Sickness presenteeism among healthcare workers (HCW) risks nosocomial infection, but its prevalence among HCW with COVID-19 is unknown. Contemporaneous interviews revealed a sickness presenteeism prevalence of 49.8% among 255 HCW with symptomatic COVID-19. Presenteeism prevalence did not differ among HCW with and without specific COVID-19 symptoms or direct patient care.

Anastrepha dissimilis is currently considered to be widely distributed in Brazil, occurring in 20 of 27 states. However, morphological differences between the holotype (from Plaisance, Haiti) and a paratype (from Pernambuco, Brazil) suggest that the Brazilian specimens are not A. dissimilis, because their aculeus tip is similar to the paratype not to the holotype. Therefore, considering the importance of integrative taxonomy for species delimitation, we used geometric and linear morphometrics and cytochrome c oxidase subunit I sequences integrated with the morphology of the aculeus tip to clarify the identity of populations previously identified as A. dissimilis from multiple Brazilian localities. Morphological data show a uniform pattern among the Brazilian populations, with some variation among specimens from the south and northeast. In addition, the geometric and linear morphometrics suggest considerable geographic variation among these populations, suggesting the existence of at least two morphs. The molecular analysis revealed that specimens from Brazil previously identified as A. dissimilis belong to Anastrepha chiclayae Greene, with a genetic distance ranging from 0.00 to 0.015%. According to our integrative analyses, specimens from Brazil formerly identified as A. dissimilis actually are A. chiclayae. Therefore, this is the first record of A. chiclayae in Brazil, and we also report that A. dissimilis does not occur in Brazil.

When thin and pliable free tissue is needed, the medial sural artery perforator (MSAP) flap provides an excellent option with minimal donor site morbidity. However, among its pitfalls include difficult patient positioning and surgeon ergonomics throughout the harvest. We describe a novel positioning technique that may significantly improve surgeon ergonomics and ease of MSAP flap harvest. A cross-legged modification may eliminate many of the issues associated with the classic frog-leg position. While the patient is cross-legged, the surgeon is afforded a normal field of view that is closer to their body, while simultaneously providing support to the lateral side of the gastrocnemius muscle. This pictorial essay describes this positioning technique and subsequent harvest. By incorporating a more ergonomic cross-legged position during flap elevation, many of thedrawbacks of the MSAP flap could be eliminated.

Managed honey bee populations have fluctuated over the past several decades in the U.S. While a single factor has not been identified for these losses, the interaction between multiple biotic and abiotic stressors have been suggested to be responsible. Of major concern are several invasive parasite and pathogen species as well as colony management. A single honey bee colony often suffers from multiple harmful agents, that may act synergistically and cause greater declines in bee health. We conducted a survey to detect known and lesser-known honey bee parasites and pathogens. While previous research has primarily focused on commercially managed colonies, research is limited to pertaining parasite and pathogen prevalence in hobbyist managed colonies. Molecular diagnostics were used to screen 541 Arkansas (AR) honey bee colonies from 107 hobbyist beekeepers for eight A. mellifera parasite and pathogen species. Colony samples were obtained between 2015–2016 and represented forty-seven of the seventy-five AR counties. Vairimorpha ceranae (11.6% occurrence) and parasite Varroa destructor (49.4% occurrence) were relatively common in AR hobbyist colonies. Interestingly, the lesser-studied pathogenic trypanosome species, Lotmaria passim, was detected in 11.3% of the colonies and widespread in twenty of the forty-seven counties sampled. None of the honey bee pathogens Vairimorpha apis, Spiroplasma apis, S. melliferum, Crithidia mellificae, or the parasitic phorid fly, Apocephalus borealis, were detected in the colonies sampled. This study provides an extensive assessment of the parasite and pathogen species occurring at the AR state-level in hobbyist-managed honey bee colonies.

