The diagnosis of functional neurological disorders is a major challenge in neurologist practice. Some clinical strategies can facilitate the recognition of functional disorders, but several pitfalls make their diagnosis difficult. Here we highlight the following points of attention during evaluation of patients with functional disorder: not all bizarre behavior is functional; not every event triggered by an emotional factor is a functional disorder; not every topographic incongruity is a functional disorder; patients may present functional and organic symptoms at the same time; psychiatric comorbid condition is not always evident in the history of a functional disorder; problematic communication at the time of diagnosis can compromise treatment and prognosis. In conclusion, we emphasize that special attention to these possible pitfalls facilitate the correct diagnosis and management of functional neurological disorders. RESUMO O diagnóstico dos transtornos neurológicos funcionais é um grande desafio na prática do neurologista. Algumas estratégias clínicas podem facilitar o reconhecimento de transtornos funcionais, porém várias armadilhas dificultam o seu diagnóstico. Destacamos aqui os seguintes pontos de atenção durante a avaliação de pacientes com transtorno funcional: nem todo comportamento bizarro é funcional; nem todo evento desencadeado por um fator emocional é um transtorno funcional; nem toda incongruência topográfica é um transtorno funcional; pacientes podem apresentar ao mesmo tempo sintomas funcionais e orgânicos; condição psiquiátrica comórbida nem sempre é evidente na história de um transtorno funcional; comunicação problemática no momento do diagnóstico pode comprometer o tratamento e prognóstico. Em conclusão, enfatizamos que atenção especial a essas possíveis armadilhas, facilitam o diagnóstico e manejo corretos dos transtornos neurológicos funcionais.

Resumo O raciocínio clínico nasceu 2500 anos atrás com Hipócrates, tendo evoluído ao longo dos séculos, e se tornado uma mistura de arte e ciência. Várias personalidades ao longo da história contribuíram para melhorar a acurácia diagnóstica. Contudo, o erro diagnóstico é ainda comum e causa um grande impacto nos sistemas de saúde. Para lidar com esse desafio, vários modelos de raciocínio clínico surgiram para sistematizar o processo de pensamento clínico. Este artigo descreve a história do raciocínio clínico e os métodos atuais de raciocínio diagnóstico, propõe um novo modelo de raciocínio clínico chamado Raciocínio Integrativo, e traz perspectivas sobre o futuro do raciocínio clínico. Abstract Clinical reasoning was born 2,500 years ago with Hippocrates, having evolved over the centuries, becoming a mixture of art and science. Several personalities throughout history have contributed to improving diagnostic accuracy. Nonetheless, diagnostic error is still common and causes a severe impact on healthcare systems. To face this challenge, several clinical reasoning models have emerged to systematize the clinical thinking process. This paper describes the history of clinical reasoning and current diagnostic reasoning methods, proposes a new clinical reasoning model, called Integrative Reasoning, and brings perspectives about the future of clinical reasoning.

SUMMARY OBJECTIVE This study aims to investigate the causes of misdiagnosis in assessing tubal patency by transvaginal real-time three-dimensional hysterosalpingo-contrast sonography (TVS RT-3D-HyCoSy), in order to improve the diagnostic efficiency of TVS RT-3D-HyCoSy. METHODS A total of 162 oviducts of 83 infertility patients were examined by TVS RT-3D-HyCoSy. These results were compared with the gold standard for laparoscopic dye studies, and the misdiagnosed cases were analyzed. RESULTS TVS RT-3D-HyCoSy revealed that 68 oviducts were unobstructed and 94 obstructed. The results for the 144 oviducts were in line with the gold standard, while those for 18 oviducts were not. The accuracy rate of the TVS RT-3D-HyCoSy was 88.9%, and the misdiagnosis rate was 11.1%. The main causes of misdiagnosis included contrast medium countercurrent and diffusion, oviduct spasm, abnormal shape or position of the oviduct, pelvic adhesion, and poor imaging operation. CONCLUSION TVS RT-3D-HyCoSy can well-evaluate tubal patency, and understand and improve the cause of misdiagnosis. Furthermore, the diagnostic efficiency of TVS RT-3D-HyCoSy can still be further improved. RESUMO OBJETIVO Este estudo tem como objetivo investigar as causas do diagnóstico equivocado na avaliação da patência tubária por meio da ultrassonografia de contraste histerosalpingo em tempo real transvaginal (TVS RT-3D-HyCoSy), a fim de melhorar a eficiência diagnóstica das TVS RT-3D-HyCoSy. MÉTODOS Um total de 162 ovidutos em 83 pacientes da infertilidade foi examinado por TVS RT-3D-HyCoSy. Esses resultados foram comparados com o padrão ouro para estudos de tintura laparoscópica, e os casos diagnosticados erroneamente foram analisados. RESULTADOS TVS RT-3D-HyCoSy revelaram que 68 ovidutos foram desobstruídos e 94 ovidutos foram obstruídos. Os resultados para os 144 ovidutos estavam em consonância com o padrão ouro, enquanto que aqueles para os 18 ovidutos, não. A taxa de acurácia do TVS RT-3D-HyCoSy foi de 88,9%, e a taxa de erro de diagnóstico foi de 11,1%. As principais causas de erro de diagnóstico incluíram contraponto e difusão do meio de contraste, espasmo do oviduto, forma ou posição anormal do oviduto, adesão pélvica e má operação de imagem. CONCLUSÃO TVS RT-3D-HyCoSy pode bem avaliar a patência tubária, e compreender e melhorar a causa do erro de diagnóstico. Além disso, a eficiência diagnóstica do TVS RT-3D-HyCoSy ainda pode ser melhorada.

