Carbon from aquatic plants and periphyton in littoral zones subsidize food webs. Field work was carried out in two different lake types in the Colombian Amazon: a várzea (Yahuarcaca) and an igapó (Pacatúa). Plants in the littoral zone of várzea Lake had higher photosynthetic biomass compared to the plants of the littoral zone of igapó, but differences in root biomass were not significant. The two littoral zones contained a relatively high species richness and diversity (Shannon-Wiener Index) of organisms associated with the roots, although no significant differences existed between the two lakes. Littoral areas may be important in relatively isolated and physically fragmented large terrestrial landscapes such as the Amazon basin.

Mainly affecting poorer populations in remote (sub)tropical areas, paragonimiasis is considered one of the world's most neglected tropical diseases by the World Health Organization. It is usually acquired by consuming undercooked freshwater crustaceans and primarily affects the lungs, but may ectopically migrate to other organs to produce a multisystemic clinical presentation. This study details what appears to be the first documented case in South America and particularly in Colombia of cerebral paragonimiasis and infection by the crab Moreirocarcinus emarginatus.After consuming this crab, a 32-year-old Colombian male developed dyspnea and headache. A chest X-ray revealed tension pneumothorax (TPT) and a thoracoscopic lung biopsy bronchiolitis, eosinophilia, and granulomatous reaction due to Paragonimus parasites.Brain tomography and MRI also showed a configuration typical of Paragonimus parasites, namely a predominantly cystic area near the left cuneus with diffusion restriction, a hemosiderin halo, and linear enhancement characteristic of the tunnel sign, indicating an adult worm moving in the brain.Cerebral paragonimiasis can occur in the Colombian population and Moreirocarcinus emarginatus can be its cause. In rural areas, education about raw crab consumption would be beneficial, and physicians addressing lung-related complaints should bear paragonimiasis in mind and ask about raw crab consumption.

An in situ experiment determined that the addition of ammonia N-NH4+, nitrate N-NO3– and phosphate P-PO43– to an oligotrophic Andean stream significantly affected diatom composition and diversity. Over a nine-month period, fertilized and non-fertilized sections of the stream were studied for their environmental characteristics and diatom composition. Nutrient concentrations were increased three times in a downstream section with respect to those occurring naturally upstream. Alpha diversity (Shannon-Wiener [H”] and Simpson [S] indices) were determined monthly. In the non-fertilized control section, diatom diversity was significantly (p ≤ 0.05) related to water flow and P and N concentrations. In the fertilized section diversity was associated to the enhanced ammonia and phosphorus. A redundancy analysis (RDA) showed that conductivity, water flow and nutrient concentrations contributed to the diatom assemblage composition (83% along the first two axes). Achnanthidium minutissimum, Cocconeis placentula, and Epithemia sorex occurrence was related to low phosphorus concentration. Epithemia adnata var. minor, Nitzschia dissipata, and Reimeria sinuata were associated with high P concentrations, high water flow and low conductivity Navicula rynchocephala, Gomphonema parvulum, Navicula capitatoradiata, and Melosira varians were associated with high P.

