Background Cardiovascular diseases are the leading cause of mortality, morbidity, and disability in the world, especially in the older adults. A relevant proportion of patients admitted to Cardiac Rehabilitation (CR) may suffer from frailty, a complex geriatric syndrome with multifactorial aetiology. Aims The hypothesis underlying the study is that frailty complicates the management of older patients undergoing CR. The main objective is, therefore, to determine the relationship between frailty and CR outcomes in hospitalized older adults. Methods The participants have been recruited among patients aged ≥ 65 years admitted at the hospital for CR. A Comprehensive Geriatric Assessment (CGA)-based Frailty Index (FI) was created following a standard procedure. The outcome was measured as the ratio between 6-min walk test (6MWT) distance at the end of CR and normal predicted values for a healthy adult of same age and gender, according to reference equations. Results The study population consisted of 559 elderly patients, 387 males (69.2%), with age of 72 (69–76) years. The most frequent diagnosis at admission was ischaemic heart disease (231, 41.5%) and overall 6MWT ratio was 0.62 ± 0.21. At the multivariable regression analysis, gender, diagnosis and FI were significantly and independently associated with 6MWT ratio (p ≤ 0.0001, p ≤ 0.001 and p ≤ 0.0001, respectively), while no significant association emerged for age. Conclusion FI resulted independently correlated to 6MWT ratio in a population of older patients undergoing in-hospital CR programs. Frailty is a multifactorial geriatric syndrome whose assessment is essential for prognostic evaluation of older patients, also in CR clinical setting.

A recent review by Kumari and Khanna examined the prevalence of sarcopenic obesity using various comorbidities, diagnostic markers, and possible therapeutic approaches. The authors discussed the strong impact of sarcopenic obesity on quality of life (QoL) and physical health. In addition, there are significant interactions among bone, muscle, and adipose tissue, and the concomitant presence of osteoporosis, sarcopenia, and obesity, termed osteosarcopenic obesity, represents a terrible trio for postmenopausal women and older adults as each of these conditions is associated with adverse outcomes in terms of morbidity, mortality, and QoL in several domains. Timely diagnosis, prevention, and pro-health education are crucial for improving QoL in patients with osteoporosis, sarcopenia, and obesity. Education and prevention play a pivotal role in the long term for individuals to have longer and healthier lives. Osteoporosis, sarcopenia, and obesity share modifiable risk factors that may benefit from physical activity, a healthy and balanced diet, and lifestyle changes. “Prevention is better than cure” and planning are proven strategies for individuals and sustainable healthcare.

Introduction Data from the “Trattamento Ormonale nello Scompenso CArdiaco” (T.O.S.CA) registry showed that heart failure (HF) represents a complex clinical syndrome with different hormonal alterations. Renal failure represents a frequent complication in HF. We evaluated the relationship between renal function and insuline-like growth factor-1 (IGF-1) deficiency and its impact on cardiovascular mortality (CVM) in patients enrolled in the T.O.S.CA. registry. Methods At the enrolment, all subjects underwent chemistry examinations, including circulating hormones and cardiovascular functional tests. COX regression analysis was used to evaluate factors related to CVM during the follow-up period in all populations, in high-risk patients and in the young-adult population. Also, we evaluate the effects of renal function on the CVM. Results 337 patients (41 deceased) were analyzed. CVM was related to severe renal dysfunction (HR stages IV–V = 4.86), high-risk conditions (HR 2.25), serum IGF-1 (HR 0.42), and HF etiology (HR 5.85 and HR 1.63 for valvular and ischemic etiology, respectively). In high-risk patients, CVM was related to IGF-1 levels, severe renal dysfunction and valvular etiology, whereas in young patients CMV was related to the high-risk pattern and serum IGF-1 levels. Conclusions Our study showed the clinical and prognostic utility of the IGF-1 assay in patients with HF.

Statin-induced autoimmune myositis (SIAM) represents a rare clinical entity that can be triggered by prolonged statin treatment. Its pathogenetic substrate consists of an autoimmune-mediated mechanism, evidenced by the detection of antibodies directed against the 3-hydroxy-3-methylglutaryl-coenzyme A reductase (anti-HMGCR Ab), the target enzyme of statin therapies. To facilitate the diagnosis of nuanced SIAM clinical cases, the present study proposes an “experience-based” diagnostic algorithm for SIAM. We have analyzed the clinical data of 69 patients diagnosed with SIAM. Sixty-seven patients have been collected from the 55 available and complete case records regarding SIAM in the literature; the other 2 patients represent our direct clinical experience and their case records have been detailed. From the analysis of the clinical features of 69 patients, we have constructed the diagnostic algorithm, which starts from the recognition of suggestive symptoms of SIAM. Further steps provide for CK values dosage, musculoskeletal MR, EMG/ENG of upper-lower limbs and, Anti-HMGCR Ab testing and, where possible, the muscle biopsy. A global evaluation of the collected clinical features may suggest a more severe disease in female patients. Atorvastatin proved to be the most used hypolipidemic therapy.