Sensory neuronopathies (SN) are a group of peripheral nerve disorders characterized by multifocal non-length-dependent sensory deficits and sensory ataxia. Its recognition is essential not only for proper management but also to guide the etiological investigation. The uncommon SN clinical picture and its rarity set the conditions for the misdiagnosis and the diagnostic delay, especially in non-paraneoplastic SN. Therefore, our objectives were to characterize the diagnostic odyssey for non-paraneoplastic SN patients, as well as to identify possible associated factors. Methods We consecutively enrolled 48 non-paraneoplastic SN patients followed in a tertiary neuromuscular clinic at the University of Campinas (Brazil). All patients were instructed to retrieve their previous medical records, and we collected the data regarding demographics, disease onset, previous incorrect diagnoses made and the recommended treatments. Results There were 34 women, with a mean age at the diagnosis of 45.9 ± 12.2 years, and 28/48 (58%) of the patients were idiopathic. Negative sensory symptoms were the heralding symptoms in 25/48 (52%); these were asymmetric in 36/48 (75%) and followed a chronic course in 35/48 (73%). On average, it took 5.4 ± 5.3 years for SN to be diagnosed; patients had an average of 3.4 ± 1.5 incorrect diagnoses. A disease onset before the age of 40 was associated to shorter diagnosis delay (3.7 ± 3.4 vs. 7.8 ± 6.7 years, p = 0.01). Conclusions These results suggest that diagnostic delay and misdiagnosis are frequent in non-paraneoplastic SN patients. As in other rare conditions, increased awareness in all the healthcare system levels is paramount to ensure accurate diagnosis and to improve care of these patients. RESUMO As neuronopatias sensitivas (NS) representam um grupo de doenças caracterizadas por ataxia sensitiva e déficits sensitivos multifocais e não-comprimento dependentes. O seu reconhecimento é fundamental para o tratamento apropriado e para a investigação de doenças associadas. O quadro clínico pouco frequente aliado à baixa prevalência, especialmente das formas não-paraneoplásicas (NSnp), colaboram para o atraso e erro no diagnóstico. Os objetivos desse trabalho são descrever a odisseia diagnóstica dos pacientes com NSnp e tentar identificar possíveis fatores associados. Métodos Foram incluídos consecutivamente 48 pacientes com NSnp acompanhados no ambulatório de doenças neuromusculares da Universidade Estadual de Campinas (Brasil). Dados demográficos e sobre o início da NS (incluindo diagnósticos que lhes foram dados e tratamentos prescritos) foram coletados. Resultados Na coorte descrita havia 34 mulheres e a idade ao diagnóstico era de 45,9 ± 12,2 anos. Os sintomas inaugurais eram sensitivos deficitários em 25/48 (52%) dos pacientes, sendo assimétricos em 36/48 (75%) e de evolução crônica em 35/48 (73%). Para 28/48 (58%) dos pacientes a NS era idiopática. Em média, os pacientes com NSnp tiveram um atraso diagnóstico de 5,4 ± 5,3 anos com uma média de 3,4 ± 1,5 diagnósticos incorretos. Pacientes com início antes dos 40 anos tiveram diagnóstico mais precoce que aqueles com início tardio (3,7 ± 3,4 vs. 7,8 ± 6,7 anos, p = 0,01). Conclusão Os dados ora apresentados sugerem que o erro e o atraso diagnóstico são frequentes e impactam os pacientes com NS. A importância do diagnóstico das NS deve ser constante em todos os níveis do sistema de saúde para o diagnóstico correto e a consequente melhora no cuidado a esses pacientes.

