Aortic aneurysm rupture is typically the failure of the aortic wall to withstand the tension placed upon it by a patient’s blood pressure. Along with clinical judgement and an assessment of signs and symptoms, a computed topography angiogram (CTA) is amongst the first line of diagnostic studies having 87-94% sensitivity for rupture. Therefore, while the possibility of a CTA not catching an aortic rupture is present, the chance is very low. Usually a missed or delayed diagnosis for this type of pathology is catastrophic resulting in patient death. We present the case report of a 68-year-old male with a contained aortic rupture that was initially missed on CTA. He presented to the ED with shock like symptoms after a syncopal episode at home. Initial computed topography (CT) scan revealed a pericardial effusion with concern for hemopericardium and mass effect on the right ventricle. A follow up CTA was obtained; however, it was negative for any acute process. Given patient’s continued instability, a TTE was performed which showed concern for a dissection flap. Subsequently, a TEE was performed showing an aortic dissection at the anterior aortic root as well as severe aortic insufficiency with a perforated right coronary cusp. The patient underwent emergent cardiac surgery and recovered well despite the delay in diagnosis. This case demonstrates the importance of a high index of suspicion for acute aortic dissection despite an initial negative CTA.

Background More than 50% of colorectal cancer (CRC)–related deaths could be prevented with early detection and treatment. Federally Qualified Healthcare Centers (FQHCs)—safety-net facilities that provide primary care services to uninsured, underinsured and minority patients—report some of the lowest CRC screening rates. Effective CRC screening interventions within FQHCs may help to promote CRC screening utilization among patients from diverse backgrounds. Focus of Article This study was designed to explore average-risk, patient perceptions of facilitators and impediments of using the Fecal Immunochemical Test (FIT) kit for CRC screening within the social marketing framework of the 4 “P’s”—product, price, place, and promotion. Research Aim To conduct formative research with priority populations (i.e., diverse, FQHC patients), assess acceptability of the FIT kit, and identify ways to better meet preferences and needs. Program Design/Approach Qualitative, Focus Groups. Importance to the Social Marketing Field Findings from this study would aid in the future development of a community-based prevention marketing intervention to promote CRC screening within FQHCs that serve diverse patient populations. Methods Focus groups were conducted in English and Spanish with CRC screening age-eligible patients from a South Florida FQHC System. Interview guides were designed to better understand FQHC patient perceptions of CRC Screening via the FIT kit (product) by up-to-date and not up-to-date participants. Additional topics included likes/dislikes (price), preferred location for completing CRC screening and receiving CRC prevention education (place), and strategies for persuasive communication to increase CRC screening among patients who visit FQHCs (promotion). Data were analyzed to identify FIT screening facilitators and impediments using thematic coding in MaxQDA. Results Five focus groups, involving 36 participants aged 50 and older, were conducted. Over half of participants self-identified as Hispanic (36%) or non-Hispanic Black (28%). Key facilitators were the convenience and simplicity of at-home testing (product), presumed affordability of it compared to colonoscopy (price), and ability to return by mail (place). Participants also shared numerous strategies for promotion, including direct referral from Primary Care Providers, mass media campaigns, mailed or electronic reminders from the clinic, and peer advocacy events in churches. Recommendations for Research or Practice Social marketing provides an adequate approach for developing a patient-centered campaign to promote CRC screening uptake at FQHCs. Future CRC screening promotional campaigns at FQHCs should build on the motivation that patients must prioritize their health by emphasizing the convenience of completing CRC screening at home with the capability of mailing it back. Clinics should also work to reduce patient responsibility by developing automated clinic system reminders to openly communicate with patients about CRC screening.

