Objectives To assess the impact on the estimated glomerular filtration rate (eGFR) of different tenofovir disoproxil/emtricitabine dosing regimens for HIV pre-exposure prophylaxis (PrEP). Patients and methods We included in the study individuals with baseline eGFR > 50 mL/min/1.73 m2 who initiated PrEP in the ongoing ANRS-PREVENIR PrEP cohort. We retrospectively classified PrEP users in three groups: ‘on-demand’ (reported at ≥75% of study visits), ‘daily’ (≥75% of study visits) or ‘switches’. We compared the area under curve (AUC) of the eGFR variation from baseline (ΔeGFR) between groups using analysis of covariance, and assessed factors associated with a negative AUC of ΔeGFR. Results From May 2017 to October 2020, 1253 PrEP-naïve participants (98% of MSM) were included in the study with a median follow-up of 22 months. 499 (40%), 494 (39%) and 260 (21%) users were in the group daily, on-demand and switches, respectively, for a median number of pills taken per week of 6, 1.7 and 4. The mean AUC of the ΔeGFR was −1.09 mL/min/1.73 m2 in the daily PrEP group, −0.69 mL/min/1.73 m2 in the switches group and +0.18 mL/min/1.73 m2 with on-demand PrEP. In a model adjusted on baseline age and eGFR, the AUC of the ΔeGFR was significantly higher with on-demand PrEP compared to daily PrEP (P = 0.037). Independent factors associated with a negative AUC of ΔeGFR were a daily PrEP regimen, a switches regimen, an age > 40 years and a baseline eGFR≥90 mL/min/1.73 m². Conclusions On-demand PrEP dosing had a smaller impact on eGFR evolution than daily PrEP, but the difference was not clinically relevant.

Background Extended-spectrum β-lactamase–producing Enterobacterales (ESBL-E) represent a major threat to public health. Little is known on their potential for sexual transmission. Methods We recruited individuals at a sexually transmitted infection and human immunodeficiency virus (HIV) outpatient clinic in Paris, France, in whom we evaluated the prevalence of ESBL-E intestinal carriage and, among those testing positive, the proportion with clearance 6 months thereafter. We compared carriage prevalence between groups using logistic regression adjusted for age, geographic origin, travel outside Europe, and antibiotic use in the past 6 months. Results A total of 2157 individuals participated, of whom 226 (10.5%) were ESBL-E carriers. The proportions of ESBL-E carriers varied across sexual groups and were as follows: HIV-negative men who have sex with men (MSM) and who were on preexposure prophylaxis (PrEP), 16.3% (41 of 251); HIV-negative MSM not on PrEP, 9.7% (47 of 487); HIV-positive MSM, 12.2% (61 of 500); HIV-negative men who have sex exclusively with women, 10.0% (44 of 439); and HIV-negative women who have sex with men, 6.9% (n = 33 of 480). After adjustment, ESBL-E prevalence was significantly higher in HIV-negative MSM on PrEP (P Conclusion ESBL-E carriage is more frequent in MSM undergoing PrEP or living with HIV and with increasing number of sexual partners. More research is warranted to understand the consequences of ESBL-E carriage in these populations and how transmission can be reduced.

BackgroundVariant-adaptated vaccines against coronavirus disease 2019 (COVID-19) as boosters are needed to increase a broader protection against SARS CoV-2 variants. New adjuvanted recombinant protein vaccines as heterologous boosters could maximize the response.MethodsIn this randomized, single-blinded, multicenter trial, adults who had received two doses of Pfizer-BioNTech mRNA vaccine (BNT162b2) 3 to7 months before were randomly assigned to receive a boost of BNT162b2, Sanofi/GSK SARS-CoV-2 adjuvanted recombinant protein MV D614 (monovalent parental formulation) or SARS-CoV-2 adjuvanted recombinant protein MV B.1.351 vaccine (monovalent Beta formulation). The primary endpoint was the percentage of subjects with a ≥10-fold increase in neutralizing antibody titers for the Wuhan (D614) and B.1.351 (Beta) SARS-CoV-2 viral strains between day 0 and day 15.FindingsThe percentages of participants whose neutralizing antibody titers against the Wuhan (D614) SARS-CoV-2 strain increased by a factor ≥10 between day 0 and day 15 was 55.3% (95% CI 43.4-66.7) in MV D614 group (n=76), 76.1% (64.5-85.4) in MV B.1.351 (Beta) group (n=71) and 63.2% (51.3-73.9) in BNT162b2 group (n=76). These percentages were 44.7% (33.3-56.6), 84.5% (74.0-92.0) and 51.3% (39.6-63.0) for the B.1.351 (Beta) viral strain, respectively. Higher neutralizing antibodies rates against Delta and Omicron BA.1 variants were also elicited after Sanofi/GSK MV Beta vaccine compared to the other vaccines. Comparable reactogenicity profile was observed with the three vaccines.InterpretationHeterologous boosting with the Sanofi/GSK Beta formulation vaccine resulted in a higher neutralizing antibody response against Beta variant but also the original strain and Delta and Omicron BA.1 variants, compared with mRNA BNT162b2 vaccine or the Sanofi/GSK MVD614 formulation. We performed a single-centre retrospective study to evaluate the effectiveness of Toxoplasma gondii prevention strategies after allogeneic stem-cell transplantation. The charts of 138 allogeneic stem-cell recipients over a 4-year period were reviewed. Forty-nine percent of patients were not receiving optimal trimethoprim-sulfamethoxazole (TMP-SMZ) prophylaxis at day +30, mainly due to persistent cytopenia. Six months after transplantation, the rate of toxoplasmosis reactivation was 11.6%, including nine cases of Toxoplasma infection and seven cases of Toxoplasma disease. Fifty-six percent of cases of reactivation occurred before day +30. Thirty-eight percent occurred in patients receiving atovaquone prophylaxis. In 57% of patients presenting with Toxoplasma disease, signs of disease were present at first evidence of Toxoplasma DNA in peripheral blood samples. This study illustrates the limitations inherent to currently used toxoplasmosis prevention strategies and argues for the use of a combined prophylactic and preemptive approach. After performing the initial study, we limited the use of atovaquone in favour of TMP-SMZ when possible, and implemented an early prevention strategy consisting of the introduction of prophylaxis starting on day of engraftment. Over the following 16 months, 88.9% of eligible Toxoplasma-seropositive patients were receiving TMP-SMZ at day +30, and the rate of early Toxoplasma reactivation was 1.5%.

