The antiviral state, an initial line of defense against viral infection, is established by a set of IFN-stimulated genes (ISGs) encoding antiviral effector proteins. The effector ISGs are transcriptionally regulated by type I IFNs mainly via activation of IFN-stimulated gene factor 3 (ISGF3). In this study, the regulatory elements of effector ISGs were characterized to determine the (epi)genetic features that enable their robust induction by type I IFNs in multiple cell types. We determined the location of regulatory elements, the DNA motifs, the occupancy of ISGF3 subunits (IRF9, STAT1, and STAT2) and other transcription factors, and the chromatin accessibility of 37 effector ISGs in murine dendritic cells. The IFN-stimulated response element (ISRE) and its tripartite version occurred most frequently in the regulatory elements of effector ISGs than in any other tested ISG subsets. Chromatin accessibility at their promoter regions was similar to most other ISGs but higher than at the promoters of inflammation-related cytokines, which were used as a reference gene set. Most effector ISGs (81.1%) had at least one ISGF3 binding region proximal to the transcription start site (TSS), and only a subset of effector ISGs (24.3%) was associated with three or more ISGF3 binding regions. The IRF9 signals were typically higher, and ISRE motifs were “stronger” (more similar to the canonical sequence) in TSS-proximal versus TSS-distal regulatory regions. Moreover, most TSS-proximal regulatory regions were accessible before stimulation in multiple cell types. Our results indicate that “strong” ISRE motifs and universally accessible promoter regions that permit robust, widespread induction are characteristic features of effector ISGs.

A közlekedési infrastruktúra és az azon közlekedő járművek a terroristák ideális célpontjai lehetnek. Ez egyaránt igaz a vasúti közlekedésre és magukra a vonatokra is, különösen a veszélyes árut szállító vonatokra. Egy esetleges terrorakció következtében kiszabaduló veszélyes anyagok sokkal nagyobb pusztítást tudnak végezni mind emberéletben, mind pedig anyagi és természeti értékben, mint a vonat felrobbantásával okozható kár. A cikkben bemutatjuk a veszélyes anyagok szállításának vasúti környezetét, majd pedig a terrorizmus és a vasúti közlekedés kapcsolatát, végül javaslatot teszünk a lehetséges védelmi módszerekre és eljárásokra.

Bevezetés: A COVID–19-pandémia egyik leginkább veszélyeztetett csoportja a 60 év felettiek populációja, hiszen sokszor krónikus betegségekkel élőként, szociálisan és technikailag is izolálódva kell megküzdeniük a világjárvány kihívásaival. Célkitűzés: Kutatásunk fő célja a pandémia direkt és indirekt hatásának elemzése a 60 év feletti magyar lakosságra vonatkozóan. Módszer: Vizsgálatunkat 60 év fölötti magyar állampolgároknál végeztük. A felméréshez a „World-Wide FINGERS SARS-CoV-2 Survey” hiteles magyar fordítását használtuk. Az adatrögzítést 2021. február 1. és 2021. június 1. között végeztük. Résztvevőnként egy alkalommal történt kérdőíves adatfelvétel. Eredmények: Vizsgálatunkban 431 fő vett részt, körükben a COVID–19-fertőzés aránya alacsony volt (6%). A legmarkánsabb változások az életmód tekintetében, hogy a válaszadók 71%-ánál emelkedett a digitális szolgáltatások használata, 47%-ánál romlott a szubjektív alvásminőség, 46%-ánál fokozódott a magányosság érzete, és 80%-ánál csökkent a barátokkal és rokonokkal történő kapcsolattartás. A résztvevők 86%-ának legalább egy krónikus betegsége volt, és a pandémia alatt 23%-uknál elmaradt a betegséghez kapcsolódó orvosi vizit. A válaszadók 45%-ánál romlott a szubjektív életminőség megítélése, és 25% szubjektív memóriazavarok megjelenését jelentette. Megbeszélés: A vizsgáltak körében a pszichoszociális életben jelentős romlás figyelhető meg. A résztvevők elszigetelődtek szociálisan a pandémia alatt, és ez jelentősen kihatott életvezetésükre is. A fizikális és mentális egészség tapasztalható változásai feltehetően a későbbiekben megmutatkoznak majd az idősebb korosztályra jellemző népbetegségek fokozottabb megjelenésében és progressziójuk gyorsulásában. Következtetés: A COVID–19-világjárvány direkt és indirekt káros hatásainak mérsékléséhez kiemelt jelentőségű annak ismerete, hogy a pandémia, illetve az annak megfékezésére tett intézkedések hogyan befolyásolják az idősek viselkedését, életmódját, valamint a krónikus betegek ellátását, gondozását. Az állami űrtevékenységek lassabban reagáltak erre a változásra, de mára ezek a miniatürizált űreszközök megjelentek a legnagyobb űrhatalmak katonai rendszereiben is. A tanulmány áttekinti a mikro- és nanoműholdak technikai jellemzőit, felépítését, a lehetséges hasznos terhek típusait és a pályára állítás sajátosságait.

