How does the market affect and redefine healthcare? The marketisation of Western healthcare systems has now proceeded well into its fourth decade. But the nature and meaning of the phenomenon has become increasingly opaque amidst changing discourses, policies and institutional structures. Moreover, ethics has become focussed on dealing with individual, clinical decisions and neglectful of the political economy which shapes healthcare. This interdisciplinary volume approaches marketisation by exploring the debates underlying the contemporary situation and by introducing reconstructive and reparative discourses. The first part explores contrary interpretations of ‘marketisation’ on a systemic level, with a view to organisational-ethical formation and the role of healthcare ethics. The second part presents the marketisation of healthcare at the level of policy-making, discusses the ethical ramifications of specific marketisation measures and considers the possibility of reconciling market forces with a covenantal understanding of healthcare. The final part examines healthcare workers’ and ethicists’ personal moral standing in a marketised healthcare system, with a view to preserving and enriching virtue, empathy and compassion.

The "small world phenomenon," popularized by Stanley Milgram, suggests that individuals from across a social network are connected via a short path of mutual friends and can leverage their local social information to efficiently traverse that network. Existing social search experiments are plagued by high rates of attrition, which prohibit comprehensive study of social search. We investigate this by conducting a small world experiment at Burning Man, an event located in the Black Rock Desert of Nevada, USA, known its unique social systems and community participation. We design location-tracking vessels that we routed through Burning Man towards the goal of finding a particular person. Along the way, the vessels logged individual information and GPS data. Two of fifteen vessels made it to their designated people, but a month after Burning Man. Our results suggest possible improvements to limit rates of attrition through community participation and a design methodology that emphasizes cultural practices to aid social experimentation., Comment: 13 pages, 5 figures

Aims: To gain insight in the prognosis and treatment of atrial fibrillation (AF) patients during 1-year follow-up in the Euro Heart Survey (EHS) on AF. Methods and results: The EHS enrolled 5333 AF patients in 2003-2004. One-year follow-up data were available for 80%. Of first detected AF patients, 46% did not have a recurrence during 1 year, paroxysmal AF largely remained paroxysmal AF (80%), and 30% of persistent AF progressed to permanent AF. Many treatment changes occurred since baseline. Oral anticoagulation was started in 19% and discontinued in 16% of all patients. Of patients initially on rhythm control 27% did not receive rhythm control during follow-up, whereas 15% of patients initially on rate control received rhythm control. Mortality was highest in permanent AF (8.2%), but also substantial in first detected AF (5.7%). In multivariable analysis, sinus rhythm at baseline was associated with lower mortality, but no significant effect was observed regarding the application of either rhythm or rate control. Conclusion: The EHS on AF provides unique prospective observational data on AF progression, long-term treatment, prognosis, and determinants of adverse outcome of the total clinical spectrum of AF in a European cardiology-based patient cohort. © The Author 2008.

Aims To describe guideline adherence and application of different stroke risk stratification schemes regarding antithrombotic therapy in real-life atrial fibrillation (AF) patients and to assess which factors influence antithrombotic management decisions. Methods and results The Euro Heart Survey enrolled 5333 AF patients in 35 countries, in 2003 and 2004. Prescription of antithrombotic drugs, especially oral anticoagulation (OAC), was hardly tailored to the patient's stroke risk profile as indicated by the joint guidelines of the American College of Cardiology, American Heart Association, and the European Society of Cardiology, ACCP guidelines, or CHADS2 and Framingham risk scores. In multivariable analysis, only a limited number of the well-known stroke risk factors triggered OAC prescription. In contrast, less relevant factors, of which clinical type of AF and availability of an OAC monitoring outpatient clinic were the most marked, played a significant role in OAC prescription. Electrical cardioversions and catheter ablations clearly triggered OAC prescription, whereas pharmacological cardioversions even in the presence of stroke risk factors did not. Conclusion Antithrombotic therapy in AF is hardly tailored to the patient's stroke risk profile. Factors other than well-known stroke risk factors were significantly involved in antithrombotic management decisions. To facilitate this tailored treatment, guideline writers and physician educators should focus on providing one uniform and easy to use stroke risk stratification scheme.