Esse trabalho utilizou como referencial Teórico-metodológico aportes da fenomenologia de Alfred Schutz, a fim de compreender os critérios de relevância mobilizados pelo sujeito social no curso de sua ação. Objective: Although autopsy is deemed the gold standard for diagnosis, its performance has been decreasing while adverse events have been increasing, of which 17% consist in diagnostic errors. The purpose of this study was to estimate the prevalence of diagnostic errors based on anatomopathological diagnosis in a Pediatric Intensive Care Unit (PICU). Methods: This is a cross-sectional, retrospective study on 31patients who died between 2004 and 2014. Diagnoses were compared in order to assess whether there was agreement between clinical major diagnosis (CMD) and the cause of death as described in the autopsy record (CDAR), which were classified according to the Goldman Criteria. Results: Of 3,117 patients, 263 died (8.4%). Autopsy was conducted in 38 cases (14.4%), and 31 were included in the study. Therewas a 67% decrease in the number of autopsies over the last 10years. Absolute agreement between the diagnoses (class V) was observed in 18 cases (58.0%), and disagreement (class I), in 11 (35.4%). Therewas greater difficulty in diagnosing acute diseases and diseases of rapid fatal evolution such as myocarditis. Sevenpatients were admitted in critical health conditions and died within the first 24 hours of hospitalization. Conclusions: Autopsy not only enables to identify diagnostic errors, but also provides the opportunity to learn from mistakes. The results emphasize the relevance of the autopsy examination for diagnostic elucidation and the creation of an information database concerning the main diagnoses of patients who rapidly progress to death in PICU, increasing the index of clinical suspicion of the team working at this unit.

Vanishing bone metastasis (pseudopathological vertebral body enhancement) is a pitfall in the interpretation of contrast-enhanced computed tomography (CT) scans of patients with thoracic vein obstruction, mainly in the superior vena cava and brachiocephalic veins, typically being related to thrombosis due to malignant tumors. On the basis of the CT findings, pseudopathological vertebral body enhancement can be misdiagnosed as sclerotic bone metastasis, leading to unnecessary treatment. Although not rare, pseudopathological vertebral body enhancement is usually underdiagnosed by radiologists. The aim of this study is to review the pathophysiology of this phenomenon, illustrating the most common collateral venous pathways in thoracic vein obstruction and making the correlation with the CT findings.

To present a cognitive map to support the radiological diagnosis of solitary bone tumors, as well as to facilitate the determination of the nature of the tumor (benign or malignant), in pediatric patients.We selected 28 primary lesions in pediatric patients, and we identified the findings typically associated with each of the diagnoses. The method used for the construction of the final cognitive map was the Bayesian belief network model with backward chaining.We developed a logical, sequential structure, in the form of a cognitive map, based on the Bayesian belief network model, with the intention of simulating the sequence of human thinking, in order to minimize the number of unnecessary interventions and iatrogenic complications arising from the incorrect evaluation of bone lesions.With this map, it will be possible to develop an application that will provide support to physicians and residents, as well as contributing to training in this area and consequently to a reduction in diagnostic errors in patients with bone lesions.Apresentar um mapa cognitivo para suporte na determinação da natureza benigna ou maligna, bem como no diagnóstico radiológico, de tumores ósseos solitários na faixa etária pediátrica.Foi realizada uma seleção de 28 lesões principais na faixa etária pediátrica e dos achados característicos e associados a cada um desses diagnósticos. O método utilizado para a construção da estrutura cognitiva final foi o modelo de rede de crenças bayesianas com a técnica de encadeamento regressivo.Foi desenvolvida uma estrutura lógica e sequencial no formato de um mapa cognitivo, segundo o modelo de rede de crenças bayesianas, na tentativa de simular o raciocínio humano sequencial e minimizar esforços e iatrogenias oriundas da avaliação equivocada de lesões ósseas.Com base nesse mapa, será possível oferecer um aplicativo para apoio a profissionais e residentes médicos e colaborar com o treinamento nessa área e, consequentemente, diminuir erros no diagnóstico de uma lesão óssea.

