En este artículo, se realiza un análisis textual de Terciopelo azul (David Lynch, 1986), centrado especialmente en la secuencia final de esta película, un thriller neo-noir que se ha convertido en una de las obras más influyentes del cine postclásico americano. Además de recorrer parte de su truculenta trama narrativa, capaz de inspirar miedo y atracción a la vez, y de revisar algunas de las interpretaciones más relevantes del film, este análisis examina con detalle el epílogo de la película, un pasaje de reminiscencias oníricas e ilusorias en el que se representa un final feliz que conecta con el principio de la cinta. Dicha lectura permite localizar una sorprendente referencia a un elemento mágico y fascinante proveniente de la célebre película musical producida por Disney Mary Poppins (Robert Stevenson, 1964). Esta y otras intertextualidades vinculadas a la infancia y adolescencia de David Lynch provocan que, en este film posmoderno, junto con la obvia presencia de lo horroroso y de lo paródico, asome también la nostalgia de lo maravilloso, la añoranza de una época y una visión del mundo en el que este parecía un lugar menos sórdido y aterrador. This article presents a textual analysis of Blue velvet (David Lynch, 1986) focusing especially on the final sequence of this film, a neo-noir thriller that has become one of the most influential works of post-classical American cinema. In addition to reviewing part of its gruesome narrative plot, capable of inspiring fear and attraction at the same time, and reviewing some of the most relevant interpretations of the film, this analysis examines in detail the epilogue of the film, a passage of dreamlike and illusory reminiscences in which a happy ending that connects with the beginning of the film is shown. This reading allows us to find a surprising reference to a magical and fascinating element from the famous Disney musical film Mary Poppins (Robert Stevenson, 1964). This and other intertextual allusions linked to David Lynch’s childhood and adolescence make it possible that in this postmodern film, along with the obvious presence of the horrific and the parodic, there is also the nostalgia for the marvellous, the longing for a time and a perception of the world in which it seemed a less sordid and frightening place.

Resumen Antecedentes y objetivos La heterogeneidad de los pacientes con insuficiencia cardiaca y fraccion de eyeccion preservada (ICFEP) es elevada, por lo que se tiende a agrupar en fenotipos para intervenir con precision. Dentro de estos, los pacientes con diabetes mellitus (DM) mantienen esta heterogeneidad. Nuestro objetivo es describir grupos de pacientes con ICFEP y DM basados en otras comorbilidades. Material y metodos Los pacientes se reclutan desde el registro nacional de insuficiencia cardiaca (RICA). Se incluyen pacientes con fraccion de eyeccion mayor o igual al 50% sin valvulopatia y con DM. Se realiza un analisis aglomerativo jerarquico con el metodo de Ward incluyendo las siguientes variables: dislipemia, hepatopatia, EPOC, demencia, enfermedad cerebrovascular, arritmia, presion arterial sistolica, indice de masa corporal (IMC), estimacion del filtrado glomerular y hemoglobina. Resultados Se incluyen 1.934 pacientes con ICFEP, de los que 907 (46,9%) tenian DM, con predominio de mujeres (60,9%) y con un IMC de 31,1 (5,9) kg/m2. Se obtienen 4 grupos: dos con elevado riesgo vascular (uno con arritmia y otro no), con 263 pacientes el primero y 201 el segundo, otro con predominio de EPOC (140 pacientes) y un ultimo grupo de 303 pacientes con mas edad pero menos comorbilidad. Conclusiones En nuestros pacientes con ICFEP y DM predomina la obesidad y el sexo femenino. Los cuatro grupos ofrecen oportunidades de tratamiento para mejorar su pronostico no solo basadas en la utilizacion de nuevos farmacos antidiabeticos sino por otras opciones que pueden suponer un punto de partida para nuevas investigaciones.

Arrhythmogenic left ventricular cardiomyopathy (ALVC) is a rare heritable heart-muscle disorder characterized by a progressive loss of left ventricular myocardium and its replacement by fibrofatty tissue. Myocarditis is an inflammatory disease of the heart that may occur secondary to infections, immune system activation or exposure to drugs. Hot phases of ALVC present with chest pain and troponin rise, mimicking acute viral myocarditis and indicate a progression of the disease. Recently, myocarditis has also been described as an infrequent complication of coronavirus disease 2019 (Covid-19) mRNA vaccines. We herein report for the first time a case of probable myocarditis induced by Covid-19 vaccine in a patient with previous medical history of ALVC. We aim to highlight the common characteristics of ALVC and Covid-19 vaccine myocarditis and work through the differential diagnosis of these two entities.

Introduction and objectives Acute heart failure (AHF), can occur as decompensated chronic heart failure (HF) or as a first episode, “new onset”. The aim of this study was to analyse the clinical characteristics and prognosis at one-year in a cohort of patients with new onset AHF. Methods Prospective observational study of 3550 patients with AHF. We compared patients with new onset HF with the others. Restricting the analysis to new onset AHF patients, we analysed the clinical characteristics, readmissions, mortality and impact of left ventricular ejection fraction on the prognosis. Results A total of 1105 (31%) patients fulfil the criteria for new onset AHF. These patients versus the rest, were younger, had a higher aetiology of hypertension and preserved left ventricular ejection fraction, less global comorbidity and better baseline overall functional status. After one year, mortality in new onset HF was less than chronic decompensated HF (15 vs. 27%; p Conclusions New onset AHF shows a clinical profile and prognosis different to that of chronic decompensated HF. Higher comorbidity, renal function and treatment post-discharge predict a higher risk of mortality.

Resumen Introduccion y objetivos La insuficiencia cardiaca aguda (ICA) puede suceder como una descompensacion de una IC cronica o como un primer episodio “de novo”. Nuestro objetivo fue analizar las caracteristicas clinicas y el pronostico al ano, en una cohorte de ICA de novo. Metodos Estudio observacional y prospectivo de 3550 pacientes con ICA. Se analizan las caracteristicas clinicas, la fraccion de eyeccion ventricular izquierda, los reingresos y factores asociados a mayor mortalidad al ano de los pacientes con ICA de novo y se comparan con el resto. Resultados Un total de 1105 (31%) pacientes, presentaron ICA de novo. Este grupo fue mas joven, con mayor etiologia hipertensiva y fraccion de eyeccion ventricular izquierda preservada, mejor estado funcional y menor comorbilidad que el resto de la cohorte. Al ano de seguimiento, la mortalidad fue menor en ICA de novo frente a IC cronica descompensada (el 15 vs. el 27%; p Conclusiones Los pacientes con ICA de novo tienen un perfil clinico diferente a la IC cronica descompensada, con un mejor pronostico. Los principales factores predictores de mortalidad al ano en ICA de novo fueron la comorbilidad global, la funcion renal y el tipo de tratamiento al alta hospitalaria.

Objective Several studies have reported that a higher degree of hemoconcentration in patients admitted for the treatment of acute heart failure (HF) constitutes a favorable prognostic factor in the year following the index episode. The objective of this study was to evaluate whether the highest degree of hemoconcentration at 3 months after admission for HF is also a prognostic factor for mortality and/or readmission in the 12 months after admission. Patients and method The hemoconcentration group was the upper quartile of the sample distributed according to hemoglobin increase at month 3 after discharge with respect to hemoglobin at the time of admission for HF in a multicenter prospective cohort of 1659 subjects with HF. Results The mean follow-up until the first event was 294 days, and a total of 487 deaths and 1125 readmissions were recorded. The hemoconcentration group had a lower risk of mortality or readmission for any cause (RR = 0.75, 95% CI: 0.51–1.09 and RR = 0.86, 95% CI: 0.70–1.05), although statistical significance was lost after multivariate analysis, while it was retained for other factors with recognized negative impact on the prognosis of patients with HF, such as age and functional class. Conclusions The degree of hemoconcentration at 3 months after admission for HF is not prognostic of readmission or death in the subsequent year.