There is no evidence that insurance-mandated weight loss before bariatric surgery affects outcomes.This retrospective study evaluated the relationship between insurance-mandated weight management program (WMP) completion before primary bariatric surgery and postoperative outcomes.Suburban academic medical center.Patients who underwent laparoscopic Roux-en-Y gastric bypass (LRYGB, n = 572) or sleeve gastrectomy (SG, n = 484) from 2014 to 2019 were dichotomized to presence (LRYGB n = 431, SG n = 348) or absence (LRYGB n = 141, SG n = 136) of insurance-mandated WMP completion. Primary endpoints included follow-up rate, percent total weight loss (%TWL), and percent excess weight loss (%EWL) through 60 months after surgery. The Mann-Whitney U test compared between-group means with significance at P.05.Follow-up rate, %TWL, and %EWL were not different (P = NS) up to 60 months postoperation between groups for either surgery. Both LRYGB and SG patients without WMP completion maintained greater %TWL (LRYGB: 34.4 ± 11.1% versus 29.8 ± 11.0%, P = .159; SG: 21.4 ± 10.0% versus 18.2 ± 10.5%, P = .456) and %EWL (LRYGB: 71.3 ± 26.3% versus 67.6 ± 26.5%, P = .618; SG: 49.2 ± 18.8% versus 47.5 ± 28.8%, P = .753) at 36 months after surgery. Secondarily, duration of time to get to surgery was significantly greater among yes-WMP patients (LRYGB: 178 days versus 121 days, P.001; SG: 169 days versus 95 days, P.001).Insurance-mandated WMP completion before bariatric surgery delays patient access to surgery without improving postoperative weight loss potential and must be abandoned.

13. Evaluating the accuracy of absorbing aerosol optical properties measured using single particle cavity ring-down spectroscopy

Single particle cavity ring-down spectroscopy (CRDS) allows direct and continuous measurements of the extinction cross-sections for levitated micron-scale aerosol particles of constant or evolving composition. Our recent interests have concerned single particle CRDS measurements for light-absorbing particles. These measurements are made on single, spherical particles evaporating over the radius range 1500–700 nm that are levitated within a 405-nm cavity ring-down spectrometer using a linear electrodynamic quadrupole trap. Here, we quantify the accuracy and precision of our cavity ring-down time measurements and the consequences of particle motion within our electrodynamic trap. Next, we estimate the uncertainty in our particle size retrievals from angularly resolved elastic light scattering measurements for particles with a range of absorption strengths. Finally, we assess the accuracy of complex refractive index values retrieved from simultaneous ring-down time and particle size measurements. The assessments account for the impacts of shot and detector noise in the measured ring-down times, the driven particle motion within the electrodynamic trap, and particle absorption strength (for imaginary refractive indices in the range ∼0.0–0.1). The real and imaginary refractive indices are retrieved to an accuracy better than 0.005 and 0.002, respectively, for almost all absorption strengths studied, and for particles of constant or evolving composition. Although particles confined in our electrodynamic quadrupole trap undergo considerable levels of driven motion (with oscillation amplitudes of several tens of micrometers), the contribution of this motion to uncertainty in retrieved refractive indices is negligible compared to contributions from particle sizing errors.

Obergefell v. Hodges, the 2015 Supreme Court decision establishing a constitutional right for same-sex couples to marry, marked the first time in the Court’s history that justices explicitly disagreed over the meaning and requirements of human dignity. In his dissenting opinion Clarence Thomas sought to reclaim rather than simply reject the language of dignity, advancing a conception of dignity that differed sharply from the conception embraced by the majority. Using this disagreement as a point of departure, this article demonstrates how dignity has served as an extra-textual value that underpins divergent visions of American constitutionalism that, in turn, inform interpretations of the Constitution’s text and history.

16. California coastal access damage from early winter 2022-2023 storms: A photo essay

Atmospheric rivers, “bomb cyclones,” storm waves and high tides wreaked havoc along the central California coast from San Francisco to Monterey. From 26 December 2022 to 11 January 2023, more than 10 inches of rain fell in central California (Landers 2023). During this same time period, higher high tides were 6-ft or greater for 10 of these days and 35-ft high waves were noted. Beaches, beach accesses, and coastal trails were on the front lines for storm defenses and damage. In addition to the extensive new coverage, the National Oceanic and Atmospheric Administration’s National Geodetic Survey (NOAA/NGS) collected continuous post-storm(s) airborne photography along the west coast that will be invaluable in analyzing future long-term shoreline changes (NOAA/NGS 2023). The following photos, taken soon after the storms, show some of the coastal damage near and in Monterey Bay.

