Item does not contain fulltext This study shows the long-term updated outcomes of a multicenter retrospective study which analyzed 843 patients with myelodysplastic syndrome (MDS) who underwent transplantation with an HLA-identical sibling donor with either reduced-intensity conditioning (RIC) in 213 patients, or standard myeloablative conditioning (MAC) in 630 patients. In multivariate analysis, the 13-year relapse rate was significantly increased after RIC (31% after MAC vs 48% in RIC; HR, 1.5; 95% CI, 1.1-1.9; P=0.04), but with no differences in overall survival (OS) (30% after MAC vs 27% in RIC; P=0.4) and PFS (29 vs 21%, respectively, P=0.3). The impact of the intensity of graft-versus-host-disease immunoprophylaxis on transplantation outcomes in patients undergoing transplantation following reduced-intensity conditioning is unclear. This study addresses this issue in 228 adult patients above 50 years of age with acute myeloid leukemia in first complete remission given peripheral blood stem cells from HLA-identical siblings after fludarabine and 2 days of intravenous busulfan reduced-intensity conditioning. A total of 152 patients received anti-thymocyte globulin, either in combination with cyclosporine A in 86 patients (group 1), or with cyclosporine A and mycophenolate mofetil or short course methotrexate in 66 patients (group 2). The remaining 76 patients did not receive anti-thymocyte globulin but were given cyclosporine A and methotrexate or mycophenolate mofetil (group 3). Incidences of grade II-IV acute graft-versus-host-disease were comparable in the three groups (16.5%, 29.5% and 19.5% in groups 1, 2 and 3, respectively, P= 0.15). In multivariate analysis, the absence of anti-thymocyte globulin was the only factor associated with a higher risk of chronic graft-versus-host-disease (P= 0.005), while the use of triple immunosuppression (group 3) was associated with an increased risk of relapse (P= 0.003). In comparison to anti-thymocyte globulin and cyclosporine A alone, the other two strategies of graft-versus-host-disease prophylaxis were associated with reduced leukemia-free survival and overall survival (P= 0.001 for each parameter), independently of the dose of anti-thymocyte globulin. These data suggest that fine tuning of the intensity of this prophylaxis can affect the out-come of transplantation and that anti-thymocyte globulin and cyclosporine A alone should be the preferred combination with the fludarabine-busulfan reduced-intensity conditioning regimen and sibling donors.

The article exposes the phenomenon of parental burnout, which is insufficiently studied in national science. The phenomenon of emotional burnout is usually studied in regards to work, especially in service occupations (“helping professions”). Nonetheless, it is found that burnout can develop in other domains, including parenting. Even though parenting is not considered a professional occupation, society’s demands towards parents and their responsibilities are incredibly high and are growing every day. Parents are not prepared for many of them. People’s deteriorating living standards, lack of political and economic stability, and the newly a newly emerged challenge of a global pandemic all cause excessive and prolonged stress in parents that may lead to parental burnout. The aim of the study is to present a theoretical analysis on parental burnout phenomenon and also substantiate importance of an exploration of the parental burnout in Ukraine.To achieve the aim of the study, the article presents a review of foreign studies on parental burnout, its particularities, antecedents, and its effect on the family’s personal and interpersonal relationships. According to the foreign studies, parental burnout is a unique and context-specific syndrome in mothers and fathers that occurs under the influence of prolonged stress caused by parenting. The phenomenon of parental burnout encompasses three dimensions: excessive exhaustion from the parental role, emotional distancing from the child and a sense of inefficiency in performing their parental role. The material presented below describes the specifics and differences of this phenomenon and professional and family burnout. The article covers and analyzes foreign research on the history of the phenomenon of parental burnout, the antecedents that have a greater and lesser impact on its development (internal and external) and the consequences of parental burnout on a personal and interpersonal levels (especially the impact on the relationship with a child, which may manifest through violence, indifference or neglect). Also, there is an overview of the pilot study results of possible antecedents of parental burnout in Ukrainian parents at the macro-, meso- and micro- levels, emphasized the need and worthwhileness of studying the phenomenon of parental burnout in Ukrainian society. Allogeneic hematopoietic transplantation is increasingly used in patients aged 55 years or more with AML. The question of whether outcomes can be improved with an allele-level 8/8 HLA-matched unrelated donor (MUD) rather than an older HLA-matched sibling (MSD, more than 55 years) is still unanswered. We thus analyzed outcomes in 714 patients aged 55 years and older with AML in first CR (CR1) who received PBSCs after a reduced-intensity conditioning hematopoietic cell transplant from a MUD (n=310) or a MSD (n=404) in a recent period (2005-2010). The 3-year cumulative incidences (CIs) of non-relapse mortality were 17% and 23% with MSD and MUD, respectively (P=0.17). The 3-year CIs of relapse were 37% and 30%, respectively (P=0.12), resulting in a 3-year CI of leukemia-free survival of 46% and 47%, respectively (P=0.51). The 3-year overall survival was 49% with both MSD and MUD. In conclusion, HLA-identical sibling donors aged 55 years or more should not be excluded because of age for patients aged 55 years and older with AML in CR1.