Resumen Objetivo Diferentes estudios senalan que la consecucion de una mayor hemoconcentracion en pacientes ingresados por insuficiencia cardiaca (IC) aguda mejora el pronostico a lo largo del ano siguiente al episodio indice. El objetivo de este estudio es evaluar si el grado de hemoconcentracion a los 3 meses tras el ingreso por IC tambien tiene valor pronostico de reingreso y/o mortalidad en los 12 meses siguientes al ingreso. Pacientes y metodo Cohorte prospectiva multicentrica de 1.659 pacientes con IC. El grupo hemoconcentracion (305 pacientes) se situo en el cuartil superior de la muestra distribuida en funcion del aumento de la hemoglobina en el mes 3 tras el alta con respecto a la hemoglobina en el ingreso por IC. Resultados Seguimiento medio hasta el primer evento fue de 294 dias, fallecieron 487 pacientes y reingresaron 1.125. El grupo hemoconcentracion mostro un riesgo menor de mortalidad o de reingreso por cualquier causa (RR = 0,75; IC 95%: 0,51-1,09 y RR = 0,86; IC 95%: 0,70-1,05), si bien la significacion estadistica se perdio tras el analisis multivariado. Sin embargo, esta significacion se mantuvo para otros factores con reconocido efecto negativo sobre el pronostico en pacientes con IC, como son la edad y la clase funcional. Conclusiones El grado de hemoconcentracion a los 3 meses tras el ingreso por IC no tiene valor pronostico de reingreso o muerte en el ano siguiente.

Introduction: Psychiatric comorbidities are common in epileptic patients, and evaluating the impact of antiepileptic drugs on patients’ moods is therefore essential. The aim of this study is to assess the effects of lacosamide on behaviour and quality of life in people with epilepsy. Methods: We conducted a multicentre prospective observational study of poorly-controlled epileptic patients who received lacosamide as an adjuvant treatment. Patients were evaluated on 4 occasions during a 12-month period. The impact of lacosamide on patients’ mood and quality of life was assessed with the Quality of Life in Epilepsy Inventory-10 (QOLIE-10), the Hospital Anxiety and Depression Scale (HADS), and the Barratt Impulsiveness Scale (BIS-11). As a secondary objective, we evaluated the effectiveness and safety of lacosamide. Results: We included 55 patients with a mean age of 47.1 ± 18.4 years. At baseline, 34.5% of the patients had psychiatric comorbidities; the mean number of crises in the previous month was 3.6 ± 4.3. The QOLIE-10 and HADS scales revealed statistically significant improvements in patients with a poor baseline condition (anxiety, depression, and/or poor quality of life). The BIS-11 scale detected no impulsive behaviour during follow-up. After 12 months of treatment, 51.9% of the patients were seizure-free and 77.8% experienced a reduction of at least 50% in seizure frequency. Adverse effects were mild in most cases; lacosamide was discontinued in 10 patients (18.2%). Conclusions: Lacosamide is a safe and effective treatment option for patients with epilepsy and psychiatric comorbidities. Resumen: Introducción: La comorbilidad psiquiátrica es común en epilepsia, de ahí la importancia de considerar en qué medida los fármacos antiepilépticos pueden influir en el estado de ánimo. El objetivo de este trabajo es analizar el efecto de lacosamida en la calidad de vida y en la conducta del paciente epiléptico en la práctica clínica. Métodos: Estudio multicéntrico, observacional y prospectivo en pacientes diagnosticados de epilepsia, mal controlados que recibieron tratamiento adyuvante con lacosamida. Mediante 4 visitas durante 12 meses se valoró el impacto del fármaco en la calidad de vida y el estado de ánimo utilizando el cuestionario de calidad de vida QOLIE-10, la escala hospitalaria de ansiedad y depresión (HADS) y la escala de impulsividad de Barratt (BIS-11), además se determinó su eficacia y seguridad. Resultados: Se incluyeron 55 pacientes, edad media 47,1 ± 18,4 años; porcentaje inicial de comorbilidad psiquiátrica 34,5% y número medio de crisis/mes previo 3,6 ± 4,3. Las escalas QOLIE-10 y HADS reflejaron mejoras estadísticamente significativas en pacientes que partían de una situación basal desfavorable (ansiedad, depresión y/o baja calidad de vida). La escala BIS-11 no detectó la aparición de conductas impulsivas durante el seguimiento. Tras 12 meses de tratamiento el 51,9% de los pacientes estuvo sin crisis, y un 77,8% presentó una reducción ≥ 50%. La mayoría de efectos adversos fueron leves, obligando a retirar el fármaco en 10 casos (18,2%). Conclusiones: Lacosamida ofrece un perfil de eficacia y seguridad favorable, y podría constituir una opción terapéutica útil en pacientes con epilepsia y comorbilidad psiquiátrica. Eslicarbazepine acetate is a novel sodium channel blocker for use in the treatment of focal onset seizures. Prospective studies on its effectiveness in monotherapy in patients with newly diagnosed partial epilepsy in routine clinical practice are scarce.To evaluate the effectiveness of eslicarbazepine as initial monotherapy in patients with newly diagnosed partial epilepsy in routine clinical practice.A prospective, multicentre, post-authorisation study. Patients with newly diagnosed partial epilepsy aged 18 years or older without previous treatment were included. The efficacy variables were: percentage of seizure-free patients, responders and reduction in monthly frequency of seizures. The safety variables analyse the 12-month retention rate and the occurrence of adverse effects.Fifty-three patients were included. The retention rate was 77.4%. At the end of the observation period, 83% of patients were seizure-free and 92.5% had reduced their baseline frequency by 50% or more. In addition, 68% of the patients reported some adverse effect and 7.5% of them dropped out of the study for this reason. The effectiveness analysis of the subgroup of patients aged 65 years or more showed no differences with respect to the overall population.Eslicarbazepine monotherapy in patients with newly diagnosed partial epilepsy, both in the general population and in the population over 65 years old, is effective and safe in routine clinical practice.Alzemon: estudio de seguimiento prospectivo del acetato de eslicarbacepina en monoterapia en pacientes con epilepsia de diagnóstico reciente.Introducción. El 68% de los pacientes notificó algún efecto adverso y el 7,5% de ellos abandonó el estudio por este motivo. El análisis de efectividad del subgrupo de 65 años o más no mostró diferencias respecto a la población global. Conclusión. La eslicarbacepina en monoterapia en pacientes con epilepsia parcial de reciente diagnóstico, tanto en la población general como en la población de más de 65 años, es eficaz y segura en la práctica clínica habitual.

[Objective]: To identify novel genetic mechanisms causing Charcot-Marie-Tooth (CMT) disease., [Methods]: We performed a next-generation sequencing study of 34 genes associated with CMT in a patient with peripheral neuropathy., [Results]: We found a non–previously described mutation in EGR2 (p.P397H). P397H mutation is located within the loop that connects zinc fingers 2 and 3, a pivotal domain for the activity of this transcription factor. Using promoter activity luciferase assays, we found that this mutation promotes decreased transcriptional activity of EGR2. In this patient, we also found a previously described nonpathogenic polymorphism in lipopolysaccharide-induced TNF-α factor (LITAF) (p.T49M). We show that the p.T49M mutation decreases the steady-state levels of the LITAF protein in Schwann cells. Loss of function of LITAF has been shown to produce deregulation in the NRG1-erbB signaling, a pivotal pathway for EGR2 expression by Schwann cells. Surprisingly, our segregation study demonstrates that p.P397H mutation in EGR2 is not sufficient to produce CMT disease. To identify novel genetic mechanisms causing Charcot-Marie-Tooth (CMT) disease.

