Surgery ,Neurology (clinical) - Abstract
Craniopharyngioma is a rare embryonic malformation, usually benign, of the sellar or parasellar regions. In this study, an uncommon case of third ventricle craniopharyngioma and a succinct review of its prevalence are presented. Even with low mortality rates, these injuries involve a high degree of endocrinological, visual, and neuropsychological morbidities, which have a huge impact on the patient's quality of life. Thus, surgical resection is the preferred therapy for tumors that compromise the flow of cerebrospinal fluid. However, due to the location of the craniopharyngioma, surgical management is accompanied by the risk of injury to important adjacent areas with postoperative repercussions. Therefore, the neurosurgeon's experience and the choice of the best surgical approach, are crucial for increasing prognosis.

- Abstract
Meningiomas arising from the falcotentorial junction are rare, and the selection of the optimal surgical approach is essential. We report a 41-year-old man presented with progressive left paresis in the lower limbs. An MRI showed a solid mass inside the third ventricle in contact with the falcotentorial dural junction. The tumor was removed by the transtentorial/transfalcine occipital approach, performed with the patient in the three-quarter prone position. The tumor was devascularized from the tentorium, then debulked and finally dissected. The affected falx and tentorium were resected, but all of patent dural venous sinuses were preserved. The tumor was a subtotal resect. Choosing the surgical approach is essential for the safe and effective removal of an FTM and preoperative imaging analysis should identify the tumor’s anatomical relations and guide toward the least disruptive route that preserves the neurovascular structures. This article aims to report a successfully treated FTM.

Presentation A 61 year old man with a history of multiple lymph node metastasis of unknown primary cancer was admitted in the Emergency Room with sudden dyspnea preceded by left leg swelling. Physical examination revealed a blood pressure of 91/70mmHg, tachycardia, tachypnea and signs of deep vein thrombosis (DVT). Diagnosis and management Blood analysis showed increased serum lactate (SL) (5,5 mmol/L), a slight increase of cardiac enzymes and hypoglycemia, and imagiologic tests showed signs of bilateral pulmonary embolism (PE) and an increase of the right cardiac chambers. The patient was hospitalized with the diagnosis of intermediate-high risk PE. Because the patient presented persistent elevated SL and borderline hypotension, fibrinolysis was performed. However, the patient maintained high SL levels and hypotensive profile. After receiving the lymph node's biopsy result, which pointed towards a follicular lymphoma, the persistent hyperlactatemia, together with the hypoglycemic profile, was interpreted as a consequence of the Warburg Effect. Luckily, the patient didn't had any side effects of the fibrinolytic treatment. Learning points Acute high risk PE is marked by the presence of haemodynamic instability at presentation, and the finding of increased SL suggests peripheral hypoperfusion. Systemic thrombolytic therapy is indicated in most of patients with high risk PE. However, there are other causes of hyperlactatemia which the physician must be aware, as it can act as a confounder of PE risk assessment. The Warburg Effect may cause elevated SL in patients with cancer, which in turn are at risk of developing PE.

This study aimed to describe the component properties of adult stems of different species of bamboo through a literature review. The data of holocellulose (cellulose + hemicellulose), lignin, ashes content, total extractives contents, and elemental chemical composition of bamboo was compared with other lignocellulosic materials. In the final considerations, it was found that there is similarity in the average holocellulose content between the species and genera of bamboo, tree species, and other alternative materials that are used in industrial processes. The lignin content varied among tree species, bamboo, and alternative materials, wherein the bamboo presented the intermediate value. The biomasses show hydrogen/carbon ratios from 0.12 to 0.14, indicating that they are highly carbonized. The average value oxygen/carbon ratios of bamboo was 0.96, which indicates there is a higher hydrophobicity in its surface. The average oxygen value of bamboo species was 44.04%, bottom than that found for wood (47.42%); the high content of oxygen leads to a higher reactivity, wherein fast biomass volatilization occurs during the thermal degradation process. The presence of oxygen decreases the energy density as well as the miscibility in hydrocarbon fuels. The highest average values of extractives were found for bamboo, while the other groups of species showed similar values. The highest ash content occurred in the group of species of alternative materials, bamboo also presented high values according to specific standards.

