The paper provides the analysis of epidemiological process of hemorrhagic fever with renal syndrome (HFRS) in the Russian Federation in the context of federal districts in 2022 and a forecast of the HFRS incidence for 2023. According to the results of the analysis, there was a three-fold increase in the morbidity rates of HFRS in Russia in 2022 as compared to the indicators of 2021. The evidence of epizootiological survey and laboratory studies in certain federal districts of the Russian Federation indicate the continuing tense epidemiological situation on HFRS. In a number of regions of the country, high risk of infection with HFRS is predicted due to the favorable natural and climatic conditions of the winter period 2022/2023 for reservoir hosts of pathogenic for humans Hantaviruses. The findings of infected rodents attest to a high probability of complication of the epidemiological situation in the territories of increased epidemic hazard as regards HFRS. more...

Currently, software products are widely used in the accounting system, which determine new directions for accountants to improve their skills in working with IT technologies. The study of new technologies that are necessary for the analysis of financial information of companies is relevant. The purpose of the study is to study modern technologies used in accounting, which are based on artificial intelligence. The scientific article deepens the concept of artificial intelligence and its use in accounting; the main mechanisms of using new technologies in accounting are considered; prospects for the development of the accountant's specialty are determined. In this study, modern information and analytical problems in accounting for accounting transactions are studied. When writing a scientific article, various methods were used, in particular, systematization, comparison and comparison. The originality and value of the research lies in the identification of modern trends in accounting automation. The article compares the characteristics of changes in the teaching of school geography courses in the Republic of Kazakhstan, the similarities and differences between traditional programs and modernized programs, focusing on changes in the state of modernization in all spheres of society's life today. The direction of the chronological development process of the textbooks of the school geography courses that have switched to the updated system and changed in the new format, is provided. A comparative analysis was made between the components of the content of school geography textbooks of the new model and the features of the geography courses of the world countries. The content and structure of modern geographical education has expanded from the perspective of individual-oriented training and features of geography classes, and looking at the world from an emotional-value attitude, we can clearly see that school geography and academic geography are completely and uninterruptedly connected in the teaching of geography. Introduction. During the COVID-19 pandemic, the effective medical rehabilitation of patients who have suffered community-acquired pneumonia is crucial, and the osteopathic correction can take its rightful place along with other rehabilitation methods. The studies of osteopathic approaches in the treatment of pneumonia are described in available sources for the inpatient stay of patients. The issues of the effectiveness of osteopathic correction in more distant periods of rehabilitation after community-acquired pneumonia are poorly covered and therefore became the basis for the presented work.The aim of the study is to research the effectiveness of the osteopathic correction inclusion in the outpatient rehabilitation program for patients after community-acquired pneumonia.Materials and methods. A prospective, controlled, randomized study was conducted at the Clinic of the Eurasian Institute of Osteopathic Medicine (Bishkek, Kyrgyz Republic) from January to June 2021. 30 patients who have suffered community-acquired pneumonia of mild and moderate severity were examined, including 15 people of the main group who received osteopathic correction every 7 days for 3–4 weeks and simultaneously performed the recommended set of exercises of physical therapy (exercise therapy); 15 people of the control group, without osteopathic correction, only a set of exercise therapy exercises was recommended for these patients. The evaluation of the effectiveness of osteopathic correction was carried out on the basis of the analysis of spirometry, pulse oximetry, functional tests of the Stange and Genchi, and the life quality by the MOS SF-36 questionnaire.Results. Objective: To compare the effects of atorvastatin monotherapy and the combination of atorvastatin with curcetin (a mixture of the bioflavonoids curcumin and quercetin) on lipid profile and inflammatory biomarkers in patients with unstable angina after COVID-19 (“Long COVID”).Material. An open simple comparative randomized study was conducted in 186 patients with unstable angina, including 77 (Group I) in whom angina destabilization occurred as a result of COVID-19 during 4-8 weeks prior to inclusion in the study, and 109 patients (Group II) in whom destabilization was not associated with infection.Results: In group I, the level of hsC-reactive protein [5,4 (2,06-7,4) g/l and IL-6 8,6 (5,4-10,3) pg/ml] was higher (P < 0,05) than in group II patients [3,8 (1,2-4,0) g/l and 6,9 (2,2-10,2) pg/ml], respectively. In subgroup I of patients after COVID-19, atorvastatin monotherapy (n = 43) did not have a significant effect after two months of treatment, while in subgroup II the combined use of atorvastatin with curcetin (n = 34) for 2 months reduced the level of hsCRP by 49,0% (P < 0,05) and Il-6 by 40,0% (P < 0,05).Conclusion. In patients with unstable angina after COVID-19, combination treatment with atorvastatin and curcetin reduced concentrations of inflammatory biomarkers compared with atorvastatin monotherapy. more...

