Abstract

Berdasarkan Firman Tuhan dalam Kej 1:27-30, manusia diciptakan dan diberi mandat untuk mengusahakan kehidupan. Allah menciptakan segala sesuatu dan memberikan kepada manusia agar manusia bisa hidup. Dalam pengertian yang demikian maka sejak awal manusia sebenarnya dipanggil untuk berwirausaha. Berwirausaha adalah suatu panggilan karena beberapa alasan, yang pertama karena Allah sendiri dalam hal menjadikan alam semesta beserta isinya itu Ia sedang melakukan peran sebagai seorang wirausahawan, demikian juga pastinya Allah menghendaki itu diwarisi oleh manusia yang dicipta menurut gambaran-Nya. Alasan yang kedua yaitu melalui berwirausaha yang baik juga menumbuhkan relasi yang baik antar sesama manusia terlebih dengan pencipta, karena manusia dimandatkan untuk berkuasa dan mengelola bumi. Ketiga, karena dengan ketersediaanya segala sesuatu Sumber Daya Alam yang diciptakan Allah ini juga mendorong manusia disamping menjaga namun juga mengusahakannya dengan inovatif dan kreatif dan diarahkan pada Allah.

2. Frameshift mutation (690delG) in cpxA contributes to the extensive drug resistance of a Serratia marcescens clinical isolate

Abstract

Objectives To identify the genetic change responsible for resistance to penicillins, extended-spectrum cephalosporins (ESCs), aminoglycosides and ciprofloxacin in a Serratia marcescens clinical isolate recovered from a pancreatic abscess 6 weeks after a WT strain was isolated from the same patient. The impact on the fitness was also assessed. Methods The genomes of both S. marcescens isolates were sequenced using Illumina technology, assembled, annotated and compared with each other. PCR amplification followed by Sanger sequencing was carried out to confirm the mutation. Complementation of the resistant isolate with a recombinant plasmid harbouring the WT gene was performed. The growth rates were measured for both isolates in LB medium. Results Comparative genomic analysis disclosed only one frameshift mutation (690delG) in the cpxA gene, which codes for the histidine kinase of a two-component system (TCS). This change introduced a premature termination codon, leading to the truncated CpxA_HatR variant that contained 234 amino acids instead of 464. Complementation, which consisted of transfer of the WT cpxA into the resistant S. marcescens derivative, restored completely its susceptibility to ESCs, aminoglycosides and ciprofloxacin, thus confirming the contribution of the CpxA_HatR variant to resistance. Growth analysis showed that the fitness of the resistant isolate was unchanged. Conclusions This study shows for the first time that constitutive activation of the Cpx pathway can per se confer resistance to ESCs and ciprofloxacin, in addition to the aminoglycoside resistance usually described. It sheds new light on the role of altered TCSs in fostering bacterial survival.

Abstract

The Si channel of advanced p-type transistors has been replaced by a compressively strained Silicon-Germanium channel (SiGe) in order to improve the device performances. The SiGe thickness and composition must be precisely controlled to reproducibly obtain the same characteristics. In this study, the benefits of X-ray Photoelectron Spectroscopy (XPS) for the process development and the industrial control of thin SiGe channel layers are shown. The use of a parallel Angle Resolved XPS (pARXPS) allowed us to obtain the germanium distribution in very thin SiGe channels, a useful information to better understand the impact of various process steps on the germanium distribution. The hybridization of in-line XPS and X-Ray Reflectivity (XRR) has been used as an industrial process control characterization method to jointly determine the SiGe channel's thickness and germanium composition. This hybrid industrial metrology technique has shown promising results.

Abstract

Sixty-three cases of Streptococcus pyogenes meningitis in adults were studied. Three predominant emm types were associated with meningitis: emm1 (44%), emm3 (11%), and emm6 (11%). Streptococcal toxic shock syndrome and mortality rates were 40% and 38%, respectively.