Bár a Covid–19 elleni küzdelem során az államok világszerte számos alapjog-korlátozó intézkedést hoztak, a védőoltások felvételére kötelezés az egyik legmarkánsabb alapjogi beavatkozásnak minősült, felvetve – többek között – az egészségügyi önrendelkezés jog, az egészséghez (testi épséghez) való jog, a munkához való jog és a világnézeti-lelkiismereti szabadság sérelmét. A tanulmány a kötelező Covid-oltás tárgyában született magyar, német és osztrák alkotmánybírósági határozatot tárgyalja, a hangsúlyt a két külföldi döntésre helyezve. A határozatok alapos elemzése mellett a másik alapvető célkitűzésünk a három alkotmánybíróság érvelésének összevetése, arra keresve a választ, hogy az egyes testületek milyen okfejtés mentén látták igazolhatónak a kötelező oltások által okozott alapjogkorlátozást. Amint látni fogjuk, az osztrák alkotmánybíróság határozatában elvégzett szükségességi-arányossági teszt meglehetősen sematikusnak tűnik, és mind a magyar, mind a német határozatokhoz képest elnagyoltnak, kevésbé módszeresnek tekinthető. Ehhez képest a német alkotmánybírósági határozat a másik „végletet” képviseli, ugyanis a szükségességi-arányossági tesztet megítélésünk szerint rendkívül precíz módon, részletesen, az egyes vizsgálati szempontokat világosan elhatárolva végezte el. Ezek alapján elsősorban a magyar és a német döntések vethetőek össze, rámutatva, hogy az alapjogi teszt során a szükségesség kérdéshez eltérő módon közelített a két testület, míg az arányosság vizsgálatánál hasonló szempontokra alapították az érvelésüket. A tanulmányból az is kitűnik, hogy az osztrák, de főleg a német határozatban jóval nagyobb hangsúlyt kapott a – némileg leegyszerűsítve – mások életére történő hivatkozás, azaz a problémakör társadalmi dimenziója, mint a hazai döntésben.

In the period between AD 1540 and 1690, Transylvania enjoyed a high degree of independence in conducting its internal and also, at times, external affairs. This led to the divergence of Transylvanian private law from that of the Kingdom of Hungary, the sovereignty of which ceased in the sense of international law following the defeat at the Battle of Mohács. This divergent development is examined in the present study from the perspective of private law along with the later convergence of legal norms to those of the Habsburg Monarchy during the latter half of the 17th century. The sources of private law as well as private law norms governing the status of persons, immovable and movable property, obligations, and inheritance are examined in detail for this period. The specific laws applicable to the Szekler, Saxon, and Romanian inhabitants of Transylvania are also presented.

High Altitude Platform Systems (HAPS)3, or pseudo satellites, are atmospheric, specifically, stratospheric Remotely Piloted Aerial Systems (RPAS).4 These systems can provide services comparable to outer space satellite systems, however, they can be operated without the need of an orbital launch capability. While their geographical coverage is limited compared to space satellites, they can provide persistent coverage over a given area for a long time, weeks or even months. Pseudo satellites can carry Earth observation or radiocommunication payloads, just like satellites.In this article I will provide an overview of HAPS capabilities and potential applications, based on an imaginary operational scenario, namely, the reconnaissance support of a law enforcement operation to defend offshore gas facilities from a planned attack by an eco-terrorist group near Venezuela. At the same time, HAPS can support any defence, emergency response or natural resource observation operation.The complex air–sea operation scenario will be presented using a simulation generated with the AGI Systems ToolKit multi-domain mission simulator software. Alzheimer’s disease is the leading cause of cognitive deterioration. Alois Alzheimer described it for the first time in 1907. In the last five decades, there were emerging numerous hypotheses about the pathomechanism of this condition. The first one, the cholinergic hypothesis resulted in important antidementia medications. Amyloid cascade theory set the direction for many years in Alzheimer research. Currently, it also contributed to the licencing of a novel pharmaceutical Aducanumab. Additionally, the latest hypotheses shed light on the new aspects of the pathology of dementia. This study emphasizes the historical landmarks of Alzheimer’s research and reveals their interactions as well.