A new processing algorithm based on fractal image compression is proposed for image processing efficiency. An image will partition into non-overlapping blocks called range blocks and overlapping blocks called domain blocks, with the domain blocks generally bigger than the range blocks, to achieve a rapid encoding time. This research introduced a new fast full-search algorithm approach that starts the search for the best matching domain in the range block from the closest points in the range blocks and expands the search until an acceptable match is found or the search is completed to save even more encoding time. The proposed fast full-search approach, despite its simplicity, is more efficient than the standard search method. The search reduction, peak signal to noise ratio, compression ratio, and encoding time of the suggested approach are all examined. The proposed method can encode a 512x512 grayscale Lena image in 0.36 seconds, with a total search reduction of 87% according to experimental results. more...

The incidence of acute destructive pancreatitis is steadily increasing worldwide and, according to the recent literature, is 300-900 patients/mil per year. The increased lethality rate of acute destructive pancreatitis in recent decades, which according to global statistics, ranges from 5% to 20% depending on the severity of the disease, is a cause for concern. It should be noted that most patients with acute destructive pancreatitis are working-age patients, which significantly increases the socio-economic aspect of the problem and urgently requires an active search for ways to solve it. It has been established that the leading pathogenetic mechanism of acute destructive pancreatitis development is enzymatic autolysis of pancreatic parenchyma by its enzymes with their subsequent release into the bloodstream and damage of other organs. more...

this paper presents data on the study of the cultural properties of the genetic variant Omicron of the SARS-CoV-2 virus of the coronavirus infection COVID-19. The most permissive biological systems for the reproduction of this virus variant are Vero, MA-104, MARC-145, PK-15, and SPEV cell cultures. The productive conditions for cultivating the genetic variant Omicron of the SARS-CoV-2 virus in the studied cell lines turned out to be the following parameters: seeding concentration of cells 200 thousand/cm3, multiplicity of infection – 0.01 TCD50/cell, virus cultivation period – 2 days, cell lesion area monolayer before collecting the virus – 75-80%, the biological activity of the virus in the range from 4.14±0.31 to 6.66±0.22 lg TCD50/cm 3. more...

