Bu çalışmada, sosyolojinin kuruluşundan itibaren en temel çalışma konularından biri olan değişme kavramının sosyoloji tarihinde ele alınış biçimlerine değinilerek, yirmi birinci yüzyılda afetler ve sonrasında yaşanan değişme süreçleriyle beraber incelenmesi amaçlanmıştır. Bu amaç doğrultusunda, klasik değişme kuramları kısaca incelenerek, değişmenin temel dinamikleri ortaya konulmuştur. Ardından, yirmi birinci yüzyılda sayıları artan doğal afetler sonrasında yaşanan travma ve yıkım sonucunda gerçekleşen fiziksel ve toplumsal değişimler dirençlilik kavramıyla ele alınarak, değişme ve sosyal değişme ilişkisine değinilmiştir. Afet sonrasında yeniden düzene dönebilme kapasitesi olarak ele alınabilecek olan dirençlilik ile değişme birbiriyle yakın ilişkili olarak tartışılmış ve özellikle dirençliliğin bir alt kategorisi olarak kırılganlık kavramı da sosyal boyutuyla incelemeye dahil edilmiştir. Değişme, dirençlilik ve kırılganlık kavramları ekseninde, toplumların afet sonrası düzene yeniden geri dönebilme veya yeni düzene uyum sağlama kapasiteleri önemli belirleyiciler olarak kabul edilmektedir. Dolayısıyla, afet yönetimi planlarında ve hazırlık aşamalarında, dirençlilik kapasitelerinin belirlenebilmesi, sosyal kırılganlık düzeyleriyle ilişkili olarak önem taşımaktadır. Ayrıca, siyasalar oluşturulurken, dikkate alınacak planlarda, değişme ve dirençlilik ilişkisine önem verilmesi, toplumların en kısa sürede yeniden düzene ve uyumluluğa kavuşabilmesine önemli katkılar sağlayacaktır.

Introduction: Gallstone disease is a disorder characterised by the formation of stones in the biliary tract. It is the most common biliary condition accounting for more than 98% of all gallbladder and biliary tract disorders. In Africa, previous studies have shown a relative rarity of this condition with a prevalence less than 5%; since it is between 2 and 5 times higher in other continents. A good knowledge of the profile of patient with gallstone disease in a surgical setting could allow to reduce gallstone disease complications and to tailor better the treatment. To our knowledge, there was no previous study about gallstone disease in this region even if there is a high prevalence of metabolic factors of gallstone disease. Methods: This study objective is to describe the epidemiological, diagnostic and therapeutic profile of patients with gallstone disease at the Department of General Surgery of Saint-Louis Hospital (Senegal). It will be a single-centre retrospective cohort study in a period of 5 years (January 2015 – December 2020). The patients’ record of the department of general surgery will be consulted and the patient contacted if there are missing data. Patients with gallstone disease diagnosed with imaging (ultrasonography and/or CT scan) regardless the presentation (asymptomatic, biliary colic, cholecystitis, common bile duct lithiasis, angio-cholitis, pancreatitis) will be included. Adults and paediatric patients will be enrolled. Patient records lacking sufficient data will be excluded. Studied parameters will be epidemiological, clinical, paraclinical and therapeutic aspects. Ethics and dissemination: Anonymity and confidentiality of information collected in patients will be respected. This research protocol will be submitted to the Ethics Committee of our institution for approval. The knowledge of the profile of patients with gallstone disease in a surgical setting could allow to reduce gallstone disease complications and to tailor better the treatment. Finally, it will help to reduce the burden of gallstone disease. Introduction Les IST constituent des cofacteurs de l’infection a VIH. Leur prise en charge adequate permet de reduire le risque de transmission du VIH. L’objectif de notre etude etait de determiner la frequence hospitaliere des signes ou syndromes d’IST chez les personnes infectees par le VIH et de decrire les aspects therapeutiques. Materiel et methodes Il s’agissait d’une etude transversale descriptive, avec un recueil retrospectif et prospectif