Se presenta una hoja didáctica basada en la comprensión lectora del artículo de referencia sobre el premio de química 2022 sobre la química clic. La estrategia se presenta para apoyar a los profesores en la difusión del conocimiento de la tecnología que aplicada a contextos biológicos es la base de lo conoce como química bioortogonal, de gran relevancia en síntesis bioquímica y farmacológica más eficiente y específica. La actividad consiste en un cuestionario (idealmente aplicado vía Google Forms, para contar con datos estadísticos de las respuestas de manera inmediata) dirigido a estudiantes nivel medio, medio superior o universitario. Esto se realiza en un contexto que permite, dependiendo de la unidad o tema de estudio del curso, introducir y revisar ejemplos de conceptos, modelos y metodologías de vanguardia en campos que van desde la química orgánica hasta la biología molecular. Asimismo, la historia del descubrimiento del sistema, el desarrollo de la tecnología para sus aplicaciones y el Premio Nobel al que fueron acreedores los tres investigadores involucrados constituyen un contexto perfecto para fomentar la reflexión acerca del carácter colaborativo de la ciencia y la frontera entre la llamada ciencia básica y la ciencia aplicada.

10. The Legitimacy Death Penalty Application of Certain Conditions in the Anti-Corruption Law

- Abstract
This article discusses the imposition of the death penalty as stipulated in article 2 paragraph (2) of the Corruption Eradication Law for perpetrators of criminal acts of corruption that are deemed to be detrimental to the State and can have a wide impact on the lives of many people. In this case, there are many pros and cons related to the imposition of the death penalty as stipulated in article 2 paragraph (2) of the Corruption Eradication Law, especially in the sentence “Certain conditions” in that article which are related to the corruption of social assistance funds for handling Covid-19. Apart from that, this article is also considered to be against the Government’s obligations in the effort to respect, protect and fulfill human rights. This article concludes that this article cannot fulfill the juridical aspect of prosecuting corruption actors because it is not included in the requirements of “certain conditions” and is also considered unconstitutional because it is not in accordance with the constitution, which provides protection for a person’s right to life. The imposition of the death penalty has also been proven to be inappropriately used in eradicating corruption, as seen in the 2019 Corruption Perception Index.

The aim of this study was to evaluate outcomes of real-world patients with aortic stenosis (AS) undergoing transcatheter aortic valve replacement (TAVR) with the 34 mm Evolut R (Medtronic, Minneapolis, Minnesota).Larger aortic annulus has been associated with increased incidence of paravalvular leaks (PVLs) after TAVR. However, little is known, so far, about the performance of the 34 mm Evolut R in this setting.From the multicenter, international, retrospective Horizontal Aorta in Transcatheter Self-expanding Valves (HORSE) registry, including patients who underwent TAVR for native severe AS, we selected patients treated with the 34 mm Evolut R evaluating procedural characteristics and VARC-2 defined device success. We also compared 34 mm Evolut R with other Evolut R sizes.Among the 4434 patients included in the registry, 572 (13%) received the 34 mm Evolut R valve. Mean age was 80.8 ± 6.5 years and the median STS PROM score was 4 [interquartile range 2-6]. Device success was achieved in 87.4% with 7.7% of PVLs; moreover, the rate of permanent pacemaker implantation (PPMI) was 22.4%. Patients who underwent 34 mm Evolut R implantation experienced more in-hospital permanent pacemaker implantation (22.4% vs. 15%; p 0.001). At multivariate analysis, 34 mm Evolut R did not affect device success (OR: 0.81 [0.60-1.09]; p = 0.151). Device success was consistent with other THVs sizes (87.4% vs. 89.6%; p = 0.157).THV replacement in patients requiring 34 mm Evolut R has an acceptable performance. Compared to other Medtronic sizes it demonstrated to be comparable in terms of device success, despite an increased rate of pacemaker implantation.