Abstract

Resume Le streptocoque du groupe A (SGA) est un germe humain responsable d’un large eventail de manifestations cliniques. Les infections invasives a SGA sont en recrudescence depuis le milieu des annees 1980. Afin d’etudier l’epidemiologie des infections invasives a SGA en France, nous avons caracterise toutes les souches invasives recues au Centre national de reference des streptocoques (CNR-Strep) entre 2007 et 2011 et analyse les questionnaires de renseignements cliniques accompagnant les souches. Pour chaque souche ont ete determines le genotype emm , les genes de superantigenes et la sensibilite aux antibiotiques. Sur les 2 603 souches invasives non redondantes de SGA etudiees, 65,1 % ( n = 1 695) provenaient d’hemoculture. Un syndrome de choc toxique streptococcique (SCTS) etait decrit dans 16,4 % ( n = 428) des cas, notamment en cas de dermohypodermite necrosante (DHN), d’infections pleuropulmonaires ou osteo-articulaires ( p ≤ 0,001). Le taux de mortalite s’elevait a 10,6 %. Au total, 102 genotypes emm differents ont ete identifies. Trois genotypes predominaient, atteignant pres de 60 % des souches : emm 1 (26,7 %), emm 28 (16,4 %), et emm 89 (12,8 %). La preponderance de chaque genotype emm etait variable selon les annees et l’âge des patients. Chez les emm 1 (36,8 %), emm 12 (12,9 %) et emm 4 (9,5 %). La distribution des genes de superantigenes ( Spe A, Spe C et Ssa ) etait restreinte a certains genotypes emm . Entre 2007 et 2011, le taux de resistance aux macrolides des SGA a diminue de 7,8 a 5,5 %. Les souches emm 1 restent les plus repandues, en particulier lors des manifestations cliniques les plus severes (SCTS, DHN).

Abstract

International audience; Evidence before this study We searched PubMed on June 30, 2016, for English-language cohort studies published since Jan 1, 1980, of patients with invasive listeriosis worldwide with the keywords " listeria " , " listeriosis " , " maternal " , and " neurolisteriosis ". Studies had to include epidemiological or clinical data on listeriosis. All clinical forms of infection were included (bacteraemia, neurolisteriosis, and maternal–neonatal infection). Host risk factors for listeriosis have been well identified, but the clinical features and prognostic factors of the disease are based on retrospective studies compiling heterogeneous data or random collected cases. Furthermore, no clinical trial has ever been done and medical management is not evidence based. Added value of the study Our study is the first prospective clinical study focusing on all forms of invasive listeriosis. The study is based on a national mandatory system that allowed the nearly complete capture of microbiologically proven cases. The study shows a higher burden of listeriosis than reported before: more than 80% of infected mothers experienced major fetal or neonatal complications (fetal loss, very high prematurity, early or late onset disease); only 39% of patients with neurolisteriosis survived and fully recovered. International audience; Severe invasive group A streptococcal diseases have re-emerged during the past 10-20 years. In order to provide a better insight into the current epidemiological situation in France, we analysed the questionnaires regarding all invasive strains received at the National Reference Center for Streptococci (CNR-Strep) between 2006 and 2010 from patients aged ≥18 and characterized them by emm typing, spe gene detection and antibiotic resistance. Abstract

Invasive group A streptococcal (GAS) infections cause significant morbidity and mortality. A national survey was initiated to assess the burden of invasive GAS infections in France, describe their clinical characteristics, and assess the molecular characteristics of GAS strains responsible for these infections. The survey was conducted in 194 hospitals, accounting for 51% of acute care hospital admissions in France. Clinical data, predisposing factors, and demographic data were obtained, and all GAS isolates were emm sequence typed. We identified 664 cases of invasive GAS infections, with an annual incidence of 3.1 per 100,000 population. The case-fatality ratio was 14% and rose to 43% in the case of streptococcal toxic shock syndrome. Bacteremia without identified focus (22%) and skin/soft tissue infections (30%) were the most frequent clinical presentations. Necrotizing fasciitis was frequent in adults (18%) and uncommon in children (3%). The 3 predominant emm types were emm 1, emm 89, and emm 28, accounting for 33%, 16%, and 10% of GAS isolates, respectively. The emm 1 type was associated with fatal outcomes and was more frequent in children than in adults. Six clusters of cases were identified, with each cluster involving 2 invasive cases due to GAS strains which shared identical GAS emm sequence types. Four clusters of cases involved eight postpartum infections, one family cluster involved a mother and child, and one cluster involved two patients in a nursing home. Invasive GAS infection is one of the most severe bacterial diseases in France, particularly in persons aged ≥50 years or when associated with toxic shock syndrome.

Abstract

Four cases of Streptococcus pyogenes infection due to an emm-type 11 strain, including one with a fatal outcome, occurred within a seven-member family. All strains shared biotype 5, pyrogenic exotoxin genes speB and speC, and resistance to kanamycin, tetracycline, macrolides and lincosamides. The identity of SmaI pulsed-field gel electrophoresis patterns confirmed their clonal origin. This highlights the ability of S. pyogenes to spread rapidly among family members. This first report of a family outbreak due to emm11 S. pyogenes reinforces the importance of surveillance of close family contacts of individuals with invasive streptococcal disease, and provides further support for antibiotic prophylaxis among the elderly.

