As many older adults with progressive chronic conditions choose to age-in-place in assisted living (AL) communities, external healthcare workers (e.g., those who provide palliative care) increasingly support AL staff in caring for residents with complex health needs. Palliative care is a branch of healthcare dedicated to preserving quality of life by attending to the physical, mental, and spiritual needs of individuals with chronic, life-threatening diseases and is well suited to manage AL residents’ progressive medical conditions. However, AL residents and their care partners often face barriers to accessing palliative care. Using data from a larger 5-year NIA-funded study, we examined AL administrator knowledge and use of palliative care in seven AL communities around the Atlanta metropolitan area that were racially, ethnically, and socioeconomically diverse. Findings from thematic analysis of semi-structured interviews with 16 administrators indicated that 15 of 16 administrators were familiar with palliative care. A minority of administrators clearly distinguished palliative care from hospice services and conceptualized it as a “bridge” to hospice services. Administrators emphasized how palliative care assists communities in caring for health concerns in-house rather than having to send residents to the hospital. Despite their positive view of palliative care, administrators described infrequent use of palliative services in their communities. Findings show that although none of the AL communities integrate palliative care with their service offerings, AL administrators see value in palliative care for their residents. We provide recommendations for improving palliative care access and quality of life for AL residents at end of life.

Sustaining a Spinal Cord Injury at any point in time is life altering ? physically, emotionally, and financially ? for all persons affected by the injury, but it can place unique challenges on younger married couples. This study examines the transition to injury for 18 couples (ages 21-55). Data were collected using individual interviews with each partner at three time points following injury, as well as observation in the rehabilitation setting (Creekview). This resulted in 96 individual interviews and 300 hours of observation. Using a combination of the life course perspective and cognitive sociology as guiding theoretical frameworks and grounded theory analysis, I examined how the health care institution influenced the couples? relationships during their rehabilitation stay and the subsequent transition home. Overall, this study found that Creekview shaped a thought community that emphasizes a return to walking and high levels of physical recovery. Patients who achieved these goals constructed positive narratives about the future while those with lower levels of recovery constructed negative narratives over time. Additionally, because of the dominant medical narrative of wait and see regarding physical recovery, many respondents constructed fuzzy narratives about the future that reflect ambiguity about what life would look like following injury. Additionally, Creekview staff and couples accepted and reinforced the dominant cultural narrative that women are natural caregivers, but larger social structures of class, gender, and the division of paid and unpaid labor work together to push some women into caregiving faster or prevent other women from engaging in caregiving. Expanding on Aneshensel et al.?s (1995) caregiving career, this study examines how younger couples move through the caregiving career when the expected outcome is not long-term care placement or death. This study identified three main types of caregivers, each with their own path of caregiving ? Naturalized, Constrained, and Resistant caregivers. Overall, the transition to injury is complex for patients and partners and this study highlights some of the ways the marital relationship is affected by a non-normative, unexpected transition.

Sociologists have only recently paid attention to how men experience physical disability. However, current research continues to ignore how different racial groups experience it. The goal of this study was to examine how black and white men experience life with a physical disability. Using qualitative research techniques involving in-depth, face-to-face interviews with 10 black and 10 white men, I focus on how meanings of disability and masculinity shift after a traumatic injury. Using symbolic interactionism and social construction as theoretical frameworks, I examine how these men formed and modified meanings for disability and masculinity through social interactions. I also analyze the strategies they use to manage a stigmatized identity. Finally, I explore how they negotiate a masculine identity within larger social contexts. My findings suggest that black and white men?s constructions of masculinity and disability are more similar than different on all levels. Furthermore, these men used three strategies to negotiate their new social identities: reinforcing idealized masculinity, modified masculinity, and lost masculinity.
