INTRODUCTION: In the 330-patient ADVOCATE trial of avacopan for the treatment of antineutrophil cytoplasmic autoantibody (ANCA)-associated vasculitis, in which 81% of patients had renal involvement, estimated glomerular filtration rate (eGFR) increased on average 7.3 ml/min per 1.73 m(2) in the avacopan group and 4.1 ml/min per 1.73 m(2) in the prednisone group (P = 0.029) at week 52. This new analysis examines the results in the patient subgroup with severe renal insufficiency at enrollment into the trial, i.e., eGFR ≤20 ml/min per 1.73 m(2). METHODS: eGFR was determined at baseline and over the course of the trial. Changes in eGFR were compared between the 2 treatment groups. RESULTS: In ADVOCATE, 27 of 166 patients (16%) in the avacopan group and 23 of 164 patients (14%) in the prednisone group had a baseline eGFR ≤20 ml/min per 1.73 m(2). At week 52, eGFR increased on average 16.1 and 7.7 ml/min per 1.73 m(2) in the avacopan and prednisone groups, respectively (P = 0.003). The last eGFR value measured during the 52-week treatment period was ≥2-fold higher than baseline in 41% of patients in the avacopan group compared to 13% in the prednisone group (P = 0.030). More patients in the avacopan group versus prednisone group had increases in eGFR above 20, 30, and 45 ml/min per 1.73 m(2), respectively. Serious adverse events occurred in 13 of 27 patients (48%) in the avacopan group and 16 of 23 patients (70%) in the prednisone group. CONCLUSION: Among patients with baseline eGFR ≤20 ml/min per 1.73 m(2) in the ADVOCATE trial, eGFR improved more in the avacopan group than in the prednisone group.

Unpaired shape translation is an emerging task for intelligent shape modelling and editing. Recent methods for 3D shape transfer use single- or multi-scale latent codes but a single generator to generate the whole shape. The transferred shapes are prone to lose control of local details. To tackle the issue, we propose a parts-to-whole framework that employs multi-part shape representation to preserve structural details during translation. We decompose the whole shape feature into multiple part features in the latent space. These part features are then processed by individual generators respectively and transformed to point clouds. We constrain the local features of parts within the loss functions, which enable the model to generate more similar shape characteristics to the source input. Furthermore, we propose a part aggregation module that improves the performance when combining multiple point clusters to generate the final output. The experiments demonstrate that our multi-part shape representation can retain more shape characteristics compared to previous approaches.

Significance: The aberrant inflammation during wound healing results in pathological scarring, such as hypertrophic scars and keloids. This adversely affects the quality of life of patients due to the disfiguring appearance as well as the symptoms of itch and pain. This review summarizes the up-to-date knowledge of the immunopathogenesis and treatment options for pathological scars.With the advent of new technologies, combined with in vitro and in vivo wound models, several inflammatory cells have been shown to have both direct or indirect effects on both wound healing and pathological scarring.Expansion of pro-fibrotic immune cells such as M2 macrophages, dendritic cells, mast cells, and Th2 cells leads to fibroblast transition to myofibroblasts via TGF-β1 signaling pathway. Appropriate management of such inflammatory responses during wound healing remains a critical issue during clinical practice.Regulating inflammation response during wound healing may be a potential therapeutic option for avoiding or reducing pathological scars.