Funding Acknowledgements Type of funding sources: None. Background Uric acid (UA) is a risk factor for coronary artery disease, and it might be a predictor of outcomes in patients (P) with acute coronary syndromes (ACS). Global Registry of Acute Coronary Events (GRACE) Score is an extensively validated tool for risk stratification in these P. The aim of this study is to evaluate the impact of hyperuricemia (HU) in the prognosis of ACS and compare it with the GRACE Score. Methods This study included all P admitted for ACS between 2007 and 2015 in a Cardiology Department. HU was defined as admission UA levels > 6 mg/dL. In-hospital mortality and all-cause mortality at 12 months after discharge were assessed. Chi-square and Mann-Whitney U tests were used for group comparisons; survival analysis used Kaplan-Meier curves and log-rank tests; and subgroup analysis was performed with an unadjusted Cox model. Results 959 P were included (mean age 67.7±12.8y, 71.1% male). HU occurred in 498 P (51.9%). Its presence was associated with older age (p140, and P with HU and GRACE score>140 (Kaplan-Meier χ²=24.17; p Conclusions In this study, HU was associated with in-hospital and 12-month follow-up mortality. Its use may allow a better risk stratification in P with ACS, particularly when combined with the most widely used tool, the GRACE score.

This article makes the case for conducting ethnographic research of a multi-scalar nature that links language policy processes and ideologies of language with everyday practices, on the ground, in local schools and classrooms. As with other researchers engaged in the ethnography of language policy (e.g. McCarty, 2011; Johnson, 2013), my concern is with the ways in which language policies in multilingual countries are translated into classroom practice, the ways teachers and school administrators understand and respond to policy changes and the ways in which communication between teachers and learners is shaped by the introduction of a new medium of instruction. The research presented here focuses on language policy and classroom practice in Timor-Leste. On Independence in 2002, Tetum and Portuguese were chosen to be the two official languages of the country and the main languages of teaching and learning in the school system. My main research sites have been primary schools and classrooms in Timor-Leste and I have adopted a linguistic ethnographic approach, combining ethnography with close analysis of classroom discourse and with discourse analysis of policy documents and interviews. I have used the concept of language ideology as an analytical lens in examining the language policy discourses of policymakers and teachers. Teachers in Timor-Leste are regarded by policymakers as the mere facilitators of the process of implementing Tetum and Portuguese language-in-education policy within the education system. Through the analysis of interview data presented here, I show that teachers assumed this role and shared the belief that Tetum and Portuguese were legitimate official languages of Timor-Leste. Then, through analysis of codeswitching in classroom interaction, in one Year 6 classroom, I show how values around Tetum and Portuguese were being discursively constructed by the teacher, particularly in talk around monolingual texts in Portuguese.

Background Metastatic pancreatic cancer (mPC) and its treatments significantly impact health-related quality of life (HRQoL). POLO, a randomized, double-blind, placebo-controlled phase 3 trial evaluated the efficacy of olaparib as maintenance therapy in germline BRCA mutated mPC patients who had not progressed during ≥16 weeks of first-line platinum-based chemotherapy. HRQoL was assessed using the EORTC QLQ-C30. To enhance score interpretation, we derived reference values for treatment-naïve mPC patients from the literature. Methods A targeted literature review identified EORTC QLQ-C30 baseline values in treatment-naïve mPC patients. Reference values were calculated by deriving means from studies meeting inclusion criteria, with scores from 0 to 100 (higher scores indicate better QoL/functioning but worse symptoms). For POLO patients, means were calculated using pooled baseline data across study arms. Results Four studies met inclusion criteria. Depending on the specific scale, sample sizes ranged from n = 466 to n = 639. Compared to newly derived reference values, POLO patients reported markedly better HRQoL scores at baseline across most scales, with eight scales showing differences of ≥10 points. POLO patients’ HRQoL scores were often close to or better than general population norm data. Conclusions This is the first study to systematically derive EORTC QLQ-C30 reference values for mPC. POLO patients had better HRQoL scores than those in the literature and similar to general population data. Comparatively high HRQoL of POLO patients are likely due to effects of prior first-line treatment and resolution of chemotherapy-related symptoms, response shift, or a combination. Newly derived reference values can enhance interpretation of mPC patients’ HRQoL. Trial registration The POLO trial was registered on 9 July 2014 with ClinicalTrials.gov as NCT 02184195.