With the ever-increasing capabilities of the smallest remote sensing satellites, a serious bottleneck is encountered at the space-ground interface. While the satellites are capable of collecting data, downlinking said data is not always straightforward. Analysis of the satellite orbits show that the most beneficial downlink station locations can be found in the polar region. This article introduces the reader to the typical Earth observation orbits, their effects on the data communication periods and describes a possible nanosatellite operational radiocommunication network.

Az iskolai tanulás teljesen új megközelítését adja az OECD „Iránytű a tanuláshoz” című anyaga azzal, hogy célként a well-being, a jóllét elérését jelöli meg. Úgy látják a szerzők, hogy a jövő komplex és bizonytalan világának ismeretlen feltételrendszeréhez történő alkalmazkodáshoz az úgynevezett transzformatív kompetenciákra van szükség, amelynek kialakítása és működtetése vezet a well-being eléréséhez. Az aktív tanulás folyamatában jelentős funkciót tulajdonítanak a pedagógusoknak a nemzetközi oktatáspolitikai vizsgálatok alapján, egyúttal jelzik, hogy milyen változásokra van szükség a pedagógusszerepben. Ennek a megváltozott pedagógusszerepnek a megvalósításában meghatározó tényező a pedagógusok jólléte. Ennek bizonyításául a pedagógusnők – hiszen ők vannak többségben a pályán – foglalkozási és pszichológiai jóllétének vizsgálatairól hozunk példákat azt követően, hogy számos jóllét-értelmezést vázolunk fel. Az oktatásügy előtt álló feladat megoldásához érdemes áttekinteni, hogy milyen értelmezései, dimenziói vannak a jóllétnek; hogyan lehet ezeket párbeszédbe állítani, a kutatási eredményeket minél jobban felhasználni a fogalom pontos és gazdag értelmezéséhez úgy, hogy az a mindennapi tevékenységek számára operacionalizálható legyen. Ehhez mutatunk be különböző megközelítéseket. Hangsúlyozzuk, hogy a pedagógusnők jólléte szoros kapcsolatot mutat a tanulók jóllétével, tehát a pedagógusok foglalkozási és pszichológiai jóllétének támogatása a minőségi tanulás-tanítás folyamatához, a 2030-ra kitűzött cél eléréséhez elengedhetetlen.

A technológia fejlődése mára lehetővé tette, hogy a légköri repülés határait új magasságokba emeljük. A sztratoszférarepülő, magaslégköri, személyzet nélküli repülőeszközök szolgáltatásai már-már műholdszerűek, ezért kapták egyik közkeletű nevüket is: pszeudoműholdak. Számos eltérést találunk azonban az űrrepülő műholdak és a magaslégköri repülő platformok, illetve a hosszú őrjáratozási idejű, de alacsonyabb repülési pályán működő, pilóta nélküli eszközök alkalmazhatósága és szolgáltatásai között. Ez a tanulmány egy valószerű műveleti szcenárió alapján bemutatja a sztratoszférarepülő pszeudoműholdakat és összehasonlítja azokat a megjelölt két másik elterjedt művelettámogató platformmal.

Az üzleti célú műholdas távközlési rendszerekben az utóbbi években több olyan műszaki megoldás jelent meg, amelyek a védelmi és biztonsági célú – ezen belül a katonai vezetést támogató – műholdas távközlési rendszerekben is komoly hozzáadott értékkel rendelkeznek. Lehetővé teszik a mozgás közbeni folyamatos, nagy sebességű adatátvitelt, növelik a természetes és emberi eredetű rádiózavarokkal (ideértve a szándékos ártó zavarást is) szembeni védettséget és biztosítják a megszokottól eltérő műholdpályák alkalmazásának lehetőségét. A tanulmány az Új Nemzeti Kiválóság Program támogatásával készült.