This was a daily diary study using intensive longitudinal methods to evaluate the dynamic structure of relationships among employees’ self-reported health, psychological capital, daily workplace incivility, and daily emotional exhaustion. We recruited 200 participants working in health and fitness clubs for experience sampling and had them file the same questionnaires for 10 consecutive working days. A total of 179 participants (men = 57, 31.85%; women = 122, 68.15%) completed the questionnaires, with a response rate of 89.5%, leading to 1790 data points at the within-person level. We used a multilevel location-scale model of dynamic structural equation modeling (DSEM) for data analysis. We ran the model in Mplus software (version 8.4) and found an autoregressive ‘carry over’ relationship between the previous day’s emotional exhaustion changes on current emotional exhaustion changes ([Formula: see text] = 0.067, CI = [0.003, 0.132]). Daily workplace incivility significantly and positively predicted daily emotional exhaustion at the within-person level ([Formula: see text] = 0.953, CI = [0.465, 1.489]). Self-reported health was negatively associated with a person’s mean ratings of daily emotional exhaustion ([Formula: see text] = −0.256, CI = [0.465, 1.489]) and moderated the strength of the workplace incivility effect on emotional exhaustion ([Formula: see text] = −0.201, CI = [−0.292, −0.121]). Finally, psychological capital was negatively related to a person’s mean ratings of daily emotional exhaustion ([Formula: see text] = −0.240, CI = [−0.377, −0.102]).

Advances in artificial intelligence and data acquisition technology are growing, the research on deep learning algorithm has gone deep into various fields. At this stage, the demand supply matching model under the comprehensive passenger transport hub travel system is obtained by analyzing the travel mode data. This paper takes traffic time as the research direction,uses machine learning and complex network theory to conduct in-depth learning algorithm research, respectively discusses, explores and forecasts the traffic supply and demand mode data, and explores the traffic mode supply and demand model under the comprehensive passenger transport hub travel service system. It is shown in traffic data that using the prediction form of deep learning to predict traffic conditions and travel pressure can enable traffic managers to master traffic dynamics and lead the direction for future traffic development. Finally, from the perspective of MaaS system, the paper uses big data and information processing methods to explore the supply and demand matching model in terms of transportation modes. The research shows that MaaS system can make overall planning in terms of traffic resources, provide reasonable travel modes for traffic travelers and match corresponding travel services. It not only makes overall planning and guarantee for travel services, but also promotes the sustainable development of the transportation supply and demand system.

Abstract: Nonalcoholic fatty liver disease (NAFLD) has received worldwide scientific attention because of its rapidly increasing prevalence, and it has emerged as a serious public health problem in end-stage liver disease. Many factors are involved in the multifactorial development and progression of liver disease by influencing multiple signaling and metabolic pathways. Currently, many studies have demonstrated the critical role of microRNA- 21 (miR-21) in NAFLD pathogenesis. In addition, many studies have found that miR-21 is highly expressed in inflammatory bowel disease, which is associated with intestinal barrier dysfunction and altered gut microbiota. In this paper, we focus on the regulatory role of miR-21 in the progression of NAFLD and its effect on the gut microbiota, summarize the involvement of miR-21 through a variety of signaling pathways and metabolic pathways, as well as discuss some predicted miR-21 target genes and miR-21 pathways for future experimental identification.

We assessed the association between long-term joint exposure to ambient air pollutants and the risk of laryngeal cancer and whether this risk was modified by genetic susceptibility. We used a multivariable Cox proportional hazards regression model to analyze data from UK Biobank to determine the relationship between long-term exposure to air pollutants–nitric oxide (NO), nitrogen dioxide (NO2), and 2.5-µm and 10-µm particulate matter (PM2.5 and PM10) and the risk of laryngeal cancer. In multivariable-adjusted models, in model 3 and compared with the participants with lower quintile scores for air pollution, the participants with the highest quintile scores for air pollution had a higher laryngeal cancer risk. The observed association was more pronounced among the participants who were female, were smokers, had a systolic blood pressure equal to or greater than 120 mmHg, and had diabetes. Compared with the participants with a low GRS and the lowest quintile score for air pollution exposure, those with an intermediate GRS and the highest quintile score for air pollution exposure had a higher risk of laryngeal cancer. Long-term exposure to NO2, NO, or PM2.5, individually or jointly, was associated with a risk of incident laryngeal cancer, especially in the participants with an intermediate GRS.