The present study sought to understand the antecedents to COVID-19 vaccination among those reporting a change in vaccine intention in order to improve COVID-19 vaccine uptake in the United States. We employed semi-structured interviews and one focus group discussion with vaccinated and unvaccinated Veterans Health Administration (VHA) employees and Veterans at three Veterans' Affairs medical centers between January and June 2021. A subset of these participants (n=21) self-reported a change in COVID-19 vaccine intention and were selected for additional analysis. We combined thematic analysis using the 5C scale (confidence, collective responsibility, complacency, calculation, constraints) as our theoretical framework with a constant comparative method from codes based on the SAGE Working Group on Vaccine Hesitancy. We generated 13 themes distributed across the 5C constructs that appeared to be associated with a change in COVID-19 vaccine intention. Themes included a trusted family member, friend or colleague in a healthcare field, a trusted healthcare professional, distrust of government or politics (confidence); duty to family and protection of others (collective responsibility); perceived health status and normative beliefs (complacency); perceived vaccine safety, perceived risk-benefit, and orientation towards deliberation (calculation); and ease of process (constraints). Key factors in promoting vaccine uptake included a desire to protect family; and conversations with as key factors in promoting vaccine uptake. Constructs from the 5C scale are useful in understanding intrapersonal changes in vaccine intentions over time, which may help public health practitioners improve future vaccine uptake.In this study of the Veteran and VA employee population, we aimed to understand what factors led to a decision to receive a COVID-19 vaccine. As part of a quality improvement project, we interviewed individuals at three Veterans’ Affairs sites in the first six months of 2021. We then used a smaller sample of 21 participants who reported a change in their intentions to receive a COVID-19 vaccine to analyze for this study. This analysis utilizes constructs from the 5C scale, which was developed to understand the conditions required for an individual to decide to receive a vaccine (confidence, collective responsibility, complacency, calculation, constraints). The coding process revealed a number of recurring themes across the interviews falling under each of the five constructs, but concepts relating to vaccine confidence (i.e., level of trust in those developing and disseminating the vaccine) were most common, and constraints (i.e., psychological and structural barriers that stand in the way of vaccination) appeared least frequently in our interviews. We found that significant motivators to receive the vaccine included a desire to protect family and conversations with trusted clinicians, particularly mental healthcare providers. Our study was unique in using the 5Cs to understand changes in vaccine changes over time. Findings show that change in vaccine attitudes is possible even in the presence of concerns and shed light on approaches that public health providers could use to improve vaccine and booster rates.

Anastrepha is the most diverse and economically important genus of Tephritidae in the American tropics and subtropics. The striking morphology of the third instars of Anastrepha caballeroi Norrbom, Anastrepha crebra Stone, Anastrepha haplacantha Norrbom & Korytkowski, Anastrepha korytkowskii Norrbom, Anastrepha nolazcoae Norrbom & Korytkowski, and three newly discovered and as yet formally unnamed species (Anastrepha sp. Peru-82, Anastrepha sp. nr. protuberans, and Anastrepha sp. Sur-16), and the more typical morphology of Anastrepha aphelocentema Stone, are described using light and scanning electron microscopy. To contribute to a better understanding of the interspecific and intraspecific variation among species in the mucronota species group and facilitate phylogenetic studies, we integrate molecular and morphological techniques to confirm the identity and describe third instars. Larva-adult associations and the identification of described larvae were confirmed using DNA barcodes. We provide diagnostic characters to distinguish larvae among these nine species of the mucronota group and separate them from those of the 29 other Anastrepha species previously described. We introduce the vertical comb-like processes on the oral margin as a novel character, and the unusual character states, including position and shape of the preoral lobe, and dentate or fringed posterior margins of the oral ridges and accessory plates. Our comparative morphology concurs with most previously inferred phylogenetic relationships within the mucronota group.

Secondary organic aerosol (SOA) is a significant component of atmospheric fine particulate matter. Mobile sources have historically been a major source of SOA precursors in urban environments, but decades of regulations have reduced their emissions. Less regulated sources, such as volatile chemical products (VCPs), are of growing importance. We analyzed ambient and emissions data to assess the contribution of mobile sources to SOA formation in Los Angeles during the period of 2009-2019. During this period, air quality in the Los Angeles region has improved, but organic aerosol (OA) concentrations did not decrease as much as primary pollutants. This appears to be largely due to SOA, whose mass fraction in OA increased over this period. In 2010, about half of the freshly formed SOA measured in Pasadena, CA appears to be formed from hydrocarbon (non-oxygenated) precursors. Chemical mass balance analysis indicates that these hydrocarbon SOA precursors (including intermediate volatility organic compounds) can largely be explained by emissions from mobile sources in 2010. Emissions from volatile chemical products (VCPs) are emerging as a major source of anthropogenic secondary organic aerosol (SOA) precursors. Respiratory syncytial virus (RSV) and influenza are important causes of disease in children and adults. In Australia, information on the burden of RSV in adults is particularly limited.We used time series analysis to estimate respiratory, acute respiratory infection, pneumonia and influenza, and bronchiolitis hospitalisations attributable to RSV and influenza in Australia during 2009 through 2017. RSV and influenza-coded hospitalisations inlt;5-year-olds were used as proxies for relative weekly viral activity.From 2009 to 2017, the estimated all-age average annual rates of respiratory hospitalisations attributable to RSV and seasonal influenza (excluding 2009) were 54.8 (95% confidence interval [CI]: 20.1, 88.8) and 87.8 (95% CI: 74.5, 97.7) per 100,000, respectively. The highest estimated average annual RSV-attributable respiratory hospitalisation rate per 100,000 was 464.2 (95% CI: 285.9, 641.2) inlt;5-year-olds. For seasonal influenza, it was 521.6 (95% CI: 420.9, 600.0) in persons aged ≥75 years. In ≥75-year-olds, modelled estimates were approximately eight and two times the coded estimates for RSV and seasonal influenza, respectively.RSV and influenza are major causes of hospitalisation in young children and older adults in Australia, with morbidity underestimated by hospital diagnosis codes.