This study aims to determine the effect of corporate governance and profitability on dividend policy based on substitution theory and/or outcome theory in the banking sector in Indonesia. Theoretically, this study contributes which theory is more dominant between substitution theory and outcome theory by linking the influence of corporate governance with dividend policy and providing information about the impact of the application of the theory on firm value and build awareness to be careful of investors. Investors in making investment decisions, especially in the banking industry in Indonesia. In addition, this study also examines the role of dividend policy in explaining the relationship between corporate governance and profitability on the value of banking firms. A sample of 135 observational data comes from 27 banking companies during 2016-2020. This research found that corporate governance has a negative and significant influence on dividend policy. In addition, the CG index is one of the factors that can explain the company's market value. Profitability is empirically proven to influence dividend policy. Dividend policy is proven to play a role in mediating the relationship between profitability and firm value. However, it is empirically proven that dividend policy cannot mediate the relationship between corporate governance and firm value.

Background: Treatment of estrogen receptor (ER)-positive breast cancer with selective estrogen receptor degraders (SERDs) frequently results in the loss or reduction of ER expression. Whether these changes are due to on-target effects of SERDs degrading ER or arise as a mechanism of tumor resistance with associated changes in cellular phenotypes remains unknown. It is critical to distinguish between these possibilities to accurately assess treatment response and determine the most appropriate subsequent therapy. To this end, we created and conducted molecular analyses on patient-derived organoid cultures from post-treatment tissue in patients receiving neoadjuvant SERD therapy for early-stage ER+ breast cancer in the I-SPY2 Endocrine Optimization Protocol (EOP). Methods: The I-SPY2 EOP study is a prospective, randomized substudy within the I-SPY TRIAL testing the oral SERD amcenestrant alone or in combination with letrozole or abemaciclib in stage 2/3 ER+ Her2-negative breast cancer. Enrollment is ongoing, with patients receiving amcenestrant neoadjuvantly for 6 months until the day before surgery. Tumor tissue is collected at baseline, 3 weeks, and at surgery. Organoids were generated from post-treatment surgical samples. Organoid cultures were optimized based on established methods (Dekkers et al., Nature Protocols, 2021) to assess ER levels and activity. Pre- and post-treatment tissue samples were also assessed for ER, PR, Ki67, and GATA3, a luminal marker and transcription factor that is functionally linked with ER, via immunohistochemistry. Results: In 7 patients with both pre- and post-treatment tissue samples including fresh surgical samples for organoid generation, the ER in baseline tumor tissue was >=90% in all patients, PR ranged from 40-90%, and Ki67 ranged from 5-30%. In post-treatment surgical tissue from these cases, ER ranged from 0-30%, PR from 0-50%, Ki67 from < 1%-10%, and GATA3 was positive in 5 of 5 cases tested to-date. The creation of organoids from residual disease at surgery was attempted for these 7 patients, with organoids successfully propagated in 5 cases thus far. 3 of 5 organoid cultures were ready for analysis and in all cases strong ER and PR expression in organoids was observed after culture for > 1 month in the absence of amcenestrant. Detailed gene expression profiling (including Mammaprint/Blueprint) and gene set enrichment analyses (GSEA) to assess for intrinsic breast cancer subtype and ER activity in each sample and corresponding organoid culture are in progress and will be reported with the full dataset. Conclusion: Patient-derived organoid culturing of residual disease after neoadjuvant endocrine therapy is feasible. Neoadjuvant treatment with a SERD can render ER and PR low or absent at the time of surgical resection, which does not necessarily imply the presence of endocrine therapy resistant disease. The use of organoids and additional IHC markers (GATA3) demonstrate that receptor negativity may be an indicator of the drug hitting its target, suggesting ER signaling is still intact. In general, patient-derived tumor organoid cultures modeling residual disease states can be a useful adjunct to existing methods used to monitor the effects of neoadjuvant endocrine therapy and is being explored in the I-SPY EOP trial. 20. A machine learning tool for identifying non-metastatic colorectal cancer in primary care

