We initiated a collaboration between local government, academia, and citizen scientists to investigate high frequencies of elevated Escherichia coli bacteria levels in the coastal Short Beach neighborhood of Branford, Connecticut. Citizen scientist involvement enabled collection of short-duration postprecipitation outfall flow water samples (mean E. coli level = 4930 most probable number per 100 mL) and yielded insights into scientific collaboration with local residents. A records review and sanitary questionnaire identified aging properties with septic systems (3.3%) and holding tanks (0.6%) as potential sources of the E. coli contamination. Biomedical science is rapidly developing in terms of more transparency, openness and reproducibility of scientific publications. This is even more important for all studies that are based on results from basic semen examination. Recently two concordant documents have been published: the 6th edition of the WHO Laboratory Manual for the Examination and Processing of Human Semen, and the International Standard ISO 23162:2021. With these tools, we propose that authors should be instructed to follow these laboratory methods in order to publish studies in peer-reviewed journals, preferable by using a checklist as suggested in an Appendix to this article.

Indium tin oxide (ITO) coatings have been proposed to reduce thermal emission losses for solar thermal applications. Unfortunately, ITO also has a large amount of free charge carriers (∼1 × 1020 per cm3), which absorb sunlight. To address this issue, we propose a nano-patterned ITO-coated quartz exhibiting both anti-reflectivity (to maximize solar transmission) and low emissivity (to minimize long wavelengths radiative losses). A record small-size nanosphere (∼60 nm) etch mask was prepared via double self-assembly, followed by dry etching and characterisation. In parallel, alternative nanopattern geometries were modelled using the Lumerical FDTD software to optimise short wavelength transmission without diminishing the inherently low emissivity of unetched ITO. It was found that an inverted moth's eye pattern (height = 250 nm and spacing = 80 nm) gave the best results at various solar concentrations (1 sun @ 100 °C, 10 suns @ 400 °C, and 100 suns @ 600 °C), resulting in ∼7% improvement in the solar weighted transmission as well as a similar boost in the overall efficiency factor for selectivity. It was concluded that if the proposed deposition/etching processes can be cost-effectively scaled in a continuous process, it would provide a net performance boost for most solar thermal technologies.

11. District-Scale VMS to Porphyry-Epithermal Transitions in Subduction to Postcollisional Tectonic Environments: The Artvin Au-Cu District and the Hod Gold Corridor, Eastern Pontides Belt, Turkey

Porphyry, epithermal, and volcanogenic massive sulfide (VMS) deposits can form together in the same mineral district in convergent margin environments. Their spatial association and superposition indicate evolving tectonic settings. The Artvin Au-Cu district is one of the major clusters of VMS bimodal-felsic, porphyry, and epithermal deposits in the Eastern Pontides belt in northeast Turkey. Whereas ore-forming processes, timing, and tectonic setting of VMS mineralization are well defined in Artvin, those for porphyry and epithermal mineralization remain less constrained. Our district-scale field study focused on the Hod gold corridor in the Artvin district, which is defined by the NE-trending alignment of the recent Au-Cu mineral discoveries (~205 t Au; ~0.33 Mt Cu; e.g., Hod Maden, Ardala-Salinbaş, and Taç-Çorak) that include Au-rich porphyry, highand intermediate-sulfidation epithermal, carbonate-replacement, and hybrid VMS-epithermal mineralization styles. Our new U-Pb, 40Ar/39Ar, and Re-Os geochronological results interpreted with previously compiled data show that magmatism in the Artvin district formed in the Carboniferous (358–325 Ma), Jurassic (182–174 Ma), Late Cretaceous (92–78 Ma), Eocene (51–40 Ma), and Oligocene (30 Ma). Porphyry and epithermal mineralization along the Hod gold corridor peaked in the Early (~113 Ma; Berta prospect) and Late Cretaceous (~86.5-82 Ma; e.g., Taç and Çorak deposits) and Eocene (~50 Ma; e.g., Ardala deposit), whereas VMS bimodal-felsic mineralization only formed in the Late Cretaceous (~91–85 Ma). Therefore, we interpret that the Hod gold corridor was a long-lived, deep crustal-scale structural feature along which the successive magmatic and mineralization events were emplaced. In addition, the timing of porphyry Cu-Mo mineralization can significantly (>20 Ma) postdate the crystallization age of the intrusive host rocks in the Artvin district, such as at Berta and Balcılı camp, which emphasizes the importance of dating mineralization directly to correctly attribute the tectonic setting. The distribution of Late Cretaceous mineral occurrences suggests a possible eastward temporal evolution from VMS (~91–85 Ma) to epithermal-porphyry systems (~86.5–82 Ma), transitioning from back-arc to arc settings at the onset of the Northern Neotethyan oceanic slab rollback and accompanied by increasingly elevated gold content eastwards across the Artvin district.