We assessed an intervention aimed at improving adherence to antiretroviral therapy (ART) among pregnant and postpartum women living with HIV (PPWLH). We randomized 133 pregnant women initiating ART in Uganda to receive text reminders generated by real time-enabled electronic monitors and data-informed counseling through 3 months postpartum (PPM3) or standard care. Intention-to-treat analyses found low adherence levels and no intervention impact. Proportions achieving ≥95% adherence in PPM3 were 16.4% vs. 9.1% (t = -1.14, p = 0.26) in intervention vs. comparison groups, respectively; 30.9% vs. 29.1% achieved ≥80% adherence. Additional analyses found significant adherence declines after delivery, and no effect on disease progression (CD4-cell count, viral load), though treatment interruptions were significantly fewer in intervention participants. Per-protocol analyses encompassing participants who used adherence monitors as designed experienced better outcomes, suggesting potential benefit for some PPWLH. SUMMARY The Brawley Seismic Zone (BSZ) is a ∼58-km-long section of the Pacific–North America Plate boundary that connects the southernmost San Andreas Fault (SAF) and the Imperial Fault in southern California. We analyse the BSZ as two segments: a north segment, dominated by SAF tectonics, and a south segment that accommodates a higher level of seismicity. The south segment includes a ∼6 km wide right lateral step-over that includes the Salton Sea geothermal field (SSGF) and Holocene subaerial lava domes at the south end of the Salton Sea, called Salton Buttes. In general, the 40 yr (1981–2021) of BSZ seismicity is characterized by transitory sequences that often are accommodated simultaneously or in quick succession on intricate ladder-like faults. These sequences rarely reactivate faults associated with previous seismicity alignments but rather activate adjacent faults, sometimes located within less than 1 km. They can include several events of similar size as the mainshock, followed by bursts of aftershocks sometimes located away from the mainshock rupture. The seismicity rate and the spatial geometrical complexity varies between the BSZ-north and BSZ-south segments. The 24-km-long BSZ-north accommodates a ∼12-km-long linear trend of seismicity that extends from the SAF terminus into the Salton Sea, where moderate-sized northeast striking sequences form the rungs in a ladder-structure in a weak crust. The seismicity in this area is most likely influenced by the stress state of the SAF. In contrast, the 34 km long BSZ-south segment, which also has a weak crust, has accommodated larger sequences that illuminate irregular ladder-type faulting, with aftershocks defining linear distributions striking either north or northeast. The focal mechanisms exhibit a mostly strike-slip style of faulting with minor dip-slip faulting in the south Salton Sea area as well as the Mesquite basin to the south. The state of stress, as determined from focal mechanisms, consists of almost horizontal σ1, and σ3 with vertical σ2. The activation angle between the trend of σ1 and the preferred nodal plane of the largest nearby event decreases systematically from north to south along the long-axis of the BSZ. In the step-over zone, the Holocene volcanism and the frequent seismicity sequences suggest crustal extension as well as associated reduced crustal strength as compared to other parts of the BSZ. The presence of weak, thin, hot crust and distributed ladder-like or conjugate predominantly strike-slip faulting suggests that the whole BSZ acts as a several kilometre wide fault zone where no prominent principal slip surfaces are required to accommodate moderate-sized (M6+) earthquakes.

