Introdução: A identidade “trans” se refere a como a própria pessoa se enxerga e se sente, cabendo o respeito de todos à identidade que a personifica, que a define enquanto ser humano. Diante de todos os obstáculos sociais que essa população enfrenta, a saúde pública se configura num grave problema para os “trans”, pois, historicamente, o acesso aos cuidados básicos com a vida vai de encontro ao preconceito e à falta de informação, dificultando um atendimento adequado. Objetivo: Analisar o acesso e acolhimento da população trans na atenção primária, como também identificar as dificuldades encontradas no processo de busca de cuidados básicos. Metodologia: Revisão de literatura de caráter descritivo nos indexadores PubMed, SciELO, LILACS, Capes Teses e Dissertações e ScienceDirect, com os descritores ‘transexualidade’, ‘transexualismo’, ‘travestismo’, ‘travesti’ e ‘transgênero’. Resultados e discussão: O Ministério da Saúde publica portarias e institui serviços de saúde específicos voltados para essa população, mas a mesma continua sendo apontada como a que mais enfrenta dificuldades para acessar esses serviços. A não observação do nome social, a incapacidade em manejar questões próprias do universo LGBTQIA+, o modelo médico não tão abrangente em sua prática levam ao constrangimento, insatisfação, medo e à insegurança das pessoas trans quando procuram os serviços de saúde. Conclusão: Para receber novos públicos é preciso criar um ambiente preparado para acesso e acolhimento de pessoas trans. Os profissionais devem conhecer e se capacitar sobre questões de gênero, para que tenham uma visão neutra do ser humano e cuidado para suas necessidades pessoais.

[Aims]: To establish the utility of flow cytometry (FCM) for screening and diagnosis of B cell non-Hodgkin lymphoma (B-NHL) from lymphoid tissue samples obtained by fine-needle aspiration (FNA). [Methods and results]: We compared prospectively FCM versus cytology/histology analysis of FNA samples for the diagnostic screening and further World Health Organization (WHO) subclassification of B-NHL. FCM and cytology showed a high degree of agreement (93%); however, diagnosis of reactive processes (RP), B-NHL and T-NHL by FCM showed higher sensitivity than cytology (92-100% versus 64-94%, respectively), without false positive NHL cases. The antibody combination used did not allow a positive diagnosis of Hodgkin lymphoma as distinct from a RP. A high concordance rate was found between FCM and histopathology (74%) in subtyping B-NHL. In this regard, mantle-cell lymphoma and chronic lymphocytic leukaemia/small lymphocytic lymphoma showed the highest degree of agreement (100% concordant rates). In turn, FCM showed higher sensitivity/specificity in classifying follicular lymphoma (FL) and large B cell lymphomas, while the opposite occurred for marginal-zone and lymphoplasmacytic lymphomas. [Conclusions]: FCM enhances the diagnostic ability of FNA cytology, playing a crucial role in a rapid and accurate differential diagnosis between RP, B-NHL and T-NHL. The term >monoclonal gammopathy> (MG) includes a group of clonal plasma cell disorders, which show heterogeneous clinical behavior. While multiple myeloma (MM) and plasma cell leukemia (PCL) are incurable malignant diseases, most patients with MG of undetermined significance (MGUS) show an indolent/benign clinical course. Evidence has accumulated which supports the role of the bone marrow microenvironment in MG. Accordingly, the survival, drug-resistance and proliferation of MM cells have been shown to be largely dependent on a supportive microenvironment. Among the different environment-associated parameters, those related to the status/activity of the immune system are particularly relevant. This review focuses on the different ways clonal plasma cells (PC) interact with the immune system in different models of MG, to characterize crucial events in the development and progression of MG. These advances may support the design of novel therapeutic approaches in patients with MG. © Wichtig Editore, 2004.

Large granular lymphocyte (LGL) leukemia is a well-recognized disease of mature T-CD8+ or less frequently natural killer cells; in contrast, monoclonal expansions of CD4+ T-LGL have only been sporadically reported in the literature. In the present article we have explored throughout a period of 56 months the incidence of monoclonal expansions of CD4+ T-LGL in a population of 2.2 million inhabitants and analyzed the immunophenotype and the pattern of cytokine production of clonal CD4+ T cells of a series of 34 consecutive cases. Like CD8+ T-LGL leukemias, CD4+ T-LGL leukemia patients have an indolent disease; however, in contrast to CD8+ T-LGL leukemias, they do not show cytopenias and autoimmune phenomena and they frequently have associated neoplasias, which is usually determining the clinical course of the disease. Monoclonal CD4+ T-LGL showed expression of TCRαβ, variable levels of CD8 (CD8-/+dim) and a homogeneous typical cytotoxic (granzyme B+, CD56+, CD57+, CD11b+/-) and activated/memory T cell (CD2+bright, CD7-/+dim, CD11a+bright, CD28-, CD62L- HLA-DR+) immunophenotype. In addition, they exhibited a Th1 pattern of cytokine production [interferon-γ++, tumor necrosis factor-α++, interleukin (IL-2)-/+, IL-4-, IL-10-, IL-13-]. Phenotypic analysis of the TCR-Vβ repertoire revealed large monoclonal TCR-Vβ expansions; only a restricted number of TCR-Vβ families were represented in the 34 cases analyzed. These findings suggest that monoclonal TCRαβ+/CD4+/NKa+/CD8-/ +dim T-LGL represent a subgroup of monoclonal LGL lymphoproliferative disorders different from both CD8+ T-LGL and natural killer cell-type LGL leukemias. Uma vez que tal conceito não fora encontrado em nenhum texto da bibliografia estudada nos apoiaremos nos princípios de Gilles Deleuze acerca dos corpóreos e incorpóreos e seu devir puro. Partindo da ideia de que a palavra autoficcionalização é um deverbal, ou seja, um substantivo que surge a partir de um verbo ficcionalizar torna-se possível entender que, enquanto fenômeno verbal proveniente do ato de escrever, o sujeito criado através da autoficcionalização será mero efeito de superfície, ou seja, um efeito possibilitado pela linguagem, mais especificamente, pela ação verbal presente no ato de escrever. Dessa forma, bastaria ao autor escrever, utilizando-se da primeira pessoa, para que, consciente ou não, produzisse esse efeito de superfície que chamaremos autoficcionalização do sujeito, que difere de autoficção, pois, ao contrário dela, não pressupõe uma relação de causa e efeito e acontece independente da vontade do autor. The purpose of this study is to develop the concept of autofictionalization of the subject in Ferreira Gullar’s poetry. Since this concept was not found in any text studied in the literature review, we hold our beliefs in Gilles Delleuze’s and Felix Guattari’s principles about the corporeal and incorporeal, and their pure transformation. Starting from the idea that the phrase autofictionalization is deverbative, i.e. a noun that comes from a verb - fictionalize – it is possible to understand that, as a verbal phenomenon from the act of writing, the subject created through autofictionalization will merely have a surface effect, i.e. an effect made possible by language, more specifically, by the verbal action present in the act of writing. Thus, an author would only need writing in first person, so that, consciously or not, this effect would be produced. Our understanding of autofictionalization – the surface effects – differs from the idea of autofiction, since, unlike the latter, it does not imply a cause and effect relationship and takes place regardless the author’s will