BACKGROUND. There is enough evidence of the negative impact of excess weight on the formation and progression of res piratory pathology. Given the continuing SARS-CoV-2 pandemic, it is relevant to determine the relationship between body mass index (BMI) and the clinical features of the novel coronavirus infection (NCI).AIM. To study the effect of BMI on the course of the acute SARS-COV-2 infection and the post-covid period.MATERIALS AND METHODS. AKTIV and AKTIV 2 are multicenter non-interventional real-world registers. The АКТИВ registry (n=6396) includes non-overlapping outpatient and inpatient arms with 6 visits in each. The АКТИВ 2 registry (n=2968) collected the data of hospitalized patients and included 3 visits. All subjects were divided into 3 groups: not overweight (n=2139), overweight (n=2931) and obese (n=2666).RESULTS. A higher BMI was significantly associated with a more severe course of the infection in the form of acute kidney injury (p=0.018), cytokine storm (pCONCLUSION. Overweight and/or obesity is a significant risk factor for severe course of the new coronavirus infection and the associated cardiovascular and kidney damage Overweight people and patients with the 1st and 2nd degree obesity tend to have a high risk of death of SARS-CoV-2 infection in both acute and post-covid periods. Solar energy generated by photovoltaic (PV) technology can be supplied to standalone systems, as it combines efficiency and cost-effectiveness. However, this combination is achieved only after considering the effects of shading, which can significantly influence electrical output. The primary factor that influences the use of solar energy in electricity generation is irradiation. PV cells are significantly impacted by shading, where the output of the PV cell reduces in the presence of a shadow. In this study, the researchers have presented an experimental analysis of how shading affects two PV cells, using the series and parallel configurations. The experimental work is installed at the University of Mosul, Department of Electrical Engineering, Renewable Lab (Iraq). MATLAB was used to simulate, evaluate, and compared the results to understand the effects of shading on PV cell output. This research offers an analytical technique to determine the probable effects of Partial shadowing conditions on PV power generation. The results provide the effects of partial shadowing in an annual performance loss of ≥10–30%. The orientation of the PV panels' tilt angle has an impact on their output power. Introduction. The problem of endoscopic treatment for urethral strictures and obliterations remains unresolved. It is necessary to note that the effectiveness depends not only on the right indications, but also on the surgical technique and the study of the postoperative period management.Objective. To evaluate the efficacy and safety of endoscopic treatment of patients with bulbar-membranous urethral obliteration.Materials and methods. The study included 103 patients aged 20 to 89 years with bulbar-membrane urethral obliteration, who, for some reason or another, cannot perform urethroplasty. The patients underwent endoscopic recanalization of the urethra under X-ray control with further circular transurethral electroresection (TUR) of the scar tissues in the urethral obliteration zone (after 6 – 7 days).Results. The average age of the patients was 61.1 ± 18.3 years. After the operation, self-urination was restored in all patients. The average maximum urine flow rate (Q max) before discharge from the hospital was 12.6 ± 0.5 ml/s. Throughout the follow-up period, Q max tended to increase and at the end of the study (36 months) reached values of 16.5 ± 0.5 ml/s. During the first year of follow-up, 18 (17.5%) patients developed recurrent urethral stricture. Seventeen (16.5%) patients underwent repeated circular TUR of scar tissue in the zone of recurrent stricture. The effectiveness of the treatment was 90.0%.Conclusion. Endoscopic urethral recanalization followed by TUR of scar tissue in the obliteration zone is an effective and safe method in the treatment of bulbar-membranous urethral obliteration with length less than 1.0 cm. more...

Objective. To assess the effectiveness of the treatment of gunshot wounds of the spine using percutaneous full-endoscopic technique.Material and Methods. Three patients with gunshot shrapnel wounds of the spine were treated using percutaneous full endoscopy.Results. The patients underwent a removal of foreign bodies (metal fragments) at the cervical, thoracic and lumbosacral levels of the spine using percutaneous full endoscopic surgery. The operations were carried out without complications, with minimal additional trauma to soft tissues and the spinal motion segment. In all three cases, there was a positive dynamics in the form of regression of the pain syndrome. There were no infectious complications.Conclusion. The successful use of percutaneous full endoscopy in the surgical treatment of blind shrapnel wounds of the spine is shown. The results indicate the expediency of further research and development of this area to address the issue of introducing the technique into the routine practice of treating gunshot wounds both in peacetime in neurosurgical hospitals and centers of spinal neurosurgery, and in wartime at the stages of specialized care. more...

At the present stage of development, the issue of housing is facing the population of the country and it can be observed that not everyone can afford to buy it. Therefore, the interaction of the state and the population in the implementation of housing loans is becoming especially relevant today. The main purpose of the study is to analyze the interaction of the state and the population in the implementation of housing loans. The main scientific and practical significance of the study lies in the fact that the authors assessed the interaction of the state and the population in the implementation of housing lending at the present stage of development. The methodological basis for the implementation of new forms of housing lending can serve as a theoretical and practical study of the basic principles of housing lending, foreign experience in this field of practice and its adaptation to modern events. As part of the work, it was revealed that in recent years the rate of increase in the issuance of housing loans has increased significantly in Kazakhstan, the growth of which was influenced by state housing lending programs and the ability to apply excess pension savings to obtain, repay mortgages and improve the living conditions of citizens. The authors also analyzed such a program as the housing and communal development program “Nurly Zher” revised for 2020– 2025. Special attention was paid to the housing lending program – “7-20-25” and “Baspana Hit”. Aim to formulate the criteria for evaluating the effectiveness of prevention of non-communicable socially significant diseases on the basis of fitness centers. Material and methods. The longitudinal retrospective study included 370 visitors of fitness centers who attended wellness programs. The adaptation potential (AP) formula according to R.M. Baevsky served the basic method for research. According to the results of screening, the visitors were divided in three groups. Hemodynamics and trophological status were measured in each group. Results. The hemodynamics and trophological status differed depending on the adaptation potential. In the group with the AP score less than 2.6, the SBP at rest was 11311.1 mmHg, DBP 68.17.4 mmHg. In the second group, the SBP was 12611.2 mmHg, DBP = 77.510 mmHg, heart rate = 7510 bpm, BMI = 26.73 kg/m2 and waist/hip ratio= 0.840.17. In the third group: SBP = 142.811.2 mmHg, DBP = 85.710 mmHg, heart rate = 7910 bpm. BMI = 29.84.8 kg/m2, waist/hip ratio =0.900.16. The level of SBP in all three groups after the functional test was 119.5 [95% CI 115123], 125.0 [95% CI 116155] and 144.5 [95% CI 133172] mmHg (H = 52.1227, p = 0.00199), respectively. During the study period, in the first group, the AP score remained at the same level in all age groups. In the second group, there was a decrease of AP score from 2.810.13 to 2.660.21 points. In the third group, we observed the decrease from 3.3 0.14 to 3.13 0.28 points with a sufficient number of visits (101 or more per year) in all age groups. Conclusion. The use of such criterion as adaptation potential makes it possible to assess the effectiveness of mass preventive measures due to more precise differentiation of visitors and the use of homogeneous impact methods. more...