Spatio-temporal variation of the aquatic macroinvertebrates in La Lindosa, Colombian Guiana. Introduction: Describing biodiversity patterns and the underlying drivers is relevant for tackling the rampant pressures on tropical freshwaters. This is particularly relevant in scarcely studied regions, such as in the Colombian Guiana shield. Objective: To describe the taxonomic and functional structure of macroinvertebrate communities and assess their variability across spatial and temporal scales. Methods: We sampled the invertebrate communities in four streams during the dry seasons of 2017 and 2018. Additionally, we assessed diversity patterns within and among streams and between different sampling months in one stream. To assess diversity patterns, we compared alpha (differences in taxon richness and abundance within communities) and beta (differences in taxon composition among communities) diversity patterns among and within rivers. Alpha diversity was estimated using the numbers of Hill, while beta diversity was estimated using the Sørensen and Morisita-Horn indices and compared using a similarities analysis (ANOSIM). Results: The taxon composition of the communities was different within (mesohabitat scale) and among streams (regional scale). However, the composition of FFGs changed among but not within streams. The comparison of the numbers of Hill and the taxon composition suggested that the mesohabitat type is a determining factor of the taxonomic but not FFG diversity across scales. At the temporal scale, alpha diversity was low for months with high precipitation (February-April) and high for the dry season (January). The temporal beta diversity was high among sampling periods, being nestedness processes driving beta diversity among years and turnover processes driving beta diversity within years. Conclusion: Streams from Serranía de la Lindosa host a high diversity of freshwater macroinvertebrates. Seasonality and differences in the in-stream habitat heterogeneity seem to determine the diversity patterns observed at different temporal and spatial scales. A deeper study is needed to understand the functioning of these ecosystems and improve the ability to generate management and conservation strategies Describir los patrones de biodiversidad y los mecanismos que los subyacen es indispensable para afrontar las desenfrenadas presiones sobre los ecosistemas de agua dulce en el trópico. Adicionalmente, se evaluaron los patrones de diversidad dentro y entre los ríos y entre diferentes meses de muestreo en uno de los cursos hídricos. Los patrones de diversidad fueron evaluados mediante la comparación de los patrones entre y dentro de los ríos de la diversidad alfa (diferencias en la riqueza y abundancia de taxones entre comunidades) y de la diversidad beta (diferencias en la composición de taxones entre comunidades). La diversidad alfa fue estimada usando los números de Hill, mientras que la diversidad beta se estimó usando los índices de Sørensen y Morisita-Horn y comparada usando un análisis de similaridad (ANOSIM). Resultados: La composición de taxones fue distinta dentro (escala de mesohábitat) y entre los ríos (escala regional). Sin embargo, la composición de FFG cambió entre, pero no dentro de los ríos. La comparación de los números de Hill y de la composición de taxones sugiere que el tipo de mesohábitat es un factor determinante de la diversidad taxonómica pero no de la diversidad de FFG en distintas escalas. En la escala temporal, la diversidad alfa fue baja para los meses con altas precipitaciones (febrero-abril) y alta para la temporada seca (enero). La diversidad beta temporal fue alta para los periodos muestreados y los procesos de anidamiento fueron el componente más notable de la diversidad beta a lo largo del año. Conclusión: Los ríos de la Serranía de la Lindosa albergan una alta diversidad de macroinvertebrados acuáticos. La estacionalidad y las diferencias en la heterogeneidad del hábitat dentro de los ríos parecen determinar los patrones de diversidad observados a diferentes escalas temporales y espaciales. Introduction: Multiple Sclerosis (MS) affects approximately 110,000 people in Italy and MS is a leading cause of disability in young adults. Rehabilitation interventions are frequently used as clinical strategies for improving or maintaining functional state. Methods: We assessed 9 patients with secondary progressive multiple sclerosis (SP-MS): Four female and five male with a mean age of 47.3 years. The mean score EDSS was 7 (values from 3.5 to 8.5). All patients were hospitalized from eight to ten weeks; MSIS-29, EBN, Hamilton, FIM, Barthel, Tinetti and FSS tests were administered. All patients received one weekly spine manipulation in accordance with Palmer, Sutherland, Makenzie procedures both in supine and upright position. Three times a week the patients received also a draining massage of head and lower limbs by Muscular Acoustic Modulator (MAM) device. Eighty minutes a day, for 6 days a week, all patients had neuro-motor rehabilitation. Objectives Forty sites were involved in this multicenter and multivendor registry, which sought to evaluate indications, spectrum of protocols, impact on clinical decision making and safety profile of cardiac magnetic resonance (CMR). Materials and methods Data were prospectively collected on a 6-month period and included 3376 patients (47.2 ± 19 years; range 1–92 years). Recruited centers were asked to complete a preliminary general report followed by a single form/patient. Referral physicians were not required to exhibit any specific certificate of competency in CMR imaging. Results Exams were performed with 1.5 T scanners in 96% of cases followed by 3 T (3%) and 1 T (1%) magnets and contrast was administered in 84% of cases. The majority of cases were performed for the workup of inflammatory heart disease/cardiomyopathies representing overall 55.7% of exams followed by the assessment of myocardial viability and acute infarction (respectively 6.9% and 5.9% of patients). In 49% of cases the final diagnosis provided was considered relevant and with impact on patient's clinical/therapeutic management. Safety evaluation revealed 30 (0.88%) clinical events, most of which due to patient's preexisting conditions. Radiological reporting was recorded in 73% of exams. Conclusions CMR is performed in a large number of centers in Italy with relevant impact on clinical decision making and high safety profile.