Background Natriuretic peptides or the blood urea nitrogen to creatinine ratio (BUN/creat) can identify high- vs low-risk renal impairment (RI) in patients with heart failure and reduced ejection fraction (HF-REF). However, the situation in HF patients with preserved ejection fraction (HF-PEF) and mid-range ejection fraction (HF-MREF) remains unclear. Methods We evaluated patients from the Spanish National Registry of Heart Failure (RICA) that were admitted to Internal Medicine units with acute decompensated HF. Median admission values were used to define elevated NT-proBNP and BUN/creat. Results A total of 935 patients were evaluated, 743 with HF-PEF and 192 with HF-MREF). In patients with both NT-proBNP and BUN/creat below median admission values, RI was not associated with mortality (HR 1.15; 95% CI 0.7–1.87, p =0.581 in HF-PEF and HR 1.27; 95% CI 0.58–2.81, p =0.548 in HF-MREF). However, in patients with both elevated NT-proBNP and BUN/creat, those with RI had worse survival than those without RI (HR 2.01, 95% CI 1.33–3.06, p p =0.005 in HF-MREF). In HF-PEF even patients with RI with only 1 of the 2 parameters elevated, had a substantially higher risk of death compared to patients without RI (HR 1.53; 95% CI 1.04 to 2.26; p =0.031). Conclusions In this clinical cohort of acute decompensated HF-PEF and HF-MREF patients, the combined use of NT-proBNP and BUN/creat stratifies patients with RI into groups with significantly different prognoses.

Introduccion. El acetato de eslicarbacepina es un nuevo bloqueante de los canales de sodio en el tratamiento de las crisis de inicio focal. Los estudios prospectivos sobre su efectividad en monoterapia en pacientes con epilepsia parcial de reciente diagnostico en la practica clinica habitual son escasos. Objetivo. Evaluar la efectividad de la eslicarbacepina en monoterapia de inicio en pacientes con epilepsia parcial de reciente diagnostico en la practica clinica habitual. Pacientes y metodos. Estudio postautorizacion prospectivo y multicentrico. Se incluyo a pacientes con epilepsia parcial de reciente diagnostico de 18 anos o mas sin tratamiento previo. Las variables de eficacia fueron: porcentaje de pacientes libres de crisis, respondedores y reduccion en la frecuencia mensual de crisis. Eventual consistency is demanded nowadays in geo-replicated services that need to be highly scalable and available. According to the CAP constraints, when network partitions may arise, a distributed service should choose between being strongly consistent or being highly available. Since scalable services should be available, a relaxed consistency (while the network is partitioned) is the preferred choice. Eventual consistency is not a common data-centric consistency model, but only a state convergence condition to be added to a relaxed consistency model. There are still several aspects of eventual consistency that have not been analysed in depth in previous works: 1. which are the oldest replication proposals providing eventual consistency, 2. which replica consistency models provide the best basis for building eventually consistent services, 3. which mechanisms should be considered for implementing an eventually consistent service, and 4. which are the best combinations of those mechanisms for achieving different concrete goals. This paper provides some notes on these important topics.

Proveniente del mundo del cómic, la figura del superhéroe debutó con éxito en el cine a finales de la década de los 70. Sin embargo, ha sido durante los últimos quince años cuando esta figura ha cobrado un extraordinario protagonismo y, en sus múltiples variantes, ha dado pie a buena parte de las franquicias cinematográficas que en la actualidad dominan las salas de cine de todo el planeta. Al hilo de este nuevo paisaje fílmico, el presente artículo se propone explorar la naturaleza del cine de superhéroes, un apabullante espectáculo audiovisual que es consumido por millones de espectadores en todo el mundo, con especial incidencia entre el público infantil y juvenil. Tal indagación conduce a examinar las narrativas desplegadas en estas series de películas, así como a analizar la propia figura del superhéroe en el contexto de un cine saturado de efectos visuales y acción trepidante. Finalmente, el artículo trata de señalar las razones de fondo que han llevado a la industria de Hollywood a apostar por el género de superhéroes como fórmula recurrente para atraer al público a las salas de cine. Coming from the field of comic books, the character of the superhero began to appear successfully in the cinema of the late 70’s. However, during the last fifteen years this type of character has taken on a very important role and, in its many variants, has given rise to much of the film franchises that currently dominate movie theatres all over the world. Following the thread of this new film landscape, this article aims to explore the nature of superhero movies, an overwhelming audiovisual spectacle that is consumed by millions of spectators around the world, with a significant impact on children and young audiences. That inquiry leads to examine the narratives unfolding in these film series, while analysing the character of the superhero within the context of a cinema saturated with visual effects and frenetic action. Finally, the article tries to point out the basic reasons that have led the Hollywood industry to focus on superhero genre as a recurring formula to bring audiences to movie theatres.

Introduction: Hyposmia and substantia nigra hyperechogenicity (SN+) are characteristic markers of Parkinson's disease (PD), although their diagnostic value in isolation may be limited. We evaluated the combined prevalence of both disorders in patients diagnosed with PD and assessed their diagnostic yield compared to a sample with essential tremor (ET) and another group of healthy subjects. Methods: Patients diagnosed with PD and ET and treated in our outpatient clinic were enrolled. Olfaction was assessed using the “Sniffin Sticks” odour identification test (SS-12) and hyperechogenicity of the substantia nigra (SN+) was assessed by transcranial duplex ultrasound. Results: A total of 98 subjects were analysed, comprising 30 with PD, 21 with ET, and 47 controls. The respective prevalence rates of hyposmia (SS-12 0.24 cm2) were 70% and 83.3% in PD, 33.3% and 9.5% in ET, and 17% and 10.6% in controls. Both markers were present in 63% of patients with PD, none of the patients with ET, and only 2 of the controls. Conclusions: Combined use of substantia nigra sonography and olfactory testing with SS-12, two rapid, safe, and accessible tests, was more specific than each isolated marker for distinguishing patients with PD from patients with ET and control subjects. Since both markers have been described in very early phases of PD, combined use may be helpful in providing early diagnosis of PD. Resumen: Introducción: La hiposmia y la hiperecogenicidad de la sustancia negra (SN+) son marcadores característicos de la enfermedad de Parkinson (EP), aunque su valour diagnóstico de forma aislada puede ser limitado. Se evalúa la prevalencia combinada de ambos marcadores en pacientes diagnosticados de enfermedad de Parkinson (EP) y su rentabilidad diagnóstica frente a una muestra con temblor esencial (TE) y otra de sujetos sanos. Métodos: Se incluyó a pacientes con diagnóstico de EP y TE procedentes de nuestra consulta externa. La olfación se evaluó con el test de identificación de olores Sniffin Sticks test (SS-12) y la evaluación de la sustancia negra mediante dúplex transcraneal. Resultados: Se evaluó a 98 individuos, 30 con diagnóstico de EP, 21 con TE y 47 controles. Las prevalencias de hiposmia (SS-12 < 8) e hiperecogenicidad de SN (área > 0,24 cm2) fueron del 70 y el 83,3% en EP, el 33,3 y el 9,5% en TE y el 17 y el 10,6% en los controles, respectivamente. La combinación de ambos marcadores estaba presente en el 63% de los pacientes con EP y en ninguno de los pacientes con TE y solo en 2 de los controles. Conclusiones: La evaluación combinada de la evaluación olfativa mediante el SS-12 y de la sustancia negra mediante ecografía, 2 test rápidos, inocuos y accesibles, mejora la especificidad aislada que cada marcador tiene en el diagnóstico de la EP frente a pacientes con TE o controles. Dado que ambos marcadores se han descrito en fases muy precoces de la EP, su aplicación podría ayudarnos en su diagnóstico precoz. This book is the result of a project financed by Banco Santander in which European Universities (Glasgow Caledonian University in Scotland and Universidad de Alicante in Spain) have worked together in order to offer Doctorate students and their supervisors the opportunity to learn from one another. Presentations, practical exercises, cultural visits and talks about methods, results and motivation of academic investigation took place between 8th and 12th of June 2015 in Alicante and between 11th and 15th January 2016 in Glasgow in order that: Examples of best practice in research were shared, problems that commonly appear during the study of a doctorate were resolved and a process was introduced to facilitate the exchange of experiences and the mutual collaboration between researchers in both Universities. This compilation of articles shows the research abilities of the participants and forms an example of the dissemination of knowledge. Banco de Santander

