Nephrotoxicity is one of the most severe side effects caused by cisplatin, limiting its use at high, effective concentrations. Dexamethasone, known for its strong anti-inflammatory and immunomodulating properties, is often used to alleviate cisplatin-induced side effects. However, dexamethasone also exhibits pro-oxidant properties and has been associated with morphological impairments and acute kidney injury. Although the mechanisms of cisplatin-induced nephrotoxicity are complex and involve numerous cellular processes, oxidative stress is widely accepted as the primary cause of this pathology. This study aims to investigate how dexamethasone, despite having effects similar to cisplatin, alleviates the side effects caused by this drug. Methods. The study measured lipid peroxide product malondialdehyde (MDA) levels using the thiobarbituric acid method and catalase activity using the molybdenum method. For histological examination, 5-6 μm thick tissue sections were prepared from samples processed with formalin and fixed with paraffin. These sections were stained with hematoxylin-eosin and observed under a light microscope. Results. Cisplatin and dexamethasone independently increased MDA levels to varying degrees. Cisplatin raised MDA by 75% in the homogenate and 38% in the supernatant, while dexamethasone increased these levels by 41% and 25%, respectively. The combined use of cisplatin and dexamethasone produced effects similar to those of dexamethasone alone. Catalase activity decreased following exposure to cisplatin (36% in the supernatant and 14% in the nucleus) and dexamethasone alone (33% in the supernatant and 24% in the nucleus). Combined use of the drugs led to a similar reduction in catalase activity. Histological analysis revealed tissue damage, supporting the pro-oxidant nature of both cisplatin and dexamethasone. Conclusions. The findings indicate that both cisplatin and dexamethasone exhibit pro-oxidant effects, as demonstrated by increased malondialdehyde levels, reduced catalase activity, and histological evidence of tissue damage. The ability of dexamethasone to mitigate cisplatin-induced side effects is likely attributable to a combination of its anti-inflammatory and immunomodulatory properties, as well as a "preventive or restraining" effect.

Purpose of the study. The purpose of the study is to improve the results of surgical treatment of patients with purulent-septic lesion of the liver substantiating the indication for resection method use. Material and methods. The work is based on a comparative analysis of the results of examination and treatment of 64 patients aged 10 to 81 years old, with chronic liver abscesses in the department of surgery and liver transplantation during the 1995–2016 periods. According to the tasks of the study patients are divided into two groups. In 30 (46,8%) patients (the study group), various types of anatomical resection of the liver with a purulent lesion were performed without its dissection during the operation; in 34 (53,2%) patients (group of comparison) they performed the disclosure, sanation, drainage of purulent cavity. The majority of patients with chronic liver abscesses treated the abscess with a puncture or drainage method under the control of ultrasound in other medical institutions of Ukraine. Results. The results of diagnosis and treatment of 64 patients for chronic liver abscesses from 1995 to 2016 are analyzed. In the study group, the patients performed anatomical resection of the liver, in the comparison group – standard surgical interventions – the disclosure and drainage of the abscess of the liver. The duration of antibiotic therapy after surgery in the comparison group is significantly higher than in the study group, respectively, 22 ± 3,4 and 5,75 ± 1,6 days; p < 0,001. Second-line antibacterial drugs after draining operations were used more often (p < 0,001) than after resection interventions, respectively, in 94,1 and 6,6% of cases. The duration of treatment of patients in the hospital after surgery in the study group was less than in the comparison group, according to 15,1 ± 0,7 and 27,3 ± 3,05 days; total 25,9 ± 1,4 and 45,7 ± 3,45 days (p < 0,001). After drainage operations, drainages from the abdominal cavity were removed later than after resection interventions, respectively, 17,1 ± 2,8 and 6,35 ± 1,1 days (p < 0,001), through. Conclusion. Resection for chronic purulentseptic lesions of the liver is the priority methods of surgical treatment. The analysis of the obtained results allows us to conclude that resection technologies contribute to the rapid normalization of physical and social rehabilitation of patients with septic liver damage.

Вступление. Единственным радикальным методом лечения перихилярной холангиокарциномы является хирургический, позволяющий улучшить отдаленные результаты и показатели выживаемости пациентов. Особую сложность представляют ситуации с инвазией опухоли в воротную вену. Цель. Изучение результатов хирургического лечения пациентов по поводу перихилярной холангиокарциномы с инвазией воротную в вену (ВВ). Материалы и методы. С 2003 по 2017 г. в клинике обширная и расширенная резекция печени по поводу перихилярной холангиокарциномы произведена 153 больным. Результаты. Для оценки характера поражения билиарного дерева использовали анатомическую классификацию перихилярной холангиокарциномы Bismuth-Corlette. У 66 (43%) больных резекция печени дополнена резекция ВВ с ее последующей реконструкцией. Показатели 1, 3- и 5-летней выживаемости после резекции ВВ составили соответственно 81,8, 40,9 и 31,8%. Выводы. Агрессивная тактика хирургического лечения перихилярной холангиокарциномы обеспечивает максимальную радикальность, позволяет увеличить резектабельность при инвазии опухоли в ВВ с приемлемой летальностью и отдаленными показателями выживаемости.