Resume Objectifs Evaluer et comparer le nouveau milieu chromogene StrepB Select™ (BioRad) au milieu Granada™ (bioMerieux) et a une gelose Columbia au sang de cheval (COH) pour le depistage des streptocoques du groupe B (SGB) dans les prelevements vaginaux (PV) de femmes enceintes. Methodes Cent quatre-vingt-dix PV ont ete analyses. Les trois milieux ensemences en parallele ont ete incubes a 37 °C et examines a 24 heures et a 48 heures. Toutes les colonies suspectes ont ete identifiees par un test d’agglutination. Resultats Trente-deux PV etaient positifs a SGB sur au moins un des milieux testes. Apres 24 heures d’incubation des SGB etaient detectes dans 16 echantillons (50 %) sur COH, 28 (87,5 %) sur StrepB Select™ et 27 (84,5 %) sur Granada™. Apres 48 heures d’incubation, 30 (93,7 %) sur 32 cultures positives a SGB ont ete obtenues pour les trois milieux. D’autres especes de streptocoques et certains enterocoques peuvent donner un aspect similaire a des SGB, ce qui necessite de confirmer l’identification. L’utilisation du kit d’agglutination Pastorex StrepB sur des colonies prelevees sur tous les types de milieux permet cette identification. Les faux-negatifs sur milieu chromogene (6 %) sont dus a des prelevements paucimicrobiens. Sur Granada™, les faux-negatifs (6 %) correspondent a des SGB non pigmentes et non hemolytiques. En revanche, ces souches sont parfaitement bien identifiees par le milieu chromogene. Les deux milieux selectifs inhibent correctement la flore saprophyte associee. Conclusion L’utilisation du milieu StrepB Select™ en routine permet une detection rapide, facile et fiable des SGB dans les PV.

Globicatella sanguinis is a rare cause of acute meningitis. We demonstrated human carriage of Globicatella by identifying cefotaxime-resistant strains in groin and rectal specimens 9 months after invasive infection. The pathogenic strain isolated from the cerebrospinal fluid and the carriage strains were accurately identified by sodA gene sequence analysis.

International audience; Environmental context The safeguarding of water supplies and drinking water is a major issue when considering human health risk management. In this context, an automatic and on-site fractionation system for the detection of organic contaminants has been developed. The main goal of this system is to establish an initial diagnosis by identifying a class of substances involved in a case of pollution. Abstract This paper proposes a new approach for the preconcentration, fractionation, prediction and detection of organic micropollutants in water. The main aim of this study was to implement an innovative fractionation method and a prediction model based on the physicochemical properties of compounds and interactions with the sorbent of solid-phase extraction cartridges. Two sorbents (Strata-SAX and Oasis-HLB) and three specific eluting solvents (mixture of methanol and sodium chloride and mixtures of acetonitrile and ultrapure water) were used to separate organic compounds into five specific fractions according to their physicochemical properties (anionic and cationic or neutral compounds with various polarity). More than 75 molecules (43 molecules individually studied and a mixture with 43 molecules, including 11 molecules individually studied) including pesticides, pharmaceuticals, endocrine disruptors and polycyclic aromatic hydrocarbons, with various properties were studied, and the results showed that the elution fraction can be predicted for more than 85% of the compounds. This methodology could simplify the analytical chain by reducing detailed analysis on limited categories of compounds, and could be used for a rapid and on-site screening of organic compounds. © CSIRO 2016.

Abstract

The prevalence of antibodies against hepatitis C virus (anti-HCV) among intravenous drug users (IVDU) has consistently been very high. Cross challenge studies in chimpanzees provide evidence that reinfection with different HCV strains may occur. In humans, reinfection with different HCV strains has been reported in multitransfused haemophiliacs and recently in IVDU but no case has been reported while on interferon (IFN) therapy. We report on a 22 year old woman who was treated with IFN alpha for HCV genotype 3a chronic infection. At six months, HCV RNA was undetectable by reverse transcription-polymerase chain reaction. In October 1997, while still on IFN, she developed an acute hepatitis after an intravenous drug injection and HCV genotype 1a infection was identified using genotyping and sequencing methods. IFN therapy was continued until August 1998, and in January 1999 HCV-RNA was not detectable. Our case indicates that the previous HCV infection might have prevented development of chronicity. An alternative explanation is that IFN, while not preventing acute hepatitis C, may prevent chronicity. The risk of multiple infection in IVDU underlines the need for preventive strategies.