Background Data on Device-related Thrombus (DRT) after left atrial appendage closure (LAAC) remain scarce. This study aimed to investigate risk factors for DRT from centers reporting to the EUROC-DRT registry. Methods We included 537 patients (112 with DRT and 425 without DRT) who had undergone LAAC between 12/2008 and 04/2019. Baseline and implantation characteristics, anti-thrombotic treatment and clinical outcomes were compared between both groups in uni- and multivariate analyses. Additional propensity-score matching (PSM) was conducted to focus on the role of implantation characteristics. Results Patients with DRT showed higher rates of previous stroke/transient ischemic attack (TIA) (49.1% vs. 34.7%, p < 0.01), spontaneous echocardiographic contrast (SEC) (44.9% vs. 27.7%, p < 0.01) and lower left atrial appendage (LAA) peak emptying velocity (35.4 +/- 18.5 vs. 42.4 +/- 18.0 cm/s, p= 0.02). Context In the last century European forests are experiencing tree damage and mortality rise and it is expected to continue due to increased disturbances under global change. Disturbances generally creates canopy gaps, which leads to secondary succession, compositional changes and landscape mosaic transformations. Forest gap characterization has traditionally been performed in light-limited tropical and boreal forests, but no studies have been found on water-limited Mediterranean forests. Characterising canopy gaps and their dynamics in Mediterranean forests will help to better understand their dynamics across landscapes under ongoing global change. Objectives We aimed to characterize canopy gaps and quantify their dynamics identifying hotspots of openings and closings in Mediterranean forests. Methods We used low density multitemporal airborne LiDAR data between 2010 and 2016, over a large region (Madrid, Spain, 1732.7 km2) with forests ranging from monospecific conifer and broadleaved to mixed forests, to delineate canopy gaps. The characterization was made through its Gap Size Frequency Distribution (GSFD) by forest type and year. We analysed canopy gap dynamics and identified statistically significant hotspots of gap openings and closings in each forest type. Results There were major differences between conifers and broadleaved forest in terms of gap characteristics and GSFD. In general, we found a great dynamism in Mediterranean forests with high rates of forest openings and closings, but a net closing trend. A high spatial heterogeneity was observed finding hotspots of gap openings and closings across the entire study area. Conclusions We characterised for the first-time large-scale structure and dynamics of canopy gaps in Mediterranean forests. Our results represents the characterisation of the GSFD of Mediterranean forests and could be considered a benchmark for future studies. The provision of up-to-date periodic maps of hotspots of gap opening, closing and net change help to understand landscape mosaic changes as well as to prioritise forest management and restoration strategies.