BackgroundCOVID-19 is a complex multisystem disease, frequently associated with kidney injury. Since the beginning of the COVID-19 pandemic, we observed a striking increase in the incidence of acute tubulointerstitial nephritis (aTIN) without or with uveitis (TINUs) among children. This prompted us to examine whether SARS-CoV-2 might be the underlying trigger.MethodsWe conducted a French nationwide retrospective cohort study. We included all consecutive children diagnosed with aTIN or TINUs of undetermined cause between April-2020 and March-2021. SARS-CoV-2 antibody responses were tested by a luciferase immunoprecipitation system and compared to age-matched controls. Immunohistochemistry, immunofluorescence and molecular microbiology analyses were performed on kidney biopsies.ResultsForty-eight children were included with a median age at diagnosis of 14.7 years (9.4-17.6). aTIN and TINUs incidence rates increased 3-fold and 12-fold, respectively, compared to pre-pandemic years. All patients had impaired kidney function with a median eGFR of 31.9 ml/min/1.73m² at diagnosis. Kidney biopsies showed lesions of acute tubulointerstitial nephritis and 25% of patients had fibrosis. No patient had concomitant acute COVID-19. All 16 children tested had high anti-N IgG titers and one had anti-S IgGs. Next-generation sequencing failed to detect any infectious agents in kidney biopsies. However, SARS-CoV-2 RNA was detected by PCR in two kidney samples supporting a potential direct link between SARS-CoV-2 and aTIN/TINUs.ConclusionsWe describe a novel form of post-acute COVID-19 syndrome in children, unique in its exclusive kidney and eye involvement, and its distinctive anti-SARS-CoV-2 N+/S-serological profile. Our results support a causal association linking SARS-CoV-2 infection to this newly-reported burst of renal/eye inflammation.

The article tries to demonstrate the fallacy of applying a restrictive interpretation. This attempt takes place on the basis of the restrictive interpretation of exemption from corporate income tax of income earned by non-profit organisations, as applied by the tax authorities and administrative courts. To this end, a critical analysis of the interpretative chaos concerning the manner of allocating income for statutory purposes is undertaken by the authors. The analysis of the relevant jurisprudence of the administrative courts shows that the courts often apply the so-called interpretative shortcuts, reaching conclusions without convincing justification of the interpretative claim. As a result, the correct interpretation of tax law is replaced by intuitive findings by the courts. Such approach is mainly based on the necessity to apply a restrictive interpretation of provisions on tax reliefs and exemptions, which has no sufficient justification in the Polish legal system. Artykuł stanowi próbę wykazania błędnego stosowania wykładni zawężającej na przykładzie zwolnienia z opodatkowania podatkiem dochodowym od osób prawnych dochodów uzyskiwanych przez organizacje non profit. Autorzy krytycznie analizują chaos interpretacyjny do-tyczący sposobu lokowania dochodów na cele statutowe, w szczególności w zakresie wydatkowania bezpośredniego i pośredniego. Analiza orzecznictwa sądów administracyjnych ukazuje, że często stosują one interpretacyjną „drogę na skróty”, tj. rozstrzygają spory bez przekonującego uzasadnienia twierdzenia interpretacyjnego, np. dotyczącego zastosowania wykładni zawężającej. W efekcie właściwa wykładnia przepisów prawa podatkowego zastępowana jest ustaleniami intuicyjnymi, opar tymi głównie na niemającej dostatecznego uzasadnienia w polskim systemie prawa konieczności stosowania wykładni zawężającej przepisów o ulgach i zwolnieniach podatkowych.