The article explores a clinical observation of a giant postbulbar parapapillary duodenal ulcer with multiple combined complications in the form of penetration into the pancreas, acute gastroduodenal bleeding and subcompensated stenosis occurred against the background of arterio-mesenteric compression of the duodenum in the subcompensation stage. The long course of the disease contributed to the development of alimentary dystrophy, cachexia and severe anemia. After stopping the bleeding, carrying out adequate preoperative preparation, the patient underwent Roux-en-Y gastric resection to “turn off” the ulcer with demucosation of the antrum and formation of the epipyloric stump of the duodenum according to the method modified in the clinic. The postoperative period was complicated by the development of a fistulous variant of duodenal stump suture failure, which was conservatively eliminated. The patient recovered. more...

Systemic lupus erythematosus is a multisystem autoimmune disease characterized by multiorgan system damage. In total 72 cases and 142 matched controls were interviewed between 2008 and 2019. Clinical data for all cases were obtained from the central patients' database. Using a multivariate model, cases with a history of hypertension tended to have an increased risk of the development of SLE (OR 3.7, 95% CI 1.36-7.9). Also, cases were more likely to report angina pectoris compared to controls (OR 4.7, 95% CI 1.6-24). There was a statistically significant association between a family history of any autoimmune disease and an increased risk of SLE (OR 2.25, 95% CI 1.25-4.05). Those who consume >200 grams of alcohol per week had a significantly lower risk of SLE, while very few participants contributed to this evidence. While those who smoke 2 to 5 cigarette packs per week had 2.64 times increased risk of SLE compared to non-smokers (OR = 2.64, 95% CI 0.97-7.18), however, none of the results on this exposure was statistically significant. SLE may be associated both with endogenous and exogenous factors. The purpose of this research is to analyze effect of extrinsic motivation and training on employee performance through career development as intervening variables at PT. Bank Negara Indonesia (Persero) Tbk, Medan Main Branch Office. This type of research is associative research. Population in this study are permanent employees at PT. Bank Negara Indonesia (Persero) Tbk, Medan Main Branch Office, as many as 159 employees. Sampling in this study with probability sampling technique. The criterion used is proportionate stratified random sampling. The number of samples is 114 respondents. The types of data used are primary and secondary data obtained from interview questionnaires and documentation studies. The data analysis method used is path analysis and partial test (t). According to global statistics, 25% of deaths every year are cases of sudden cardiac death. Implantation of cardioverter-defibrillators is currently used as its prevention. In the coming years, the world is projected to see an increase in the number of implantations of cardioverter-defibrillators in high-income countries, as well as an expansion of the range of medical devices offered, including subcutaneous cardioverter-defibrillators, which are currently being considered as a promising alternative. We systematized the literature data presented in various international studies evaluating the efficacy and safety of the use of subcutaneous cardioverter-defibrillators. Areview of the medical literature published in the period from 2015 to 2022 was carried out using the information and analytical systems MEDLINE, Scopus, Clinicaltrials.gov, Google Scholar and Web of Science. The review was performed in accordance with the checklist of preferred reporting items for systematic reviews and meta-analyses. 398original publications were selected, of which 50original scientific articles on the research topic were included in the analysis. Modern international studies have demonstrated the efficacy and safety of the use of subcutaneous implantable cardioverter defibrillators. An improvement in the performance of unmotivated discharges over time was noted, which was associated with the improvement of implantable cardioverter-defibrillators. The operating time for implantation of subcutaneous cardioverter-defibrillators was significantly less compared to transvenous cardioverter-defibrillators. Subcutaneous cardioverter-defibrillators were associated with a lower incidence of perioperative complications and high efficacy of shock therapy. Subcutaneous cardioverter-defibrillators were as effective as transvenous implantable cardioverter-defibrillators and did not have the risk of complications associated with implantation of endocardial electrodes. Comparative data of domestic and foreign scientists on the mechanism of exposure of Helicobacter pylori (НР) to the pathogenesis of resistant iron deficiency anaemia in children are given.It was determined that the development of iron deficiency anaemia in children with HP infection is influenced by many mechanisms, therefore, in the treatment of children with this pathology, the appointment of anti- HP bacterial therapy with ferrotherapy, taking into account age-related characteristics and needs, is justified. more...

