Helicobacter pylori, a gram-negative bacterium is a common infection in the developing world. Epidemiology involves the study of the distribution and determinants of health in a given population, and the subsequent application of this study to improve outcomes. This book aims to address a range of common clinical problems in paediatric dental practice. The format takes a logical approach to problem solving through history taking, clinical examination and investigation which are the main steps of patient management. Clinical and radiographical photographs are included in each case to ensure understanding and facilitate further critical thinking as different opinions may arise f rom different clinicians. Discussions are inserted to give the rationales for the decisions made in each case. This book is primarily intended for the dental undergraduates although it is also suitable for any clinician who would like to revisit problem solving in paediatric dentistry.

This book argues that the choice of treatment methods for acute pneumonia depends on the concept of the disease. It shows that modern ideas about the nature of lung tissue inflammation do not pay due attention to some fundamental axioms of medical science, which is the reason for the long-term tendency to decrease the results of treatment of this category of patients. This book examines the regulation and practice of medical decision-making where the context is that of a multiple pregnancy and where the question is whether or not to carry out a fetal reduction procedure. It concerns three main lines of inquiry: first, the nature of fetal reduction and the legal ground(s) for termination typically relied upon; secondly, the extent to which legal, ethical, and professional norms guide or constrain this particular kind of decision-making; and, thirdly, the adequacy of these norms. The book uses empirical sources to develop its analysis, contributing new insight and the kind of evidence necessary to shape regulation, clinical practice, and future research.The key findings show that fetal abnormality is often given as the justifying ground; that the legal, ethical, and professional norms offer little explicit guidance for fetal reduction: and on the general question of termination, ethical norms suffer from a high level of contestation, the key norms in the UK abortion legislation are unclear and disconnected from practice, and professional norms are only marginally more adequate. Given the indeterminacy of these norms, it is no surprise that the evidence indicates that doctors are only weakly guided by them in making their decisions. Various recommendations are advanced in this book, including the need for a situational emphasis on shared decision-making and patient-centred care.

2020年は新型コロナウイルス感染症で一色になってしまっていますが、腎細胞癌（RCC）治療に関してもいくつかの重要な進展がみられています。既存の治療では、IO doublet(ニボルマブ+イピリムマブ併用療法)の長期成績（4年(ESMO2020)）、ペムブロリズマブ+アキシチニブ併用療法の中期成績（約2年(ASCO2020)）、IO後のTKI治療の日本人データの発表（ASCO-GU）もありました。新規治療、薬剤では、ASCO2020において乳頭状腎細胞癌に対するMET-TKIであるsavolitinibの結果が報告され、ESMO2020において無治療進行症例に対するニボルマブ+カボザンチニブ併用療法の第Ⅲ相臨床試験、経口HIF-2 a阻害薬の第Ⅱ相臨床試験、第Ⅰb試験ではありますがアテゾリズマブ+カボザンチニブ併用療法の結果が報告されています。なかでも現場のわれわれの現時点での課題は、1次治療の選択かと思います。 本号では、近畿大学の野澤先生、浜松医科大学の三宅先生、新潟大学の山名先生に1次治療選択の考え方について執筆をお願いしました。また、リスク分類別という観点からの腎細胞癌の長期成績について、徳島大学の高橋先生、広島大学の亭島先生、防衛医科大学校の伊藤先生にご執筆いただきました。また、【Round Table Discussion】ではIO+TKI併用療法施行症例に焦点を当ててディスカッションを行いました。また【特別寄稿】として、改訂された『腎癌取扱い規約』の主な改訂内容について、北海道大学の大澤先生に執筆いただいております。 本号もまた、少しでも皆様の日常診療の一助になれば幸いです。