Background: Primary health care (PHC) is often the first point of contact when diagnosing colorectal cancer (CRC). Human limitations in processing large amounts of information warrant the use of machine learning as a diagnostic prediction tool for CRC. Aim: To develop a predictive model for identifying non-metastatic CRC (NMCRC) among PHC patients using diagnostic data analysed with machine learning. Design and setting: A case-control study containing data on PHC visits for 542 patients >18 years old diagnosed with NMCRC in the Vastra Gotaland Region, Sweden, during 2011, and 2,139 matched controls. Method: Stochastic gradient boosting (SGB) was used to construct a model for predicting the presence of NMCRC based on diagnostic codes from PHC consultations during the year before the date of cancer diagnosis and the total number of consultations. Variables with a normalised relative influence (NRI) >1% were considered having an important contribution to the model. Risks of having NMCRC were calculated using odds ratios of marginal effects. Results: Of the 361 variables used as predictors in the stochastic gradient boosting model, 184 had non-zero influence, with 16 variables having NRI >1% and a combined NRI of 63.3%. Variables representing anaemia and bleeding had a combined NRI of 27.6%. The model had a sensitivity of 73.3% and a specificity of 83.5%. Change in bowel habit had the highest odds ratios of marginal effects at 28.8. Conclusion: Machine learning is useful for identifying variables of importance for predicting NMCRC in PHC. Malignant diagnoses may be hidden behind benign symptoms such as haemorrhoids.

We present the results of a model-based search for continuous gravitational waves from the low-mass X-ray binary Scorpius X-1 using LIGO detector data from the third observing run of Advanced LIGO, Advanced Virgo and KAGRA. This is a semicoherent search which uses details of the signal model to coherently combine data separated by less than a specified coherence time, which can be adjusted to balance sensitivity with computing cost. The search covered a range of gravitational-wave frequencies from 25Hz to 1600Hz, as well as ranges in orbital speed, frequency and phase determined from observational constraints. No significant detection candidates were found, and upper limits were set as a function of frequency. The most stringent limits, between 100Hz and 200Hz, correspond to an amplitude h0 of about 1e-25 when marginalized isotropically over the unknown inclination angle of the neutron star's rotation axis, or less than 4e-26 assuming the optimal orientation. The sensitivity of this search is now probing amplitudes predicted by models of torque balance equilibrium. For the usual conservative model assuming accretion at the surface of the neutron star, our isotropically-marginalized upper limits are close to the predicted amplitude from about 70Hz to 100Hz; the limits assuming the neutron star spin is aligned with the most likely orbital angular momentum are below the conservative torque balance predictions from 40Hz to 200Hz. Assuming a broader range of accretion models, our direct limits on gravitational-wave amplitude delve into the relevant parameter space over a wide range of frequencies, to 500Hz or more., 19 pages, Open Access Journal PDF