Frailty is a clinically measurable state of vulnerability to developing increased dependency and/or mortality when exposed to a stressor. Chronic diseases, aggressive treatments, antibiotic overuse, microbiota changes, immune senescence, and increased use of medical devices and implants (i.e., central lines and catheters) expose modern patients to healthcare-associated infections (HAIs), multidrug-resistant bacteria, and new and unusual opportunistic pathogens. Older adults are among the main victims of HAIs and are associated with high costs, disability, morbidity, and mortality. Ralstonia pickettii is an emerging opportunistic pathogen that causes rare nosocomial infections in frail individuals. Herein, we present a case of bloodstream infection caused by R. pickettii in an 88-year-old woman with a relatively mild course. In addition to describing this unusual finding, this report discusses the problem of HAIs in older adults. Older age, comorbidities, and hospital admissions were among the main risk factors for HAIs. Adherence to guidelines, training, auditing, and surveillance is crucial for reducing the burden of HAIs in acute settings. Furthermore, avoiding incongruous hospitalizations would have positive implications both for preventing HAIs and improving patient quality of life.

From such examples of the past, Lionello will be able to benefit greatly, learning, for example, to recognise flatterers.

Early diagnosis of neurodevelopmental impairments in preterm infants is currently based on the visual analysis of newborns' motion patterns by trained operators. To help automatize this time-consuming and qualitative procedure, we propose a sustainable deep-learning algorithm for accurate limb-pose estimation from depth images. The algorithm consists of a convolutional neural network (TwinEDA) relying on architectural blocks that require limited computation while ensuring high performance in prediction. To ascertain its low computational costs and assess its application in on-the-edge computing, TwinEDA was additionally deployed on a cost-effective single-board computer. The network was validated on a dataset of 27,000 depth video frames collected during the actual clinical practice from 27 preterm infants. When compared to the main state-of-the-art competitor, TwinEDA is twice as fast to predict a single depth frame and four times as light in terms of memory, while performing similarly in terms of Dice similarity coefficient (0.88). This result suggests that the pursuit of efficiency does not imply the detriment of performance. This work is among the first to propose an automatic and sustainable limb-position estimation approach for preterm infants. This represents a significant step towards the development of broadly accessible clinical monitoring applications.

Across a wide range of issues and a variety of sources, scientific information often gets lost in translation, failing to properly inform, educate, and engage publics in a meaningful way. But science communication is like a wine—it is better when it is outside the box. One promising tactic for more effective scientific communication is the use of humor to both engage and educate less interested and knowledgeable citizens on important topics. Here, we investigate whether the identity of a speaker influences perceptions of their effectiveness at connecting with and engaging public audiences. We conducted a 2 (Gender: Female vs. Male) × 2 (Race: Black Speaker vs. White Speaker) × 2 (Credentials: Scientist vs. Comedian) between-subjects experiment and found that race and gender of the source, relative to their credentials, were not significant factors for predicting the perceptions of communicator effectiveness. We also found that experienced humor, or mirth, moderated the relationship between the speaker’s credentials and perceived effectiveness. We discuss the implications of our findings for science communication research and practice.

