ADHERENCIA AL PROTOCOLO DE VIGILANCIA ACTIVA EN SALAMANCA: 2014-2020 y DESCRIPTIVO DE LOS PACIENTES, influenciado por el Covid 19., [ES]El cáncer de próstata es uno de los tumores que más afecta a los varones de la cuidad de Salamanca, para subsanar esta enfermedad se ha apostado por Vigilancia Activa, una alternativa a la cirugía para aquellos pacientes con bajo riesgos. A esta patología hay que sumarle algunos factores que influyen en la misma como la ansiedad, su calidad de vida ,para ello se ha pensado en la incorporación de una enfermera gestora de los casos uro-oncológicos, para valorar a los pacientes, servir de enlace entre paciente y equipo de trabajo, de esta manera se puede mejorar de manera óptima la atención sanitaria de los pacientes.

Backgrounds Patient-reported outcome measurements are important for urinary incontinence. We analyze self-assessed patient satisfaction and defne the clinical profle of patient with highest satisfaction with the adjustable transobturator male system (ATOMS). Methods Patient perception of results was evaluated in a series of 181 patients after ATOMS adjustment. Baseline incontinence severity was defned in pads-per-day (PPD) as mild (2), moderate (3–5) or severe (≥6), and dryness as use of none or one security PPD. Post-operative pain at discharge was evaluated by 0–10 visual analogue scale and complications by Clavien–Dindo classifcation. Multivariate analysis was performed to anticipate “very much better” than baseline perception on patient global impression of improvement and a predictive nomogram was developed. Results Dryness was achieved in 80.7% (94.9% mild, 80.8% moderate and 65.8% severe groups). Introduction Adjustable transobturator male system (ATOMS) is a surgical device developed to treat male stress urinary incontinence (SUI) after prostate surgery. The objective was to assess the effectiveness of the ATOMS device to treat male SUI as described in the literature. Methods Two independent reviewers identified studies eligible for a systematic review and meta-analysis of various sources written in English, German and Spanish, using the databases PubMed, EMBASE and Web of Science. We excluded studies on female incontinence. We employed the DerSimonian and Laird method for defining heterogeneity, calculating the grouped standard mean deviation (SMD). The primary objective of this review is the assessment of clinical efficacy based on the achievement of dryness after device adjustment, defined as use of no pad or one safety pad per day (PPD). The secondary objective was focused on analysing improvement of incontinence with the device. Magnitude of effect was calculated by analysing decrease in pad count (PPD) and/or in 24-h pad test. Number and severity of complications according to Clavien–Dindo classification were also reviewed. Results The pooled data of 1393 patients from 20 studies (13 retrospective and 7 prospective) showed that treatment with ATOMS resulted in a mean 67% dryness rate and 90% improvement after adjustment. Mean total number of system fillings per patient was 2.4. Mean pad count and 24-h pad test decrease were − 4.14 PPD and − 443 cc, respectively. Objetivo: Describir las tendencias en el manejo actual de la litiasis urinaria en el entorno de la Confederación Americana de Urología (CAU) para reconocer patrones de práctica clínica e identificar necesidades educativas. Material y método: Encuesta en línea compuesta por 31 preguntas de respuesta múltiple (español y portugués) a través www.caunet.org revelando datos demográficos, patrones de práctica clínica y actitudes ante situaciones clínicas específicas. Resultados: Cuatrocientos sesenta y tres profesionales procedentes de Brasil, México, Argentina, España, Colombia, Chile y otros (3,96% de la membresía CAU) completaron la encuesta. Todos los participantes practican algún tipo de tratamiento para la litiasis urinaria: 98,5% ureteroscopia (URS) semirrígida, 83,8% nefrolitotomía percutánea (NLP), 78,2% URS flexible y 67,2% litotricia