The Bacillus Calmette-Guerin (BCG) is a life-attenuated form of Mycobacterium bovis widely used as immunotherapy for localized bladder cancer. Adverse reactions to intravesical BCG instillations are rare. We describe a 70-year-old man with a history of an aortobifemoral bypass graft, placement of a synthetic mesh for treatment of a ventral hernia and, most recently, superficial bladder cancer treated with BCG therapy. Ten months after his final intravesical BCG instillation, he complained of fever and asthenia. After 12 months of investigation, he was diagnosed with Mycobacterium bovis infection of his aortobifemoral bypass graft and abdominal mesh, with Streptococcus intermedius superinfection. The bypass graft was excised and replaced with an in situ arterial allograft, the abdominal mesh was removed, and treatment started with amoxicillin, isoniazid, rifampicin and ethambutol. Several additional vascular interventions were needed for allograft degradation, but 12 months after the final procedure, outcome was good. Among 35 cases of mycotic aneurysm reported after BCG therapy in the last 10 years, only one involved a vascular prosthesis. Surgical repair of such aneurysms using prosthetic grafts is commonly performed, associated with anti-mycobacterial treatment. Prognosis is poor with mortality of 14% (4/35) and a 26% rate of aneurysm recurrence under treatment (9/35).

Background Kaposi sarcoma (KS)–associated herpesvirus (KSHV) subtype depends mostly on patient origin. The current study aimed to assess KSHV diversity in a population of men who have sex with men (MSM) living in France. Methods The study included 264 patients. In 65 MSM, including 57 human immunodeficiency virus (HIV)–infected men with KS, multicentric Castleman disease, or primary effusion lymphoma and 8 HIV-uninfected men receiving HIV preexposure prophylaxis (PrEP), we performed KSHV typing with K1 open reading frame Sanger and KSHV whole-genome sequencing. In 199 other patients, we performed real-time polymerase chain reaction screening for the new variant. Results We found that 51% of KSHV-strains were subtype C (85% C3), and 33% were subtype A. Four patients with severe KSHV disease (2 with visceral KS, 1 with multicentric Castleman disease, and 1 with primary effusion lymphoma) and 1 asymptomatic PrEP user had a new variant resembling the Ugandan subtype F, but with different K1 open reading frame and KSHV whole-genome sequences and a different epidemiological context (MSM vs African population). Its prevalence was 4.5% in Caucasian MSM, and it was absent in other epidemiological groups. Conclusions Subtype C predominated among MSM living in France. The new F variant was identified in Caucasian MSM and associated with severe KSHV disease, suggesting that subtype F could be split into F1 and F2 variants. Careful screening for this variant may be required in MSM, given the severe clinical presentation of associated diseases.

Persistent post-acute coronavirus disease 2019 (COVID-19) symptoms (PACSs) have been reported up to 6 months after hospital discharge. Herein we assessed the symptoms that persisted 12 months (M12) after admission for COVID-19 in the longitudinal prospective national French coronavirus disease cohort.Hospitalized patients with a confirmed virological diagnosis of COVID-19 were enrolled. Follow-up was planned until M12 after admission. Associations between persistence of ≥3 PACSs at M12 and clinical characteristics at admission were assessed through logistic regression according to gender.We focused on participants enrolled between 24 January 2020 and 15 July 2020, to allow M12 follow-up. The M12 data were available for 737 participants. Median age was 61 years, 475 (64%) were men and 242/647 (37%) were admitted to intensive care units during the acute phase. At M12, 27% (194/710) of the participants had ≥3 persistent PACS, mostly fatigue, dyspnoea and joint pain. Among those who had a professional occupation before the acute phase, 91 out of 339 (27%) were still on sick leave at M12. Presence of ≥3 persistent PACS was associated with female gender, both anxiety and depression, impaired health-related quality of life and Medical Muscle Research Council Scale57. Compared with men, women more often reported presence of ≥3 persistent PACSs (98/253, 39% vs. 96/457, 21%), depression and anxiety (18/152, 12% vs. 17/268, 6% and 33/156, 21% vs. 26/264, 10%, respectively), impaired physical health-related quality of life (76/141, 54% vs. 120/261, 46%). Women had less often returned to work than men (77/116, 66% vs. 171/223, 77%).One fourth of the individuals admitted to hospital for COVID-19 still had ≥3 persistent PACSs at M12 post-discharge. Women reported more often ≥3 persistent PACSs, suffered more from anxiety and depression and had less often returned to work than men.