Background The benefits of low-dose ketamine for patients with treatment-resistant depression (TRD) and prominent suicidal ideation require further investigation. The effects of treatment refractoriness, the duration of the current depressive episode, and the number of prior antidepressant failures on ketamine efficacy also require clarification. Methods We recruited 84 outpatients with TRD and prominent suicidal ideation—defined as a score ≥4 on item 10 of the Montgomery–Åsberg Depression Rating Scale (MADRS)—and randomized them into 2 groups to receive 0.5 mg/kg ketamine or 0.045 mg/kg midazolam. We assessed depressive and suicidal symptoms prior to infusion; 240 minutes post infusion; and 2, 3, 5, 7, and 14 days post infusion. Results According to the MADRS scores, the antidepressant effect (P = .035) was significantly noted in the ketamine group up to 14 days than in the midazolam group. However, the antisuicidal effect of ketamine, as measured by the Columbia-Suicide Severity Rating Scale Ideation Severity Subscale (P = .040) and MADRS item 10 (P = .023), persisted only 5 days post infusion. Furthermore, the antidepressant and antisuicidal effects of ketamine infusion were noted particularly in patients whose current depressive episode lasted Conclusions Low-dose ketamine infusion is a safe, tolerable, and effective treatment for patients with TRD and prominent suicidal ideation. Our study highlights the importance of timing; specifically, ketamine is more likely to achieve therapeutic response when the current depressive episode lasted

Supervisory mentoring represents a type of social dilemma called a delayed social fence. This study adopts a social dilemma perspective to examine how the three types of psychological contracts (balanced, relational, and transactional) perceived by supervisors differently influence their mentoring. Drawn on social dilemma perspective, we proposed that supervisory mentoring would be more likely to occur when supervisors perceived benefit return from their mentoring provision in a timely manner. The results obtained from a sample of 596 supervisor–subordinate matched data from the self-reported questionnaires completed by 225 sales agent teams in the insurance industry in Taiwan support our predictions. Consistent with the social dilemma perspective, supervisory mentoring is more likely among subordinates whose supervisors perceived balanced psychological contract, while supervisory mentoring is less likely among subordinates whose supervisors perceived transactional psychological contract. Furthermore, we found that supervisory mentoring is positively related to subordinate performance. Our mentor-centric multilevel framework helps identify the social dilemma nature underlying mentoring provision, and verify the positive influence of mentoring on protégé performance.

Magnesium (Mg) based biomaterials have gained increased attention in various health care applications, as orthopedic transplants in particular. It is also reported that they also have anti-tumor properties. Mg based alloy materials can actively interfere with the growth of tumor cells, which may be its degradation products, including OH− and H2. During the degradation of Mg, ions and hydrogen (H2) are continuously increasing and pH value and osmotic pressure in the microenvironment of the material surface are increased at the same time. Such near-surface effects can also have a dramatic impact on residual tumor cells, interfere with the cycle of tumor cells as well as reduce the content of reactive oxygen species. Therefore, the intake level of Mg based alloys and Mg content will affect the growth activity of many tumor cells. Keeping these points in view, this article reviews the role of Mg in tumor microenvironment and underlying molecular mechanisms, in particular it’s degradation behavior.

Despite the remarkable clinical responses achieved with immune checkpoint blockade therapy, the response rate is relatively low and only a subset of patients can benefit from the treatment. Aberrant RNA accumulation can mediate IFN signaling and stimulate an immune response, suggesting that targeting RNA decay machinery might sensitize tumor cells to immunotherapy. With this in mind, we identified an RNA exoribonuclease, XRN1, as a potential therapeutic target to suppress RNA decay and stimulate antitumor immunity. Silencing of XRN1 suppressed tumor growth in syngeneic immunocompetent mice and potentiated immunotherapy efficacy, while silencing of XRN1 alone did not affect tumor growth in immunodeficient mice. Mechanistically, XRN1 depletion activated IFN signaling and the viral defense pathway; both pathways play determinant roles in regulating immune evasion. Aberrant RNA-sensing signaling proteins (RIG-I/MAVS) mediated the expression of IFN genes, as depletion of each of them blunted the elevation of antiviral/IFN signaling in XRN1-silenced cells. Analysis of pan-cancer CRISPR-screening data indicated that IFN signaling triggered by XRN1 silencing is a common phenomenon, suggesting that the effect of XRN1 silencing may be extended to multiple types of cancers. Overall, XRN1 depletion triggers aberrant RNA-mediated IFN signaling, highlighting the importance of the aberrant RNA-sensing pathway in regulating immune responses. These findings provide the molecular rationale for developing XRN1 inhibitors and exploring their potential clinical application in combination with cancer immunotherapy.Significance:Targeting XRN1 activates an intracellular innate immune response mediated by RNA-sensing signaling and potentiates cancer immunotherapy efficacy, suggesting inhibition of RNA decay machinery as a novel strategy for cancer treatment.