Self-diffusion, or the flux of mass of a single species within itself, is viewed as an independent phenomenon amenable to treatment by the introduction of an auxiliary field of diffusion velocities. The theory is shown to be heuristically derivable as a limiting case of that of an ordinary binary mixture.

Aims: Although diabetes is known to be a major contributor to cardiovascular diseases, as well as an independent predictor for adverse outcomes in patients with coronary artery disease (CAD), information on the prognosis of patients with CAD and newly diagnosed diabetes or impaired glucose regulation (IGR) is scarce. The objective of this study was to explore 1-year outcome in relation to different glucometabolic states of patients participating in the Euro Heart Survey on diabetes and the heart. Methods and results: In 4676 out of 4961 patients, information on the relation between 1-year outcome and glucometabolic state, which was based on oral glucose tolerance test (OGTT) or fasting glucose plasma, was available. A normal glucose metabolism was identified in 947 patients, IGR (impaired fasting glucose or impaired glucose tolerance) in 1116 patients, and diabetes in 1877 patients of whom 1425 were previously diagnosed and 452 newly diagnosed. In total, 736 patients could not be classified, as no OGTT or fasting plasma glucose was performed. Previously recognized and newly detected diabetes was associated with an increased risk of 1-year mortality when compared with patients with normal glucose regulation [hazard ratio (HR) 2.4, 95% confidence interval (CI) 1.5-3.8 and HR 2.0, 95% CI 1.1-3.6, respectively)]. IGR, however, could not be identified as an independent predictor for 1-year mortality (HR 1.1, 95% CI 0.6-1.9). Conclusion: This study confirmed that patients with CAD and known diabetes are at high risk for mortality and cardiovascular events and demonstrated that patients with newly diagnosed diabetes are at intermediate risk for adverse outcomes. IGR, however, could not be identified as an independent predictor for adverse outcomes during the 1-year follow-up period. © The European Society of Cardiology 2006. All rights reserved.

Objectives: This study sought to investigate gender-related differences in patients with atrial fibrillation (AF) in Europe. Background: Gender-related differences may play a significant role in AF. Methods: We analyzed the data of 5,333 patients (42% female) enrolled in the Euro Heart Survey on Atrial Fibrillation. Results: Compared with men, the women were older, had a lower quality of life (QoL), had more comorbidities, more often had heart failure (HF) with preserved left ventricular systolic function (18% vs. 7%, p < 0.001), and less often had HF with systolic dysfunction (17% vs. 26%, p < 0.001). Among patients with typical AF symptoms (56% of women, 49% of men), there was no gender-related difference in the choice of rate or rhythm control. Among patients with atypical or no symptoms (44% of women, 51% of men), women less frequently underwent rhythm control (39% vs. 51%, p < 0.001) than did men. Women underwent less electrical cardioversion (22% vs. 28%, p < 0.001). Prescription of oral anticoagulants was identical (65%) in both genders. One-year outcome was similar except that women had a higher chance for stroke (odds ratio 1.83 in multivariable regression analysis, p = 0.019). Conclusions: Women with AF had more comorbidities, more HF with preserved systolic function, and a lower QoL than men. In the large group with atypical or no symptoms, women were treated appropriately more conservatively with less rhythm control than men. Women had a higher chance for stroke. Long-term QoL changes and other morbidities and mortality were similar. © 2007 American College of Cardiology Foundation.

Hypoplastic left heart syndrome continues to be one of the most complicated and devastating of all the congenital heart defects. Despite advances in surgical palliation, patients continue to face a high percentage of morbidity and mortality. This syndrome can be associated with multiple other genetic conditions although we believe we have found the first case that is associated with a congenitally prolonged QTc. We will discuss this presentation and the importance of considering multiple syndromes in those with congenital heart disease.