To evaluate the accuracy of a semiautomatic quantification of left ventricular (LV) volumes and ejection fraction (EF) using two-dimensional (2D) feature tracking imaging (FTI).Thirty-four consecutive subjects (11 patients with dilated cardiomyopathy, 13 with hypertrophic cardiomyopathy, and 10 subjects with no cardiac disease) underwent, on the same day, trans-thoracic echocardiography (TTE) examination, FTI, and cardiac magnetic resonance imaging (MRI), as gold standard, in order to quantify LV volumes and EF. The echocardiographic quantification of LV volumes and EF was determined from four- and two-chamber views using both standard TTE Biplane Simpson's method and a semiautomatic border detection based on FTI. Furthermore, the time for data analysis for each method was measured.The time required for semiautomatic analysis of volumes and EF was significantly lower (P0.0001) by FTI (71 seconds) in comparison with standard biplane Simpson's method (93 seconds). LV volumes obtained by FTI were significant underestimated (P0.001) in comparison with MRI. Bland-Altman analysis of EDV and ESV using FTI and cardiac MRI showed a low level of agreement for EDV (mean difference = 40.8; SD = 39) and ESV (mean difference = 38.1; SD = 42). On the contrary, no significant difference between FTI and MRI in assessing the LVEF was found; furthermore, a very low bias (2 ± 12) by Bland-Altman analysis was found between FTI and cardiac MRI for the quantification of EF.Semiautomatic quantification of LV volumes using FTI allows an accurate, rapid, easy and reliable assessment of LV EF and a rough estimation of LV volumes.

This study sought to test the vasoreactivity of the translocated coronary arteries after arterial switch operation (ASO) using quantitative angiographic analysis and intracoronary Doppler flow wire velocimetry.Late coronary artery events occur in 3% to 8% of patients after the ASO. Previous studies of coronary flow reserve have yielded disparate results.Nineteen children previously underwent ASO (13 boys, age 5.4 +/- 3.2 years, weight 22.3 +/- 10.6 kg), and six control patients were enrolled in the study. Each patient underwent quantitative angiographic assessment of the epicardial coronary arteries before and after administration of nitroglycerin and coronary blood flow volume assessment before and after administration of adenosine and acetylcholine. The results were compared between groups.Epicardial coronary artery dilation in response to intracoronary nitroglycerin was significantly less in the ASO group than in the control group (left anterior descending [LAD], 5.0 +/- 0.05% vs. 18.0 +/- 4.5%, p = 0.0009; right coronary artery [RCA], 4.0 +/- 0.07% vs. 32.7 +/- 12.7%, p = 0.006). Moreover, the coronary blood flow volume reserve was reduced in ASO patients compared with control patients after intracoronary infusion of acetylcholine (2.3 +/- 0.9 vs. 4.9 +/- 1.7, p = 0.0003) or adenosine (2.7 +/- 1.5 vs. 5 +/- 0.5, p = 0.002).Epicardial coronary arteries fail to dilate normally in children after ASO, and the calculated coronary flow volume reserve is consequently reduced.

Among the differential techniques of remote sensing employed to study the atmosphere and it’s composition, the dial is one of the most powerful, mainly in the detection of concentration of the gas traces. This technique, as known, is based on the knowledge of the interaction between the laser radiation and the molecular compound in studying. In particular it needs, with high precision, the value of the absorption coefficients for the defined wavelengths used in carrying the measurements out.

Introduction. There are only a few reported cases of Salmonella enterica serotype Brandenburg foodborne outbreaks in the literature. In Italy Brandenburg is consistently present among the top-ten serotypes from human source, but at low prevalences. Methods. Fifty-five S. Brandenburg isolates from human, animal, environmental and food sources, including twelve isolates from a foodborne outbreak, were genotyped by PFGE. Results and Discussion. Eight pulsogroups and 19 pulsotypes were detected, with a unique pulsotype being attributed to the outbreak strains. Molecular subtyping can reliably complement the epidemiological investigations. Moreover, mapping molecular types of Salmonella isolates from human and non-human source may greatly contribute to risk assessment, by tracking possible animal sources, so improving cost-effectiveness of the prevention and control strategies.

Among different anatomical causes of subaortic obstruction, anomalous attachment of the mitral valve on ventricular septum is one of the rarest. We report our experience with 4 such cases of subaortic obstruction. Their age at first observation ranged between 1 and 6 months. All patients had viscero-atrial situs solitus, levocardia and normal atrio-ventricular and ventriculo-arterial connections. None had associated cardiac septal defects. Cases with atrio-ventricular canal were excluded. All patients showed a systolic ejection murmur and electrocardiographic features of left ventricular hypertrophy; 3 presented congestive heart failure in the first 3 months of life. Diagnosis was made by 2D-echocardiography and cardiac catheterization in 3 patients and intraoperatively in 1. Only in the last 2 patients, correct diagnosis was made at the initial 2D-echocardiographic examination. Due to left ventricular hypertrophy, in the first 2 patients hypertrophic obstructive cardiomyopathy was originally suspected. One patient died intraoperatively during attempted transaortic resection of subaortic obstruction. Other 2 patients underwent left ventricle-aortic conduit implantation. Of these 2 patients, 1 died 3 months later for sepsis and the other was reoperated upon 3 years later for mitral valve replacement and conduit take-down, with good clinical result. The fourth patient is waiting for surgical intervention. Subaortic obstruction due to anomalous attachment of the mitral valve on ventricular septum may present with early congestive heart failure; the best diagnostic tool is 2D-echocardiography. Left ventricle-aortic conduit may represent a surgical alternative to transaortic resection.

