chronic liver diseases (CLDs) are major health problems that require complex and costly treatments. Liver-specific clinical outcome indicators (COIs) able to assist both clinicians and administrators in improving the value of care are presently lacking. The Value-Based Medicine in Hepatology (VBMH) study aims to fill this gap, devising and testing a set of COIs for CLD, that could be easily collected during clinical practice. Here we report the COIs generated and recorded for patients with HBV or HCV infection at different stages of the disease. Methods/Results: In the first phase of VBMH study, COIs were identified, based on current international guidelines and literature, using a modified Delphi method and a RAND 9-point appropriateness scale. In the second phase, COIs were tested in an observational, longitudinal, prospective, multicentre study based in Lombardy, Italy. Eighteen COIs were identified for HBV and HCV patients. Patients with CLD secondary to HBV (547) or HCV (1391) were enrolled over an 18-month period and followed for a median of 4years. The estimation of the proposed COIs was feasible in the real-word clinical practice and COI values compared well with literature data. Further, the COIs were able to capture the impact of new effective treatments like direct-acting antivirals (DAAs) in the clinical practice. Conclusions: The COIs efficiently measured clinical outcomes at different stages of CLDs. While specific clinical practice settings and related healthcare systems may modify their implementation, these indicators will represent an important component of the tools for a value-based approach in hepatology and will positively affect care delivery.