Background The HFA-PEFF diagnostic algorithm is a recently published tool to help in the diagnosis of heart failure with preserved ejection fraction (HFpEF). One of the echocardiographic diagnostic criteria is left ventricular (LV) relative wall thickness (RWT), an index of LV concentricity. LV wall thickness can be measured by echocardiography at the posterior wall (PW) and/or the interventricular septum (IVS) in parasternal long axis view. There are three methods of RWT calculation: RWTPW= 2xPW/LV dimension at end diastole (LVDd) – the most used method, RWTIVS = 2 x IVS / LVDd and RWTPW + IVS = (PW + IVS) / LVDd. This study compares the prognostic value of these 3 methods of calculation in patients with acute HFpEF. Methods All patients admitted with acute HFpEF in a Cardiology Department during 7 years were included. RWT was considered elevated if superior to its median and was calculated with the 3 formulas. In-hospital mortality (IHM) was evaluated. The primary endpoint (EP) was a composite of all-cause mortality or hospitalization for HF during follow-up of 24 months. Statistical analysis used chi-square and Mann-Whitney U tests, binary logistic regressions, and Kaplan-Meier curves. Results 478 patients were studied (61.3% female, mean age 79.4±8.3 years). Mean RWTPW, RWTIVS and RWTPW+IVS were 0.46±0.16, 0.50±0.17 and 0.48±0.16, respectively. IHM was 3.4% and primary EP occurred in 57.8%. High RWTPW was associated with higher LV ejection fraction (LVEF) (p There was no statistically significant difference between patients with high and low RWT, calculated using the 3 formulas, in other indices that are commonly used to assess diastolic function, namely in e' and E/e'. None of the 3 methods of RWT calculation was a predictor of IHM. Survival analysis showed that patients with high RWTPW had higher incidence of the primary EP (43.2% vs. 16.8%, Kaplan-Meier χ2=5.99; p=0.014), but not patients with high RWTIVS (Kaplan-Meier χ2=0.23; p=0.631) or RWTPW+IVS (Kaplan-Meier χ2=1.92; p=0.166). RWTPW was a predictor of primary EP (OR 1.81; 95% CI 1.15–2.85; p=0.011) and this result was independent from e' and E/e' (OR 2.96; 95% CI 1.08–8.10; p=0.035). Conclusion In this study comparing 3 formulas for calculation of RWT, RWTPW had better risk prediction during follow-up than RWTIVS or RWTPW+IVS. RWTPW was a predictor of all-cause mortality and hospitalization for HF, and was independent from e' and E/e', indexes that are also recommended in HFA-PEFF diagnostic algorithm. Therefore, the formula incorporating PW should be preferred in the evaluation of patients with suspected or diagnosed HFpEF. Funding Acknowledgement Type of funding sources: None.