extracorpórea. Los médicos más jóvenes tienden a realizar NLP (p < 0,001) y la proporción de usuarios de URS flexible es mayor en los países de habla portuguesa (p = 0,037). La fuente de energía principal empleada es el láser (60,7%). Practican NLP de calibre reducido 15,3%, más en hospitales universitarios (p < 0,01) y profesionales de mayor edad (p < 0,01). Solo el 3,2% utiliza acceso percutáneo al riñón guiado exclusivamente por ultrasonido (US), especialmente profesionales mayores (p < 0,001). Utiliza posición supina el 40,4%, más en España (p < 0,001) y en entorno universitario (p = 0,017). Practican NLP sin nefrostomía el 3,9%. Conclusiones: El manejo de litiasis en el entorno CAU sigue un perfil similar al reconocido en las guías clínicas europea y americana, aunque se reconocen interesantes variaciones en función de la edad y de laspreferencias de los profesionales. Se identifican como posibles áreas de mejora NLP de calibre reducido y punción guiada por US. Objective: To describe the trends in the current management of urolithiasis in the Confederación Americana de Urología (CAU) setting to recognise patterns of clinical practice and identify educational needs. Material and method: An online survey was created with 31 multiple-choice questions (Spanish and Portuguese) through www.caunet.org, which revealed demographic data, patterns of clinical practice and approaches for specific clinical conditions. Results: A total of 463 practitioners from Brazil, Mexico, Argentina, Spain, Colombia, Chile and other countries (3.96% of the members of CAU) completed the survey. All participants performed some type of urolithiasis treatment: 98.5% performed semirigid ureteroscopy (URS), 83.8% performed percutaneous nephrolithotomy (PCNL), 78.2% performed flexible URS, and 67.2% performed extracorporeal lithotripsy. The youngest physicians tended to perform PCNL (P < .001), and the proportion of users of flexible URS was greater in the Portuguese countries (P = .037). The main energy source was laser (60.7%). Small-calibre PCNL was performed by 15.3% of the respondents, more often in university hospitals (P < .01) and by older practitioners (P < .01). Only 3.2% of the respondents used percutaneous access to the kidney guided exclusively by ultrasound, especially the older practitioners (P < .001). The supine position was used by 40.4% of the respondents, more often in Spain (P < .001) and in the university setting (P = .017). PCNL without nephrostomy was practiced by 3.9% of the respondents. For flexible URS, 19.2% of the respondents did not use ureteral access sheaths, and the older practitioners preferred thinner calibre sheaths (P < .001). Conclusions: The management of lithiasis in the CAU setting follows a profile similar to that recognised in the European and American clinical guidelines, although there are interesting variations based on the practitioner's age and preferences. We identified potential areas for improvement in small-calibre PCNL and ultrasound-guided puncture. AIM: This study evaluated using urine dipstick tests with the clean-catch method to screen for urinary tract infection (UTI) in febrile infants under 90 days of age. METHODS: We carried out a comparative diagnostic accuracy study of infants under 90 days old, who were studied for unexplained fever without any source, in the emergency room of a hospital in Madrid from January 2011 to January 2013. We obtained matched samples of urine using two different methods: a clean-catch, standardised stimulation technique and catheterisation collection. The results of the leucocyte esterase test and nitrite test were compared with their urine cultures. RESULTS: We obtained 60 pairs of matched samples. A combined analysis of leukocyte esterase and, or, nitrites yielded a sensitivity of 86% and a specificity of 80% for the diagnosis of UTIs in clean-catch samples. The sensitivity of leukocyte esterase and, or, nitrites in samples obtained by catheterisation were not statistically different to the clean-catch