International audience; BackgroundThere are few data available regarding the use of on-demand pre-exposure prophylaxis (PrEP) for HIV prevention. We aimed to assess PrEP effectiveness, adherence, and safety in adults using daily or on-demand PrEP.MethodsWe conducted a prospective observational cohort study (ANRS PREVENIR) at 26 sites in the Paris region, France. We enrolled HIV-negative adults (aged ≥18 years) at high risk of HIV infection who were starting or continuing PrEP. PrEP was prescribed as a fixed-dose combination of tenofovir disoproxil and emtricitabine (245 mg and 200 mg, respectively, per pill). PrEP could be prescribed as a daily regimen with one pill per day or, in men who have sex with men (MSM) or in transgender women who have sex with men, as an on-demand regimen following the IPERGAY dosing recommendation. At enrolment and every 3 months thereafter, participants were tested for HIV and provided information regarding the PrEP dosing regimen used. Adherence to PrEP was assessed by self-report and by tenofovir diphosphate concentrations in dried blood spots. The primary outcome of HIV-1 incidence was assessed using Poisson regression among participants who started PrEP. This study is registered with ClinicalTrials.gov, NCT03113123, and EudraCT, 2016A0157744.FindingsBetween May 3, 2017, and May 2, 2019, 3082 people were assessed for eligibility and 3065 participants were enrolled. 3056 (99·7%) of 3065 participants reported using PrEP and were included in the analyses. The median age was 36 years (IQR 29–43), 1344 (44·0%) of 3056 participants were PrEP-naive, and 3016 (98·7%) were MSM. At enrolment, 1540 (50·5%) of 3049 participants opted for daily PrEP dosing and 1509 (49·5%) opted for on-demand PrEP dosing; these proportions remained stable during follow-up. Median follow-up was 22·1 months (IQR 15·9–29·7) and incidence of study discontinuation was 17·6 participants (95% CI 16·5–18·7) per 100 person-years. At the data cutoff on Sept 30, 2020, there had been six HIV-1 seroconversions (three participants using daily PrEP and three using on-demand PrEP; all were MSM) over 5623 person-years. Overall HIV-1 incidence was 1·1 cases (95% CI 0·4–2·3) per 1000 person-years, and did not differ between participants using daily PrEP and those using on-demand PrEP (incidence rate ratio 1·00, 95% CI 0·13–7·49; p=0·99). Four participants (two using daily PrEP and two using on-demand PrEP) discontinued PrEP due to treatment-related adverse events (nausea [n=2], vomiting and diarrhoea [n=1], and lumbar pain [n=1]).InterpretationIn this study, which enrolled mainly MSM, HIV-1 incidence on PrEP was low and did not differ between participants using daily PrEP and those using on-demand PrEP. Introduction A l’echelle mondiale, la tuberculose reste la premiere cause de deces d’un agent infectieux unique en 2019. La letalite de cette pathologie est beaucoup plus importante dans les pays du Sud, mais n’est pas pour autant nulle en France. Mourir d’une maladie curable et d’evolution classiquement lente dans un pays possedant une couverture maladie universelle parait inacceptable. Le but de ce travail etait d’identifier les facteurs de risque de deceder de tuberculose en France. Materiels et methodes Nous avons mene une etude de cohorte retrospective dans deux hopitaux francais, entre 2013 et 2018. Tous les patients presentant une infection a Mycobacterium tuberculosis complex confirmee a la culture et avec un antibiogramme multisensible etaient inclus. Les donnees socio-demographiques, biologiques et cliniques ont ete recueillies a partir des dossiers medicaux informatises. Une analyse univariee puis multivariee via un modele de Cox a ete realisee afin d’identifier les facteurs de risque de mortalite intra-hospitaliere par tuberculose. Resultats 523 patients ont ete inclus, dont 362 hommes (69 %) avec un âge moyen de 37 ans. 430 (82 %) patients etaient nes a l’etranger, dont 308 sur le continent africain (59 %). 24 patients sont decedes au cours de l’hospitalisation (letalite de 4,5 %), et la tuberculose a ete consideree comme la cause directe du deces dans 3/4 des cas. L’âge (Hazard Ratio 1,1 pour chaque annee supplementaire), avoir un logement precaire (HR 5,9), etre ne en France (HR 8,0), avoir une consommation excessive d’alcool (HR 4,2) avoir un antecedent de cancer (HR 7,1), avoir une forme de tuberculose neuromeningee ou miliaire (HR 8,2) etaient associes a un sur-risque de mortalite intra-hospitaliere. Conclusion La letalite de la tuberculose reste elevee pour une maladie curable. Notre etude confirme un role clef de la precarite. De maniere plus inattendue, les patients nes en France etaient plus a risque de deceder que les migrants. Cela souleve de nombreuses questions, dont celle d’un retard diagnostique eventuel. Dans tous les cas, ces resultats soulignent que la tuberculose ne doit pas etre consideree comme une maladie du passe en France ni comme une problematique ne touchant que les populations immigrees.

Patients with hematological malignancies and COVID-19 display a high mortality rate. In such patients, immunosuppression due to underlying disease and previous specific treatment impairs humoral response, limiting viral clearance. Thus, COVID-19 convalescent plasma (CCP) therapy appears to be a promising approach through the transfer of neutralizing antibodies specific to SARS-CoV-2.Our study reports the effect of CCP in a cohort of patients with hematological malignancies and COVID-19 between 1 May 2020 and 1 April 2021. Overall, 112 hospitalized patients with severe COVID-19 (83 B-cell neoplasm, 19 plasma cell neoplasm, and 10 myeloid neoplasm) were included. The overall survival of the whole cohort was 65% [56–74.9] and 77.5% [68.5–87.7] for patients with B-cell neoplasm. Prior anti-CD20 monoclonal antibodies therapy was associated with better overall survival whereas age, high blood pressure, and COVID-19 severity were associated with a poor outcome after CCP transfusion. A retrospective analysis in the subgroup of COVID-19 patients with B-cell neoplasm treated with CCP (n=81) was compared to a similar group of patients (n=120) treated only with standard of care. An inverse probability of treatment weighting (IPTW) approach, performed to limit confusion and immortality bias, revealed a decreased mortality of 63% (95% CI=31%–80%) in the main analysis and 50% (95% CI=28%–66%) in the overall population of the CCP-treated group of patients with similar findings in the other sensitivity analyses.Convalescent plasma may be beneficial in COVID-19 patients with B-cell neoplasm who are unable to mount a humoral immune response. Objective To genetically characterize clusters of virulence factors (VFs) among extended spectrum β-lactamase (ESBL)-producing Escherichia coli and Klebsiella pneumoniae and assess whether these clusters are associated with genetic determinants or clinical outcomes. Methods One hundred forty-eight E. coli and 82 K. pneumoniae clinical isolates were obtained from 213 patients in Paris, France. Isolates underwent ESBL characterization, MultiLocus Sequence Typing (MLST) typing and phylogenetic group identification. Detection of ten E. coli and seven K. pneumoniae VF-encoding genes were assessed, from which a k-medians partition algorithm with Jaccard similarity measure was used to construct clusters. Results CTX-M was the predominant ESBL and susceptibility to trimethoprim-sulfamethoxazole (32%), ciprofloxacin (22%) and aminoglycosides (32%) was low. Introduction Le diagnostic microbiologique repose principalement sur des methodes de culture, de serologies ou de PCR ciblees. Certaines maladies infectieuses restent neanmoins difficiles a diagnostiquer. La metagenomique clinique (MGC) est une technique de sequencage a haut debit permettant le diagnostic etiologique de processus infectieux, independamment de la nature du pathogene responsable. Cette technique, qui constitue un challenge par la complexite de sa mise en œuvre, pourrait ameliorer le diagnostic microbiologique. Neanmoins, peu d’etudes ont ete publiees sur le benefice pour les patients. Ce travail est un retour d’experience de l’utilisation en routine de cette technologie innovante. Materiels et methodes Nous avons realise une etude retrospective cherchant a decrire les caracteristiques cliniques et microbiologiques de tous les patients ayant beneficie d’un MGC prospectivement entre le 15/02/18 et le 15/01/20 pour le diagnostic de leur maladie infectieuse ainsi que l’impact du resultat. Les prelevements ont ete traites par le protocole MetaMIC incluant une pre-extraction par bead-beating, une extraction par QiaSymphony, une preparation de banque ADN et ARN (Illumina), un sequencage sur NextSeq500 (Illumina) et une analyse par MetaMIC analysis. Resultats Pendant le periode de l’etude, 208 prelevements ont ete techniques. La MGC etait demandee par les cliniciens pour le diagnostic d’infections neurologiques (40 % ; 85/208) principalement des encephalites (n = 67), mais egalement d’infections disseminees (25 % ; 51/208) et cardiovasculaires (6 % ; 13/208). Les prelevements etaient positifs dans 33 % des cas (69/208), dont 65 % (45/69) pour une bacterie et 55 % (38/69) pour un virus. Au total, 47 des 69 resultats positifs (23 %) ont ete consideres comme contributifs par les cliniciens. Parmi ces cas, l’analyse a abouti a la decouverte d’un pathogene que les recherches standard n’avaient pas permis d’identifier comme une endocardite a Bartonella quintana, une aortite a Haemophilus influenzae, ou encore des encephalites a AMPV-1 et a Orthobunyaviridae (decouverte d’un nouveau virus). L’analyse retrospective de ces cas montre l’interet de la MGC dans les cas d’infections decapitees, dues a des microorganismes inattendus, non detectes par les techniques habituelles ou inconnues. Conclusion Cette etude represente la plus large cohorte de patients ayant beneficie d’une MGC jamais presentee. Les demandes recues par le laboratoire correspondaient souvent a des situations d’impasse diagnostique. Malgre cela, la MGC a permis de poser des diagnostics correspondant a des infections rares ou des situations inhabituelles, suggerant de tres bonnes performances. A l’avenir, des etudes evaluant la MGC en premiere ligne dans des indications plus courantes devront confirmer ces performances et permettront d’elargir ses indications.