A geostacionárius pálya jelenleg a műholdas távközlési rendszerekben legelterjedtebben alkalmazott műholdpálya. Ennek elsősorban műszaki okai vannak. Ezt a pályát azonban sok technikai és jogi kihívás fenyegeti. Erre a múltban több példát láttunk, és a jövőben a helyzet várhatóan romlik. Fel kell tehát készülni más pályatartományok alkalmazására. Ez a tanulmány megoldási lehetőségként a geoszinkron pályamagasság feletti, úgynevezett szuperszinkron pályatartomány alkalmazását mutatja be. András II (1205-1235) imposed new taxes to be able to support his lavish lifestyle, but he mostly outsourced the collection of these taxes. To increase the number of his allies despite the general discontent, the king gave away royal estates one after another. In response to the protests of the royal servants and the lords on Imre’s side, András II issued the Golden Bull of 1222, in which he promised to abide by the law and to abandon his lavish lifestyle. The Golden Bull was named after the pendant seal that authenticated the charter. From the time of King Béla II onwards, the king used a gold seal on all important documents, but only the Golden Bull of 1222 is usually written with capital letters. It should be added that our historiography also mentions two other golden bulls: the second one was issued by András II in 1231 at the request of the high priests and two of his sons. The third golden bull was issued by King Béla IV, together with his two sons, Stephen the Younger and Béla, Prince of Slavonia.

“The executive power is the moving force of a government. It represents, in the political system, that mysterious principle which, in moral man, unites action to the will”. Although more than 200 years have passed since Jacques Necker, the finance minister for Louis XVI, completed his essay on the executive power, his definition still holds in many respects. However, the term ‘executive’ as the name of a branch of government may be misleading. As Vile notes, the executive “gets its name from one of its major functions, that of putting the law into effect”. It goes without saying that the executive means more than just implementing laws passed by the legislature. Although explaining the concept of the executive and enumerating its task would greatly exceed the scope of this chapter, it is worth recalling some thoughts of Mansfield, who claimed in his seminal work on modern executive power that it is the executive power that made the principle of separation of powers workable. In his view, “for executive power, always ready for emergency, ensured that the power of government was not diminished, much less stalemated, when it was separated into three branches”. His volume revolves around the idea of ambivalence of the modern executive: “The beauty of executive power, then, is to be both subordinate and not subordinate, both weak and strong. It can reach where law cannot, and thus supply the defect of law, yet remain subordinate to law”. The chapter unfold as follows (since the current volume devotes a whole chapter to the powers of the heads of state, this chapter focuses on governments): in the first section, we outline the structure of the executive power in the eight examined states. The second section is devoted to the governments as key actors of the executive power, dealing with their status, composition, competencies and formation while paying special attention to the question of responsibility. The third section briefly outlines the territorial level of the executive.

Solubility parameters are widely used in the polymer industry and are often applied in the high pressure field as well as they give the possibility of combining the effects of all operational parameters on solubility in a single term. We demonstrate a statistical methodology to apply solubility parameters in constructing a model to describe antisolvent fractionation based chiral resolution, which is a complex process including a chemical equilibrium, precipitation and extraction as well. The solubility parameter used in this article, is the Hansen parameter. The evaluation of experimental results of resolution and crystallization of ibuprofen with (R)-phenylethylamine based on diastereomeric salt formation by gas antisolvent fractionation method was carried out. Two sets of experiments were performed, one with methanol as organic solvent in an undesigned experiment and one with ethanol in a designed experiment. The utilization of D-optimal design in order to decrease the necessary number of experiments and to overcome the problem of constrained design space was demonstrated. Linear models including dependence of pressure, temperature and the solubility parameter were appropriate to describe the selectivity of the GASF optical resolution method in both sets of experiments.