Background Accelerated epigenetic ageing can occur in untreated HIV infection and is partially reversible with effective antiretroviral therapy (ART). We aimed to make a long-term comparison of epigenetic ageing dynamics in people with HIV during untreated HIV infection and during suppressive ART., Methods In this longitudinal study, conducted over 17 years in HIV outpatient clinics in Switzerland, we applied 5 established epigenetic age estimators (epigenetic clocks) in peripheral blood mononuclear cells (PBMCs) in Swiss HIV Cohort Study participants before or during suppressive ART. All participants had a longitudinal set of PBMC samples available at four timepoints (T1-T4). T1 and T2 had to be 3 years or longer apart, as did T3 and T4. We assessed epigenetic age acceleration (EAA) and a novel rate of epigenetic ageing., Findings Between March 13, 1990, and Jan 18, 2018, we recruited 81 people with HIV from the Swiss HIV Cohort Study. We excluded one participant because a sample did not meet quality checks (transmission error). 52 (65%) of 80 patients were men, 76 (95%) were white, and the median patient age was 43 (IQR 37 center dot 5-47) years. Per year of untreated HIV infection (median observation 8 center dot 08 years, IQR 4 center dot 83-11 center dot 09), mean EAA was 0 center dot 47 years (95% CI 0 center dot 37 to 0 center dot 57) for Horvath's clock, 0 center dot 43 years (0 center dot 3 to 0 center dot 57) for Hannum's clock, 0 center dot 36 years (0 center dot 27 to 0 center dot 44) for SkinBlood clock, and 0 center dot 69 years (0 center dot 51 to 0 center dot 86) for PhenoAge. Per year of suppressive ART (median observation 9 center dot 8 years, IQR 7 center dot 2-11), mean EAA was -0 center dot 35 years (95% CI -0 center dot 44 to -0 center dot 27) for Horvath's clock, -0 center dot 39 years (-0 center dot 50 to -0 center dot 27) for Hannum's clock, -0 center dot 26 years (-0 center dot 33 to -0 center dot 18) for SkinBlood clock, and -0 center dot 49 years (-0 center dot 64 to -0 center dot 35) for PhenoAge. Our findings indicate that people with HIV epigenetically aged by a mean of 1 center dot 47 years for Horvath's clock, 1 center dot 43 years for Hannum's clock, 1 center dot 36 years for SkinBlood clock, and 1 center dot 69 years for PhenoAge per year of untreated HIV infection; and 0 center dot 65 years for Horvath's clock, 0 center dot 61 years for Hannum's clock, 0 center dot 74 years for SkinBlood clock, and 0 center dot 51 years for PhenoAge, per year of suppressive ART. GrimAge showed some change in the mean EAA during untreated HIV infection (0 center dot 10 years, 0 center dot 02 to 0 center dot 19) and suppressive ART (-0 center dot 05 years, -0 center dot 12 to 0 center dot 02). We obtained very similar results using the rate of epigenetic ageing. Contribution of multiple HIV-related, antiretroviral, and immunological variables, and of a DNA methylation-associated polygenic risk score to EAA was small., Interpretation In a longitudinal study over more than 17 years, epigenetic ageing accelerated during untreated HIV infection and decelerated during suppressive ART, highlighting the importance of limiting the duration of untreated HIV infection.

In multilingual societies, it is common to encounter different language varieties. Various approaches have been proposed to discuss different mechanisms of language shift. However, current models exploring language shift in languages in contact often overlook the influence of language ideologies. Language ideologies play a crucial role in understanding language usage within a cultural community, encompassing shared beliefs, assumptions, and feelings towards specific language forms. These ideologies shed light on the social perceptions of different language varieties expressed as language attitudes. In this study, we introduce an approach that incorporates language ideologies into a model for contact varieties by considering speaker preferences as a parameter. Our findings highlight the significance of preference in language shift, which can even outweigh the influence of language prestige associated, for example, with a standard variety. Furthermore, we investigate the impact of the degree of interaction between individuals holding opposing preferences on the language shift process. Quite expectedly, our results indicate that when communities with different preferences mix, the coexistence of language varieties becomes less likely. However, variations in the degree of interaction between individuals with contrary preferences notably lead to non-trivial transitions from states of coexistence of varieties to the extinction of a given variety, followed by a return to coexistence, ultimately culminating in the dominance of the previously extinct variety. By studying finite-size effects, we observe that the duration of coexistence states increases exponentially with network size. The article examines the neighbourhood of Lower Ninth Ward in New Orleans following the Hurricane Katrina in 2005. It focuses on the rebuilding project initiated by Make it Right Foundation (2007-ongoing), founded by actor Brad Pitt. The article looks into media images of the rebuilding of Lower Ninth, and notes that the rebuilt architecture cannot be analysed outside of its relationship with media and images. The rebuilt architecture in Lower Ninth relates to image in multiple levels - it represents image of social justice and resilience, and aims to assign a new (image) value to the suburb. The article analyses the rebuilding within the framework of the relationships between contemporary architecture and media image and argues that these relationships are more complex than perceived by current architectural literature. It sees these relationships as productive in the case of this rebuilding, and aims to expand the space of their analysis.