Although humans are considered to be face experts, there is a well-established reliable variation in the degree to which neurotypical individuals are able to learn and recognise faces. While many behavioural studies have characterised these differences, studies that seek to relate the neuronal response to standardised behavioural measures of ability remain relatively scarce, particularly so for the time-resolved approaches and the early response to face stimuli. In the present study we make use of a relatively recent methodological advance, multi-variate pattern analysis (MVPA), to decode the time course of the neural response to faces compared to other object categories (inverted faces, objects). Importantly, for the first time, we directly relate metrics of this decoding assessed at the individual level to gold-standard measures of behavioural face processing ability assessed in an independent task. Thirty-nine participants completed the behavioural Cambridge Face Memory Test (CFMT), then viewed images of faces and houses (presented upright and inverted) while their neural activity was measured via electroencephalography. Significant decoding of both face orientation and face category were observed in all individual participants. Decoding of face orientation, a marker of more advanced face processing, was earlier and stronger in participants with higher levels of face expertise, while decoding of face category information was earlier but not stronger for individuals with greater face expertise. Taken together these results provide a marker of significant differences in the early neuronal response to faces from around 100 ms post stimulus as a function of behavioural expertise with faces. - Abstract
The research aims to explore the effect of financial reporting aggressiveness, tax reporting aggressiveness and corporate governance (CG). One manager is required to optimize the amount of profit earned from shareholders, one of which is by choosing various accounting policies that aim to produce financial reports with high profits. Tax planning activities by selecting accounting policies have a minimal impact on taxable income (taxable profit), as well as earnings management activities aimed at maximizing net income (accounting profit), both of which are reflected in the income statement. CG implementation is very important for a company, besides being able to reduce conflicts of interest between principals and agents, CG is an obligation that must be carried out by companies, so that the company's credibility in the public eye can be increased. Corporate governance can also limit the space for management to take aggressive tax actions and aggressive financial reporting. This study uses a quantitative method and the data used is secondary data with a sample of manufacturing companies listed on the IDX for the 2018-2020 period which was sourced on the official website of the Indonesia Stock Exchange. The results showed that the aggressiveness of financial reporting has a positive effect on the aggressiveness of tax reporting. Tax reporting aggressiveness has a positive effect on financial reporting aggressiveness, CG has no effect on financial reporting aggressiveness. 31. Are You Still Using 6-Volt Batteries for Your Insect Traps?

The most prevalent insect sampling and surveillance problem is powering insect traps in the field. Most modern light traps use 6-V power supplies such as the Centers for Disease Control and Prevention (CDC) suction trap. Buck converter modules efficiently reduce 12-V direct current power to 6-V, which permits the use of higher voltage batteries with lower voltage traps, resulting in longer operational duration and reduced labor requirements associated with replacing and recharging batteries in the field. We evaluated several battery configurations of 6- and 12-V lead-acid batteries in various sizes (10–20 ampere-hours) and addressed, in the circuit design, common problems that occur when using the buck converter (such as crossing polarity and excessive battery depletion). The efficacy of each configuration was assessed by measuring the voltage and suction while powering a 6-V CDC light trap. The buck converter permitted the use of cheaper and more commonly available 12-V batteries to run the CDC light traps and resulted in longer effective operation time as measured by air speed.

This paper describes multidimensional modeling of ablation heat transfer with a focus on ablative insulation materials used in rocket motor nozzles. Phenomenology is discussed, and detailed three-dimensional math models are presented along with a finite element solution strategy. Implementation into a computer program named Hero is described, including a description of the parallel processing capability that enables computational support for large finite element models. A validation case is presented using measurement data from booster motors of the space shuttle program. The validation results are quantified in terms of the probabilistic propensity for misprediction of the char depth at varying levels. The validation results show a trend toward the overprediction of char depth by around 4%. The standard deviation of percent misprediction is shown to be between 6 and 16%, depending on boundary condition fidelity. This suggests that accuracy to three standard deviations can be expected to fall in the range of 18–48% for ablation modeling in rocket nozzles. This paper provides an important reference for multidimensional ablation heat transfer analysis and quantification of related modeling uncertainty.

This paper discusses the design of a net-zero single-family house in Chattanooga, Tennessee, which was proposed for the 2022 Solar Decathlon Design Challenge entry. The site was in a 100-year flood zone and registered as a national and local historic district. The interdisciplinary team consisted of University of Tennessee Chattanooga (UTC) students from the interior architecture and engineering departments who embraced the challenges of the local climate and the restrictions of the historic district guidelines. This entry, Moc House, focused on the integration of passive design and active HVAC systems including photovoltaic systems in the process. In responding to recent housing market changes in Chattanooga due to the COVID-19 pandemic, a young professional couple in their 30s who have two children was identified as the target market which guided the space planning. Market analysis, including ROI, was conducted in addition to construction cost calculations. Building materials were selected to enhance the durability and resilience of the house structure. Finally, energy analysis was conducted to check the project’s Home Energy Rating using Ekotrope and found it less than zero.