The title compounds were synthesized by alkylation of 5-[(4-dimethylamino)phenyl]-1,3,4-oxadiazole-2-thiol with benzyl chloride or 2-chloro-6-fluorobenzyl chloride in the presence of potassium carbonate. Forensic identification and modern biometric technologies have a common basis and have been closely developing together in various ways. Today new biometric technologies have emerged that utilize previously inaccessible properties and characteristics of humans. The article examines how forensic identification principles are applied in the creation of such new technologies.Promising areas of collaboration between forensic experts and biometric technology specialists include collaborative studying the frequency and identification significance of external human characteristics, as well as individual features of movements and actions, determined by habits and skills and used for forensic identification. Treatment of patients with the simultaneous presence of Chiari malformation type I (MC I) and hydrocephalus may include both one-stage and two-stage surgical interventions. The article describes the two-stage surgical treatment experience performed for the first time in the Republic of Tajikistan. Description of the case. A 50-year-old female patient with a three-year history of bursting headache, dizziness, nausea, and vomiting developed impaired coordination of movements and decreased visual acuity. Computed tomography (CT) revealedsigns of MC I: descent of the cerebellar tonsils 5 mm below the level of the foramen magnum and occlusive triventricular hydrocephalus with symmetrical enlargement of the lateral and third ventricles. The patient underwent surgery – endoscopic third ventriculocisternostomy (ETV) in combination with decompressive suboccipital craniectomy, which was supplemented by CI laminectomy. After the surgery, the symptoms regressed. According to the control CT scan, there was a moderate decrease in the size of the lateral and third ventricles, restoration of the normal anatomical structure of the posterior cranial fossa. more...

In article, the technological effect of oil production increase by the inverted nine-point development system into a single-row system transformation is quantified. The anticipatory watering of the production wells located on the same row with the injection wells in the direction of regional stress in the inverted nine-point development system implemented at the oil facility BP14 object is shown. The effect of anticipatory watering is explained by the formation and growth of auto-hydraulic fractures on injection wells. With the effect and in order to increase the watering implementation coefficient the program of transferring highly watered and idle production wells to injection was started at the aim to form the single-row development system directed along the lineregional stress. Analysis of the ten-year project implementation actual results showed that the system transformation on BP14 object will increase the cumulative oil production on nearby 20 % on the latest stage of development. more...

The purpose of this article is to examine borderline cultural practices that aid in the development of a modern person. We focus on the practices of consumption and childhood, which take shape at the frontier of global and local cultures, to explore their anthropo-creating function. We have identified two groups of cultural practices: socializing and individualizing, and we have synthesized subjective characteristics of cultural practices that best fulfill the function of creating a person. The article also presents a mechanism for developing consumer behavioral strategies that are not determined solely by the logic of the historical process of the existence of local cultures. It highlights the inclusive potential of global culture, which recognizes significant differences among peoples in their ideas about the world, value systems, and diverse trends and outcomes of historical development.