26. THE EUROPEAN MONETARY INTEGRATION TRAP: INCOMPLETE SOVEREIGNTY AND THE STATE-MIMICKING METHOD

General Medicine - Abstract
The author identifies the two main (external and internal) dimensions of incomplete sovereignty in the EMU and the respective caveats affecting the scope of the single monetary policy, here described as a ‘monetary policy integration trap’. The author details the main implications caused by this curtailed sovereignty both in its external and internal dimensions – e.g. on the one hand, the polarisation of external positions and, on the other hand, the effects of limited European fiscal/budgetary sovereignty and the atypical interaction between the latter and the single monetary policy. Finally, the way the E(M)U has in recent years addressed this integration trap is analysed, making use of a heterodox method here labelled as the ‘State-mimicking’ method. The main conclusion is that such a method is the possible yet imperfect policy solution to bypass the monetary policy integration trap, given the E(M)U’s ontological ambiguities - a quasi-State afraid to become one.

General Medicine - Abstract

The electron binding energies and Kohn-Sham electronic density of states of liquid HCN were calculated by combining Born-Oppenheimer molecular dynamics with electron propagator theory and density functional theory. A modified long-range corrected Perdew-Burke-Ernzerhof exchange-correlation functional (LC-wPBE), where the parameter w was tuned for reproducing the electron binding energies of the HCN monomer and small aggregates was applied in the liquid state calculations. The average HOMO energy of HCN is red-shifted by ∼ 0.8 eV relative to its gas-phase value. The vertical electron affinity of liquid HCN is estimated as 1.08 ± 0.38 eV.

Funding Acknowledgements Type of funding sources: None. INTRODUCTION Implantable loop recorders (ILR) are a powerful diagnostic tool for heart rhythm diseases, and particularly useful when symptoms are infrequent or when long-term data are required. The main indication is for study of syncope/presyncope due to suspected cardioinhibitory etiology. OBJECTIVE To evaluate the diagnostic profitability of ILR in patient (P) with syncope/ presyncope and to evaluate the effect of symptons on follow-up. METHODS Included P undergoing ILR implantation in 6 consecutive years, to study syncope/presyncope. Information was collected on P characteristics, indication, diagnostic outcome and subsequent management and complications. A follow-up (FU) of 1 year and 3 years were done. Diagnostic outcome was based in symptom-rhythm correlation. Symptons and correlation to ECG during the FU and management after the diagnosis were assessed. RESULTS 99 P were selected. Evaluation of syncope in 91.9% (n = 91) and presyncope in 8.1% (n = 8). 54.5% female, median age 59.4 ± 17.4 years. 55.8% (n = 53) completed the 3 years FU and 84.2% (n = 80) completed the 1 year FU. Death occurred in 4% (n = 4) during the FU. ILR results led to device implantation in 35% of the P (22 pacemakers and 1 ICD) in 1 year FU, with a median time to implantation of 11.6 months after ILR, and the majority of ILR motivated by syncope (87%). If we consider only P that finished the 3 years of FU, in 47.2% P were implanted a device. The most common arrhythmic finding were AV block (47.8%), followed by sinus pauses / asystole (43.5%), AF with slow ventricular rate (4.3%) and VT (4.3%). 4.9% of P experienced a complication related to the device (2 infection and 3 non-infectious pain), that resulted in explantation. 60.9% of the P were symptomatic during the FU, with 24.1% achieving symptom-rhythm correlation and 36.8% who did not. Device implantation was associated with the presence of symptons in FU (82.6% vs 17.4%, p = 0.012) and symptom-rhythm correlation (95% vs 5%,p = 0.001). 38.7% (n = 27) of the P finished the 3 years of FU without a diagnostic outcome or detectable event, with 44.4% being assymptomatic and 55.6% presenting symptoms without ECG correlation. Compared to the P that implanted a device, this type of P was frequently of female sex (57.6% vs 30.4%, p = 0.045), younger age (54.8 ± 18.1 vs 65.8 ± 12.8 years, p = 0.014), with less cardiovascular risk factors like dyslipidemia (37.0% vs 78.3%, p = 0.003) and arterial hypertension (48.1% vs 73.9%, p = 0.05), AF (0% vs 19%, p = 0.025), previous history of myocardial infarction and percutaneous coronary intervention (3.2% vs 17.4%, p = 0.05; 3.0% vs 21.7%, p = 0.028) and more frequently with history of depression (51.9% vs 22.7%, p = 0.037). CONCLUSION In this study, ILR monitoring led to a device implantation in 47.2% of the P that finished the 3 years FU. The presence of symptons and symptom-rhythm correlation during FU was associated with device implantation.