Objectives To follow-up the radiographic bone level changes and the clinical outcomes of immediately provisionalized and conventionally restored implants with a hydrophilic surface following 5 years of function. Materials and methods This was a 5-year follow-up of a prospective, randomized, single-blind controlled study involving 16 of the 24 originally recruited patients in need of a single-tooth replacement in the esthetic area. Implants were either immediately provisionalized with a non-occluding temporary crown (test group, n = 7), or left without a crown (control group, n = 9). In both groups, the definitive restoration was placed 16 weeks after implant placement. Radiographic and clinical parameters were evaluated at 36, 48, and 60 months post-implant placement, together with implant survival and success rates. The esthetic outcomes were measured with the Papilla Fill Index (PFI) and the Pink Esthetic Score (PES). Results At 60 months, similar peri-implant bone loss was observed in the test (-0.42 mm ±0.17 mm) and in the control (-0.37 mm ±0.35 mm) groups. A tendency for an improved esthetic outcome from implant loading to the subsequent follow-ups was noticed in both groups. Both groups presented with high levels of long-term implant survival and success. Conclusions This study supports non-functional immediate provisionalization as a viable long-term option for the management of single-tooth implants in the esthetic area. However, the small sample size does not allow statistical inference at 60 months of follow-up and future adequately powered studies are warranted.

Debrecen’s marshalling yard and railway facilities were dealt with an over whelming air raid by the US Air Force on 2 June 1944. As the result of the attack, the town also sustained major damage. However, out of the keyparts of the industrial and transport infrastructure, only Debrecen’s transport company sustained irreparable damage. In my study, with the help of archive resources, I represent the lines and sites of the Debrecen Local Railway and the Debrecen Nyírbátor Railway Public Limited Company, the catastrophic bombing damage of 2 June 1944, the aftermath of the bombing, and the steps take into restart transport.

This paper presents the outer space as a military operational domain, describes the various means an adversary could affect our space capabilities, and also our waysto protect them.

Nationale Symbole sind in einem jeden Staat wesentliche Kennzeichen des Zusammenhalts und der gemeinsamen Geschichte, und spielen daher auch bezuglich des Nationalbewusstseins der Landesbevolkerung eine wichtige Rolle. Es ist nicht von ungefahr, dass sie nicht nur moralisch, sondern auch juristisch geschutzt werden. Im Laufe der mehr als tausendjahrigen Geschichte des ungarischen Staates etablierten sich mehrere nationale Symbole, die teilweise auch in Ungarns internationalen Beziehungen zum Warenzeichen des Landes wurden.

This article investigates the impact of electoral reforms on entry barriers in political markets. The discussion starts by delineating the theoretical boundaries of various political markets, such as those for participation, parties and government. By taking a cue from industrial organisation theory, the article offers an analysis of entry barriers, both hard and soft, along with their operationalisation for empirical research. Based on this theoretical framework, a single hypothesis is investigated. It posits that the modification of the entry barriers in the market for parties leads to changes in the concentration of the popular vote for party lists. An observable implication of this relationship would be if an electoral reform that raises entry barriers led to subsequent increases in the Herfindahl index (a measure of market concentration), and vice versa. This proposition is empirically tested by a comparative analysis of a new database covering Czechia, Hungary, Poland and Slovakia. The analysis offers support for the following proposition: in most cases the changes in the entry barriers led to a corresponding change of concentration in the market for parties.

Attila Horváth and Elvira H. Tóth excavated a relatively small Avar burial site of 68 graves at Szabadszállás between 1972 and 1975. Based on the findings the site was in use between the second half of the 7th century and the middle of the 8th century. Most of the graves have been robbed or with a few grave goods. However, during the 1974 works, a dual mount was found among the belt mounts with griffins of the no 34 undisturbed male burial. Only one similar piece is known in the whole country.

In a dictatorship of the kind built by the Soviets, power was exercised in every sector in the form of brutal, blatant and uncontrolled governance. The ideology termed, in most places, Marxism–Leninism was imposed on the people as a kind of a “state religion”. From nurseries to universities, from adult education to the media, the official doctrines were hammered into people’s heads and claimed to give answers to just about any question. The Communist Party wished to use schools to create obedient citizens. The 1950 curriculum set the objective of “teaching pupils to become conscious, disciplined citizens of the People’s Republic, who are loyal to the working class and build Socialism.”

Ha a pedagógus etikával, vagyis a szaketikák egyik képviselőjével kívánunk foglalkozni, a vizsgálat rögtön nehézségekbe ütközik. Nem csak a pedagógus etika hazai szakirodalma mondható csekélynek, de ugyanez a megállapítás érvényes az általános szaketikákkal foglalkozó művekre is. Súlyos hiányosság a szakirodalmi kidolgozatlanság, hiszen az érvényben lévő pedagógus etikai kódex(ek) vizsgálatához elengedhetetlen az elméleti háttér ismerete, ami feltehetőleg nem csak az adott szakma megállapításaiból táplálkozik, hanem egyéb szakmák etikai diskurzusaiból is, melyek már sokkal régebben vizsgálják saját tevékenységük erkölcsi aspektusait.

