Dabei wird zu der Frage der Schmerzauslosung und der Entstehung der Appendicitis im allgemeinen Stellung genommen. Die Lehre vom Primaraffekt der Appendicitis wird mit den klinischen Befunden als unvereinbar abgelehnt. Das Prodromalstadium der Appendicitis wird als ein nervaler Insult wahrscheinlich auf allergischer Grundlage beruhend aufgefast. 2. Das operative Material wurde mit derBielschowsky-Gros-Methode untersucht. Es fanden sich Neubildungen des Nervengewebes, die als Wucherungsvorgange des Terminalreticulums gedeutet wurden. Diese Wucherung geht in einem hyperplastischen Hullgewebe vonstatten, dessen Natur vorlaufig nicht naher bestimmbar ist. Sie beginnt an der Innenseite der Muscularis propria, erreicht weiterhin die Muscularis mucosae und breitet sich in der Propria der Schleimhaut aus.

In the absence of marked intraluminal contamination a mucocele of the appendix may be formed whenever the rate of appendiceal secretion exceeds the rate of absorption and proximal drainage. The majority of mucoceles develop distal to an inflammatory stricture. During a forty-year period eleven cases in which the mucocele was not the result of inflammatory stricture were encountered at the clinic. In seven cases the appendiceal lumen was patent, the mucocele being produced by partially obstructing carcinoma of the cecum; in three cases obstruction was caused by a carcinoid, and in one case obstruction resulted from a polyp. Two mucoceles were associated with appendiceal endometriomas but it was impossible to prove that the endometrial tissue had caused obstruction.

One hundred ninety-eight cases of lymphoid hyperplasia of the appendix, some of which had other associated pathology, have been analyzed. Comparison has been made with an analogous series of acute appendicitis in children. There is no precise differential diagnosis between acute appendicitis and lymphoid hyperplasia of the appendix. Each case has to be watched carefully, and any increase in local signs is an indication for surgery. Lymphoid hyperplasia is frequently the precursor of acute appendicitis.

Diverticulosis of the vermiform appendix of the colon is encountered infrequently in roentgenographic study of the intestinal tract. Whether discovery of these diverticula is important has yet to be determined. It is conceivable, however, that they might have clinical significance. It seems reasonable, therefore, for radiologists to keep this entity in mind and to mention its discovery with at least as much care as that accorded to diverticula in other portions of the alimentary canal. This report concerns two cases in which diverticula of the appendix were visualized in the course of radiologic examination of the colon by barium enema. The first was in a 51-year-old Negro mortician, referred for care because of addiction to Demerol, of two years duration. He had had numerous symptoms for which complete evaluation was undertaken. The appendix when situated in its infantile position in the right hypochondrium represents the highest point of the right side of the colon and may normally remain filled with intestinal gas. 3. As such it casts a linear gas shadow visible in the right hypochondrium. 4. The differential diagnosis from gas contained in the gall bladder, biliary tree and abscess cavities is considered. 5. The possible importance of these radiological appearances in plain films of acute abdominal conditions is indicated and discussed.

A case is reported of a patient with a mucocele of the appendix which presented as a recurring abscess in the right groin.

Mucocele of the appendix was recognized by Rokitansky1in 1842, and was described by Virchow in 1863.2Fere applied the name "mucocele" in 1877.3In 1916 Dodge4found only 142 cases of mucocele reported in the literature, and in 1928 Weaver5added 26 cases, including 2 of his own. Mucocele of the appendix has remained a relatively infrequent disease, occurring in from 0.2%6to 1.5%7of autopsies. Jirka and Scuderi8found an incidence of 0.23% of mucoceles in appendectomies, as compared to an incidence of only 0.043% in an almost equal number of autopsy specimens, indicating that the symptoms are severe enough to lead to operation in these patients.