This study aims to analyse the effectiveness of a psychosocial intervention programme in preventing loneliness and increasing self-efficacy and social participation among older women living alone in Spain. A sequential, nested experimental design was used, with a primary quantitative quasi-experimental pre-post study with a control group (CG) and a secondary qualitative study. A total of 48 women participated, and 34 of them (the experimental group, EG) received a 25-hour psychosocial support intervention delivered by volunteers who focused on three aspects: conversation, attribution retraining and behavioural activation. The other women remained on the waiting list (CG = 14). The women (EG and CG) were all interviewed before and after the intervention and a follow-up of the EG was performed at 6 months (N = 25). Semi-structured interviews were used to collect sociodemographic and health-related data, as well as data on satisfaction with the programme and its benefits. Validated instruments were used to collect data on the dependent variables (self-efficacy in ageing, subjective social participation and loneliness). Quantitative and qualitative methods were used for data analysis. Compared to the pre-test results, women in the EG improved their self-efficacy in ageing (t-test: -2.246; p: 0.031; d: 0.30), and, more specifically, their self-efficacy in managing socio-emotional problems such as loneliness (t-test: -1.995; p: 0.054; d: 0.25) and in managing their health (t-test: -2.450; p: 0.020; d: 0.47). No significant differences were observed between the follow-up and the post-test results, meaning that the changes identified after the intervention remained stable. Four additional categories of benefits were identified in the participants' discourses. In conclusion, the intervention appears to be beneficial and effective in improving self-efficacy, which is a well-established predictor of loneliness in old age, in a group of vulnerable individuals. Further studies on this type of community intervention are necessary to prevent loneliness among older people.

Objectives The aim of this study was to evaluate clinical characteristics, mechanisms of failure, and outcomes of transcatheter aortic valve replacement (TAVR) explantation. Background Surgical explantation following TAVR may be required for structural valve degeneration, paravalvular leak, infection, or other reasons. However, in-depth data on indications and outcomes are lacking. Methods Data from a multicenter, international registry (EXPLANT-TAVR) of patients who underwent TAVR explantation were reviewed retrospectively. Explantations performed during the same admission as initial TAVR were excluded. Clinical and echocardiographic outcomes were evaluated. Median follow-up duration was 6.7 months (interquartile range [IQR]: 1.0-18.8 months) after TAVR explantation and was 97.7% complete at 30 days and 86.1% complete at 1 year. Results From November 2009 to September 2020, 269 patients across 42 centers with a mean age of 72.7 ± 10.4 years underwent TAVR explantation. About one quarter (25.9%) were deemed low surgical risk at index TAVR, and median Society of Thoracic Surgeons risk at TAVR explantation was 5.6% (IQR: 3.2%-9.6%). The median time to explantation was 11.5 months (IQR: 4.0-32.4 months). Balloon-expandable and self-expanding or mechanically expandable valves accounted for 50.9% and 49.1%, respectively. Indications for explantation included endocarditis (43.1%), structural valve degeneration (20.1%), paravalvular leak (18.2%), and prosthesis-patient mismatch (10.8%). Redo TAVR was not feasible because of unfavorable anatomy in 26.8% of patients. Urgent or emergency cases were performed in 53.1% of patients, aortic root replacement in 13.4%, and 54.6% had concomitant cardiac procedures. Overall survival at last follow-up was 76.1%. In-hospital, 30-day, and 1-year mortality rates were 11.9%, 13.1%, and 28.5%, respectively, and stroke rates were 5.9%, 8.6%, and 18.7%, respectively. Conclusions The EXPLANT-TAVR registry reveals that surgical risks associated with TAVR explantation are not negligible and should be taken into consideration in the lifetime management of aortic stenosis.