The dried flowers of Chamaemelum nobile (L.) All. have been used in traditional medicine for different conditions related to the spasm of the gastrointestinal system. However, there have been no experimental studies to support the smooth muscle relaxant effect of this plant. The aim of our research was to assess the effects of the hydroethanolic extract of Roman chamomile, its fractions, four of its flavonoids (apigenin, luteolin, hispidulin, and eupafolin), and its essential oil on smooth muscles. The phytochemical compositions of the extract and its fractions were characterized and quantified by HPLC-DAD, the essential oil was characterized by GC and GC-MS. Neuronally mediated and smooth muscle effects were tested in isolated organ bath experiments on guinea pig, rat, and human smooth muscle preparations. The crude herbal extract induced an immediate, moderate, and transient contraction of guinea pig ileum via the activation of cholinergic neurons of the gut wall. Purinoceptor and serotonin receptor antagonists did not influence this effect. The more sustained relaxant effect of the extract, measured after pre-contraction of the preparations, was remarkable and was not affected by an adrenergic beta receptor antagonist. The smooth muscle-relaxant activity was found to be associated with the flavonoid content of the fractions. The essential oil showed only the relaxant effect, but no contracting activity. The smooth muscle-relaxant effect was also detected on rat gastrointestinal tissues, as well as on strip preparations of human small intestine. These results suggest that Roman chamomile extract has a direct and prolonged smooth muscle-relaxant effect on guinea pig ileum which is related to its flavonoid content. In some preparations, a transient stimulation of enteric cholinergic motoneurons was also detected. The essential oil also had a remarkable smooth muscle relaxant effect in this setting. Similar relaxant effects were also detected on other visceral preparations, including human jejunum. This is the first report on the activity of Roman chamomile on smooth muscles that may reassure the rationale of the traditional use of this plant in spasmodic gastrointestinal disorders.

Summary Background Chronic inflammation is believed to be a major mechanism underlying the pathophysiology of type 2 diabetes. Periodontitis is a cause of systemic inflammation. We aimed to assess the effects of periodontal treatment on glycaemic control in people with type 2 diabetes. Methods In this 12 month, single-centre, parallel-group, investigator-masked, randomised trial, we recruited patients with type 2 diabetes, moderate-to-severe periodontitis, and at least 15 teeth from four local hospitals and 15 medical or dental practices in the UK. We randomly assigned patients (1:1) using a computer-generated table to receive intensive periodontal treatment (IPT; whole mouth subgingival scaling, surgical periodontal therapy [if the participants showed good oral hygiene practice; otherwise dental cleaning again], and supportive periodontal therapy every 3 months until completion of the study) or control periodontal treatment (CPT; supra-gingival scaling and polishing at the same timepoints as in the IPT group). Treatment allocation included a process of minimisation in terms of diabetes onset, smoking status, sex, and periodontitis severity. Allocation to treatment was concealed in an opaque envelope and revealed to the clinician on the day of first treatment. With the exception of dental staff who performed the treatment and clinical examinations, all study investigators were masked to group allocation. The primary outcome was between-group difference in HbA 1c at 12 months in the intention-to-treat population. This study is registered with the ISRCTN registry, number ISRCTN83229304. Findings Between Oct 1, 2008, and Oct 31, 2012, we randomly assigned 264 patients to IPT (n=133) or CPT (n=131), all of whom were included in the intention-to-treat population. At baseline, mean HbA 1c was 8·1% (SD 1·7) in both groups. After 12 months, unadjusted mean HbA 1c was 8·3% (SE 0·2) in the CPT group and 7·8% (0·2) in the IPT group; with adjustment for baseline HbA 1c , age, sex, ethnicity, smoking status, duration of diabetes, and BMI, HbA 1c was 0·6% (95% CI 0·3–0·9; p Interpretation Compared with CPT, IPT reduced HbA 1c in patients with type 2 diabetes and moderate-to-severe periodontitis after 12 months. These results suggest that routine oral health assessment and treatment of periodontitis could be important for effective management of type 2 diabetes. The dried flowers of Chamaemelum nobile (L.) All. have been used in traditional medicine for different conditions related to the spasm of the gastrointestinal system. However, there have been no experimental studies to support the smooth muscle relaxant effect of this plant. The aim of our research was to assess the effects of the hydroethanolic extract of Roman chamomile, its fractions, four of its flavonoids (apigenin, luteolin, hispidulin, and eupafolin), and its essential oil on smooth muscles. The phytochemical compositions of the extract and its fractions were characterized and quantified by HPLC-DAD, the essential oil was characterized by GC and GC-MS. Neuronally mediated and smooth muscle effects were tested in isolated organ bath experiments on guinea pig, rat, and human smooth muscle preparations. The crude herbal extract induced an immediate, moderate, and transient contraction of guinea pig ileum via the activation of cholinergic neurons of the gut wall. Purinoceptor and serotonin receptor antagonists did not influence this effect. The more sustained relaxant effect of the extract, measured after pre-contraction of the preparations, was remarkable and was not affected by an adrenergic beta receptor antagonist. The smooth muscle-relaxant activity was found to be associated with the flavonoid content of the fractions. The essential oil showed only the relaxant effect, but no contracting activity. The smooth muscle-relaxant effect was also detected on rat gastrointestinal tissues, as well as on strip preparations of human small intestine. These results suggest that Roman chamomile extract has a direct and prolonged smooth muscle-relaxant effect on guinea pig ileum which is related to its flavonoid content. In some preparations, a transient stimulation of enteric cholinergic motoneurons was also detected. The essential oil also had a remarkable smooth muscle relaxant effect in this setting. Similar relaxant effects were also detected on other visceral preparations, including human jejunum. This is the first report on the activity of Roman chamomile on smooth muscles that may reassure the rationale of the traditional use of this plant in spasmodic gastrointestinal disorders.