Introduction La diffusion des EBLSE est un probleme majeur de sante publique. Les differents maillons de transmission des EBLSE ne sont pas tous elucides. La transmission orofecale et donc sexuelle de nombreux agents infectieux a ete montree, y compris pour les SARM. Nous postulons que les EBSLE peuvent se transmettre par voie sexuelle. Materiels et methodes Nous avons realise une etude de cohorte transversale chez 2095 personnes a haut risque d’IST (en CeGIDD ou dans un SMIT). Les consultants repondaient a un questionnaire de pratique puis realisaient un auto-ecouvillon anal a la recherche de BMR/BHRe. Chez les patients porteurs, un recueil de selles avec quantification de la EBLSE et un suivi a 6 mois etaient realises. Resultats Parmi les 2095 personnes inclus a l’etude, il y avait 417 (19,9 %) femmes heterosexuelles, 488 (23,3 %) hommes ayant des rapports sexuels avec des hommes (HSH) VIH-negatifs non sous prophylaxie pre-exposition au VIH (PrEP), 251 (12,0 %) HSH VIH-negatifs sous PrEP, 500 (23,9 %) HSH VIH-positifs et 439 (21,0 %) hommes exclusivement heterosexuels VIH-negatifs. L’âge mediane etait a 32 ans [IQR : 25–44]. Le nombre median de partenaires sexuels depuis les derniers 6 mois etait a 4 [IQR : 2–11] et etait plus eleve chez les HSH VIH- sous PrEP (18, IQR : 7–33) et moins eleve chez les femmes heterosexuelles (2, IQR : 1–3) ou hommes heterosexuels (2, IQR : 1–4) (p Conclusion La colonisation a EBLSE est associee au nombre de partenaires sexuels dans les 6 derniers mois, independamment d’avoir pris des antibiotiques, suggerant fortement une transmission sexuelle des EBLSE dans des rapports homo- ou heterosexuels.

Introduction Le depistage systematique de Chlamydia trachomatis (CT) chez les migrants n’est pas recommande par la Haute Autorite de sante (HAS) en France ni par l’European Center for Disease Prevention and Control (ECDC). Afin d’evaluer l’interet d’integrer le depistage systematique de CT au bilan de sante des migrants, nous avons estime la prevalence du portage de CT et identifie les facteurs de risques associes chez ceux venant consulter en centre de depistage des infections sexuellement transmissibles (CeGIDD). Materiels et methodes Tous les sujets majeurs asymptomatiques consultant dans le CeGIDD d’un centre hospitalier universitaire entre janvier 2017 et decembre 2018 ont ete inclus dans l’analyse. Ceux nes en France ont ete definis comme des « non-migrants », ceux nes a l’etranger et vivant en France depuis moins d’un an comme des « primo-arrivants » et ceux vivant en France depuis plus d’un an comme « migrants de longue duree ». Le depistage de CT a ete effectue par biologie moleculaire sur des prelevements urinaires, vaginaux, oraux ou rectaux en fonction du genre et des pratiques sexuelles. Les prevalences ont ete estimees et les OR ajustes (ORa) ont ete calcules par regression logistique avec le logiciel Stata®. Resultats Sur la periode d’etude, 4948 individus dont 771 (16 %) migrants ont ete analyses. La prevalence de CT etait de 7,7 % (n = 41) parmi les « migrants de longue duree », 5,9 % (n = 14) parmi les « primo-arrivants » et 5,1 % (n = 212) parmi les « non-migrants ». Par rapport aux individus « non-migrants » le risque de portage de CT parmi les migrants etait plus eleve chez les « primo-arrivants » et chez les « migrants de longue duree », ORa = 2,50 [IC95 % : 1,19–5,24] et ORa = 2,04 [IC95 % : 1,43–2,93] respectivement. Les autres facteurs associes au portage de CT etaient : le nombre de partenaires sexuels dans l’annee precedente (ORa = 1,51 [IC95 % : 1,06–2,17] pour 6–10 et ORa = 2,47 [IC95 % : 1,44–4,23] pour > 10 partenaires), l’orientation sexuelle (ORa = 0,26 [IC95 % : 0,14–0,48] pour les hommes ayant des relations avec des hommes), l’âge (ORa = 4,48 [IC95 % : 2,35–8,56] pour les Conclusion Au vu de la prevalence plus elevee chez les personnes migrantes que chez les non-migrantes, le depistage systematique de CT devrait etre recommande chez tous les migrants, et des conseils de prevention ciblee, en particulier chez les femmes jeunes et en contexte de multipartenariat.