This short paper was submitted in reply to the European Commission’s call for contributions on competition policy supporting the Green Deal. It welcomes the change in the Commission policy towards expressly incorporating sustainability in ‘private’ competition law. Regarding State aid, the paper argues that while Hinkley Point has shifted the balancing of positive effects to Article 107(3)(b) TFEU, sustainability may still be controlled under (c) insofar as it designates the economic activity benefiting from the aid. Moreover, Hinkley Point reinforces the conditionality of aid to compliance with environmental rules, which should be implemented in notification and de minimis regulation. On anti-competitive behaviour, the paper holds that existing rules are sufficiently flexible to exclude a restriction of competition or apply a Wouters justification to arrangements pursuing sustainability, and this should be reflected in guidance and regulation. It nevertheless cautions against balancing detriment to consumers against benefits to society under Article 101(3) TFEU, since it unduly stretches that provision and is likely to underestimate the importance of sustainability. In relation to merger control, the paper suggests that theories of harm to innovation and the analysis of acquisition markets should be expressly related to sustainability. It further asks whether merger control should be subject to the same conditionality of environmental compliance as State aid.

ABSTRACT Are buildings collectible? Some people collected projects, like the Tremaine couple, for whom Niemeyer and Johnson designed houses that they never built; some exhibitions incorporated architectural prototypes, such as the house installed by Breuer in MoMA’s garden in 1949. But in most cases, works of architecture enter museums (and historiography) by their representations. These representations - photographs, drawings - usually compose catalogs that, in a way, enjoy a life independent from their exhibitions of origin, whose longevity they surpass. Latin American Architecture Since 1945, the catalog of the homonymous exhibition organized by Henry-Russell Hitchcock for MoMA in 1955, is now widely recognized as a central piece in the historiographical construction of Latin American modern architecture, like Brazil Builds by Philip Goodwin (1943). Architectural catalogs are bibliographic products where pictures have a predominant role over texts. They fall into the category of the picture collection, or the photographic album. But despite the priority of image over word, the catalog is also a narrative. The purpose of this text is to examine the intrinsic narrative condition of the catalog as a photographic album and as an architecture collection, in its correlations with the historiographic methods and with the formation of the modern architecture concept, by using Latin American Architecture Since 1945.

The notion of collusion is commonly understood as serving a jurisdictional function, determining the scope of Article 101(1) TFEU widely and not prejudging the substantive analysis of a restriction of competition. Hence, it would only be limited by the definition of agreements, decision by an association of undertakings, and concerted practices. This study argues that there is no such limit insofar as the classification as a particular form of collusion is irrelevant. Collusion is instead limited by its characterisation as coordination, represented in the requirements of a single continuous infringement of contributing to common objectives and knowledge of the conduct of the parties. The study also argues that collusion is further subject to substantive limits, notably those fleshed out as the principle of independence of economic action also applying to restrictions of competition. These limits explain why coordination such as oligopolistic alignment, public distancing, and peripheral facilitation is not caught by Article 101(1) TFEU.