OBJETIVO: Descrever os fatores relacionados ao falso diagnóstico de restrição de crescimento fetal (RCF). MÉTODOS: Foram incluídas 48 gestantes encaminhadas ao nosso serviço com suspeita de RCF, não confirmada após o nascimento. Estas foram comparadas ao grupo de gestantes com RCF confirmada e foram descritas características relacionadas a esses falso-positivos. Os dados foram analisados utilizando-se o programa Statplus para Mac(r), versão 5.8. Os resultados obtidos no estudo foram divididos em variáveis categóricas e contínuas para análise. OBJETIVOS: determinar os motivos de encaminhamento e diagnósticos de crianças com anemia para serviço especializado. MÉTODOS: coorte histórico e concorrente de crianças com anemia encaminhadas ao Serviço de Hematologia do Hospital das Clínicas da Universidade Federal de Minas Gerais, Belo Horizonte, Minas Gerais, Brasil, entre maio de 1999 e dezembro de 2001. RESULTADOS: foram avaliadas 153 crianças menores de 16 anos, sendo 83% procedentes de Centros de Saúde. Aproximadamente um terço das crianças não recebeu qualquer terapêutica no serviço de origem. Cento e onze (71,3%) tiveram diagnóstico de anemia ferropriva. Treze crianças (8,6%) tiveram diagnóstico de talassemia minor; outras treze crianças apresentaram outros diagnósticos de anemias e 11,5% das crianças não apresentavam anemia. Os valores médios de hemoglobina na abordagem pelo hematologista foram significativamente maiores que os dos serviços de origem (9,7±1,9 vs. 8,9±2,0); p=0,001. Somente 16,2% crianças não responderam ao primeiro tratamento com sais de ferro oral, sendo o sulfato ferroso o principal medicamento prescrito (79,3%). CONCLUSÕES: A deficiência de ferro foi a principal etiologia das crianças encaminhadas por motivo de anemia para serviço especializado. Encaminhamentos desnecessários de indivíduos com anemia ferropriva para serviços especializados representam prejuízo aos pacientes e ônus excedente para o sistema de saúde. OBJECTIVES: to determine the reasons for diagnosis of anemia among children and referral to specialized services. METHODS: an historical and concurrent cohort study was carried out among anemic children referred to the Hematology Service of the Federal University of Minas Gerais's Clinical Hospital, Belo Horizonte, in the State of Minas Gerais, Brazil, between May 1999 and December 2001. RESULTS: 153 children aged under 16 were evalua-ted, 83% of whom had been referred by Health Centers. Approximately one third of the children did not receive any treatment at the referring service. One hundred and eleven (71.3%) were diagnosed as having iron-deficiency anemia. Thirteen children (8.6%) received a diagnosis of thalassemia minor; and a further thirteen other anemia-related diagnoses. 11.5% had no kind of anemia. The mean hemoglobin levels were significantly higher when tested by a hematologist than at the referring services (9.7±1.9 vs. 8.9±2.0); p=0.001. Only 16.2% of children did not respond to initial treatment with oral iron salts, with iron (II) sulfate being the medication usually prescribed (79.3%). CONCLUSIONS: iron deficiency was the main cause of children being referred to a specialized service for reason of anemia. Unnecessary referral of individuals with iron-deficiency anemia is potentially harmful to patients and puts an excessive burden on the health system.

Para refletir sobre essas questões, apresento o acompanhamento longitudinal de uma criança que frequenta o Centro de Convivência de Linguagens, o CCazinho, localizado nas dependências do Instituto de Estudos da Linguagem IEL/UNICAMP. O acompanhamento de LP, sujeito desta pesquisa, ocorreu entre agosto de 2009 e novembro de 2013. LP foi encaminhado ao CCazinho porque sua professora suspeitava de que ele tivesse Distúrbio de Aprendizagem, uma vez que apresentava problemas para ler e escrever. Ao longo do acompanhamento orientado pelas perspectivas de cérebro, sujeito e linguagem construídas pela Neurolinguística Discursiva, vimos que esta suspeita não se confirma, já que LP conseguiu superar suas dificuldades escolares e desconstruir o estigma de criança com problemas de aprendizado que passou a carregar. Desse modo, esta pesquisa busca, a partir da apresentação da história de LP, debater sobre o excesso de diagnósticos relacionados ao aprendizado que assola as crianças, sobretudo as de escola pública, e analisar o efeito que tais diagnósticos podem ter na vida delas Abstract: This research aims to debate topics related to reading, writing and learning process that are currently seen by professionals from the clinical area as symptoms of learning-related pathology. To debate these matters, I present the longitudinal follow-up of a child attending the Center of languages, the CCazinho, located at the Institute for the Study of Language IEL / Unicamp. The follow-up of LP, subject of this research hapened between August of 2009 and November of 2013. LP was sento to CCazinho because his teacher suspected that he had a learning disorder, since he had trouble reading and writing. Throughout the monitoring guided by the discoursive perspectives of brain, subject and language constructed by Neurolinguistic, we saw that this suspicion was not confirmed, since LP has managed to overcome his learning difficulties and deconstruct the stigma of a child with learning problems that he started to live with from the moment his teacher suspected that he had a disease.