Background Malnutrition is associated with poor prognosis in several cardiovascular diseases. However, its prognostic impact in patients undergoing transcatheter edge‐to‐edge mitral valve repair (TEER) is not well known. This study sought to assess the prevalence, clinical associations, and prognostic consequences of malnutrition in patients undergoing TEER. Methods and Results A total of 892 patients undergoing TEER from the international MIVNUT (Mitral Valve Repair and Nutritional Status) registry were studied. Malnutrition status was assessed with the Controlling Nutritional Status score. The association of nutritional status with mortality was analyzed with multivariable Cox regression models, whereas the association with heart failure admission was assessed by Fine‐Gray models, with death as a competing risk. According to the Controlling Nutritional Status score, 74.4% of patients with TEER had any degree of malnutrition at the time of TEER (75.1% in patients with body mass index 2 , 72.1% in those with body mass index ≥25 kg/m 2 ). However, only 20% had moderate–severe malnutrition. TEER was successful in most of patients (94.2%). During a median follow‐up of 1.6 years (interquartile range, 0.6–3.0), 267 (29.9%) patients died and 256 patients (28.7%) were admitted for heart failure after TEER. Compared with normal nutritional status moderate–severe malnutrition resulted a strong predictor of mortality (adjusted hazard ratio [HR], 2.1 [95% CI, 1.1–2.4]; P P =0.015). Conclusions Malnutrition is common among patients submitted to TEER, and moderate–severe malnutrition is strongly associated with increased mortality and heart failure readmission. Assessment of nutritional status in these patients may help to improve risk stratification.

Background Valve-in-valve (ViV) transcatheter aortic valve implantation (TAVI) can be completed by bioprosthetic valve fracture (BVF) to reduce final transvalvular gradients. The aim was to compare outcomes in ViV-TAVI patients with versus without BVF. Methods Consecutive patients undergoing ViV-TAVI procedure in four international centers were included, from 2010 to 2021. We used a 1:2 propensity score-matching method to compare postprocedural hemodynamic, complications, and long-term outcomes. Patients were matched for baseline characteristics, time since prior surgery, and characteristics of surgical bioprothesis (type and size). Results A total of 390 patients were analyzed, including 40 BVF. Propensity matching 1:2 yielded 38 patients in BVF group and 76 patients in no-BVF group. There was no difference in procedural complications rate and in-hospital deaths (5.1%) between the two groups. Post-procedural hemodynamic parameters significantly improved using BVF: aortic valve area (1.4 cm2 [IQR: 1.23 to 2.3] vs. 1.3 cm2 [IQR: 1.05 to 1.61], p=0.008), mean aortic gradient (12mmHg [IQR: 7.5 to 16.5] vs. 17mmHg [IQR: 11 to 22], p=0.008) and peak velocity (2.2m/s [IQR: 1.8 to 2.7] vs. 2.6m/s [IQR: 2.2 to 3.1], p=0.027). BVF had an additional benefit in the smallest surgical valve (≤21 mm). The use of BVF was independently associated with improved hemodynamic parameters. Overall survival in the matched cohort was 87.8±3.7% at 2-year follow-up, without difference between groups (87.5±6.9% in the BVF group vs. 88.4±4.2% in the no-BVF group, p=0.85). Conclusion Compared to ViV-TAVI alone, BVF was safe and improved immediate hemodynamic and long-term outcomes, especially in patients with small surgical aortic bioprosthesis. Since the initial experience with the CoreValve (Medtronic, Minneapolis, Minnesota), there have been continuous iterations of this valve system in order to improve procedural success and reduce periprocedural complications. The Evolut Pro, Pro+, and FX are the latest generations of this transcatheter heart valve (THV).This review paper aims to analyze the main characteristics and clinical evidence about the Evolut Pro THV and summarize the main iterations of the newer generation Evolut FX valve system.The Evolut Pro system has been associated with good clinical outcomes and excellent valve hemodynamic performance including reduced rates of paravalvular leaks. Technical enhancements to improve valve positioning, orientation, and vascular access have been implemented in the newer generation Evolut FX system.