特発性正常圧水頭症（iNPH）は決して新しい疾患ではなく、その歴史は1965年のHakim S, Adams RDらの報告にさかのぼることができる。彼らはこの報告のなかで、精神活動の低下（認知症様の症状）、歩行障害、尿失禁を呈する高齢者のうち、著明な脳室拡大を認めるにもかかわらず、腰椎穿刺で測定した脳脊髄液圧が180mmH2O以下と比較的低く、しかし、脳脊髄液短絡術（シャント手術）を行うと上記の症状が著明に改善する患者のいることを指摘した。1970年代に入り本邦においても、iNPHは「治療可能な認知症」として注目されるようになったが、シャント手術により症状改善をみることが疾患概念の前提となっているがゆえに、本来はシャント手術の適応にならない症例に手術が行われるという不幸な結末がもたらされることになった。当然のことながら、認知機能障害、歩行障害、排尿障害は高齢者にみられるAlzheimer病やParkinson病などの神経変性疾患の症状であるため、これらの疾患とiNPHをいかに鑑別するかが大きな問題として認識されるようになった。すなわち神経変性疾患の患者でも、特に高齢者では脳萎縮にともなって脳室は拡大してくるが、シャント手術を行っても症状が改善することはなく、iNPH症例に対する手術適応をいかに厳密に判断するかが喫緊の課題となった。このような背景のなか、2004年に本疾患に関する診断・治療のガイドラインの初版が刊行された。2010年にはiNPHに関する医師主導型多施設前向きコホート研究（SINPHONI）の成果が報告され、この結果を受けて、2011年にはMRI所見を重視したガイドラインの改訂版（第2版）が発行された。その後、SINPHONI-2や全国疫学調査などのエビエンス・レベルの高い研究結果が続々と報告されるようになり、これらの研究成果を踏まえてガイドラインの再改訂が必要と判断し、この度第3版として『特発性正常圧水頭症診療ガイドライン』を発刊する運びとなった。厚生労働科学研究費補助金難治性疾患政策研究事業「特発性正常圧水頭症の診療ガイドライン作成に関する研究」班と日本正常圧水頭症学会の共同事業として実施された今回の改訂は、従来と同様にEBMの考え方に従い「Minds診療ガイドライン作成の手引き」2014年度版に準拠してその作業を行った。ガイドライン第3版は2部構成となっており、前半では従来の教科書的な内容を踏襲し、後半に18項目のクリニカルクエスチョン（CQ）に対する回答と解説を掲載した。 2004年のガイドライン初版刊行から15年余りが過ぎようとしているが、その間に本邦におけるiNPHの認知度は格段に高まり、iNPHの診断と治療は新たな局面を迎えるに至ったといっても過言ではない。今回のガイドライン第3版がiNPHの診断と治療をさらに精緻なものとし、多くの患者とその家族さらにiNPHの診療に関わる医療従事者に福音をもたらすことを期待したい。 （新井　一「序」より）