Objective To genetically characterize clusters of virulence factors (VFs) among extended spectrum β-lactamase (ESBL)-producing Escherichia coli and Klebsiella pneumoniae and assess whether these clusters are associated with genetic determinants or clinical outcomes. Methods One hundred forty-eight E. coli and 82 K. pneumoniae clinical isolates were obtained from 213 patients in Paris, France. Isolates underwent ESBL characterization, MultiLocus Sequence Typing (MLST) typing and phylogenetic group identification. Detection of ten E. coli and seven K. pneumoniae VF-encoding genes were assessed, from which a k-medians partition algorithm with Jaccard similarity measure was used to construct clusters. Results CTX-M was the predominant ESBL and susceptibility to trimethoprim–sulfamethoxazole (32%), ciprofloxacin (22%) and aminoglycosides (32%) was low. Introduction Les migrants sont tres exposes a la multiresistance aux antibiotiques et aux infections en raison d’une prevalence elevee dans les pays d’origine et conditions de vie precaires pendant et apres la migration. Il existe peu de donnees sur la colonisation par des Bacteries MultiResistantes (BMR) et Hautement Resistantes emergentes (BHRe) chez les migrants arrivant en France. Par ailleurs, il n’existe pas de recommandation francaise sur le depistage des maladies infectieuses des primo-arrivants. Materiels et methodes Les migrants, arrives depuis moins de 1 an, consultant ou hospitalises dans un service de maladies infectieuses et tropicales d’un centre hospitalo-universitaire ont ete inclus. L’objectif principal etait d’evaluer le taux de colonisation par une BMR (Staphylococcus aureus resistant a la meticilline (SARM), enterobacterie productrice de β-lactamase a spectre etendu (EBLSE) ou une BHRe (enterobacterie productrice de carbapenemase, enterocoque resistant a la vancomycine) et les facteurs de risque associes. Les prevalences du VIH, de l’Ag HBs, du VHC, de la syphilis, de Chlamydia trachomatis (CT), de Neisseria gonorrhoeae et de schistomiases ont ete evaluees. Les antecedents d’abus physique ou sexuel et les symptomes de depression ou syndrome de stress post-traumatique (PTSD) ont egalement ete recueillis. Resultats Sur 1 an, 101 patients ont ete inclus dont 79 etaient originaires d’Afrique subsaharienne (79 %) et 45 etaient des demandeurs d’asile (45 %). Cinq souches de SARM (5,4 %, n = 92) et 15 EBLSE (16,3 %, n = 92) ont ete isolees a partir des prelevements de depistage. Aucune BHRe n’a ete retrouvee. En analyse univariee, la colonisation par une BMR etait significativement associee a la seropositivite au VIH et a la duree de la migration. Dans cette population, la prevalence du VIH, des infections chroniques par le VHB et le VHC a ete estimee a 40 % [IC 95 %, 30,1–49,1], 33 % [IC 95 %, 23,5–41,8] et 5 % [IC 95 %, 0,7–9,2] respectivement. Onze patients sur 73 avaient une serologie schistosomiase positive (15 %). Six patients sur 77 avaient une infection a CT (7,8 %). Des antecedents de violence physique pendant la migration ont ete signales chez 43 % des patients (41/96) et des violences sexuelles chez 43 % des femmes (12/28). Trente et un patients (32 %) presentaient des symptomes de depression ou de PTSD. Conclusion Le taux de colonisation par une BMR est eleve dans cette population ainsi que le taux d’infections bacteriennes ou virales et de troubles psychiatriques. Des recommandations sur la prise en charge des migrants apparaissent ainsi necessaires.

Introduction La tuberculose est la premiere cause de mortalite par un seul agent infectieux dans le monde. L’Organisation mondiale de la sante a etabli comme objectif prioritaire de sante publique une reduction de 95 % du nombre de deces entre 2015 a 2035. Pour relever ce defi, une meilleure comprehension des facteurs pronostiques de deces par tuberculose semble indispensable. Dans les pays a forte endemie, le VIH et la denutrition sont des facteurs de risque de mortalite, mais ces donnees sont rares en Europe. Dans un contexte d’augmentation de la prevalence de la tuberculose en Ile de France en 2016 et en 2017, nous avons estime la mortalite par tuberculose multisensible et les facteurs de risque associes au deces chez les patients hospitalises. Materiels et methodes Il s’agit d’une etude de cohorte retrospective portant sur le suivi de patients âges de plus de 18 ans, presentant une tuberculose multisensible confirmee bacteriologiquement et hospitalises entre 2013 et 2017 dans un centre hospitalo-universitaire. Resultats Parmi 182 patients presentant une tuberculose multisensible, 12 sont decedes (taux de mortalite : 6,6 % IC95 % [3,8–11,2]). La localisation de l’atteinte etait exclusivement pulmonaire dans 7 cas, exclusivement neuromeningee dans 1 cas et disseminee dans 4 cas. Aucun patient n’etait infecte par le VIH. Cinq d’entre eux etaient de nationalite francaise et 7 patients de nationalite etrangere, dont 5 vivant en France depuis plus de 5 ans et 2 depuis moins d’un an. Une situation de grande precarite (sans domicile fixe, situation irreguliere) a ete retrouvee chez 50 % de ces patients, tandis qu’une intoxication ethylique a ete rapportee chez 58 % d’entre eux. La cause du deces a ete consideree comme directement imputable a la tuberculose dans 83 % des cas (2 SDRA, 1 choc septique, 1 defaillance neurologique, 6 defaillances multiviscerales dans un contexte de denutrition severe), indirectement dans 1 cas (infection du liquide d’ascite concomitante chez un patient cirrhotique), non-decrite dans 1 cas. Conclusion Alors que la tuberculose multisensible beneficie en France d’un traitement simple et gratuit dans un contexte sanitaire favorable a une prise en charge rapide, le taux de mortalite releve dans la population de notre etude apparait particulierement eleve. L’analyse des caracteristiques des patients montre que la grande precarite et l’alcoolisme (mais pas la migration recente) semblent etre les facteurs de risque les plus associes au risque de deceder d’une tuberculose multisensible et implique de reconsiderer la politique sanitaire a destination des plus exclus de la societe.