Objective: The aim of this study was to determine whether patients with heart failure (HF) who recover left ventricular ejection fraction (LVEF), termed here as ‘Rec-HF', have a distinct clinical profile and prognosis compared with patients with HF and reduced LVEF (HF-REF) or HF and preserved LVEF (HF-PEF). Methods: We evaluated and classified patients from the Spanish Heart Failure Registry into three categories based on enrollment/follow-up echocardiograms: HF-PEF (LVEF ≥50%), HF-REF (LVEF persistently Results: A total of 1,202 patients were included, 1,094 with HF-PEF, 81 with HF-REF and 27 with Rec-HF. The three groups included patients of advanced age (mean age 75 years) with comorbidities. Rec-HF patients were younger, with a better functional status, lower prevalence of diabetes mellitus, dementia and cerebrovascular disease, and higher prevalence of COPD. The etiology of HF was more frequently ischemic and alcoholic and less frequently hypertensive. After a median follow-up of 367 days, the unadjusted hazard ratios for death in the Rec-HF versus HF-PEF and HF-REF groups were 0.11 (95% CI 0.02-080; p = 0.029) and 0.31 (95% CI 0.04-2.5; p = 0.274). Results were statistically nonsignificant in multivariate-adjusted models. Conclusion: Rec-HF is also present in elderly patients with HF but it is necessary to further investigate the natural history and optimal pharmacologic management of this ‘new HF syndrome'.

Epilepsy causes psychiatric disorders in 20-40% of patients impacting negatively on their quality of life. Lacosamide is a new antiepileptic as adjunctive therapy in partial seizures with or without generalization.We conducted a study to assess the impact of lacosamide as to the quality of life of epileptic patients. We used the HAD scale for anxiety and depression and QOLIE-10 scale for quality of life. We evaluated the efficacy and tolerability.We collected prospectively poorly controlled epileptic patients are and added lacosamide treatment. Baseline visit, at 3 and 6 months were performed. The questionnaires are completed and the epilepsy information has been collected.31 patients, age 45.5 ± 17.2 years, 64.5% males are included. Number of previous monthly crisis 1.6 ± 1.8. HAD anxiety scale shows a significant improvement at 3 and 6 months. HAD scale for depression reflects a significant improvement in quality parameters. QOLIE-10 shows significant improvement for the group with low quality of life after 3 and 6 months. After 6 months 61.3% of patients have a seizure reduction equal or more than 50% and 54.8% are seizure free. Dizziness is the most common side effect (22.8%). 74.2% continued treatment.Lacosamide may improve anxiety, depression and quality of life of epileptic patients regardless of seizure control. Response to treatment, adherence and side effects are similar to previous studies.Efecto de la lacosamida sobre la calidad de vida del paciente con epilepsia.Introduccion. La epilepsia provoca trastornos psiquiatricos en un 20-40% de los pacientes y repercute de forma negativa en su calidad de vida. La lacosamida es un nuevo antiepileptico que se utiliza como terapia añadida en crisis parciales con o sin generalizacion. Objetivo. Hemos realizado un estudio para valorar el impacto de la lacosamida en cuanto a la calidad de vida del paciente con epilepsia. Se han utilizado la escala hospitalaria de ansiedad y depresion (HADS) y la escala de calidad de vida en la epilepsia (QOLIE-10). Se ha valorado la eficacia y la tolerabilidad. Pacientes y metodos. Se recogen prospectivamente pacientes con epilepsia mal controlada y a los que se añade lacosamida. Se realiza una visita basal, a los tres y a los seis meses. Se cumplimentan los cuestionarios y se recaban los datos sobre la epilepsia. Resultados. Se incluyen 31 pacientes, con una edad media de 45,5 ± 17,2 años, un 64,5% varones. El numero de crisis mensuales previas es de 1,6 ± 1,8. La HADS para ansiedad muestra una mejoria significativa a los tres y seis meses. La HADS para depresion refleja una mejoria significativa en los parametros cualitativos. La QOLIE-10 muestra mejoria significativa para el grupo con baja calidad de vida previa a los tres y seis meses. Tras seis meses, el 61,3% de los pacientes presenta una reduccion de las crisis igual o superior al 50%, y el 54,8% esta libre de crisis. El mareo es el efecto secundario mas frecuente (22,8%). El 74,2% continua con el tratamiento. Conclusiones. La lacosamida podria mejorar la ansiedad, depresion y calidad de vida del paciente epileptico con independencia del control de las crisis. La respuesta al tratamiento, la adhesion y los efectos secundarios son similares a estudios previos.

El desarrollo creativo de Apocalypse Now (Francis Ford Coppola, 1979) estuvo caracterizado por una revision constante del guion, un rodaje anormalmente extenso y accidentado y una postproduccion muy laboriosa y en absoluto convencional. Todo ello propicio una marcada metamorfosis respecto del proyecto original de John Milius, inicialmente sustentado en la novela El corazon de las tinieblas de Joseph Conrad y en una serie de ingredientes belicos tratados desde un prisma psicodelico (Cowie, 2001). Este trabajo de recortes, modificaciones y anadidos --de recomposicion, en suma-- efectuado por Coppola alcanza sus puntos algidos en el inicio y el final del film, dos momentos que se remiten y complementan mutuamente y que establecen una cierta circularidad en el texto. Asi, el devenir constructivo (y discursivo) de Apocalypse Now presenta una notable similitud con los procesos de elaboracion onirica descritos por Freud en La interpretacion de los suenos, de lo que se desprende una poetica cinematografica que en unas ocasiones se mueve entre el sueno y la alucinacion, en otras es de perfil netamente narrativo, en otras aun se presenta desgarrada e hiriente, y cuyo tono dominante resulta ser mas bien de pesadilla.

La katabasis o viaje al inframundo es un motivo recurrente en la mitología grecolatina, en la que varios de sus héroes llevan a cabo un viaje de esta índole. Un buen número de textos artísticos desde la Antigüedad hasta nuestros días se ha centrado también en esta temática, que cuenta así con una amplia difusión. Tomando como referencia a la Eneida y comparando distintos textos clásicos que se ocupan de este motivo, en el presente escrito se realiza un análisis textual que trata de aislar los elementos narrativos más destacados del relato general de la katabasis. De dicho análisis se desprende que el viaje a ese otro mundo subterráneo y oscuro que es el Hades constituye fundamentalmente un regreso al interior femenino, en una especie de parto reverso que conduce hasta los orígenes del sujeto. En ciertos casos, además, este viaje incluye el encuentro con una figura paterna de la que emana una revelación sobre el destino del héroe. The katabasis or journey to the underworld is a recurrent motif in the graecolatin mythology, in which some of their heroes undertake this type of voyage. A huge number of artistic texts from Antiquity to our day have al so been focused on this topic, being very disseminated as a result. Taking The Aeneid as a reference point and comparing different classical texts that are dedicated to this motif, we are going to proceed on these lines to the textual analysis by trying to isolate the more highlighted narrative elements in the general story of the katabasis. What can be deduced from the analysis is that the journey to that other subterranean and dark world, Hades, represents fundamentally a return to the feminine interior, a kind of inside out birth that leads to the origins of the subject. In some cases, this travel involves an encounter with a paternal figure from which emanates a revelation of the hero's destiny.

Hyposmia and substantia nigra hyperechogenicity (SN+) are characteristic markers of Parkinson's disease (PD), although their diagnostic value in isolation may be limited. We evaluated the combined prevalence of both disorders in patients diagnosed with PD and assessed their diagnostic yield compared to a sample with essential tremor (ET) and another group of healthy subjects.Patients diagnosed with PD and ET and treated in our outpatient clinic were enrolled. Olfaction was assessed using the "Sniffin' Sticks" odour identification test (SS-12) and hyperechogenicity of the substantia nigra (SN+) was assessed by transcranial duplex ultrasound.A total of 98 subjects were analysed, comprising 30 with PD, 21 with ET, and 47 controls. The respective prevalence rates of hyposmia (SS-128) and SN+ (area.24cm(2)) were 70% and 83.3% in PD, 33.3% and 9.5% in ET, and 17% and 10.6% in controls. Both markers were present in 63% of patients with PD, none of the patients with ET, and only 2 of the controls.Combined use of substantia nigra sonography and olfactory testing with SS-12, two rapid, safe, and accessible tests, was more specific than each isolated marker for distinguishing patients with PD from patients with ET and control subjects. Since both markers have been described in very early phases of PD, combined use may be helpful in providing early diagnosis of PD.