- Abstract
This article evaluates the technical feasibility of using low-resolution thermal detectors to monitor the thermal comfort of swine through surface temperature. We built two low-resolution detectors prototypes and compared their captures to a commercial high-resolution sensor. We carried out the experiments at Roçadinho Farm, a pig farm property located in the municipality of Capoeiras, Pernambuco. This study showed that, by adopting the proposed usage practices, the thermal detectors evaluated have the potential for appropriately monitoring environmental thermal comfort in swine farming.

Although there are no convincing evidences of detrimental effect of SARS-CoV2 infection on the cerebellum, the COVID-19 pandemic could impact the life quality of patients with cerebellar ataxias, but few studies have addressed this concern. To assess the motor and mental health changes caused by the COVID-19 pandemics in Cuban patients with cerebellar ataxias, three hundred four patients with cerebellar ataxias and 167 healthy controls were interviewed for risks of exposure to COVID-19, and the self-perception of the pandemics’ impact on the disease progression and on the mental health. All subjects underwent the Hospital Anxiety and Depression Scale. The patients reported low exposition to SARS-CoV2 infection, but one case was confirmed with a mild COVID-19. Overall, depressive and anxiety symptoms were significantly and marginally increased in patients, respectively, with higher scores in cases with severe and moderate ataxia. Positive patient’s impression of psychopathological changes was associated to increased age, age at onset, and anxiety. Sixty-seven patients had a positive self-perception of ataxia progression, which was mainly influenced by higher anxiety scores but not by the adherence to at-home exercise programs. However, the practice of physical exercise was related with lower depression and anxiety scores, but this therapeutical effect was not significantly influenced by the disease stage. We demonstrated the negative effect of the COVID-19 pandemic on the mental and motor deficits in Cuban patients with cerebellar ataxias and the positive effect of the at-home physical exercise programs on their mental well-being. These findings give rationales to develop tele-medicine approaches to minimize these health impacts and to study the long-term effects of such sequelae and accordingly define their treatments.

Introduction The Centers for Disease Control and Prevention's Tips From Former Smokers® campaign encourages smokers to get help with quitting smoking by promoting 1-800-QUIT-NOW. Campaign advertisements featuring an offer of help with obtaining free cessation medication aired nationally on radio for 2 weeks in 2016. Similar advertisements aired nationally on TV for 3 weeks in 2017. The comparison period of 2016 radio campaign and 2017 TV campaign was used to examine the characteristics of quitline registrants by a media referral source (TV or radio). Methods Data on the number and demographics of quitline registrants in 2016 and 2017 were obtained from the Centers for Disease Control and Prevention's National Quitline Data Warehouse. The investigators conducted t-tests to assess the demographic differences between registrants who heard about the quitline through the radio advertisements and those who heard about it through the TV advertisements during the comparison period. This analysis was conducted in 2019. Results The registrants who heard about the quitline from radio advertisements were more likely to be male, younger, and have more years of education. However, the registrants who heard about the quitline from TV advertisements were more likely to be Black, non-Hispanic, and have fewer years of education. Conclusions The findings suggest that the demographic profiles of quitline registrants vary significantly based on how registrants hear about the quitline (via radio or TV). These differences in the characteristics of registrants can help inform the tobacco control mass media purchasing strategies and may enable media efforts to target the specific subgroups of smokers in a better way.

Directly modulated lasers (DML) dominate the short reach optical interconnect market. For years, the bandwidth of commercial DMLs has been limited to a level of 25 GHz, which can support a speed up to around 100 Gb/s via advanced modulations like Nyquist-shaped 4-ary pulse amplitude modulation (PAM-4), carrier-less amplitude phase (CAP) modulation, subcarrier multiplexing, and discrete multitone (DMT). To scale with the capacity evolution of interconnect applications, it is in urgent demand to develop DMLs operated at 50G class and even beyond. In this article, we exploit a newly designed 50G-class short-cavity distributed Bragg reflector (DBR) laser, called the two- κ DBR laser, and demonstrate its capability of supporting 200 Gb/s per wavelength direct modulation and direct detection transmissions. Combining high-speed electronics with the 50G-class DML, we demonstrate a variety of advanced digital modulations including Nyquist-shaped 100-GBaud PAM-4, 80-GBaud PAM-8, and 50-GHz DMT signals, with the maximum net bit rate of 236 Gb/s. The DML exhibits an ultra-low chirp parameter of 1.0, which enables isolator-free operation and dispersion-tolerant transmission over tens of kilometers at O-band.