Abstract

Unstable chromosome aberrations were scored in peripheral blood lymphocytes (PBL) serially collected from 21 breast cancer patients before and after radiotherapy (RT), chemotherapy (CT) and combined treatments. Local radiotherapy as treatment for mammary cancer induced unstable chromosome aberrations in peripheral blood lymphocytes. Only a fraction of these lymphocytes were exposed to irradiation during treatment and the chromosomal damage observed in PBL was equivalent to that induced by irradiation in vitro with 2 Gy at high dose rate, i.e., about 4% of the total dose delivered locally. Chemotherapy alone did not induce such anomalies. Apart from the observed interindividual variations in either the level or the fate of dicentrics with time, different features of chromosome damage were found when chemotherapy was given before or after local cobaltotherapy: secondary chemotherapy did not alter the frequency and the overdispersed distribution of dicentrics observed after first-line radiotherapy; in contrast, when CT was given before radiotherapy, a lower dicentric frequency was scored, the distribution of dicentrics was not always found to be overdispersed and there was a time-dependent decrease in dicentrics after in vivo exposure.

Abstract

Lymphocytes from 43 breast cancer patients were tested for their DNA-repair ability and in vitro chromosomal radiosensitivity. Lymphocytes were collected before and after treatment with radiotherapy or chemotherapy or both, and then irradiated in vitro. The aim was to detect alterations of these 2 indicators of radiosensitivity, in relation to cancer status or medical treatment. Patients before treatment were significantly deficient in DNA-repair ability but had a normal chromosomal radiosensitivity as compared to healthy donors. When assessed after treatment, DNA-repair ability and the frequency of in vitro-induced chromosome anomalies were modified according to the type of treatment. A reduced DNA-repair ability was observed for patients after radiotherapy but not after chemotherapy. In vitro-induced dicentrics and acentrics were not modified to the same extent according to the treatment. A decreased number of acentrics (the most frequently observed alteration) was preferentially associated with a more reduced DNA-repair ability. Interindividual differences of response to in vitro irradiation tested by both assays were observed between patients who had undergone similar treatments. The possibility that these assays could be used for predicting individual susceptibility to radiation or chemotherapy drug exposure is discussed.

We studied the genetic organization of bla ACC-1 in 14 isolates of Enterobacteriaceae from France, Tunisia, and Germany. In a common ancestor, IS Ecp1 was likely involved in the mobilization of this gene from the Hafnia alvei chromosome to a plasmid. Other genetic events involving insertion sequences (particularly IS 26 ), transposons (particularly Tn 1696 ), or sulI -type integrons have occurred, leading to complex genetic environments.

We report the first case of necrotizing fasciitis due to the uncommon Gram-negative pathogen Sphingobacterium multivorum in an immunocompromised patient, who presented with septic shock. This case adds necrotizing fasciitis to the spectrum of S. multivorum -related infections and highlights the emergence of Gram-negative bacteria in severe soft tissue infections.

Abstract

Labelling white blood cells with 99mTc-hexamethyl propylene amine oxime (HMPAO) is associated with the incorporation of radioactivity by granulocytes, but also by lymphocytes and monocytes1,2. Since 99mTechnetium emits low and moderate energy electrons (0,4 to 17 KeV) whose range is smaller than the cell dimensions, a significant cell irradiation may result from the presence of this radionuclide within the cells, and probably within their nucleus3,4. This is of major importance for lymphocytes, which are proliferating and very radiosensitive circulating cells. A study including physical and biological dosimetries, and the evaluation of the viability of the labelled lymphocyte was carried out to evaluate the consequences of this labelling on the lymphocyte survival.

We describe here a method which, in a cell population observed at this stage, allows retrospective estimation of the frequencies of the unstable aberrations induced at the time of irradiation, and their use as a dosimeter. The laws controlling the behavior of unstable aberrations during mitosis were defined from a large-scale experiment on irradiated human lymphocytes. For cells undergoing the first, second, or third mitosis after irradiation, relationships were determined between the frequency, at irradiation time, of acentric fragments not arising from formation of dicentrics or rings, and the ratio of dicentrics and centric rings appearing without acentric fragments to the total number of dicentrics plus rings. On the basis of this ratio, the method described here provides an assessment of the postirradiation mitotic activity in a cell population. This assessment permitted estimation of the cell distribution and frequency of dicentrics plus centric rings, and of the frequency of acentric fragments at the time of irradiation. The use of this method for retrospective dosimetry after whole-body irradiation under various conditions of exposure is illustrated.