The aim of the study was to identify possible mechanisms that could reproduce the recurrence of CVD in older men against the background of non-steroidal anti-inflammatory drugs and antibiotics, thereby justifying why age and sex may be risk factors for acute pyelonephritis in urolithiasis. Material and methods. The results of a study of 88 men with chronic obstructive pulmonary disease, including 45 patients over 65 years of age (mean age 74.0 ± 1.3 years) and 43 patients in the age range of 55-65 years (mean age of patients 61.0 ± 0, 8 year). In the phase of remission of CVD, 48 patients were examined and in the phase of relapse-40 patients. The study included patients who took non-steroidal anti-inflammatory drugs (NSAIDs) and antibiotics in a standard dose of at least 5-10 days before hospitalization. From the peripheral blood by centrifugation, plasma-enriched plasma was isolated. Platelet content in 1 μl was 200,000 ± 20,000. To stimulate platelets, adrenaline and ADP (Sigma, USA) were used at an effective concentration (EC50) of 5 μM, which caused platelet aggregation (ATC) in healthy individuals (10 donors) at the level of 50 ± 5%. The aggregation of Tc was evaluated on a Chrono log analyzer (USA). The formation of platelet-leukocyte aggregates (TPA) was modeled in vitro by incubation of stimulated platelets (epinephrine at a concentration of EC50) and intact leukocytes isolated from the peripheral blood of patients with CVD. The number of intact TL was assessed after the color of blood smears according to the Romanovsky-Giemsa method. Results. In the phase of remission of CVD on the background of the appointment of NSAIDs and antibiotics, in patients of the two age groups studied, it was not possible to detect differences in the response of leukocytes. The article includes the data of long-term monitoring observations taken from 60 meteorological stations located at 450-3200 m above the sea level. It presents the results of study of time of occurrence, duration and spatial variability of bioclimatic rhythms of main ecotypes of agricultural crops under the conditions of high altitudes of the Republic of Armenia. It was found out that vertical gradients of the average periods of main phases of grow of winter wheat, potatoes, grapes and livestock pasturage within highland pastures fluctuate within 3-5 days / 100 m. Agro-ecological conditions for bioclimatic rhythms along high-altitude zones are estimated. The high-altitude regularities are determined and correlation dependence curves of time of occurrence of bioclimatic rhythms from absolute altitude are identified with specification of ways for optimizing heat and moisture availability and productivity of these crops. The probability of crop damage from drought, frosts in different periods of vegetation is calculated. Agro-climatic parameters of critical periods of growth and development of crops under study within high-altitude zones are determined which makes it possible to ensure production of stable and high yields. It became clear that the highest altitude for obtaining two crops a year after harvesting early spring grains, vegetables, potatoes is 1400 m above the sea level – in arid regions and 1200 m - in wet regions. It is proposed to develop a natural conveyor for fresh vegetables, potatoes, maize, fruit, dairy and other products using the vertical zonality of crops ripening and livestock pasturage within the summer pasture zone.

This mechanisms includes: pathoaffective, somatogenically-organic, patopersonologic, sociogenic, abstinent, intoxicative, emotional and of deprivation. Typical structural and dynamic versions of depressive and manioform syndromes was described. Keywords: polydrug use, psychopathogenesis, affective disorders, depression, manioform syndrome. Реферат С целью разработки типологии психопатогенетических механизмов формирования аффективных нарушений в структуре аддиктивной патологии было обследовано 100 пациентов КУ «Запорожский областной наркологический диспансер» ЗОР, страдающих аффективными расстройствами в структуре полинаркоманий. При использовании клинико-психопатологического метода была сформирована интегративная систематика этиопатогенетических механизмов формирования аффективных нарушений в структуре аддиктивной патологии, куда вошли следующие механизмы: патоаффективный, соматогенно-органический, патоперсонологический, социогенный, абстинентный, интоксикационный, эмоционально-депривационный. Описаны типичные структурно-динамические варианты депрессивных и маниоформных синдромов. Ключевые слова: полинаркомания, психопатогенез, аффективные нарушения, депрессия, маниоформный синдром.

Aim. Pathoplastic modification of addictive disorders with affective spectrum violations, leads to the formation of the psychopathological cluster that have specific structural and dynamic features.Methods and results. In order to assess the dynamics of change of disease-relation type by LOBY questionnaire, 100 patients from «Zaporozhye Regional Narcological Dispensary» suffering from polydrug usage and affective spectrum disorders were examined in the following clinical periods: withdrawal state, further inpatient and outpatient follow-up.Conclusion. The solidity of background psychopathological disorders in perspective of their affiliation to somatogenically-organic register, for the entire study contingent was found. Dynamics of change of disease-relation type illustrates partially reversible character of these disorders.

To investigate the role of NO in glomerular alteration at diabetes mellitus the quantitative analysis of normal, hypertrophic and sclerotic renal corpuscles (RC) was performed in rats with normal and decreased eNOS activity after 14 days, 1, 2 and 3 months after alloxane administration. It was shown that NO deficient increase the rate of diabetic glomerulopathy development and progression. The Relation between altered (hypertrophic and sclerotic) and normal RC reflects the amount of nephrons involved in the pathogenic changes and can be used to prognoses of diabetic nephropathy (DN) development. Analysis of eNOS reserve activity allows to perform the individual strategy of DN diagnostics and treatment.

Реферат С целью разработки типологии психопатогенетических механизмов формирования аффективных нарушений в структуре аддиктивной патологии было обследовано 100 пациентов КУ «Запорожский областной наркологический диспансер» ЗОР, страдающих аффективными расстройствами в структуре полинаркоманий. При использовании клинико-психопатологического метода была сформирована интегративная систематика этиопатогенетических механизмов формирования аффективных нарушений в структуре аддиктивной патологии, куда вошли следующие механизмы: патоаффективный, соматогенно-органический, патоперсонологический, социогенный, абстинентный, интоксикационный, эмоционально-депривационный. Описаны типичные структурно-динамические варианты депрессивных и маниоформных синдромов. Ключевые слова: полинаркомания, психопатогенез, аффективные нарушения, депрессия, маниоформный синдром.