49. Aboveground Biomass Along an Elevation Gradient in an Evergreen Andean–Amazonian Forest in Ecuador

The aim of this research was to assess how aboveground biomass (AGB) changes along an altitudinal gradient in evergreen Andean–Amazonian forests [Evergreen Lower Montane Forest, northeastern Cordillera of the Andes; Evergreen Piedmont Forest (EPF), northeastern Cordillera of the Andes; and Evergreen Lowland Forest (ELF), Napo–Curaray] from 373 to 1,826 m above sea level (m.a.s.l.). All trees measured in nine permanent 0.36-ha plots (60 × 60 m) were ≥ 10-cm (diameter at breast height) in the aforementioned ecosystems. We assessed tree density, basal area, species richness, and AGB. In the tree inventory, we measured 2,132 trees (Lower Montane Forest 687, Piedmont Forest 773, and Lowland Forest 672). AGB (Mg ha–1) increases with decreasing elevations: 310.26 ± 81.59 (SD) for the Evergreen Lower Montane Forest, 347.73 ± 90.38 (SD) for the EPF, and 377.39 ± 42.73 (SD) for the ELF. Otoba glycycarpa and Alchornea latifolia were the species with the highest biomass importance value (BIV) in the Evergreen Lower Montane Forest (1,421–1,826 m.a.s.l.), Spirotheca rosea and Pouteria glomerata in the EPF (892–1,195 m.a.s.l.), and Otoba glycycarpa and Iriartea deltoidea in ELF (373–394 m.a.s.l.). Biomass contribution along the altitudinal gradient in the evergreen Andean–Amazonian forests is concentrated in few species. This has implications in forest management and reforestation programs where the main focus is on carbon sequestration.

50. Early and mid-term outcomes of transcatheter tricuspid valve repair: systematic review and meta-analysis of observational studies

Severe tricuspid regurgitation (TR) is associated with poor prognosis when left untreated, and a growing number of studies on transcatheter tricuspid valve repair (TTVr) have been published over the last few months.We performed a comprehensive systematic review of published literature providing clinical data on TTVr for patients with significant TR. Early and mid-term clinical and echocardiographic outcomes were evaluated. Risk ratios (RR) or mean differences (MD) were obtained when comparing pre- and postprocedural data. A sensitivity analysis was also performed according to the main approach for repair (edge-to-edge vs annuloplasty).A total of 19 studies (all observational or single-arm trials) and 991 patients who underwent isolated TTVr were included. Thirty-day mortality and stroke rates were 2.8% and 0.2%, respectively. Pooled random-effects resulted in a significant reduction of ≥ severe TR (RR, 0.33; 95%CI, 0.26-0.42; P.001), vena contracta width (MD, 5.9mm; 95%CI, 4-7.9; P.001), right ventricular end-diastolic diameter (MD, 3.5mm; 95%CI, 2.5-4.5; P.001), and New York Heart Association (NYHA) class III or IV at last follow-up (RR, 0.32; 95%CI, 0.27-0.37; P.001). Bleeding complications and residual ≥ severe TR were numerically higher in the annuloplasty-like group compared with edge-to-edge repair (13.3% vs 3.8% for bleeding and 40.4% vs 27.9% for residual severe TR).Among 991 patients comprising the early experience for several TTVr devices, there was a statistically significant reduction in ≥ severe TR, NYHA class III-IV, vena contracta width and right ventricular end-diastolic diameter after TTVr. Thus far, the edge-to-edge approach seems to be associated with a better safety profile.