Mental health disorders pose a significant challenge to society. The Bayesian perspective on the mind offers unique insights and tools that may help address a variety of mental health conditions. Psychopathological dysfunctions are often connected to altered predictive and active inference processes, in which cognitive and physiological pathogenic beliefs shape the clinical condition and its symptoms. However, there is a lack of general empirical models that integrate cognitive beliefs, physiological experience, and symptoms in healthy and clinical populations. In this study, we examined the relationship between altered predictive mechanisms, interoception, and pathological bodily distortions in healty individuals and in individuals suffering from anorexia nervosa (AN). AN patients (N=15) completed a Virtual Reality Full-Body Illusion along with interoceptive tasks twice: at hospital admission during an acute symptomatological phase (Time 1) and after a 12-week outpatient clinical weight-restoring rehabilitative program (Time 2). Results were compared to a healthy control group. Our findings indicated that higher levels of interoceptive metacognitive awareness were associated with a greater embodiment. However, unlike in healthy participants, AN patients' interoceptive metacognition was linked to embodiment even in multisensory mismatching (asynchronous) conditions. In addition, unlike in healthy participants, higher interoceptive metacognition in AN patients was related to prior abnormal bodily distortions during the acute symptomatology phase. Prediction errors in bodily estimates predicted posterior bodily estimate distortions after the illusion, but while this relationship was only significant in the synchronous condition in healthy participants, there was no significant difference between synchronous and asynchronous conditions in AN patients. Despite the success of the rehabilitation program in restoring some dysfunctional patterns in the AN group, prediction errors and posterior estimate distortions were present at hospital discharge. Our findings suggest that individuals with AN prioritize interoceptive metacognitive processes (i.e., confidence in their own perceived sensations rather than their actual perceptions), disregarding bottom-up bodily inputs in favour of their prior altered top-down beliefs. Moreover, even if the rehabilitative program partially mitigated these alterations, the pathological condition impaired the patients' ability to coherently update their prior erroneous expectations with real-time multisensory bottom-up bodily information, possibly locking the patients in the experience of a distorted prior top-down belief. These results suggest new therapeutic perspectives and introduce the framework of regenerative virtual therapy (RVT), which aims at utilizing technology-based somatic modification techniques to restructure the maladaptive priors underlying a pathological condition.

Nonconventional clinical presentations of diseases are common in older adults. Even dramatic events, such as foreign body (FB) inhalation, can occur in a subtle and non-specific manner. Pill aspiration is a rare yet overlooked cause of airway injury. It accounts for approximately 7% of all FB aspirations. In contrast, oral dysphagia and polypharmacology, mainly administrated in solid oral dosage forms (SDOF), like tablets and pills, are common conditions in older adults. Herein, we present a case of SDOF aspiration in a 78-year-old man. FB inhalation developed with general clinical deterioration and neurological impairment (delirium) rather than overt respiratory symptoms. Bronchoscopy provided remarkable images of this unexpected finding. Caregivers and healthcare workers must be aware of the risk of SDOF aspiration and adopt proper safety measures. Early recognition and bronchoscopy for diagnostic and therapeutic purposes can be lifesaving in such cases.

COVID-19 pandemic forced countries to adopt strategies aimed at responding to the health emergency by containing contagion. Lockdowns have ensured the achievement of this goal but imposed substantial restrictions to the freedom of movement and resulted in social isolation for a large share of vulnerable people. The aim of the present study was to evaluate the impact of COVID-19 pandemic and associated emergency restriction measures on the quality of life, lifestyle habits, and psychosocial status in older adults.

This qualitative study explored the effects of human–animal relationships on care-farms, with specific attention to the context of trauma histories. We questioned how the interpretative act and belief in identifying shared narratives of prior suffering could change how people relate to their own narratives of trauma and grief and to animals. Drawing on a study of grieving individuals’ experiences on a care-farm providing support and psychoeducation to individuals who have experienced traumatic grief, we present the results of an in-depth qualitative survey. As part of the study, participants were asked to reflect on whether it was important that the service-provider’s model included helping rescue animals: 91% answered affirmatively. Participants were invited to expand discursively why, or why not, this had been meaningful to them. Our results show that participants assigned benefits from personally identifying a “shared narrative” of trauma with the animals, that witnessing a level of rehabilitation and resilience in animals with trauma histories was meaningful for participants for their own integration of grief, and that being able to contribute “care” for animals provided a mechanism for compassionate practice. Our findings suggest that animals with loss and trauma biographies may provide unique and unexpected psychological benefits to humans facing grief and trauma. We are not suggesting that animals who have a traumatic past have an inherent capacity for providing salutary benefit or that such animals should be engaged to provide therapeutic opportunities. Rather, we emphasize the importance of narrative and how such narratives change how participants relate to, and interact with, animals. Our research serves as an important reminder that “therapy animals” are living beings with their own life histories and experiences. Careful thought needs to be given when working with animals in a therapeutic context in order to protect both vulnerable humans and animals.

A study is currently underway in the United States using the opioid antagonist naltrexone to treat prolonged grief, which is conceptualized in the study’s proposal as an addiction disorder. The researchers’ stated intention is to use the pharmaceutical agent to disrupt the griever’s capacity to engage in social bonding to eliminate craving for the person who died. We believe this approach is misguided for a number of reasons. It demeans the importance of the relationship between the bereaved and the deceased loved one, further isolates grievers from the very social support networks that could help facilitate adaptation to bereavement, and could have a disproportionate negative impact on marginalized communities, who tend to rely more heavily on informal sources of support. We argue that social connection is at the very core of healing and that disregarding and interfering with this capacity could have widespread detrimental effects on grievers.