samples (p = 0.592). CONCLUSION: Performing urine dipstick tests using urine samples obtained by the clean-catch method was an accurate screening test for diagnosing UTIs in febrile infants of less than 90 days old. This provided a good alternative to bladder catheterisation when screening for UTIs. Abstract The predictive value of traditional cardiovascular risk estimators is limited, and young and elderly populations are particularly underrepresented. We aimed to investigate the urine metabolome and its association with cardiovascular risk to identify novel markers that might complement current estimators based on age. Urine samples were collected from 234 subjects categorized into three age-grouped cohorts: 30–50 years (cohort I, young), 50–70 years (cohort II, middle-aged), and > 70 years (cohort III, elderly). Each cohort was further classified into three groups: (a) control, (b) individuals with cardiovascular risk factors, and (c) those who had a previous cardiovascular event. Novel urinary metabolites linked to cardiovascular risk were identified by nuclear magnetic resonance in cohort I and then evaluated by target mass spectrometry quantification in all cohorts. A previously identified metabolic fingerprint associated with atherosclerosis was also analyzed and its potential risk estimation investigated in the three aged cohorts. Three different metabolic signatures were identified according to age: 2-hydroxybutyrate, gamma-aminobutyric acid, hypoxanthine, guanidoacetate, oxaloacetate, and serine in young adults; citrate, cyclohexanol, glutamine, lysine, pantothenate, pipecolate, threonine, and tyramine shared by middle-aged and elderly adults; and trimethylamine N-oxide and glucuronate associated with cardiovascular risk in all three cohorts. The urinary metabolome contains a metabolic signature of cardiovascular risk that differs across age groups. These signatures might serve to complement existing algorithms and improve the accuracy of cardiovascular risk prediction for personalized prevention. OBJECTIVE: This study aimed to assess the population at risk of infection by extended-spectrum beta-lactamase (ESBL)-producing organisms, using clinical criteria. MATERIALS AND METHODS: All urine cultures positive for Enterobacteriaceae in a Spanish hospital department from January 2010 to 2014 were reviewed. All isolates with ESBL-positive strains were collected, and isolates received during the first week of each month with ESBL-negative strains from symptomatic patients hospitalized or admitted to the emergency room. Multivariate analysis of the factors involved was undertaken and a nomogram developed to predict the probability of infection by ESBL-producing microorganisms. En el curso de-nuestras disecciones, hemos encontrado la siguiente disposición anatómica en el Aparato Urinario de un sujeto traído a la sala de Trabajos Prácticos del Instituto.Los alumnos a quienes les corres pondía verificar la preparación de un Riñón en un cadáver X, se sorprendieron al comenzar el trabajo, ya que aparentemente no existía riñón en el lado derecho. Fui consultado del particular, por lo cual me acerqué a comprobar lo aseverado por dichos estudiantes.En efecto, a la simple inspección, y luego de reclinar hacia los lados las asas intestinales, se observa el peritoneo parietal, abombado, mirando hacia delante; a la palpación profunda, no existe la demarcación normal del riñón contenido en su celda, sino un engrosamiento abollado, duro, de consistencia renitente, sin fluctuación, lo cual a primera vista da la impresión de tratarse de un absceso perirrenal, por lo que practico una incisión en cruz, a fin de descubrir el contenido de esta tumoración; el corte es resistente al cuchillo y deja descubierta una superficie fibrosa de color amarillento, que da el aspecto de un endurecimiento que va a la caseificación; en otros sitros vecinos de la incisión hay depresiones que dejan huella a la presión con el pulpejo del dedo. No existe forma ni remota de un riñón normal.