Cél: Jelen klinikai vizsgálat célja, hogy nem kárieszes cervikális lézióval (NCCL) társuló Miller I–II típusú tükörszimmetrikus ínyrecessziók műtéti korrekciója során megvizsgáljuk, hogy egy nanotechnológiás polimerlakkal fedett üvegionomer tömőanyag (GIC) milyen sikerrel használható NCCL komplex sebészi-restauratív ellátásában. Továbbá vizsgáltuk, hogy a kötőszöveti graft (SCTG), illetve zománc mátrix derivátum (EMD) használata javít-e az eredményeken.Anyag és módszer: 20 parodontálisan egészséges páciens, 40 tükörszimmetrikusan elhelyezkedő NCCL-val rendelkező Miller I–II típusú ínyrecesszióját vizsgáltuk. A főbb vizsgálati paraméterek a recesszió mélysége (AGR) és szélessége (IRSZ), klinikai tapadási nívó (CAL), tasakmélység (PD), ínyvérzési index (BOP) és szubjektív fognyaki érzékenység (DE) voltak. A pácienseket elektronikus randomizációval műtét előtt két csoportra osztottuk annak eldöntésére, hogya mindkét oldalon alkalmazott módosított koronálisan elcsúsztatott lebenytechnikát (MCAF) SCTG-tal (MCAT-SCTG csoport), vagy EMD-mal (MCAT-EMD csoport) egészítsük ki. További elektronikus randomizáció alapján a töméssel ellátandó oldalakat választottuk ki. A teszt oldalon (T) Equia® (GC Europe, Leuven, Belgium) tömőanyag került behelyezésre, míg a kontroll oldalon az abradált dentin felszínén csupán gyökérsimítást végeztünk.Eredmények: Műtétek után hat hónap elteltével mindkét sebészi technika a T és K fogaknál is statisztikailag szignifikánsan csökkentette az AGR-t és IRSZ-t és javította a CAL-t (p ≤ 0,05). A fognyaki fedés mértéke az MCAF-SCTG csoportban közel megegyezett a T (76,92 ±19,23%) és a K oldalon (73,07 ±21,93%). Az MCAF+EMD csoportban viszont a T oldal (40,0 ±32,0%) elmaradt a K oldal (66,66 ±35,0%) értékeitől. A BOP értékek statisztikailag szignifikáns mértékben emelkedtek T oldali fogaknál a K oldalhoz képest, sebészi terápiától függetlenül. A PD értékek romlottaka T oldalon a K oldalhoz képest, sőt a PD növekedés statisztikailag szignifikáns mértéket öltött a SCTG csoportban az EMD csoporthoz képest. A DE minden csoportban statisztikailag szignifikánsan csökkent, kivéve az MCAF-EMD K csoportját, ahol a csökkenés nem volt szignifikáns.Következtetések: Vizsgálati eredményeinket összefoglalva megállapíthatjuk, hogy ínyrecesszió fedésére töméssel vagy tömés nélkül mindkét sebészi módszer alkalmas volt. A fognyaki érzékenység csökkentésére az MCAF-EMD tömés nélkül nem elegendő. A vizsgált nanotöltésű polimerlakkal fedett GIC alkalmazása bármelyik műtéti technika kiegészítéseként jelentősen csökkentette a fognyaki érzékenységet, ugyanakkor szubgingivális kiterjesztése nagyobb szondázási tasakmélységgel és gyulladásra utaló magasabb ínyvérzési értékekkel társul.