Introduccion. Los estudios postautorizacion son importantes para confirmar si los resultados de los ensayos clinicos se reproducen en la practica clinica habitual. Objetivo. Evaluar la efectividad y seguridad del fingolimod en la practica clinica en la provincia de Alicante. Pacientes y metodos. Estudio multicentrico retrospectivo de pacientes con esclerosis multiple remitente tratados con fingolimod. Se recogen las caracteristicas demograficas, clinicas y farmacologicas. Se describe la efectividad del farmaco –tasa anualizada de brotes (TAB) y porcentaje de pacientes libres de brotes– al ano y a los dos anos de tratamiento en relacion con el ano previo y datos de efectos secundarios. Resultados. Se incluyo a 89 pacientes. El tratamiento previo fue inmunomodulador (interferon beta o acetato de glatiramero) en 54 pacientes y natalizumab en 32. Cincuenta pacientes cambiaron por fracaso con el inmunomodulador y 31 por serologia positiva del virus JC (VJC+). La TAB global disminuyo el 67,3% el primer ano (p < 0,0001) y el 84,1% el segundo (p = 0,0078). Disminuyo en los pacientes con fracaso del inmunomodulador (el 85,6% el primer ano, p < 0,0001; el 88,9% el segundo ano, p = 0,0039) y aumento de forma no significativa en los pacientes VJC+ en el primer ano. El porcentaje de pacientes libres de brotes en la poblacion global aumento del 32,6 al 68,1% en el primer ano (p < 0,0019) y al 82,6% en el segundo (p = 0,0215). Este aumento no se observo en los pacientes VJC+. Trece pacientes tuvieron efectos secundarios, que obligaron a la retirada del farmaco en dos de ellos. Conclusion. En la practica clinica de la provincia de Alicante, el fingolimod mostro una efectividad y una seguridad ligeramente superiores a las de los ensayos clinicos.

To describe the involvement of cocaine in cerebral vascular pathology in young patients.The work consists in a descriptive study of the role of cocaine in patients with acute stroke under the age of 50 years admitted to the neurology service over a period of four years. Eighteen patients with positive levels of cocaine on admission and 79 patients with negative levels were analysed. Different variables that define the profile of vascular risk, characteristics of the stroke and the morbidity and mortality associated to them are collected and analysed.Males were predominant and there was a non-significant higher proportion of vascular risk factors in the control group (55.6% versus 64.6%). The group of consumers presented a significantly lower mean age (35.2 ± 8.9 versus 41.5 ± 7.7 years), higher consumption of toxic substances (tobacco, alcohol and cannabis), hyperCKemia (27.8% versus 5.1%) and psychiatric disorders (16.7% versus 3.8%) (p0.05). The ischaemic stroke in the anterior territory subtype was predominant in both groups. There was a clear tendency towards normality in complementary tests and a higher rate of complications (33.3% versus 15.2%) and mortality (11.1% versus 3.8%) among consumers (p0.05).Cocaine is a risk factor that must be taken into account in young adults: it is associated to stroke at earlier ages than is considered normal, with a tendency towards normality in complementary tests and a higher rate of hospital morbidity and mortality.

Lacosamide is a new antiepileptic drug with a novel mechanism of action, as it selectively promotes the slow inactivation of voltage-dependent sodium channels without affecting fast inactivation. There are studies in the literature regarding its effectiveness in controlling neuropathic pain.We describe the use of intravenous lacosamide in the treatment of three patients with neuropathic pain: a woman with neuropathic pain in the first branch of the right trigeminal nerve during the acute phase of herpes zoster, a woman with central pain secondary to Dejerine-Roussy syndrome due to a malignant brain tumour, and a man with facial pain due to infiltration of the trigeminal nerve by a secondary lymphoma of the central nervous system. In the three cases, the administration of intravenous lacosamide has led to a considerable improvement in pain. The lacosamide dose has been 200 mg/day with excellent tolerability.Lacosamide can be an effective and well-tolerated alternative in the treatment of neuropathic pain and, moreover, its intravenous use can achieve pain control faster or be suitable when it is not tolerated orally.

Patients were classified based on their neuropsychiathric symptom profile and serum lithium levels.Sixteen of 150 inpatients had lithium intoxication: 58.3% women and 43.8% men; 49.19% +/- 18.49% years old. Lithium was used as treatment of bipolar disorder in 87.5% of cases. The most frequent cause of intoxication was attempted suicide. Using neuropsychiatric parametres, intoxication was moderate in 50% of cases, and mild in 25% and severe or very severe in 25%. Using serum lithium levels, intoxication was very severe in 31.35%, severe in 25%, and slight-moderate in 43.7%. Conservative measures were used as the most frequent treatment (50%), and haemodialfiltration was needed in 37.5%. Mean stay was 4,8 days in acute intoxication, and 11.2 days in chronic. Sequelaes were found in two patients (ataxia). Death was not present.Lithium intoxications can involve severe complications, even death. Narrow control is encouraged in polymedicated and elderly patients, and in concommitant treatment with antidepressant and neuroleptics.

Pituitary apoplexy is a rare clinical entity. It is a rare cause of stroke, whose pathogenic mechanism has not been fully understood. Compression in intracavernous carotid artery and vasospastic mechanism have been described. It may initially begin as a meningeal syndrome, in which neuroimaging techniques may be fundamental, above all resonance magnetic imaging for a correct diagnosis of the disease and its complications.We report the case of a 23 year-old male who suffered a massive stroke due to bilateral carotid compression in its intracavernous portion due to apoplexy of a previously unknown pituitary tumor. The diffusion sequences and acute angioresonance of the circle of Willis are presented.This is a rare entity with controversial management. An exhaustive review of cases and series of patients with pituitary apoplexy related stroke is also presented.

Strokes are the leading cause of epileptic seizures in adults and account for 50% of seizures in those over the age of 65. These patients present certain specific characteristics with respect to the remaining population, which makes it necessary to look for medication that are suited to their particular case.To describe the effectiveness and safety of levetiracetam (LEV) in monotherapy in elderly patients with post-stroke epileptic seizures.A prospective evaluation was conducted of a series of cases consisting of patients over the age of 60 years who had suffered a stroke and had had at least one epileptic seizure in the late post-stroke phase (more than two weeks). Demographic data and the characteristics of the epilepsy and the stroke were collected. Patients began treatment with LEV in monotherapy, underwent check-ups at one and six months of treatment, and the effectiveness and safety of the drug were evaluated.The sample consisted of 25 patients with a mean age of 75.2 +/- 7.6 years. They presented an average of 3.2 +/- 5.6 post-stroke seizures and the time elapsed since the stroke was 38.3 +/- 81.8 months. After six months' follow-up, 76% of the patients were still receiving treatment with LEV. Of the patients under treatment, 89.5% were free from seizures. Side effects that could be attributed to LEV were noted by 28% of patients, but did not compel them to stop treatment.LEV in monotherapy can be a safe, effective therapeutic option for elderly patients who have presented epilepsy following a stroke.

To describe the use of preadmission statins in patients with cerebrovascular accident and the possible predictive factors.Cross-sectional observational study of 795 consecutive patients with acute cerebrovascular accident. We assessed the differences among patients who were on preadmission statins (161) and those who were not (634), regarding vascular risk factors and clinical and neurosonological atherothrombotic disease markers. For univariate analysis, we used squared chi test, and for multivariate analysis, logistic regression analysis.Preadmission statins were 20.3%. In high vascular risk patients defined based on National Cholesterol Education Program Adult Treatment Panel III (NCEP-ATP III), this reached 28% and it might be 72%. Predictive factors for preadmission statins use were, in a positive sense, the antecedent of hypercholesterolemia diagnosis (OR = 189; 95% CI = 58-615; por = 0.001) and stroke (OR = 2.1; 95% CI = 1.2-3.6; por = 0.01), and in a negative sense, smoking (OR = 0.38; 95% CI = 0.18-0.81; p = 0.012).In our population of patients with stroke, the predictive factors of preadmission statins did not adjust to the current therapeutic NCEP-ATP III recommendations; treatment with statins in high vascular risk population was way below the indications, it was 28% and it might be 72%.