This paper analyzes A.K. Oppenheim’s original works on the transition of deflagration to detonation and reviews them from the perspective of new numerical and experimental results recently obtained on such phenomena. Particular attention is focused on processes happening in the boundary layer of the tube walls ahead of the accelerating flame. The results of the theoretical analyses of temperature variations inside developing boundary layer are presented and compared to the temperature variation in a free stream away from the boundary layer. Analyses of temperature increase in such layers clearly indicate that the self-ignition of the mixture happens in the boundary layer ahead of the propagating flame front. New experimental results obtained recently by a research group from the A. V. Luikov Heat and Mass Transfer Institute in Minsk, Belarus, combined with previously conducted theoretical analyses and numerical simulations, show clearly and unambiguously that the origin of the “explosion in the explosion”, postulated by A. K. Oppenheim in 1966, is always responsible for the Deflagration-Detonation Transition (DDT) in gases and is located in the boundary layer ahead of the accelerating flame front.

A computationally efficient model is proposed to analyze plasma-assisted combustion using nanosecond repetitive pulsed (NRP) plasmas. The NRP plasma discharge is placed in the oxidizer stream of a counter-flow diffusion flame. The effect of changing the flow rate and the pulse repetition frequency (PRF) of a continuous NRP plasma discharge on the temperature and species profiles of a counter-flow diffusion flame is investigated numerically. The results confirm that oxygen atom and nitrogen vibrational states are the most important species to enhance combustion. The results also show that kinetic effects are much more significant for higher PRF and lower pulse voltage. In addition, when steady plasma profiles are used instead of unsteady plasma profiles, the extinction strain rates increase by 25.8%, 21.1%, and 10.8% for PRF equal to 1, 2, and 4 kHz, respectively.

Mobile sources are responsible for a substantial controllable portion of the reactive organic carbon (ROC) emitted to the atmosphere, especially in urban environments of the United States (U.S.). We update existing methods for calculating mobile source organic particle and vapor emissions in the U.S. with over a decade of laboratory data that parameterize the volatility and organic aerosol (OA) potential of emissions from onroad vehicles, nonroad engines, aircraft, marine vessels, and locomotives. We find that existing emission factor information from teflon filters combined with quartz filters collapses into simple relationships and can be used to reconstruct the complete volatility distribution of ROC emissions. This new approach consists of source-specific filter artifact corrections and state-of-the-science speciation including explicit intermediate volatility organic compounds (IVOCs), yielding the first bottom-up volatility-resolved inventory of U.S. mobile source emissions. Using the Community Multiscale Air Quality model, we estimate mobile sources account for 20–25 % of the IVOC concentrations and 4.4–21.4 % of ambient OA. The updated emissions and air quality model reduce biases in predicting fine-particle organic carbon in winter, spring, and autumn throughout the U.S. (4.3–11.3 % reduction in normalized bias). We identify key uncertain parameters that align with current state-of-the-art research measurement challenges.

Distributed Acoustic Sensing (DAS) is a promising technique to improve the rapid detection and characterization of earthquakes. Previous DAS studies mainly focus on the phase information but less on the amplitude information. In this study, we compile earthquake data from two DAS arrays in California, USA, and one submarine array in Sanriku, Japan. We develop a data-driven method to obtain the first scaling relation between DAS amplitude and earthquake magnitude. Our results reveal that the earthquake amplitudes recorded by DAS in different regions follow a similar scaling relation. The scaling relation can provide a rapid earthquake magnitude estimation and effectively avoid uncertainties caused by the conversion to ground motions. Our results show that the scaling relation appears transferable to new regions with calibrations. The scaling relation highlights the great potential of DAS in earthquake source characterization and early warning.

