AMAÇ: Miksiyosistoüretrografi; vezikoüreteral reflü ve mesane anatomisini değerlendirmede kullanılmakta olan önemli bir tetkiktir. Miksiyosistoüretrografi çekimi sırasında kullanılan üriner kateterler idrar yolu enfeksiyonunu kolaylaştıran risk faktörlerindendir. Bu çalışmada üriner kateterizasyonun idrar yolu enfeksiyonu oluşumuna etkisinin değerlendirilmesi amaçlandı. YÖNTEMLER: : Çalışmaya Temmuz 2010-Temmuz 2012 ayları arasında Celal Bayar Üniversitesi Hafsa Sultan Hastanesi Çocuk Nefroloji polikliniğinde tekrarlayan idrar yolu enfeksiyonu nedeniyle izlenen ve miksiyosistoüretrografi çekilen 102 çocuk hasta( 62 kız, 40 erkek) alındı. Tüm hastalardan miksiyosistoüretrografiden hemen önce ve iki gün sonra olmak üzere idrar kültürü alındı.BULGULAR: : Miksiyosistoüretrografiden hemen önce alınan idrar kültürlerinin %2,9' unda, miksiyosistoüretrografiden iki gün sonra alınan idrar kültürlerinin ise %11,7'sinde anlamlı bakteriüri saptandı. Katetere bağlı üriner sistem enfeksiyonu tanısı alan çocukların cinsiyeti arasında istatistiksel olarak anlamlı fark saptanmadı (p= 0.07). Hastalar 5 yaş üzeri ve 5 yaş ve altı olarak iki gruba ayrıldığında, yaş ve katetere bağlı üriner sistem enfeksiyonu arasında istatistiksel olarak anlamlı ilişki olduğu görüldü(p=0.01). Miksiyosistoüretrografi öncesi ve sonrası idrar kültürlerinde en sık izole edilen mikroorganizma Escherichia Coli’ydi.SONUÇ: Mesane kateterizasyonunu gerektiren durumlarda temizlik kurallarına uyulması ve vezikoüreteral reflü tanısında hala altın standart olan miksiyosistoüretrografinin yerini alacak yeni tanı yöntemlerinin geliştirilmesi katetere bağlı üriner sistem enfeksiyonlarının önlenmesinde etkili olacaktır., OBJECTIVE: Micturating cystourethrogram is an important tool to assess vesicoureteral reflux and bladder anatomy. The catheters that are being used during micturating cystourethrogram procedures are one of the risk factors that directly cause urinary tract infections. The aim of this study is to assess the effect of urinary catheterization on forming urinary tract infections. METHODS: One hundred two patients (62 boys, 40 girls) which were being evaluated in Celal Bayar University Hafsa Sultan Hospital Pediatric Nephrology policlinic due to recurrent urinary tract infections and underwent micturating cystourethrogram procedure between July 2010-2012 were included in the study. Urine cultures were taken from all patients before and two days after micturating cystourethrogram.RESULTS: Two point nine percent of the urine cultures taken right before micturating cystourethrogram procedures and 11,7% of urine cultures taken right after micturating cystourethrogram procedures were positive for significant bacteriuria. Gender has no significant influence (p=0,07) over catheter-associated urinary tract infection. We have seen a significant relation between catheter-associated urinary tract infection and age when the patients were divided into two groups as the age of five and under with above it (p=0,01). Escherichia Coli was the major pathogen that grows on urine cultures taken both before and after micturating cystourethrogram procedures.CONCLUSION: Following the sterilization directives on situations which require urinary catheterization and developing new diagnostic procedures that can replace micturating cystourethrogram which it is still the gold standard in vesicoureteral reflux diagnosis would be effective on preventing catheter-associated urinary tract infection.