des donnees, menee sur une periode de 5 ans (du 1er janvier 2009 au 31 decembre 2013) dans 4 sites de suivi de PVVIH a Dakar. Les signes cliniques a l’examen initial des patients et les premieres examens bacterio-virologiques etaient pris en compte dans notre etude. Les donnees etaient recueillies sur Excel et analysees grâce au logiciel IBM SPSS Statistics 20. Resultats Nous avons recense 703 personnes infectees par le VIH dont 96 avec des signes d’IST, soit une frequence hospitaliere de 13,65 %. Le sex-ratio etait de 0,46 et l’âge moyen de 40 ans ± 11 ans avec des extremes de 16 et 73 ans. Il existait des antecedents d’IST dans 5,4 % (n = 38) des cas. Les ulcerations genitales etaient trouvees dans 52 % (n = 50) des cas, les leucorrhees dans 59 % (n = 57), les condylomes dans 19,8 % (n = 19) et les douleurs pelviennes chroniques dans 43 % (n = 42). Aucun cas d’ecoulement uretral n’a ete trouve. Ces manifestations etaient associees dans 60,4 % des cas (n = 58). Le prelevement vaginal, effectue dans 31,5 % (n = 18), isolait un germe dans 61 % (n = 11). Les germes responsables d’IST trouves etaient les suivantes : Chlamydia trachomatis (n = 5), Mycoplasma genitalium (n = 4), Trichomonas vaginalis (n = 1). La serologie syphilitique, effectuee chez 387 patients, etait positive chez 2,5 % (n = 10). Une positivite de l’Ag HBs etait notee chez 7,53 % (n = 33) des 438 patients chez lesquels cet examen etait effectue. Les recommandations nationales sur la prise en charge syndromique d’une ulceration genitale, des leucorrhees et des douleurs pelviennes etaient respectivement adoptees dans 4 % (n = 2), 6,9 % (n = 3) et aucun cas. Une guerison, apres traitement syndromique, etait notee dans 81,1 % (n = 73). Conclusion La frequence des signes d’IST est elevee chez les personnes infectees par le VIH. Malgre le faible taux de recherche microbiologique, les recommandations nationales sur la prise en charge syndromique des IST sont rarement respectees.

Aktif yurttaşlık söylemiyle ortaya çıkan bu tartışmalar daha çok katılım, sivil toplum ve çoğulculuk kavramları ekseninde sürdürülmektedir. 2007 yılında Muğla-Akyaka Beldesinde gerçekleştirilen Birleşmiş Milletler Kalkınma Fonu, Yerel Yönetim Reformuna Destek Projesi, Vatandaş Memnuniyet Anketi verilerinden elde edilen sonuçlar üzerine kurgulanan araştırmada, vatandaşlara belediyenin hizmetlerini ve hizmet kalitesini; sorumluluk alanlarını sorgulatarak, beklentilerini saptamaya çalışırken belediye vatandaş ilişkisine daha demokratik bir yaklaşım getirmeyi amaçlamaktadır. Bu çalışma, Akyaka Belediyesi sınırları içinde yaşayan vatandaşların Belediyenin hizmetlerinden memnuniyetini ölçmek ve sorunlarını saptamak amacıyla, Yerel Yönetimlere Destek Reformu çerçevesinde Akyaka’da Nisan ile Haziran 2007 Tarihlerinde gerçekleştirilen Vatandaş Memnuniyet Anketinin verilerinden yola çıkılarak hazırlanmıştır., In recent years, democratic governance, demands for a democratic lifestyle and changing perceptions of citizenship derived from these demands, constitute the most significant areas of debate in social sciences. Emerging around the axis of active citizenship, these debates mainly correspond with concepts of “participation”, “civil society” and “plurality”. This study was based upon data from the United Nations Development Programme, the Support for Local Administration Reform Project and a Citizen Satisfaction Survey all carried out in the Municipality of Akyaka in 2007. By gauging Citizens’ opinions on the responsibilities and the quality of service of the Municipality, this study aims to democratize the relationship between the Municipality and its Citizens by determining the mutual responsibilities of each. The Study was formulated upon the data gathered from the Citizen Satisfaction Survey, within the scope of the Support for Local Administration Reform Project, carried out between April and June 2007, to determine the level of satisfaction and problems of citizens from services provided by the Municipality of Akyaka