Introduction Les personnes vivant avec le VIH (PVVIH) peuvent developper des pathologies auto-immunes dont la prevalence semble etre proche de celle de la population generale. Des traitements immunosupresseurs (IS) peuvent etre indiques, pour lesquels les prescripteurs peuvent avoir des reticences du fait d’un potentiel surrisque infectieux. L’objectif de cette etude etait d’evaluer la prevalence des complications infectieuses sous immunosupresseurs chez les PVVIH, d’evaluer leur efficacite et leur impact sur le bilan immunovirologique. Materiels et methodes Il s’agissait d’une etude retrospective multicentrique descriptive. Les PVVIH de chaque centre etaient selectionnes selon la classification CIM-10 des PAI. Les PVVIH etaient inclus s’ils etaient majeurs, sous IS depuis plus d’1 an, sans antecedent de transplantation d’organe ou de neoplasie. Les donnees concernant les PAI, les infections et les caracteristiques immunovirologiques des patients etaient recueillies sur un document standardise. Resultats Neuf mille quatre cent quatre-vingt PVVIH ont ete selectionnes, parmi lesquels 138 avaient une PAI et 32 etaient sous IS (23 %). L’âge median etait de 34,5 ans [26 ;29], 44 % (14/32) etaient des femmes. La duree mediane de l’infection VIH etait de 9,8 ans (4,17,5), le mode de contamination etait heterosexuel a 47 % (15/32), et 16 % (5/32) presentaient un stade C dans leur histoire. Ils presentaient principalement une spondylarthropathie (28 %, 9/32), une MICI (22 %, 7/32) et un lupus erythemateux systemique (6 %, 2/32). Les IS les plus frequemment utilises etaient le methotrexate (37 %,12/32), suivi de l’azathioprine et de l’infliximab (16 %, 5/32). Quatorze patients (43 %) ont necessite plusieurs lignes therapeutiques. Des corticoides ont ete associes chez 56 % des patients (18/32). La mediane du suivi sous traitement etait de 3,8 ans [2,7 ;5,9]. Au dernier suivi 81 % des patients sont en remission de leur maladie auto-immune. La mise sous IS ne semble pas avoir modifies defavorablement les parametres immunovirologiques sur le long terme. Sous traitement, en excluant les IST, 16 evenements infectieux ont ete declares (0,5 evenement/individus), concernant 9 patients (28 %). Sept evenements ont necessite une hospitalisation (44 %). Les infections pulmonaires etaient le type d’infection le plus frequent. Aucun deces n’a ete associe a un evenement infectieux. Conclusion Il ne semble pas y avoir de risque infectieux majore sous IS chez les PVVIH comparativement a la population generale sous IS, avec une bonne reponse therapeutique et une stabilite du bilan immunovirologique. Lorsqu’ils sont indiques et que le bilan immunovirologique le permet, les IS doivent etre envisages chez les PPVIH.

Introduction Le diagnostic microbiologique de la tuberculose pleurale ou peritoneale est souvent difficile a obtenir avec la culture du liquide pleural ou peritoneal. La biopsie pleurale ou peritoneale, consideree comme le « gold standard », est un examen plus sensible (environ 50–60 %) mais est une procedure invasive. Le diagnostic est donc souvent indirect, associant des arguments clinicobiologiques et anatomopathologiques sans documentation microbiologique. Or, la propagation de souches resistantes aux antibiotiques rend necessaire l’obtention de l’antibiogramme afin d’optimiser l’antibiotherapie. Materiels et methodes Nous avons inocule dans des flacons d’hemoculture BD BACTEC™ MycoF lytic 6 liquides pleuraux et 1 liquide d’ascite de 7 patients differents. Nous avons compare retrospectivement leurs taux de positivite a ceux obtenus par culture conventionnelle (BD MGIT tube™) de liquides et de biopsies pleurales et peritoneales pour 18 autres patients dont les dossiers etaient disponibles. Resultats Dans le groupe BD BACTEC™ MycoF lytic, une documentation microbiologique a ete retrouvee dans 86 % des cas (6/7) contre 72 % dans le second groupe (13/18). Le delai de positivite des cultures etait de 24 jours en mediane dans le 1er groupe et de 31 jours dans le 2e groupe. L’antibiogramme a ete obtenu en 54 jours dans le 1er groupe, contre 60 jours dans le 2e groupe. Le delai median d’introduction du traitement etait de 9 jours dans le 1er groupe contre 10 dans le 2e. Conclusion L’inoculation du liquide pleural et peritoneal sur milieux BD BACTEC™ MycoF lytic semble etre au moins aussi sensible que le gold standard. Il s’agit d’une procedure simple qui permet d’eviter les procedures invasives, sans retarder le debut de l’antibiotherapie. Cependant, notre etude repose sur un echantillon de petite taille et manque de puissance. Une etude sur un plus grand nombre de patients est actuellement en cours.