Objetivo: Investigar si la sobreexpresión de los niveles proteicos de p53, MIB-1 y PECAM-1 ocupa un lugar de interés en la predicción de pronóstico del carcinoma de células transicionales del tracto urinario superior (CCT-TUS) de asiento primario en la pelvis renal. Material y método: Análisis uni y multivariante para la predicción de pronóstico en una serie de 82 pacientes con CCT-TUS de pelvis renal sin metástasis al diagnóstico (N0/Nx M0) tratados mediante nefroureterectomía exclusivamente. Se evalúan parámetros clínico-patológicos (edad, sexo, grado, extensión tumoral, variedad histológica, patrón de crecimiento, invasión vascular, infiltración del parénquima renal, necrosis tumoral) y expresión inmunohistoquímica (IHQ) de p53, MIB-1 (ki-67) y PECAM-1 (CD31) en secciones realizadas con microarray de tejido (TMA). Resultados: El 47,6% fueron lesiones de alto grado según la clasificación USIP-OMS. El patrón de crecimiento fue plano en 15,85%. La distribución por categoría T fue: 3,7% pTa, 51,2% pT1, 11% pT2, 29,3% pT3 y 4,9% pT4. El seguimiento medio fue 46,8 + 38,5 (rango: 4-172) meses. La mediana de supervivencia se alcanzó a los 57 (IC 95%: 44-63) meses. El análisis univariante reveló que la supervivencia en estos pacientes se asocia a tamaño tumoral (p = 0,028), variedad histológica (p < 0,0001), patrón de crecimiento (p < 0,0001), grado (p < 0,0001), pT (p = 0,01), invasión vascular (p = 0,025), necrosis (p = 0,004) y sobreexpresión de p53 (p = 0,0006), PECAM-1 (p = 0,0036) y MIB-1 (p = 0,0038). El modelo de regresión de Cox mostró que alto grado (HR = 4,2 [IC 95%: 1,28-13,79]; p = 0,018), patrón de crecimiento plano (HR = 2,52 [IC 95%: 1,05-6,03]; p = 0,038) y sobreexpresión de p53 (HR = 2,8 [IC 95%: 1,22-6,44]; p = 0,015) fueron variables predictivas independientes. Conclusión: El grado histológico, el patrón de crecimiento tumoral y la sobreexpresión de p53 se establecen como los principales factores predictivos de pronóstico en CCT-TUS primario de pelvis renal. No se reproduce el valor independiente de MIB-1 objetivado en otros estudios. Objective: Determine whether the overexpression p53, MIB-1 and PECAM-1 of protein levels is of interest in predicting the prognosis of transitional cell carcinoma of the upper urinary tract (TCC-UUT) with the primary seat in the renal pelvis. Material and method: A univariate and multivariate analysis was conducted for prognosis prediction in a series of 82 patients with TCC-UUT of the renal pelvis who had no metastases at diagnosis (N0/Nx M0) and were treated exclusively with nephroureterectomy. We assessed clinicopathological parameters (age, gender, tumor grade and extent, histological variety, growth pattern, vascular invasion, infiltration of the renal parenchyma, tumor necrosis) and the immunohistochemical expression of p53, MIB-1 (ki-67) and PECAM-1 (CD31) in sections performed with tissue microarray (TMA). Results: A total of 47.6% of the patients had high-grade lesions according to the USIP-WHO classification. The growth pattern was flat in 15.85%. The distribution by T category was: 3.7% pTa, 51.2% pT1, 11% pT2, 29.3% pT3 and 4.9% pT4. The mean follow-up was 46.8+38.5 (range, 4-172) months. The median survival was reached at 57 (95% CI 44-63) months. The univariate analysis revealed that survival in these patients is associated with tumor size (P = .028), histological variety (P < .0001), growth pattern (P < .0001), grade (P < .0001), pT (P = .01), vascular invasion (P = .025), necrosis (P = .004) and overexpression of p53 (P = .0006), PECAM-1 (P = .0036) and MIB-1 (P = .0038). The Cox regression model showed that high-grade (HR, 4.2; 95% CI 1.28-13.79; P = .018), flat growth pattern (HR, 2.52; 95% CI 1.05-6.03; P = .038) and p53 overexpression (HR, 2.8; 95% CI 1.22-6.44; P = .015) were independent predictors. Conclusion: Histological grade, tumor growth pattern and p53 overexpression were established as the primary predictors of prognosis for primary TCC-UUT of the renal pelvis. Benign prostatic enlargement is the main cause of male lower urinary tract symptoms (LUTS). The distinction and quantification of voiding, storage and postmicturition symptoms help clinicians decide on a modern pharmacological therapeutic approach