Introduction: The Flexor Carpi Ulnaris (FCU) is a part of the palmar the forearm muscle group and one of the most important muscles for upper limb functioning - is responsible for flexion and adduction of the hand at the radio-carpal joint. There are clinically significant but rare anatomical variations of FCU. The variability of the FCU has not been described up to now, and no typology of the muscle based on its more variable terminal attachment has been created. Aim of the study: Determination of FCU muscle typology based on available fetal material. Material and methods: A total of 114 human fetuses (53 female, 61 male) between 117 and 197 days of fetal life were eligible for the study. Preparations were carried out using classical anatomical techniques based on a previously published procedure. Thanks to that significant anthropometric landmarks were visible for the gathering of metric measurements. Metric measurements were taken and statistically analysed using R-Project software. Results: A new typology was created based on variable muscle insertions. Additionally, the presence of an atypically located, additional, separated muscle belly was described. A comparison of measurements of the left upper limb in relation to the right upper limb showed significant differences for forearm length to the anthropometric point of the stylion radiale, limb length, total FCU length and FCU length which means that the left limb is longer than the right limb. A comparison of FCU insertion types between left and right upper limb showed there’s no significant difference between counts of each type. Conclusion: The FCU is a muscle that is easy to palpate and may therefore act as a topographical marker for healthcare professionals. Knowledge of its variability is not only of theoretical importance but also has clinical significance. The current publication demonstrates presence of variability in FCU terminal attachment. Certainly, this topic requires further research and continued work on a detailed understanding of forearm anatomy in the fetal period.

Introduction Acute kidney injury (AKI) has been identified as one of the most common and significant problems in hospitalized patients with COVID-19. However, studies examining the relationship between COVID-19 and AKI in low- and low-middle income countries (LLMIC) are lacking. Given that AKI is known to carry a higher mortality rate in these countries, it is important to understand differences in this population. Methods This prospective, observational study examines the AKI incidence and characteristics of 32,210 patients with COVID-19 from 49 countries across all income levels who were admitted to an intensive care unit during their hospital stay. Results Among patients with COVID-19 admitted to the intensive care unit (ICU), AKI incidence was highest in patients in LLMIC, followed by patients in upper-middle income countries (UMIC) and high-income countries (HIC) (53%, 38%, and 30%, respectively), whereas dialysis rates were lowest among patients with AKI from LLMIC and highest among those from HIC (27% vs. 45%). Patients with AKI in LLMIC had the largest proportion of community-acquired AKI (CA-AKI) and highest rate of in-hospital death (79% vs. 54% in HIC and 66% in UMIC). The association between AKI, being from LLMIC and in-hospital death persisted even after adjusting for disease severity. Conclusions AKI is a particularly devastating complication of COVID-19 among patients from poorer nations where the gaps in accessibility and quality of healthcare delivery have a major impact on patient outcomes.

25. Assessment of the clinical feasibility of detecting subtle blood-brain barrier leakage in cerebral small vessel disease using dynamic susceptibility contrast MRI