The A-dependence of the quasielastic A(e,e'p) reaction has been studied at SLAC with H-2, C, Fe, and Au nuclei at momentum transfers Q^2 = 1, 3, 5, and 6.8 (GeV/c)^2. We extract the nuclear transparency T(A,Q^2), a measure of the average probability that the struck proton escapes from the nucleus A without interaction. Several calculations predict a significant increase in T with momentum transfer, a phenomenon known as Color Transparency. The purpose of this document is to release, into the Hanford Document Control System, FA1/0991, Sludge Particle Separation Efficiencies for the Rectangular SCS-CON-230 Container, by M. Epstein and M. G. Plys, Fauske & Associates, LLC, June 2009. The Sludge Treatment Project (STP) will retrieve sludge from the 105-K West Integrated Water Treatment System (IWTS) Settler Tanks and transfer it to container SCS-CON-230 using the Settler Tank Retrieval System (STRS). The sludge will enter the container through two distributors. The container will have a filtration system that is designed to minimize the overflow of sludge fines from the container to the basin. FAI/09-91 was performed to quantify the effect of the STRS on sludge distribution inside of and overflow out of SCS-CON-230. Selected results of the analysis and a system description are discussed. The principal result of the analysis is that the STRS filtration system reduces the overflow of sludge from SCS-CON-230 to the basin by roughly a factor of 10. Some turbidity can be expected in the center bay where the container is located. The exact amount of overflow and subsequent turbidity is dependent on the density of the sludge (which will vary with location in the Settler Tanks) andmore » the thermal gradient between the SCS-CON-230 and the basin. A high-resolution (sigma_instr. = 1.5 MeV) search for narrow states (Gamma < 10 MeV) with masses of M_x approx 1500-1850 MeV in ep -> e'K^+ X, e'K^- X and e' pi^+ X electroproduction at small angles and low Q^2 was performed. These states would be candidate partner states of the reported Theta^+(1540) pentaquark. No statistically significant signal was observed in any of the channels at 90% C.L. Upper limits on forward production were determined to be between 0.7% and 4.2% of the Lambda(1520) production cross section, depending on the channel and the assumed mass and width of the state., 5 pages, 5 figures, to appear in Phys. Recoil proton polarization observables were measured for both the p($\vec {\rm e}$,e$^\prime\vec{\rm p}\,$) and d($\vec {\rm e}$,e$^\prime\vec{\rm p}\,)$n reactions at two values of Q$^2$ using a newly commissioned proton Focal Plane Polarimeter at the M.I.T.-Bates Linear Accelerator Center. The hydrogen and deuterium spin-dependent observables $D_{\ell\ell}$ and $D_{{\ell}t}$, the induced polarization $P_n$ and the form factor ratio $G^p_E/G^p_M$ were measured under identical kinematics. The first measurements of the induced proton polarization, P_n, for the 12C (e,e'\vec{p}) reaction are reported. The experiment was performed at quasifree kinematics for energy and momentum transfer (\omega,q) \approx (294 MeV, 756 MeV/c) and sampled a recoil momentum range of 0-250 MeV/c. The induced polarization arises from final-state interactions and for these kinematics is dominated by the real part of the spin-orbit optical potential. The distorted-wave impulse approximation provides good agreement with data for the 1p_{3/2} shell. The role of the sympathetic nervous system in the renal vasoconstriction induced by sodium restriction has been assessed in normal man in balance on a 10-mEq sodium intake. Renal blood flow was measured with radioxenon injected into the renal artery. Phentolamine infused into the renal artery at rates of up to 3 mg/min did not increase net renal perfusion or alter its intrarenal pattern. That dose of phentolamine was at least 30 times the threshold for significant blockade of the renal vascular effects of a large dose of epinephrine injected intra-arterially. The results suggest strongly that neither circulating catecholamines nor the sympathetic nerves play a central role in the renal vascular response to salt restriction in normal man.