Transtemporal insonation by means of transcranial color-coded duplex sonography (TCDS) with low frequency probes (2-2.5 MHz) makes it possible to not only evaluate the blood vessels of the circle of Willis but also of brain parenchymal structures. A reduced number of studies, none of them in our area, have reported its usefulness for the evaluation of the ventricular system in adults. Aim. To evaluate ability of the transcranial duplex to measure the third ventricle diameter in a sample of patients with different neurological illnesses.Patients of all ages admitted to our neurology ward and whose diagnostic procedure included a brain magnetic resonance (MRI) were recruited. The largest third ventricle diameter was measured with both the TCDS and MRI in axial planes. We report the results observed with both techniques and the lineal correlation between both of them.We included 30 patients with mean age 48.5 years and 66.6% females. The third ventricle diameter (average +/- standard deviation) was 3.97 +/- 2.58 mm as measured with TCDS and 3.70 +/- 2.27 mm on MRI. Linear correlation between the values observed was high (r=0.85).Transcranial duplex is a cheap, safe and easy to perform technique. It has similar results in comparison with those obtained with the MRI when the third ventricle size is measured.

Only a minority of patients with acute stroke are eligible for fibrinolysis, and the main reason for this is that time runs out and goes beyond the therapeutic window. The chief delay occurs prior to arrival at the hospital, but there is also a nosocomial delay, which has received far less attention.The purpose of our study was to describe the data on delays occurring before and after arrival at the hospital in a sample of patients with acute stroke, and to analyse possible associated factors, including the overload on health care services in the Emergency department, an aspect that has not previously been evaluated.Epidemiological data, times of delays before and after entering the hospital, means of transport used and the health care workload in the Emergency department of the day of admission were collected for all the stroke patients admitted to the Stroke Unit of the Hospital General Universitario de Alicante throughout the period under study.From an initial sample of 460 patients, 423 were finally included in the study, with a mean delay before and after admission to hospital of 3.99 and 2.36 hours, respectively. Use of an ambulance and the haemorrhagic and transient ischemic attack subtypes were linked to a shorter delay before reaching the hospital, and arrival during the night shift was the only factor associated to a longer delay once inside the hospital.In our environment, there is a need to establish strategies aimed at shortening the times stroke patients take to reach the hospital and the time that elapses before they are attended.

Dizziness is a common symptom at the outpatient clinic of family doctors. Its origin is usually multifactorial and its outcome is often benign. However, exists a tendency to relate the dizziness with a cerebrovascular disturbance. AIM. To determine if there are cerebrovascular disorders in patients with chronic dizziness using a non invasive technique.A prospective study was conducted. It included 404 patients without limit of age. The patients were evaluated in a Neurology Outpatient Clinic, to select those patients with chronic instability. A neurosonographic exam was performed to all those selected patients. This exam included colour duplex of the cervical arteries and transcranial Doppler.Up to 54 % of the patients who were included in the study had a normal carotid study. For the rest of the patients, the thickness intima-media was the most prevalent finding. At the vertebrobasilar system the study of the vertebral arteries was completely normal in 81.7% followed by the presence of microangiopathy in 12.1%. The basilar system was also normal in a high figure (78%) followed by the microangiopathy (17.1%).The vascular disturbances in the vertebrobasilar system are an exceptional finding in patients with chronic instability.

In Western countries, neurological disorders secondary to toxic nutritional problems usually present as isolated cases that are generally associated to identifiable causes (alcoholism, eating disorders, absorption disorders, use of medicines) that reduce the availability of basic nutrients, especially B group vitamins, but also folic acid (FA). The optic nerves and the peripheral axons are frequent target organs in this type of pathology, but leukoencephalopathy and spinal cord involvement may also appear, often in combination.We describe the case of a 38-year-old female smoker with a heavy alcohol habit, who developed a subacute clinical pattern of, predominantly axonal, sensitive peripheral polyneuropathy, with vegetative fibre involvement. She also presented involvement of the posterior spinal cord, which gave rise to an ataxic disorder in the gait, as well as a severe bilateral retrobulbar optic neuropathy. Likewise, she presented macrocytosis (MCV: 118) due to megaloblastosis. She was also found to have a FA deficit but a normal vitamin B12 metabolism. With the help of supplementary vitamins, stopping drinking and the regularisation of her diet, the patient presented progressive clinical improvement, and was able to walk without support at 3 months and almost completely recovered her sight, which was corroborated by an improvement in the studies of both visual and somatosensorial evoked potentials.In our community, alcoholism is a frequent cause of nutritional deficiencies, which lead to neurological problems. FA is one of the nutrients that become deficient in alcoholics. More and more descriptions are being reported of peripheral polyneuropathy, retrobulbular optic neuropathy, myelopathy or leukoencephalopathy associated to FA deficiency, above all in patients with a history of alcoholism.

Introduccion. La epilepsia provoca trastornos psiquiatricos en un 20-40% de los pacientes y repercute de forma negativa en su calidad de vida. La lacosamida es un nuevo antiepileptico que se utiliza como terapia anadida en crisis parciales con o sin generalizacion. Objetivo. Hemos realizado un estudio para valorar el impacto de la lacosamida en cuanto a la calidad de vida del paciente con epilepsia. Se han utilizado la escala hospitalaria de ansiedad y depresion (HADS) y la escala de calidad de vida en la epilepsia (QOLIE-10). Se ha valorado la eficacia y la tolerabilidad. Pacientes y metodos. Se recogen prospectivamente pacientes con epilepsia mal controlada y a los que se anade lacosamida. Se realiza una visita basal, a los tres y a los seis meses. Se cumplimentan los cuestionarios y se recaban los datos sobre la epilepsia. Resultados. Se incluyen 31 pacientes, con una edad media de 45,5 ± 17,2 anos, un 64,5% varones. El numero de crisis mensuales previas es de 1,6 ± 1,8. La HADS para ansiedad muestra una mejoria significativa a los tres y seis meses. La HADS para depresion refleja una mejoria significativa en los parametros cualitativos. La QOLIE-10 muestra mejoria significativa para el grupo con baja calidad de vida previa a los tres y seis meses. Tras seis meses, el 61,3% de los pacientes presenta una reduccion de las crisis igual o superior al 50%, y el 54,8% esta libre de crisis. El mareo es el efecto secundario mas frecuente (22,8%). El 74,2% continua con el tratamiento. Conclusiones. La lacosamida podria mejorar la ansiedad, depresion y calidad de vida del paciente epileptico con independencia del control de las crisis. La respuesta al tratamiento, la adhesion y los efectos secundarios son similares a estudios previos.

Objetivo. Describir la implicacion de la cocaina en la patologia vascular cerebral en pacientes jovenes. Pacientes y metodos. Es un estudio descriptivo del papel de la cocaina en pacientes con ictus agudo menores de 50 anos ingresados en el servicio de neurologia durante cuatro anos. Se analizan 18 pacientes con niveles de cocaina positivos en el momento del ingreso y 79 pacientes con niveles negativos. Se recogen y analizan distintas variables que definen el perfil de riesgo vascular, caracteristicas del ictus y morbimortalidad asociada a estos. Resultados. Se obtiene un predominio de varones y mayor proporcion de factores de riesgo vascular no significativa en el grupo control (55,6% frente a 64,6%). El grupo de consumidores presenta de manera significativa una menor edad media (35,2 ± 8,9 frente a 41,5 ± 7,7 anos), mayor consumo de toxicos (tabaco, alcohol y cannabis), hiperCKemia (27,8% frente a 5,1%) y trastorno psiquiatrico (16,7% frente a 3,8%) (p 0,05). Conclusiones. La cocaina es un factor de riesgo que hay que tener en cuenta en adultos jovenes, asociado a ictus a edades mas tempranas de lo habitual, con tendencia a la normalidad de las pruebas complementarias y una mayor morbimortalidad hospitalaria.