Introduction Pres de 200 patients consultent quotidiennement dans le service d’accueil des urgences (SAU) d’un hopital universitaire. Certains d’entre eux necessitent un avis en maladies infectieuses ou une reevaluation apres prise en charge initiale. Depuis novembre 2017, un circuit court a ete mis en place entre le SAU et l’hopital de jour (HDJ) du service de maladies infectieuses et tropicales (MIT) : les patients sont adresses en HDJ via un signalement par fax apres leur passage aux urgences. L’objectif de cette evaluation est de demontrer la pertinence de cette consultation post-urgence (CPU) et de decrire le service rendu au malade. Materiels et methodes Entre novembre 2017 et janvier 2018, tous les patients consultant au SAU pour une pathologie infectieuse et necessitant un avis ulterieur ont ete signales a l’HDJ par fax. Les patients ont ensuite ete convoques par telephone, dans un delai variable, selon l’indication. Les motifs de venue aux urgences et les caracteristiques des patients ont ete collectes a l’aide des logiciels de suivi clinico-administratifs URQUAL, ORBIS et DIAMM. Resultats Cinquante-neuf patients ont ete adresses a l’HDJ pendant la periode de l’analyse, 38 d’entre eux ont honore leur consultation (64 %). L’âge moyen des patients etait de 44 ans et 21 etaient de sexe masculin (56 %). Les diagnostics initiaux retenus aux urgences etaient : 11 pathologies respiratoires (29 %), 9 digestives (23 %), 7 urinaires (18 %), 6 cutanees (16 %), 3 tropicales (8 %), 1 infection sexuellement transmissible et 1 mise a jour de vaccination. Parmi ces patients, 16 etaient sortis du SAU avec une antibiotherapie (42 %). Cette antibiotherapie a ete modifiee pour 1 patient (reduction du spectre) et arretee pour 1 patient (12 %). Une antibiotherapie a ete instauree chez 3 autres patients (19 %). Deux patients ont ete hospitalises et 7 ont beneficie d’un suivi prolonge (18 %). Au total, 14 patients ont vu leur prise en charge modifiee par cette consultation (37 %). Ces donnees seront actualisees pour les 4 mois a venir soit en juin 2018. Conclusion La mise en place d’un circuit court pour CPU a permis de reevaluer des patients atteints de pathologies infectieuses variees. La plupart des patients ont honore leur consultation. Une adaptation de l’antibiotherapie a pu etre realisee (modification ou instauration). L’extension de cette consultation semble importante dans le cadre d’une politique de meilleure prescription des antibiotiques. Au total, plus d’1/3 des patients ont vu leur prise en charge modifiee.

Objectives Boosted PIs are commonly prescribed in patients presenting with advanced HIV infection. We assessed the efficacy and tolerability of once-daily ritonavir-boosted atazanavir or darunavir plus two NRTIs in HIV-1-infected ART-naive patients with severe immunosuppression, targeting at least an 85% success rate at week 48. Methods This 48 week, open-label, non-comparative, randomized, multicentre trial included ART-naive patients with CD4 cell counts 1000 copies/mL and without genotypic mutations conferring resistance to the study drugs. Patients were randomized (1:1) to receive once-daily atazanavir/ritonavir (300/100 mg) or darunavir/ritonavir (800/100 mg) plus tenofovir disoproxil fumarate/emtricitabine or abacavir/lamivudine. The primary endpoint was treatment success, defined as plasma HIV-1 RNA ≤50 copies/mL at week 48 and no permanent PI/ritonavir discontinuation. The study was registered with ClinicalTrials.gov (NCT01928407). Results One hundred and twenty patients were enrolled: 77% were men, 30% were born in Africa and 39% had AIDS. The median baseline CD4 and plasma HIV-RNA values were 69 cells/mm(3) and 5.4 log10 copies/mL. All but four patients received tenofovir disoproxil fumarate/emtricitabine. The week 48 treatment success rate was 66% (95% CI 54%-78%) with atazanavir/ritonavir and 80% (95% CI 68%-89%) with darunavir/ritonavir. The median CD4 cell count increased similarly in the two groups (Δweek 48 to week 0: +194 cells/mm(3)). Adverse events occurred in 23 and 18 patients, respectively. Conclusions Despite good adherence, neither study regimen reached the predefined objective, suggesting a need for more potent regimens for patients with advanced HIV infection.

To the Editor: Klebsiella pneumoniae is mainly responsible for hospital-acquired urinary tract infections, bacteremia, pneumonia and intra-abdominal infections. However, since the mid-1980s, K. pneumoniae has also been described as the cause of highly invasive community-acquired infections (1,2). The K. pneumoniae isolates associated with such infections are often hypermucoviscous and frequently belong to the capsular serotypes K1 or K2. Two of the most extensively studied genes associated with invasive infections are a mucoviscosity-associated gene A (magA) in serotype K1 and a regulator of mucoid phenotype A (rmpA). These strains of hypervirulent K. pneumoniae (hvKP) are now circulating worldwide (1,2).

Background: Assessing the satisfaction of patients about the health care they have received is relatively common nowadays. In France, the satisfaction questionnaire, I-Satis, is deployed in each institution admitting inpatients. Internet self-completion and telephone interview are the two modes of administration for collecting inpatient satisfaction that have never been compared in a multicenter randomized experiment involving a substantial number of patients. Objective: The objective of this study was to compare two modes of survey administration for collecting inpatient satisfaction: Internet self-completion and telephone interview. Methods: In the multicenter SENTIPAT (acronym for the concept of sentinel patients, ie, patients who would voluntarily report their health evolution on a dedicated website) randomized controlled trial, patients who were discharged from the hospital to home and had an Internet connection at home were enrolled between February 2013 and September 2014. They were randomized to either self-complete a set of questionnaires using a dedicated website or to provide answers to the same questionnaires administered during a telephone interview. As recommended by French authorities, the analysis of I-Satis satisfaction questionnaire involved all inpatients with a length of stay (LOS), including at least two nights. Participation rates, questionnaire consistency (measured using Cronbach alpha coefficient), and satisfaction scores were compared in the two groups. Results: A total of 1680 eligible patients were randomized to the Internet group (n=840) or the telephone group (n=840). The analysis of I-Satis concerned 392 and 389 patients fulfilling the minimum LOS required in the Internet and telephone group, respectively. Introduction Les bacteriemies sont des pathologies graves avec une morbi-mortalite importante. Le delai d’introduction de l’antibiotherapie efficace est un facteur pronostic majeur des bacteriemies. L’emergence et la diffusion des bacteries multiresistantes rendent de plus en plus difficile la juste prescription des antibiotiques. L’identification rapide de la bacterie et de ses genes de resistance au cours d’une bacteriemie permettrait d’optimiser l’antibiotherapie ce qui ameliorerait la prise en charge du malade, limiterait la selection de bacteries encore plus resistantes et diminuerait le cout pour l’hopital. Nous avons realise une etude observationnelle evaluant les pratiques d’antibiotherapie et les consequences cliniques de ces pratiques avant et apres utilisation du Verigene® (Nanosphere, USA). Materiels et methodes Tout patient hospitalise ayant un premier episode de bacteriemie en 2015 a ete inclus et les donnees ont ete recueillies de maniere prospective (groupe V). Les patients ayant eu une bacteriemie en 2014 (avant l’utilisation du Verigene®) ont ete inclus et leurs donnees ont ete recueillies de maniere retrospective (groupe T). L’objectif principal de ce projet etait de documenter l’impact de l’utilisation du test Verigene® sur le delai d’antibiotherapie efficace. Les objectifs secondaires etaient de documenter son impact sur le delai d’antibiotherapie optimale, la mortalite precoce et a J30, la consommation d’antibiotiques a dispensation controlee, la duree d’hospitalisation globale et en unite de soins intensifs. Resultats Cent vingt-sept patients du groupe V ont ete compares a 94 patients du groupe T. Les 2 groupes etaient comparables en termes de sexe, âge, comorbidites et facteurs d’immunodepression. L’âge median etait de 64 ans. Deux tiers des hemocultures etaient positives a bacilles Gram negatif et 20 % a Staphylocoque sp. Le delai entre le prelevement et l’identification bacterienne etait de 28 heures dans le groupe V et 52 heures dans le groupe T (p Conclusion Le Verigene® a ete efficace pour identifier les especes bacteriennes et les genes de resistance avec un gain de 24 heures par rapport a la procedure bacteriologique classique. L’impact clinique sur l’antibiotherapie n’a pas pu etre demontre dans cette etude.