PURPOSE: Cerebral small vessel disease (cSVD) involves several pathologies affecting the small vessels, including blood-brain barrier (BBB) impairment. Dynamic susceptibility contrast (DSC) MRI is sensitive to both blood perfusion and BBB leakage, and correction methods may be crucial for obtaining reliable perfusion measures. These methods might also be applicable to detect BBB leakage itself. This study investigated to what extent DSC-MRI can measure subtle BBB leakage in a clinical feasibility setting.METHODS: In vivo DCE and DSC data were collected from fifteen cSVD patients (71 (±10) years, 6F/9M) and twelve elderly controls (71 (±10) years, 4F/8M). DSC-derived leakage fractions were obtained using the Boxerman-Schmainda-Weisskoff method (K2). K2 was compared with the DCE-derived leakage rate Ki, obtained from Patlak analysis. Subsequently, differences were assessed between white matter hyperintensities (WMH), cortical gray matter (CGM), and normal-appearing white matter (NAWM). Additionally, computer simulations were performed to assess the sensitivity of DSC-MRI to BBB leakage.RESULTS: K2 showed significant differences between tissue regions (P < 0.001 for CGM-NAWM and CGM-WMH, and P = 0.001 for NAWM-WMH). Conversely, according to the computer simulations the DSC sensitivity was insufficient to measure subtle BBB leakage, as the K2 values were below the derived limit of quantification (4∙10-3 min-1). As expected, Ki was elevated in the WMH compared to CGM and NAWM (P < 0.001).CONCLUSIONS: Although clinical DSC-MRI seems capable to detect subtle BBB leakage differences between WMH and normal-appearing brain tissue it is not recommended. K2 as a direct measure for subtle BBB leakage remains ambiguous as its signal effects are due to mixed T1- and T2∗-weighting. Further research is warranted to better disentangle perfusion from leakage effects.

We consider linear boundary value problems for higher-order parameter-elliptic equations, where the boundary data do not belong to the classical trace spaces. We employ a class of Sobolev spaces of mixed smoothness that admits a generalized boundary trace with values in Besov spaces of negative order. We prove unique solvability for rough boundary data in the half-space and in sufficiently smooth domains. We present a man in his 30s with acute anterior myocardial infarction due to thrombotic occlusion of the left anterior descending artery and subsequent left ventricular thrombus formation after high-dose recreational use of nitrous oxide (N2O). Initial questioning for use of illicit substances was negative, but low vitamin B12 levels and severely elevated homocysteine levels prompted us to interrogate for the use of laughing gas. On questioning, the patient admitted to have used this substance, which he presumed to be innocent. Neither percutaneous coronary intervention with balloon dilatation nor intravenous glycoprotein IIb/IIIa receptor antagonist, nor continuous use of anticoagulation and double antiplatelet therapy resulted in thrombus resolution. Due to a severely reduced left ventricular function, despite 3 months on heart failure therapy, the patient is being counselled for intracardiac defibrillator implantation. We conclude that N2O, notably when consumed in conjunction with other proatherogenic substances, is associated with thrombosis: a relation possibly mediated by severe hyperhomocysteinaemia.

34. Idiopathic chronic eosinophilic pneumonia: a differential diagnosis of lower respiratory tract infection

A 43-year-old woman presented with a presumed lower respiratory tract infection, with symptoms of persistent cough, lethargy, fevers and night sweats. Initial general practitioner assessment revealed raised C reactive protein and a leucocytosis comprising both a neutrophilia and an eosinophilia. The patient was initially treated for bacterial pneumonia. Despite treatment, the patient’s condition did not improve and hospital admission was arranged for further investigation. Initial physical examination was unremarkable. A chest X-ray revealed bilateral, symmetrical, peripheral consolidation with an upper zone predominance. Subsequently, endobronchial washings revealed abundant eosinophils. A diagnosis of idiopathic chronic eosinophilic pneumonia was made, and the patient responded well to oral corticosteroids with complete resolution of radiological appearances 1 month later.

A woman in her late 20s presented with headaches and subacute encephalopathy. MRIs showed multiple punctate subcortical and periventricular white matter hyperintensities with diffusion restriction, infratentorial lesions, leptomeningeal enhancement of the cervical spinal cord, brainstem and cerebellum and two areas of high-signal abnormality at T4 and T6 raising suspicion for multiple sclerosis or acute disseminated encephalomyelitis.Further studies and evolution of her symptoms during her hospital stay confirmed the clinical triad of encephalopathy, branch retinal artery occlusions and hearing loss pathognomonic for Susac’s syndrome.While cervical spinal cord and cauda equina involvement have been reported in Susac’s syndrome previously, no thoracic spinal cord involvement has been reported.We report the novel MRI finding of thoracic spinal cord involvement in Susac’s syndrome. In order to avoid misdiagnosis, neurologists and neuroradiologists should be aware that any part of the spinal cord can be involved in Susac’s syndrome.

