Sleep-disordered breathing (SDB) is now a significant health problem in today's culture. It ranges from a spectrum of abnormal conditions during sleep from the primary snorer to mild, moderate, or severe obstructive sleep apnea (OSA). SDB also comprises other conditions, such as sleep-related hypoventilation, sleep-related hypoxemia, and central sleep apnea syndromes. : One of the components of the pathophysiology of OSA that remain unclear is the association of allergic rhinitis (AR) in the evolution of OSA. Several studies relate the co-existence of OSA and AR in the common clinical practice, but its correlation was not clear. This review article aimed to review the pathophysiological relationship between OSA and AR in terms of the role of chemical mediators and the effect of AR treatment in support of OSA. : The symptoms of AR further accelerate the clinical progression to OSA development. Inflammatory mediators such as histamine, cysteinyl leukotrienes, and interleukins are found at a high level in AR, which can aggravate AR symptoms such as nasal obstruction, rhinorrhea, and itchiness, which can then lead to sleep disruption in OSA patients. In addition, OSA patients also have increased chemical mediators such as tumor necrosis factor, interleukin 6, and 1, which would activate the T helper 2 phenotypes that can aggravate AR symptoms. This vicious cycle can potentiate each other and worsen the condition. Few studies have shown that treatment of AR can improve OSA, especially the use of intranasal steroid and leukotriene receptor antagonists. : A detailed evaluation of rhinitis symptoms should be made for OSA patients so that they can benefit not only from the improvement of AR but also the good sleep quality.

Obstructive sleep apnea (OSA) is a heterogenous chronic disorder causing hypoxemia, excessive daytime sleepiness, non-refreshing sleep, nocturia, morning headache, irritability, and memory loss. Cardiovascular disease, cognitive impairment, metabolic disorders, and depression are its long-term consequences. The difficulty in treating patients is due to poor compliance, failure to obtain the desired outcome, and complication arising from the multimodality treatment. Direct targeted therapy may overcome these issues. Identification of its phenotypes improves understanding of the disease mechanism, the risk for adverse effects, and predicting response to targeted therapy. Phenotyping of OSA allows treating patients according to their inherent disease and not based on a “one size fits all” method, which may not be applicable for all patients. This approach may improve patients’ compliance with treatment, minimize the associated morbidities, and consequently improve their quality of life.

H1-antihistamines are recognized to be effective for conditions such as allergic rhinitis and chronic spontaneous urticaria. However, management of such conditions in the real-world primary care setting may be challenging due to diverse patient-specific considerations, the wide range of antihistamines available, choice of other treatment modalities, and the complexity of interpreting specialist treatment algorithms. Despite regular updates to international guidelines, regional/national surveys of healthcare professionals show a clear gap between guidelines and real-world practice, particularly at the primary care level. This article thus presents the consensus opinion of experts from relevant specialties in Malaysia - allergology, pediatrics, otorhinolaryngology, and dermtology - on harmonizing the use and choice of antihistamines in primary care. Patient profiling is recommended as a tool to guide primary care practitioners in prescribing the appropriate antihistamine for each patient. Patient profiling is a three-step approach that involves 1) identifying the individual's needs; 2) reviewing patient-specific considerations; and 3) monitoring treatment response and referral to specialists in more severe or difficult-to-treat cases. Concurrently, guidelct 3ines should be reviewed and updated periodically to include recommendations that are easily actionable for primary care practitioners.

Chronic rhinosinusitis (CRS) affects 14% of the general population. The Sino-Nasal Outcome Test 22 (SNOT-22) is a reliable instrument assessing the impact of CRS. This study aimed to examine the reliability and construct validity of the translated Bahasa Malaysia version of SNOT-22.This cross-sectional study was conducted in the Otorhinolaryngology clinic in Universiti Sains Malaysia (USM). Seventy CRS respondents and 39 healthy participants were included. The Bahasa Malaysia translated SNOT-22 (bmSNOT-22) was produced using rigorous forward and backward translation. Statistical analyses used included feasibility, Cronbach's alpha, intraclass correlation coefficient, Pearson's correlation coefficient and factor analysis.The feasibility was 97.2% in the initial test and 100% in the retest. The Cronbach's alpha was 0.89 in the initial test. The average intraclass correlation coefficient (ICC) was 0.90, indicating good test-retest reliability. The bmSNOT-22 discriminated between the control group and patients (The bmSNOT-22 is reliable, and validity established therefore recommended for Malaysia's clinicians and researchers as a measurement tool for the outcome in sino-nasal disorders such as rhinosinusitis and nasal polyps.