Background/Context: Research suggests that sexual harassment and assault are distressingly common occurrences in middle school settings. However, prevention efforts have largely focused on secondary and post-secondary settings. While research-based initiatives to discuss consent could be effective, currently there is a dearth of literature on middle school students’ beliefs or attitudes on consent and sexual assault, which could inform such initiatives. Purpose/Objective/Research Question/Focus of Study: Using the concept of “consent scripts,” this study asks: How do seventh- and eighth-grade students define, proscribe, understand, and operationalize consent as a concept? Research Design: We surveyed 177 middle school students about their ideas regarding consent and their analysis of scenarios of dubious consent, and conducted follow-up in-depth semi-structured interviews with 66 of the participants. Conclusions/Recommendations: We identify four consent scripts prevalent among middle school students 1) consent works differently within relationships; 2) seeking consent is a form of empathy; 3) acts of seeking or violating consent are informed by norms of masculinity; and 4) close friends will respect norms of consent. These findings can inform sexual assault prevention and intervention efforts in the middle school context.

41. Critical metal resources in Democratic People’s Republic of Korea

Critical metals (CMs), including rare-earth elements (REEs), lithium (Li), and beryllium (Be), provide the foundation for the development of advanced technologies and are essential to national security and economic development in many countries. While China has most of the world’s REE processing capacity, the Democratic People’s Republic of Korea (DPRK) reportedly possesses significant untapped CM resources. With the size and total value estimated to be worth trillions of dollars, its large resources are reputedly the DPRK’s most significant internal asset. Despite the oft-cited abundance of CMs, evidence-based analyses of the mineral resources are limited. We provide a comprehensive analysis of CM resources in the DPRK based on geological analysis of potential deposits. Three main REE deposits at Ryongpo, Cheolsan, Abdong, and various Li and Be deposits are investigated. Extensive analyses indicate that previous estimates of a large CM resource base for the DPRK are exaggerated. There is no clear evidence for significant resources that are economical. While there remains a possibility of identifying new ore deposits, for the DPRK to access its CM resources, it will need to develop a nationwide exploration and exploitation strategy to attract foreign investments. A strategy of investment and investigation might provide a springboard for cooperation between the DPRK and other nations as these resources are discovered and developed.

The purpose of our study was to evaluate the use of minimally invasive Marshmallow Swallow Study (MSS) as a preoperative screening technique for patients and correlate marshmallow results with High Resolution Manometry (HRM) results and post-operative dysphagia following antireflux surgery.A retrospective electronic chart review of data on 61 adult patients undergoing MSS was completed. Patients were included if they completed the MSS as well as: HRM and/or antireflux surgery.Sixty-one patients completed MSS. Of the 37 patients that completed HRM, 27 had abnormal results. 23 of these 27 patients who had abnormal results also failed MSS. The NPV for a failure on the MSS and abnormal HRM results is 0.767(p-value = 0.360). The PPV for a pass on the MSS and no post-operative dysphagia is 0.833 (p-value = 0.656).MSS is an effective screening tool for ruling out major esophageal motility disorders and can prevent need for invasive HRM.

Studies of institutional change identify critical junctures from the position of hindsight. But this perspective prioritises antecedent factors that downplay the role of agency around moments of potential change. This article looks at changes in the ways in which agents use temporal language to identify events or periods as moments of (possible) juncture. It combines quantitative and qualitative methods of text analysis, drawing on a corpus of British parliamentary speeches from 1811–2019. The article first analyses changes in the strategic use of the term crisis over time, paying particular attention to significant shifts in its politicisation and temporalisation. It then identifies three distinct components to the contestation of crises: over their identification, evaluation and proposed prescriptions. We suggest that studies of critical juncture ought to focus more on the use of temporal language around possible junctures to better understand the political dynamics at moments of heightened uncertainty.