MyoAdenylate Deaminase Deficiency (MADD) is a relatively common metabolic disorder of the skeletal muscle. Patients with MADD usually show an impaired bioenergetic production and a clinical spectrum with either exercise-induced muscle pain, fatigue and/or rhabdomyolysis. Left ventricular hypertrophy as well as other types of cardiac involvement have been reported in patients with primary MADD. We describe herein a case of a 61-year-old woman with biochemical and genetic evidence of Myo-Adenylate Deaminase deficiency, in whom we found a right ventricular hypertrophic cardiomyopathy leading to severe outflow tract dynamic obstruction.

The Ca2+-binding protein of the EF­hand type, S100B, is an intracellular regulator and an extracellular signal. Within cells S100B interacts with several proteins thereby regulating energy metabolism, Ca2+ homeostasis, protein phosphorylation and degradation, and cell locomotion, proliferation and differentiation. Once secreted/released, S100B exerts autocrine and paracrine effects on responsive cells by engaging the receptor for advanced glycation end products. We report an unusual case of a 35-year-old Ivorian migrant with an abdominal mass and medical history relevant for human immunodeficiency virus-2 positivity with a CD4/CD8 ratio of 0.63; Mantoux and lymphocyte stimulation tests (QuantiFERON) were positive. 3D-CT images revealed a voluminous non-homogeneous retroperitoneal mass in the right abdominal region presenting no significant contrast impregnation. Thoraco-abdominal aorta presented diffuse-altered morphology with multiple ectasias throughout its course and an aneurysm at the level of the subrenal tract. The patient underwent vascular surgery. Mycobacterium tuberculosis complex was detected by polymerase chain reaction performed on intraoperative tissue specimens. Postoperative course was uneventful. After surgery, 3D-CT images showed no signs of malfunction of the prosthesis. At last, at 6-month follow-up, the patient was well. Cross-sectional imaging techniques, such as contrasted-CT, are essential and allow for making diagnosis, assessing disease activity, and evaluating post-treatment condition. 3D reconstruction permits an appropriate patient care by means of an excellent visualization and staging of the disease process.

AIM: We tested the relationship between hepatic haemodynamics assessed by Doppler ultrasonography and liver stiffness assessed by Transient Elastography in hepatitis C related chronic liver disease. METHODS: Three liver Doppler ultrasound parameters (hepatic artery resistance index, splenic artery resistance index and waveform pattern in hepatic veins) and liver stiffness measured by Transient Elastography were analysed in one hundred consecutive patients affected by hepatitis C related chronic liver disease. RESULTS: Hepatic and splenic arteries resistance indexes correlate significantly (p13 kPa, whereas a splenic artery resistance index cut-off value of 0.56 provided sensitivity and specificity respectively of 81.3% and 48.5%. The coincidental finding of both resistance indexes above the respective cut-off values showed a good accuracy in identifying patients with liver stiffness values >13 kPa (accuracy=78%, +LR=2.90, -LR=0.31). A significant difference in liver stiffness values was evident between patients with triphasic and bi- or monophasic waveform pattern (p=0.005). CONCLUSIONS: Hepatic and splenic arteries resistance indexes and the hepatic veins waveform pattern assessed by Doppler ultrasound may provide information similar to that of Transient Elastography in hepatitis C related chronic liver disease.

The current standard therapy for the treatment of chronic hepatitis C virus (HCV) is the combination of peginterferon and ribavirin, although many patients fail to clear the virus and their retreatment options are still unsatisfactory. Thymosin alpha1 (Talpha1) is an immunomodulating agent that has been proposed as complementary therapy for chronic HCV, especially in the setting of difficult-to-treat patients. The aim of this study was to evaluate, in patients nonresponsive to previous Peg-based therapy, the effect of standard antiviral therapy with or without Talpha1 on peripheral lymphocyte subsets. Twenty-four patients, 12 receiving Talpha1 and 12 standard therapy, were enrolled. Peripheral subpopulations were analyzed by flow cytometry. Although the addition of Talpha1 did not seem to significantly modify the T-lymphocyte subpopulations, as comparable behaviors were observed in the CD4 and CD8 longitudinal evaluation, Talpha1 produced an earlier increase of natural killer cells. An accurate selection of HCV patients who can benefit from immunomodulation is needed.