We present a case of piriformis syndrome in a woman in her 30’s following low energy trauma, presenting with unilateral lower limb weakness, altered sensation and urinary retention. CT imaging revealed a bulky piriformis muscle which was further clarified on MRI as an intramuscular haematoma within the left piriformis causing compression of the left lumbosacral plexus. Haematoma formation was exacerbated due to use of an antiplatelet medication the patient was taking for Moyamoya disease, which carries an increased risk of cerebrovascular accident. Surgical exploration of the piriformis and sciatic nerve was undertaken and confirmed a haematoma within the piriformis. A full release of the piriformis tendon was undertaken, and the sciatic nerve was inspected, no further abnormality was found. After review in clinic post-discharge, the patient reported normal sensation and normal muscle power in her feet.

The purpose of the article/hypothesis: The goal of this paper is to analyze tax evasion with special emphasis on gender. Factors influencing tax noncompliance such as age, income, education, confidence in government, political scale and religiosity are analyzed. Methodology: Tax evasion is analyzed based on the most recent (Wave 7) World Value Survey data with the significance of differences between respondents, correlation and regression models analysis. Results of the research: It has been found that there are significant differences between the global approach and groups of males and females when the acceptance of cheating on taxes is taken into consideration. The analysis of groups reflected the fact that all factors influence respondents in the same way without distinguishing between males and females.

In April 1998, as part of a project to collect biopsy samples of putative pygmy blue whales (Balaenoptera musculus brevicauda) in the waters around the Republic of the Maldives, Indian Ocean, incidental sightings of cetaceans encountered were recorded. Using modified line-transect methods and handheld binoculars, a total of 267 sightings of 16 species of whales and dolphins were recorded during 20 at-sea days in the northeastern part of the atoll. Significant results include the following: (1) cetaceans were abundant and species diversity was high, including nearly every pantropical species of pelagic cetacean; (2) the spinner dolphin (Stenella longirostris) was by far the most common species encountered (56 sightings) and also had the largest mean school size ( = 50.3 individuals); (3) blue whales were rare; only four individuals were sighted; (4) a large concentration of Bryde’s whales (28 sightings in two days) was apparently feeding in nearshore waters; (5) this paper reports the first records for the Maldives of Cuvier’s beaked whale (Ziphius cavirostris), Blainville’s beaked whale (Mesoplodon densirostris) and the dwarf sperm whale (Kogia sima): the latter was particularly common (17 sightings); (6) the spotted dolphin (Stenella attenuata) was rare and almost always associated with yellowfin tuna (Thunnus albacares), spinner dolphin, or seabirds, as has been reported in the eastern Pacific and western Indian oceans.

In this study, 50%-50% Ni-8YSZ and Ni-BCZY27 composites were exposed to CH4 in the absence and presence of H2O. Carbon deposition at 750 °C and carbon removal were estimated by exposure to steam using operando vibrational emission spectroscopy and thermal imaging. Carbon removal reveals more CO2 + CO formation from Ni-8YSZ compared to Ni-BCZY27, suggesting more carbon on the surface of the former. Carbon removal from composites exposed to CH4 alone indicates limited formation and fast depletion of CO over Ni-BCZY27. Over Ni-8YSZ, CO and CO2 depletion is gradual and sustained, suggesting carbon formation over Ni-BCZY27 is restricted and full oxidation readily occurs. Methane cracking over Ni-8YSZ is accompanied by more cooling compared to Ni-BCZY27. Wet reforming results in similar cooling over Ni-8YSZ and Ni-BCZY27. The results are discussed in the context of recent work using operando spectroscopies of fuel utilization over Ni-8YSZ SOFCs.