A key trait conferring flood tolerance is the ability to grow adventitious roots as a response to submergence. The genetic traits of deepwater rice determining the development and characteristics of aquatic adventitious roots (AAR) had not been evaluated. We used near-isogenic lines introgressed to test the hypothesis that the impressive shoot elongation ability of deepwater rice linked to quantitative trait loci 1 and 12 also promote the development of AAR. The deepwater rice genotype NIL-12 possessed expanded regions at the stem nodes where numerous AAR developed as a response to submergence. Two types (AR1 and AR2) of roots with distinct timing of emergence and large differences in morphological and anatomical traits formed within 3 (AR1) to 7 (AR2) d of submergence. The mechanical impedance provided by the leaf sheath caused AR2 to emerge later promoting thicker roots, higher elongation capacity and higher desiccation tolerance. Upregulation of key genes suggests a joint contribution in activating the meristem in AAR enhancing the development of these in response to submergence. The morphological and anatomical traits suggested that AR2 is better adapted to long-term flooding than AR1. We therefore propose that AR2 in deepwater rice functions as an evolutionary defence strategy to tackle periodic submergence.

The aim of this study was to investigate the relationship between day-1 urine cadmium excretion and 30-day mortality in patients with acute myocardial infarction (AMI) at two centers.A total of 286 patients (222 males and 64 females) with AMI from Huashan Hospital, Shanghai and Chang Gung Memorial Hospital, Taiwan were enrolled. Basic vital signs, history, laboratory results, and day-1 urine excretion of cadmium (D1UECd) were recorded. Disease severity was assessed during the first hospitalization using Killip score, APACHE II score, and SOFA score. The main endpoint was 30-day mortality.Among the 286 patients, 218 were from Chung Gung Memorial Hospital and 68 were from Huashan Hospital with an average age of 64.2 years. Forty (14%) patients died within 30 days after AMI. The average 24-h urine cadmium level among the Chung Gung Memorial Hospital cohort was 1.5 ± 2.4 μg compared to 1.7 ± 1.7 μg among Huashan Hospital cohort, both higher than the local populations. A higher D1UECd level was significantly associated with a greater risk of 30-day mortality (odds ratio 1.68, 95% confidence interval 1.30-2.16) after controlling for a number of covariates. The ability of D1UECd to discriminate 30-day mortality was excellent, with a very high area under the curve (87.2%, 95% CI 82.0-92.5%).D1UECd was positively correlated and an independent predictor of 30-day mortality in the enrolled AMI patients. D1UECd may be a simple, objective prognostic scoring system in AMI patients.

The ninth phase of the medical network of the Ministry of Health and Welfare has established one of its overall goals as establishing a healthcare system that is resilient and smart. As in the past, in 2022, the Joint Commission of Taiwan continued to conduct a census of the development of eHealth in medical institutions in Taiwan. Affected by the COVID-19 pandemic, video teleconferencing for disease diagnosis and treatment, digital learning systems, and distant teaching have been more widely introduced compared to the application of environmental smart management in outpatient clinics, emergency departments, and inpatient clinics. In the following year, the application of artificial intelligence technology will be a key development point for medical centers; by contrast, the focus of non-medical centers will be information security. The top three key factors for promoting eHealth among medical institutions will be hospital policies, medical personnel demands, and health policies, with the only difference being that local community hospitals will focus more on hospital policies and health policies. &nbsp