Contexte L’InVS et le CNR des streptocoques ont realise une enquete prospective pour determiner l’incidence nationale des infections invasives a Streptococcus pyogenes (taux de letalite et de sequelles eleves pouvant survenir sous la forme de cas groupes), la frequence des differentes formes cliniques, et caracteriser les souches impliquees. Methodes L’enquete a ete realisee entre novembre 2006 et novembre 2007 dans 214 etablissements hospitaliers representant 57 % des admissions en medecine en France. Les infections invasives a S. pyogenes etaient identifiees par les microbiologistes et les questionnaires completes avec l’aide des referents en infectiologie. Les souches de S. pyogenes ont ete caracterisees par leurs marqueurs moleculaires et leur sensibilite aux antibiotiques. Resultats Au total 660 cas d’infections invasives et 611 souches de S. pyogenes ont ete etudies. S. pyogenes a ete isole d’une hemoculture pour 69 % des cas, d’un autre site normalement sterile pour 19 % et d’un site non sterile pour 11 % ; 17 % des cas avaient moins de 15 ans et 32 % plus de 70 ans ; 24 % des patients presentaient une lesion cutanee, 24 % un autre facteur de risque et 14 % les deux. La letalite a 7 jours de 15 % atteignait 45 % en cas de choc. Un membre de l’entourage presentait un facteur risque d’infection streptococcique dans 12 % des cas. Six episodes de cas groupes d’infections du postpartum et deux episodes en milieu familial ou institutionnel ont ete identifies. Parmi 56 genotypes emm , trois : emm 1, emm 89 et emm 28, representaient plus de 60 % des souches ; 11 % des souches etaient resistantes a l’erythromycine et 15 % l’etaient a la tetracycline. Conclusion L’etude a permis des estimations d’incidence et confirme la severite des infections invasives a S. pyogenes et le risque de cas secondaires notamment nosocomiaux.

Abstract

The effects of handling antineoplastic drugs were examined in 17 nurses working in a pneumology department in a hospital in the Paris area. Twenty-five others in the same department who were not handling such drugs acted as controls. Exposure to antineoplastic drugs was evaluated by questionnaire and by assessment of urine mutagenicity. The cytogenetic repercussions of exposure were assessed by examining sister chromatid exchanges and chromosomal abnormalities in circulating lymphocytes. The urine of the exposed nurses was significantly more mutagenic than that of the controls. No cytogenetic abnormalities were revealed in the exposed subjects since both these and the control nurses exhibited similar frequencies of chromosomal abnormalities and similar average numbers of sister chromatid exchanges in circulating lymphocytes. However, in smokers, the urine was significantly more mutagenic compared to that of nonsmokers, chromosomal abnormalities were twice as frequent, and the number of sister chromatid exchanges was significantly larger. Comparison of these results with the findings of other authors suggests that the moderate exposure to antineoplastic drugs prevailing in this pneumology department (i.e., an average of about 11 infusions per week) does not cause detectable cytogenetic abnormalities even though it significantly increases urine mutagenicity.

SummaryCynomolgus monkeys (Macaca fascicularis) were exposed to fractionated whole-body γ-irradiation at high and low dose rates for 4 or 5 weeks. The time-dependence of chromosomal aberrations was studied in relation to the number of lymphocytes during irradiation and after exposure for periods of up to about 600 days for chromosomal aberrations and 200 days for lymphocyte counts. Additivity of the daily effects on the number of chromosomal aberrations was observed during the exposures. Immediately after the end of the exposures the number of chromosomal aberrations decreased to reach low values. The disappearance of chromosomal aberrations seemed to be related to recovery of the lymphocyte counts. The data presented here emphasize the different kinetic patterns of chromosomal aberrations after fractionated and acute irradiation.

Abstract

Changes in the numbers of peripheral lymphocytes with chromosome aberrations were observed in cynomolgus monkeys after fractionated or acute 60Co irradiation at the same total doses. Immediately after irradiation, the yield of dicentrics decreased when the dose was fractionated but remained constant for 20 to 80 days, depending on the dose, when irradiation was acute. Lymphopenia was greater than expected when the dose was fractionated. The kinetics of the loss of chromosome aberrations and the changes in peripheral lymphocyte count occurring in monkeys after acute irradiation were compared to those observed in accidentally irradiated men.