Objectives/hypothesis Olfactory dysfunction has been observed as one of the clinical manifestations in COVID-19 patients. We aimed to conduct a systematic review and meta-analysis to estimate the overall pooled prevalence of olfactory dysfunction in COVID-19 patients. Study design Systematic review and meta-analyses. Methods PubMed, Scopus, Web of Science, Embase, and Google Scholar databases were searched to identify studies published between 1 December 2019 and 23 July 2020. We used random-effects model to estimate the pooled prevalence with 95% confidence intervals (CIs). Heterogeneity was assessed using the I2 statistic and Cochran's Q test. Robustness of the pooled estimates was checked by different subgroup and sensitivity analyses This study is registered with PROSPERO (CRD42020183768). Results We identified 1162 studies, of which 83 studies (n = 27492, 61.4% female) were included in the meta-analysis. Overall, the pooled prevalence of olfactory dysfunction in COVID-19 patients was 47.85% [95% CI: 41.20-54.50]. We observed olfactory dysfunction in 54.40% European, 51.11% North American, 31.39% Asian, and 10.71% Australian COVID-19 patients. Anosmia, hyposmia, and dysosmia were observed in 35.39%, 36.15%, and 2.53% of the patients, respectively. There were discrepancies in the results of studies with objective (higher prevalence) versus subjective (lower prevalence) evaluations. The discrepancy might be due to false-negative reporting observed in self-reported health measures. Conclusions The prevalence of olfactory dysfunction in COVID-19 patients was found to be 47.85% based on high-quality evidence. Due to the subjective measures of most studies pooled in the analysis, further studies with objective measures are advocated to confirm the finding. Introduction Tonsillectomy is frequently associated with postoperative pain of considerable duration, which is usually accompanied by the substantial consumption of both opioid and non-opioid analgesic such as NSAIDs and local anaesthetics. Objective The aim of this study was to evaluate the efficacy between 2% viscous lignocaine and sodium diclofenac based upon the visual analogue scores (VASs), consumption of pethidine 0.5 mg kg−1 as the rescue drug postoperatively and time taken to resume feeding. Methods 130 patients aged between 5 and 12 years old were randomly allocated into 2 groups to be given either 2% viscous lignocaine 4 mg kg−1 body weight topically post-tonsillectomy or sodium diclofenac 1 mg kg−1 per-rectal post-induction of anaesthesia. Postoperatively visual analogues score was done for 24 h, the amount of pethidine given and time when the patient start taking oral feeding of clear fluid, soft diet and normal diet were documented. Results There was no significant difference in the visual analogue scores in both groups, however the requirement of pethidine as the rescue drug postoperatively was significant 2 h post-tonsillectomy (p = 0.023) in viscous lignocaine group compared to sodium diclofenac. The time taken to resume oral feeding and soft diet was also significant in viscous lignocaine group (p = 0.016 and p = 0.007) whereas there was no significant in taking normal diet. Conclusion: We concluded that 2% viscous lignocaine applied topically post-tonsillectomy is comparable to sodium diclofenac per-rectal in providing analgesia and faster oral feeding.

Eagle’s syndrome represents a symptomatic styloid process elongation or calcification of stylohyoid or stylomandibular ligament. The symptoms include the throat pain radiating to ipsilateral ear or foreign body sensation in the pharynx causing odynophagia and dysphagia. It is commonly unilateral and bilateral cases are rare. We report a case of bilateral elongation of styloid processes treated surgically by transoral approach.