45. Comparison Analysis of EVA and ROA Methods in Assessing The Financial Performance of The Company

- Abstract
This study aims to determine the company's financial performance by using a comparative analysis of the Economic Value Added (EVA) and Return On Assets (ROA) methods. This study employed pre-field research techniques and on-site fieldwork to conduct descriptive qualitative research. Secondary data, data gathering methods utilizing report studies and literature analyses, and data analysis methods utilizing data collection, data reduction, data presentation, and data inference techniques are all used as the data sources. To verify the accuracy of the data, the author uses three sources and three different techniques. The results showed that EVA INDF in the 2014-2016 period produced an EVA value < 0, which could be interpreted as unfavorable, ICBP, MYOR, and ROTI in the 2014-2018 period, on average, produced an EVA value > 0 which could be construed as positive. Except for ROTI in the last three years which resulted in an EVA < 0. Meanwhile, ROA INDF, ICBP, MYOR, and ROTI in 2014-2018 produced an average ROA value above the industry standard or can be said to be good. Except for INDF in 2015, 2017, and 2018, the average ROA value is below industry standards. This study concludes that EVA analysis is better because if the average value of EVA is> 0, then the company can provide added value to investors. At the same time, the ROA analysis produces an average ROA value below the industry standard.

Computational hydrological models and simulations are fundamental pieces of the workflow of contemporary hydroscience research, education, and professional engineering activities. In support of hydrological modelling efforts, web-enabled tools for data processing, storage, computation, and visualization have proliferated. Most of these efforts rely on server resources for computation and data tasks and client-side resources for visualization. However, continued advancements of in-browser, client-side compute performance present an opportunity to further leverage client-side resources. Towards this end, we present an operational rainfall-runoff model and simulation engine running entirely on the client side using the JavaScript programming language. To demonstrate potential uses, we also present an easy-to-use in-browser interface designed for hydroscience education. Although the use case presented here is self-contained, the core technologies can extend to leverage multi-core processing on single machines and parallelization capabilities of multiple clients or JavaScript-enabled servers. These possibilities suggest that client-side hydrological simulation can play a central role in a dynamic, interconnected ecosystem of web-ready hydrological tools.

There has been a global surge in adolescents' use of electronic nicotine delivery systems (vaping), cannabis (vaped and edible), and prescription opioids, collectively termed ECPO. The nature of ECPO use can make it difficult to detect due to few obvious immediate physical and behavioural signs, as well as subtle long-term effects that allow adolescents to transition from initial exploration into hazardous ECPO use without easy detection by care providers. Here, we address the nature of the presentation of ECPO use in adolescents (roughly age 13-18 years), including challenges in detecting use and related complications, which affect screening, prevention, and intervention. We begin by reviewing empirical data on these difficult to detect effects in adolescents, including acute effects at cellular and neural levels and long-term neurocognitive and developmental changes that precede outwardly detectable physical signs. We then provide concrete approaches for providers to screen for ECPO use in adolescents even in the absence of overt physical and behavioural symptoms. Finally, we conclude with direct practice recommendations for prevention and intervention.

Here, we present a novel case of a patient with chronic lymphocytic leukemia (CLL) who received CTLA-4 and then PD-1 immune-checkpoint blockade (ICB) as treatment for concomitant metastatic melanoma. Whereas the metastatic melanoma was responsive to ICB, the CLL rapidly progressed (but responded to ICB cessation and ibrutinib). There were no new genetic mutational drivers to explain the altered clinical course. PD-1/PD-L1/PD-L2 and CTLA-4/CD80/CD86 expression was not increased in CLL B cells, CD8+ or CD4+ T-cell subsets, or monocytes. The patient’s CLL B cells demonstrated strikingly prolonged in vitro survival during PD-1 blockade, which was not observed in samples taken before or after ICB, or with other patients. To our knowledge, a discordant clinical course to ICB coupled with these biological features has not been reported in a patient with dual malignancies.