The development of novel non-destructive methodologies for the verification of electronic component authenticity is imperative in reducing the economic and safety risks associated with the use of counterfeited products. Currently, inspection methods rely on a series of electrical and physical tests that usually are labor intensive, destructive, and still not 100% effective, given their dependence on human interpretation. In this paper, the authors present an alternative method using solid-phase microextraction (SPME) and comprehensive two-dimensional gas chromatography with electron ionization time-of-flight mass spectrometry (GCxGC/EI-TOF-MS) to chemically characterize electronic components. The novel approach outlined in this study utilized the high sensitivity of GCxGC/EI-TOF-MS systems to consistently identify more than 300 volatile markers in two different groups of 14-pin, 16-bit microcontrollers and one group of 16 GB microSDs from three different manufacturers. Results showed that the chemical characterization of the electronic components was effective, repeatable, and non-destructive. The significant volatile markers were found to be dependent on the manufacturer, age, and type of electronic component. Thus, the list of significant volatile markers of each electronic component could potentially serve as a basis for verifying the source of an electronic component. Compared to existing physical verification methods, this novel approach is faster and less ambiguous, and avoids the overlook or false identification of counterfeits. In fact, this methodology could potentially indicate if further inappropriate manipulations have taken place on the component, based on the presence and quantity of 1,3,5-cycloheptatriene, thereby reducing the risk of counterfeit components being installed in electronic systems. Using GCxGC/EI-TOF-MS and SPME techniques may be helpful in reducing the quantity of counterfeit electronic components found in warfare systems used by the United States Department of Defense (DoD), which is especially at risk, due to the rapid aging of deployed systems and a vast span of interconnected logistics supply chains that provide components for installation in such warfare systems.

This study aims to assess the relationship between nutritional status and cognitive decline in a population of non-institutionalized elderly patients evaluated in a geriatric outpatient setting. Data from 144 patients admitted to the general geriatric outpatient services of Luigi Vanvitelli University Hospital (Naples, Italy) and San Felice a Cancello Hospital (San Felice a Cancello, Italy) were collected between January 1 and December 31, 2017. Cognitive function was evaluated through the mini-mental state examination and nutritional status was assessed through mini nutritional assessment. Logistic regression was used to assess the association between clinical characteristics and nutritional status, and it is reported as unadjusted, adjusted for age and sex, and adjusted for other potential confounding factors. In the unadjusted model, poor nutritional status was associated with a worse cognitive deficit (odds ratio (OR)=2.36; 95% confidence interval (CI)=1.77-3.55). The association remained significant after adjusting for age and sex (OR=2.36; 95% CI=1.77-3.55) and in the fully adjusted model (OR=2.92; 95% CI=1.90-5.64). In patients with dementia, malnutrition represents a significant burden that grows heavier as the disease progresses.

BACKGROUND Occupational eMental Health (OeMH) interventions significantly reduce the burden of mental health conditions. However, the successful implementation of OeMH interventions is influenced by many implementation strategies, barriers and facilitators across contexts. Therefore, there is a need for practical guidance on the key implementation factors that organizations must address. Stakeholder consultations secure a valuable source of information on these key strategies, barriers and facilitators that are relevant to the successful implementation of OeMH interventions. OBJECTIVE The objective of this study was to develop a brief checklist to guide the implementation of OeMH interventions. METHODS We created a comprehensive checklist moving from a set of strategies, barriers, and facilitators relevant to the implementation of OeMH interventions that were identified in a recently published systematic review. We then relied on a two-stage stakeholder consultation process to reduce the comprehensive checklist to a brief checklist comprising key implementation factors. At the first stage, stakeholders evaluated the relevance and feasibility of items on the comprehensive checklist using a web-based survey. At the second stage, stakeholders were interviewed to elaborate on the most relevant barriers and facilitators shortlisted from the first stage. RESULTS Twenty-six stakeholders completed the web survey (response rate: 25%) and four stakeholders participated in individual interviews. The EMPOWER OeMH implementation checklist comprises 28 items including 9 strategies, 8 barriers and 11 facilitators. CONCLUSIONS Through our two-stage stakeholder consultation we developed a brief implementation checklist that provides organizations with a guide for the implementation of OeMH interventions. Future research should empirically validate the effectiveness and usefulness of the checklist.