Introduction L’infection a Chlamydia trachomatis (Ct) est l’infection sexuellement transmissible (IST) la plus frequemment rapportee dans le monde. Son incidence est en augmentation depuis 10 ans en France. L’epidemiologie a ete bien decrite chez les femmes mais il existe peu de donnees chez les hommes. Le depistage est realise de maniere ciblee chez les femmes de moins de 25 ans dans des centres gratuit de depistage et de diagnostic des infections sexuellement transmissibles (CeGIDD). Les recommandations insistent sur la necessite de realiser des etudes complementaires avant de l’etendre a la population generale. Le depistage systematique des hommes a recemment ete mis en œuvre au CeGIDD de notre hopital. L’objectif de notre etude etait de determiner la prevalence et les facteurs de risques actuels de l’infection uro-genitale a Ct chez les hommes consultant au CeGIDD afin d’evaluer la pertinence d’un tel depistage dans ces structures et de mieux cibler le depistage en population generale. Materiels et methodes Un depistage par PCR Kit CT/NG Abbot Realtime sur les urines du premier jet a ete propose a tous les hommes consultant au CeGIDD de notre hopital d’avril a octobre 2016. Les caracteristiques demographiques (âge, sexe, origine) et du comportement sexuel (orientation sexuelle declaree, nombres de partenaires dans les 12 derniers mois, date du dernier rapport non protege, antecedent d’IST) etaient recueillies par un questionnaire au cours de la consultation. Resultats Pendant la duree de l’etude, 627 hommes ont ete depistes dont 36 etaient positifs. La prevalence etait de 5,7 % dans notre population et de 6,3 % chez les moins de 30 ans. La mediane d’âge etait de 27 ans IIQ[25–35]. Les consultants etaient majoritairement europeens (n = 27, 75 %), heterosexuels (n = 29, 80,6 %) et avaient eu un nombre median de 4,5 partenaires dans les 12 derniers mois. En analyse univariee les patients infectes avaient un nombre de partenaires median plus eleve (4,5 vs 3, p = 0,03), avaient eu plus de rapports sexuels non proteges de moins de 6 semaines (66,7 % vs 42,5 %, p = 0,004) et etaient plus souvent symptomatiques que les patients non infectes (16,7 % vs 5,4 %, p = 0,01). En analyse multivariee, le caractere symptomatique (OR = 3,5[1,4–9], p = 0,01) et l’antecedent de rapport sexuel non protege datant de moins de 6 semaines (OR = 2,71[1,33–5,52], p = 0,004) restaient significatifs. Conclusion La prevalence de l’infection a Ct est elevee chez les hommes consultant au CeGIDD. Ces resultats apportent des arguments supplementaires en faveur d’un depistage systematique des hommes de moins de 30 ans consultant en CeGGID et posent la question du depistage en population generale. Des etudes complementaires de cout-efficacite devront etre menees pour reactualiser les recommandations de depistage.

Depuis la decouverte de son role dans les colites postantibiotiques en 1978, Clostridium difficile est devenu le principal agent de diarrhees associees aux soins. Les souches toxinogenes sont responsables de 10 a 25 % des diarrhees postantibiotiques et de plus de 95 % des cas de colites pseudomembraneuses (CPM). Depuis une dizaine d’annees, C. difficile a connu une evolution epidemiologique tout a fait particuliere a travers le monde : i) de nombreuses epidemies d’infections severes ont ete decrites, ii) l’incidence des infections a C. difficile (ICD) a augmente dans de nombreux hopitaux d’Amerique du Nord et d’Europe [1], iii) une souche dite « hypervirulente » (souche 027/NAP1/BI) a emerge et diffuse rapidement [2, 3], et iiii) davantage d’echecs cliniques ont ete rapportes avec le metronidazole [4].

Summary There has been a major paradigm shift in the management of HIV infected patients, with earlier initiation of antiretroviral treatment and lifelong exposure to drugs for which long-term safety issues must be faced by clinicians. Within the past 5 years, new drugs from both previously established and novel therapeutic classes have been released that tend to be safer and more efficient than their former combinations. Although hepatotoxicity was one of the most common side effects from initial antiretrovirals, phase II/III safety data regarding liver tolerance from more recent drugs are reassuring. However, data on the long-term exposure to these therapeutic options are needed, and a handful of case reports are emerging, reporting rare but potentially life-threatening adverse hepatic events in patients with hepatitis co-infection or taking other hepatotoxic drugs.

The outcome of bacterial bloodstream infections during pregnancy has greatly improved over the last few decades. However, there are no recent data on the characteristics of bacteremia in pregnant women. The aim of this study was to describe clinical and microbiological features of bacteremia and to assess maternal and fetal outcome. This retrospective study was conducted in the obstetrics departments of five teaching hospitals in Paris, France, from 2005 to 2009. The incidence of bacteremia was 0.3%. The most common sources of bacteremia were chorioamnionitis (47%) and the most common pathogen isolated was Escherichia coli. Empirical antimicrobial therapy was inappropriate in 29% of bacteremia cases, mostly (65%) when secondary to infection with an aminopenicillin-resistant microorganism. Bacteremia during pregnancy was associated with a 10% fetal mortality. Bacteremia during pregnancy is a rare occurrence, but it is associated with an unexpectedly poor fetal outcome and a high mortality rate.