The electronic properties of azobenzene (AB) in interaction with gold clusters and adsorbed on the Au(111) surface are investigated by adopting a near-Hartree–Fock–Kohn–Sham (HFKS) scheme. This scheme relies on a hybrid Perdew–Burke–Ernzerhof functional, in which the exact non-local HF exchange contribution to the energy is taken as 3/4. Ionization energies and electron affinities for gas phase AB are in very good agreement with experimental data and outer valence Green’s function) calculations. The presence of C—H⋯Au interactions in AB–Aun complexes illustrates the role played by weak interactions between molecular systems and Au nanoparticles, which is in line with recent works on Au–H bonding. In AB–Aun complexes, the frontier orbitals are mainly localized on the gold platform when n ≥ 10, which indicates the transition from a molecular to a semiconducting regime. In the latter regime, the electronic density reorganization in AB–Aun clusters is characterized by significant polarization effects on the Au platform. The accuracy of the near-HFKS scheme for predicting adsorption energies of AB on Au(111) and the interest of combining exact non-local HF exchange with a non-local representation of the dispersion energy are discussed. Taking into account the significant computational cost of the exact non-local HF exchange contribution, calculations for the adsorption energies and density of states for AB adsorbed on Au(111) were carried out by using a quantum mechanics/molecular mechanics approach. The results strongly support near-HFKS as a promising methodology for predicting the electronic properties of hybrid organic–metal systems.

Background Stroke is the second leading cause of death and a leading cause of disability worldwide. Motor imagery is a technique that can be utilized in the rehabilitation process to improve the lives of patients with a functional disability acquired by this pathology. Aim To evaluate the effects of motor imagery as a complementary intervention for the rehabilitation of stroke patients. Methods We conducted a systematic review in MEDLINE/PubMed, Scopus, Web of Science, and PEDro databases. We included randomized controlled trials (RCTs) that used motor imagery as a complementary resource for the rehabilitation of patients affected by stroke, who had motor function and functional independence as outcomes. Results Of the 1,473 studies found, ten RCTs were included. Regarding the interventions, motor imagery was associated with traditional rehabilitation, virtual reality, physical practice, structured progressive circuit class therapy, and electromyography. The upper and lower extremity performance were accessed through the Fugl-Meyer Assessment (FMA) and gait speed, respectively. Although the practice of motor imagery at least twice a week during three weeks showed to be effective in improving the motor performance of post-stroke patients, the studies’ protocols present a high heterogeneity, with training session times lasting between 30 to 180 minutes and a post-stroke invention window of one to 12 months. Conclusions Motor imagery has been shown to be an efficacious technique in the treatment of post-stroke patients when used as a complement to traditional rehabilitation techniques. However, greater standardization of interventions and studies with higher methodological quality are required to determine further conclusions.

The present study originated from the reflection on the question of the qualification of the worker, expropriated/alienated from the means of production and from the fruits of his work in the context of the destructive regressive crisis of capital. When addressing the issue of worker training at the Federal Institute of Science and Technology of Espírito Santo – Ifes – Campus Ibatiba the thesis discusses the issue of professional qualification at Federal Institutes. Was this formation reproducing social relations of production or counter-hegemonic? In his proposal, the central question ends: whether the teaching of Ifes Ibatiba is a teaching aimed at terminality – professional qualification in a specific area, with the consequent containment of graduates in their access to higher education –; or does it present itself as an opportunity for continued access to university? When taking the case of a concrete campus as a reference, it takes with analyze the formation of its graduates in the first four years since its creation. Taking the historical-dialectical method as a reference, the study bases its reflection on the dialectical concrete situation of worker training in the context of capitalist hegemony. When analyzing the policies that led to the creation of the Federal Education Network, it appears that it presents itself as the achievement of a government policy and not a State policy. In a look at the reality of the Ifes – Campus Ibatiba it appears that there is progress in its pedagogical proposal in relation to the reality of educational institutions in the region of the municipality, as well as limitations to be overcome. Using a double methodology to collect material – first a Google Docs questionnaire, sent to all graduates of the period in question and later an interview with a selected group – the work aimed at observing teaching at Ifes – Ibatiba from experience of those who experienced this process. It is concluded from the data presented by the research that although, in a reality dialectically marked by the hegemony of capital, the formation of Ifes – Ibatiba in the years 2011 to 2017 enabled a transformation in the life of its graduates, by opening them the possibility of access to higher education courses, previously seen as inaccessible by them. Such a reality at the level of micro-reproduction of social relations of production is presented as an achievement, although in the dynamics of macro-reproduction they are still subject to the ailments of the market and capital.