A highly significant correlation has been demonstrated between abnormalities at the interlobar and arcuate artery level in selective renal arteriograms of sixty-six patients with essential hypertension and several clinical indices including the age of the patient and the duration, severity and presence of complications of hypertension. The vascular abnormality at this level also shows a significant correlation with intrarenal hemodynamics assessed by 133 Xe washout and renal function. The hemodynamic and arteriographic changes suggest the presence of focal cortical areas of decreased perfusion in many patients, which may well contribute to the progress and severity of the hypertension. The arterial changes at this level have also been shown to progress with age in a normal population. This age-related change is potentiated in hypertensive subjects.

SummaryThe effect of epinephrine injected into the renal artery has been studied as a model for the assessment by angiography and xenon washout of the control and relative responsiveness of the renal cortical vasculature to such stimuli in man and to ascertain whether a quantitatively significant shunt mechanism can be demonstrated in the human kidney. The findings suggested a dominant sensitivity of the cortical vasculature and rule out the presence of a quantitatively significant shunt mechanism in this setting. The pattern of recovery from the effects of epinephrine suggests that patchy areas in the cortex recover a normal rate of perfusion while other areas remain extremely vasoconstricted. This pattern of perfusion appears to be characteristic of the kidney in a large number of clinical situations.

The simultaneous determination of renal blood flow with 133 Xe washout and renin activity in arterial and renal vein blood has allowed calculation of net renin secretion from the kidneys of twenty-seven patients with essential and accelerated hypertension. Renin secretion was very low in patients with uncomplicated essential hypertension in whom renal hemodynamics and selective renal arteriograms were normal. A significant increase in renin secretion was found in patients with essential hypertension complicated by moderately severe small artery disease in the kidney and reduced renal blood flow. The observation that patients with small artery disease involving the kidney had both a significant increase in renin secretion rate and significantly higher blood pressure levels raises the possibility that the increased renin secretion contributed to the severity of the hypertension in these patients. Patients with malignant hypertension had a very large increase in the rate of renin secretion.

End-stage renal disease (ESRD) is a major public health problem worldwide. The past decade has witnessed increased interest and investigative attention on therapeutic manoeuvres to retard the progression of renal disease. Reduction of albuminuria is regarded as a surrogate endpoint, because this phenomenon seems to be associated with preservation of GFR (principal endpoint) in diabetic nephropathy. Studies from our laboratory suggest that reduction in albuminuria during ACE inhibition constitutes a predictor of an attenuated rate of decline in GFR in early diabetic nephropathy. Because the rate of decline in kidney function is used to assess prognosis and the efficacy of therapy on progression of renal disease, a valid method for the determination of GFR is essential. During the last two decades, several radioactive and non-radioactive filtration markers have been validated and found to be both accurate and precise compared to inulin. Plasma clearance of such filtration markers is frequently applied in clinical trials in diabetic nephropathy because patients suffering from this complication frequently also suffer from diabetic cystopathy, precluding measurements of urine volume. Measurements of serum creatinine, creatinine clearance, calculated creatinine clearance by use of the Cockroft-Gault formula must all be regarded as surrogate measurements for glomerular filtration rate. Consequently, care must be taken to select appropriate filtration markers in order to rigorously evaluate therapeutic trials in progressive renal disease.

In the framework of the EU-project SOLZINC, a 300 kW solar chemical pilot plant for the production of zinc by carbothermic reduction of ZnO was experimentally demonstrated in a beam-down solar tower concentrating facility of Cassegrain optical configuration. The solar chemical reactor, featuring two cavities, of which the upper one is functioning as the solar absorber and the lower one as the reaction chamber containing a ZnO/C packed bed, was batch-operated in the 1300–1500 K range and yielded 50 kg/h of 95%-purity Zn. The measured energy conversion efficiency — ratio of the reaction enthalpy change to the solar power input — was 30%. Zinc finds application as a fuel for Zn-air batteries and fuel cells, and can also react with water to form high-purity hydrogen. In either case, the chemical product is ZnO, which in turn is solar-recycled to Zn. The SOLZINC process provides an efficient thermochemical route for the storage and transportation of solar energy in the form of solar fuels.