for male LUTS. Muscarinic receptors antagonists are now considered safe therapy in males and a fixed-dose combination of oral controlled absorbed system solifenacin and tamsulosin is advantageous compared to tamsulosin monotherapy in males with predominant storage symptoms when validated instruments such as International Prostatic Score Symptoms (IPSS) or Total Urgency Frequency Score (TUFS) are used. This combination therapy is well tolerated and maintains symptomatic improvement in the long term. Different options of managing male LUTS with predominant storage symptoms include a combination of tamsulosin and solifenacin, antimuscarinics alone – some with flexible dosing, and the beta-3 agonist mirabegron. Tailored pharmacological therapy for the particular patient is not a reality yet, but judicious use of the different alternatives could bring varied new therapeutic solutions for male LUTS including benign prostatic enlargement and benign prostatic obstruction. Background: Leukoerythroblastosis, a non-specific and often short-lasting response of the bone marrow to different diseases such as malignancies or infections, is characterized by the presence in the peripheral blood of immature red and white cells. Methods: We present a case of leukoerythoblastosis occurring in a 24 months old Mozambican girl, in the context of a severe malaria episode and an associated urinary tract infection. Peripheral blood smear was used for diagnosis of malaria and leukoerythroblastosis. Enterobacter cloacae isolation and antibiotic susceptibility testing were performed by conventional microbiology. Results: Peripheral blood smear was positive for Plasmodium falciparum and showed a leukoerythroblastosis with red cell anisopoikilocytosis and left shifted neutrophils. Urine culture confirmed the presence of a multi-resistant E. cloacae. Treatment of underlying conditions resolved the leukoerythroblastic reaction. Conclusions: Leukoerythroblastosis may be related to different infectious diseases and may also appear in the context of severe malaria. Bacterial superinfection needs to be investigated.

Con el transcurso de los años, las mascotas, cada vez más, van adquiriendo la consideración de miembros, casi de pleno derecho, de la familia; en consecuencia, los mayores cuidados de que disfrutan, junto con la evolución de la medicina veterinaria, están haciendo que esos “nuevos miembros de la familia”, vivan más tiempo, alcanzando edades antes impensables; el problema que se deriva de esta nueva situación reside en que estos nuevos ancianos necesitan unos cuidados muy específicos y están más predispuestos a padecer enfermedades crónicas, por lo que la ciencia veterinaria se está viendo obligada a llevar a cabo más estudios y nuevos avances diagnósticos y terapéuticos, tanto a nivel físico como de laboratorio, para estos pacientes geriátricos, tanto en el caso de la especie felina como de la canina, con el fin de proporcionarles la mejor calidad y el mayor tiempo de vida posibles. Las técnicas de diagnóstico por imagen, en especial los distintos modos de ecografía, junto con los marcadores de la función renal, como SDMA, creatinina y urea, y la valoración específica de ciertos parámetros de la orina, como UPC y densidad urinaria, actualmente, nos permiten observar, identificar y clasificar, tanto los pequeños cambios en la función renal, como las grandes alteraciones que podría suponer el padecimiento, por parte de un animal, de una enfermedad renal, ya sea incipiente o avanzada. En este trabajo de tesis doctoral, hemos utilizado 40 ejemplares caninos, de diferentes razas, sexos y tamaños, a los que dividimos en 2 grupos, uno de adultos, de entre uno y 6 años, y otro de geriátricos, mayores de 7 años, unos clientes del Hospital Veterinario de la Universidad de León y otros derivados del Centro Veterinario Cabo Peñas de Candás, Asturias. Para el estudio se han utilizado un total de 40 estudios ecográficos completos del aparato urogenital, tanto en modo B como en Doppler, y 40 estudios analíticos laboratoriales completos, referidos al perfil renal, con sus 40 urianálisis correspondientes. A la vista de los resultados obtenidos, hemos comprobado que los diferentes medios que hemos utilizado para