Psykisk helse hos eldre tar utgangspunkt i normale sider ved menneskers aldring, for eksempel hva som skjer med personligheten vår når vi blir eldre, hvordan søvnmønsteret endres, eller hvordan seksualitet ikke slutter å være viktig selv om vi er over 65 år. For å kunne gi eldre god nok og tilstrekkelig hjelp med psykiske vansker er det helt nødvendig å ha solid kunnskap om normal aldring, biologisk psykologi, nevropsykologi, helsepsykologi og klinisk psykologi. I denne 5. utgaven av boken er alle kapitlene gjennomgående oppdatert i samsvar med kunnskapsutviklingen innenfor fagområdet. Forfatterne har lagt vekt på å trekke ut de store linjene av den rikholdige litteraturen som finnes om hudpleie og sårbehandling. 日進月歩の臨床皮膚科学から落後しないためには、緻密な情報収集と正確な理解が必須であるが、多忙な日常診療のなかで、常にすべての領域のアップデートは至難の業に近い。そんなとき、私はこれまで耳慣れないキーワードに遭遇したら、きっちり検証して、その意味と臨床的意義を頭の中のデスクトップに保存するという手法をとってきたが、なかなか時間と労力を要する作業であった。1997年に「キーワードを読む皮膚科」を刊行したのは、その技法を紹介したかったことと、その時点でのキーワードを整理しようと考えたからに他ならない。それから20年以上を経て満を持して企画したのが本書「最新キーワードで読み解くALL About 皮膚科学」である。 今回は、この数年の学会報告や論文を渉猟し、臨床皮膚科学におけるホットトピックと思われる100を超えるニューキーワードを厳選し、その領域の第一人者にまずキーワードの概説、その臨床的意義をコアメッセージとして簡潔・明快に記載していただいた。続いて、今回の企画のキモである「キーワードを読み解く」というサブタイトルで、このキーワードが日常診療でどういう意味を持ち、どのように活かされるべきかという視点からわかりやすく解説していただいた。最後に、さらなる興味のある読者向けに「読むべき文献3編」を挙げていただき、知的好奇心の充足を心掛けた。キーワードの概説に目を通すだけでも知識の整理には十分であろうが、可能な限り「キーワードを読み解く」まで目を通していただければ本書の有用性はますます高まると思われる。本書を通読するだけで、最新の臨床皮膚科学全般の進歩に精通できると思われるのが「ALL About 皮膚科学」と命名した所以でもある。 本書は私の210冊目の単行本となるが、いずれも臨床皮膚科学の最前線を一人でも多くの皮膚科医に明示するという確固たる信念に基づいて編集したものばかりである。そのなかで、皮膚科領域の新しいキーワードをインプットしていくことは地道ではあるが着実に皮膚科学の臨床的力量を増幅することに繋がると信じたい。企画・立案からほぼ半年で刊行できたのはひとえにご執筆いただいた先生方と熱意あるメディカルレビュー社の担当者の努力の賜物である。ここに編集者として深謝したい。 （宮地良樹「序文」より）

Lung cancer is one of the most common cancers in men and women worldwide. Early diagnosis of lung cancer can significantly increase the chances of a patient's survival, yet early detection has historically been difficult. As a result, there has been a great deal of progress in the development of accurate and fast diagnostic tools in recent years. Lung Cancer and Imaging provides an introduction to the methods currently used in lung cancer diagnosis and the promising new techniques that are emerging. Areas covered include the major trends and challenges in lung cancer detection and diagnosis, classification of cancer types, lung feature extraction in joint PET/CT images, and algorithms in the area of low dosage CT lung cancer images.

Hvordan virker lov- og forskriftsreguleringer? Hvor mye kan man vite om hvordan en regel vil virke? Og hva kan rettsvitenskapen bidra med i analysen av nåværende og framtidige regelvirkninger? De siste årene har norske reguleringer blitt kritisert for å bygge på et for tynt analysegrunnlag, blant annet av OECD. Idealet kritikerne fremhever er inspirert av nyere amerikansk praksis: Reguleringer bør bygge på mest mulig fullstendige og presise årsak-virkning-analyser, både før og etter reguleringen. En slik tilnærming vil være vanskelig å forene med den tradisjonelle norske fremgangsmåten, som vektlegger samarbeid og deltakelse i beslutningsprosessen og som har særtrekk som gjør det vanskelig å utforme detaljerte årsak-virkningsanalyser. Når det i tillegg finnes holdepunkter for at den nordiske og norske reguleringsmåten fungerer relativt godt, kan det være gode grunner for å fastholde grunntrekkene i norsk reguleringsprosess. I boken drøftes også hvilken rolle rettsvitenskapen bør spille i årsak-virkning-analyser av reguleringer. Tradisjonelt har rettsvitenskapens viktigste oppgave vært å analysere retten slik den er (gjeldende rett). Dermed har juristene i større grad lært å lese og tolke rett enn å lage rett - juristen lærer å lese, men ikke å skrive, lover.