Genetic screening paradigms for congenital and infantile nephrotic syndrome are well established; however, screening in adolescents has received only minor attention. To help rectify this, we analyzed an unselected adolescent cohort of the international PodoNet registry to develop a rational screening approach based on 227 patients with nonsyndromic steroid-resistant nephrotic syndrome aged 10-20 years. Of these, 21% had a positive family history. Autosomal dominant cases were screened for WT1, TRPC6, ACTN4, and INF2 mutations. All other patients had the NPHS2 gene screened, and WT1 was tested in sporadic cases. In addition, 40 sporadic cases had the entire coding region of INF2 tested. Of the autosomal recessive and the sporadic cases, 13 and 6%, respectively, were found to have podocin-associated nephrotic syndrome, and 56% of them were compound heterozygous for the nonneutral p.R229Q polymorphism. Four percent of the sporadic and 10% of the autosomal dominant cases had a mutation in WT1. Pathogenic INF2 mutations were found in 20% of the dominant but none of the sporadic cases. In a large cohort of adolescents including both familial and sporadic disease, NPHS2 mutations explained about 7% and WT1 4% of cases, whereas INF2 proved relevant only in autosomal dominant familial disease. Thus, screening of the entire coding sequence of NPHS2 and exons 8-9 of WT1 appears to be the most rational and cost-effective screening approach in sporadic juvenile steroid-resistant nephrotic syndrome.

Uyku çocuğun sağlıklı büyümesinde önemli yer tutar. Bu nedenle uyku bozukluğu hem çocuk hem aile için büyük sorun yaratır. Son yıllarda geliştirilen aktigrafi çocuk ve adolesanlarda uyku ve uyanıklık siklusunu ölçen motor aktiviteleri hassas bir şekilde algılayan, el veya ayak bileklerine takılarak kullanılan, istirahat ve aktivite paternlerinin dijital ortamda kaydedilmesine ve depolanmasına olanak sağlayan; küçük, hafif, saat şeklinde taşınabilir cihazdır. İçerdiği mikrosensorların kapasitelerine bağlı olarak değişen sürelerle aktivite kaydı yapabilen ve non-dominant ele takılarak kullanılan bu cihazlar uyku günlükleri ile birlikte, uyku yakınması olan kişilerde uzun süreli uyku takibi amacı ile kullanılmaktadırlar. Çocukluk çağında uykuyu etkileyecek patolojiler aktigrafi ile tespit edilebilmektedir. Polisomnografiye alternatif olarak geliştirilen ve kullanımı kolay olan bu cihaz ile çocukluk çağı uyku bozuklukları kolayca tanınıp takip edilebilir., Sleeping is an important factor for the growth of children in a healthy way. Therefore, sleeping disturbances may cause serious problems both for children and their families. Recently developed actigraphy is a small and light device, portable like a watch to perceive motor activities which measures sleep and wakefulness cycles sensitively in children and adolescents, worn on hand and foot ankles, enables resting and activity patterns to be recorded and stored in a digital media. These devices, that can record activities in variable periods depending on capacities of microsensors that they include and worn on non-dominant hand are being used for long term follow-up of sleeping with sleeping diaries of people who complain about sleeping disturbances. The pathologies that will affect sleeping during childhood can be detected by actigraphy. Sleeping disturbances of childhood can easily be identified and followed with this device which can easily be used and was developed as an alternative to polysomnography

Renal glukozüri normal kan glukoz konsantrasyonu varlığında, genel renal tübüler disfonksiyon bulgusu olmadan üriner glukoz atılımı olarak tanımlanmaktadır. Proksimal tübülde glukoz reabsorbsiyonunda sekonder aktif transportu sağlayan taşıyıcı proteinlerden SGLT2’yi kodlayan SLC5A2 genindeki mutasyon sonucu gelişmektedir. Saptanan glukozüri nedeniyle değerlendirilen 10 yaşında kız çocuğunun vital bulguları ve sistem muayeneleri normaldi. Hastada 21.2-32 g/gün glukozüri saptandı. Proteinüri saptanmadı. Tübüler fonksiyonları, kreatinin klirensi, serum üre, kreatinin, ürik asit, bikarbonat, elektrolit değerleri, açlık kan şekeri, HbA1c ve OGTT, idrar aminoasid kromatografisi normal saptandı. İdrar şeker kromatografisinde glukoz dışında şeker atılımı