Here the Human Pangenome Reference Consortium presents a first draft of the human pangenome reference. The pangenome contains 47 phased, diploid assemblies from a cohort of genetically diverse individuals1. These assemblies cover more than 99% of the expected sequence in each genome and are more than 99% accurate at the structural and base pair levels. Based on alignments of the assemblies, we generate a draft pangenome that captures known variants and haplotypes and reveals new alleles at structurally complex loci. We also add 119 million base pairs of euchromatic polymorphic sequences and 1,115 gene duplications relative to the existing reference GRCh38. Roughly 90 million of the additional base pairs are derived from structural variation. Using our draft pangenome to analyse short-read data reduced small variant discovery errors by 34% and increased the number of structural variants detected per haplotype by 104% compared with GRCh38-based workflows, which enabled the typing of the vast majority of structural variant alleles per sample.

- Abstract
Purpose: Treatments are limited for metastatic melanoma and metastatic triple-negative breast cancer (mTNBC). This pilot phase I trial (NCT03060356) examined the safety and feasibility of intravenous RNA-electroporated chimeric antigen receptor (CAR) T cells targeting the cell-surface antigen cMET. Experimental Design: Metastatic melanoma or mTNBC subjects had at least 30% tumor expression of cMET, measurable disease and progression on prior therapy. Patients received up to six infusions (1 × 10e8 T cells/dose) of CAR T cells without lymphodepleting chemotherapy. Forty-eight percent of prescreened subjects met the cMET expression threshold. Seven (3 metastatic melanoma, 4 mTNBC) were treated. Results: Mean age was 50 years (35–64); median Eastern Cooperative Oncology Group 0 (0–1); median prior lines of chemotherapy/immunotherapy were 4/0 for TNBC and 1/3 for melanoma subjects. Six patients experienced grade 1 or 2 toxicity. Toxicities in at least 1 patient included anemia, fatigue, and malaise. One subject had grade 1 cytokine release syndrome. No grade 3 or higher toxicity, neurotoxicity, or treatment discontinuation occurred. Best response was stable disease in 4 and disease progression in 3 subjects. mRNA signals corresponding to CAR T cells were detected by RT-PCR in all patients’ blood including in 3 subjects on day +1 (no infusion administered on this day). Five subjects underwent postinfusion biopsy with no CAR T-cell signals seen in tumor. Three subjects had paired tumor tissue; IHC showed increases in CD8 and CD3 and decreases in pS6 and Ki67. Conclusions: Intravenous administration of RNA-electroporated cMET-directed CAR T cells is safe and feasible. Significance: Data evaluating CAR T therapy in patients with solid tumors are limited. This pilot clinical trial demonstrates that intravenous cMET-directed CAR T-cell therapy is safe and feasible in patients with metastatic melanoma and metastatic breast cancer, supporting the continued evaluation of cellular therapy for patients with these malignancies.

Home dialysis modalities (home hemodialysis [HD] and peritoneal dialysis [PD]) are associated with greater patient autonomy and treatment satisfaction compared with in-center modalities, yet the level of home-dialysis use worldwide is low. Reasons for limited utilization are context-dependent, informed by local resources, dialysis costs, access to healthcare, health system policies, provider bias or preferences, cultural beliefs, individual lifestyle concerns, potential care-partner time, and financial burdens. In May 2021, KDIGO (Kidney Disease: Improving Global Outcomes) convened a controversies conference on home dialysis, focusing on how modality choice and distribution are determined and strategies to expand home-dialysis use. Participants recognized that expanding use of home dialysis within a given health system requires alignment of policy, fiscal resources, organizational structure, provider incentives, and accountability. Clinical outcomes across all dialysis modalities are largely similar, but for specific clinical measures, one modality may have advantages over another. Therefore, choice among available modalities is preference-sensitive, with consideration of quality of life, life goals, clinical characteristics, family or care-partner support, and living environment. Ideally, individuals, their care-partners, and their healthcare teams will employ shared decision-making in assessing initial and subsequent kidney failure treatment options. To meet this goal, iterative, high-quality education and support for healthcare professionals, patients, and care-partners are priorities. Everyone who faces dialysis should have access to home therapy. Facilitating universal access to home dialysis and expanding utilization requires alignment of policy considerations and resources at the dialysis-center level, with clear leadership from informed and motivated clinical teams.