Enteric glial cells (EGCs), the major components of the enteric nervous system (ENS), are implicated in the maintenance of gut homeostasis, thereby leading to severe pathological conditions when impaired. However, due to technical difficulties associated with EGCs isolation and cell culture maintenance that results in a lack of valuable in vitro models, their roles in physiological and pathological contexts have been poorly investigated so far. To this aim, we developed for the first time, a human immortalized EGC line (referred as ClK clone) through a validated lentiviral transgene protocol. As a result, ClK phenotypic glial features were confirmed by morphological and molecular evaluations, also providing the consensus karyotype and finely mapping the chromosomal rearrangements as well as HLA-related genotypes. Lastly, we investigated the ATP- and acetylcholine, serotonin and glutamate neurotransmitters mediated intracellular Ca2+ signaling activation and the response of EGCs markers (GFAP, SOX10, S100β, PLP1, and CCL2) upon inflammatory stimuli, further confirming the glial nature of the analyzed cells. Overall, this contribution provided a novel potential in vitro tool to finely characterize the EGCs behavior under physiological and pathological conditions in humans.

Metabolite profiling has significantly contributed to a deeper understanding of the biochemical metabolic networks and pathways in cancer cells. Metabolomics-based biomarker discovery would greatly benefit from the ability to interrogate retrospective annotated clinical specimens archived as formalin-fixed, paraffin-embedded (FFPE) material. Mass spectrometry–based metabolomic analysis was performed in matched frozen and FFPE human prostate cancers as well as isogenic prostate cancer cell lines. A total of 352 and 460 metabolites were profiled in human tissues and cell lines, respectively. Classes and physical–chemical characteristics of the metabolites preserved in FFPE material were characterized and related to their preservation or loss following fixation and embedding. Metabolite classes were differentially preserved in archival FFPE tissues, regardless of the age of the block, compared with matched frozen specimen, ranging from maximal preservation of fatty acids (78%) to loss of the majority of peptides and steroids. Generally, FFPE samples showed a decrease of metabolites with functional groups, such as carboxamide. As an adjunct technique, metabolic profiles were also obtained in situ from FFPE tissue sections where metabolites were extracted in a manner that preserves tissue architecture. Despite the fact that selected metabolites were not retained after processing, global metabolic profiles obtained from FFPE can be used to predict biologic states and study biologic pathways. These results pave the way for metabolomics-based biomarker discovery/validation utilizing retrospective and clinically annotated FFPE collections.Implications: Metabolic profiles can be performed in archival tissue and may be used to complement other profiling methods such as gene expression for biomarker discovery or pathway analysis in the assessment of biologic states. Introduction: The subcutaneous implantable cardioverter defibrillator (S-ICD) represents a major advancement in ICD technology. Inappropriate shocks (IAS) occur in more than 3.1% of the population with S-ICD each year and are usually followed by admission to the emergency department (ED). In this setting, the disabling of IAS is mandatory during a pseudo-electrical storm (ES). This report describes the strategies that can be followed in order to temporarily inhibit IAS in critical care settings with the use of magnets. Case presentation: An S-ICD was implanted more than 6 weeks prior to presentation in a 68-year-old man with hypertrophic cardiomyopathy. In the ED, the patient experienced 3 IAS in the last hour. A Medtronic magnet was applied to stop IAS, as the specific programmer was not available. The maneuver interrupted the IAS. In order to verify the universal magnetic response of the S-ICD, six different magnets and one smartphone with MagSafe technology were tested. All magnet models suspended arrhythmia detection and IAS, while the smartphone did not cause magnet interferences. Conclusions: This report demonstrates the safety and efficacy of all clinical magnet models in inhibiting IAS. In case of pseudo-ES, any type of magnet allows ED providers to easily and rapidly disable the functionality of the devices when appropriate.

Two new records of the tanaid species Zeuxo holdichi and Apseudopsis tridens are reported for the Venice lagoon. Soft bottom sediment samples were collected as part of a monitoring survey to evaluate the ecological status of the lagoon within the European Directive 2000/60/EC. Specimens of Z. holdichi and A. tridens were found in four and two sampling stations respectively, located in diferent parts of the lagoon. The tanaidomorph species Z. holdichi was previously reported for the Atlantic coasts of France, Spain, Portugal and for southwest England, while the apseudomorph tanaid A. tridens was described only for the Israeli coasts. Therefore, this study represents the frst record of Z. holdichi for the Mediterranean Sea and the frst observation of A. tridens in Italian waters. Despite the low intrinsic dispersal potential of these tanaid species, their native origin or their possible alien status in Italian waters is debated. The dubious native origin of Z. holdichi and the possible underestimation of both species’ presence in Italian Seas due to the lack of taxonomic specialists, attribute to these tanaids a cryptogenic nature. The ability of both species to cope with the diferent environmental parameters characterizing the lagoon may increase the probability of competition with other species that have similar ecological and trophic guilds. published