Objectives To compare the radiographic bone changes, clinical parameters and aesthetic outcomes of immediately provisionalised and conventionally restored implants at 12- and 24-months post-implant placement. Material and methods In 24 patients, 24 bone level implants with a hydrophilic (SLActive) surface were placed in healed sites and they were either immediately provisionalised with a non-occluding temporary crown (test group) or left without a crown (control group). In both groups, the definitive restoration was placed 16 weeks after implant placement. Clinical and radiographic parameters were calculated at 12- and 24-months post-implant placement, together with implant success/survival rates according to three different sets of criteria. The aesthetic outcome was evaluated through the Papilla Fill Index and the Pink Aesthetic Score. Results The mean marginal bone loss at 1 year was −0.73 mm (SD 0.83 mm) in the test group and −0.22 mm (SD 0.46 mm) in the control group (p > .05). Whilst 100% survival rate and positive aesthetic outcomes were recorded in both groups, three patients of the test group did not fulfil all success criteria. Conclusions Immediate provisionalisation may represent a viable option for the replacement of single missing teeth, with radiographic, clinical and aesthetic results comparable to those of conventionally loaded implants at 2 years of follow-up.

A kúszó parodontális tapadás (creeping attachment) jelenségét az irodalomban leírták mind epitelializált (eCTG), mindpedig szubepiteliális kötőszöveti autograft (sCTG) műtétek után. A jelen esetsorozat célja volt a kúszó tapadás jelenségétbemutatni egy friss eset részletes tárgyalásával, valamint néhány évtizeden keresztül követett esetben. Az elsőeset egy magasan tapadó frenulummal társuló, mély, Miller III. osztályú ínyrecesszió szabad eCTG-vel történő korrekciója,első lépésben vesztibulum-mélyítéssel. A műtét nemcsak a vesztibulum mélyülését eredményezte, hanem a 3 évesposztoperatív szakaszban a recesszió 95%-os fedése és jelentős keratinizált íny(KG)-szélesedés és biotípusváltás volttapasztalható. A második eset egy Miller II. osztályú, nagy kiterjedésű cervikális abrázióval társuló ínyrecesszió ellátásátköveti, amelyben az sCTG műtét után 2 hónappal még csak 60%-os gyökérfedést produkált. 3 éves követés soránközel 95%-os fedés és jelentős biotípus-váltás következett be. A 3. és 4. eset, amelyből az első egy sCTG, a másodikegy hagyományos eCTG érését mutatja be több év távlatából. A közvetlen posztoperatív szuboptimális eredményeka 10 éves követéses periódusban jelentős javulást mutattak, így a Miller II. osztályú recesszió esetében teljes gyökérfedést,a másikban (Miller III) pedig közel teljes gyökérfedést és a KG jelentős apikális irányú kiterjedését tapasztaltuk.A kúszó tapadás jelensége gyakran előfordul, de az irodalommal ellentétben a szerzők tapasztalata szerint a folyamatnem korlátozódik csupán a posztoperatív első két évre, hanem folyamatosan változik és javul az évek során. UgyanakkorMiller III. típusú defektusnál a teljes gyökérfedés azonban még évek múltán sem prognosztizálható.