Background South Africa bears a large HIV burden with 7.8 million people with HIV (PWH). However, due to suboptimal antiretroviral therapy (ART) adherence and retention in care, only 66% of PWH in South Africa are virally suppressed. Standard care only allows for suboptimal adherence detection when routine testing indicates unsuppressed virus. Several adherence interventions are known to improve HIV outcomes, yet few are implemented in routinely due to the resources required. Therefore, determining scalable evidence-based adherence support interventions for resource-limited settings (RLS) is a priority. The multiphase optimization strategy (MOST) framework allows for simultaneous evaluation of multiple intervention components and their interactions. We propose to use MOST to identify the intervention combination with the highest levels of efficacy and cost-effectiveness that is feasible and acceptable in primary care clinics in Cape Town. Methods We will employ a fractional factorial design to identify the most promising intervention components for inclusion in a multi-component intervention package to be tested in a future randomized controlled trial. We will recruit 512 participants initiating ART between March 2022 and February 2024 in three Cape Town clinics and evaluate acceptability, feasibility, and cost-effectiveness of intervention combinations. Participants will be randomized to one of 16 conditions with different combinations of three adherence monitoring components: rapid outreach following (1) unsuppressed virus, (2) missed pharmacy refill collection, and/or (3) missed doses as detected by an electronic adherence monitoring device; and two adherence support components: (1) weekly check-in texts and (2) enhanced peer support. We will assess viral suppression ( Discussion To our knowledge, ours will be the first study to use the MOST framework to determine the most effective combination of HIV adherence monitoring and support intervention components for implementation in clinics in a RLS. Broad-scale data show SARS-CoV-2 occurrence in white-tailed deer throughout much of their range in the conterminous United States and reinforce findings of considerable SARS-CoV-2 infection and exposure. Volatile chemical products (VCPs) have recently been identified as potentially important unconventional sources of secondary organic aerosol (SOA), in part due to the mitigation of conventional emissions such as vehicle exhaust. Here, we report measurements of SOA production in an oxidation flow reactor from a series of common VCPs containing oxygenated functional groups and at least one oxygen within the molecular backbone. These include two oxygenated aromatic species (phenoxyethanol and 1-phenoxy-2-propanol), two esters (butyl butyrate and butyl acetate), and four glycol ethers (carbitol, methyl carbitol, butyl carbitol, and hexyl carbitol). We measured gas- and particle-phase products with a suite of mass spectrometers and particle-sizing instruments. Only the aromatic VCPs produce SOA with substantial yields. For the acyclic VCPs, ether and ester functionality promotes fragmentation and hinders autoxidation, whereas aromatic rings drive SOA formation in spite of the presence of ether groups. Therefore, our results suggest that a potential strategy to reduce urban SOA from VCPs would be to reformulate consumer products to include less oxygenated aromatic compounds.

Patients harboring germline breast cancer susceptibility genes 1 and 2 (BRCA1/2) mutations are predisposed to developing breast, pancreatic, and ovarian cancers. BRCA2 plays a critical role in homologous recombination (HR) DNA repair and deleterious mutations in BRCA2 confer sensitivity to PARP inhibition. Recently, the PARP inhibitors olaparib and rucaparib were FDA approved for the treatment of metastatic breast cancer and patients with recurrent ovarian cancer with mutations in BRCA1/2. Despite their initial antitumor activity, the development of resistance limits the clinical utility of PARP inhibitor therapy. Multiple resistance mechanisms have been described, including reversion mutations that restore the reading frame of the BRCA2 gene. In this study, we generated olaparib- and rucaparib-resistant BRCA2-mutant Capan1 cell lines. We did not detect secondary reversion mutations in the olaparib- or rucaparib-resistant clones. Several of the resistant clones had gene duplication and amplification of the mutant BRCA2 allele, with a corresponding increase in expression of a truncated BRCA2 protein. In addition, HR-mediated DNA repair was rescued, as evidenced by the restoration of RAD51 foci formation. Using mass spectrometry, we identified Disruptor Of Telomeric silencing 1-Like (DOT1L), as an interacting partner of truncated BRCA2. RNAi-mediated knockdown of BRCA2 or DOT1L was sufficient to resensitize cells to olaparib. The results demonstrate that independent of a BRCA2 reversion, mutation amplification of a mutant-carrying BRCA2 contributes to PARP inhibitor resistance.

NRF2 is a transcription factor serving as a master regulator of the expression of many genes involved in cellular responses to oxidative and other stresses. In the absence of stress, NRF2 is constantly synthesized but maintained at low levels as it is targeted by KEAP1 for ubiquitination and proteasome-mediated degradation. NRF2 binds KEAP1 mainly through a conserved “ETGE” motif that has also been found in several other proteins, such as DPP3, which has been shown to bind KEAP1 and enhance NRF2 function upon overexpression. Here we demonstrate the interaction between endogenous DPP3 and endogenous KEAP1. We further show that the DPP3–KEAP1 interaction is strongly induced by hydrogen peroxide and that DPP3 is required for timely NRF2 induction and nuclear accumulation in the estrogen receptor (ER)-positive MCF7 breast cancer cells. Moreover, we present evidence that the binding of DPP3 to KEAP1 stabilizes the latter. Finally, we show that DPP3 is overexpressed in breast cancer and that elevated levels of DPP3 mRNA correlate with increased NRF2 downstream gene expression and poor prognosis, particularly for ER-positive breast cancer. Our studies reveal novel insights into the regulation of NRF2 and identify DPP3 and an NRF2 transcriptional signature as potential biomarkers for breast cancer prognosis and treatment.