Introduccion. La lacosamida es un nuevo farmaco antiepileptico con un mecanismo de accion novedoso al favorecer, de forma selectiva, la inactivacion lenta de los canales de sodio dependientes del voltaje, sin actuar sobre la inactivacion rapida. Existen estudios en la bibliografia sobre su eficacia para el control del dolor neuropatico. Casos clinicos. Se describe el uso de la lacosamida intravenosa en el tratamiento de tres enfermos con dolor neuropatico: una mujer con dolor neuropatico en la primera rama del trigemino derecho durante la fase aguda de un herpes zoster, una mujer con un dolor central secundario a un sindrome de Dejerine-Roussy por una neoplasia cerebral y un varon con dolor facial debido a una infiltracion del trigemino por un linfoma secundario del sistema nervioso central. En los tres casos, la administracion de lacosamida intravenosa ha significado una llamativa mejoria del dolor. La dosis de lacosamida ha sido de 200 mg/dia, con una excelente tolerabilidad. Conclusion. La lacosamida puede ser una alternativa eficaz y bien tolerada en el tratamiento del dolor neuropatico y, ademas, el uso intravenoso puede acelerar el control del dolor o ser adecuado en caso de no tolerar la via oral.

La dependencia al alcohol es una enfermedad cronica y su tratamiento comienza con la desintoxicacion, continuandose con la rehabilitacion. Se presenta un estudio descriptivo y retrospectivo de las desintoxicaciones de alcohol en 147 pacientes ingresados en nuestra unidad en el periodo 2003-2005. La media de edad es de 46.07 anos, 77.6% hombres y 22.4% mujeres. Ingreso por sindrome de deprivacion alcoholica en 31 pacientes (21.1%), desintoxicacion programada en 116 pacientes (78.9%). Segun la escala CIWA-Ar: abstinencia grado leve 100 pacientes (68.5%), moderada 35 (24%), grave 11 (7.5%). La estancia media global fue de 11.5 dias, sin diferencias estadisticamente significativas. Grupo Leve con proporcion significativamente menor de hepatopatia, grupos Moderado y Grave proporcion significativamente alta. Al comparar la media de la edad, estancia, cantidad de alcohol y parametros de consumo cronico (ferritina, fe, VCM, UBE, GPT, GGT, bilirrubina y Mg), se encuentran diferencias significativas entre las medias de los grupos Leve y Grave para la GPT y bilirrubina total. Se produjeron crisis epilepticas en 11 pacientes, 4 pertenecian al grupo programado, 7 con intensidad moderada-grave. No se ha evidenciado relacion entre la intensidad del sindrome de deprivacion, edad y sexo. Mayor intensidad en la escala CIWA-Ar (grados moderado y grave) entre los pacientes con sindrome de deprivacion ya iniciado comparados con los ingresados programados, que predominantemente presentaban un grado leve. La CIWA-Ar es un instrumento util para evaluar los sintomas de deprivacion, el riesgo de complicaciones y planificar el tratamiento. La actitud clinica debe facilitar el acceso del paciente a los recursos sanitarios para el tratamiento de su adiccion, normalizando la asistencia durante el ingreso hospitalario.

Objetivos. Describir el antecedente de uso de estatinas en los pacientes con ictus isquemico y sus posibles factores predictores. Pacientes y metodos. Estudio observacional transversal de 795 pacientes consecutivos con ictus isquemico. Evaluamos diferencias entre los pacientes con antecedente de uso de estatinas (161) y los que no lo tenian (634), en cuanto a factores de riesgo vascular y marcadores de enfermedad aterotrombotica clinicos y neuroecograficos. Utilizamos para el analisis univariante el test de chi al cuadrado y para el analisis multivariante los calculos de regresion logistica. Resultados. El antecedente de uso de estatinas fue de un 20,3%. En los pacientes con alto riesgo vascular definidos basados en criterios del National Cholesterol Education Program Adult Treatment Panel III (NCEP-ATP III) fue del 28% y debio ser del 72%. Los factores predictores del uso de estatinas fueron, en un sentido positivo, el antecedente de hipercolesterolemia (OR = 189; IC 95% = 58-615; p £ 0,001) y de ictus (OR = 2,1; IC 95% = 1,2-3,6; p £ 0,01), y en un sentido negativo, ser fumador (OR = 0,38; IC 95% = 0,18-0,81; p = 0,012). Conclusiones. En nuestra poblacion de pacientes con ictus, los factores predictores del antecedente de uso de estatinas no se ajustaron a las recomendaciones terapeuticas actuales; el tratamiento con estatinas en la poblacion con alto riesgo vascular estuvo muy por debajo de lo indicado, fue del 28%, debiendo alcanzar el 72% segun el criterio NCEP-ATP III.

Introduccion. Los ictus son la primera causa de crisis epilepticas en adultos y los responsables del 50% de crisis en mayores de 65 anos. Estos pacientes presentan unas caracteristicas propias respecto al resto de la poblacion, por lo que se hace necesario buscar farmacos que resulten apropiados. Objetivo. Describir la eficacia y tolerabilidad del levetiracetam (LEV) en monoterapia en pacientes ancianos con crisis epilepticas postictus. Pacientes y metodos. Se evaluo prospectivamente una serie de casos formada por pacientes mayores de 60 anos que hubieran sufrido un ictus y presentaran, al menos, una crisis epileptica en la fase tardia (mas de dos semanas) postictus. Se recabaron datos demograficos, caracteristicas de la epilepsia y del ictus. Los pacientes iniciaron tratamiento con LEV en monoterapia, fueron controlados al mes y a los seis meses de tratamiento, y se evaluo la eficacia y tolerabilidad del farmaco. Resultados. Se incluyeron 25 pacientes con edad media de 75,2 ± 7,6 anos. Presentaron una media de 3,2 ± 5,6 crisis postictus y un tiempo trascurrido desde el ictus de 38,3 ± 81,8 meses. Tras seis meses de seguimiento, el 76% de los enfermos continuaba tratamiento con LEV. De los pacientes con el tratamiento, el 89,5% se mantenia libre de crisis. El 28% habia presentado algun efecto secundario atribuible al LEV que no obligo a la suspension del tratamiento. Conclusion. El LEV en monoterapia puede ser una opcion terapeutica eficaz y segura para aquellos pacientes ancianos que hubieran presentado epilepsia tras sufrir un ictus.

Introduccion. Solo una minoria de pacientes con ictus agudo es candidata a fibrinolisis, y el principal motivo es sobrepasar temporalmente la ventana terapeutica. El principal retraso se produce hasta la llegada al hospital, pero tambien existe, y ha sido menos estudiado, un retraso intrahospitalario. Objetivo. Describir los datos de retraso extrahospitalario e intrahospitalario en una muestra de pacientes con ictus agudo y analizar los posibles factores asociados, entre ellos la sobrecarga asistencial en el area de Urgencias, aspecto previamente no evaluado. Pacientes y metodos. Se recogieron de forma sistematica los datos epidemiologicos, los tiempos de retraso extrahospitalario e intrahospitalario, el medio de trasporte empleado y la carga asistencial en Urgencias en el dia del ingreso de todos los pacientes con ictus que ingresaron en la Unidad de Ictus del Hospital General Universitario de Alicante durante el periodo de estudio. Resultados. De una muestra inicial de 460 pacientes, se incluyeron en el estudio 423, con una mediana de retraso extrahospitalario e intrahospitalario de 3,99 y 2,36 horas, respectivamente. El uso de ambulancia y los subtipos hemorragico y accidente isquemico transitorio se asociaron a un menor retraso extrahospitalario, mientras que la llegada durante el turno de noche fue el unico factor asociado a un mayor retraso intrahospitalario. Conclusiones. Se evidencia la necesidad en nuestro medio de iniciar estrategias encaminadas a mejorar las latencias con la que los pacientes con enfermedad cerebrovascular acuden y se atienden en el ambito hospitalario.