saptanmadı. Hastanın anne ve babasında açlık kan şekerleri normal iken, tam idrar tetkiklerinde; babada 100mg/dl glukoz atılımı saptandı. Büyüme gelişme geriliği, poliüri, polidipsi ve dehidratasyon bulgularının bulunmaması, glukozürinin tanısal değerlendirilmesi açısından yapılan tetkik sonuçları ile olgumuzda renal glukozüri tanısı konmuştur., Renal glucosuria is defined as urinary glucose excretion in the presence of a normal blood glucose concentration and absence of general renal tubular dysfunction. It is caused by the mutations in the SLC5A2 gene, which encodes for SGLT2, one of the transporter proteins that provides secondary active transport during glucose reabsorption in proximal tubule. A 10 years old female patient, who was evaluated for glucosuria, had normal vital signs and physical examination. It has been determined that she had 21.2-32 g/day glucosuria. No proteinuria was detected. Tubular functions, creatinine clearence, serum levels of creatinine, urea, uric acid, bicarbonate, electrolytes, fasting blood glucose, HbA1c and OGTT, urinary amino acid chromatography were normal. In urinary sugar chromatography, secretion of sugars other than glucose was not detected. While fasting blood glucose values of her parents were normal, her father’s urinary glucose level was 100 mg/dl. According to the examination results, the patient was diagnosed as “renal glucosuria” as there were no other possible causes such as a developmental delay, poliuria, polydipsia, or dehydration

İdrar yolu enfeksiyonu erişkinlerde olduğu gibi, çocuklarda da sık karşılaşılan bir sorundur. Bazensadece klinik tanıya dayanarak ampirik antibiyotik tedavisi gerekebilir. Bu çalışmada, çocuklardakiidrar yolu enfeksiyonları etyolojisinde yer alan mikro-organizmaların ve bunların antimikrobiyalduyarlılıklarının belirlenmesi amaçlanmıştır. Ocak 1998-Aralık 2002 tarihleri arasında Moris ŞinasiÇocuk Hastanesine başvuran ve klinik olarak idrar yolu enfeksiyonu tanısı konulan 1361 hastanınlaboratuvar kayıtları retrospektif olarak incelenmiştir. Alınan idrar örneklerinin standart yöntemlerleekimlerini takiben % 29.9’unda üreme olduğu tespit edilmiştir. İzole edilen bakteriler sırasıyla; 267(%65.6) E.coli, 40 (%9.8) Klebsiella spp, 37 (%9.0) Proteus spp, 34 (%8.3), diğer Gram negatifbakteriler ve 30 (%7.3) Gram pozitif bakterilerdir. İzolatların Ampisilin (AMP), Amoksisilin-Klavulonat(AMC), Sefuroksim (CXM), Seftriakson (CRO), Trimetoprim-Sulfometaksazol (TMP-SXT), Gentamisin(GN) ve Amikasin (AK)’ne direnç oranları sırasıyla; % 73.7, % 37.1, % 17.7, % 0.0, % 48.8, % 14.3ve % 0.0 olarak bulunmuştur. Bu sonuçlara göre izole edilen mikroorganizmaların ikinci kuşaksefalosporinler ve beta-laktamaza dirençli penisilinlere duyarlı oldukları bulunmuş ve çocuklardakiidrar yolu enfeksiyonunun ampirik tedavisinde ilk seçenek olabilecekleri, dirençli olgularda iseaminoglikozidlerin alternatif olabileceği düşünülmüştür., Urinary tract infection (UTI) is a common problem encountered in children as well as adults.Sometimes empirical antibiotic treatment might be used depending upon the clinical diagnosis. Theaim of the study was to investigate the common etiologic microorganisms causing UTI and theirantimicrobial sensitivities in children. Urine samples of 1361 children who were evaluated clinically asUTI in Moris Şinasi Pediatric Hospital between January 1998 and December 2002 were investigated.Bacterial growth was noted in 29.9% of the urine samples when the sensitivities of the isolated 407strains to antimicrobials were investigated by standard methods. The isolated bacteria were asfollows: 267 (65.6%) E. coli, 40 (9.8%) Klebsiella spp, 37 (9.0%) Proteus spp, 34 (8.3%), other Gramnegative bacteria and Gram positive bacteria 30 (7.3%). Resistance rates to Ampicillin (AMP),Amoxillin-Clavulonat (AMC), Cefuroxim (CXM), Ceftriakson (CRO), Trimethoprim-Sulfometaxazol(TMP-SXT), Gentamicin (GN) and Amikacin (AK) were found as 73.7 %, 37.1 %, 17.7 %, 0.0 %, 48.8%, 14.3 % and 0.0 %, respectively. Second generation cephalosporins and beta-lactamase resistantpenicillins were found to be effective and they may be the drugs of first choice in the empiricaltreatment of childhood UTI. Amino glycosides may be an alternative in resistant cases.