diagnosticar precozmente la enfermedad renal crónica, lejos de competir entre sí, son complementarios, por lo que todos ellos son necesarios para el desarrollo de la práctica clínica actual, gracias a las virtudes y características que aporta cada uno de ellos y a que ninguno de ellos es capaz, por sí solo, de documentar completa y precozmente los procesos del aparato urogenital. La SDMA, por su parte, de todos los valores estudiados, se muestra como el más práctico y precoz, a la hora de diagnosticar la enfermedad renal; además, se complementa a la perfección con el resto de pruebas, para el desarrollo de una práctica clínica eficiente, sencilla y rápida, a la hora de diagnosticar los procesos renales, así como de evaluar su desarrollo con el fin de colaborar en ponerles freno. A lo largo de este trabajo, hemos podido constatar la significativa relación estadística existente entre variables como la edad y el peso, y los diferentes valores obtenidos mediante ecografía, tanto en modo B como Doppler, y a través de los análisis sanguíneos y urinarios. En relación con la edad de los animales, encontramos diferencias significativas entre los grupos, en cuanto a las medidas ecográficas de la pelvis renal y al valor de la densidad urinaria, lo que obliga a tener en cuenta la edad de estos pacientes, a la hora de valorar si ciertos resultados son normales dependiendo de la edad, o si son anómalos y demuestran o anticipan la inminente aparición de una enfermedad del aparato urogenital. En cuanto al diagnóstico ecográfico, encontramos una relación de los índices de resistencia y pulsatilidad con el tamaño de la corteza renal; así como de la longitud renal con el valor del índice de resistencia en el riñón derecho; por otro lado, estos índices no mostraron ninguna relación estadística con los valores obtenidos de los análisis sanguíneos y de los urianálisis realizados, cuestión que difiere de otros estudios y que nos hace pensar que, en nuestro caso, no se encontraron correlaciones debido a que todos los animales eran sanos; puesto que es en los enfermos en los que, hasta ahora, se han establecido esas relaciones. Por todo lo anteriormente expuesto, pensamos que es necesario profundizar en la investigación con este mismo patrón de estudio, incluyendo un mayor número de animales de los mismos grupos de edad e incluyendo también pacientes enfermos, con el fin de valorar si existen o no esas relaciones, y de elaborar un tabla de valores con la que predecir y evaluar el desarrollo de la enfermedad renal crónica

Objectives: We compared perioperative results and complications of reconstructive surgery of the urinary tract performed using a multichannel platform through the umbilicus and one additional 3.5-mm with a cohort of patients simultaneously treated with conventional 4-port laparoscopy. Materials and Methods: Matched-pair study comparing perioperative outcomes, postoperative visual analogue pain scale (VAPS) and morbidity of 2-port (n = 20) and 4-port (n = 10) laparoscopic reconstructive urological surgery. Preoperative and perioperative data compared included demographics, type of surgery, operative time, blood loss, decrease in serum hemoglobin, operative complications, length of stay and postoperative complications according to Clavien-Dindo classification. Results: There was no significant difference between groups regarding age, gender, body mass index, American Society of Anesthesiologists score, type of surgery, operative time, operative complications and intraoperative or postoperative transfusion. Estimated blood loss was lower using reduced-port approach. VAPS at postoperative day one was significantly lower for 2-port approach and so was the length of stay. Patient satisfaction with the wound was higher for 2-port surgery. Differences were not observed in number and severity of postoperative complications. Conclusions: Urological reconstructive operations can be safely performed using the hybrid laparoendoscopic single-site umbilical approach, resulting in lower blood loss, higher patient satisfaction and lower postoperative pain, which also facilitate earlier hospital discharge, than the same reconstructive procedures performed through multiport conventional laparoscopy.