Introduccion. El mareo es un sintoma muy frecuente en las consultas de atencion primaria. Su origen es generalmente multifactorial y tiene un curso benigno. Existe, sin embargo, una tendencia a relacionar el mareo con una alteracion vascular cerebral. Objetivo. Determinar si existen alteraciones cerebrovasculares en los pacientes con mareo cronico utilizando una tecnica no invasiva. Pacientes y metodos. Se realizo un estudio prospectivo que incluia a 404 pacientes sin restriccion de edad. Se evaluo a los pacientes en la consulta de neurologia, con el objetivo de seleccionar los que tenian inestabilidad cronica. A los pacientes incluidos se les realizo un estudio neurosonologico con ecografia Doppler con color de las arterias cervicales y Doppler transcraneal. Resultados. En el 54 % de los pacientes, el estudio neurosonologico carotideo fue rigurosamente normal. En el resto de los pacientes, el hallazgo mas prevalente fue la ateromatosis carotidea no estenosante. En el sistema vertebrobasilar, el estudio de las arterias vertebrales fue completamente normal en el 81,7%, seguido de la presencia de microangiopatia en un 12,1%. El estudio de la arteria basilar fue tambien normal en una alta proporcion de pacientes (78%), seguido de la presencia de microangiopatia en un 17,1%. Conclusiones. Las alteraciones vasculares vertebrobasilares son un hallazgo excepcional en pacientes con inestabilidad cronica.

Introduccion. La presencia de una diplopia binocular de instauracion rapida obliga a un amplio diagnostico diferencial. Entre las causas mas frecuentes en personas adultas encontramos la patologia vascular, postraumatica, tumoral y miopaticas. Sin embargo, para realizar el diagnostico diferencial debemos tener en cuenta etiologias menos frecuentes, como enfermedad desmielinizante, fistulas carotidocavernosas y sindrome de Tolosa Hunt, entre otras. Presentamos el caso de un paciente que ingreso en nuestro servicio por presentar diplopia de instauracion aguda secundaria a hematoma espontaneo del recto inferior izquierdo. Caso clinico. Mujer de 54 anos con antecedentes de hipercolesterolemia, fumadora y alergia a contrastes yodados, que consulta por diplopia de instauracion aguda acompanada de dolor ocular izquierdo e inestabilidad. A la exploracion neurologica destaca paresia de la musculatura extraocular dependiente del III par craneal izquierdo con ptosis palpebral izquierda, paresia del recto inferior, sin afectacion pupilar. Resto de exploracion, dentro de la normalidad. En la tomografia axial computarizada craneal se observa una hemorragia en recto inferior. En la resonancia magnetica craneal se observo aumento de volumen del recto inferior izquierdo con signos de sangrado, y el resto de exploraciones complementarias fueron normales. La paciente evoluciona favorablemente y desaparece progresivamente la diplopia. Conclusion. La hemorragia espontanea de los musculos extraoculares, aunque infrecuente, debe considerarse en el diagnostico diferencial de oftalmoplejia dolorosa con diplopia en pacientes sin patologia de base, especialmente en ausencia de otras alteraciones neurologicas.

Introduccion. La asociacion entre factores de riesgo cardiovascular y ateromatosis carotidea ha demostrado ser mas importante en sujetos de mediana edad, en comparacion con ancianos. Objetivo. Estudiar la frecuencia de ateromatosis carotidea e intracraneal en una poblacion mayor de 80 anos y comparar la presencia de anomalias neurosonologicas segun sean pacientes con ictus o no. Comparar los resultados segun sexo. Pacientes y metodos. Se estudiaron los pacientes mayores de 80 anos recogidos en nuestro laboratorio de Neurosonologia con la realizacion de un estudio con Doppler carotideo y transcraneal. Se analizaron los pacientes segun sufriesen ictus isquemico o no. Carotideamente se diferencio entre normal, ateromatosis no significativa y ateromatosis significativa (estenosis > 50%). Intracranealmente se estudio la arteria cerebral media (normal, estenosis, microangiopatia y postestenosis) y la arteria basilar (normal, estenosis, microangiopatia e hiperdinamica). Resultados. Recogimos 832 pacientes ?342 varones (44,1%), edad media 83,63 ± 3,25 anos?, 527 (63,3%) con ictus. El estudio carotideo fue patologico, con mayor frecuencia en pacientes con ictus (32,1% frente al 41,9%; p = 0,002). La presencia de ateromatosis se asocio de forma significativa a un mayor riesgo de sufrir un ictus y con mayor intensidad a mayor gravedad de esta. Estudios carotideos patologicos con mas frecuencia en varones (28,7% frente al 40%; p < 0,001). No diferencias significativas intracranealmente en ningun parametro analizado. Conclusion. Alta frecuencia de ateromatosis carotidea en mayores de 80 anos. A pesar de la edad, la ateromatosis carotidea aparece todavia con mayor frecuencia en pacientes con ictus y en varones, y se asocia de forma significativa al riesgo de sufrir un ictus. Sin diferencias intracranealmente.

Introduccion. La lamotrigina (LTG) es un nuevo antiepileptico indicado para todo tipo de crisis parciales y tonicoclonicas generalizadas, tanto en monoterapia como politerapia. El 20-30% de los pacientes epilepticos tienen un mal control de las crisis, pese a un tratamiento adecuado. Objetivo. Presentamos nuestra experiencia en cuanto a eficacia y tolerabilidad a largo plazo de la LTG en crisis refractarias. Pacientes y metodos. Estudiamos consecutivamente durante 10 anos a los pacientes que iniciaron tratamiento con LTG tras un mal control de sus crisis, pese al tratamiento con dos o mas farmacos antiepilepticos en monoterapia o en combinacion. Recogimos sus datos epidemiologicos y clinicos. Estudiamos la eficacia del tratamiento, diferenciando entre remision (ausencia de crisis), mejoria (reduccion mayor o igual al 50% de las crisis), ineficacia (reduccion menor del 50%), agravamiento (aumento de las crisis), y su tolerabilidad (efectos secundarios y abandono del tratamiento). Resultados. Se recogieron 39 pacientes ?18 hombres (46,2%), edad media de 30,9 ± 13,7 anos?; el seguimiento medio fue de 27,6 ± 23,4 meses. El 43,7% presentaba epilepsia criptogenica, el 28,5%, sintomatica, y el 28,5%, idiopatica. El EEG fue patologico en el 53,8% de los pacientes y la neuroimagen, en el 43,6%. Tras la introduccion de la LTG, el 33% de los pacientes quedo libre de crisis, el 43,6% mejoro, en el 18,3% de ellos fue ineficaz y el 5,1% empeoro. El 56,4% abandono el tratamiento: el 23,1% por control insuficiente de las crisis, el 17,9% por efectos secundarios y el 15,4% por incumplimiento terapeutico. Conclusion. El tratamiento con LTG logra una alta eficacia en el control de las crisis en los pacientes con epilepsia refractaria, si bien esta limitado por una elevada tasa de abandonos terapeuticos.

Introduccion. La estenosis carotidea se ha descrito asociada a los infartos lacunares (IL) con una frecuencia variable, pero su relacion etiopatogenica todavia es controvertida. Objetivo. Describir la prevalencia de alteraciones carotideas en el estudio neurosonologico de una muestra de pacientes con IL. Pacientes y metodos. Se analizaron los resultados del estudio neurosonologico de los pacientes con criterios clinicorradiologicos de IL que ingresaron en nuestra Unidad de Ictus durante el periodo de estudio. Resultados. Se estudiaron 140 pacientes, y fue normal el estudio en solo 42% de los casos. Se encontro una estenosis > 50% en un 10% de los casos y placa sin estenosis en un 40,3% de los pacientes. En los pacientes sin placa existia un engrosamiento del grosor intima-media en un 7,2% de los casos. Discusion. En la mayoria de los pacientes con IL existen anomalias del estudio carotideo mediante ultrasonografia.
